Abstract:Two clinical ablation protocols, 2C3L and stepwise, have been routinely used in our group to treat atrial fibrillation (AF), but with a less than 60% long-term arrhythmia-free outcome achieved in patients. The goal of this study was to examine the underlying mechanism of low success in clinical outcome. MRI images from one patient were used to reconstruct a human atrial anatomical model, and fibrotic tissue was manually added to represent the arrhythmia substrate. AF was induced with standard protocols used in clinical practice. 2C3L and stepwise were then used to test the efficacy of arrhythmia termination in our model. The results showed that re-entries induced in our model could not be terminated by using either 2C3L or the stepwise protocol. Although some of the induced re-entries were terminated, others emerged in new areas. Ablation using only the 2C3L or stepwise method was not sufficient to terminate all re-entries in our model, which may partially explain the poor long-term arrhythmiafree outcomes in clinical practice. Our findings also suggest that computational heart modelling is an important tool to assist in the establishment of optimal ablation strategies.
Catheter ablation for persistent atrial fibrillation with left ventricular systolic dysfunction: Who is the best candidate?

Abstract:Background Tachycardia-induced cardiomyopathy is poorly recognized pre-ablation. It remains unclear of better patient selection and timing for catheter ablation in persistent atrial fibrillation (PerAF) with heart failure (HF). Methods Consecutive patients with PerAF and left ventricular ejection fraction (LVEF) <50% referred for AF ablation were retrospectively included. The impact of LV size, heart rate (HR), and LVEF pre-ablation were analyzed for assessing LV systolic function recovery, defined as LVEF increase of >= 20% or to a value >= 55% after ablation. Results A total of 120 patients (2017-2020) were included. After 19 +/- 14 months post ablation, LVEF improvement was similar in patients with normal or dilated LV (18.3 +/- 9.4% vs. 16.1 +/- 10.8%, P = .25), rapid or controlled HR (19.5 +/- 10% vs. 16.1 +/- 10%, P = .09), but higher in HFrEF (HF with reduced EF) than HFmrEF (HF with midrange EF) (21.6 +/- 10.3% vs. 14.9 +/- 9.3%, P < .01). There was more LV systolic function recovery in those with normal to moderate LV dilation (80%, odds ratio [OR] 15.22, P < .01), HR >= 80 bpm (79%, OR 5.38, P < .01) and HFmrEF (80%, OR 4.03, P < .01). The overall AF freedom was similar between normal and dilated LV (59% vs. 62%, P = .95), rapid and controlled HR (67% vs. 56%, P = .18), and HFmrEF and HFrEF (65% vs. 50%, P = .19). Conclusion Catheter ablation is effective independent of LV dilation, rate control or HFrEF. Patients with normal to moderate LV dilation, resting HR >= 80 bpm and HFmrEF may be candidates for early PerAF ablation to achieve LVEF normalization.

Abstract:Recent advances in techniques, technologies and proven superiority over anti-arrhythmic drugs have made catheter ablation the cornerstone of management for atrial fibrillation (AF), which has shown a steady increase in prevalence in the ageing population worldwide. The aim of therapeutic interventions is to achieve stable sinus rhythm that would improve the quality of life and reduce the risk of AF-associated complications. Pulmonary veins (PVs) were first described as the source of initiation of ectopic triggers driving AF, which led to the establishment of PV isolation (PVI) as the most widely practiced procedure to treat AF. Antral PVI is still recognized as the stand-alone ablation strategy for newly diagnosed paroxysmal AF (PAF). However, in non-PAF patients, PVI seems to be inadequate and several adjunctive strategies, including ablation of left atrial posterior wall and non-PV triggers, AF mapping and ablation of rotors and drivers, ethanol infusion of vein of Marshall and renal denervation, etc. have been reported with mixed results. Recent trials have also documented the benefits of early rhythm control in preventing cardiovascular events in addition to slowing the progression of PAF to more persistent forms. Similarly, very late relapse of the arrhythmia after successful PVI has drawn attention to the critical role of non-PV triggers and highlighted their relevance as potential ablation targets during repeat procedures. Ablation technology is also under constant evolution with the introduction of non-thermal energy sources and new tools to create durable lesions. This review summarizes the indications, advancements, and future perspective of AF ablation. Summarizes the different facets of atrial fibrillation (AF) ablation including history, indications, current ablation strategies, quality of life, cognition and progression of AF, and the future perspectives.

Abstract:The ablation of persistent atrial fibrillation (AF) poses many challenges compared with ablation for paroxysmal AF, including greater procedural complexity, longer procedural time, unclear endpoints, increased patient comorbidity with a greater risk of procedural complications, and lower success rate. Nevertheless, using a combination of patient selection, careful procedural planning of both ablation strategy and endpoints, and by setting realistic patient expectations, successful ablation may be achieved. Further improvements will come from continued technical advances as well as from greater mechanistic understanding of persistent AF, including the physiologically-targeted ablation of localized rotors and focal sources that have recently been shown to maintain human AF.

Abstract:Manufacturing companies of today are under pressure to run their production most efficiently in order to sustain their competitiveness. Manufacturing systems usually have bottlenecks that impede their performance, and finding the causes of these constraints, or even identifying their locations, is not a straightforward task. SCORE (Simulation-based COnstraint REmoval) is a promising method for detecting and ranking bottlenecks of production systems, that utilizes simulation-based multi-objective optimization (SMO). However, formulating a real-world, large-scale industrial bottleneck analysis problem into a SMO problem using the SCORE-method manually include tedious and error-prone tasks that may prohibit manufacturing companies to benefit from it. This paper presents how the greater part of the manual tasks can be automated by introducing a new, generic way of defining improvements of production systems and illustrates how the simplified application of SCORE can assist manufacturing companies in identifying their production constraints.

Abstract:Atrial fibrillation (AF) is the most common arrhythmia in the adult population and catheter ablation has emerged as an important rhythm-control strategy and is the most common cardiac ablation procedure performed worldwide. The antiarrhythmic drugs have demonstrated moderate efficacy in long-term maintenance of sinus rhythm; moreover, they are often not tolerated and are associated with adverse events. Catheter ablation has proven to be effective in treating AF, although long-term outcomes have been significantly less favorable in persistent AF than in paroxysmal. The current guidelines recommend catheter ablation as class I indication for patients whom antiarrhythmic drugs have failed or are not tolerated, and as first-line rhythm-control therapy in selected patients with symptomatic AF. Advances in technology and innovative ablation protocols resulted in a remarkable improvement of the efficacy outcomes after pulmonary vein isolation. This review seeks to provide an updated report of the current practices and approaches, and to describe the latest advances in technology that aim to improve procedural safety, efficacy and to reduce procedural requirements in terms of duration and fluoroscopy exposure.

Abstract:PURPOSE: Catheter ablation of atrial fibrillation (AF) is now one of the most frequently performed ablation procedures, but there are currently 2 important challenges: achieving permanent/durable rather than transient pulmonary vein isolation (PVI) and improving the results of ablation for the wider patient population with persistent AF.METHODS: Recent technical advances in the technique of ablation and the results of clinical trials aimed at achieving more permanent and durable PVI are reviewed. We also summarize recent advances in identifying atrial fibrosis and in understanding the pathophysiology of AF relevant to selecting patients for ablation of persistent AF.FINDINGS: The use of contact force-sensing technology, adenosine testing after ablation, and pace capture-guided ablation all have the potential for achieving more durable ablation. Selection of patients suitable for ablation of persistent AF may be improved by assessing the extent of atrial fibrosis with delayed enhancement imaging with cardiac magnetic resonance or by assessing the pattern of atrial electrical activity with the use of complex atrial electrograms. Advances in treatment are likely to result from the recognition of localized rotors and focal sources as primary sustaining mechanisms for all types of human AF and in the use of noninvasive mapping for their identification. Linear ablation to supplement PVI may improve the results of AF ablation.IMPLICATIONS: Rapidly unfolding advances in the techniques of AF ablation and the understanding of mechanisms of AF hold promise for improving the durability of PVI and for extending the technique to carefully selected patients with persistent AF.

CLINICAL EFFECT ANALYSIS OF ATRIAL FIBRILLATION AFTER CATHETER ABLATION

Abstract:Atrial fibrillation (AF) is one of the most common cardiac arrhythmias, which is harmful to human health and quality of life. AADs are limited in clinical because of their side effects, so in recent years, RFCA has become one of the effective treatments for AF, whose clinical application is showing a clear upward trend. In order to explore the efficacy and safety of catheter ablation for AF, we have followed up the patients with AF undergoing catheter ablation in this study. A cohort of 39 patients with AF after RFCA from 2008 to 2011 was studied, 25 male and 14 female; the average age was 61.08±10.17 (36–74) years old. Among them, there were 32 cases with paroxysmal AF, and seven cases with persistent AF. 19 cases were complicated with hypertension, six cases with coronary heart disease, seven cases with diabetes and two cases with cerebral infarction. All the patients suffered from obvious clinical discomfortable symptoms. Although treated by one to two kinds of AADs already, the effects were poor. CPVI was performed in the all of them (five cases recurred, 13.51%). The endpoint of the ablation was complete electronic isolation of all the pulmonary veins (PVs). The patients who were converted to sinus rhythm in the cause of ablation were validated by Lasso electrodes, and could not be evoked atrial arrhythmia again. However, those who were failed to be converted to sinus rhythm would be validated after electroversion. For those who weren't isolated PVs completely, the conducted gap was initially posited by the mapping catheter, and the earliest fragmented potential was targeted for ablation till the complete electronic isolation of all the PVs. (1) The results of catheter ablation: 37 cases were successfully achieved immediately the endpoint after the ablation. Surgery average time of operation was (174.89±35.05) min, average X-ray exposure time (33.62±16.44) min, average discharge time (40.78±11.61) min; No severe complications such as pulmonary vein stenosis, pericardial tamponade, cardio-esophagus fistula, stroke occurred during and after the procedure. One case with paroxysmal AF was converted to auricular flutter after ablation, who were failed to be converted to sinus rhythm by synchronised cardioversion, but converted after Marshall Ligament ablation; three cases with persistent AF undergoing ablation were turned to be sinus rhythm by cardioversion; one case with persistent AF undergoing ablation was converted to sinus rhythm by intravenous irapbullitt. Five cases developed atrial tachyarrhythmias in 5–7 days after ablation: four cases appeared paroxysmal AF with fast ventricular rate; one case appeared paroxysmal atrial tachycardia; another one was sinus bradycardia, junctional escape. All the patients didn't suffer dysponea, cough, haemoptysis and so on before discharge, moreover distention of jugular vein, pulsus paradoxus and pericardial rub were not found through physical examination. No severe complication happened during and after the procedure, and all of them maintained sinus rhythm before discharge. (2) Follow-up: 37 cases were followed-up after 6–12 months, average 6±2 months, only one case lost communication and one case died of congestive heart failure. Five cases recurred: one case with paroxysmal AF who recurred after simple CPVI refused re-ablation, decreased episode frequency and took amiodarone to maintain sinus rhythm. One patient who performed CPVI and atria sinistrum isthmus ablation, three cases with paroxysmal AF appeared atrial arrhythmia after RFCA, but they refused re-ablation temporarily because of decreasing episode frequency. 36 cases took warfarin according to monitoring INR periodically, while one case took warfarin only 2 months after ablation because of hemorrhinia. Dysponea, cough related to pulmonary vein stenosis, and thromboembolism complications didn't occur for all of the cases. The LAD of 29 cases recovered normal size (LAD≤35 mm) on cardiac ultrasound 3 months after ablation. The lower the preoperative LVEF is, the more obvious the improvement is after RFCA. Catheter ablation is safe and effective for treating AF.

Abstract:Atrial fibrillation (AF) is a chronic heart rhythm disorder, characterized by exacerbations interspersed with clinical remissions. Antiarrhythmic drugs (AADs) are traditionally considered to be the preferred initial therapy for the maintenance of sinus rhythm however, these medications have modest efficacy and significant adverse effects. Recent clinical trials have evaluated the role of catheter ablation as the initial therapeutic intervention, demonstrating that cryoballoon ablation significantly reduces atrial tachyarrhythmia recurrence and arrhythmia burden, produces clinically meaningful improvements in symptoms and quality of life, and significantly decreases healthcare resource utilization. In contrast to AADs, catheter ablation appears to be a disease modifying therapy, significantly reducing the progression to more advanced forms of AF. These findings are relevant to patients, providers, and healthcare systems, helping inform the decision regarding the initial choice of rhythm-control therapy in patients with treatment-naïve AF.

Abstract:Atrial fibrillation (AF) is the most common arrhythmia in patients affected by cardiomyopathies. Reports estimate a prevalence of 27% in patients with hypertrophic cardiomyopathy (HCM) and 40% in patients with cardiac amyloidosis (CA). The presence of AF typically results in progressive functional decline, an increased frequency of hospitalizations for heart failure, and a higher thromboembolic risk. Medical management using mainly beta-blockers or amiodarone has produced variable outcomes and a high rate of recurrence. Catheter ablation reduces symptom burden and complications despite a moderate rate of recurrence. Recent evidence suggests that an early rhythm control strategy may lead to more favorable short- and long-term outcomes. In this review, we summarize contemporary data on the management of AF in patients with cardiomyopathy (HCM and CA) with particular reference to the timing and outcomes of ablation procedures.

Abstract:Objectives Atrial fibrillation (AF) is the most common cardiac arrhythmia. Due to the insufficient efficacy of antiarrhythmic drugs and their adverse side effects, there has been considerable interest in the interventional treatment of AF, including both catheter ablation and surgical ablation. Surgical ablation or "maze procedure" is a treatment option for patients with AF undergoing concomitant or isolated cardiac surgery. Design We performed a retrospective study of prospectively collected data. This study aimed to investigate short- and long-term outcomes of patients who underwent the surgical ablation of AF. Outcomes variables include freedom from recurrent atrial arrhythmias and mortality at 1- 3- 5- and 7-year follow-up. We also identify risk factors for arrhythmia recurrences and mortality. Setting Israel's largest university tertiary care center. Participants The study population included 668 patients, operated between 01.01.2006 and 30.06.2022. All data of patients were extracted from our departmental database. Interventions Concomitant or stand-alone surgical AF ablation. Measurements and Main Results The mean follow-up duration was 106 ± 66.7 months. Freedom from atrial fibrillation was 97.6% (615 patients). Mortality was 3% (20 patients) after a 1-year follow-up, 95.3% (574 patients), and 6.1% (45 patients) after a 3-year follow-up, 90.1% (396 patients) and 9.1% (61 patients) after a 5-year follow-up, and 77.5% (308 patients) and 10.8% (72 patients) after 7-year follow-up respectively. According to logistic regression analysis, age and female gender determined the 7-year recurrence of AF. The risk factors for 7-year mortality were diabetes mellitus, age and valvular surgery Conclusion Surgical ablation had a high success rate, with freedom from recurrent atrial arrhythmia at 1- 3- 5- and 7-years follow-up. Age and female gender were factors that determined the 5- and 7-year recurrence of AF.

Abstract:Atrial fibrillation (AF), the most common sustained arrhythmia, is associated with a range of symptoms, including palpitations, cognitive impairment, systemic embolism, and increased mortality. It places a significant burden on healthcare systems worldwide. Despite decades of research, the precise mechanisms underlying AF remain elusive. Current understanding suggests that factors like stretch-induced fibrosis, epicardial adipose tissue (EAT), chronic inflammation, autonomic nervous system (ANS) imbalances, and genetic mutations all play significant roles in its development. In recent years, the advent of wearable devices has revolutionized AF diagnosis, enabling timely detection and monitoring. However, balancing early diagnosis with efficient resource utilization presents new challenges for healthcare providers. AF management primarily focuses on stroke prevention and symptom alleviation. Patients at high risk of thromboembolism require anticoagulation therapy, and emerging pipeline drugs, particularly factor XI inhibitors, hold promise for achieving effective anticoagulation with reduced bleeding risks. The scope of indications for catheter ablation in AF has expanded significantly. Pulsed field ablation, as a novel energy source, shows potential for improving success rates while ensuring safety. This review integrates existing knowledge and ongoing research on AF pathophysiology and clinical management, with emphasis on diagnostic devices, next-generation anticoagulants, drugs targeting underlying mechanisms, and interventional therapies. It offers a comprehensive mosaic of AF, providing insights into its complexities.
Preferential Formation of MT1/MT2 Melatonin Receptor Heterodimers with Distinct Ligand Interaction Properties Compared with MT2 Homodimers

Abstract:Heterodimerization has been documented for several members of the G protein-coupled receptor (GPCR) superfamily, including the closely related MT1 and MT2 melatonin receptors. However, the relative abundance of hetero-versus homodimers and the specific properties, which can be attributed to each form, are difficult to determine. Using a bioluminescence resonance energy transfer (BRET) donor saturation assay, we show that half-maximal MT1/MT2 heterodimer formation is reached for expression levels as low as ∼4000 receptors per cell. The relative propensity of MT1 homodimer and MT1/MT2 heterodimer formation are similar, whereas that for the MT2 homodimer formation is 3- to 4-fold lower. These data indicate that both the relative expression level of each receptor isoform and the affinities between monomers may determine the actual proportion of homo- and heterodimers. The specific interaction of ligands with the MT1/MT2 heterodimer was studied using a BRET-based assay as a readout for the conformational changes of the heterodimer. An MT1/MT2 heterodimer-specific profile and ligands selective for the MT1/MT2 heterodimer compared with the MT2 homodimer could be identified. Classic radioligand binding and BRET studies suggest that heterodimers contain two functional ligand binding sites that maintain their respective selectivity for MT1 and MT2 ligands. Occupation of either binding site is sufficient to induce a conformational change within the heterodimer. Taken together, these results show that the probability of GPCR heterodimer formation may be equal to or even higher than that of the corresponding homodimers and that specific properties of heterodimers can be revealed using a BRET-based ligand/receptor interaction assay.

Abstract:Atrial fibrillation (AF), the most prevalent cardiac arrhythmia has witnessed significant advancements in treatment modalities, transitioning from invasive procedures like the maze procedure to minimally invasive catheter ablation techniques. This review focuses on recent improvements in anesthetic approaches that enhance outcomes in catheter AF ablation. We highlight the efficacy of contact force sensing catheters with steerable introducer sheaths, which outperform traditional catheters by ensuring more effective contact time and lesion formation. Comparing general anesthesia with conscious sedation, we find that general anesthesia provides superior catheter stability due to reduced respiratory variability, resulting in more effective lesion formation, and reduced pulmonary vein reconnection. The use of High Frequency Jet Ventilation (HFJV) under general anesthesia, delivering low tidal volumes, effectively minimizes left atrial movement, decreasing catheter displacement and procedure time, and reducing recurrence in paroxysmal AF. An alternative, High Frequency Low Tidal Volume Ventilation (HFLTV) using conventional ventilators, also shows improved catheter stability and lesion durability compared to traditional ventilation methods. However, a detailed comparative study of HFJV, HFLTV, and Conventional Mechanical Ventilation (CMV) in catheter ablation for AF is lacking. This review emphasizes the need for such studies to identify optimal anesthetic techniques, potentially enhancing patient outcomes in AF treatment. Our findings suggest that careful selection of anesthetic methods, including ventilation strategies, plays a crucial role in the success of catheter ablation for AF, warranting further research for evidence-based practice.

Abstract:Glucosidase II has been purified from crude pig liver microsomes by a convenient procedure involving DEAE-Sephacel, Con A-Sepharose and affinity chromatography on N-5-carboxypentyl-1-deoxynojirimycin-AH-Sepharose. Specific binding of glucosidase II to the affinity matrix required its prior separation from glucosidase I, which was accomplished by fractional Con A-Sepharose chromatography. The three-step procedure yielded, with approximately 15% enzyme recovery, a > 190-fold enriched glucosidase II, consisting of two proteins (107 kDa and 112 kDa). Both polypeptides are N-glycosylated with probably one glycan chain, in line with their binding to Con A-Sepharose. Immunological cross-reactivity and other experimental data indicate that the 107 kDa N-glycoprotein is derived from the 112 kDa species by partial proteolysis. The occasional presence of a 60 kDa peptide co-eluting with the catalytic activity suggests that glucosidase II may be associated with other protein subunit(s) in a heteromeric membrane complex. Glucosidase II hydrolyzes the alpha1,3-glucosidic linkages in Glc(2-1)-Man9-GlcNAc2, as well as synthetic alpha-glucosides, efficiently but does not remove the distal alpha1,2-linked glucose in Glc3-Man9-GlcNAc2. The enzyme has a pH optimum close to 6.5 and is not metal ion-dependent. Catalytic activity is strongly inhibited by basic sugar analogues including 1-deoxynojirimycin (dNM; app. Ki approximately 7.0 microM), N-5-carboxypentyl-dNM (app. Ki approximately 32 microM) and castanospermine (app. Ki approximately 40 microM). Substitution of the 3-OH or 6-OH group in dNM by a fluoro group reduces the inhibitory potential drastically. We conclude from these observations that the two hydroxy groups are essential for inhibitor/substrate binding due to their ability to interfere as hydrogen bond donors. A polyclonal antibody raised against the 107 kDa polypeptide reacted specifically with two proteins from different cell types on Western blots. Their molecular masses were identical with those from pig liver microsomes, pointing to a highly conserved amino acid sequence of glucosidase II. This suggests that the variance in molecular mass for glucosidase II reported for the enzyme from other tissues and species may be due to partial proteolysis.
Pulsed Field Ablation to Treat Atrial Fibrillation: A Review of the Literature

Abstract:Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia and catheter ablation, which can be used in symptomatic patients refractory to antiarrhythmic therapy. Pulmonary vein isolation (PVI) remains the cornerstone of any ablation procedure. A major limitation of current catheter ablation procedures is important to recognize because even when the PVI is performed in highly experienced centers, PVI reconnection was documented in about 20% of patients. Therefore, better technology is needed to improve ablation lesions. One of the novelties in recent years is pulsed filed ablation (PFA), a non-thermal energy that uses trains of high-voltage, very-short-duration pulses to kill the cells. The mechanism of action of this energy consists of creating pores in the myocardiocyte cell membrane in a highly selective and tissue-specific way; this leads to death of the target cells reducing the risk of damage to surrounding non-cardiac tissues. In particular during the animal studies, PVI and atrial lines were performed effectively without PV stenosis. Using PFA directly on coronary arteries, there was no luminal narrowing, there has been no evidence of incidental phrenic nerve injury, and finally, PFA has been shown not to injure esophageal tissue when directly applied to the esophagus or indirectly through ablation in the left atrium. The aim of this review is to report all published animal and clinical studies regarding this new technology to treat paroxysmal and persistent AF.

Atrial Fibrillation Ablation in Heart Failure With Reduced vs Preserved Ejection Fraction

Abstract:Importance Catheter ablation is associated with reduced heart failure (HF) hospitalization and death in select patients with atrial fibrillation (AF) and heart failure with reduced ejection fraction (HFrEF). However, the benefit in patients with HF with preserved ejection fraction (HFpEF) is uncertain. Objective To investigate whether catheter ablation for AF is associated with reduced HF-related outcomes according to HF phenotype. Data Source A systematic search of MEDLINE, Embase, and Cochrane Central was conducted among studies published from inception to September 2023. Study Selection Parallel-group randomized clinical trials (RCTs) comparing catheter ablation with conventional rate or rhythm control therapies in patients with HF, New York Heart Association functional class II or greater, and a history of paroxysmal or persistent AF were included. Pairs of independent reviewers screened 7531 titles and abstracts, of which 12 RCTs and 4 substudies met selection criteria. Data Extraction and Synthesis Data were abstracted in duplicate according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses ( PRISMA ) reporting guideline. Pooled effect estimates were calculated using random-effects Mantel-Haenszel models. Interaction P values were used to test for subgroup differences. Main Outcomes and Measures The primary outcome was HF events, defined as HF hospitalization, clinically significant worsening of HF, or unscheduled visits to a clinician for treatment intensification. Secondary outcomes included cardiovascular and all-cause mortality. Results A total of 12 RCTs with 2465 participants (mean [SD] age, 65.3 [9.7] years; 658 females [26.7%]) were included; there were 1552 participants with HFrEF and 913 participants with HFpEF. Compared with conventional rate or rhythm control, catheter ablation was associated with reduced risk of HF events in HFrEF (risk ratio [RR], 0.59; 95% CI, 0.48-0.72), while there was no benefit in patients with HFpEF (RR, 0.93; 95% CI, 0.65-1.32) ( P for interaction = .03). Catheter ablation was associated with reduced risk of cardiovascular death compared with conventional therapies in HFrEF (RR, 0.49; 95% CI, 0.34-0.70) but a differential association was not detected in HFpEF (RR, 0.91; 95% CI, 0.46-1.79) ( P for interaction = .12). Similarly, no difference in the association of catheter ablation with all-cause mortality was found between HFrEF (RR vs conventional therapies, 0.63; 95% CI, 0.47-0.86) and HFpEF (RR vs conventional therapies, 0.95; 95% CI, 0.39-2.30) groups ( P for interaction = .39). Conclusions and Relevance This study found that catheter ablation for AF was associated with reduced risk of HF events in patients with HFrEF but had limited or no benefit in HFpEF. Results from ongoing trials may further elucidate the role of catheter ablation for AF in HFpEF.

Invasive strategies for rhythm control of atrial fibrillation: a narrative review

Abstract:Atrial fibrillation (AF) is the most common sustained tachyarrhythmia and its increasing prevalence has resulted in a growing healthcare burden. A recent landmark randomized trial, the EAST-AFNET 4 (Early Treatment of Atrial Fibrillation for Stroke Prevention Trial), highlighted the importance of early rhythm control in AF, which was previously underemphasized. Rhythm control therapy includes antiarrhythmic drugs, direct-current cardioversion, and catheter ablation. Currently, catheter ablation is indicated for patients with AF who are either refractory or intolerant to antiarrhythmic drugs or who exhibit decreased left ventricular systolic function. Catheter ablation can be categorized according to the energy source used, including radiofrequency ablation (RFA), cryoablation, laser ablation, and the recently emerging pulsed field ablation (PFA). Catheter ablation techniques can also be divided into the point-by-point ablation method, which ablates the pulmonary vein (PV) antrum one point at a time, and the single-shot technique, which uses a spherical catheter to ablate the PV antrum in a single application. PFA is known to be applicable to both point-by-point and single-shot techniques and is expected to be promising owing to its tissue specificity, resulting in less collateral damage than catheter ablation involving thermal energy, such as RFA and cryoablation. In this review, we aimed to outline catheter ablation for rhythm control in AF by reviewing previous studies.

Abstract:Atrial fibrillation is the commonest clinical arrhythmia, is increasing in incidence and prevalence, and is associated with substantial morbidity and mortality. The arrhythmia may be paroxysmal (self-limiting), persistent (amenable to cardioversion), or permanent. Especially in its paroxysmal form, atrial fibrillation may be initiated by rapidly firing foci, generally located in the proximal pulmonary veins. Sustained atrial fibrillation is maintained by an atrial tissue substrate capable of accommodating many meandering wavelets. With continuing arrhythmia, the electrophysiological properties of the atria change and further facilitate continuing fibrillation. Treatment is aimed at prevention of thromboembolic complications, restoration and maintenance of sinus rhythm, and control of ventricular rate during atrial fibrillation. With greater understanding of the arrhythmia mechanisms, it is becoming possible to offer targeted curative treatments to more and more patients.

Abstract:Establishing a successful and sustaining atrial fibrillation (AF) program should be current standard of care in cardiac surgery by providing not only sinus rhythm restoration but improving survival significantly. Recognition of AF as a serious cardiac disease impacting a considerable number of patients in terms of stroke, heart failure development and mortality is key when implementing a protocol to identify patients suitable for surgical ablation and a treatment standard according to the underlying pathology. The ablation strategy resulting in the best potential rhythm outcome should be chosen, and the applied access should follow this decision in a way that the optimal lesion set can be executed completely. A thorough follow-up is required to confirm results or make necessary adjustments to the implemented treatment protocol. This should be done in close collaboration with other treatment specialists such as cardiologists, electrophysiologists, primary care physicians, or neurologists. This collaboration might also facilitate an expansion of the program towards stand-alone ablation, ventricular tachycardia ablation, and inappropriate sinus tachycardia ablation.
