Abstract:Morphologic examination of 3 ears from 2 dogs with aural hematomas revealed that the hematomas had developed within the pinnal cartilage. The cartilage had fractured from perichondrium to perichondrium and in some areas clefts had formed within the cartilage. Cartilage fracture could have resulted from wave motions set up in the pinna during head shaking. An explosive burst of head shaking exceeding a critical head velocity or a more gentle but sustained head shaking establishing a sinusoidal wave motion within the ear could have caused cartilage fracture.

Abstract:Background: Otohematoma or aural hematoma in dogs is blood pooling within the ear cartilage plate, and stems from several causes (ear infections, ectoparasites and others). The formation of hematoma is by vascular injury, causing bleeding, generated by self-injury. This disease occurs especially in dogs of pendulous ears, and the concave surface of the ear is considered as the most frequent. The therapy to correct the lesion can be a clinical procedure; however, surgical drainage is the treatment of choice. This work aims to record the epidemiological aspects related to aural hematoma in dogs, as well as classify such patients as to the physical condition and anesthetic risk.Materials, Methods & Results: Twenty five dogs with aural hematoma submitted to surgical procedure were followed. During the data collection period (seven months), surgery to correct aural hematoma represented 19% of surgeries performed in dogs (total of surgeries: 474). The percentage of canine breeds affected was: mixed breed (64%; 16/25), german shepherd (24%; 6/25), labrador (4%; 1/25), waimaraner (4%; 1/25) and daschund (4%; 1/25). Most occurrences were observed in females aged 3-12 years. Most of the animals showed only one pina affected (96%; 24/25), and 52% (13/25) had hematoma in the right and 44% (11/25) in the left ear. Some of the patients had no hematologic abnormalities (48%; 12/25), but some animals showed normocytic normochromic anemia (36%, 9/25), leukocytosis (8%; 2/25) and thrombocytopenia (8%; 2/25). Among animals, there were only dogs classified as ASA II and ASA III with similar rates (56% and 44% respectively). No deaths were observed among cases analyzed.Discussion: The high prevalence of the disease in mongrel dogs can be explained by the great number of mongrel animals in the service analyzed. Aural hematoma was more frequent in older dogs probably because animals in this age group are more affected by otitis externa, which can predispose them to aural hematoma. Otohematoma was common in animals with pendulous ears, probably because these dogs are more prone to ear diseases than animals with erect ears. Females are more affected, and these data are different to those of other studies, and may be related to changes of the clientele among males and females. Only 4% (1/25) showed bilateral aural hematoma which, actually, is less common. Haematological abnormalities observed may be related to the appearance of aural hematoma, but were unspecific and common to several diseases. Animals classified as ASA II entered in this category for being carriers of mild haematological disorders, especially anemia (36%; 9/25). Dogs classified as ASA III showed signs of moderate systemic disease, and the main suspected disease was ehrlichiosis, which may be associated with aural hematoma. Animals underwent surgical procedure even though being classified as ASA II and III. Such procedure caused no detectable adverse effects and no deaths were recorded. It is expected that mortality is higher in patients classified in a higher grade. The absence of deaths can be related to ASA classification, which was only up to Grade III (moderate systemic disease). Moreover, surgery to drain aural hematoma is considered free of contamination, minimally invasive and without the involvement of important anatomical structures that could systemically compromise patients.

On the nature of canine aural haematoma and its treatment with continuous vacuum drainage

Abstract:Objectives: To evaluate continuous vacuum drainage from the convex side of the pinna for surgical treatment of aural haematoma in dogs. To investigate aural haematoma fluid and cartilage samples harvested during surgery in an attempt to elucidate the etiopathogenesis of the lesion. Materials and methods: Ten client-owned dogs with aural haematoma were treated by convex-side vacuum drainage using different types of vacuum drains and containers and were followed-up for at least 6 months. Systemic blood samples and aural haematoma fluids were analysed for biochemical, haematologic and electrophoretic content. Cartilage samples were examined histologically. Results: Nine of the 10 dogs were successfully treated without recurrence at 6 months postoperatively and with excellent cosmetic results. In one case, infection required early drain removal and delayed healing was associated with wrinkling of the pinna. Fluid sample analysis suggested that "aural haematoma" contains a transudate that accumulates within a cartilage-lined cavity. Histologic examination of the tissue samples revealed clefts of the pinna cartilage, with the luminal surface often lined with granulation tissue. Fluids and cartilage lacked evidence of inflammation. Clinical significance: This technique was characterised by patient comfort, tolerance of drains, absence of dressings and good cosmetic outcome. The results of fluid analysis suggest that the term "aural seroma" would be more appropriate for this condition.

Abstract:The persistence of the hepatitis B virus surface antigen (HBsAg) was used as an index of the survival time of this virus within the gastro-intestinal tract of the potential southern African medicinal leech, Asiaticobdella buntonensis. HBsAg was tested for in blood/faecal material at five intervals over 15 weeks. Samples from both the midgut and the rectum remained positive for the entire test period, although with decreasing strength. The results are compared with reports on other arthropods which indicate increasing antigen persistence with increasing body size. The findings implicate medicinal leeches as mechanical vectors of HBV and possibly of other medically important viruses, and argue against using leeches of suspect or unknown origin in the alleviation of venous congestion in failing microsurgical procedures.

Abstract:In H. pylori, we examined the composition of total lipids. In thin-layer chromatography, three kinds of glycolipids were detected, which were named as G-1, G-2 and G-3. G-3 contained phosphates in addition to sugars. Total lipids consisted of 6.0% neutral lipids (weight %), 20.6% glycolipids (G-1, G-2) and 73.4% phospholipids (including G-3). Total glycolipids were approximately 25% of total lipids. After methanolysis, cholesterol was detected in the hexane extractable fraction and glucose was detected by GLC in the methanol fraction after trimethylsilylation, in all glycolipids. Therefore, these glycolipids are cholesteryl glycosides. Cholesteryl glycosides are found very rarely in bacteria, and have been detected only in a few species of mycoplasmas and spirochetes.

Abstract:Ear disease, such as otitis externa, resulting in aggressive head shaking or ear scratching, is the most common cause of the development of aural hematomas in dogs and cats. An underlying immunologic cause has also been proposed to explain cartilage and blood vessel fragility. Numerous options exist for management of aural hematomas, from medical management alone with corticosteroids, to simple hematoma centesis, to surgical intervention. Because this condition is usually secondary to another disease process, regardless of mode of treatment, likelihood of recurrence is low if the underlying condition is managed properly.
Canine and feline aural hematoma: clinical, experimental, and clinicopathologic observations

Abstract:The pathogenesis of canine and feline aural hematoma (AH) was investigated. Clinical observations, selected experimental procedures, and clinicopathologic examinations were done on 40 dogs and 20 cats affected with AH. Eighteen healthy dogs and 14 healthy cats were used as the controls. The results of this investigation provide a valid basis for questioning the conventionally held view that AH is caused by trauma and for postulating that the actual cause is immune mediated.

Abstract:Background: Aural hematomas are not uncommon in ruminants' clinical practice; however, there is a lack of information regarding their management in newborn ruminants, especially for complicated cases with rupture of the hematoma and secondary ear pinna necrosis. Case description: A 3-day-old orphan goat kid was admitted due to swelling on the left ear pinna and trauma on the right pina caused by biting by other goats. The swelling on the left ear which was located at the convex surface was soft, painless, and fluid-filled, suggestive of aural hematoma located at the convex surface. The right pinna was swollen, bleeding, and extremely painful at palpation. The skin was necrotized at the distal 2/3rd of the convex surface and the 1/2 of the concave surface. Underneath the necrotized skin of the convex surface, blood, and blood clots were trapped, and there was a pocket between the remaining normal skin and the cartilage indicating possible rupture of aural hematoma. The kid was surgically treated under general anesthesia with xylazine and ketamine. The aural hematoma was drained by the convex surface using a Penrose tube after flushing the cavity with 2 mg dexamethasone. The trauma of the right pinna was left to heal by secondary intention after resection of all necrotized, edematous tissues, and blood clots. Post-surgery, the animal was treated with parenteral antibiotic administration and daily application of a topical antiseptic solution. The Penrose tube was removed after 5 days, and the animal recovered uneventfully. Conclusions: The present case indicates that aural hematoma can occur in newborn goat kids secondary to ear pinna biting and might evolve to pinna necrosis. In addition, tube drainage after flushing the cavity with corticosteroids appears to be an effective treatment approach without requiring bandaging post-operatively.

Abstract:Energy independence, reduction of CO2 emissions and increasing oil prices are the key drivers for the development of new transportation technologies, among which the Battery Electric Vehicles. Instead of representing merely a load, they can provide ancillary services to the power grid: such integration of vehicles into the grid is known as V2G. The continuous charging and discharging of the batteries to provide services to the grid tends to degrade the vehicle batteries, decreasing their maximum life. Therefore it is of interest developing a management strategy that tends to maximize the life of the batteries. To this end it is of fundamental importance being able to assess the state of health of a battery. In this paper a methodology for the measurement of the health is described, and the first experimental results are provided.

Abstract:Porcine ear necrosis (PEN) is characterized by ulcerative lesions of the ear auricle. To investigate that problem, three farms with PEN in nursery pigs were included, and the study aim was to characterize PEN and the potential role of pathogens and mycotoxins. Within each farm, one batch of weaned piglets was included and the prevalence and severity of PEN were monitored for 6-7 weeks. Within each batch, 30 PEN-affected/non-affected animals were randomly selected. Blood samples were taken from these animals, to assess the systemic presence of pathogens and mycotoxins, as well as punch biopsies from the ear auricle for histopathological examination. From 10 animals, scrapings and swabs from the lesions were subjected to nanopore metagenomic sequencing and bacteriological cultivation, respectively. In all three farms, lesions appeared within 3-4 weeks post-weaning. The prevalence at the end of the nursery was 33%, 24%, and 46% for farms A, B, and C, respectively. Most affected pigs had mild to moderate lesions. Blood samples revealed low to very low levels of pathogens and mycotoxins. Different bacteria such as Staphylococcus, Streptococcus, Fusobacterium, Mycoplasma, and Clostridium species were identified by sequencing in the scrapings. The first two pathogens were also most often identified in bacterial cultures. Mycoplasma hyopharyngis was only found in PEN-affected pigs. Histopathological changes were primarily observed in the outer layer of the epidermis. The results suggest that PEN lesions develop by damage to the outer part of the skin e.g. by ear suckling or biting, followed by multiplication of opportunistic pathogens.
Reporting the epidemiology of aural haematoma in dogs and proposing a novel aetiopathogenetic pathway

Abstract:To evaluate the incidence and risk factors for aural haematoma in dogs under primary veterinary care in the UK. A cohort study design. Dogs diagnosed with aural haematoma during 2016 were identified from the VetCompass database. Univariable and multivariable logistic regression modelling were used for risk factor analysis. There were 2,249/905,554 dogs diagnosed with aural haematoma during 2016. The estimated one-year incidence risk for aural haematoma was 0.25% (95% confidence interval 0.24-0.26). After accounting for confounding factors, 14 breeds showed increased odds and 20 breeds showed reduced odds of aural haematoma compared with crossbred dogs. Breeds with the highest odds included Bull Terrier (OR 7.42, 95% confidence interval 4.39-12.54), Saint Bernard (OR 7.28, 95% confidence interval 3.58-14.81) and French Bulldog (OR 6.95, 95% confidence interval 5.55-8.70). Increasing age, increasing bodyweight and breeds with V-shaped drop and semi-erect ear carriage also showed increased odds of aural haematoma. Associations between ear carriage within breeds and the risk of aural haematoma suggest that trauma along the line of cartilage folding within V-shaped and semi-erect ears may trigger aural haematoma. New knowledge of key breed predispositions will contribute to improved breed health control strategies.
Hematocele in Labyrinth Causing Sensorineural Hearing Loss in Guinea Pigs

Abstract:Objective To investigate whether the hematocele in perilymphatic space will affect the animal’s hearing function and induce the pathological morphologic change or not.Methods The blood with anticoagulant, which was taken from the guinea pig itself,was injected into the scala tympani through the opening at cochlear basal turn bone wall. Before and after oparation,the EcochG was examined.The redistribution of the infected blood in the labyrinth was observed by magnetic resonance imaging (MRI) in different intervals after infecting blood into scala tympani. The change of the injected blood in perilymphatic space and the pathological morphologic changes of labyrinth was observed under light microscopy in three different stages after surgery (1 day, 7 days, and 30 days).The ultramicroscopic pathological changes of Corti organs, spiral ganglions and the strial vascularis were observed under transmission electronic microscopy.Results After injecting blood into scala tympani, the guinea pigs showed different degrees of hearing loss in two experimental groups.There was no difference between these two groups in the type and level of hearing loss.There was high signals in the labyrinth on enchanced T1-weighted MRI after injecting blood into perilymphatic space.Conclusion The hematocele in the labyrinth can damage the guinea pig's hearing.There is no difference between the two different amount blood (5 μl and 10 μl) experimental groups in the type and levels of hearing loss. The hematocele in perilymphatic space can induce the inflammatory reaction in perilymphatic and endolymphatic cavities.The injected blood will be absorbed and/or change into fibrous reticulation.
Platelet- Rich Plasma Treatment Supported by Ultrasound Detection of Septa in Recurrent Canine Aural Hematoma: A Case Series

Abstract:Aural hematoma is a common pathological condition in veterinary practice with a high incidence rate in dogs. Drainage, corticosteroid injections, and surgical approaches represent the common treatments in these clinical cases. However, surgery leaves visible signs and is usually correlated with recurrence, scars, and deformation of the treated pinna. For this reason, more effective and less invasive methods have been proposed over the years. Platelet-Rich Plasma (PRP) is one of the most promising options due to its pro-regenerative properties and capability to modulate the inflammatory state. The present work reports 12 cases of canine aural hematoma treated with PRP. The PRP treatment was combined with an ultrasound evaluation of the pinna to detect and treat all involved septa. The results show that relatively large volumes (2 mL) of PRP associated with an ultrasound guide are safe and efficacious in the treatment of canine aural hematoma requiring a maximum of two infiltrations, both in acute and chronic conditions. All the patients recovered their normal ear thickness (compared with the controlateral one) without relapses, averaging 38.5 days from their first treatment (10-90 days; SD: 24.7). The key role of PRP combined with a tailored diagnosis process carried out by the veterinarian, which included using an ultrasound system and the proper bandage, suggests that this approach may represent a valid alternative to surgery and corticosteroids.

Abstract:One of the difficulties in studying the pathogenesis of epileptiform convulsions is due to the fact that a great variety of apparently heterogeneous causes such as trauma, cerebral edema, anoxemia, and disturbance of acid-base equilibrium may induce convulsions. If some similarity can be found between various factors such as genetic constitution, biochemical or physiological changes and disturbances, and epileptogenic agents, all of which can bring on convulsive behavior, some progress in the understanding of the pathogenesis of convulsions may be expected. A fruitful approach to an understanding of the physiology of convulsions may be provided by a study of rabbits and mice genetically predisposed to convulsive behavior in response to auditory stimulation. The extensive literature on these strains has been reviewed by Antonitis, Crary, Sawin and Cohen (4), Bevan (5), Finger (10), Ginsburg (15), and Vicari (27, 28). At the present time two main approaches are being followed in studies exploring the physiological mechanisms underlying audiogenic seizures. It has been suggested that oxidative processes supplying energy for the nervous system lag behind energy expenditure during convulsive activity. Whether or not this theory is applicable also to audiogenic seizures cannot be decided, since neurochemical studies of the type performed during and after convulsions induced by various pharmacological epileptogens are still lacking in mice suffering from audiogenie seizures (19, 23). However, the protective effect against recurrence of audiogenic seizures exerted by such substances as glutamic acid, glucose, pyruvate, thiouracil, and a number of citric acid cycle intermediates has been interpreted to mean that the differential susceptibility to audiogenic seizures in different strains of mice is a function of different metabolic capacities of the central nervous system (Ginsburg and Hovda (16); Ginsburg, Ross, Zamis and Perkins, (18); Vieari, (27); Vieari, Tracy and Jongbloed, (28)). Homeopathic treatment for aural hematoma in cat: Case report

Abstract:Aural hematoma is defined by the accumulation of serosanguineous fluid within the ear pinna in small animals, typically stemming from traumatic incidents. Inadequate treatment may lead to recurring episodes and structural deformities in the patient's ear. In veterinary clinical practice, this condition exhibits a high incidence among dogs, but is relatively rare in cats. Conventional treatments employed for the physiological and aesthetic restoration of the affected area are typically invasive and often entail significant costs. Therefore, this article aims to present a case study involving a 15-year-old female mixed-breed feline diagnosed with an aural hematoma. The patient had previously undergone various forms of conventional treatment, none of which proved successful. Thus, a complementary treatment approach utilizing oral homeopathy was selected, as it represented the sole available option at that particular juncture. The homeopathic medicines were selected according to the law of similars. The treatment consisted of the following medications: Arsenicum album 30CH; Bellis perennis 30CH and Rhus Toxicodendron 30CH. After 20 days of treatment, the animal exhibited an ear pinna devoid of fluid accumulation and recurrence. The ear pinna was in excellent condition, except for the presence of scars along the ear, resulting in a residual deformation. Hence, homeopathic therapy holds significant promise as a therapeutic modality for managing aural hematomas in cats, obviating the necessity for invasive procedures. Under this perspective, this article represents one of the initial reports documenting such a treatment approach in feline cases.

Abstract:The morphology of the hair cells of the inner ear end organs from the domestic pig (Sus scrofa) have been studied using a combination of Scanning and Transmission Electron Microscopy (SEM and TEM), revealing hair cells from the cochlea and vestibule using a novel surgical and technical approach. This is the first time that the inner ear hair cells from S. scrofa have been studied, thus providing useful anatomical information on the morphology of the hair cells from the cochlea, saccule and utricle from a large mammal. Anatomical information in relation to the morphology of the inner ear is of considerable importance, both in the pathological diagnosis of trauma and in the development of cochlea implants and other biotechnological systems associated with the enhancement of hearing. Standard fixation protocols using cardiac perfusion was not employed in this study as this method cannot always be applied, such as the pathological examination of the human ear, or the study of animals protected by endangered species legislation. With the exception of a very few countries, cetaceans cannot be killed for research purposes, yet physiological information on the inner ear from these species is urgently required for ecological assessment reasons. Supporting the use of S. scrofa as a model for cetacean hearing research is that this animal is a member of the order Artiodactyla, which includes both the hippopotamus and cetaceans. Being of a similar size, the pig is an ideal subject for developing protocols and surgical techniques required to investigate both the human and small cetacean auditory systems.

Abstract:Cochlear otosclerosis, in the histopathological sense, is not uncommon but remains a clinical enigma. Similarly the aetiology of otospongiosis has remained obscure. Nevertheless the enzymatic concept of the initiation and mechanism of the underlying process has been recognized and confirmed together with the essential function of the matrix vesicles forming the site of primary enzymatic calcification. It may be concluded that otospongiosis is a primary lytic process initiated by lysosomal enzymes of matrix vesicles activated in the cartilaginous cell rests of the area. The cause of the process is still unknown.

Abstract:Background: Otohematoma is common in dogs and is characterized by blood accumulation between the skin and cartilage of the outer ear. While the etiology is related to trauma, most cases have a predisposing condition. Treatment must drain the hematoma and maintain appropriate skin apposition to the ear cartilage. Treatment can be surgical, but there are also conservative options such as puncture drainage, followed by intralesional injection of glucocorticoids. This alternative method is less invasive than surgery, with an equivalent success rate. This study aimed to describe intralesional injection of corticosteroids for the treatment of dogs with otohematoma at a veterinary clinic.Materials, Methods & Results: Otohematoma was diagnosed and treated in 23 dogs (14 males and 9 females, weighing 9.6 ± 2.7 kg) at a reference private veterinary clinic. The dogs were chemically restrained, and their ears were cleaned with chlorhexidine. The lower face of each ear with otohematoma was then punctured with a needle coupled to a syringe to drain the liquid. Following drainage, the equipment was removed and the collected liquid volume was measured. A 0.5 mg/kg dose of methylprednisolone acetate was prepared and diluted in saline (0.9% NaCl) to a volume equivalent to 1/10 of the previously drained content volume from the otohematoma. The prepared solution was then injected into the drained ear. Additionally, each patient was treated for the original cause of the otohematoma, according to conventional protocols. Eight animals (34.78%) had bilateral otohematoma and 15 (65.22%) presented with unilateral lesions. Leukocytosis was observed in most patients. Other laboratory alterations present in the studied dogs were thrombocytopenia, leukopenia, and anemia. Twenty (86.96%) patients presented with otitis externa. All patients were reassessed one week after the initial treatment, and 19 (82.60%) fully recovered. The remaining 4 (17.40%) required a new procedure to drain a small accumulation of liquid. Patients were then reassessed on the 15th day when full recovery was observed in 2 (8.7%). The remaining two (8.7%) underwent new drainage on the 15th day, accompanied by a new infusion of corticosteroids, returning recovered on the 21st day.Discussion: Consistent with the previous reports, unilateral otohematomas were more frequently observed in the present study. In most animals, the cause of otohematoma was otitis externa. This disease causes intense itching that leads to autotrauma, resulting in the rupture of vessels and accumulation of fluid between the skin and cartilage of the ear. Leukocytosis detected in the majority of patients was related to external otitis. The other hematological changes observed are common in the disease. The objectives of otohematoma therapy in dogs are to identify and eliminate the source of auricular pruritus, provide adequate drainage of the hematoma content, and maintain the appropriate apposition between the skin and cartilage in the ear. In all 23 patients in this study, these aims were met, culminating in favorable outcomes. The described method led to recovery for most patients (82.60%) in the first week and a success rate of 100% by the end of 21 days. These results observed in animal patients are similar to those seen in humans, where drainage leads to total regression of the disease. Glucocorticoids were chosen for treatment of otohematoma because of their anti-inflammatory effects. Additionally, methylprednisolone acetate, an ester whose absorption was very slow, was used to allow for a longer anti-inflammatory effect.

Abstract:Many diseases can affect the pinna of the dog and cat. These diseases represent most categories of cutaneous disease, from infectious to neoplastic. Although some diseases affect primarily the pinna, most pinnal dermatoses occur in conjunction with other integumentary disorders. One should approach the diagnostic work-up of pinnal disease in an organized manner, obtaining first a thorough history and physical examination. A systematic approach is required in the diagnostic evaluation in order to arrive at a tentative or definitive diagnosis. Skin scrapings, cytologic examination, and culture for dermatophytes provide the minimum database. A variety of diagnostic tests, including hematology, serum biochemical profiles, urinalysis, serology, intradermal skin testing, hypoallergenic diet trials, histopathology, and direct immunofluorescence, may be necessary in some cases before a definitive diagnosis is reached. The ears are somewhat difficult to biopsy, although in many instances histopathologic examination is essential to the diagnosis. In this author's opinion, it is preferable to use a no. 15 scalpel blade rather than a punch biopsy when obtaining a pinnal biopsy. This allows the veterinarian to obtain an elliptical or wedge biopsy, which can frequently be sutured. In some instances, biopsies are required from the ear margin and a full ear thickness biopsy is obtained. Sutures may not hold in this situation, and cautery may be necessary for hemostasis. As in other situations in which the ear is bleeding (aural hematomas, vasculitis, fly bite dermatitis, and so on), bandaging and immobilization of the ear may be necessary to prevent further trauma. Although symptomatic therapy is sometimes helpful when treating pinnal diseases, a definitive diagnosis is preferred. This is especially important following the second or third presentation of an animal for ear disease. Although ear diseases are frequently viewed as a nuisance rather than a serious condition, anyone who has owned or worked closely with an animal suffering from chronic otitis externa should appreciate the animal's discomfort and the owner's frustration. All too frequently, the chronic otitis externa is the result of a hypersensitivity (either inhalant or food) that has been overlooked in the attempt to treat the subsequent infectious otitis. Thus, in animals with recurrent otitis externa, every attempt should be made to identify and treat the underlying etiology.
