Abstract:Japan's rapidly aging and high-mortality society necessitates a wider awareness and implementation of advance care planning. This Yamagata Cohort study investigated local residents' preferences for where they would like to spend their final days, and the underlying factors associated with those preferences with a self-administered questionnaire survey of local residents aged 40 years and over . Logistic regression analyses were used to assess those factors and, specifically, the choice of "Home" as the preferred place for end-of-life residence. Among the 10,119 responders, 61% chose their home as the most desirable place to spend their final days. The multiple logistic regression analysis showed that the independent factors associated with the choice of "Home" were: male, older age, not living with someone who needs care, not discussing the end of life, currently happy, struggling to live on current income, not feeling anxious or depressed, and current place of residence the same as their grandparents' birthplace. This suggested that reducing the burden of home care and addressing frequent emotional issues such as happiness and anxiety could increase the number of people choosing "Home". Open-ended comments indicated the importance of getting information and options, and discussing the choice of place for terminal care in light of individual backgrounds including having reservations about family. Support and systems are needed to understand what community residents consider important when deciding where to spend their final days, and to bridge the gap between their desired location and their actual end of life.
The Impact of Continuous Use of Home Health Care Resources on End-of-Life Care at Home in Older Patients with Cancer: A Retrospective Cohort Study in Fukuoka, Japan

Abstract:This study aimed at examining the effect of continued use of home health care resources on end-of-life care at home in older patients with cancer. This retrospective cohort study was conducted using medical and long-term care claims data of 6435 older patients with cancer who died between April 2016 and March 2019 in Fukuoka Prefecture. The main explanatory variables were enhanced home care support clinics and hospitals (HCSCs), enhanced HCSCs with beds, conventional HCSCs, other HCSCs, and home visit nursing care. The covariates were sex, age, required level of care, and the Charlson Comorbidity Index. A logistic regression model was used. The results of the multilevel logistic regression analysis showed that the following were significantly associated with end-of-life care at home: use of enhanced HCSCs with beds (odds ratio, OR: 8.66; 95% confidence interval, CI: [4.31-17.40]), conventional HCSCs (OR: 5.78; 95% CI: [1.86-17.94]), enhanced HCSCs (OR: 4.44; 95% CI: [1.47-13.42]), home-visit nursing care (OR: 1.86; 95% CI: [1.42-2.44]), and a severe need for care (OR: 3.89; 95% CI: [2.92-5.18]). The results suggest that the continued use of home health care resources in older patients with cancer who require out-of-hospital care may lead to increased end-of-life care at home. Particularly, use of enhanced HCSCs with beds is most strongly associated with end-of-life care at home.

Impact of home visiting nurses on home death proportion in Japan: A nationwide longitudinal ecological study

Abstract:Objective: Home visiting nurses contribute to end-of-life home care in an aging society. However, few previous studies reported patient outcomes based on nursing practices. This study aimed to examine the correlation between the number of them and the change in home death proportion. Methods: We divided the number of home visiting nurses into four categories: absent, shortage, medium, and abundant. This study adopted the interaction term between the nurse categories and year as the major exposure variable, and home death proportion per municipality as the objective variable. We estimated the average marginal effects (AME) as the change in home death proportion from 2015 to 2020. Results: The total number of home visiting nurses was 36,483 in 2015 and 65,868 in 2020. The coefficients of the interaction term were statistically significant in medium and abundant municipalities (Medium: 1.26 (95% CI: 0.49-2.04), Abundant: 2.15 (95% CI: 0.76-3.55)). Increased home death proportion were estimated as AME: 1.56% (95% CI: 0.99-2.13), 1.35% (95% CI: 0.85-1.84), 2.82% (95% CI: 2.30-3.35), and 3.71% (95% CI: 2.44-4.99) in the absent, shortage, medium, and abundant areas, respectively. Conclusions: To increase the proportion of home deaths, municipalities require a certain number of home visiting nurses.
Advance care planning supported by clinicians trained for end-of-life care in Japanese nursing homes

Abstract:In Japan, which has the highest rate of aging worldwide, the study of advance care planning (ACP) in nursing homes has only just begun. The aim of this study was to evaluate the utility of three clinical strategies on nursing home ACP, supported by trained ACP clinicians. The optimised strategy was to determine a policy on nursing home ACP, involving 1 doctor, 2 nurses, 1 social worker, and 1 facility director. The policy allowed residents and their families to opt out of undesired medical procedures. The proactive strategy was to conduct a lecture on ACP, involving 51 of 109 nursing home residents' families. We investigated the families' preferences regarding care of their dying relative in the nursing home before and after the lecture, even when death certificates were not promptly issued because many Japanese nursing homes have no full-time doctors. The reactive strategy was to perform a multi-occupational interview on ACP. Two nurses selected families of 21 frail residents with deteriorating physical conditions. The primary end-point was the number of residents who died in the nursing home in accordance with their wishes. The lecture significantly increased the number of families choosing terminal care in the nursing home (p <0.01). Intervention by these three strategies significantly increased the number of residents dying in the nursing home in accordance with their wishes (p <0.01). Intervention by these three strategies is useful and this presentation will demonstrate to other Japanese health professionals the technique of performing ACP in Japanese nursing homes.

Abstract:We revealed regional disparities in home‐visit rates for older adults in Japan, with urban areas having higher rates from enhanced home care clinics/hospitals, while depopulated regions rely on conventional ones or general clinics. These rates are significantly influenced by medical and long‐term care resources, as well as regional factors. Aim As aging populations shift health care from hospitals to communities, Japan has implemented policies to promote home health care. This study explored regional differences in home health care recipients among older adults and related factors. Methods We used nationwide data from 2020 to describe the proportion of older adults receiving regular home visits and the medical institutions utilized across secondary medical areas: urban, middle and depopulated areas. We examined factors associated with the proportion of patients receiving regular home visits. Exposures included each secondary medical area's medical and long‐term‐care (LTC) resources, adjusted for the older adult population; proportion of single‐person households; and regional factors. We performed a multivariate negative binomial distribution analysis. Results A total of 333 secondary medical areas were included. Urban areas had more patients receiving regular home visits, primarily from enhanced home care support clinics/hospitals (HCSCs). Fewer patients received regular home visits in depopulated areas, and conventional HCSCs were more common. Multivariate analysis revealed that the number of conventional HCSCs (coefficient, 0.17 [95% confidence intervals (CI), 0.08 to 0.26]), enhanced HCSCs (coefficient, 0.21 [95% CI, 0.14 to 0.29]) and population density (coefficient, 0.10 [95% CI, 0.02 to 0.19]) were positively associated with higher home‐visit rates. Beds in LTC welfare facilities (coefficient, −0.10 [95% CI, −0.19 to −0.01]) and beds in LTC health facilities (coefficient, −0.09 [95% CI, −0.17 to 0.00]) were negatively associated. Conclusions Policies to promote home health care have led to high home‐visit rates in urban areas. Medical and LTC resources and regional factors influence disparities. As Japan's population ages, it is crucial to recognize these disparities and develop medical and LTC systems tailored to each region's characteristics. Geriatr Gerontol Int 2024; ••: ••–••.

Enhancing end-of-life care quality and achieving a good death for the elderly in Japan

Abstract:Background: In the context of an aging populations, there is an escalating need for palliative care tailored to the needs of the elderly. This study aimed to assess differences in symptoms and good death among the elderly, along with the structures and processes involved in end-of life care, and to explore the impact of age on achieving a good death. Methods: We conducted a questionnaire survey for bereaved family members of patients with cancer, heart disease, stroke, pneumonia, and kidney failure in 2019 and 2020. The study population was categorized into the following age groups: ≤64, 65-74, 75-84, and ≥85. The outcomes included symptom intensity, achievement of a good death, and receipt of quality care. Results: In total, 62,576 bereaved family members agreed to participate in the survey (response rate; 54.0 %). The weighted percentages of 'severe' and 'very severe' symptoms decreased with age. These trends were observed across age groups, even among the elderly. The strongest effect of age on achieving a good death was found for 'feeling that life is complete' with reference to those aged ≤64 years: 65-74 years (odds ratio [OR]; 2.09, 95 % CI; 1.94 to 2.25), 75-84 years (OR; 4.86, 95 % CI; 4.52 to 5.22) and ≥85 years (OR; 12.8, 95 % CI; 11.9 to 13.8). Conclusion: Age-specific differences were observed in quality of death, quality of care, and symptom intensity. It is important to provide individualized consideration for each age group rather than categorizing them broadly as the elderly when caring for them.

Abstract:Introduction: The purpose of this study was to clarify the needs in end-of-life care management for elderly persons requiring long-term care at home.Methods: Semi-structured interviews were conducted for 20 people working in the community, and were analyzed qualitatively and descriptively.Results: Support to enable discharge at home, formation of a care team to help achieve a desired life and desired end, family support for the entire end-of-life period, support for making timely and informed decisions, control of symptoms to achieve desired results, and both mental and spiritual support for end-of-life acceptance were provided to realize the individuals' desires.Conclusion: This study suggested that it is possible to provide high-quality care management for the elderly requiring long-term care at home in the end-of-life period.

Abstract:Advance care planning (ACP) allows individuals to plan the health care which dignifies their personal values at the terminal phase of their illnesses. The Japanese government previously endorsed the concept of ACP in the guideline on end-of-life care and community nurses in Japan could play an increased role to improve its utilization by patients. The current study aimed to identify challenges surrounding ACP practice in the home environment in Japan from the community nurses’ perspectives. Semi-structured interview was performed on eleven community nurses working in metropolitan areas in Japan. Qualitative data was collected and analyzed using Braun & Clarke’s six-step framework of thematic analysis (2006). Five themes were identified as barriers to effective ACP utilization: complexity surrounding family power, informed consent and discussion, cultural influence, longer life versus better life, and absence of frameworks and guidelines. The first three themes were further subdivided into subthemes: imbalance in family power and family guilt, inadequate information for decision-making and ‘no one talk about it’, and traditional ways of thinking and taboo surrounding talking about death, respectively. The identified issues surrounding the current ACP practice in Japan were interconnected and reflective of the social, cultural, legal, and ethical aspects of life and care in Japan. This study highlighted the importance of respecting patients’ preferences in care, which should be additionally protected by establishing clear policy and legal frameworks on ACP
Use of Nursing Support Among Nurses for Caregiver Burden in Family Caregivers of Terminally Ill Patients with Cancer in Palliative Care Units in Japan: Multisite Cross-Sectional Study

Abstract:Purpose: This study explores the use of nursing support among nurses for caregiver burden in family caregivers of terminally ill patients with cancer in palliative care units (PCUs). Methods: Requests were sent to 389 institutions, and cooperation was received from 162 PCUs. Nurses at 162 PCUs were asked to participate in an Internet survey regarding nursing practices for caregiver burden in Japan. The frequency of six nursing support practices (extracted in a scoping review) was reported using a 5-point Likert scale. Results: The response rate was 22.3% (539/2448). Support for reducing caregiver stress was the most frequently provided nursing support (mean Likert score: 2.41 for monthly prognosis and 2.42 for weekly prognosis). Psychological and educational support was mainly provided via non-face-to-face (telephone) (mean Likert score: 2.26 for monthly prognosis and 2.21 for weekly prognosis) and face-to-face methods (mean Likert score: 2.32 for monthly prognosis and 2.29 for weekly prognosis). Conclusion: Nursing support was provided through telephone support and face-to-face interactions and aimed at reducing caregiver stress among nurses and family caregivers of patients with terminal cancer in PCUs. In this study, the trends in nursing support were similar for patients with a prognosis of weeks or months.
Gender differences in wishes and feelings regarding end-of-life care among Japanese elderly people living at home

Abstract:Objective: This study aimed to explore the gender differences in wishes and feelings regarding end-of-life care among Japanese elderly people requiring home care services. Patient/Materials and Methods: This study was a secondary analysis of the qualitative data previously compiled from a total of 102 elderly people living at home. The data was retrospectively collected from the participants' nursing records, which included a designated advance care planning (ACP) form completed between January and July 2015. Out of the 102 participants, 86 men and women who were either living alone or with a spouse were selected for the present analysis. We reviewed the participants' ACP forms based on which of the sentiments on the following checklist were expressed: anxiety about the future, abandonment of control, clinging to current daily life, inadequate support from spouse, and a tendency to delegate decision-making. Results: The most commonly expressed feeling was abandonment of control, among both men and women. Among elderly people living alone, women were more likely to want to be surrounded by good, caring people when approaching the end of their lives. Among elderly people living with a spouse, women were more likely to want to delegate decision-making to others. Conclusion: Our results pointed to a gender difference in the attitudes of elderly people toward interactions with the people surrounding them during the end-of-life decision-making process. In order to provide better overall care, health care professionals must come to realize the importance of this gender difference, as it has an impact on the ACP choices made by elderly people living in the community.
Preferred and Actual Places of Death Among Patients with Advanced Cancer: A Single-centre Cohort Study in Japan

Abstract:Objectives: Achieving a 'good death' is one of the important goals of palliative care. Providing goal-concordant care and an environment tailored to the patient's preferences can contribute to a 'good death'. However, the concordance rate between the preferred and actual places of death among advanced cancer patients in Japan is less explored. This study aimed to identify the concordance between patients' preferred and actual places of death and the associated factors among patients with advanced cancer in Japan. Materials and methods: Patients with advanced cancer who underwent chemotherapy at Tohoku University Hospital between January 2015 and January 2016 were enrolled and followed up for 5 years. The enrolled patients were asked about their preference for their place of death. The response options were: "Own home," "General ward" and "Palliative care unit (PCU)." We compared the actual place of death with the patient's preference through a follow-up review of the medical records. Results: A total of 157 patients with advanced cancer were enrolled between January 2015 and January 2016. Of these patients, 22.9% (11/48) died at home according to their preference, 64.0% (16/25) in the general ward and 37.9% (11/29) in the PCU. Only thirty-seven (37.3%) patients died where they wanted, based on the comparison between patients' preferences and actual places of death. Conclusion: The concordance rate between the preferred and actual places of death is not high in Japan. Improving concordance between patients' preferences and actual places of death has the potential to improve end-of-life care.

At this point the sol is diluted and elastic and quasielastic light scattering measurements are made at intermediate momentum transfer. The static scattering exponent for the diluted sol is found to be in good agreement with the percolation prediction of 1.6, and the quasielastic scattering measurements demonstrate the flexibility of the sol clusters.

Evaluation of a training program for community-based end-of-life care of older people toward aging in place: A mixed methods study

Abstract:Aims: To evaluate a training program that supports community-based service staff in implementing aging-in-place and end-of-life care programs. Background: Globally, as the population ages, the need for end-of-life care has never been greater. Since Japan is facing the issues of a super-aged population sooner than most countries, there is a particularly urgent need to enhance end-of-life care for older people. Most Japan's older people wish to spend their final days at home, however, only 11-13% end their lives at home, while 73% die in hospitals. As part of care system, small-scale community-based service for aging-in-place established across Japan in 2006 managed locally. These are flexible services using home or facility care or both for the individual to live long term at their preferred place. We developed the end-of-life care educational training program to encourage behavioral changes in community-based service staff of various readiness levels. Design: Mixed methods study design. Methods: A pre-post evaluation of knowledge, confidence and attitude toward end-of-life care, combining quantitative and qualitative data from 53 community-based service staff members who participated in the training program from September 2017 to September 2019. Participants were informed of the end-of-life care process using focus group discussions about end-of-life care and completed surveys evaluating the program before, immediately following and three months after the training. We used the four-level Kirkpatrick model as the evaluation indicator. Results: Quantitative analysis results indicated that participants were satisfied with the training program. Their knowledge scores regarding end-of-life care significantly improved; they also experienced confidence gains and changes in attitudes, becoming more approving of end-of-life care. Qualitative data revealed details of participants' satisfaction and what was learned. Through the focus group discussions, they created action plans for implementing end-of-life care programs, which some had accomplished in their local centers by the three-month follow-up. Conclusions: These results suggest that the training program is effective for promoting end-of-life care through community-based services in Japan. Conducting follow-up training could effectively strengthen participants' commitment to end-of-life care at community-based services. Use of the training program is expected to promote services to support aging-in-place.
Overall survival of homebound patients without cancer receiving inpatient respite care in a hospital ward in Japan: a retrospective observational study

Abstract:Objectives This study aimed to investigate the relationship between the use of inpatient respite care and the overall survival of homebound patients without cancer admitted to a hospital ward in order to assess the potential impact of inpatient respite care on the duration of home care. Design This was a single-centre, hospital ward-based retrospective observational study. Setting and participants From March 2011 to September 2018, 393 cancer-free older patients (median age, 84.0 years; 53.9% women) receiving continuous medical care at home through clinics were enrolled upon admission to a hospital ward. Primary outcome measures Continuous cumulative survival curves were generated using the Kaplan-Meier method for two groups: inpatient respite care users and non-users. Additionally, prognostic factors associated with all-cause mortality were assessed using the Cox proportional hazards model. Results The Kaplan-Meier curves for inpatient respite care users without cancer admitted to the hospital ward demonstrated a longer median survival time than non-users. Subgroup analyses for patients with or without neurological disorders yielded similar results. The HR for inpatient respite care use, after adjusting for age, sex and other confounding variables, was 0.480 (95% CI: 0.328 to 0.703, p<0.001). Conclusions Homebound patients without cancer receiving inpatient respite care during the study period in Japan demonstrated higher overall survival than those who did not receive respite care. Subgroup analysis of patients with neurological disorders yielded similar results. Further studies are needed to investigate the benefits of inpatient respite care, including the exploration of appropriate methods for its use.

The importance of family caregiving to achieving palliative care at home: a case report of end-of-life breast cancer in an area struck by the 2011 Fukushima nuclear crisis

Abstract:Rationale: The primary setting of palliative care has shifted from inpatient care to patients’ residences. Family caregiving is essential for patients with life-limiting illnesses to receive palliative care at home, however little information is available regarding potential interventions to achieve palliative homecare for those without sufficient support from family members in various settings, including disasters. Patient concerns: In March 2011, Fukushima, Japan experienced an earthquake, tsunami and nuclear disaster. In August 2015, a 59-year-old Japanese female presented to our hospital, located 23 km north of Fukushima Daiichi Nuclear Power Plant, with a right breast ulcer. Diagnoses: The patient was diagnosed with stage IV breast cancer. Interventions: The patient's general condition gradually worsened despite a one-year course of chemotherapy, and she became bedridden after a fall in October 2016. Although the patient wished to receive palliative homecare, this appeared challenging to achieve because she resided alone in a temporary housing shelter. Although she originally lived with her family in Odaka District, Fukushima, she relocated outside of the city following evacuation orders after the disaster. The evacuation orders for Odaka District were still in effect when she returned to the city alone in 2014. We contacted her sister who moved apart from her during the evacuation, and explained the necessity of family caregiving to enable her palliative homecare. Outcomes: The sister decided to move back to their original residence in Odaka District and live with the patient again. The patient successfully spent her end-of-life period and died at home. Lessons: Health care providers and community health workers may need to take a pro-active approach to communicating with family members to draw informal support to enable patients’ end-of-life management according to their values and preferences. This is a lesson which may be applicable to broader healthcare settings beyond cancer, or disaster contexts, considering that population ageing and social isolation may continue to advance worldwide.

Abstract:Objective: To examine variations in end-of-life care for breast cancer, heart failure, and dementia patients. Data and methods: Data from four Norwegian health registries were linked using a personal identification number. Longitudinal trends over 365 days and the type of care on the final day of life were analyzed using descriptive techniques and logistic regression analysis. Results: Patients with dementia were more commonly placed in nursing homes than patients in the two other groups, while patients with heart failure and breast cancer were more frequently hospitalized than the dementia patients. Breast cancer and heart failure patients had a higher likelihood of dying at home than dementia patients. The higher the number of general practitioners, the higher was the probability of home-based end-of-life care for cancer patients, while an increasing non-physician healthcare workers increased the likelihood of home-based care for the other patient groups. Conclusions: Diagnoses, individual characteristics, and service availability are all associated with the place of death in end-of-life care. The higher the availability of health care services, the higher also is the probability of ending the life at home.

Abstract:In older patients who had difficulty returning home after hospitalization, the lack of a description of ACP in home medical care may have been an influencing factor. ACP could help continue home medical care. ACP, advanced care planning; AOR, adjusted odds ratio. Aim To examine the actual conditions of older patients receiving home medical care after hospitalization over a period of 2 years in Japan. Methods The study population included 102 participants, aged ≥65 years, receiving home medical care, who consented to participate in the Osaka Home Care Registry (OHCARE) study in Japan over a period of 2 years. We investigated the actual conditions for returning home after hospitalization. Results The median age of the 102 participants was 84 years, and 61 (59.8%) were women. In the group that returned home, 42 (55.3%) of the respondents desired to recuperate in a familiar place, as in advanced care planning (ACP). During the 2‐year follow‐up period, the group that did not return home had significantly more deaths. A multivariate analysis showed the association in the presence of ACP (odds ratio: 4.72, 95% confidence interval: 1.60–13.86) and cardiac disease (odds ratio: 0.25, 95% confidence interval: 0.08–0.76). The lack of ACP in the medical records when the patient was admitted to the hospital may have prevented the return home. Conclusion In older patients who had difficulty returning home after hospitalization, the lack of ACP in home medical care may have been an influencing factor.
