Concordance of chronic diseases among middle-aged and elderly couples in China based on the data of CHARLS 2018

Abstract:Objective To investigate the concordance of different chronic diseases among middle-aged and elderly couples in China, to provide scientific reference for the prevention and management of chronic diseases in middle-aged and elderly people. Methods Data were derived from the China Health and Retirement Longitudinal Study(CHARLS), with 5 365matched couples included. The t-test and χ~2 test were used to compare demographic characteristics and the prevalence of each chronic disease between husbands and wives. Multivariate logistic regression models were used to estimate the concordant association between husbands and wives. Results Co-morbidities with the same chronic diseases were common among 5 365 couples. Among them, the concordance prevalence of depression was the highest at 25.4%, with a higher prevalence of 40.69% in wives than 27.38% in husbands(χ~2=211.169, P<0.001). The concordance prevalence of tumor was the lowest at 0.09%, with a higher prevalence of 2.65% in wives than 1.62% in husbands(χ~2=13.498, P <0.001). The concordance of most chronic diseases between husbands and wives was found significant. For these chronic diseases, the risk of having the same chronic disease increased by 28% to 192% for wives and 29% to 122% for husbands if the spouse was sick. The top three chronic diseases with increased risk were depressive symptoms, arthritis or rheumatism and kidney disease, and the increased risk was 192%(OR=2.92, 95%CI: 2.57-3.31), 109%(OR=2.09, 95%CI: 1.85-2.35), and 108%(OR=2.08, 95%CI: 1.63-2.65) for wives and 122%(OR=2.22, 95%CI: 1.98-2.49), 106%(OR=2.06, 95%CI: 1.82-2.32), and107%(OR=2.07, 95%CI: 1.62-2.64) risk for husbands, respectively. Conclusion Spouses of patients with chronic diseases are at high risk of the same chronic diseases. Communities should provide basic prevention guidance for the spouses of patients with chronic diseases to jointly reduce the risk of single and co-existing chronic diseases.
