Abstract:Two men sought treatment at an emergency room with apparent gonococcal proctitis. Further evaluation after failure of antibiotic therapy in both patients revealed that rectal damage resulting from the patients' sexual habits had resulted in a severe traumatic proctitis. The discovery of several organisms, including Neisseria gonorrhoeae and two nonpathogenic amebas, was probably only incidental. Two points are emphasized: the importance of obtaining a complete sexual history for every patient treated for venereal disease, and the inherent complications associated with anorectal manipulation.
Assessing Reliability of Naïve Respondent-driven Sampling Samples by Using Repeated Surveys Among People Who Inject Drugs (PWID) in New Jersey

Abstract:Introduction Respondent-driven sampling (RDS) is widely used to sample populations with higher risk of HIV infection for whom no sampling frames exist. However, few studies have been done to assess the reliability of RDS in real world settings. Methods We assessed the reliability of naïve RDS samples using five rounds of the National HIV Behavioral Surveillance – People Who Inject Drugs surveys in Newark, New Jersey from 2005 to 2018. Specifically, we compared the distributions of time-insensitive demographic characteristics in temporally adjacent RDS samples with Monte Carlo Two-Sample Kolmogorov-Smirnov Test with 100,000 replicates. The distributions of time-sensitive demographic characteristics were also compared as sensitivity analyses. Results The study showed that repeated RDS samples among people who inject drugs in the greater Newark area, New Jersey were reliable in most of time-insensitive demographics and recruitment homophily statistics. Sensitivity analyses of time-sensitive demographics also presented consistencies in most of temporally adjacent samples. Conclusions In conclusion, RDS has the potential to provide reliable samples, but demographic characteristics of RDS samples may be easily biased by homophily. Future studies using RDS may need to pay more attention to potential homophily bias and consider necessary diagnostic procedures and sample adjustments.

Hidden population size estimation from respondent-driven sampling: a network approach

Abstract:Estimating the size of stigmatized, hidden, or hard-to-reach populations is a major problem in epidemiology, demography, and public health research. Capture-recapture and multiplier methods have become standard tools for inference of hidden population sizes, but they require independent random sampling of target population members, which is rarely possible. Respondent-driven sampling (RDS) is a survey method for hidden populations that relies on social link tracing. The RDS recruitment process is designed to spread through the social network connecting members of the target population. In this paper, we show how to use network data revealed by RDS to estimate hidden population size. The key insight is that the recruitment chain, timing of recruitments, and network degrees of recruited subjects provide information about the number of individuals belonging to the target population who are not yet in the sample. We use a computationally efficient Bayesian method to integrate over the missing edges in the subgraph of recruited individuals. We validate the method using simulated data and apply the technique to estimate the number of people who inject drugs in St. Petersburg, Russia.

Abstract:Respondent-driven sampling (RDS) is a widely used method for generating chain-referral samples from hidden populations. It is an extension of the snowball sampling method and can, given that some assumptions are met, generate unbiased population estimates. One key assumption, not likely to be met, is that the acquaintance network in which the recruitment process takes place is undirected, meaning that all recruiters should have the potential to be recruited by the person they recruit. Using a mean-field approach, we develop an estimator which is based on prior information about the average indegrees of estimated variables. When the indegree is known, such as for RDS studies over internet social networks, the estimator can greatly reduce estimate error and bias as compared with current methods; when the indegree is not known, which is most common for interview-based RDS studies, the estimator can through sensitivity analysis be used as a tool to account for uncertainties of network directedness and error in self-reported degree data. The performance of the new estimator, together with previous RDS estimators, is investigated thoroughly by simulations on networks with varying structures. We have applied the new estimator on an empirical RDS study for injecting drug users in New York City.

Abstract:Learning about the social structure of hidden and hard-to-reach populations --- such as drug users and sex workers --- is a major goal of epidemiological and public health research on risk behaviors and disease prevention. Respondent-driven sampling (RDS) is a peer-referral process widely used by many health organizations, where research subjects recruit other subjects from their social network. In such surveys, researchers observe who recruited whom, along with the time of recruitment and the total number of acquaintances (network degree) of respondents. However, due to privacy concerns, the identities of acquaintances are not disclosed. In this work, we show how to reconstruct the underlying network structure through which the subjects are recruited. We formulate the dynamics of RDS as a continuous-time diffusion process over the underlying graph and derive the likelihood for the recruitment time series under an arbitrary recruitment time distribution. We develop an efficient stochastic optimization algorithm called RENDER (REspoNdent-Driven nEtwork Reconstruction) that finds the network that best explains the collected data. We support our analytical results through an exhaustive set of experiments on both synthetic and real data.

Abstract:Respondent-driven sampling (RDS) is a link-tracing procedure used in epidemiologic research on hidden or hard-to-reach populations in which subjects recruit others via their social networks. Estimates from RDS studies may have poor statistical properties due to statistical dependence in sampled subjects' traits. Two distinct mechanisms account for dependence in an RDS study: homophily, the tendency for individuals to share social ties with others exhibiting similar characteristics, and preferential recruitment, in which recruiters do not recruit uniformly at random from their network alters. The different effects of network homophily and preferential recruitment in RDS studies have been a source of confusion and controversy in methodological and empirical research in epidemiology. In this work, we gave formal definitions of homophily and preferential recruitment and showed that neither is identified in typical RDS studies. We derived nonparametric identification regions for homophily and preferential recruitment and showed that these parameters were not identified unless the network took a degenerate form. The results indicated that claims of homophily or recruitment bias measured from empirical RDS studies may not be credible. We applied our identification results to a study involving both a network census and RDS on a population of injection drug users in Hartford, Connecticut (2012-2013).

Abstract:Respondent-driven sampling (RDS) is a method of chain referral sampling popular for sampling hidden and/or marginalized populations. As such, even under the ideal sampling assumptions, the performance of RDS is restricted by the underlying social network: if the network is divided into communities that are weakly connected to each other, then RDS is likely to oversample one of these communities. In order to diminish the “referral bottlenecks” between communities, we propose anti-cluster RDS (AC-RDS), an adjustment to the standard RDS implementation. Using a standard model in the RDS literature, namely, a Markov process on the social network that is indexed by a tree, we construct and study the Markov transition matrix for AC-RDS. We show that if the underlying network is generated from the Stochastic Blockmodel with equal block sizes, then the transition matrix for AC-RDS has a larger spectral gap and consequently faster mixing properties than the standard random walk model for RDS. In addition, we show that AC-RDS reduces the covariance of the samples in the referral tree compared to the standard RDS and consequently leads to a smaller variance and design effect. We confirm the effectiveness of the new design using both the Add-Health networks and simulated networks.

Abstract:Problem-oriented policing strategies have been regarded as promising approaches for disrupting open-air drug markets in vulnerable communities. Pulling levers deterrence interventions, which are consistent with the problem-oriented framework, have shown potential as an effective mechanism for reducing and preventing youth, gun, and gang violence. This study examines the effect of a strategic, pulling levers intervention that was implemented by law enforcement officials in Rockford, Illinois, to address drug markets in a high crime neighborhood. The initiative builds on a similar effort developed in High Point, North Carolina, and represents an extension of pulling levers that was originally developed in Boston. The impact evaluation uses a mixed method of quantitative hierarchical growth curve models and qualitative interviews with residents. Study findings suggest that the Rockford strategy was associated with a statistically significant and substantive reduction in crime, drug, and nuisance offenses in the target neighborhood. Results from this examination have implications for both research and public policy.

Abstract:Respondent-driven sampling (RDS) is a chain-referral method for sampling members of a hidden or hard-to-reach population such as sex workers, homeless people, or drug users via their social network. Most methodological work on RDS has focused on inference of population means under the assumption that subjects' network degree determines their probability of being sampled. Criticism of existing estimators is usually focused on missing data: the underlying network is only partially observed, so it is difficult to determine correct sampling probabilities. In this paper, we show that data collected in ordinary RDS studies contain information about the structure of the respondents' social network. We construct a continuous-time model of RDS recruitment that incorporates the time series of recruitment events, the pattern of coupon use, and the network degrees of sampled subjects. Together, the observed data and the recruitment model place a well-defined probability distribution on the recruitment-induced subgraph of respondents. We show that this distribution can be interpreted as an exponential random graph model and develop a computationally efficient method for estimating the hidden graph. We validate the method using simulated data and apply the technique to an RDS study of injection drug users in St. Petersburg, Russia.

Effective sanction avoidance in drug trafficking: The construction and validation of the restrictive deterrence utility scale using a sample of female drug offenders

Abstract:Drug traffickers perform many crime strategies to avoid legal sanctions. The use of any crime strategy is mainly for reducing the risk of arrest, which has been the focus of research on restrictive deterrence. This study aims to develop a Restrictive Deterrence Utility Scale (RDUS) to measure the effectiveness of crime strategies used in drug trafficking. Survey data were collected from August to October 2021, including 294 female drug offenders aged between 21 and 61 years serving sentences with a median length of around 10 years in a Southern city in Mainland China. Exploratory and confirmatory factor analysis was conducted for scale investigation. Three valid and reliable factors for the RDUS were found: anomaly avoidance, reduction of exposure time, and the use of weapons and tools. Composite reliability, convergent validity, discriminant validity of three factors were confirmed with Cronbach α being higher than 0.9, that being higher than correspondent Average Variance Extracted which all being higher than 0.5 and higher than correspondent factor correlation. The RDUS offers a helpful method for assessing the motivation for drug traffickers to persist in drug crime and for formulating appropriate interventions for drug prohibition.
Effect of particle variation on wear rates of posterior composites.

Abstract:PURPOSE To evaluate the relationship between filler size, shape, mechanical properties and wear resistance of posterior resin composite was evaluated. MATERIALS AND METHODS Six UDMA/TEGDMA-based experimental resin composites consisting of different types of filler particles were investigated. These included four sized spherical fillers, (9.88 microns, 2.01 microns, 0.62 micron, 0.20 micron) and two irregular, (9.46 microns, 1.97 microns). Property measurements consisted of hardness and compressive strength. Wear resistance was evaluated by a three-bodied in vitro wear test. RESULTS The composites containing the smallest sized spherical particle exhibited maximum mechanical strength and maximum wear resistance. The composites containing the largest spherical particle exhibited minimum wear resistance. There were no significant differences amongst those containing similar sized fillers regardless of particle shape. However, differences between the 0.20 microns spherical filled composite and other composites were statistically significant. SEM analysis showed that only the larger sized filler exhibited some exfoliation after wear testing. The smaller size spherical filler exhibited better mechanical strength and substantially higher wear resistance.

Abstract:Abstract Background This study is part of a state-wide effort to promote the safe disposal of prescription medications and mitigate prescription drug misuse. The objective of this study was to evaluate the implementation of a two-component prevention intervention through Community Prevention Organizations (CPOs) in Texas. The first component involved the distribution of in-home disposal products (IHDP) and the second focused on providing education of the risks of prescription drug misuse. Methods This study followed a mixed methods sequential explanatory study design. In the quantitative phase, the extent to which CPOs carried out the intervention was determined by the distribution rate – a proportion representing the number of IHDP distributed to end users from the amount of IHDP the CPO was shipped. This measure was used to organize the CPOs in to one of three performance categories. In the qualitative arm of the study, stratified random sampling was used to select five CPOs from each performance strata to participate in an in-depth, semi-structured interview about their distribution activity. The interview guide and the data analysis were guided by Bowen’s Feasibility Framework. The interviews were transcribed and analyzed using a content analysis approach by two research team members. All qualitative analyses were conducted in ATLAS.ti© V7. Results There was a total of 47 CPOs contacted and asked to be part of this study. Of them, 44 CPOs participated in the quantitative phase of the study. This phase revealed that all CPOs had existing relationships with organizations throughout the community such as pharmacies and schools that could act as points of distribution. Following the quantitative phase, 15 CPOs were selected for more in-depth interviews about their distribution practices. In the qualitative phase, this finding was reinforced through the theme “partnerships with local institutions and ability to implement the intervention at community events”. Similarly, education promotion efforts were unanimously emphasized as a strategy to increase utilization of IHDP among end users. All CPOs indicated that the intervention was supplemental to their overall goals. Conclusion CPOs have unparalleled access to community events, local institutions, and the general population they serve, thus, they have the potential to be active facilitators in implementing prevention interventions.

Abstract:People who inject drugs are an important population to study in order to reduce transmission of blood-borne illnesses including HIV and Hepatitis. In this paper we estimate the HIV and Hepatitis C prevalence among people who inject drugs, as well as the proportion of people who inject drugs who are female in Mauritius. Respondent driven sampling (RDS), a widely adopted link-tracing sampling design used to collect samples from hard-to-reach human populations, was used to collect this sample. The random walk approximation underlying many common RDS estimators assumes that each social relation (edge) in the underlying social network has an equal probability of being traced in the collection of the sample. This assumption does not hold in practice. We show that certain RDS estimators are sensitive to the violation of this assumption. In order to address this limitation in current methodology, and the impact it may have on prevalence estimates, we present a new method for improving RDS prevalence estimators using estimated edge inclusion probabilities, and apply this to data from Mauritius.

Abstract:Background: Valid measurement of drug use in patients enrolled in clinical trials that treat substance use disorder is vital to determine the trial's outcome. Self-reports are often used but their validity has been studied with mixed results. Urinalysis may sometimes be employed as an alternative or supplement to self-reports. Objectives: This study examined how estimating drug use by either method would affect the results from a randomized clinical trial conducted in a methadone treatment program. At the initial Baseline interview and four follow-up interviews, participants were asked about their drug use history and provided a urine specimen for drug testing. Results: In most cases, the urinalyses detected more drugs than the patients had reported using. A major exception was heroin, whose use was an eligibility criterion for enrollment in the study and methadone treatment. Conclusions: The patients' self-reports would have led us to conclude that the use of heroin and fentanyl had declined from the initial Baseline interview to the final follow-up interview, while the urinalysis results indicated no change in exposure to heroin and an increase in exposure to fentanyl. Clinical trials would be well served to employ the use of biological tests in addition to self-reports to measure recent drug use and to accurately estimate the efficacy of the experimental protocols and patients' exposure to drugs.

Optimizing a community-engaged multi-level group intervention to reduce substance use: an application of the multiphase optimization strategy

Abstract:Background: Rates of alcohol and illicit drug use (AIDU) are consistently similar across racial groups (Windsor and Negi, J Addict Dis 28:258-68, 2009; Keyes et al. Soc Sci Med 124:132-41, 2015). Yet AIDU has significantly higher consequences for residents in distressed communities with concentrations of African Americans (DCAA - i.e., localities with high rates of poverty and crime) who also have considerably less access to effective treatment of substance use disorders (SUD). This project is optimizing Community Wise, an innovative multi-level behavioral-health intervention created in partnership with service providers and residents of distressed communities with histories of SUD and incarceration, to reduce health inequalities related to AIDU. Methods: Grounded in critical consciousness theory, community-based participatory research principles (CBPR), and the multiphase optimization strategy (MOST), this study employs a 2 × 2 × 2 × 2 factorial design to engineer the most efficient, effective, and scalable version of Community Wise that can be delivered for US$250 per person or less. This study is fully powered to detect change in AIDU in a sample of 528 men with a histories of SUD and incarceration, residing in Newark, NJ in the United States. A community collaborative board oversees recruitment using a variety of strategies including indigenous field worker sampling, facility-based sampling, community advertisement through fliers, and street outreach. Participants are randomly assigned to one of 16 conditions that include a combination of the following candidate intervention components: peer or licensed facilitator, group dialogue, personal goal development, and community organizing. All participants receive a core critical-thinking component. Data are collected at baseline plus five post-baseline monthly follow ups. Once the optimized Community Wise intervention is identified, it will be evaluated against an existing standard of care in a future randomized clinical trial. Discussion: This paper describes the protocol of the first ever study using CBPR and MOST to optimize a substance use intervention targeting a marginalized population. Data from this study will culminate in an optimized Community Wise manual; enhanced methodological strategies to develop multi-component scalable interventions using MOST and CBPR; and a better understanding of the application of critical consciousness theory to the field of health inequalities related to AIDU. Participatory Research With an Online Drug Forum: A Survey of User Characteristics, Information Sharing, and Harm Reduction Views

Abstract:Visitors to a popular online drug forum completed an online survey between November 2011 and January 2012, which covered (1) demographic characteristics, (2) substance use (including nonmedical prescription opioid use), (3) forum activity, and (4) harm reduction beliefs. The study sample (N = 897) primarily included Caucasian males in their twenties from the United States, the United Kingdom, Australia, and Canada. The practice of harm reduction was overwhelmingly endorsed by participants. Current nonmedical prescription opioid users reported more activity in forums and past substance abuse treatment. The study's implications and limitations are noted and future research is suggested.

Abstract:Assessment, Ahead of Print. Behavioral economic approaches to measuring cannabis demand represent a well-validated, low-cost method of assessing risk for hazardous cannabis use. One widely used measure of cannabis demand is the Marijuana Purchase Task (MPT), which has shown good psychometric properties across multiple samples. However, preliminary data suggest that changes could improve task ecological validity and acceptability. Using a predominantly White convenience sample, this study aimed to develop a revised MPT that uses a modern dispensary scenario to better reflect national trends in cannabis use (e.g., multiple forms of cannabis consumption). Participant inattentiveness due to increased task length and difficulty estimating purchases for the next month may have impacted demand measures. Lessons learned are discussed to inform future efforts to assess cannabis demand in a manner that is more reflective of naturalistic use: including minimizing participant burden, setting higher price ceilings to increase task utility, and considering person-level factors that may influence demand.

Abstract:Purpose of study: The aim of the study is to describe the prevalence of illicit drug use in England and Wales using data from the UK Survey of Non-Medical Use of Prescription Drugs (NMURx) programme and to compare against the well-established Crime Survey England and Wales (CSEW). The rationale is that recreational and illicit drug use is common, but the prevalence is difficult to estimate with personal interviewing methods. Study design: We compared two cross-sectional population surveys (NMURx, n=8903 and CSEW, n=20 685) with data regarding self-reported recreational drug use and demographics. NMURx is an online survey using non-probability sampling methodology with preset demographical quotas based on census data. CSEW surveys drug use via computer-assisted self-interviewing as part of a computer-assisted personal-interviewing crime survey. Results: Cannabis was the most frequently used drug regardless of demographics. Prevalence of drug use for specific substances was generally higher for males, younger ages and students. The relationship between income and drug misuse is less clear. Self-reported prevalence of drug use in the NMURx survey is consistently higher than CSEW (absolute difference 1%-3 % across substances and timescales) and persists after stratification for gender, age, student status and household income. Conclusions: The NMURx survey has a broad reach of participants, and a sampling scheme that achieves external validity, compared with general population demographics. NMURx's online format allows flexibility in items surveyed and in response to emerging trends. The self-reported drug use in the NMURx cohort is comparable, although slightly higher, than the CSEW estimates.
Impaction-Based Exhaled Breath Sampling for Substance Monitoring: A Prospective Pilot Study (DrugXhale).

Abstract:For decades, intake monitoring of drugs using urine as the matrix of choice is the gold standard in drug treatment centers. A properly conducted urine drug test can identify recent use of prescribed, non-prescribed and illicit drugs. However, issues like adulteration, substitution and privacy issues have driven the search for alternative matrices. This prospective pilot study evaluates the use of an impaction-based breath sampling device, Breath Explor®, as an alternative to traditional urine-based drug monitoring. Breath samples were analyzed using a validated 32-component liquid chromatography-tandem mass spectrometry (LC-MSMS) method. Recovery data represent the efficiency of extracting the analytes from the breath devices. Both automated and manual processing of the Breath Explor® devices showed mean recovery rates ranging from 39.5% to 55.4% for the 32 analytes. Despite the small number of subjects, breath analysis proved to be a convenient and easy-to-use methodology. An overall kappa-values of 0.5 indicated a moderate level of agreement with urine analysis, underscoring its potential as a complementary diagnostic tool. All participants tested positive in their breath sample for methadone (70% methadone and 100% EDDP), while a significant portion (90%) tested positive for 6-monoacetylmorphine (6-MAM). This innovative approach offers several advantages, including non-invasiveness, reduced risk of adulteration, and the ability to perfom repeated automated sampling and confirmation testing. These findings suggest that breath-based substance monitoring could complement or even replace traditional urine-based methods in clinical practice.
