College Students' Reflections on Their Experience Facilitating a Photovoice Research Project with BIPOC Older Adults and Frontline Healthcare Workers

Abstract:The photovoice literature has focused mainly on research collaborations between researchers in academia and community partners. There is limited evidence of undergraduate students facilitating photovoice research projects with underserved Black, Indigenous, and persons of color (BIPOC). Students' participation in photovoice research increases their understanding of local issues and can empower them to engage with local communities to create change. This qualitative study explored students' experiences facilitating photovoice research with BIPOC older adults and frontline healthcare workers. In this study, we trained college students in photovoice methodological skills by introducing photovoice as a participatory qualitative methodology, explained ethics issues and the informed consent process, and clarified the steps and requirements for the photovoice project. Students were paired with BIPOC older adults and frontline healthcare workers. They facilitated the photovoice research project in four stages. Throughout the study, students critically reflected and wrote about the various aspects of their experience facilitating the photovoice research activities based on weekly structured reflection questions. In their weekly journal, student facilitators commented on how both older and younger BIPOC participants were friendly and kind, flexible with the research activity scheduling, inspiring with their stories, and selfless. Students' reflections indicated that their participants experienced greater emotional and psychological burdens during the COVID-19 pandemic, exacerbated by the long-standing epidemic of systemic racism. Student facilitators overwhelmingly recognized participants' resilience and perseverance despite these life challenges. Our findings highlight the importance of photovoice in developing and strengthening college students' qualitative research skills and stimulating critical thinking and creativity, a sense of critical consciousness about social issues and society, and a sense of empowerment. Our results will be relevant to hospital/long-term care managers, policymakers, and researchers.
Preparation for Rural Practice with a Multimodal Rural Emergency Medicine Curriculum

Abstract:Rural regions face emergency medicine (EM) physician shortages. Most training programs are located in cities and lack rural clinical experiences, didactics, and mentorship to excite and prepare residents for rural EM practice. There is limited data on optimal training methods for preparing residents for rural practice. To address this need for rural EM training and workforce, we developed a rural EM curriculum. We began with a two-year case review from critical access emergency departments. Rural EM skills were defined and taught using lectures, simulation cases, and clinical rotations. We obtained quantitative and qualitative feedback from the first ten residents participating in the curriculum. Qualitatively, 10/10 residents gained new skills and found these experiences valuable to their training and career choice, with 100% expressing interest in rural practice and 75% choosing a rural practice. Quantitatively, residents managed a greater variability in patient acuity and volume and performed a greater variety of procedures compared to their academic center rotations, all while gaining unique skills from the challenges of a rural environment. Focused rural emergency medicine clinical experience and didactic training during residency are a promising approach to bridge the gap between urban tertiary care training programs and rural emergency care needs.

Abstract:An extension of the capacitated, fixed charge, multicommodity network flow problem with an uncertain demand of services and survivability constraints was designed and modeled as a stochastic programming problem. A polynomial algorithm based on the GRASP metaheuristic with a construction phase of a survivable topology and a local search phase based on a key-path decomposition of the graph and a feasible key-path replacement was proposed to solve it. The heuristic algorithm was tested for several problem instances, and its solutions were compared with a branch-and-cut solver. The heuristic reached good quality solutions for the tested cases.

Abstract:Objective Rural rotations have become increasingly common. However, little is known about whether they offer educational benefits for residents who do not eventually practice in rural facilities. Design Starting with an ACGME-provided roster of general surgery programs, the authors conducted a website review (12/2022-7/2023) of the 342 civilian programs located in U.S. states to identify 81 programs (24%) offering elective or required rural rotations in the U.S. Because previous research is scant, the authors then adopted a qualitative approach and designed a semistructured interview schedule to learn from program leaders what educational benefits their rural rotations might provide residents and whether any noted benefits applied to all residents or just those open to practicing in a rural location. The first author conducted 30-minute telephone interviews with 58 general surgery program leaders for a participation rate of 72% (58 of 81). Interviews were conducted in 2 blocks (8/2023-11/2023 and then 7/2024-8/2024) and focused on the 2023-24 academic year. Interviews were recorded, transcribed, and analyzed collaboratively by the first three authors. Results All program leaders described how their rural rotations provide educational benefits that transcend the issue of practice location. The authors identify and describe 4 prominent themes from the interviews: access to cases, particularly those deemed core or "bread-and-butter" general surgery; exposure to rural practitioners who model broad-spectrum general surgery; insights into resource-limited practice settings; and the learning advantages of increased responsibility, leadership development, and mentorship typically present in small rural facilities. Conclusions Program leaders touted the educational benefits of rural rotations for all residents, a finding that might prompt more programs to consider such opportunities. Academic-rural partnerships could become a routine component of residencies.

Development of 'rural week' for medical students: impact and quality report

Abstract:Introduction: The Rural Undergraduate Support and Coordination program funds medical schools to provide all medical students some time in rural Australia throughout their course. The University of Adelaide has developed a rural week program for both first and second year students to fulfill part of this objective. Methods: First year students' rural week is an introduction to a range of rural health issues, Indigenous culture and rural lifestyle issues. Second year students choose either a clinical rural week with a general practitioner or a week of Indigenous cultural learning with the Adnyamathanha people in the Flinders ranges, South Australia. Evaluation data were collected from students, practitioners and university staff during rural weeks in 2003 and 2004 using quantitative and qualitative methods. Results: First year students increased their knowledge of and interest in rural medicine and enjoyed their (limited) clinical interaction with patients. Second year students appreciated the clinical experience and valued the welcome they received from doctors and practice staff. Those who chose Indigenous cultural programs appreciated the opportunity to interact with and learn from Indigenous people. General practitioners valued contributing to student knowledge and skills and the opportunity to promote rural practice. Volunteer community members were very enthusiastic about meeting the students and their generosity had a significant impact on the students' ideas about rural lifestyle. University academic and administrative staff found rural week very intensive work but experience and thorough preparation now ensures that few problems occur. Conclusion: Rural weeks are now an established part of the first year and second year curriculum at the University of Adelaide Medical School. The ability to provide small groups of students with an intense introduction to rural practice and/or Indigenous culture results in a positive change in opinion about this aspect of medical education. Students now have first-hand experience to positively influence their choice of rural education or Indigenous health options in the medical course. Problems do occur with students being away from the traditional classroom, and protocols have been put in place for behaviour of all parties. Despite the rural weeks program being very demanding on staff energy, financial resources, general practices and rural communities, we feel that this program is valuable and sustainable. Further experience will determine if rural weeks function effectively as a recruitment strategy for the long rural placements offered by the Rural Clinical School.
Evolution of cooperative behavior in a spatial iterated prisoner's dilemma game with different representation schemes of game strategies

Abstract:The iterated prisoner's dilemma (IPD) game has been frequently used to examine the evolution of cooperative behavior among agents in the field of evolutionary computation. A number of factors are known to be related to the evolution of cooperative behavior. One well-known factor is spatial relations among agents. The IPD game is often played in a grid-world. Such a spatial IPD game has a neighborhood structure which is used for local opponent selection in the IPD game and local parent selection in genetic operations. Another important factor is the choice of a representation scheme to encode each strategy. Different representation schemes often lead to totally different results. Whereas the choice of a representation scheme is known to be important, a mixture of different representation schemes has not been examined for the spatial IPD game in the literature. This means that a population of homogeneous agents with the same representation scheme has been assumed. In this paper, we introduce a different situation to the spatial IPD game in order to examine the evolution of cooperative behavior under more general assumptions. The main novelty of our spatial IPD game is the use of a mixture of different representation schemes. This means that we use a population of inhomogeneous agents with different representation schemes. Another novelty is the use of two neighborhood structures, each of which is used for local opponent selection and local parent selection. Under these specifications, we show a number of interesting observations on the evolution of cooperative behavior.
Evaluation of a multi-site health services psychology training program for telehealth and integrated behavioral health

Abstract:Introduction: Training future providers in telehealth and integrated care models can improve access and outcomes, especially among rural and underserved populations. The (blinded) project implemented behavioral health training for health service psychology doctoral students with three partner organizations. Trainees received both experiential and didactic training in telehealth and integrated behavioral health. Telehealth was utilized for remote warm hand-offs, hybrid shared appointments, therapy sessions, coordination with providers, and supervision. Program elements included opportunities for consultations with experts in other disciplines, supportive mentorship, exposure to various parts of a healthcare system, and interactions with diverse clients. Methods: The (blinded) training program evaluated trainee outcomes using fourteen interviews and three focus groups. Interviews and focus groups examined aspects of the program that contributed to trainees' knowledge, skills, and attitudes. Results: Evaluation results revealed increased levels of trainee confidence, autonomy and independence. Training reportedly enabled improved ability to collaborate and communicate with other professions, increased flexibility and adaptability, and openness to others' ideas. Trainees reported the program's use of telehealth enhanced awareness of their own skills and team members' perspectives of technology in care delivery. Discussion: Descriptions of the three care models, lessons learned, and qualitative results about trainee outcomes can be translated into best practices for workforce development and enhance psychology trainees' self-awareness and ability to incorporate others' viewpoints about technology and treatment approaches into healthcare.

Preparing behavioral health clinicians for success and retention in rural safety net practices

Abstract:Purpose This study assesses how, among behavioral health clinicians working in rural safety net practices, the amount of exposure to care in rural underserved communities received during training relates to confidence in skills important in their work settings, successes in jobs and communities, and anticipated retention. Methods This study uses survey data from Licensed Clinical Social Workers, Licensed Professional Counselors, and Psychologists working in rural safety net practices in 21 states while receiving educational loan repayment support from the National Health Service Corps, from 2015 to April 2022. Findings Of the 778 survey respondents working in rural counties, 486 (62.5%) reported they had formal education experiences with medically underserved populations during their professional training, for a median of 47 weeks. In analyses adjusting for potential confounders, the estimated amount of rural training exposure was positively associated with a variety of indicators of clinicians' integration and fit with their communities as well as with longer anticipated retention within their rural safety net practices. The amount of training in care for rural underserved populations was not associated with clinicians' confidence levels in various professional skills or successes in their work, including connection with patients and work satisfaction. Conclusions Formal training in care for underserved populations is a large part of the education of behavioral health clinicians who later work in rural safety net practices. More training in rural underserved care for these clinicians is associated with greater integration and fit in their communities and longer anticipated retention in their practices, but not with skills confidence or practice outcomes.

Motivators and barriers for rural community preceptors in teaching: A qualitative study

Abstract:Purpose: Increasing challenges in recruiting and retaining community-based teaching physicians (e.g., community preceptors) call for a better understanding of motivators and barriers community preceptors perceive in their teaching role. Given the importance of medical school partnerships with community-based sites for student training, it is essential to understand the perspectives of community preceptors as teaching physicians in a context away from the medical school, such as rural, and the factors affecting their career choice to engage in teaching while practising medicine. Methods: We conducted semi-structured interviews with rural community preceptors and used open coding to conceptualise data and axial coding to connect codes into categories. We used the socio-cognitive career theory framework to organise categories into themes. Results: Eleven rural community preceptors from two medical schools participated. Specialties included family medicine, internal medicine and paediatrics; clinical practice and teaching experience ranged from 3-36 and 2-29 years, respectively. Readiness for teaching ('self-efficacy') was pivotal in community preceptors' decision to teach and derived largely from vicarious learning from teaching attendings in medical school or residency; social persuasion and encouragement from clinical partners; and their accomplishments as practising physicians. However, limited faculty development, incomplete knowledge of expectations, disengagement from the medical school and lack of current mentors hindered their self-confidence. Teaching fulfilled their aspirations ('outcome expectations') to give back to the profession, but they felt undervalued and disconnected from other clinician educators. Teaching increased job satisfaction, but clinical workload, and financial impact impeded their goals for achieving excellence ('performance'). Conclusions: Self-efficacy was a pivotal motivator in rural community preceptors' decision to teach. Role models from early training inspired them to teach. Internal awards sustained teaching efforts. Future research should explore structural barriers influencing rural community preceptors' teaching experiences to better support their career choice to become medical educators.
Incorporating Geographic Information Systems (GIS) into program evaluation: lessons from a rural medicine initiative

Abstract:Purpose: To address the shortage of physicians practicing in rural areas of Michigan, the Wayne State University School of Medicine developed an integrated rural core curriculum to interest students in rural practice careers. Here we focus on the evaluation strategy used to determine the extent to which students in the new rural medicine interest group who self-identified as selecting a rural clerkship or externship did secure a clinical training experience in a rural setting. Methods: Three measures of rurality were compared to determine whether students were placed in rural training settings: (1) the percentage of the county living in rural areas; (2) a county-level dichotomous measure of rural/nonrural; and (3) a dichotomous measure based on urban area boundaries within the county. Practice address and geographic data were integrated into geographic information systems software, which we used to map out rural characteristics of Michigan counties through a process called thematic mapping; this shows characteristic variation by color-shading geographic features. In addition, reference maps were created showing the boundaries of urban areas and metropolitan/micropolitan areas. Once these processes were completed, we overlaid the practice location on the contextual-level geographic features to produce a visual representation of the relationship between student placement and rural areas throughout the state. Results: The outcome of student placement in rural practices varied by the definition of rural. We concluded that, although students were not placed in the most rural areas of Michigan, they received clerkship or externship training near rural areas or in semirural areas. Conclusion: This process evaluation had a direct impact on program management by highlighting gaps in preceptor recruitment. A greater effort is being made to recruit physicians for more rural areas of the state rather than urban and semirural areas. Geographic information systems mapping also defined levels of ruralism for students to help them make informed selections of training sties. This is especially important for students who are not sure about a rural experience and might be discouraged by placement in a remote rural area.
Enhancing Nursing Education through Mobile Health Clinics: Aligning AACN Core Competencies with Pediatric Clinical Experiences in Rural and Indigenous Communities

Abstract:Aim: This study provides insights into student perceptions of a hands-on, interprofessional pediatric clinical experience within Montanas tribal communities through the Mobile Health Training Program (MHTP). Specifically, it explores how the MHTP aligns with AACN core competencies and evaluates the impact of these practical experiences on nursing students competencies and readiness. Background: A well-prepared nursing workforce is crucial for addressing the healthcare needs of rural, underserved, and Indigenous communities in the United States. Montana's Indigenous communities face significant systemic economic and social challenges that limit access to healthcare services and contribute to a disproportionate disease burden. Over the past two years, the MHTP at a land grant university in the western U.S. conducted preschool health screening clinics for children aged 0-5 at four partner Tribal nations in Montana. Design: This qualitative study captures and analyzes the experiences and perceptions of undergraduate nursing students participating in the MHTP at a land grant university in the western U.S. Data were collected through post-MHTP clinic surveys administered through Qualtrics, after the completion of each 2-3 day MHTP clinic. Methods: A total of 160 nursing undergraduate nursing students completed post-clinical surveys between September 2022 and May 2024. Using inductive and deductive coding approaches, survey responses were analyzed qualitatively to identify themes related to rural healthcare, culturally responsive care, exposure to diverse communities, and professional growth. Results: The MHTP experience was found to be valuable by 95% of students. Four main themes emerged: insight into rural healthcare, practicing culturally responsive care, exposure to diverse communities, and professional growth. American Indian/Alaska Native student perspectives highlighted the need for culturally reflective opportunities for all students. The MHTP effectively aligned with AACN competencies, enhancing skills in patient-centered care, cultural competence and humility, interprofessional collaboration, and systems-based practice. Conclusion: The MHTP represents a contemporary and practical model in nursing education by integrating mobile, immersive, and patient-focused clinical experiences, while also providing students an opportunity to engage with high priority communities. Students valued working with rural and Indigenous populations, recognizing the critical need for culturally responsive care and professional development. This study contributes to the limited literature on rural and Tribal clinical placements, highlighting the importance of balancing simulation with direct clinical practice. The MHTP effectively equips nursing students to navigate the complexities of healthcare systems in rural and underserved settings and advancing nursing education to address workforce needs in these areas.

Clinical Education models in Rural Practice Settings: A Scoping Review

Abstract:Aim The objective of this review is to identify and synthesize the literature on clinical nursing education models in rural settings, with the goal of developing a better understanding of effective clinical education models suitable for rural nursing education. Background Clinical education is an integral part of nursing education, yet very little clinical education occurs in rural and remote areas. This leaves the rural landscape vulnerable to inadequate healthcare staffing because many graduates will begin their nursing practice in the geographical areas where they studied. The rural nursing workforce is currently insufficient to meet the health care needs of rural populations. This insufficiency is likely to worsen because statistical trends suggest that rural and remote communities might be among the worst affected by the global nursing shortage. Many new graduate nurses are ill prepared for rural nursing practice, primarily due to limited exposure to rural nursing content and rural clinical experiences in their entry-to-practice education. Increasing opportunities for nursing students to learn in rural clinical settings will likely support the recruitment and retention of nurses in rural practice. Despite the key role of rural nursing education in sustaining the rural health workforce, little is known about rural clinical learning in nursing education. Design A scoping review Methods This scoping review was conducted using the steps outlined by Arksey and O'Malley with updated methodological guidance from the Joanna Briggs Institute. Results Of 1880 potential data sources, 82 were included in data analysis. Although no distinct model of rural clinical nursing education could be identified, several features of clinical nursing education were described in the context of rural clinical learning. Preceptorship was the most used mentorship/clinical supervision model (n=41). The most common placement design was a block model design (n=41) lasting between one and 16 weeks, with most learners engaging in episodic care with individual clients (n=42). Interestingly, 24 sources reported learners engaging in rural clinical education across multiple contexts (e.g., acute care, home care, and primary care). Conclusion These findings suggest there exists great variability in rural clinical learning opportunities for nursing students. Academic practice partnerships could continue to be leveraged to provide learners with rural clinical education that meets the needs of both the educational institution as well as the rural communities in which learning takes place. An area of further research would be to focus on formal evaluation of the effectiveness of clinical education models in the rural context.

Student-centred photovoice as a mechanism for home-school interaction: Teacher perceptions of efficacy

Abstract:Objective: The school and home environments play a significant role in shaping the health behaviours of children. Understanding students’ home environments is essential for teachers to recognise and meet their students’ needs, while collaborative partnerships between the school and home have been shown to result in academic success and improved behaviour management. This study explores the unique features of photovoice as a student-centred approach to understanding the links between the school and home environments, and its feasibility to be implemented independently by teachers in the classroom. Design: Descriptive qualitative method. Setting: A Project Promoting healthy Living for Everyone in Schools (APPLE Schools) is a school-based health promotion project being conducted in 70 school communities across northern Alberta, Northwest Territories, and Manitoba, Canada. Method: One-on-one interviews with teachers who were involved in an initial photovoice project ( n = 3) and researcher field notes from observations conducted over a period of 8 months were used. Data were analysed using latent content analysis. Results: Strengths, limitations and future directions of photovoice were identified. The strengths of using photovoice included genuine student participation, strengthened communication between the school and home, and the ability to address multiple learning domains. Limitations were cost, privacy and parental support. Teachers shared promising ideas about photovoice being used for health promotion advocacy. Conclusion: Photovoice can be used by teachers as tool to strengthen the relationship between the home and the school environments. Future use of photovoice in schools is encouraged.

Photovoice in Aggression Management Training for Medical and Nursing Students—A Pilot Study

Abstract:Aggression towards medical staff in the healthcare workplace is a common global concern. Measures to mitigate consequences of patient aggression include training through Aggression Management Programs (AMPs), which have been shown to increase students' self-efficacy and self-confidence. To encourage better engagement with a 30 h required AMP training, the study piloted an adapted photovoice activity with 58 students of medicine and nursing. Each student took one to three photos depicting their perceptions, feelings, and experiences of patient aggression in the workplace and discussed them in a course session. Their photos showed types of aggression in psychiatric settings, and their consequences for patients and students. Photo strategies included showing 'actors' or toy figures in aggressive encounters; tools to control aggression in psychiatric settings (e.g., mechanical restraints and syringes); and symbolic photos showing violence to the heart (emotional impact). Adding photovoice elements to the established AMP training appeared to contribute to student reflection on their individual perspectives on patient aggression in the workplace and help students to link their subjective experiences and theoretical learning. In future, incorporating pre-test and post-test questionnaires measuring empathy, attitudes, or critical thinking could help to decipher any changes in AMP effectiveness due to the use of a self-directed photovoice activity.

Abstract:Introduction: Many health systems struggle in the provision of a sustainable and an efficient rural health workforce. There is evidence to suggest that Longitudinal Integrated Clerkships (LIC) placing student learners in rural community settings have positively impacted the provision of rural health care services The recruitment and engagement of students in rural LIC have significant challenges. This study explored best practice methods of recruiting and supporting the transition of medical students into rural LIC. Methods: The study took place during the 2021 Consortium of Longitudinal Integrated Clerkships Conference, a virtual event hosted by Stellenbosch University, South Africa. Participants consisted of delegates attending the Personally Arranged Learning Session (PeArLS) themed 'Secrets to success'. The session was recorded with the participants' consent and the recordings were transcribed verbatim. Data was uploaded to NVivo software and coded and analyzed using constant comparative analysis. Salient themes and patterns were identified. Results: Thirteen attendees participated in the PeArLS representing a range of countries and institutions. Strategically marketing the LIC brand, improving the LIC program profile within institutions by bridging logistics, and the need to scaffold the transition to the rural LIC learning environment emerged as key themes for success. The attendees highlighted their experiences of using peer groups, early exposure to rural LIC sites, and student allocation strategies for promotion. Unique learning styles adopted in LIC models, student anxiety and the importance of fostering supportive relationships with stakeholders to support students in their transition to the LIC environment were discussed. Discussion: This PeArLS highlighted successful systems and processes implemented in rural settings across different countries to recruit and manage the transition of medical students to rural LIC. The process proved to be a quick and efficient way to elicit rich information and may be of benefit to educationists seeking to establish similar programs or improve existing rural LIC.

Abstract:Introduction: Design thinking is a creative problem-solving framework that can be used to better understand challenges and generate solutions in health professions education, such as the barriers to rural education. Rural education experiences can benefit students, providers, and patients; however, placement in and maintenance of rural education experiences offer unique challenges. Design thinking offers strategies to explore and address these challenges. Methods: This study used a design thinking framework to identify barriers of student placement in rural locations; this was accomplished using strategies to empathize with users (eg, students, practitioners, and administrators) and define the problem. Data were collected from focus groups, interviews, and a design thinking workshop. Design activities promoted participant discussion by drawing pictures, discussing findings, and creating empathy maps of student experiences. Qualitative data were analyzed to identify salient barriers to rural experience selection and opportunities for support. Result: Focus group (n = 6), interview (n = 13), and workshop participants (n = 18) identified substantial advantages (eg, exposure to a wider variety of patients, less bureaucracy and constraints, more time with faculty) and disadvantages (eg, isolation, lack of housing, and commuting distances) of rural experiences. Participants identified physical, emotional, and social isolation as a significant barrier to student interest in and engagement in rural experiences. Workshop participants were able to generate over 100 ideas to address the most prominent theme of isolation. Discussion: Design thinking strategies can be used to explore health professions education challenges, such as placement in rural settings. Through engagement with students, practitioners, and administrators it was identified that physical, social, and emotional isolation presents a significant barrier to student placement in rural experiences. This perspective can inform support systems for students, preceptors, and communities that participate in rural educational experiences.

Abstract:Objectives: The Accreditation Council for Graduate Medical Education expects specialties to teach and assess proficiency in culturally competent care. However, little guidance has emerged to achieve these goals. Clinical training within socioeconomically disparate settings may provide an experiential learning opportunity. We sought to qualitatively explore resident experiences working in the generic clinical learning environments (i.e., exposure to socioeconomically diverse patients across different training sites) and how it shapes cultural competency-related skill development. Methods: Residents were recruited from emergency medicine (EM) programs. We used purposeful sampling across all postgraduate years and elicited experiences related to working at the different sites related to cultural identity, frustrating patient encounters, vulnerable populations, and development of health disparities/social determinants of health knowledge. Individual structured interviews were conducted via phone between May and December 2016. Interviews were audiotaped, transcribed, anonymized, and analyzed using systematic and iterative coding methods. Results: Twenty-four interviews revealed three main themes. EM residents' experiences caring for patients across sites shaped their understanding of: (1) potential patient attributes that affected the clinical encounter, (2) difficulties in building rapport had adverse effect on the clinical evaluation, and (3) residency program and training experiences shaped their clinical preparedness and willingness to work in underserved areas. Conclusion: Assessing the impact disparate clinical setting exposures have on trainees' preparedness to care for socioeconomically diverse patients can provide valuable insight for medical educators into barriers and facilitators to delivering optimal learning and patient care. Participants provided a breadth of stories illuminating their real-world consciousness and competency with meeting the needs of diverse populations and their access to varied educational outlets to grapple with the disparities they observed. More research is needed to uncover effective strategies to help residents thrive and feel more prepared to care for diverse populations.
Exploring learning characteristics and progression of GP trainees based in regional, rural and remote settings: A qualitative study

Abstract:Objective Globally, most doctors train and work in metropolitan areas but a key strategy for developing the rural health workforce is expanding rural training. The aim of this study was to describe the scope and quality of learning along with skill acquisition of GP trainees based in regional, rural, and remote settings. Setting Regional, rural and remote settings in Queensland Australia excluding Brisbane. Participants Thirty‐seven general practice trainees who had undertaken their first community placement were recruited from regional, rural and remote learning settings within Queensland. Design Qualitative descriptive methodology based on constructionist epistemology was employed which allowed for the exploration of current GP trainees' experiences. Trainees were interviewed and data were thematically analysed as to the scope and quality of learning by the setting of training. Learning experiences were then mapped against the Dreyfus and Dreyfus model to explore skill acquisition. Results In terms of the scope and quality of learning, rural and remote trainees mainly focused on the diverse and unique (sometimes challenging) experiences their setting offered compared with regionally based trainees. Mapping of the trainee comments to the Dreyfus model of skill acquisition demonstrated that regardless of setting, equivalent learning occurred by GP trainees. Conclusions This exploratory study provides evidence that rural and remote learning may provide a more diverse and challenging experience. It suggests an equivalence of quality of education and skill acquisition across settings rural, regional and remote. This suggests that the training distribution policies may not disadvantage GP trainees and the scope and quality of more remote learning may increase uptake of remote placements.
