Abstract:Objective To determine the feasibility of ex vivo liposome-mediated gene transfer ( Lac-Z gene ) to lung isografts and to get expression. Methods An orthotopic rat left lung transplant model was developed with the use of a modification of the “cuff” technique. 18 SD rats were divided into three groups randomly. Gene transfer group ( group 1 ): PSV-β-gal/ liposome ( Lipofectin R○ ) complex was transfered into the donor left lung via the left pulmonary vein route at the time of lung harvesting. Liposome group ( group 2 ) and Control group ( group 3 ) : Grafts underwent the same procedure but received liposome and 0 9% saline solution respectively. Donor lungs were harvested one day after transplantation. The transgene expression of Lac-Z gene was detected by histochemical staining. Results Transgene expression of β-gal in group 1 was detected in bronchial epithelial cells and alveolar epithelial cells. But no expression of β-gal was found in groups 2 and 3. Conclusion Ex vivo plasmid vector/liposome complex-mediated gene transfer to lung isografts via the left pulmonary vein route is feasible. This study provides the scientific proof to the feasibility of functional gene therapy of complications such as ischemia-reperfusion injury, rejection and infection after rat lung transplantation.
Intravenous aprotinin attenuates lung ischemia-reperfusion injury in a rat lung transplantation model

Abstract:Objective To evaluate the effect of intravenous aprotinin application on ischemiareperfusion iniury after lung trailsplantation.Methods An isehemia-reperfusion model after lung transplantation in rats was established and received intravenous aprotinin.Cytokine levels and myeloperoxidase activity were measured in order to explore the mechanism.Rtsults Intravenous aprotinin attenuated ischemia-reperfusion injury estimated by blood gas and wet/dry ratio.The levels of IL-2 in bronchoalveolar lavage[(113±32)μg/L vs(162±43)μg/L,P＜0.05],serum levels of IL-8[(7.26±1.01)ng/Lvs(9.43±0.97)ng/L,P＜0.05]and TNF-α[(152.3±36.4)ng/L vs(211.6±52.7)ns/L,P＜0.05],and lung tissue myelopcroxidase activity[(2.36±0.62)U/g vs(3.98±0.36)U/g,P＜0.05]were significantly reduced in the aprotinin group.Conclusion Apmtinin attenuates lung ischemia-reperfusion iniury after lung transplantation by lessen the release of IL-2,inhibiting the activation of TNF-α and production of IL-8.and suppressing the accumulation,activation and deganulation of neutrophils.
L-Alanyl-L-Glutamine Alleviated Ischemia-Reperfusion Injury and Primary Graft Dysfunction in Rat Lung Transplants

Abstract:Background: Concern of ischemia-reperfusion injury reduces utilization of donor lungs. We hypothesized adding L-alanyl-L-glutamine (L-AG) to preservation solution may protect donor lungs from ischemia-reperfusion injury through its multiple cytoprotective effects. Methods: A lung transplantation cell culture model was used on human lung epithelial cells and pulmonary microvascular endothelial cells, and the effects of adding different concentrations of L-AG on basic cellular function were tested. Rat donor lungs were preserved at 4 °C with 8 mmol/L L-AG for 12 h followed by 4 h reperfusion or monitored for 3 d. Lung function, lung histology, inflammation, and cell death biomarker were tested. Computerized tomography scan was used and metabolomic analysis was performed on lung tissues. Results: Cold preservation with L-AG improved cell viability and inhibited apoptosis in cell culture. Rat donor lungs treated with L-AG during cold storage showed decreased peak airway pressure, higher dynamic compliance and oxygenation ability, reduced lung injury, apoptosis, and oxidative stress during reperfusion. L-AG treatment significantly changed 130 metabolites during reperfusion, with enhanced amino acid biosynthesis and tricarboxylic acid cycle. Furthermore, cold storage with L-AG decreased primary graft dysfunction grade, improved oxygenation, reduced pulmonary atelectasis, sign of infection, and pneumothorax in a rat left lung transplant 3-d survival model. Conclusions: Adding L-AG to cold preservation solution reduced lung injury and alleviated primary graft dysfunction by inhibiting inflammation, oxidative stress, and cell death with modified metabolic activities.
MG53 mitigates warm ischemic lung injury in a murine model of transplantation

Abstract:Objectives: Lung transplant warm ischemia-reperfusion injury (IRI) results in cellular injury, inflammation, and poor graft function. Mitsugumin 53 (MG53) is an endogenous protein with cell membrane repair properties and the ability to modulate the inflammasome. We hypothesize that the absence of circulating MG53 protein in the recipient increases IRI, and higher levels of circulating MG53 protein mitigate IRI associated with lung transplantation. Methods: To demonstrate protection, wild-type (wt) lung donor allografts were transplanted into a wt background, a MG53 knockout (mg53-/-), or a constitutively overexpressed MG53 (tissue plasminogen activator-MG53) recipient mouse after 1 hour of warm ischemic injury. Mice survived for 5 days after transplantation. Bronchioalveolar lavage, serum, and tissue were collected at sacrifice. Bronchioalveolar lavage, serum, and tissue markers of apoptosis and a biometric profile of lung health were analyzed. Results: mg53-/- mice had significantly greater levels of markers of overall cell lysis and endothelial cell injury. Overexpression of MG53 resulted in a signature similar to that of wt controls. At the time of explant, tissue plasminogen activator-MG53 recipient tissue expressed significantly greater levels of MG53, measured by immunohistochemistry, compared with mg53-/-, demonstrating uptake of endogenous overexpressed MG53 into donor tissue. Conclusions: In a warm IRI model of lung transplantation, the absence of MG53 resulted in increased cell injury and inflammation. Endogenous overexpression of MG53 in the recipient results in protection in the wt donor. Together, these data suggest that MG53 is a potential therapeutic agent for use in lung transplantation to mitigate IRI.
FR167653 ameliorates pulmonary damage in ischemia-reperfusion injury in a canine lung transplantation model

Abstract:Background: Interleukin (IL)-1 and tumor necrosis factor-alpha (TNF-alpha) are recognized as important factors in ischemia-reperfusion (I/R) injury. FR167653 has been characterized as a potent suppressant of IL-1 and TNF-alpha production. We previously reported that FR167653 suppressed the expression of IL-1 beta mRNA after reperfusion and ameliorated pulmonary I/R injury following 3-hour left lung warm ischemia in dogs. The aim of this study was to investigate the effects of FR167653 on I/R injury in a canine left, single, lung transplantation model. Methods: We used 10 pairs of weight-matched dogs. We assigned 5 pairs to the FR group, in which each animal received FR167653 (1 mg/kg/hr) IV from 30 minutes before ischemia until 2 hours after reperfusion; we treated the transplanted lungs with FR167653 after the onset of reperfusion. The others were assigned to the control group. After 8-hour preservation with 4 degrees C Euro-Collins solution, orthotopic left, single, lung transplantation was performed. During a 5-minute clamping test at the right pulmonary artery of each recipient, the left (transplanted) pulmonary arterial pressure (L-PAP), left (transplanted) pulmonary vascular resistance (L-PVR), arterial oxygen pressure (PaO(2)), and alveolar-arterial oxygen pressure difference (A-aDO(2)) were measured. We harvested transplanted lung specimens for histologic study, and we counted polymorphonuclear neutrophils (PMNs), which were identified by staining with naphthol AS-D cholroacetate esterase. Pulmonary perfusion and ventilation scintigraphy (Tc-99m-MAA and Xe-133) were performed. We observed the animals for 3 days after transplantation. Results: The PAP, L-PVR, PaO(2), and A-aDO(2) revealed significantly (p < 0.05) better function in the FR group than in the control group. Histologically, lung edema was milder, and PMN infiltration was significantly (p < 0.05) lower in the FR group than in the control group. Xe-133 and Tc-99m-MAA were widely distributed throughout the graft lung in the FR group. Three-day survival rates in FR and control groups were 60% and 20%, respectively. Conclusions: FR167653 appears to generate a protective effect on I/R injury in lung transplantation in dogs.

Abstract:Considerable evidence indicates that peripheral nerve fibres play an active regulatory role during eczematous reactions. Many basic features of the inflammatory process are potentially subject to neuropeptidergic modulation, such as vascular changes, cellular trafficking, activation and trophism, In vivo and in vitro investigations suggest that neuropeptides are part of the complex network of mediators that initiate and maintain the eczematous process in the skin. In general, peptidergic fibres seem to exert a global protective role, but with specific and selective effects in the different phases. Evidence that the inflammatory process induces biochemical changes at a neuronal level has also been obtained. The possible mediators of the neuronal recruitment in the course of the eczematous reaction are still unknown. Nevertheless, nerve growth factor, which is locally increased during inflammatory processes and may induce peptidergic alterations, should be considered as a potential candidate molecule.
Early Protection by Resveratrol in Rat Lung Transplantation

Abstract:BACKGROUND Resveratrol is a multifunctional bioactive substance that has effects in anti-inflammation and prevention of ischemia-reperfusion injury. This study compared the inflammation and expression of related proteins during the early stages after transplantation to explore the effects and mechanisms of resveratrol on transplanted lung. MATERIAL AND METHODS Sprague-Dawley rats were randomized to receive pretreatment of resveratrol suspension (60 mg/kg; RES group), dexamethasone (1 mg/kg; DEM group), or normal saline solution (2 mL/kg; control group) 1 h before lung transplantation. The cytokine concentration in the serum and bronchoalveolar lavage fluid (BALF) of the recipients was determined 24 h after transplantation. Histopathologic evaluation, including lung injury score, and the expression of necroptosis-associated proteins was assessed. RESULTS Histopathologic evaluation showed pneumocyte damage and endothelialitis associated with hemorrhage in the alveoli in the control group, the severity of which was greater than that in the other 2 groups. The levels of interleukin-6 and tumor necrosis factor-a in the serum and BALF of the RES and DEM groups were lower than those in the control group. The expression of necroptosis-associated proteins in the RES group was lower than that in the control group, and was inversely proportional to lung injury. CONCLUSIONS Pretreatment with resveratrol protected rat lung in the early stages after transplantation. We determined a relationship between necroptosis-associated proteins and transplanted lung injury, which suggests that the mechanism of lung transplantation-associated ischemia-reperfusion injury may be related to necroptosis.

Abstract:Papillary and surface micropuncture in Munich-Wistar rats was used to assess the role of proximal segments of superficial and juxtamedullary (JM) nephrons, the distal tubule of superficial nephrons, and the terminal collecting duct in acid excretion. The relative role of these segments in ammonium production, bicarbonate reclamation, and net acid formation was assessed under hydropenic conditions and after a chronic acid load. In these two settings the proximal segment of both kinds of nephrons is the major site of ammonium production and bicarbonate reclamation. However, this segment's contribution to net acid formation was only significant during acidosis. On the other hand, segments beyond the distal tubule appear to be the major site of acid formation. In situ pH measurements were lower in these nephron segments and fell even more after the induction of an acidosis. Ammonia appears to enter fluid between the end of the distal tubule and the base of the collecting duct. In vivo pH measurements made near the bend of Henle's loop of JM nephrons were more alkaline than near the end of the proximal tubule of superficial nephrons. It is postulated that this difference in pH allows ammonium to dissociate, permitting the movement of ammonia out of the tubule lumen and into collecting duct fluid where it is protonated and, therefore, reentrapped. This process is enhanced by the ingestion of a chronic acid load.

Comparation of the Effects of Ulinastatin and N-desulfated Heparin on Ischemia-reperfusion Injury after Lung Transplantation in Rats

Abstract:Objective: To build an orthotopic single left lung allograft animal model in rats and compare the effects of ulinastatin (UTI) and non-anticoagulant N-desulfated heparin (NNH) on lung ischemia-reperfusion injury after lung transplantation. Methods: 24 Male SD rats weighted 250-350g were randomly divided into 3 groups. At the time of reperfusion, the control group were given saline 1. 2ml/kg (n = 8); the UTI group were given UTI 10,000 U/kg (n=8); the NNH group were given NNH 12mg/kg , we completed 24 rat left lung transplantation assisted by mechanical ventilation and microscopy. 2 hours after reperfusion, the transplanted lung's pulmonary vein blood gases, wet dry ratio(W/D) and myeloperoxidase(MPO) were measured. Tumor necrosis factor-α(TNF-α) and interleukin 8(IL-8) were measured by method of ELISA. Intercilluar adhesion molecule-1( IC AM-1) mRNA in lung tissue were measured by method of RT-PCR, pathology slice were done routinely. Results: Among 24 transplanted rats, 21 survived at the end point, the transplantation success rate was 87. 5% and the procedure success rate was 100%. Among the 3 failure cases, 2 died in 75 minutes and 90 minutes after reperfusion respectively. Another one was still alive, however, thrombus was found in the pulmonary artery and vein at autopsy, thus the transplanted lung was not perfused. PaO2 in the UTI group(289. 68±5. 65mmHg) and the NNH group(315. 26 ± 14. 91mmHg) was significantly higher (P0.0001) than the control group (236. 80 ±13. 90mmHg ), and the NNH group was singnificantly higher than the UTI group(P 0.0001). W/D ratio has no difference(P=0.18) within the UTI group(4.74 ±0. 28) ,the NNH group (4. 87 ±0. 34) and the control group (5. 00 ± 0. 12), MPO activity in the UTI group(0. 52 ± 0. 25) was lower than the control group(0. 84±0. 22, P0.05), and the difference within the NNH group(0. 64 ± 0. 24), the control group and the UTI group was not apparent(P=0. 12, P=0. 29). In the lung tissue, the concentrations of TNF-a in the UTI group (138. 61 ± 15. 30) were significantly higher than the control group(77. 54 ± 10.14, P0. 0001), the NNH group(52. 85 ± 6. 55) was significantly lower than the control group(P0.0001). In the lung tissue, the concentrations of IL-8 in UTI group(173. 45± 27. 63) were significantly higher than the control group(76. 22 ?12. 42, P0.0001) and the NNH group(61. 34 ± 9. 86,P0. 0001), and there was no difference between the control group and the NNH group(P=0. 12). ICAM-1 mRNA in the UTI group(0. 37 ± 0. 14) and the NNH group(0. 38 ± 0. 11 )5. 00 ± 0. 12, P0. 01 and P0. 0001) were significantly lower than the control group(1. 74 ±0. 39,P0. 0001), and there was no difference between the UTI group and the NNH group(P=0. 94). Histological appearances showed less lung tissue damage in the UTI and NNH group. Conclusion: Rat left lung transplantation model with non-vessel endo-thelium cuff technique is easy to do and acceptable for short period research. UTI and NNH both prevented lung reperfusion injury after lung transplantation in rats. UTI can significantly reduce oxygen derived free radicals. NNH depressed the expression of proinflammatory fator and improved the function of transplanted lung, and have more significantly effect on preventing the lung reperfusion injury after lung transplantation in rats.
Transient receptor potential vanilloid 4 channel inhibition attenuates lung ischemia-reperfusion injury in a porcine lung transplant model

Abstract:Objective Transient receptor potential vanilloid 4 (TRPV4) is a nonselective cation channel important in many physiological and pathophysiological processes, including pulmonary disease. Using a murine model, we previously demonstrated that TRPV4 mediates lung ischemia-reperfusion injury, the major cause of primary graft dysfunction after transplant. The current study tests the hypothesis that treatment with a TRPV4 inhibitor will attenuate lung ischemia-reperfusion injury in a clinically relevant porcine lung transplant model. Methods A porcine left-lung transplant model was used. Animals were randomized to 2 treatment groups (n = 5/group): vehicle or GSK2193874 (selective TRPV4 inhibitor). Donor lungs underwent 30 minutes of warm ischemia and 24 hours of cold preservation before left lung allotransplantation and 4 hours of reperfusion. Vehicle or GSK2193874 (1 mg/kg) was administered to the recipient as a systemic infusion after recipient lung explant. Lung function, injury, and inflammatory biomarkers were compared. Results After transplant, left lung oxygenation was significantly improved in the TRPV4 inhibitor group after 3 and 4 hours of reperfusion. Lung histology scores and edema were significantly improved, and neutrophil infiltration was significantly reduced in the TRPV4 inhibitor group. TRPV4 inhibitor-treated recipients had significantly reduced expression of interleukin-8, high mobility group box 1, P-selectin, and tight junction proteins (occludin, claudin-5, and zonula occludens-1) in bronchoalveolar lavage fluid as well as reduced angiopoietin-2 in plasma, all indicative of preservation of endothelial barrier function. Conclusions Treatment of lung transplant recipients with TRPV4 inhibitor significantly improves lung function and attenuates ischemia-reperfusion injury. Thus, selective TRPV4 inhibition may be a promising therapeutic strategy to prevent primary graft dysfunction after transplant.

Protective Effects of Ulinastatin on Ischemia-Reperfusion Iiyury During Rat Non-Heart Beating Donor Lung Transplantation

Abstract:Objective To investigate the protective effects of ulinastatin (UTI) on ischemiareperfusion injury of donor lungs,and the possible mechanism.Method Forty male SD rats were randomly divided into two groups:group C as control group and group U as UTI group.In group C donor lungs were antegradely flushed with 20 ml of cold (4 C) low potassium dextron (LPD) solution and 5 ml retrogradely.Meanwhile,in group U,UTI (500 000 U/L) was added in LPD solution and the same doses were used.According to the time after initiation of reperfusion,each group was divided into two subgroups:30 min (subgroup A) and 1 h (subgroup B).Arterial blood samples were collected for blood gas analysis.Lung samples were obtained at the end of reperfusion (30 min or 1 h).Microscopic examination of the donor lungs was conducted.Besides,the pulmonary water index (W/D),tissue malondialdehyde (MDA) and superoxide dismutase (SOD) content,and mRNA expression of tumor necrosis factor (TNF-a),intercellular adhesion molecule 1 (ICAM-1) and interleukin 10 (IL-10) were also measured.Results (1) One h after reperfusion,oxygenation index in group U was higher than that in group C (P =0.025) ; (2) The levels of W/D in subgroup A and subgroup B of group U were decreased as compared with group C (P =0.005 and P =0.006) ; (3)The microscopic changes of donor lung tissues in group U were lessen than in group C; (4) In subgroup A of group U,MDA content was decreased (P=0.039),and SOD content was increased (P=0.035),and similar results could be observed in subgroup B of group U (P =0.006 and P =0.030 respectively); (5) As compared with group C,the mRNA expression of TNF-α in group U was decreased at the time of 30 min after reperfusion (P =0.000),but no significant change was found at the time of 1 h (P =0.139).The mRNA expression of ICAM-1 was not decreased evidently at the time of 30 min (P=0.062),but significantly decreased at the time of 1 h (P=0.001).The mRNA expression of IL-10 was increased in subgroups A and B (P =0.004 and P =0.000 respectively).Conclusion This study demonstrated that UTI had protective effects of reducing ischemia-reperfusion injury on the donor lungs after lung transplantation in rat non-heart beating donor models.

Abstract:Purpose: Lung ischemia-reperfusion injury (LIRI) after lung transplantation can lead to primary graft dysfunction. Budesonide can improve endothelial function to reduce lung injury. This study was aimed to examine the effects of budesonide on LIRI and potential mechanisms. Methods: Wistar rats were randomized and transplanted with syngeneic left lung or received the sham surgery. The recipients were instilled with saline or budesonide immediately after reperfusion. The mean arterial pressure (MAP), blood gas, and lung histology were analyzed. The ratios of wet to dry lung weights, the levels of total proteins, tumor necrosis factor (TNF)-α, interleukin (IL)-1β, IL-6, and IL-10, and neutrophil elastase in bronchoalveolar lavage fluid (BALF) were measured. The levels of malondialdehyde (MDA), myeloperoxidase (MPO), and xanthine oxidase (XO) in the lung, and the levels of plasma lymphocyte function-associated antigen (LFA)-1 and P-selectin were determined. Results: Compared with the saline group, treatment with budesonide significantly increased blood PaO2, but reduced PaCO2, and mitigated lung damages after reperfusion, the levels of BALF proteins, and the ratios of wet to dry lung weights in rats. Furthermore, treatment with budesonide significantly decreased the levels of MDA, MPO, and XO in the lung and the levels of TNF-α, IL-1β, IL-6, and neutrophil elastase, but increased IL-10 in the BALF, accompanied by significantly reduced levels of serum P-selectin and LFA-1 in rats. Conclusions: Budesonide effectively mitigated LIRI and ameliorated the lung function by attenuating oxidative stress and inflammation following syngeneic lung transplantation.

Abstract:Lung ischaemia reperfusion injury (LIRI) is the major cause of primary lung dysfunction after lung transplantation. Lipoxin A4 inhibits the oxidative stress and inflammation. This study aimed to evaluate the potential protective effect of lipoxin A4 on LIRI in rats.SD (Sprague-Dawley) rats were randomised into the sham, LIRI and LA4 groups. Rats in the sham group received anaesthesia, thoracotomy and intravenous injection of saline, while those in the LIRI or LA4 group received left lung transplantation and intravenous injection of saline or lipoxin A4, respectively. After 24 h of reperfusion, the PaO2/FiO2 (Partial pressure of O2 to fraction inspiratory O2), wet/dry weight ratios and protein levels in lungs were measured to assess the alveolar capillary permeability. The oxidative stress response and inflammation were examined. The histological and apoptosis analyses of lung tissues were performed via HE staining (Haematoxylin-eosin staining) and TUNEL assay, respectively. The effects of lipoxin A4 on the endothelial viability and tube formation of hypoxaemia and reoxygenation-challenged rat pulmonary microvascular endothelium cells were determined.Lipoxin A4 significantly ameliorated the alveolar capillary permeability, reduced the oxidative stress and inflammation in transplanted lungs. The histological injury and apoptosis of lung tissues were also alleviated by lipoxin A4. In vitro lipoxin A4 treatment promoted the endothelial tube formation and improved the endothelial viability.Lipoxin A4 protects LIRI after lung transplantation in rats, and its therapeutic effect is associated with the properties of anti-inflammation, anti-oxidation, and endothelium protection.Lung transplantation is a treatment approach for the patients with lung disease.LIRI is the major cause of postoperative primary lung dysfunction.Lipoxins A4 exhibits strong anti-inflammatory properties.

Abstract:Objective:To investigate the treatment effects of Sivelestat Sodium (SS) on lung function of ischemia-reperfusion injury after rat left unilateral lung orthotopic transplantation.Methods:Rats were subjected to unilateral left lung orthotopic transplantation. A total of 60 male Sprague-Dawley rats were randomly divided two groups,each group was further subdivided into sham,control,SS group (n=6,per subgroup),animas were sacrificed at different reperfusion periods of 2 h and 24 h,respectively. Oxygenation indexes,lung injury scores,wet/dry ratio,myeloperoxidase (MPO) activity,Malondialdehyde (MDA) and cytokine-induced neutrophil chemotactic factor (CINC) in the transplanted lung and serum were measured. Results:The values were 388,179 and 265 for PO2/FiO2; 3.2,6.5 and 5.0 for lung injury pathology score; 3.5,5.6 and 5.2 for wet-to-dry ratio; 2.3,5.2,4.4 for MPO activity,0.8,2.6,1.6 for MDA activity in lung tissue,2.3,8.1,5.2 for MDA in serum,26.4,114.4,19.9 for CINC activity (Sham,control,and SS groups,respectively) at 2 h reperfusion. After 24 h reperfusion,these values were 403,155 and 293 for PO2/FiO2; 3.7,11.0 and 6.7 for lung injury pathology score; 3.7,6.4 and 5.2 for wet-to-dry ratio; 2,3,7.2,5.0 for MPO activity; 0.7,1.5,1.2 for MDA activity in lung tissue,2.5,5.5,4.6 for MDA in serum,29.3,43.1,32.1 for CINC activity. Our data support that IR causes significant lung injury,which can be reduced by SS treatment.Conclusion:Administration of SS attenuated IR injury by inhibiting oxidation,inflammation and neutrophil priming after rat unilateral left lung orthotopic transplantation.

Abstract:Background: The preventive effect of inhaled nitric oxide (NO) and pentoxifylline (PTX) administered during reperfusion has been demonstrated on experimental models of lung ischemia/reperfusion (I/R) injury but this strategy is not validated in clinical lung transplantation. The aim of this study was to assess retrospectively the protective effect of inhaled NO and PTX after lung transplantation. Methods: Twenty-three consecutive patients who received inhaled NO (10 ppm) and PTX (NO-PTX group) at the time of reperfusion were compared retrospectively with (1) 23 consecutive patients transplanted just before the use of NO-PTX (control group 23); (2) 95 patients representing all the patients of the series who did not receive NO-PTX (control group 95), with respect to I/R injury related complications. In particular, the incidence of pulmonary reimplantation edema and early hemodynamic failure, the PaO2/FIO2 ratio as well as the duration of mechanical ventilation and the 2-month mortality rates were compared. Results: Reimplantation edema was observed in 6/23 patients (26%) in the NO-PTX group vs. 13/23 patients (56%) in the control group 23 (P=0.035) and 48/95 patients (50%) in the control group 95 (P=0.035). The worst PaO2/FIO2 ratio during the first three postoperative days was 240-102 mmHg in the NO-PTX group vs. 162+/-88 mmHg (P=0.01) and 176+/-107 mmHg (P=0.01) in the control group 23 and the control group 95, respectively. The duration of mechanical ventilation was 2.1+/-2.4 days in the NO-PTX group vs. 7+/-9 days in the control group 23 (P=0.02) and 6+/-7 days in the control group 95 (P=0.01). The 2-month mortality rate was 4.3% in the NO-PTX group vs. 26% (P=0.04) and 21% (P=0.07) in the control group 23 and the control group 95, respectively. Conclusions: The marked decrease in the incidence of allograft dysfunction compared with two historical control groups suggests that PTX and inhaled NO given before and throughout reperfusion are protective against I/R injury in the setting of clinical transplantation.

Abstract:OBJECTIVE:To investigate the effects of ginaton against ischemia-reperfusion injury on the autograft after lung autotransplantation.METHODS:Models of lung autotransplantation were established in 18 New Zealand rabbits were established. The 18 rabbits were randomly divided into 3 equal groups: group of simple ischemia-reperfusion (group I/R), undergoing ischemia by blocking the left pulmonary artery for 2 h and then re-perfusion for 90 min; group with perfusion of low potassium dextran solution (group LPD), undergoing perfusion of LSD solution before ischemia; and group with treatment of ginaton (group LPD + E), undergoing intravenous injection of ginaton 15 min before ischemia. Arterial blood samples were collected before ischemia, and 15, 60, and 90 min after re-perfusion to examine the alveolar oxygen pressure (PaO2). Serum tumor necrosis factor-alpha (TNF-alpha) was monitored before ischemia, and 30, 60, and 90 min after re-perfusion. Then the left lungs were taken out to undergo detection of dry/wet ratio (D/W), pathological examination, and contents of myeloperoxidase (MPO) and the malondialdehyde (MDA) in the lung tissues.RESULTS:(1) The PaO2 decreased significantly after reperfusion in all groups. And the PaO2 values at different time points of Group LPD + E were all significantly higher than those of Group I/R (all P < 0.01), however, there were no significant differences between Group LPD and Group LPD + E. (2) The TNF-alpha level after reperfusion increased in Group I/R and Group LPD, while in Group LPD + E it increased only 60 min and 90 min after the reperfusion. The TNF-alpha levels after reperfusion at all time points of Group LPD + E were all significantly lower than those of the other 2 groups (all P < 0.05). (3) The MPO and MDA levels at all time points after re-perfusion of Group LPD + E were all significantly lower than those of the other 2 groups (all P < 0.01). (4) The value of D/W ratio of Group LPD + E was significantly higher than those of the other 2 groups (both P < 0.01). (5) Pathological examination showed that the lung tissue lesion of Group I/R was severe. Interstitial inflammatory cell infiltration, intra-alveolar inflammatory cell aggregation, exudation and even hemorrhage could be observed. The pathological lesion of Group LPD + E was mild, no significant inflammatory cell infiltration or exudation was observed.CONCLUSION:Ginaton provides a protective effect against ischemia-reperfusion injury on the autograft after lung autotransplantation. The mechanism may be related with antioxidation, inhibition of neutrophil aggregation, and TNF-releasing.
