Abstract:BACKGROUND:Retinal detachment (RD) secondary to macular hole (MH) is a common complication in highly myopic eyes, usually leading to a poor visual prognosis. The purpose of this study was to evaluate the surgical outcome of silicone oil (SO) tamponade and internal limiting membrane (ILM) peeling in the treatment of RD caused by MH (MHRD) in highly myopic eyes with chorioretinal atrophy, and to identify clinical factors associated with the anatomical outcomes.METHODS:We retrospectively reviewed 21 eyes of 21 highly myopic patients affected by RD secondary to MH and chorioretinal atrophy. All eyes were treated with pars plana vitrectomy (PPV) with ILM peeling and SO tamponade. Anatomical success was defined as reattachment of the retina with the closure of the MH, as assessed by optical coherence tomography (OCT), after SO removal. Logistic regression was performed to determine the clinical factors influencing anatomical success.RESULTS:The mean patient age was 59.95 years [standard deviation (SD), 10.39; range, 34-77 years] and the mean axial length was 30.58 mm (SD, 1.52; range, 27.99-34.51 mm). After the first surgical procedure, the anatomical success rate was 61.9% (13 eyes in 21 eyes), with initial retinal attachment of 16 eyes (76.2%). A second surgical approach was performed for the five eyes with persistent or recurrent RD, and the final retinal reattachment rate was 100% (21/21). Logistic regression analysis showed that no specific factors were significantly associated with anatomical success.CONCLUSIONS:Primary silicone oil tamponade and ILM peeling can be a practical treatment for repairing MHRD in highly myopic eyes with chorioretinal atrophy.

Abstract:Purpose: To investigate the prognostic factors for recurrent rhegmatogenous retinal detachment following silicone oil removal. Methods: This retrospective review included 147 consecutive patients with rhegmatogenous retinal detachment treated with silicone oil tamponade at a high-volume referral-based tertiary hospital between January 2012 and May 2022. All patients underwent follow-up for a minimum of 6 months after subsequent silicone oil removal. The primary outcome measure was the rate of recurrent retinal detachment following silicone oil removal, and the secondary outcome was best-corrected visual acuity after silicone oil removal. Results: The mean silicone oil tamponade duration was 4.7 ± 5.01 months (range, 1-38 months; median, 3 months), and the recurrent retinal detachment rate after silicone oil removal was 15.6%. Logistic regression analysis revealed that argon endolaser photocoagulation during silicone oil removal (odds ratio [OR], 0.309; 95% confidence interval [CI], 0.106-0.898; p = 0.031) was associated with a lower rate of anatomical success after silicone oil removal. Demographics, preoperative ocular characteristics, proliferative vitreoretinopathy, previous scleral encircling or buckling, previous retinectomy, concomitant phacoemulsification, duration of silicone oil tamponade, and gas tamponade after silicone oil removal were not significantly associated with recurrent retinal redetachment after silicone oil removal. Duration of silicone oil tamponade (OR, 1.226; 95% CI, 1.073-1.402; p = 0.003) and recurrent retinal detachment after silicone oil removal (OR, 3.400; 95% CI, 1.311-8.817; p = 0.012) were associated with poor visual outcomes after silicone oil removal. Conclusions: Among all factors examined in this study, including the duration of silicone oil tamponade, laser retinopexy was the only significant prognostic factor for recurrent retinal detachment after silicone oil removal. A longer duration of silicone oil tamponade was associated with worse visual outcomes and a lower rate of visual improvement after silicone oil removal.

Abstract:Background: Cystoid macular edema (CME) can develop following silicone oil placement in complex vitreoretinal surgeries, contributing to poor visual outcomes. In this study, we investigated the clinical and surgical characteristics associated with the development of CME following the use of silicone oil (SO) in pars plana vitrectomy (PPV) surgeries. Methods: We conducted a retrospective chart review of patients who underwent implantation of SO during PPV from 2010 to 2020 by a single surgeon. Patient demographics, type of oil, duration of oil tamponade, retinectomy size, diabetic status, lens status, prior panretinal photocoagulation, visual acuity, and incidence of CME were reviewed. Results: This study included 43 eyes from 40 patients who underwent SO tamponade for retinal detachment (RD) surgery. The mean duration of SO tamponade was 15.7 ± 12.7 months (range: 1-58 months). The most common indication for surgery was diabetic tractional RD (32.7%), followed by traumatic RD (16.3%) and rhegmatogenous RD with proliferative vitreoretinopathy (11.6%). Of the 43 eyes, 18 (41.9%) developed CME for the first time after PPV with SO placement, with 8 (44%) resolving within a year of oil removal. The mean duration for the development of CME was 9 months. A logistic regression model showed that a scleral buckle procedure and poor initial vision were statistically significant factors for predicting the development of CME (ORs: 11.65 and 16.06, respectively). Overall, 91% of the patients had stable or improved vision after surgery. Conclusions: The use of a scleral buckle procedure and poor initial vision are significant factors for predicting CME following silicone oil tamponade in PPV surgeries, with 41.9% of patients developing CME with an average duration of 9 months. Recognizing such factors can lead to early monitoring and prompt management of CME. Meeting presentation: Partial analyses were presented at the ASRS 2020 conference. Abstract:Purpose To investigate the risk factors associated with retinal detachment recurrence after first vitrectomy in high myopic eyes with macular hole retinal detachment (MHRD). Methods Patients with high myopic eyes with MHRD who underwent pars plana vitrectomy and silicone oil (SO) tamponade with a follow-up period more than 12 months and more than 3 months after SO removal were included in this retrospective study. Logistic regression was performed to determine the risk factors associated with retinal re-detachment. Results A total of 45 eyes from 43 patients were included in this study (11 male and 34 female patients). The retinal re-detachment rate after the first removal of silicon oil was 35.5% (16/45) in a mean postoperative follow-up time of 35.64 +/- 32.94 months. Complete macular atrophy on fundus photography (odds ratio (OR) = 17.021, 95% confidence interval (95% CI): 2.218-130.609, p = 0.006) was a risk factor for MHRD after SO removal, while internal limiting membrane (ILM) peeling (OR = 0.091, 95% CI: 0.013-0.633, p = 0.015) and duration of SO tamponade (OR = 0.667, 95% CI: 0.454-0.980, p = 0.039) were protective factors. Conclusion For high myopic eyes with MHRD, complete macular atrophy was a significant risk factor for retinal re-detachment after silicon oil removal. ILM peeling and the duration of silicon oil tamponade were protective factors.

Abstract:Aim: To investigate the cause of visual loss following removal of intraocular silicone oil in patients who underwent vitrectomies for retinal detachment or giant retinal tear. Methods: The clinical records of three cases with visual loss following removal of silicone oil were reviewed. Investigations carried out included fundus fluorescein angiogram, optical coherence tomography, and electrophysiological studies. Results: Visual acuities dropped from 6/9 to 6/36 in two cases and 6/24 to 3/24 in the third. None of the three cases had macula detachment at any stage. Fundus fluorescein angiogram and optical coherence tomography were normal in all cases. Pattern electroretinogram showed reduced amplitudes of the P50 and N95 components. Multifocal electroretinogram indicated a selective damage to the central part of the macula. Conclusions: The results suggest that the abnormality arises predominantly in the central part of the macula, especially the outer and middle layers. However, the exact mechanism still remains obscure.

Abstract:Background: To evaluate retinal structural and functional changes after silicone oil (SO) removal in eyes with macula-off rhegmatogenous retinal detachment (RRD). Methods: Best-corrected visual acuity (BCVA) testing, microperimetry, and optical coherence tomography angiography were performed in 48 eyes with macula-off RRD before and 3 months after SO removal. The values of healthy contralateral eyes were used as control data. Correlations between retinal vessel density (VD), retinal nerve fiber layer thickness (RNFLT), the interval between retinal detachment and surgery, the duration of SO tamponade, the follow-up time after SO removal, and visual function were analyzed. Results: Significant increases in 2˚ fixation rate (FR), 4˚ FR, 2˚ mean retinal sensitivity (MRS), 6˚ MRS, parafoveal superficial capillary plexus VD and RNFLT were observed after SO removal (all P < 0.05). The increase of 2˚ MRS and 6˚ MRS were correlated with the duration of SO tamponade and the follow-up time after SO removal respectively (all P < 0.05). The last 2˚ MRS and 6˚ MRS were correlated with the duration of SO tamponade, the interval between retinal detachment and surgery, and the follow-up time after SO removal (all P < 0.01). The last FR in RRD eyes was close to that of contralateral eyes (P > 0.05). Conclusion: Retinal structure and function improved to different degrees after SO removal. Fixation stability and retinal sensitivity increased more than BCVA postoperatively. Retinal sensitivity, which was affected by the interval between retinal detachment and surgery and the duration of SO tamponade, gradually recovered after SO removal.

Abstract:A retrospective study of the effects of silicone oil removal was carried out on 85 patients who had undergone pars plana vitrectomy and silicone oil exchange for giant retinal tears or proliferative vitreoretinopathy. Silicone oil was removed either as part of the treatment of anterior segment complications such as glaucoma and keratopathy (25 patients) or in order to prevent these complications (60 patients). The major complications of the removal of silicone oil were retinal redetachment (25%), hypotony (16%), and expulsive haemorrhage (1%). The length of time that the oil remained in the eye and the presence of anterior segment complications did not appear to have an effect on the rate of retinal redetachment or hypotony.

Abstract:Background: Silicone oil (SO) is a long-term tamponade for repair of complex retinal pathology but has limitations including late redetachment. This study describes our experience with SO tamponade for repair of retinal detachment with proliferative vitreoretinopathy (PVR), with attention to anatomic and functional outcomes. Methods: Retrospective consecutive case series of eyes with retinal detachment (RD) complicated by proliferative vitreoretinopathy (PVR) receiving SO tamponade at the University of Wisconsin between 2013 and 2019. Group 1 defined as primary SO placement; Group 2 had SO placed after failing prior retinal detachment repair. Results: Inclusion criteria of SO placement for repair of RD with PVR was met for 117 eyes. The final reattachment rate was 84% for all eyes, with no difference between Groups 1 and 2. Vision improvement was 2.1 lines for Group 1 (p = 0.06 from baseline) and 4.6 lines for Group 2 (p < 0.0001). The mean number of silicone oil placements was 1.4. Less improvement in vision was noted with repeat SO placement, though overall functional vision of 5/200 or better was achieved in 63.2% of patients. Conclusions: SO tamponade allows long-term anatomical stabilisation and substantial vision recovery in eyes with retinal detachment complicated by PVR. Rates of anatomic and functional success have improved significantly when compared to prior studies using oil tamponade for repair of PVR.

Abstract:PURPOSE: We studied the results and complications of silicone oil removal applying basic criteria for the selection of eyes.METHODS: The criteria for oil removal: 1. Completely attached retina for at least 4 weeks, 2. Absence of tractions and active proliferations, 3. Preoperative vision > 0.01. The oil was removed from 90 of 261 oil filled eyes (removal rate: 34.5%). 83 eyes were included in the further study.FOLLOW-UP: 1.3-62.5 months (mean 15.7).RESULTS: Visual acuity improved (> or = 1 line) in 40 of the 83 eyes (48.2%), remained unchanged in 22 eyes (26.5%) and deteriorated (> or = 1 line) in 21 eyes (25.3%).COMPLICATIONS: reproliferations (42.2%), retinal detachment (20.5%), pressure rises (27.7%), lens opacification (36.7%), severe keratopathy (8.4%).CONCLUSION: Silicone oil removal has a distinct rate of complications, despite preoperative selection of eyes with a better prognosis. Reduction of reproliferations and weighing of risk and benefit could improve the results.
The feature and risk factors of recurrent retinal detachment in silicone oil-tamponaded eyes

Abstract:Objective: To evaluate the feature and risk factors of recurrent retinal detachment in silicone oil-tamponaded eyes. Methods: 52 eyes of 52 cases with recurrent retinal detachment followed vitrectomy and silicone oil tamponade were analyzed retrospectively, the risk factors were measured by Logistic regression analysis. Results: (1) The recurrent retinal detachment occurred in 10 days to 12 months (4.60 ± 2.86 months) post-operatively. (2) 59.6% eyes showed recurrent inferior retinal detachment, 15.4% was totally retinal detachment, 9.6% was temporary retinal detachment, 9.6% was local retinal detachment in peripheral area, only 3.8% was superior retinal detachment, 1.8% was macular detachment. (3) 33 eyes (63.5%) had retinal breaks,23 eyes (69.7%) with small holes peripherally, 2 eyes with horse-shoe tears peripherally, 4 eyes (12.1%) with breaks in posterior ratina, 2 eyes (6.1%) with macular hole, giant tears was seen in 2 eyes (6.1%). (4) The reformation of proliferation was found in 29 eyes (55.8%), C2 13eyes, C3 6 eyes, D1 7eyes, D3 3eyes, 4 eyes with anterior PVR. The proliferation located inferiorly in 20 eyes (69.0%), posterior in 6 eyes (20.7%), periphery in 5 eyes (17.2%). 9 eyes had subretinal proliferation. (5) 1 eye had CNV under the macular, 1 eye had cystoids macular edema, perfluorocarbon liquids entered subretinal in 1 eye, silicone oil entered subretinal in 4 eyes. (6) The risk factors for recurrent retinal detachment in silicone oil-tamponaded eyes were PVR grade (P = 0.023) and operative times (P = 0.030) before silicone oil was tamponaded. Conclusion: The recurrent retinal detachment in silicone oil-tamponaded eyes is probably the result probably of persistent retinal traction, non-closed primary breaks and reproliferation. The risk factors is PVR grade and operative times before silicone oil tamponaded.
Retinal vascular changes after Silicon Oil removal in the Eye with Rhegmatogenous Retinal detachment

Abstract:Background: This study aims to examine vessel density changes in the optic nerve and macula following silicone oil removal (SOR) surgery in eyes with rhegmatogenous retinal detachment (RRD) at different time points by Optical Coherence Tomography Angiography (OCTA) in compared to the contralateral eye. Methods: A total of 43 eyes from 43 patients with silicone oil in their eyes for 3-9 months underwent OCT-A using AngioVue and optic disc-associated vessel density (VD) and thickness, macular-associated VD and thickness, Foveal avascular zone (FAZ) area, FAZ perimeter (PERIM), Acircularity index (AI), vessel density within a 300 μm wide region of the FAZ were compared between eyes. OCTA scans were performed one week before SOR and one month and three months after SOR. Results: The mean age of participants was 52.8 years (SD = 15.85) and a median visual acuity was 0.8 (range: 0.5-1.0). Notably, male participants constituted 67.4% of the sample. The preoperative mean value BCVA (logMAR) of patients was 0.73, and 3 months post-oil removal was 0.7727. Regarding optic disc parameters, RNFL thickness and vessel density (VD) measurements Peripapillary, whole disc, inside disc, and Disc Angio (superior, Nasal, inferior, temporal) did not change. In analyzing macular thickness parameters, all of them (Whole and Fovea, parafoveal, and Perifovea) remained unchanged. Examining macular vessel density parameters revealed no significant changes across superficial and deep retinal layers. Finally, the comparison of the foveal avascular zone (FAZ) area and flow density (FD) parameters demonstrated consistent measurements with non-significant alterations observed in FAZ size (p = 0.6) and FD values (p = 0.49) over the monitored duration. Conclusion: There was no change in peripapillary VD and macular vessel density of the superficial capillary plexus (SCP) and deep capillary plexus (DCP) after silicone oil removal. FAZ and full retinal thickness remained stable 3 month after SOR. Clinical trial number: Not applicable.
Coil Protection Using Small Helical Coils for Wide-Neck Intracranial Aneurysms: A Novel Approach

Abstract:BACKGROUND AND PURPOSE: A number of remodeling or protective techniques available to treat wide-neck intracranial aneurysms are increasingly being used, provided that the shape/type of aneurysm, vessel diameter, and inherent course of the vessel are conducive to their use. The purpose of this study was to describe a novel method using coil protection for treatment of wide-neck aneurysms. MATERIALS AND METHODS: This technique involves sequential maneuvers to the aneurysm and affected branch artery. A microcatheter is first introduced into the aneurysmal sac, and another microcatheter is introduced into the entrance of the branch artery, followed by partial deployment of a small helical coil into the branch artery. A framing coil is then placed within the aneurysmal sac, under the protection of the helical coil. After completion of the first coil insertion, the helical coil should be retrieved to confirm the stability of the framing coil. The helical coil can also serve as a filler. RESULTS: This technique was successfully applied to 12 intracranial saccular aneurysms of the MCA bifurcation (5 patients); anterior communicating artery (3 patients); and A1 and M1 segments, distal ACA, and basilar tip (1 patient each). Selective endovascular treatment was successfully performed and resulted in excellent outcomes in all patients. There were no complications directly related to coil protection. CONCLUSIONS: Our small study suggests that coil protection can be a safe alternative to traditional remodeling or protective techniques when those techniques have failed or are not possible due to vascular geometry. It is particularly suited for the treatment of wide-neck aneurysms arising from small and acutely angulated branching vessels.

Abstract:Although it is widely accepted to use silicone oil in the treatment of very complex retinal detachments, there is no definite agreement on when and why silicone oil should be removed. Frequently found is a statement on the necessity of silicone oil removal after a certain period of time. However, the rate of retinal redetachment after silicone oil removal varies widely and appears to correlate with the underlying disease process and its severity. The literature on strategies of silicone oil removal, the rate of complications, and, thus, the risk-benefit ratio is scant. Therefore, for the discussion of silicone oil removal we rely on personal experience and the rather rare studies on silicone oil removal. Regarding the data given in the literature, the rate of vitreoretinal complications after silicone oil removal, even in cases with a clinically stable-appearing retinal situation, is rather high in severe proliferative vitreoretinopathy (PVR) and lower in most advanced cases of severe proliferative diabetic retinopathy (PDR) requiring silicone oil tamponade. The benefits of silicone oil removal are better in cases of cytomegalovirus retinitis or other situations without a PVR component. Silicone oil removal has to be considered a procedure of ill-defined risks, especially if silicone oil is really used as a last therapeutic resort in most severe cases of complicated retinal detachment. Further, exact criteria for the timing and safe removal of silicone oil in these complex vitreoretinal disorders still needs to be defined. A strategy for the removal of silicone oil is discussed.

A Clinical Study of Recurrent Retinal Detachment in Silicone Oil-Retained Eye

Abstract:Purpose To present the reasons of recurrent retinal detachment(RRD)in oil-retained eyes and evaluate the management of RRD.Methods Records of 54 patients(54 eyes,38 males,16 females)were retrospectively reviewed. Results All the eyes with RRD were examined.37 eyes(68.5%)had recurrent inferior retinal detachment.The inferior open retinal breaks were noticed in 22 of 35 eyes with breaks(62.9%).The reformation of inferior proliferation within vitreous cavity was seen in 53.7% of 41 eyes with proliferation.51 eyes were operated on again.Among them 38 eyes had vitreoretinal surgery after silicone oil was removed.The last follow-up data of completed retinal reattachment(CRR)was 35 eyes(35/38).7 oil-retained eyes underwent scleral buckling surgery.The data of CRR was 5 eyes.Conclusion The result indicated that the pathological and physical change in oil-retained eye was most often in the inferior retina.The retinal reattachment could be obtained in most eyes after subsequent surgery.

Visual Outcomes and Refractive Status after Combined Silicone Oil Removal/Cataract Surgery with Intraocular Lens Implantation

Abstract:Purpose: To evaluate refractive status and identify predictors of surgical success following a combined silicone oil removal/cataract surgery with intraocular lens (IOL) implantation procedure. Methods: In this single-armed, retrospective study, we reviewed patients who underwent vitreoretinal surgery followed by a combined silicone oil removal/cataract surgery procedure between 2009 and 2013. Preoperative data included patient demographics, refractive status, IOL power, and axial length (measured with the IOL Master). Postoperative data were obtained from the 8-week follow-up visit and from the last follow-up visit attended that included refractive error (RE) evaluation (e.g., myopic, hyperopic, and astigmatic). Associations between variables and refractive status were examined. Blindness was defined as a best-corrected visual acuity (BCVA) worse than 3/60. Results: Nighty-eight eyes were ultimately included in analyses. Following surgery, 37.0% of eyes achieved BCVA better than 6/18. The incidence of blindness (BCVA worse than 3/60) was reduced from 47.0% before surgery to 17.3% after surgery. Additionally, 33.7% of eyes did not require refractive correction. Forty-two percent of eyes were under-corrected (>0.5 D hyperopia) following surgery. Age, gender, silicone oil viscosity, axial length, IOL type, initial vitreoretinal pathology, surgeon, and IOL calculation formula were not significantly associated with surgical outcomes (all P > 0.05). Conclusion: A combined silicone oil removal/cataract surgery with IOL implantation procedure restored functional vision in approximately one-third of cases. However, nearly half of patients were under-corrected. Unfortunately, we did not identify any factors that predicted surgical success.
Longitudinal evaluation of retinal thickness and OCTA parameters before and following silicone oil removal in eyes with macula-on and macula-off retinal detachments

Abstract:BackgroundTo investigate the longitudinal retinal vascular and structural alterations by using OCTA following successful repair of retinal detachment (RD) by using silicone oil (SO) endotamponade.MethodsA prospective cohort study was performed on 24 eyes of 24 subjects; 17 of them with macula-off and seven with macula-on RD. At least four OCTA scans were generated for each study eye; two before and two after SO removal (SOR). At each session, retinal thickness (RT), vascular density (VD) in the superficial (SCP) and deep capillary plexuses (DCP) and foveal avascular zone(FAZ) size were measured within the fovea, parafovea and whole macular image. The changes in repeated measures of the same parameters especially before and after SOR were performed and compared within macula-on and macula-off groups and between fellow eyes.Results”In the macula-off eyes, RT measurements within the whole macula, fovea and parafovea reduced as compared with their fellow eyes(p < 0.0001, p = 0.001 and p < 0.0001, respectively) and did not improve after SOR. Additionally, all of the whole macular, foveal, and parafoveal VD measurements at SCP and foveal VD at DCP were less than their fellow eyes after SOR (p = 0.026, p = 0.023 p = 0.026, and p = 0.002, respectively). In macula-on eyes, RT measurements decreased within the parafovea and whole macular area before SOR (p = 0.018 and p = 0.011, respectively) but improved after SOR. Also, VD measurements did not change during follow-up and were not statistically different than their fellow eyes (p > 0.05). FAZ enlargement was found in eyes with macular-off RD after SOR, whereas it was not observed with macular-on RD (p = 0.038and p = 0.237, respectively).ConclusionsTreatment of macula-off RD with SO tamponade has been associated with vascular retinal abnormalities which did not improve following SOR. On the contrary, successful treatment of RD with SO tamponade in macula-on eyes VD measurements of SCP and DCP were not statistically different than their fellow eyes.
