Abstract:Objectives To investigate the relationship between chronic cough and gastroesophageal reflux(GER)in children.Methods Twenty-four-hour esophageal pH monitoring was performed in 170 cases(patient group)with chronic cough( 4 weeks),and 32 asymptomatic children served as control group.Results All the parameters of esophageal acid reflux including episodes of acid reflux,number of the reflux longer than 5 min,the longest reflux time(min),percentage time of pH 4 and Boix-Ochoa score were significantly higher in patient group than that in control group(P 0.01=.According to the diagnostic criteria of GER(Boix-Ochoa score 11.99 and reflux index ≥ 4.0%),pathological GER was detected in 63 of 170(37.1%)in patient group,while it was only 3.1%(1/32)in controls,with the statistical difference(χ2 = 14.327,P 0.01).The significant differences were also observed in the positive rate of GER among the three different age group,they were 47.4%(27/57),41.5%(17/41)and 26.4%(19/72)in patients aged from two months to one years,less than 3 years and more than 3 years,respectively(χ2 = 6.453,P 0.05).Conclusions It was suggested that GER could be the underlying cause of chronic cough in children,especially in those aged less than 3 years.24-hour ambulatory esophageal pH monitoring is a useful test for identifying the etiology of chronic cough in children.
Characteristics of Airway Inflammatory Cells and Cytokines in Patients with Gastroesophageal Refluxinduced "Asthma"

Abstract:Objective To observe the feature of airway inflammation in patients troubled by gastro-oesophageal reflux (GER) with asthma-like symptoms. Methods Thirty-four cases of GER with asthma symptoms, 12 with asthma and 11 healthy subjects were enrolled in this study. Sputum was induced by hypertonic saline aerosol inhalation in all patients and healthy subjects, and the sputum differential cell counts were determined. The concentration of IL-8, -6, and -5 in the supernatant of induced sputum was measured by using enzyme linked immunosorbert assay (ELISA) respectively. Results The sputum eosinophilic percentage in GERDa patients was significantly decreased (P ＜0. 05 ) and the sputum neutrophil percentage was increased as compared with those in asthmatic patients ( P ＜ 0. 05 ). The concentrations of IL-5 (52.86 ±20.25, 55.69 ± 17.72, 70.56 ± 12.63), IL-6 (22. 14 ±9. 18, 18.62 ±9.94, 15.63 ±13.61 ) in the sputum supernatant in all patients were significantly increased as compared with healthy controls (P ＜ 0. 05), and the concentration of IL-5 in asthmatic patients were higher than in GERDa patients (P ＜0. 05). The concentration of IL-8 in GERD patients (308. 33 ± 178.60, 234. 28 ± 130. 98) was significantly increase as compared with healthy controls ( 143.26 ± 32.71, P ＜ 0. 05 ). There was no significant difference in the concentration of IL-8 between asthmatic patients and healthy controls ( P ＞ 0. 05 ).Conclusion Asthma and gastroesophageal reflux are both characterized by airway inflammation. Differed with asthma obtained by airway eosinophilic inflammation and mainly elevated IL-5, the GER-triggered airway inflammation is characterized by a neutrophilic airway inflammation and mainly enhanced levels of IL-8. GER does aggravate pre-existing airway inflammation in asthmatic patients through related cytokines which is the linkage between them.
Chronic Laryngopharyngeal Inflammation Associated with Gastroesophageal Reflux in Children

Abstract:Objectives To investigate the relationship between chronic laryngopharyngeal inflammation and gastroesophageal reflux(GER)in children.Methods Twenty-four-hour esophageal pH monitoring was performed in 37 cases(patient group)with chronic refractory laryngopharyngeal inflammation( 4 weeks),and 30 asymptomatic children served as control group.Results Each parameter of esophageal acid reflux in patient group such as the episodes of acid reflux,the frequency of reflux longer than 5 min,the longest reflux time(min),the percentage time of pH 4 and Boix-Ochoa score were significantly higher than that in control group(all P 0.01).According to the diagnostic criteria of pathological GER(Boix-Ochoa score 11.99 and reflux index ≥ 4.0%),pathological GER was detected in 40.5%(15/37)of patients with chronic laryngopharyngeal inflammation,while it was 3.3%(1/30)in control.There is statistical significance between the two groups(χ2 = 12.617,P 0.01).Conclusions It suggests that GER is closely related to chronic refractory laryngopharyngeal inflammation in children.When routine treatment to childhood chronic laryngopharyngeal inflammation fails,GER may be considered for differential diagnosis.

Abstract:ABSTRACT The biosynthesis of cysteine in bacteria and plants is carried out by a two-step pathway, catalyzed by serine acetyltransferase (SAT) and O-acetylserine sulfhydrylase (OASS; O-acetylserine [thiol] lyase). The aerobic form of OASS forms a tight bienzyme complex with SAT in vivo, termed cysteine synthase. We have determined the crystal structure of OASS in complex with a C-terminal peptide of SAT required for bienzyme complex formation. The binding site of the peptide is at the active site of OASS, and its C-terminal carboxyl group occupies the same anion binding pocket as the α-carboxylate of the O-acetylserine substrate of OASS. These results explain the partial inhibition of OASS by SAT on complex formation as well as the competitive dissociation of the complex by O-acetylserine.

Abstract:Gastroesophageal reflux disease (GERD) as a possible cause of chronic cough is known for decades. However, more than 75% of patients with extraoesophageal symptoms do not suffer from typical symptoms of GERD like pyrosis and regurgitations and have negative upper gastrointestinal endoscopy. For such a condition term laryngopharyngeal reflux (LPR) was introduced and is used for more than two decades. Since the comprehensive information on relationship between chronic cough and LPR is missing the aim of this paper is to summarize current knowledge based on review of published information during last 13 years. Laryngopharyngeal reflux is found in 20% of patients with chronic cough. The main and recognized diagnostic method for LPR is 24-h multichannel intraluminal impedance-pH (MII-pH) monitoring, revealing reflux episodes irritating the upper and lower respiratory tract mucosa. The treatment of LPR should be initiated with dietary and lifestyle measures, followed by proton pump inhibitor (PPI) therapy and other measures. Despite progress, more research is needed for accurate diagnosis and targeted therapies. Key areas for exploration include biomarkers for diagnosis, the impact of non-acid reflux on symptom development, and the efficacy of new drugs. Further studies with a focused population, excluding other causes like asthma, and using new diagnostic criteria for LPR are essential. It’s crucial to consider LPR as a potential cause of unexplained chronic cough and to approach diagnosis and treatment with a multidisciplinary perspective.

Abstract:Although the aetiology of chronic cough in guidelines is clearly stated as asthma and related syndromes, gastro-oesophageal reflux disease (GORD), and upper airways disease, the inflammatory mechanisms underlying these conditions differ. Recent studies on asthma have increasingly focused on its molecular phenotypes instead of clinical characteristics. Here, we proposed the hypothesis that divides cough into two groups; the eosinophilic and neutrophilic. This division will enhance our ability to recognise the type of airway inflammation which, as a consequence will lead us to more targeted and personalized treatment approaches.

Abstract:Background: Fractional exhaled nitric oxide (FeNO) measured at multiple exhalation flow rates can be used as a biomarker to differentiate central and peripheral airway inflammation. However, the role of alveolar nitric oxide (CaNO) indicating peripheral airway inflammation remains unclear in gastroesophageal reflux-associated cough (GERC). Objectives: We aimed to characterize the changes in alveolar nitric oxide (CaNO) and determine its clinical implication in GERC. Design: This is a single-center prospective observational study. Methods: FeNOs at exhalation flow rates of 50 and 200 ml/s were measured in 102 patients with GERC and 134 patients with other causes of chronic cough (non-GERC). CaNO was calculated based on a two-compartment model and the factors associated with CaNO were analyzed. The effect of anti-reflux therapy on CaNO was examined in 26 GERC patients with elevated CaNO. Results: CaNO was significantly elevated in GERC compared with that in non-GERC (4.6 ± 4.4 ppb versus 2.8 ± 2.3 ppb, p 5 ppb) had more proximal reflux events (24 ± 15 versus 9 ± 9 episodes, p = 0.001) and a higher level of pepsin (984.8 ± 492.5 versus 634.5 ± 626.4 pg/ml, p = 0.002) in sputum supernatant than those with normal CaNO. More GERC patients with high CaNO required intensified anti-reflux therapy (χ 2 = 3.963, p = 0.046), as predicted by a sensitivity of 41.7% and specificity of 83.3%. Cough relief paralleled a significant improvement in CaNO (8.3 ± 3.0 versus 4.8 ± 2.6 ppb, p < 0.001). Conclusion: Peripheral airway inflammation can be assessed by CaNO measurement in GERC. High CaNO indicates potential micro-aspiration and may predict a necessity for intensified anti-reflux therapy.

Abstract:Objective: To investigate the differentiation of airway inflammation, bronchial hyperresponsiveness and anti-asthma therapy responses between the Cough variant asthma (CVA) and classic asthma (CA) patients with FEV1%≥80% predicted. Methods: 402 patients of suspected CA and 544 patients of chronic cough with FEV1 ≥80% predicted were enrolled in the first monocentre retrospective cross-sectional study. Further prospective monocentre study was conducted and 66 patients of suspected asthma with negative bronchodilation test (BDT) but positive bronchial challenge test (BCT) were enrolled and followed up for 4 weeks. Results: Both CA and CVA patients showed decreased FEV1%, MEF50%, MEF25%, and MMEF% values; increased EOS% and FENO values. CA patients had higher FENO values than CVA patients (36.0ppb vs. 24.0ppb, p<0.0001). The predictive value of FENO for positive BCT in chronic cough patients was much lower than that in suspected CA patients (AUC=0.603 vs. 0.728). After anti-asthma therapy of 4 weeks, dramatically improvement of symptoms, large and small airway function were found both in CVA and CA groups with no difference between groups. The 2 most valuable spirometric variables for predicting better anti-asthma response were ΔFEV1 (cut-off values=90ml for CA and 110ml for CVA) and ΔFEV1% (cut-off values=3.49% for CA and 2.59% for CVA) in CA and CVA patients respectively. Conclusion: CVA patients showed less airway eosinophilic inflammation than CA patients. Most CA and CVA patients can benefit promptly from anti-asthma therapy,the improvement of FEV1 and FEV1% in BDT may predict anti-asthma therapy response.
INVESTIGATION OF PEPSIN LEVELS IN BRONCHIAL LAVAGE IN PATIENTS WITH INTERSTITIAL LUNG DISEASE AND CHRONIC COUGH

Abstract:Aim: Pepsin is an enzyme that helps digest protein secreted only from the gastric chief cell in an inactive state. Pepsin is a good marker for acidic gastroesophageal reflux (GER). Its presence in sputum or saliva is considered pathologic. In GER, cough is stimulated by broncho-esophageal neurogenic reflex and aspiration of gastric contents into the airways. GER is the most common cause of cough. Gastric acid reflux is also thought to play a role in Interstitial Lung Disease (ILD) etiology. In many studies, pepsin and bile acid levels in bronchial lavage were high in patients with interstitial lung disease and chronic cough. In our study, we aimed to evaluate pepsin levels in bronchial lavage in patients with ILD and chronic cough and to investigate the relationship between symptoms and reflux treatment. Methods: Between January 2021 and February 2022, 212 patients who underwent bronchoscopy in our tertiary clinic were evaluated. These patients were divided into three groups: 52 patients with interstitial lung disease, 81 patients with chronic cough, and 79 patients who underwent bronchoscopy with a pre-diagnosis of lung cancer as the control group. Bronchial lavage obtained by bronchoscopy was analyzed for pepsin levels. Results: Shortness of breath and cough were the most common symptoms in all three groups. Pepsin levels were 16.71±8.6 ng/ml in the chronic cough group, 15.6±8.9 ng/ml in the ILD group, and 10.58±5.4 ng/ml in the lung cancer (control) group. Pepsin levels in the ILD and chronic cough group were statistically significantly higher than in the lung cancer group (p:0.00). There was no statistical difference between the ILD group and the chronic cough group regarding pepsin levels. It was found that pepsin levels were lower in the three groups who received anti-reflux treatment. There was no difference in pepsin levels between ILD subgroups. Conclusion: Pepsin levels in bronchial lavage were higher in the ILD and chronic cough groups. This suggests that reflux may be involved in the etiology of chronic cough and ILD. Low pepsin values in patients receiving anti-reflux therapy have shown that occult reflux may occur. In our study, the high level of pepsin in bronchial lavage, especially in the chronic cough and ILD group, may be instructive in the etiology and treatment planning of the disease.

Abstract:BACKGROUND: Bronchial provocation tests are standard for diagnosing the etiology of chronic cough, but they are time consuming and can induce severe bronchospasm. A safer and faster clinical examination to predict bronchial hyperresponsiveness (BHR) is needed. OBJECTIVE: The objective of this study was to investigate whether small-airway function tests can predict BHR in adult patients with chronic cough. METHODS: A retrospective, cross-sectional study of diagnoses made using spirometry and bronchial provocation test results was performed in 290 patients with chronic nonproductive cough. BHR-predictive values were analyzed via the area under receiver operating characteristic curves (AUCs). Optimal cutoff values were determined by maximizing the sum of sensitivity and specificity. RESULTS: Patients with chronic cough with BHR showed lower forced expiratory flow between 25% and 75% (FEF25%-75%), higher fractional exhaled nitric oxide (FENO), and a higher percentage of eosinophils in blood than patients without BHR (P < .0001 for all). The AUCs of FENO and FEF25%-75% for a BHR diagnosis were 0.788 (95% CI, 0.725-0.851) and 0.702 (95% CI, 0.641-0.763), respectively. Optimal cutoff values were 43 ppb for FENO and 78.5% for FEF25%-75%, with negative predictive values of 85.38% and 81.34%, respectively. The combined use of FENO and FEF25%-75% increased the AUC to 0.843 (95% CI, 0.794-0.892), significantly higher than either FENO (P = .012) or FEF25%-75% (P < .0001) alone. CONCLUSIONS: Small-airway dysfunction is present in patients with chronic cough and BHR. FEF25%-75% has value as a negative predictive parameter for BHR, especially when combined with FENO. FENO > 43 ppb and FEF25%-75% < 78.5% strongly predicted a positive bronchial provocation test. (C) 2017 American Academy of Allergy, Asthma & Immunology

Cough reflex sensitization from esophagus and nose

Abstract:The diseases of the esophagus and nose are among the major factors contributing to chronic cough although their role in different patient populations is debated. Studies in animal models and in humans show that afferent C-fiber activators applied on esophageal or nasal mucosa do not initiate cough, but enhance cough induced by inhaled irritants. These results are consistent with the hypothesis that activation of esophageal and nasal C-fibers contribute to cough reflex hypersensitivity observed in chronic cough patients with gastroesophageal reflux disease (GERD) and chronic rhinitis, respectively. The afferent nerves mediating cough sensitization from the esophagus are probably the neural crest-derived vagal jugular C-fibers. In addition to their responsiveness to high concentration of acid typical for gastroesophageal reflux (pH < 5), esophageal C-fibers also express receptors for activation by weakly acidic reflux such as receptors highly sensitive to acid and receptors for bile acids. The nature of sensory pathways from the nose and their activators relevant for cough sensitization are less understood. Increased cough reflex sensitivity was also reported in many patients with GERD or rhinitis who do not complain of cough indicating that additional endogenous or exogenous factors may be required to develop chronic coughing in these diseases.
Fat Laden Macrophages in Tracheal Aspirates as a Marker of Reflux Aspiration: A Negative Report

Abstract:Objectives Refluxed gastric material aspirated into the lungs is an important cause of acute and chronic pulmonary disease. Currently, the presence of fat‐laden macrophages (FLM) in tracheobronchial secretions of children is a conventional marker for reflux aspiration. However, this assay is limited by its apparent lack of specificity. The aim of this study was to reevaluate the role of this assay in diagnosing reflux aspiration. Methods The tracheal aspirates of sixty‐four consecutive children with clinically significant gastroesophageal reflux undergoing upper gastrointestinal endoscopy under general anesthesia, and 34 other children from the routine operative schedule were evaluated. Both groups were further subdivided on the basis of presence or absence of associated respiratory symptoms. After intubation, tracheal aspirates were collected, fixed, and stained for FLM. By grading the amount of intracellular fat present, a semiquantitative lipid index was computed. Tracheal aspirates with a lipid index of 100 or greater were considered positive. Results Twenty‐four of the 64 children with reflux symptoms and 14 of 34 children without reflux symptoms were positive for FLM. Sixteen of 37 children with both gastroesophageal reflux and respiratory symptoms and 10 of 26 children negative for both tested positive for FLM. The mean lipid index of the subgroup of children with both reflux and respiratory symptoms was not significantly different from that of the subgroup that was negative for both conditions. Despite computing a semiquantitative lipid index, an index of 100 or greater only had a sensitivity of 38% and specificity of 59%. Conclusion Assay of FLM in the tracheal aspirates of children considered at risk of reflux aspiration is not a sensitive or specific as a marker for reflux aspiration.

The role of neutrophils in chronic cough

Abstract:Chronic cough is a common disorder lasting more than 8 weeks and affecting all age groups. The evidence supporting the role of neutrophils in chronic cough pathology is based on many patients with chronic cough developing airway neutrophilia. How neutrophils influence the development of chronic cough is unknown. However, they are likely involved in multiple aspects of cough etiology, including promoting airway inflammation, airway remodeling, hyper-responsiveness, local neurogenic inflammation, and other possible mechanisms. Neutrophilic airway inflammation is also associated with refractory cough, poor control of underlying diseases (e.g., asthma), and insensitivity to cough suppressant therapy. The potential for targeting neutrophils in chronic cough needs exploration, including developing new drugs targeting one or more neutrophil-mediated pathways or altering the neutrophil phenotype to alleviate chronic cough. How the airway microbiome differs, plays a role, and interacts with neutrophils in different cough etiologies is poorly understood. Future studies should focus on understanding the relationship between the airway microbiome and neutrophils.

Abstract:Typical reflux symptoms associated with gastroesophageal reflux-related cough have diagnostic implications. Questionnaires including these symptoms can be used as valuable screening tools for potential candidates. When reflux monitoring provides the objective evidence of abnormal reflux, the favorable response to anti-reflux therapy is essential for the diagnostic confirmation, in which anti-acid drugs are still the first-line choice. The benefits of neuromodulators in the treatment of refractory gastroesophageal reflux-related cough need to be further clarified. However, anti-reflux surgery may be a promising curative therapy for some patients with definitive surgical indications.

The study protocol consisted of inhalation challenge with methacholine and exercise challenge. Count of blood eosinophil cells and level of blood total IgE were studied. Serum eosinophil cationic protein (ECP) concentration was measured in the asthmatic bronchitis group.RESULTS:The positive ratio to methacholine response was 77.8% in chronic asthmatic bronchitis group, whereas the ratios were 100.0% and 41.4% in bronchial asthma and simple chronic bronchitis groups respectively. According to the response to exercise challenge, patients with chronic asthmatic bronchitis were divided into positive and negative groups. The cumulative dose of methacholine producing a 20% fall in FEV(1) (PD(20)-FEV(1)) was different between the two groups. In the positive group (9 patients), none was negative with methacholine response and PD(20)-FEV(1) being similar to the bronchial asthma group. The blood total IgE and ECP levels were significantly higher in the positive than in the negative group.CONCLUSION:The characteristics of airway reactivity in chronic asthmatic bronchitis are midway between bronchial asthma and simple chronic bronchitis. The nature of chronic asthmatic bronchitis may be different in different patients.
Gastro-Esophageal Reflux Is Not a Common Cause of Chronic Cough; A Singapore Case Series

Abstract:Background: Gastro-esophageal reflux disease (GERD) is believed to be a common cause of chronic cough. There is a paucity of data on GERD-related cough (GERC) from Singapore. Our aim was to examine the prevalence, demographics and clinical features of GERC patients seen in a large teaching hospital in Singapore.Methods: We did a retrospective review of patients referred to the respiratory clinic for evaluation of chronic cough (≥ 6 weeks in duration). All patients diagnosed with GERC were further classified into 2 groups based on the likelihood of esophageal reflux being the cause of cough. 1) Likely GERC and 2) Possible GERC. We describe the demographics, clinical characteristics and the outcomes of these patients.Results: Of the 330 chronic cough patients seen over a 6 years period, 45 patients (13%) were diagnosed with GERC. Most were women (69%), the median age was 53 years and the median duration of symptoms was 26 weeks. Fourteen patients were in the Likely group and 31 in the Possible group. Throat symptoms or signs were found in 77 % of patients.Conclusions: Amongst patients referred for cough to a specialist clinic, GERD was not seen as a common cause. Throat signs and symptoms were common and could add weight to the diagnosis of GERC. There was no particular timing for the cough with regards to day or night.

Abstract:The bacteria of the Bacteroides fragilis group are considered important clinical pathogens and they are the most common anaerobes isolated from human endogenous infections. In this study, the susceptibility patterns to antibiotics and metals of 114 species of the B. fragilis group isolated from children with and without diarrhea were determined. Susceptibility was assayed by using an agar dilution method with Wilkins-Chalgren agar. All B. fragilis strains were resistant to lead and nickel, but susceptible to metronidazole and imipenem. beta-lactamase production was detected by using biological and nitrocefin methods, respectively, in 50% and 90.6% of the isolates of children with diarrhea and in 60% and 90% of the isolates of children without diarrhea. Our results show an increase of antibiotics and metals resistance in this microbial group, and a periodic evaluation of the antimicrobial susceptibility is needed. In Brazil, the contamination for antibiotics or metal ions is often observed, and it is suggested an increase the antimicrobial resistance surveillance of this microbial group, mainly those isolated from children's diarrhea.
