Abstract:Objective To investigate the correlation between severe pre-eclampsia and the changes of factors such as GMP-140,D-dimer (D-D) in pro-coagulation. Methods 200 pregnant women of 20 to 36 gestational weeks were collected and classified into 4 groups according to the gestational weeks. PT,APTT,and the level of PLT,GMP-140,D-D,FIB in the peripheral blood were detected. The pregnant women who had two or more abnormal indexes were considered as high risk,and were divided into two groups randomly. The treatment group was dealt with intravenous drip of heparin and salvia,while the control group was given normal sodium instead. The indexes of blood coagulation system were measured dynamically and the disease incidence was collected. Results The levels of PLT,PT and APTT were significantly lower in heavy pre-eclampsia patients (P<0.05),with the concentrations of GMP-140 and D-D in plasma significantly higher (P<0.05),compared to the control group. As for predicting,when the level of GMP-140 greater than or equal to 68.0 lg/L and D-D greater than or equal to 0.28mg/L,the positive predictive value of the incidence of heavy pre-eclampsia was 55.5% while the negative predictive value reached 78.1%. The specificity and sensitivity were 51.3% and 81.2% in the first group,respectively. In the second group,these indexes were 82.1%,79.3%,68.2% and 84.4% respectively. In the third group ,they were 85.3%,83.1%,76.4% and 85.3% respectively. In the forth group ,they were 87.2%,89.1%,80.8% and 90.6% respectively. The incidence rate of severe pre-eclampsia was 7.5% in the treatment group and 17.76% in the control group. There was a significant difference between the two groups (P<0.05). Conclusion GMP-140 and D-D are two reliable indicators for pre-eclampsia in patients of pre-coagulation. Moreover,the incidence of pre-eclampsia can be degraded if heparin and salvia are used early.
Efficacy of Phloroglucinol for Acceleration of Labour: a Systematic Review and Meta‑analysis

Abstract:Purpose To assess the efficacy of phloroglucinol for acceleration of labour. Methods Randomized controlled trials (RCTs) comparing phloroglucinol with placebo were searched in PubMed, Embase and the Cochrane Library. Literatures were collected up to April 2020. Primary outcomes were the duration of labour and average blood loss. Finally, a total of 4 RCTs, 377 patients were included in this meta-analysis. The included RCTs were analyzed by the software Rev Man 5. 3. Results In the phloroglucinol group, the duration of the first stage was reduced by 116.04 min (95% CI 107.71 to 124.68), and the duration of the second stage was reduced by 10.75 min (95% CI 8.79 to 12.70). The average blood loss was reduced by 16.07 ml, which was statistically different from the control group. Conclusion The application of phloroglucinol is proved to be effective for accelerating the labour process, reducing the risk of maternal and neonatal complications.
Predator biodiversity strengthens herbivore suppression.

Abstract:Species diversity at lower trophic levels generally improves ecosystem functioning. However, the impact of greater predator diversity on herbivore regulation is uncertain because predator species both compete with and prey on each other. In a large-scale field experiment we examined the relationship between predator species diversity and the suppression of two herbivores, green peach and cabbage aphids, on collard plants. We show that, for both aphid species, the strength of herbivore suppression increased with higher predator biodiversity. Greater resource exploitation by predators in diverse communities generally led to improved predator survivorship and reproduction. Herbivore population size was negatively correlated with plant biomass, providing evidence that greater aphid suppression leads to improved plant growth. Our study suggests a harmonious relationship between predator conservation and herbivore control, and a relatively weak role for predator interference, within this community.
A Multi-Center Study on the Clinical Value of Phloroglucinol on the Threatened Abortion Treatment During the First 20 Weeks of Pregnancy

Abstract:Objective To evaluate the medical effects of phloroglucinol on the threatened abortion treatment during the first 20 weeks of pregnancy.Methods 127 cases of threatened abortion patients were recruited from four hospitals from 2010 to 2011.The patients were divided into four groups.Group A (4-12 weeks after pregnancy) and group C (12-20 weeks after pregnancy) were given with both phloroglucinol and routine drugs.Group B (4-12 weeks after pregnancy) and group D (12-20 weeks after pregnancy) were given with routine drugs.Vaginal bleeding,lower abdominal pain,the levels of HCG and progesterone,and the pregnancy outcomes were monitored.Results The incidence of vaginal bleeding in the group A and B patients was decreased from 86.84% to 18.42% and 64.10% to 20.51% in B group,respectively,and the difference was significant (P0.05).The incidence of abdominal discomfort in the group A and B patients was decreased from 44.74% to 7.89% and 28.20% to 5.13%,respectively,and the difference was not significant.During the 4-8 weeks of pregnancy,patients receiving the combined treatment had an increased in the level of HCG from (44612± 49365)IU/L to (120802.67±56900)IU/L.Patients receiving conventional treatment also had an increased in the level of HCG from (28080±43892)IU/L to (55 166±36 250)IU/L,and the difference was significant (P 0.05).The treatment days of A group and B group were (7.97±3.83)days and (11.44±6.32)days,the treatment days were (6.42±3.12)days and (9.53±7.75)days in C group and D group,there is significant difference between A group and B group (P 0.01),and between C group and D group (P0.05).There was fewer untoward reaction after given Phloroglucinol.Conclusions Phloroglucinol is safe and effective in the treatment of threatened abortion patients..Vaginal bleeding and abdominal pain can be controlled within a relatively short period after given phloroglucinol and routine drugs.It can increase the level of HCG in the early pregnancy patients.
Study of ligustrazine combined with bitter salt on pregnancy induced hypertension syndrome

Abstract:Objective It is to discuss the effect of ligustrazine combined with bitter salt on pregnancy induced hypertension syndrome (PIH) and the treatment mechanism through observing the changes of placental structure.Methods Sixty-six patients were randomly divided into control group (34 patients) and treatment group (32 patients). The patients in control group were injected or intravenous dripped with 25% bitter salt, the patients in treatment group were added intravenous dripped with ligustrazine. After treatment, the placental tissues were gathered and made pathology observation, and the changes of ultrastructure were observed with transmission electromicroscopy.Results The exponent scores of PIH and average artery pressure in treatment group were significantly lower than that in control group (P0.001), and the effect of depressing blood pressure and albuminuria was heightened. The syncytiotrophoblast cell tubercles of PIH placenta were increased, the syncytiotrophoblast cells faded. In treatment group, the vacuoles in syncytiotrophoblast cell cytoplasms were reduced, the surface microvilli were increased and the endodermis cell organelles of the capillary were recovered.Conclusion Ligustrazine combined with bitter salt can improve the blood-supply of the placenta on some degree, so it is an effective and safe method on PIH.
[Evaluation of the Efficacy of Ligustrazine Collaborated with Magnesium Sulfate in the Treatment of Pregnancy-Induced Hypertension in Rats].

Abstract:The aim of this study was to evaluate the outcome of treatment of pregnancy--induced hypertension (PIH) in rats by Ligustrazine collaborated with magnesium sulfate. PIH rat models were induced with Nomega-nitro-L-arginine methyl ester (L-NAME) infusing at 7 mg/kg per day via caudal vein for four days, then treated with Ligustrazine, magnesium sulfate, or both for three days. Rat blood pressure level was measured by the tail-cuff method, 24 hours urine protein was also assayed. The blood pressure and urine proteins of grouped PIH rats were recorded before the start and after the termination of therapy. The body length and the weight of fetal rats, the weight of placentals from pregnant rats were measured. The placental tissues, livers, kidneys of rats were investigated with integrated methods such as histopathologic observation with light microscopy, ultrastructural observation with transmission electron microscopy. L-NAME administration in pregnancy rats during the late pregnancy period had resulted in an rise of blood pressure, an increasing of urine protein, death rate of rat fetus, incidence of teratogenesis, and so on. Three groups of PIH rats treated with single magnesium sulfate, Ligustrazine, or Ligustrazine combined with magnesium sulfate showed an obvious dropping of the proteinuria, decompression of blood pressure (p<0.01, p<0.001), especially the treatment efficacy in the group of Ligustrazine combined with magnesium sulfate was more significant effective than other two groups (p<0.01, p<0.001). The treatment with both Ligustrazine and magnesium sulfate could increase the body length of newly born rats, the body weight of tomites and the placental weight, furthermore, reduce the rate of the teratosis of hindlimb-shortness (p<0.001). There were diffuse focal necrosis areas in the livers of PIH rats, their glomerular basement membrane had thickened extensively, the glomerular endothelium had swelled, extensive edema in the epithelia of renal tubule was demonstrated. The decidua and basal zone of the placentae of PIH rats all thickened, the microvilli of trophoblasts decreased. After treatment with ligustrazine especially with both Ligustrazine and magnesium sulfate, the necrosis of hepatocytes disappeared. The thickening of glomerular basement membrane in the group of ligustrazine or both Ligustrazine and magnesium sulfate treatment reduced; Moreover in the latter group the morphology of glomerular endothelium essentially recovered, the edema in cytoplasms of renal tubular epithelium reduced. The placental lesions were also relieved. The present results indicated the therapeutic effect by Ligustrazine collaborated with magnesium sulfate was better than a single use of Ligustrazine or magnesium sulfate. There were pathological alteration involved ischemia and anoxic in the placental tissues of PIH rats, resembled the placental pathological alteration of the human cases with PIH. The treatment with ligustrazine, and especially both Ligustrazine and magnesium sulfate in PIH rats could obviously relieve the lesions in lives, kidneys and placentae.
Efficacy and safety of Nicardipine in treatment of severe hypertensive disorder complicating pregnancy

Abstract:Objective To evaluate the clinical efficacy and safety of nicardipine in treatment of hypertensive disorder complicating pregnancy. Methods Sixty-two women with severe hypertensive disorder complicating pregnancy registered to three hospitals between March 2009 and March 2010 were included in this study.These patients were divided into two groups randomly to receive treatment with labetalol(n=30) or nicardipine(n=32).Changes of blood pressure before and after treatment,perinatal outcomes and side effects of medication were observed in both groups. Results In nicardipine group,the target levels of blood pressure were achieved at 1 hour after medication.The medication was shifted to oral use in 13 cases,which 7 patients required treatment in combination with other antihypertensive drugs.In the labetalol group,the target levels of blood pressure were achieved at 4 hours after medication.The medication was shifted to oral use in 9 cases,which 11 patients required treatment in combination with other antihypertensive drugs.There were a significant difference in antihypertensive effects between two groups(P<0.05).The birth weight and placenta weight in nicardipine group were significantly greater than those in labetalol group(P<0.05).There was no statistic difference in changes of heart rate before and after treatment in two groups(P>0.05). Conclusion Nicardipine may provide effective control of blood pressure and better pregnancy outcome for women with severe hypertensive disorder complicating pregnancy.
Efficacy of nifedipine tablets plus aspirin in patients with gestational hypertension and the effect on coagulation function and hemorheology

Abstract:Objective: This study aimed to investigate the clinical efficacy of Nifedipine tablets plus Aspirin for hypertensive disorder complicating pregnancy and the effect on coagulation function and hemorheology. Methods: A retrospective analysis of the clinical data from 108 patients with gestational hypertension hospitalized between March 2016 and March 2017 was carried out. These patients were randomly assigned into a research group and a control group, with 54 patients in each group. Patients were treated with Aspirin in the control group, and the combination of Nifedipine tablets and Aspirin in the research group so as to compare the clinical efficacy and the effect on coagulation function and hemorheology after therapy. Albumin, total protein, 24-hour urinary protein and mean arterial pressure (MAP) were observed and compared between the two groups. Results: The overall effective rate of treatment in the research group was significantly higher than that in the control group (P < 0.05). The prothrombin time (PT) and fibrinogen (FIB) levels were markedly superior to those before therapy in the research group (P < 0.001), and the PT and FIB after treatment were remarkably better in the research group than in the control group (P < 0.001). The levels of plasma viscosity (PV), low-shear whole blood viscosity (LBV), and high-shear whole blood viscosity (HBV) after therapy were markedly reduced than before therapy in the two groups (P < 0.05), and the levels of PV, LBV, and HBV after therapy were significantly lower in the research group than those in the control group (P < 0.05). Adverse reactions after therapy were reported at a significantly lower incidence in the research group than the control group (P < 0.05). Adverse pregnancy outcomes were reported at a significantly lower incidence in the research group than in the control group (P < 0.05). The research group significantly outperformed the control group on Albumin, total protein, 24-hour urinary protein and mean arterial pressure (P < 0.05). Conclusion: Nifedipine tablets plus Aspirin for patients with gestational hypertension can effectively improve coagulation function and hemorheological parameters, with high safety.

Abstract:Objectives: Interleukin-18 (IL-18) is a proinflammatory cytokine with pleiotrophic qualities and its roles in pregnancy, labor onset and pregnant complications have been proposed. However, the alterations of serum and placental IL-18 have been less investigated. The objective of the current investigation was to detect IL-18 concentrations in serum and placentas from patients with preeclampsia and women with normal pregnancy.Methods: 27 patients with preeclampsia and 28 women with normal pregnancy were recruited. Blood and placental samples were taken and IL-18 concentrations in serum and placental homogenate were analyzed by enzyme-linked immunosorbent assay (ELISA). The levels of IL-18 in serum and placenta were compared between preeclampsia and control.Results: Both serum and placental levels of IL-18 were significantly increased in preeclampsia as compared with control (P = 0.014 and 0.003, respectively). Serum and placental levels of IL-18 were not significantly different in preeclamptic women who received dexomethasone and those who did not (P = 0.223 and 0.330, respectively). Serum and placental IL-18 levels were not significantly different between preeclamptic women who delivered before 36 complete gestational weeks and those who delivered after 36 complete weeks (P = 0.616 and 0.869, respectively). There were no significant differences in serum and placental IL- 18 between women with mild preeclampsia and those with severe preeclampsia (P = 0.056 and 0.357, respectively).Conclusions: Increased serum and placental levels of IL-18 were observed in preeclampsia. Changes and clinical significance of serum inflammatory factors in the treatment of pregnancy hypertension syndrome with magnesium sulfate combined with nifedipine

Abstract:Curative effect of magnesium sulfate combined with nifedipine on pregnancy-induced hypertension and the effect of serum inflammatory factors were investigated. A total of 188 cases of patients were collected as the research subjects. They all had pregnancy-induced hypertension, and were admitted to The First People's Hospital of Yunnan Province hospital from June 2016 to February 2018. There were, 94 patients treated with magnesium sulfate in the control group, and further 94 patients treated with magnesium sulfate combined with nifedipine in the study group. ELISA was used to detect the expression levels of suppressors of cytokine signaling-3 (SOCS-3), interleukin-10 (IL-10) and interleukin 18 (IL-18), and the relationship between serum inflammatory factors and efficacy was analyzed. The curative effect and eutocia rate in the study group were significantly higher than those in the control group (P<0.05), and the cesarean section rate was lower than that of the control group (P<0.05). In the study group, the adverse reactions were significantly lower than those in the control group (P<0.05). After treatment, the expression levels of serum SOCS-3 and IL-10 in the study group were significantly higher than those in the control group (P<0.05), while the expression level of serum IL-18 was significantly lower than that in the control group (P<0.05). The area under the curve (AUC) of the predictive value of SOCS-3, IL-10 and IL-18 in pregnancy-induced hypertension was 0.717, 0.727 and 0.725, respectively. The best specificity was 76.19, 52.98 and 61.90%, respectively, when the cut-off value was <0.553 ng/l, 48.06 ng/ml and 269.46 ng/ml, and their sensitivity was 70.00, 94.74 and 85.00%, respectively. In conclusion, magnesium sulfate combined with nifedipine significantly improved the disease course of pregnancy-induced hypertension. The levels of SOCS-3, IL-10 and IL-18 in patients are correlated with the curative effect of pregnancy-induced hypertension, suggesting that they have important value in the treatment and monitoring of gestational hypertension.
The effect of phloroglucinol on pain in first-trimester surgical abortion: a double-blind randomized controlled study

Abstract:Background: First-trimester surgical abortion is a common procedure. Pain control during this procedure is still an unsolved problem. Study design: In this randomized, double-blind placebo-controlled study, women presenting for first-trimester surgical abortion received intramuscular phloroglucinol (4 mL) or placebo (normal saline, 4 mL). Visual analog scales (VAS) for pain immediately and 30 min after the procedure and side effects of the drug were recorded. Results: There was no significant difference between groups in the pain level immediately and 30 min after the procedure; 70.7% of the phloroglucinol group (n=58 cases) and 56.9% of the placebo group (n=58 cases ) reported mild pain; 27.6% and 34.5%, respectively, reported moderate pain; and 1.7% and 8.6%, respectively, reported severe pain. Thirty minutes after the procedure, the median pain score was reduced to 1.3 in both groups. Postoperative side effects were reported, but there was no significant difference between groups for nausea or vomiting and blood pressure. Conclusion: The use of this dose of phloroglucinol, during first-trimester abortion by suction evacuation under local anesthesia with lidocaine, did not relieve pain, but caused no side effects.

Abstract:Gestational hypertension is a common disease in clinical practice, which does great harm to the mother and infant. The purpose of this study was to investigate the relationship between 25-hydroxyvitamin D, sFlt-1, and PLGF and hypertensive disorder complicating pregnancy. Specimen preparation: after delivery or placental caesarean section, in order to avoid calcification and necrosis in the middle of the placenta, an area of about 1.5 cm × 1.5 cm × 1.5 cm should be separated immediately. After dehydration, use a Citadel 2000 dryer to dry it and place it in a block of saline for xylene immunohistochemical staining. Statistical processing was performed according to the proportion of positive cells in each part and the depth of staining. Placental tissue collection and treatment: within 20 minutes after the delivery of the placenta, two pieces of the placental tissue (about 1.0 cm × 1.0 cm × 1.0 cm) were taken from the central zone of the placental maternal surface without obvious bleeding and calcification. They were rinsed repeatedly in normal saline, fixed in 10% neutral formaldehyde solution for 24 hours, dehydrated using an automatic dehydrator, and embedded in paraffin for detection. Before the study, 20 ml of distilled water was added to the sample to stand for 20 minutes; the Cobas E610 immunoanalyzer was turned on, and sFlt-1 and PLGF (placental growth factor) were selected. The serum 25-hydroxyvitamin D level of pregnant women was detected in the fasting state at 24–28 weeks of gestation, and the best collection time was 8 : 00–11 : 00 in the morning. 5 ml of the whole blood sample without anticoagulant was collected and stored at 0–4°C in a cold storage and dark environment. The serum was obtained by high-speed centrifugation within 24 hours after collection. The method is suitable for the quantitative determination of 25-OH-vitamin D in human serum. In the hypertensive pregnancy group, the level of 25-hydroxyvitamin D was 18.44 ± 3.48 ng/ml, and the sFlt-1/PLGF level was highest at 0–5 weeks followed by 5–10 weeks, 10–15 weeks, and 15–20 weeks. This study provides new ideas and experimental clues for the prevention and treatment of pre-eclampsia.

Clinical study of diclofenac, phloroglucinol and progesterone in the treatment of acute renal colic

Abstract:Objective To study the clinical effect of diclofenac,phloroglucinol and progesterone on acute renal colic.Methods From November 2011 to March 2013,146 patients with acute renal colic in our hospital were randomly divided into treatment group and control group,with 73 cases in each group.They were treated with phloroglucinol and progesterone,and the patients in treatment group were treated with diclofenac on the basis of phloroglucinol and progesterone.The clinical effects were observedafter treatment.Results The VAS value of treatment group before and after treatment was(7.54 ± 2.11) and (2.57 ± 1.32) respectively.The VAS value of control group before and after treatment was (7.34 ± 2.32) and (3.76 ± 1.22) respectively.The VAS value of treatment group was significantly higher than that of control group after treatment (t =2.57,P ＜ 0.05).There was no significant difference in the incidence of complications between the two groups(P ＞0.05).And the total treatment efficiency of treatment group was also significantly higher than that of control group (x2 =4.76,P ＜ 0.05).Conclusions The clinical effect of diclofenac combined with phloroglucinol and progesterone on acute renal colic is good,it can effectively relieve the pain,improve the total effective rate,so is worthy to be popularized in cilinics.
Efficacy of Magnesium Sulfate and Labetalol in the Treatment of Pregnancy-Induced Hypertension and Its Effect on Anxiety and Depression: A Retrospective Cohort Study

Abstract:Background: In this study, the effect of magnesium sulfate and labetalol in treating pregnancy-induced hypertension (PIH) and its influence on anxiety and depression in patients are observed, and new ideas for treating anxiety and depression in PIH are introduced. Methods: A retrospective cohort study was conducted to select patients with PlH diagnosed from July 2020 to July 2023 from Affiliated Hospital of Electronic Science and Technology University and Chengdu Women' s and Children's Central Hospital in Chengdu of Sichuan Province. The changes in blood pressure, Edinburgh Postnatal Depression Scale (EPDS), and generalized anxiety disorder 7 (GAD-7) in patients with hypertensive pregnancy were collected and analyzed. Results: In our investigation, 219 patients completed the study, and 36.1% (79/219) of them developed anxiety and depression. According to whether the patients were treated with magnesium sulfate and labetalol, 49 cases were assigned to the magnesium sulfate and labetalol treatment (MSLT) group, and 30 cases were assigned to the conventional treatment (CT) group. Edinburgh Postnatal Depression Scale scores and GAD-7 scores in the MSLT group were significantly lower than those in the CT group, indicating that magnesium sulfate and labetalol can improve anxiety and depression in hypertensive patients during pregnancy. The difference was statistically significant (P < .05). According to the changes in systolic blood pressure, the clinical efficacy of patients was evaluated, and no significant difference in efficacy existed between the MSLT and CT groups. Conclusion: Magnesium sulfate and labetalol can control the blood pressure of patients with PIH and indirectly improve anxiety and depression in patients with PIH, thereby introducing new ideas for the treatment of PIH accompanied by anxiety and depression.
Efficacy of magnesium sulfate combined with nifedipine for pregnancy-induced hypertension syndrome and its relation to glucose and lipid metabolism

Abstract:Objective: To determine the efficacy of magnesium sulfate combined with nifedipine for pregnancy-induced hypertension syndrome (PIHS) and its influence on glucose and lipid metabolism. Methods: The clinical data of 124 cases of PIHS treated in Jiangxi Jiujiang Maternal and Child Care Centers from March 2020 to June 2022 were collected and retrospectively analyzed. Among them, 58 patients who received magnesium sulfate alone were enrolled as a control group, and the other 66 given magnesium sulfate combined with nifedipine were enrolled as a study group. The two groups were compared for treatment efficacy, blood pressure, fasting blood glucose (FBG) and blood lipid indexes (triglycerides (TG), high-density lipoprotein-cholesterol (HDL-C), total cholesterol (TC), and low-density lipoprotein - cholesterol (LDL-C)). Multivariate logistic regression analysis was performed to analyze the factors affecting outcome. Results: The study group showed a significantly higher total effective rate than the control group (P=0.008). After treatment, the study group showed significantly lower systolic blood pressure (SBP) and diastolic blood pressure (DBP) levels than the control group (P<0.001). After treatment, the study group also showed lower levels of FBG, TC, TG, and LDL-C and a higher HDL-C level than the control group (P<0.001). Additionally, the incidences of cesarean section and postpartum hemorrhage were lower in the study group than those of the control group (both P<0.05). The two groups were not significantly different in premature delivery or low neonatal birth weight (both P>0.05), and the incidence of adverse reactions of the two groups was also not greatly different (P>0.05). According to multivariate logistic regression analysis, higher BMI (OR: 3.087, 95% CI: 1.295~7.358) and higher SBP (OR: 1.220, 95% CI: 1.001~1.487) at admission were independent risk factors for poor efficacy, while combined therapeutic regimen (OR: 0.018, 95% CI: 0.001~0.228) was an independent protective factor. Conclusion: Magnesium sulfate combined with nifedipine can deliver a powerful clinical efficacy for patients with PIHS by lowering blood pressure and the incidence of adverse pregnancy outcomes and by improving glucose and lipid metabolism.
[The usefulness of CT in the preoperative diagnosis of metastatic lung nodules--a study with resected cases].

Abstract:The reliability of the preoperative CT diagnosis was studied in patients with pulmonary metastases from osteosarcoma and colorectal cancer as compared with surgical and pathological findings. Seventy-six lesions among 111 surgically proven metastatic nodules were detected preoperatively by CT. But the predictive rate varied greatly depending upon the site and size of the lesions. CT is not so useful in subpleural and apical sites or with nodules of less than 0.2 cm in diameter.
Effect of Magnesium Sulfate Combined with Labetalol in the Treatment of Pregnancy-Induced Hypertension and Its Impact on Adverse Pregnancy Outcomes

Abstract:Objective: To investigate the effect of magnesium sulfate combined with labetalol in treating hypertension in pregnancy (HIP) and its impact on adverse pregnancy outcomes. Methods: A total of 150 HIP patients treated at the Qujing Maternal and Child Healthcare Hospital from June 2020 to June 2023 were randomly assigned using the lottery method. They were divided into the control group (treated with magnesium sulfate alone, 75 cases), and the intervention group (magnesium sulfate + labetalol combined treatment, 75 cases). Blood pressure levels, total adverse reaction rates, total adverse pregnancy outcome rates, and total adverse fetal outcome rates were compared between the two groups. Results: Compared to the control group’s diastolic (87.94 ± 4.86 mmHg) and systolic (138.52 ± 9.23 mmHg) blood pressures, the intervention group showed lower levels of diastolic (78.95 ± 3.57 mmHg) and systolic (129.88 ± 7.47 mmHg) blood pressures (P < 0.05). The intervention group exhibited a lower total adverse reaction rate (4.00%) compared to the control group (14.67%; P < 0.05). Similarly, the total adverse pregnancy outcome rate in the intervention group (4.00%) was significantly lower than that in the control group (18.67%; P < 0.05). Furthermore, the total adverse fetal outcome rate in the intervention group (6.67%) was lower than that in the control group (22.67%; P < 0.05). Conclusion: The combination of magnesium sulfate and labetalol demonstrates significant efficacy in treating HIP. It effectively lowers blood pressure, improves pregnancy and fetal outcomes, is highly safe, and deserves promotion and application.

Abstract:Objective: To investigate the effects of combined medication of nifedipine and magnesium sulfate on the blood pressure, pregnancy-associated plasma protein A (PAPP-A), vascular endothelial growth factor (VEGF), nitric oxide (NO), homocysteine (Hcy) and von Willebrand factor (vWF) in gestational hypertension patients. Methods: A total of 220 gestational hypertension patients were enrolled as the subjects, and divided into two groups randomly, i.e. the observation group and the control group. In observation group, patients took combined medication of nifedipine and magnesium sulfate, while those in the control group only took magnesium sulfate for treatment. Clinical efficacy, and the changes in blood pressure, PAPP-A, VEGF, NO, Hcy and vWF before and after treatment were compared between two groups. Results: In the observation group and the control group, total effectiveness rates were 92.7% and 70.9%, respectively (p < 0.05). After treatment, we found significant decreases in PAPP-A, VEGF, NO, Hcy and vWF in patients of two groups, with more significant decreases in the observation group (p < 0.05). Incidence rates of the adverse reactions in two groups were 5.5% and 6.4%, respectively, without any statistically significant differences (p > 0.05). In the observation group, patients had fewer complications (p < 0.05). Conclusion: Combined medication of magnesium sulfate and nifedipine can decrease the levels of PAPP-A, VEGF, NO, Hcy and vWF in serum as well as the blood pressure of patients with gestational hypertension, with a reduction in incidence rate of complications and improvement in efficacy.
Effect of Blood Pressure Control Level on Perinatal Outcomes in Women with Mild-Moderate Gestational Hypertension

Abstract:Objective To investigate the effect of blood pressure (BP) control level on perinatal outcomes in women with mild-moderate gestational hypertension (GHp). Methods Totally,344 women diagnosed initially as mild-moderate GHp who delivered in Peking University Third Hospital from January 2012 to December 2016 were recruited. They were divided into four groups according to the stabilized level of BP during pregnancy. (1) Group A:BP<130/80 mmHg(1 mmHg=0.133 kPa);(2) Group B:BP(130-139)/(80-89) mmHg; (3) Group C: BP (140-149)/(90-99) mmHg; (4) Group D: BP (150-159)/(100-109)mmHg. The clinical profile and incidence of severe GHp, pre-eclampsia with proteinuria (PE+Upro), severe pre-eclampsia (sPE), small-for-gestational age (SGA) were compared among the four groups. Student t-test was preformed to normal distributive data and Kruskal-Wallis test was used to non-normally distributed variables. Chi-square test was used in count data. Logistic regression analysis was adopted for multiple-factor analysis. Results (1) The incidence of severe GHp in group A was lower than group B (P<0.05). The incidences of severe GHp and sPE in the group B was lower than those in group C (P<0.05). While there was no difference in the incidence of PE+Upro and SGA among the four groups (P>0.05). And the incidence of severe GHp in group D had no difference with group A, B, C (P>0.05). (2) In the 48 patients who used medications to control BP, the occurence of severe GHp in those whose initial BP was(140-149)/(90-99)mmHg was lower than those of≥160/110 mmHg (P<0.05). But the incidence of severe GHp had no significant difference between patients whose initial BP was(140-149)/(90-99)mmHg and patients whose initial BP was(150-159)/(100-109)mmHg (P>0.05). The initial BP level had no impact on the incidence of PE+Upro, sPE and SGA (P>0.05). (3) Multivariate logistic regression analysis showed that the BP level before using medications (OR=3.566, 95%CI:1.080-11.771, P=0.037) and the BP level maintained (OR=4.787, 95%CI:1.115-20.551,P=0.035) were independent factor that affected the incidence of severe GHp. Edema (OR=2.651, 95%CI:1.628-4.316 P=0.000), fetal growth restriction(FGR;OR=1.103, 95%CI:1.427-5.914,P=0.002)and the onset gestational age of GHp (OR=0.755, 95%CI:0.578-0.985,P=0.038) were independent factors that affected the incidence of PE+Upro. The tendency of FGR (OR=17.787, 95%CI:1.833-40.396 P=0.000), history of PE (OR=5.294, 95%CI:1.086-25.800,P=0.039) and the BP level during pregnancy (OR=2.109, 95%CI:1.274-3.491,P=0.004) were independent factors affecting the incidence of sPE. FGR tendency was independent factor affecting the incidence of SGA (OR=25.622, 95%CI:2.596-252.864,P=0.005). Conclusion A satisfied control of BP is helpful to reduce severe GHp and sPE, but the incidence of SGA does not affected.
