Abstract:Objective To investigate clinical effect and complication of percutaneous kyphoplasty (PKP) in treatment of multi-segmental osteoporotic vertebral compressive fractures ( OVCF) . Methods A total of 73 patients with osteoporotic vertebral compressive fractures OVCF during February 2008 and July 2013 were recruited in this study, which included 60 patients with two-segmental OVCF and 13 patients with three-segmental OVCF, and there were 159 vertebral bodies altogether. All new fractured vertebral bodies were treated with PKP. Conditions of symptom improve-ment were evaluated by Visual analogue score ( VAS) and Oswestry disability index ( ODI) sore before and after surgery. Radiographic outcomes were evaluated using indexes such as vertebral compression degree, restoration degree of vertebral height and kyphosis losing degree in postoperative 1 year. Conditions of distribution and penetrance of bone cement were studied by postoperative computed tomography scan. Results Symptomatic relief rate of patients were 95. 89% (70/73), and postoperative computed tomography scan showed that the total penetrance of bone cement was 31. 51%. Values of VAS and ODI score in postoperative 1 week and 1 year were lower than those before operation(P<0. 01), and values of VAS and ODI score in postoperative 1 year were lower than those in postoperative 1 week (P<0. 01). Conclusion PKP can effectively treat multi-segmental OVCF with satisfactory analgesic effect, but incident rates of bone cement leak-aging and second fracture are high, and therefore anti-osteoporotic medication should be supplemented.

Abstract:Objective: To evaluate the clinical effect of percutaneous curved kyphoplasty (PCK) for osteoporosis vertebral compression fractures (OVCF). Methods: This is a prospective study.Patients with OVCF who underwent PCK at the Department of Orthopedics,Beijing Chaoyang Hospital, Capital Medical University from June 2018 to June 2019 were included.All the operations were performed by the same surgeon.X-ray examination was performed before and after the operation to measure the vertebral height and Cobb angle.The visual analogue scale (VAS) and Oswestry disability index (ODI) scores were evaluated before and after the operation,and the amount of bone cement injected was record.The leakage rate and distribution of bone cement was observed by CT examination after the operation,and the postoperative complications was collected during the follow-up.Paired-t test was used to compare the related indexes before and after operation. Results: There were 32 patients in our study,including 8 males and 24 females,aged (74.9±9.9) years (range:64 to 81 years).The intraoperative bone cement injection volume was (4.2±1.5) ml(range:2 to 6 ml).According to the classification of distribution of bone cement,28 cases were rated as type Ⅰ and 4 cases were rated as type Ⅱ. Bone cement leakage was observed in 12 cases (37.5%),and there was no intraspinal leakage or venous leakage.The vertebral height was improved from (21.9±6.2) mm preoperatively to (24.3±4.3) mm postoperatively(t=-2.836,P=0.008),Cobb angle improved from(M(QR))14°(15°)preoperatively to 12.5°(12.75°)postoperatively(Z=-1.950,P=0.051),VAS improved from 6.8±0.7 preoperatively to 1.7±0.8 postoperatively (t=28.946,P<0.01),ODI score improved from 73.4±7.3 preoperatively to 21.3±5.7 postoperatively (t=32.250,P<0.01).The patients were followed up for (19.7±3.7) months (range:15 to 29 months).One patient had refracture (3.1%,1/32),and no other complications such as neurological dysfunction and pulmonary embolism occurred. Conclusions: The clinical effect of PCK in the treatment of OVCF was satisfactory.This technique could reduce the difficulty of puncture to a certain extent,and be beneficial to the distribution of bone cement.
A comparative study of percutaneous kyphoplasty and conservative therapy on vertebral osteoporotic compression fractures in elderly patients

Abstract:Objective: To compare the therapeutic efficacy of percutaneous kyphoplasty (PKP) with that of conservative treatment in elderly patients with osteoporotic vertebral compression fractures (OVCFs). Methods: A total of 80 elderly patients with osteoporotic vertebral compression fractures admitted to our hospital from January, 2013 to June, 2015 were randomly assigned by a random number table to receive PKP (the PKP Group, n=40) or conservative treatment (the CT Group, n=40). The two groups were compared in improvements in vertebral fracture restoration and symptom relief before and after treatment. Their improvements in vertebral height and Cobb angle of kyphosis before and after treatment were also measured. In addition, their pain relief and physical functions were assessed with the use of Visual analog scales (VAS) pain scores and Oswestry Disability Index (ODI) scores. Results: The postoperative vertebral height restoration (14.1 mm) of the patients in the PKP Group was significantly improved as compared with that before treatment, and the difference was statistically significant (P<0.05); but no significant improvements were found in the CT group; the decrease in Cobb angle was significantly greater in the PKP Group than in the CT group (P<0.05). Improvements in VAS pain scores and ODI scores at various time points postoperatively were also significantly greater in the PKP Group than in the CT group (P<0.05). Conclusion: The PKP treatment could provide immediate pain relief for the patients and takes the advantages of bringing more significant improvements in vertebral fracture restoration, better correction in kyphosis and weight-bearing capacity at an earlier stage. However, although conservative treatment might lead to smaller trauma, it cannot bring complete restoration in compressed vertebral height and in Cobb angle. Also, it was found to have a high incidence of complications. In such cases, the patients’ quality of life warrants further improvement.

Abstract:Purpose There is still debate over whether vertebroplasty (VP) or kyphoplasty (KP) is superior for the treatment of osteoporosis vertebral compression fractures (VCFs). We performed a systematic review and meta-analysis of randomised and non-randomised controlled trials comparing VP with KP to reach a relatively conclusive answer. Methods We searched computerised databases comparing efficacy and safety of VP and KP in osteoporotic fractures. These trials reported pain relief (Visual Analogue Scale), disability (Oswestry disability score) and complications (i.e., cement leakage, incident fractures) as the primary outcome. Results Eight studies involving 848 patients were identified. The outcome showed that VP is more effective in the short-term (no more than seven days) pain relief. Kyphoplasty had a superior capability for intermediate-term (around three months) functional improvement. As for long-term pain relief and functional improvement, there is no significant difference between these two interventions. Consistently, both interventions have similar risk for subsequent fracture and cement leakage. Conclusion Thus considering the higher cost of the KP procedure, we recommend VP over KP for the treatment of osteoporotic VCFs.

Abstract:Objective To explore the surgical curative efficacy of percutaneous kyphoplasty(PKP)on the treatment of multi-level osteoporotic vertebral compression fractures(OVCFs).Methods PKP were performed in 47 patients(104 injured vertebrae) with multi-level OVCFs.The vertebral sagittal index(VSI)and Cobb′s angle of the injured vertebrae were measured to evaluate the recovery of vertebral anatomy with the X-ray pre-,or post-operatively.Visual analogue scale(VAS)and oswestry disability index(ODI)were calculated to evaluate the release of pain and the recovery of spinal function pre-operatively and on 1 week,1 year postoperatively.Results All patients were followed up for an average of 17.3 months(ranged from 12 to 24 months).VSI was 0.63±0.31 pre-operatively,0.94±0.12 one week post-operatively and 0.92±0.32 one year post-operatively.The difference of VSI in pre-operative group was statistically significant compared to 1 week post-operative group or 1 year post-operative group(P0.01);The Cobb′s angle was 39.12±5.53 degree pre-operatively,8.24±4.11 degree one week post-operatively and 8.66±6.57 degree one year post-operatively.The difference of Cobb′s angle in pre-operative group was statistically significant compared to 1 week post-operative group or 1 year post-operative group(P0.01);The VAS score was 9.26±0.60 pre-operatively and 1.79±1.21 one week post-operatively,1.90±0.86 one year post-operatively.There was significant difference between pre-operative group and 1 week post-operative group or 1 year post-operative group(P0.01).The ODI score was 82.71±3.67 pre-operatively and 19.50±2.73 one week post-operatively,24.14±4.10 one year post-operatively.There was significant difference between pre-operative group and 1 week post-operative group or 1 year post-operative group(F=261.32,P0.01).Conclusions It is minimal invasive by using PKP for the treatment of multi-level OVCFs.Satisfied recovery can be expected in vertebral height,Cobb′s angle,relief of the pain and restoration of spinal functional.
The efficacy of percutaneous kyphoplasty on osteoporotic vertebral compression fractures and its effects on the quality of life in the elderly

Abstract:Objective: To investigate the efficacy of percutaneous kyphoplasty (PKP) on osteoporotic vertebral compression fractures (OVCF) and its effects on the quality of life (QOL) in the elderly. Methods: A prospective study was conducted on 126 elderly OVCF patients. They underwent surgery and were divided into an observation group (treated with percutaneous vertebroplasty, n=63) and a control group (treated with PKP, n=63). The two groups were given routine anti-osteoporosis treatment after admission. The Cobb angles of the injured vertebrae and the anterior height of the vertebral bodies before and after the operations were compared between the two groups. The Oswestry Disability Index and the Visual Analogue Scale scores before the operations, at three months after the operations, and at six months after the operations were compared between the two groups. The QOL scores before the operations and at six months after the operations were compared between the two groups. The postoperative complications were also compared between the two groups. Results: Compared with before the operations, the Oswestry Disability Index and Visual Analogue Scale scores in the two groups were significantly decreased at three and six months after the operations (all P<0.001), and the scores in the observation group were significantly lower than the scores in the control group (all P<0.05). Compared with before the operations, the Cobb angles of the injured vertebrae in the two groups were significantly reduced immediately after the operations, and the Cobb angles in the observation group were significantly smaller than they were in the control group (all P<0.001). There was an opposite trend in the anterior height of the vertebral body (all P<0.001). Six months after the operations, the two groups’ QOL scores were significantly higher than they were before the operations, and the observation group’s scores were significantly higher than the control group’s (all P<0.001). The incidence of postoperative complications in the observation group was significantly lower than it was in the control group (3.17% vs. 15.87%, P<0.05). There were no severe complications such as lower-limb venous thrombosis or pulmonary embolism in the two groups. Conclusion: Compared with percutaneous vertebroplasty, PKP can significantly relieve pain, restore vertebral height, improve vertebral dysfunction and postoperative QOL in elderly OVCF patients, and it is very safe and worthy of clinical promotion.

Abstract:Objective To assess the clinical effect of percutaneous kyphoplasty(PKP) for fresh and old osteoporotic vertebral compressive fractures(OVCFs). Methods The study involved 58 patients with OVCFs in single vertebra treated by PKP from September 2010 to October 2012,all the patients had unambiguous history of trauma. There were 22 patients with old fractures in group A at least 8 weeks after injury,36 patients with fresh fractures in group B within 8 weeks after injury. We recorded visual analog scale(VAS),the relative height of anterior and middle column of the vertebra and the local kyphotic angle before and after the operation, and the amount of cement put in was recorded. Results The difference of the cement amount between the two groups was significant(P0.05). The VAS scores,relative height of anterior and middle column vertebra and the kyphotic angle were all improved significantly after PKP in both two groups(P0.05). Conclusion PKP is an effective treatment for patients with fresh or old painful osteoporotic vertebral fractures in terms of pain relief and functional recovery. However,for the old fractures,the height of anterior and middle column vertebra and the kyphotic angle will not be improved as perfectly as the fresh fractures,and the rate of refracture of the cemented vertebra is higher.
Analysis of Pre- and Postoperative Pain Variation in Osteoporotic Vertebral Compression Fracture Patients Undergoing Kyphoplasty

Abstract:BACKGROUND Balloon kyphoplasty (KP) has been widely applied in the treatment of elderly patients with osteoporosis vertebral compression fracture (OVCF), but there has been little research on the pain relief effect. Therefore, we performed this study of patients who received KP. The study included a set of fluoroscopy tests and follow-up evaluation, which aimed to verify the effectiveness of kyphoplasty in controlling back pain associated with OVCFs. MATERIAL AND METHODS Forty-three OVCF patients underwent kyphoplasty: 21 were allocated to an intervention group and 22 were allocated to a control group, and the 2 groups received treatment with different KP instruments. The variation of vertebral height was measured on X-ray and change of signal of MRI was recorded. The pain was assessed by VAS score and diagram, and physical function was evaluated by ODI. The complications after surgery were recorded and collated during 2 years of follow-up. RESULTS The intervention group showed no significant difference on the VAS and ODI compared to the control group (p>0.05). There was no difference in the VAS with different degrees of radiological change (p>0.05). Signal change on MRI imaging was rare. CONCLUSIONS Kyphoplasty is a positive way to alleviate early-onset OVCF pain. The change of BME extent in the treated level is unrelated to the relief of back pain after KP.

Abstract: This study provides a baseline examination of teenage sexual relationships in 1991 in a sample drawn from 9 state and independent schools in southwest England; a follow up was conducted in 1992. 1025 students in year 11 and aged 15-16 responded in 1991; the follow up in 1992 showed 315 returning the questionnaire. Information was requested on knowledge of sexually transmitted diseases (STDs), age at first intercourse, sexual intercourse without use of contraception, the frequency of "one night stands" or relationships measured in days, and knowledge of friends who had STDs. Relative risk ratios were calculated for those engaging in sexual activity before and after the age of 16 years. The results showed that in the follow up more girls and persons with more academic interests, as represented by the school subjects taken for the General Certificate of Secondary Education, responded. There were no differences by social class. The average age of the sample was 16.9 years. 54% were sexually active. 84 had sexual intercourse when they were under 16 years and 84 has sexual intercourse between the ages of 16 and the administration of the questionnaire. Those engaging in intercourse prior to the age of 16 were twice as likely to not have used contraception (relative risk [RR] = 1.86, confidence interval [CI] = 1.33 - 2.60 at 95%). They also were more likely to have had sex within a short relationship (RR = 2.24, CI = 1.38 - 3.62). They were 3 times more likely to know a close friend with an STD (RR = 2.97, CI = 1.55 - 5.70). Only 5 of the nonsexually active teenagers knew someone with an STD. The extent of sexual activity was lower than reported from other surveys. The results confirm prior understanding that younger teenagers engage in more sexual risk taking and are less likely to use contraception. The suggestion is that this risk-taking behavior is related to the cognitive development of younger teenagers of the age of first intercourse.
The Influence of Intravertebral Cleft on the Clinical Effect of Percutaneous Kyphoplasty

Abstract:Objective To evaluate the influence of intravertebral cleft on the clinical effect of percutaneous kyphoplasty (PKP).Methods One-hundred and eighty three patients with OVCFs who underwent PKP (from December 2009 to December 2011) were divided into IVC group and non-IVC group according to their radiographic features.In IVC group,there were 5 males and 39 females,with an average age of 71.6 years (range,56 to 89 years).And in non-IVC group,there were 22 males and 117 females,aged from 51 to 91 years (average,70.2 years).The distributions of fractured vertebra body,bone cement injection volume,bone cement leakage incidence and types,the postoperative height of fractured vertebral body,as well as the visual analogue scale (VAS) and Oswestry disability index (ODI) were compared between the two groups.Results The distributions of fractured vertebra body between two groups were different.Fractured vertebra body in IVC group mainly located from T11 to L2.Fractured vertebra body in non-IVC group located from T11 to L5,and from T7to T9.The bone cement injection volume and leakage incidence had no significant difference between IVC group and non-IVC group (3.4-5.6 ml vs.3.5-5.1 ml; 45.3％ vs.41.9％).However,the types of bone cement leakage were different.In the IVC group,bone cement mainly leaked into perivertebral soft tissues,while in the non-IVC group it mainly leaked along blood vessels.In both groups,the heights of the fractured vertebral bodies were significantly improved,and the restoration of vertebral height in IVC group was more evident than that in the non-IVC group.The postoperative VAS and ODI had no statistical difference between IVC group and non-IVC group (2.8±1.1 vs.2.4±0.7; 29.3％±6.8％ vs.27.6％±6.9％).Conclusion The osteoporotic compression vertebral fracture mainly located in the range from T12 to L2 vertebra.The application of PKP could obtain a very good result in the treatment of OVCF with intravertebral cleft,moreover,the anterior vertebral height can increase remarkablely.
A Comparison of Percutaneous Kyphoplasty with High-Viscosity and Low-Viscosity Bone Cement for Treatment of Osteoporotic Vertebral Compression Fractures: A Retrospective Study

Abstract:Background Osteoporotic vertebral compression fracture (OVCF) has become a health issue of worldwide concern. Percutaneous kyphoplasty (PKP) is one of the main surgical methods for OVCFs. This study aimed to evaluate and compare the clinical efficacy and safety of PKP with high- and low-viscosity bone cement for OVCFs. Methods Totally 62 patients with single-level OVCF were enrolled in this study from December 2018 to April 2021. Among them, 32 cases underwent PKP with high-viscosity bone cement, while 30 cases underwent PKP with low-viscosity bone cement. Visual analog scale (VAS) scores and Oswestry disability index (ODI) scores were used in the pre- and post-operative period to assess patients’ rehabilitation. Compression rates of anterior vertebra height (AVH) and posterior vertebra height (PVH) were analyzed to evaluate the restoration of vertebra height. Leakage rates and locations were recorded to show clinical safety. Results VAS and ODI scores both significantly improved in 2 groups at 1 day, 1 month, and 3 months after surgery. Compression rates of AVH and PVH at 1 day and 3 months after PKP were lower than those before surgery. However, there was no significant difference in VAS scores, ODI scores, and compression rates between both groups. However, PKP with high-viscosity bone cement achieved a lower bone cement leakage rate significantly, which showed the safety of high-viscosity bone cement in PKP. Conclusions PKP with high- and low-viscosity bone cement both improved the recovery of patients and restored vertebra heights. Notably, PKP with high-viscosity bone cement can achieve favorable clinical outcomes as well as lower bone cement leakage rate.

Vertebroplasty versus kyphoplasty in osteoporotic vertebral compression fracture: a meta-analysis of prospective comparative studies

Abstract:PurposeThe goal of this article is to evaluate the efficacy and the safety of the percutaneous vertebroplasty (PVP) versus percutaneous kyphoplasty (PKP) in dealing with the osteoporotic vertebral compression fracture (OVCF).MethodsIn July 2014, a comprehensive systematic computer-based online search was performed by using the databases of PubMed, EMBASE, Cochrane Library, Web of Science, Wan Fang, and the China Biological Medicine. Only prospective comparative trials (PCT) and randomized controlled trials (RCT) that compared PVP with PKP were included. Trials were screened based on the inclusion and exclusion criteria previously formed. The Cochrane collaboration guidelines were also used to assess the quality of these included studies. The primary data of these studies [volume of the cement, postoperative vertebral height, visual analog scale (VAS) score and Oswestry Disability Index (ODI) score after the surgery, and so on] were carefully abstracted and processed by Revman 5.2.0 software The publication bias of the main results (cement leakage and adjacent-level fracture) were examined by Stata 12.0 (Begg and Egger test). Furthermore, the stability of the main results were also detected by sensitivity and cumulative analyses.ResultsSix RCT and 14 PCT studies involving 1,429 patients met our criteria and were included finally. Comparing these two methods, the PKP group took more operation time [SMD = 0.66, 95 % CI (0.28, 1.03), p = 0.0006] with higher anterior vertebral body height [SMD = 1.40, 95 % CI (0.49, 2.32), p = 0.003], greatly reduced Cobb angle in the long run [SMD = −0.61, 95 % CI (−1.04, −0.19), p = 0.005] and had lower risk of cement leakage. However, in VAS scores and ODI scores after the surgery whether for the short-term efficacy (no more than 1 week after the surgery) or long-term efficacy (more than six months), Cobb angle in the short run and new fracture in the adjacent level, no statistically differences were found between the two groups.ConclusionsBased on current evidence, PVP takes less time in the operation, while it has greater risk of cement leakage, was inferior in reducing Cobb angle in the long term and results in lower anterior vertebral body height after the surgery. For pain relief, which is the main desire of the patients, both procedures provide significant improvement in VAS and ODI pain scores. PVP is still an effective procedure.

Radiographic measures included anterior vertebral height (AVH), thoracic kyphosis (TK), lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), and sacral slope (SS). Clinical function measures included Oswestry disability index (ODI) and visual analog scale (VAS) scores. Results: There were no significant differences in the preoperative basic data between the groups classified as IVC and NIVC. However, both groups showed significant improvements in AVH and TK throughout the follow-up periods compared to the preoperative measurements ( P < 0.05). The recovery of AVH in the IVC group was found to be inferior to that in the NIVC group at 3 years after operation ( P < 0.05). There were no significant differences in LL, PI, PT and SS in both groups compared with the preoperative results and no statistically significant differences between the two groups at the same follow-up time ( P > 0.05). The VAS and ODI scores during all follow-up periods were significantly lower than those before operation ( P < 0.05). At 3 years after operation, the VAS and ODI scores of the IVC group were higher than those of the NIVC group ( P < 0.05). Conclusion: PKP is an adoptable measure to treat thoracic OVCFs with or without IVC. Our study revealed that the NIVC group was superior to the IVC group in terms of improved vertebral height and pain recovery at long-term follow-up (3 years). Keywords: percutaneous kyphoplasty, thoracic, osteoporotic vertebral compression fracture, intervertebral cleft, sagittal balance Aging populations worldwide have raised concerns about osteoporosis and osteoporotic vertebral compression fractures (OVCFs), and the incidence of OVCFs continues to increase as the elderly population increases. 1 OVCFs are known to affect quality of life and mortality by causing persistent pain, kyphosis, and mobility limitation. 2 In 1987, Galibert introduced vertebroplasty, in which medical bone cement (polymethyl methacrylate) is injected into a fractured vertebra. 3 Subsequently, the utilization of this technique has become widespread in minimally invasive surgery for osteoporotic vertebral fractures due to its capacity to promptly alleviate pain and facilitate early recuperation. 4 OVCFs often frequently manifest in the thoracolumbar spine; however, the thoracic vertebra exhibits distinctive characteristics in comparison to the lumbar vertebra, including steep and diminutive pedicles, a reduced angle between the pedicle and the vertebral body, and a narrower anterior margin of the vertebral body. 5 In response to these characteristics of thoracic vertebrae, PKP for thoracic vertebra is more risky and more difficult to puncture. Vertebrae above T10 are often operated on with the extrapedicular approach, which is different from the transpedicular approach of lumbar vertebrae. Consequently, it is imperative to differentiate between thoracic osteoporotic compression fractures and lumbar osteoporotic compression fractures. The occurrence of intravertebral cleft (IVC) following OVCF is not a rare phenomenon. 6 These clefts, also known as intravertebral vacuum phenomena or Kümmell signs, have been extensively documented in the literature on medical imaging. 7 IVC was first described by Maldague in 1978, has long been considered the result of local bone ischemia associated with nonunion vertebral collapse. 8 On radiographs, they manifest as a radiolucent shadow within the vertebra, appearing as a transverse, linear, or semilunar shape. Abstract:Abstract Background The purpose of this study is to explore the therapeutic effect of percutaneous kyphoplasty (PKP) combined with anti-osteoporosis drug, zoledronic acid, on postmenopausal women with osteoporotic vertebral compression fracture (OVCF) and to perform an analysis of postoperative bone cement leakage risk factors. Methods A total of 112 OVCF patients, according to therapeutic regimens, were divided into control group ( n = 52, treated with PKP) and observation group ( n = 60, treated with PKP and zoledronic acid injection). Results Postoperative tumor necrosis factor-α and interleukin-6 levels were significantly decreased in the two groups, compared with those before treatment (both P < 0.05); bone mineral density (BMD), serum bone gla protein (BGP), and vertebral height ratio of injured vertebrae were significantly increased, and procollagen type I N-terminal propeptide (PINP), Cobb angle, visual analogue scale/score (VAS), and Oswestry disability index (ODI) were significantly decreased compared with those before treatment (all P < 0.05). There were significantly higher changes in difference value of BMD, PINP, BGP, vertebral height ratio of injured vertebrae, Cobb angle, VAS, and ODI levels and significantly better therapeutic effect in the observation group than those in the control group (all P < 0.05). Multivariate logistic regression analysis showed that the use of zoledronic acid, vertebral height ratio of injured vertebrae, and ODI were independent factors affecting the therapeutic effect, and that the dosage of bone cement, and peripheral vertebrae wall damage were independent risk factors causing postoperative bone cement leakage. There were no significant differences in postoperative bone cement leakage rate between the two groups. Conclusions Peripheral vertebrae wall damage and the dosage of bone cement are independent risk factors causing bone cement leakage in OVCF patients treated with PKP. PKP combined with zoledronic acid has an improvement effect on the condition of postmenopausal women with OVCF and reduces the inflammation and pain in patients, which is beneficial to clinical treatment.

Abstract:Background: Percutaneous vertebroplasty (PVP) and kyphoplasty (PKP) have been widely used to treat neurologically intact osteoporotic Kümmell's disease (KD), but it is still unclear which treatment is more advantageous. Our study aimed to compare and investigate the safety and clinical efficacy of PVP and PKP in the treatment of KD. Methods: The relevant data that 64 patients of neurologically intact osteoporotic KD receiving PVP (30 patients) or PKP (34 patients) were analyzed. Surgical time, operation costs, intraoperative blood loss, volume of bone cement injection, and fluoroscopy times were compared. Occurrence of cement leakage, transient fever and re-fracture were recorded. Universal indicators of visual analogue scale (VAS) and Oswestry disability index (ODI) were evaluated separately before surgery and at 1 day, 6 months, 1 year, 2 years and the final follow-up after operation. The height of anterior edge of the affected vertebra and the Cobb's angle were assessed by imaging. Results: All patients were followed up for at least 24 months. The volume of bone cement injection, intraoperative blood loss, occurrence of bone cement leakage, transient fever and re-fracture between two groups showed no significant difference. The surgical time, the operation cost and fluoroscopy times of the PKP group was significantly higher than that of the PVP group. The post-operative VAS, ODI scores, the height of the anterior edge of the injured vertebrae and kyphosis deformity were significantly improved in both groups compared with the pre-operation. The improvement of vertebral height and kyphosis deformity in PKP group was significantly better than that in the PVP group at every same time point during the follow-up periods, but the VAS and ODI scores between the two groups showed no significant difference. Conclusion: PVP and PKP can both significantly alleviate the pain of patients with KD and obtain good clinical efficacy and safety. By contrast, PKP can achieve better imaging height and kyphosis correction, while PVP has the advantages of shorter operation time, less radiation volume and operation cost.

Abstract:BackgroundIntravertebral cleft is a structural change in osteoporotic vertebral compression fractures (OVCF), which is the manifestation of ischemic vertebral osteonecrosis complicated with fracture nonunion and pseudoarthrosis and appears in the late stage of OVCF. Despite numerous studies on OVCF, few aim to evaluate the clinicoradiological characteristics and clinical significance of intravertebral cleft in OVCF. This study investigates clinicoradiological characteristics of intravertebral cleft in OVCF and the effect on the efficacy of percutaneous balloon kyphoplasty (PKP).Materials and MethodsPKP was performed on 139 OVCF patients without intravertebral cleft (group A) and 44 OVCF patients with intravertebral cleft (group B). The frequency distribution of the affected vertebral body, bone cement infusion volume, imaging manifestation, leakage rate and type, preoperative and postoperative height of the affected vertebral body, visual analog scale (VAS) and Oswestry disability index (ODI) score were evaluated.ResultsSignificant differences were found in the frequency distribution of the affected vertebral body and bone cement leakage type between the two groups (P < 0.05). However, differences in bone cement infusion volume and leakage rate (P > 0.05) were not detected. In both groups, the postoperative height of the affected vertebral body was significantly improved (P < 0.05). The restoration of vertebral body height in group B was more evident than that in group A (P < 0.05). The preoperative VAS and ODI scores in group B were significantly higher than those in group A (P < 0.05). After surgical treatment, pain relief and daily activity function in both groups were significantly improved (P < 0.05), and no significant difference in postoperative scores was detected between the two groups (P > 0.05).ConclusionIntravertebral cleft exhibits specific clinical and imaging as well as bone cement formation characteristics. PKP can effectively restore the affected vertebral body height, alleviate pain, and improve daily activity function of patients.

Abstract:STUDY DESIGN Data was gathered from biomechanical testing of 10 thoracic human cadaveric spines. Spines were tested intact and with a Luque rectangle fixed with wire or cable. OBJECTIVES To compare the rigidity of fixation and intraspinal penetration of sublaminar monofilament wire and multistrand cable under identical conditions using human cadaveric spines. SUMMARY OF BACKGROUND DATA Reports of neurologic and mechanical complications associated with sublaminar wiring techniques have led to the recent development of more flexible multistrand cable systems. The relative performance of flexible cable versus monofilament wire has not been explored fully in a controlled mechanical environment. METHODS A servohydraulic mechanical testing machine was used to measure the static mechanical stiffness of sublaminar wire or cable fixation in conjunction with a Luque rectangle for thoracic human cadaveric spine segments in flexion-extension and torsion modes. Cyclic testing was performed in the flexion-extension mode. Intraspinal penetration of wires and cables was measured. RESULTS Spine fixation with sublaminar wire and cable resulted in constructs of equal stiffness in flexion-extension and torsion modes. Cyclic testing also indicated similar fatigue profiles for wire- and cable-instrumented spines. Wire and cable fixed spines displayed greater stiffness than the intact spines. Cable encroachment of the spinal canal was less than that seen with wire. CONCLUSIONS Sublaminar multistrand cable may be a rational alternative to monofilament wire in segmental spinal instrumentation because it provides less encroachment into the spinal canal. Further, cadaveric spines instrumented with wire and cable display equivalent mechanical behavior, statically and under cyclic loading. The potential advantages of cable, however, must be balanced against a substantial increase in cost relative to wire.

Abstract:Objective: To explore the appropriate bone cement filling ratio in percutaneous kyphoplasty (PKP) for the treatment of osteoporotic vertebral compression fractures (OVCF). Methods: Clinical and radiological data from 150 OVCF patients treated with PKP were retrospectively analyzed. Patients were categorized into three groups based on bone cement filling ratio: low ( 0.6) filling ratio groups. The clinical characteristics (age, gender, BMI, etc.) and related study data (bone cement leakage and its location, pre/post-operative Visual Analogue Scale (VAS), pre/post-operative Oswestry Disability Index (ODI), vertebral height restoration, kyphotic Cobb angle, etc.) among the three groups were compared using statistical software to compare to identify the most appropriate cement filling ratio. Results: The 0.4-0.6 group presented a lower cement leakage rate compared to the >0.6 group, and there were no significant differences in pre-operative VAS, post-operative day 2 VAS, post-operative month 1 VAS, and pre-operative ODI (p>0.05). However, significant differences were observed in post-operative month 3 VAS (p=0.002), post-operative day 2 ODI (p=0.002), post-operative month 1 ODI (p<0.001), and post-operative month 3 ODI (p 0.6" group at the 3-month follow-up. While presenting the best vertebral height restoration, the ">0.6" group also exhibited the greatest variability. Additionally, no significant difference in Cobb angle changes was observed among the groups. Conclusion: A bone cement filling ratio of 0.4-0.6 in PKP treatment for OVCF strikes a favorable balance between complication reduction and positive patient outcomes, warranting it as an optimal filling volume.

Abstract:Objective To analyze the clinical effect of percutaneous pedicle screw fixation and percutaneous kyphoplasty (PKP) for osteoporotic vertebral compressive fractures(OVCFs). Methods From October 2011 to May 2013,we selected 49 mild to moderate osteoporotic patients with single fractured vertebra,20 treated by percutaneous pedicle screw fixation and 29 by PKP, all the patients had unambiguous history of trauma. We recorded the operation time,the amount of blood loss,visual analog scale(VAS),the relative height of anterior and middle column of the vertebra and the local kyphotic angle before and after the operation. Results The difference of operation time,the amount of blood loss between the two group were significant (P<0.05). The VAS scores,the relative height of anterior and middle column of the vertebra,the kyphotic angle improved significantly after both operation(P<0.05). Conclusion Percutaneous pedicle screw fix-ation is an effective treatment for patients with mild to moderate osteoporotic vertebral fractures in terms of pain relief,the height of anterior and middle column vertebra and the kyphotic angle improving as perfectly as the PKP.

Abstract:Abstract Background This study was performed to investigate the clinical efficacy of percutaneous kyphoplasty (PKP) for vertebral compression fractures with different bone mineral densities (BMD). Methods We performed a retrospective analysis of 232 patients with single-segment vertebral compression fractures who underwent PKP. Patients were divided into the normal BMD, osteopenia, and osteoporosis groups according to their average lumbar BMD before surgery. The visual analog scale (VAS) was used to compare differences in pain relief before and after surgery in each group. Corrections of the wedge angle and kyphotic angle before and after surgery were observed using anteroposterior and lateral radiographs and compared among the groups, as was the incidence of bone cement leakage. Results Patients were followed up for 6–12 months, with an average follow-up time of 9.12 ± 1.68 months. The VAS score, wedge angle, and kyphotic angle of the three groups of patients decreased significantly at the end of the follow-up ( P 0.05). Conclusions PKP has a positive effect on vertebral compression fractures with different BMD, and is especially suitable for osteoporotic vertebral compression fractures.
