Abstract:Mucosal melanoma (MM) is an uncommon melanoma subtype affecting mucosal surfaces of the head and neck, anorectal region, and vulvovaginal area. We aimed to present our experience at a tertiary-level hospital regarding MM diagnosis, management, monitoring of progression, mutations, and outcome predictors. We performed a registry-based cohort study including MM cases diagnosed from 2012 to 2022 and retrospectively characterized somatic mutations on BRAF, NRAS and c-KIT. We employed Kaplan–Meier curves, log-rank tests, and Cox regression analysis to explore prognostic factors and survival outcomes in a cohort of 35 patients, mainly women (63%) with a median age of 70 years. Predominantly, MM occurred in the vulvovaginal region (48.6%). At diagnosis, 28.6% had lymph node involvement, and 31.4% also had distant metastasis. Mutations in BRAF and c-KIT were identified in 3/35 (9%) and 2/6 patients (33%), respectively. Surgery was performed in 71.4% of patients, and most received systemic treatment (65.7%). Lower disease stage, thinner Breslow depth, and surgical resection were associated with improved overall survival. Notably, age, sex, radiotherapy, and BRAF mutant status did not affect survival. Standard management typically involves immunotherapy. Cases with BRAF or c-KIT mutations may be considered for targeted therapies. Unfortunately, MM prognosis remains unfavorable, with a less than 50% survival rate at 2 years.

Mucocutaneous Melanoma - A Diagnostic and Therapeutic Problem

Abstract:Mucosal melanoma, or so-called mucosal-oral melanoma is a rare but serious diagnostic and therapeutic problem. The "primary mixed" mucocutaneous forms of melanoma, which affect both the mucosa and the adjacent skin, are also particularly problematic and rare. Given that the staging, diagnosis, and treatment of mucosal (oral) melanoma differs from that of cutaneous melanoma, staging in mixed melanoma (primary mucocutaneous melanoma) as well as decisions for each subsequent diagnostic and therapeutic step should be individualized and modified according to the recommendations of the respective two classifications (for cutaneous but also mucosal melanomas), while at the same time or at least to a large extent overlapping with them. In practice, the following paradoxes occur during staging - there are melanomas with the same tumor thickness, but in different stages, which should be treated in a different, consensus-based way. At the same time, it would be appropriate for the surgical interventions to be in accordance with the patient's wishes for minimal trauma/reduced risk of developing facial disproportion. We present the case of a 69-year-old patient with a newly-developed lesion in the area of the mucosa of the upper lip and adjacent skin, which was identified as a primary mucocutaneous form of melanoma after surgical removal. The complex pathogenesis of the disease is discussed herein, emphasizing the role of UV radiation, iatrogenic immunosuppression with mycophenolate mofetil, tacrolimus, and prednisolone (due to severe glomerulonephritis leading to kidney transplantation), as well as the potential possible but speculative pathogenetic role of acetyl salicylic acid, etc. Primary mucosal and mucocutaneous forms of melanoma remain a challenge for clinicians, and steps for their diagnosis and treatment should be an expression of multidisciplinary, consensual solutions.

Abstract:e20036 Background: Mucosal melanoma (MM) is the second common subtype in Asians with a percentage of 22-25%; but there have been few epidemiologic studies. So we performed a retrospective analysis of MM about its clinical presentation, gene mutation, systemic therapy (tx) and prognosis. Methods: Using institutional databases, we identified patients (pts) with MM who were treated in Chinese melanoma centers between 2006-2014 with basic demographics and clinical outcomes. Results: 463 MM pts were included. Median age at diagnosis 56 years (range, 17-86); 61% female; primary site: 149 anal-rectal, 125 genitourinary, 174 head/neck, 15 others. Mutation (mut) status: 14.1% (45/319) KIT mut; 9.7% (31/319) BRAF mut; 10.3% (33/319) NRAS mut. Stage I-II/ III/IV at referring – 36%/17%/47%, M1a/b/c – 17%/18%/65%. The median overall survival (mOS) was 28.0 mo (95% CI 25.1-30.9 mo). Multivariate analysis showed that clinical stage and LDH level were significant prognostic factors for OS. The application of adjuvant therapy and LDH level were significant prognostic factors for disease free survival. For 293 pts who developed stage IV, mOS was 14.0 mo, for M1a, M1b, M1c, 20.2, 16.0, 11.1 mo respectively (p = .004). In 1st-line setting, the objective response rate (RR), disease control rate (DCR) and median progress free time (PFS) was 11.8%, 54.8%, and 3.1 mo (95% CI 2.6-3.8 mo), most of the pts using dacarbazine/temozolomide, cisplatin plus rhEndostatin had RR, DCR and PFS, 12.3%, 64.6%, and 4.5 mo respectively. In 2nd-line setting, paclitaxel plus carboplatin was common regimen, RR, DCR and PFS was 8.3% 61.1% and 3.3 mo (95% CI 2.0-4.4 mo). The mPFS of 3rd-line tx (including Abraxane, investigational targeted drugs, etc) was 1.3 mo without complete regression and partial regression. 8/20 pts harboring c-kit mutation used imatinib, 3/8 pts had SD for a median PFS of 6.7 mo. 5/15 pts harboring BRAF mutation used vemurafenib, 3/5 pts got response, mPFS cannot be evaluated. Conclusions: The prognosis for MM is still poor, clinical stage has its significance for OS. For pts with stage IV, the efficacy of systemic tx includes chemotherapy and targeted therapy is modest. Further investigation into the biology and treatment of MM is needed.

Abstract:Mucosal melanoma (MM) is a rare melanoma subtype that originates from melanocytes within sun-protected mucous membranes. Compared with cutaneous melanoma (CM), MM has worse prognosis and lacks effective treatment options. Moreover, the endogenous or exogenous risk factors that influence mucosal melanocyte transformation, as well as the identity of MM precursor lesions, are ambiguous. Consequently, there remains a lack of molecular markers that can be used for early diagnosis, and therefore better management, of MM. In this review, we first summarize the main functions of mucosal melanocytes. Then, using oral mucosal melanoma (OMM) as a model, we discuss the distinct pathologic stages from benign mucosal melanocytes to metastatic MM, mapping the possible evolutionary trajectories that correspond to MM initiation and progression. We highlight key areas of ambiguity during the genetic evolution of MM from its benign lesions, and the resolution of which could aid in the discovery of new biomarkers for MM detection and diagnosis. We outline the key pathways that are altered in MM, including the MAPK pathway, the PI3K/AKT pathway, cell cycle regulation, telomere maintenance, and the RNA maturation process, and discuss targeted therapy strategies for MM currently in use or under investigation.
The landscape of mucosal melanoma: A long way to go.

Abstract:Mucosal melanoma is a rare disease, accounting for approximately 1.4% of all Caucasians’ melanomas and 22% of Asian melanomas, which are commonly found in the nasal cavity or sinuses, anorectum, and vulvovagina. Nearly half of the mucosal melanomas are amelanotic. The most common sites of metastatic disease are the liver and the lungs. The prognosis may be similar regardless of anatomic site of origin. The genetics of mucosal melanoma thatg originates from melanocytes within mucosal epithelia has not been fully explored. Data suggest BRAF may occur in only 3–15% of all cases of mucosal melanoma. Mutations in KIT can be identified in 7–17% of all patients with mucosal melanoma. As to the adjuvant therapy, the only published randomized evidence supporting adjuvant therapy after resection of mucosal melanoma was from Lian et al. They randomized 189 patients with resected mucosal melanoma to observation, 1 year of high-dose IFN-α2b, or thrice-weekly chemotherapy with temozolomide plus cisplatin for six cycles. Adjuvant therapy was associated with a trend toward improved survival and HDI was less effective than temozolomide-based chemotherapy for patients with resected mucosal melanoma in respect to RFS. The preliminary interim analysis of a phase III trial (NCT03435302) of 204 patients with mucosal melanoma comparing temozolomide-based chemotherapy with HDI showed that the chemo was superior to HDI. Several clinical trials on adjuvant therapy of mucosal melanomas are ongoing, including NCT03178123 (a phase III trial of PD-1 inhibitors VS HDI) and NCT03241186 (phase II trial of nivolumab + ipilimumab). The clinical trials for systemic treatment of unresectable or metastatic mucosal melanoma were limited. A pooled analysis of the outcomes of patients with metastatic mucosal melanoma treated in trials either with single-agent nivolumab or the combination of ipilimumab and nivolumab compared outcomes to those of patients with cutaneous melanoma. The ORR for mucosal melanoma receiving nivolumab monotherapy or nivolumab combined with ipilimumab were lower than cutaneous melanoma, 23.3% vs. 40.9%, 37.1% vs. 60.4%. PD-1 monoclonal antibody was less effective in mucosal melanomas maybe because of low PD-L1 expression and tumor mutation burden (TMB). At the 2018 ASCO meeting, the results of phase II study of JS001 (PD-1 inhibitors) were reported. 128 melanoma patients were enrolled. Subgroup analysis showed that patients with mucosal origin were less effective (mucosal 0%). How to increase the efficacy for the mucosal melanoma? As we all know, liver metastases are common in mucosal melanomas and difficult to handle. A phase Ic study of oncolytic virus (OrienX010) injected into liver metastases demonstrated good tolerance and efficacy in Chinese patients. The next step is to combine oncolytic virus injected into liver lesions with PD-1 antibody to improve the efficacy of liver metastasis. On the other hand, TKIs are found to have an influence on microenvironment in tumor and facilitate immunotherapy. We conducted a phase Ib study of JS001 in combination with axitinib in metastatic mucosal melanoma. The results were reported in ASCO 2018, which were encouraging to achieve ORR of 66.7%. Recurrent activating mutations in KIT have been identified in 15% of patients, but KIT inhibitors have not produced durable clinical responses. Patients with mucosal melanoma have a poor prognosis in part due to the lack of targeted therapies. Imatinib may remain the first-choice treatment option for patients with a KIT mutation. Genotyping of Mucosal Melanoma.

Abstract:Mucosal melanoma is rare and associated with extremely poor prognosis. Mucosal melanoma has historically been refractory to traditional therapeutic approaches. Recently molecularly based targeted drugs show great success in melanoma. The success of these drug strategies can be partially attributed to the identification of the genetic alterations responsible for the development and progression of metastatic melanoma. This review will focus on genes involved in two major mucosal melanoma-related signaling pathways, the RAS/RAF/mitogen activated protein kinase (MAPK) pathway and the phosphatidylinositol 3-kinases (PI3K)-AKT pathway, and detail the current understanding of their roles in melanoma progression. Additional mutations in key genes, such as KIT, GNAQ and MITF, in mucosal melanoma will also be introduced. Finally, an overview of the current targeted therapy landscape will be provided.

Abstract:Objective To investigate the clinicopathologic characteristics,differential diagnosis,therapy and prognosis of primary sinonasal mucosal malignant melanoma.Methods With review of literature,the clinicolpathologic data were retrospectively analyzed while characteristics of morphology and immunohistochemisty of the tumor cells were observed in 4 patients with primary sinonasal mucosal malignant melanoma.Results The primary sinonasal mucosal malignant melanoma was diagnosed in 3 female patients and 1 male with mean age of 75.5(65-90) years.The clinical manifestation was nasal obstruction,epistaxis or neoplasm.Microscopically,the heteromorphism of the tumor cells was great and the morphologic features were various.The tumor consisted mainly of epithelioid cells and/or small round cells with local spindle cells.Melanin was noticed only in part of the cases and myxoid change was found in one case.Still we observed in one case that pigmented mucosal melanoma cells became bigger in size and hypopigmented with clear cytoplasm,so-called "balloon degeneration",after local recurrence.Immunohistochemisty showed that the tumor cells were all positive for HMB45,vimentin and S-100 protein in a varying degree,but NSE,LCA,SMA,CD99 and desmin negative,while focal and weak-positive expression of CK and CD68 was also noted in one cases,and Ki-67 expressied in a different level.Conclusion The primary sinonasal mucosal malignant melanoma is rare and highly heterogenous.The application of immunohistochemistry plays an important role in its diagnosis and differential diagnosis.
Outcomes of Primary Mucosal Melanoma Originated from Sinonasal Tract

Abstract:Purpose Mucosal melanoma of the head and neck is a rare entitywith a poor prognosis. Sinonasal tract is a common occurrence site for this malignancy. The aim of this study is to provide an analysis of 25 patients diagnosed with sinonasal mucosal melanoma (SNMM) treated in a single institution.Patients and methods The medical charts of 25 SNMM patients without distant metastasis treated from 1996 to 2007 were reviewed. Patients were re-staged according to the 7th edition of American Joint Committee on Cancer (AJCC) tumor-node-metastasis (TNM) staging system for head and neck melanoma. Fourteen and 11 patients were diagnosed with T3 or T4 disease, respectively. Three patients had lymph node metastasis at diagnosis. Surgical resection was performed in 21 patients(84%), and adjuvant radiation therapy was given to all patients except for one. Several patients also received chemotherapy or biological treatment.Results The 5-yearlocal- regional control (LRC) rate was 54 %. The 5-year overall survival (OS) and disease-free survival (DFS) rates were 41 and 38 %, respectively. Recurrences were observed in 10 patients and distal failures in 9 cases. On univariate analysis, locally advanced stage (IV) was a poor prognostic factor for OS and DFS (P< 0.05). In addition, patients diagnosed with T4 lesions had significantly reduced local-regional control as compared to those with T3 diseases ( 18 vs. 77 %, P=0.02). Multivariate analysis revealed that T classification was the only significant factor for local-regional control.Conclusion SNMM is a devastating tumor, with high risk for local failure as well as distant metastases. The stage of the primary disease, i.e., the extent of tumor infiltration, is the predominant factor influencing the prognosis. OS and DFS after surgery followed by radiation therapy remain suboptimal. Further researches for novel treatment strategies such as targeted therapy are necessary to improve the outcomes.
Characteristics and prognosis of primary mucosal melanoma of nasal cavity and paranasal sinuses: a clinical analysis of 94 patients

Abstract:Objective To investigate the clinical features and survival status of mucosal melanoma of the nasal cavity and paranasal sinuses and to analyze the prognostic factors.Methods A retrospective analysis was performed on the clinical data of 94 patients with mucosal melanoma of the nasal cavity and paranasal sinuses treated from January 2000 to December 2012.Of the 94 patients,50 were male,and 44 were female.The median age of onset was 60 years (range,26-85 years).The primary sites were nasal cavity (86 patients),maxillary sinus (7 patients),and nasopharynx (1 patient).Cervical lymph node metastasis was observed in 10 patients (7 patients before treatment,2 patients during treatment,and 1 patient after treatment).No patient had distant metastasis.Patients were treated with surgery ± radiotherapy.The Kaplan-Meier method was used to calculate survival rates,and the logrank test was used for univariate prognostic analysis;the Cox regression model was used for multivariate prognostic analysis.Results The 1-,3-,and 5-year sample sizes were 80,54,and 50,respectively.The 1-,3-,and 5-year disease-related survival rates were 71％,33％,and 22％,respectively.Univariate analysis showed that the prognostic factors were age over 55 years (P =0.034),involvement of the posterior naris (P =0.011),involvement of the maxillary sinus (P =0.009),involvement of the hard palate (P =0.003),cervical lymph node metastasis (P =0.001),and therapeutic method (P =0.038).Multivariate analysis showed that the prognostic factors were involvement of the posterior naris (P =0.027),involvement of the orbit (P =0.005),and involvement of the hard palate (P =0.003).Conclusions The distant metastasis and local recurrence rates are high among patients with mucosal melanoma of the nasal cavity and paranasal sinuses,so combination therapy is imperative.Cervical lymph node metastasis rate is low.Rational clinical staging needs to be further explored.

Abstract:While mucosal-based melanomas of the head and neck region are uncommon lesions, when they do arise they usually follow an inexorably aggressive course. Experience with these tumors is, necessarily, limited; as such, well-worked out treatment protocols for the treatment of such lesions are in short supply. It appears as though mucosal melanomas (MuMs) develop more frequently in the nasal cavity and paranasal sinus region, and less often in the oral cavity. It seems that the incidence of nodal metastasis is significantly lower for sinonasal MuMs than it is for MuMs of the oral cavity; this observation may influence decisions about performing neck dissection as a function of location of the primary MuM. At present, surgical excision remains the mainstay of treatment; however, anatomical complexities within the region can hamper attempts at complete excision. Radiotherapy has not traditionally been relied on for routine treatment of MuM, although some recent reports have challenged this view. Chemotherapy is, at present, employed principally in the treatment of disseminated disease and for palliation. As a diagnostic matter, MuM belongs to the class of tumors that, on light microscopy, may with some regularity be confused with other malignancies (including sarcomas, plasmacytomas, and carcinomas); as a consequence, this is a diagnosis which is often best confirmed by way of ancillary testing via immunohistochemical studies. A better grasp of the best means of treating MuM will likely come only when large referral centers are able to pool their experiences with these uncommon yet virulent malignancies.

Abstract:Knowledge of key drivers and therapeutic targets in mucosal melanoma is limited due to the paucity of comprehensive mutation data on this rare tumor type. To better understand the genomic landscape of mucosal melanoma, here we describe whole genome sequencing analysis of 67 tumors and validation of driver gene mutations by exome sequencing of 45 tumors. Tumors have a low point mutation burden and high numbers of structural variants, including recurrent structural rearrangements targeting TERT, CDK4 and MDM2. Significantly mutated genes are NRAS, BRAF, NF1, KIT, SF3B1, TP53, SPRED1, ATRX, HLA-A and CHD8. SF3B1 mutations occur more commonly in female genital and anorectal melanomas and CTNNB1 mutations implicate a role for WNT signaling defects in the genesis of some mucosal melanomas. TERT aberrations and ATRX mutations are associated with alterations in telomere length. Mutation profiles of the majority of mucosal melanomas suggest potential susceptibility to CDK4/6 and/or MEK inhibitors.

Abstract:Objective To determine the clinical presentation and outcome of patients with mucosal melanoma in China.Methods The elements of the database included basic demographic data of patients and prognosticators previously reported in literature,as well as the follow-up data including clinical outcome after treatment.The American Joint Committee on Cancer(AJCC) staging system(7th edition) was used for either clinical or pathological staging.Medical record of all patients with pathologically diagnosed mucosal melanoma consulted in our center since 2006 were retrieved and reviewed.Statistical analyses including survival and multivariate analyses of factors associated with survival were respectively performed by Kaplan-Meier method and Cox proportional hazard model.Results A total of 212 cases of mucosal melanoma were identified and reviewed.The mean age was 55.8 years with male-to-female ratio 1∶1.46.Head and neck was the most common site(45.3%).Bleeding was the most common initial symptom(40.6%).In their primary lesion,49.5% of patients had definitive ulceration with a median thickness 3.8mm(0.6-16.0mm).Lung(55.2%) was the most common metastatic site.The mainstay of therapy for mucosal melanoma was still surgery.The 1-,2-and 5-year overall survival(OS) rates were 83.7%,68.3% and 26.1%,with a median OS of 33.0 months(95% CI: 22.9-43.1months).The results of multivariate analysis indicated that stage at diagnosis and serum lactate dehydrogenase(LDH) level were two significant predictive factors for OS,while the application of adjuvant therapy and LDH were the independent factors influencing disease free survival(DFS).Conclusion Head and neck was the most common primary site of mucosal melanoma.In their primary lesion half of patients had definitive ulceration and got a much thicker tumor invasion.Clinical staging was significantly associated with clinical outcome in terms of OS,and the use of adjuvant therapy could significantly improve the DFS of patients with stage Ⅰ-Ⅲ.
Whole-exome Sequencing Reveals Mutational Profiles of Anorectal and Gynecological Melanoma

Abstract:Mucosal melanoma is a rare and highly malignant type of melanoma. Among the sites that mucosal melanoma arises, anorectal and gynecological melanoma has more aggressive behavior and worse prognosis. There was no effective therapy for mucosal melanoma at present. Only a small number of mucosal melanoma patients which harbor mutations in BRAF or KIT benefit from targeted therapy. So it's an urgent need to identify more actionable mutations in mucosal melanoma. To identify more potential therapeutic targets in mucosal melanoma, 48 samples were collected from 44 patients with anorectal or gynecological melanoma and subjected to whole-exome sequencing. The tumor mutation burden was low with a median of 1.75 mutations per Mb. In chromosomal level, 1q, 6p and 8q of mucosal melanoma were significantly amplified while 9p, 10p, 10q, 16p and 16q were significantly deleted. Muc16 was the most frequently mutated oncogene in our samples(25%). The mutation frequency of KIT(20%) was comparable to the "triple-wild" genes-NRAS(20%), NF1(20%), and BRAF(11%). KMT2D mutation was found in 18.18% patients, which is previously unidentified. MAPK signaling pathway and lysine degradation were the most frequently mutated pathways. Moreover, patients with TP53 mutations tend to have worse clinical outcome (median survival time 19 vs. 50 months, log-rank P = 0.006). 2000 ore mutated genes involved in MAPK signaling pathway were identified, which expand the patients potentially benefit from ample MAPK inhibitors. KMT2D could be a potential therapeutic target. Moreover, TP53 could be a potential prognosis marker for mucosal melanoma.

Abstract:Malignant melanoma is a highly aggressive cancer arising from the skin, retina, and mucosal lining of the respiratory, gastrointestinal (GI), or genitourinary tracts, all of which contain melanocytes. Mucosal or extracutaneous melanomas (ECMs) are rare accounting for 1% of all melanomas. We herein report a case of a metastatic mucosal melanoma presenting as occult blood loss anemia. A 58-year-old male presented with generalized weakness, anorexia, weight loss, and intermittent melena for one year. On exam, he was tachycardic, borderline hypotensive, and pale without epigastric tenderness. Labs showed severe anemia [hemoglobin, Hgb 3.8 mg/dL, mean corpuscular volume (MCV) 72 fl] for which he received two units of red cells. Endoscopy revealed an 8 mm non-bleeding, gastric ulcer with a raised border and a clean base on the wall of the gastric body. Histologic analysis was consistent with malignant melanoma displaying strong positivity for S-100, Melan A, and HMB 45 stains. The CT of the abdomen revealed multifocal metastatic disease with subcutaneous, intramuscular, and perinephric implants with suspicion of small bowel carcinomatosis. The patient underwent an excisional biopsy for the abdominal wall mass and surgical pathology confirmed melanoma. The patient is planned to be started on immunotherapy for advanced disease. Most melanomas found in the GI tract are metastatic. Mucosal melanoma presenting as a gastric ulcer is extremely rare. As a result, metastasis from other sites must be ruled out before making a diagnosis of primary gastric melanoma (PGM). In our case, a widespread disease with unknown primary elucidated the diagnosis but post-operative inspection failed to find any potential lesion on the skin, genitals, or other organs, suggesting the possible diagnosis of metastatic gastric melanoma. However, follow-up is still required to confirm the diagnosis according to the established criteria. Pathologic diagnosis of melanoma requires the identification of melanin in the cytoplasm and immunohistochemistry with specific markers such as S-100, Melan A, and HMB-45. Although the pathologic diagnosis of PGM is similar to cutaneous melanoma, preoperative diagnosis is difficult due to the extremely low incidence, lack of obvious melanin pigmentation, similar microscopic patterns as more common gastric cancers, and lack of awareness among physicians and pathologists. The prognosis of mucosal melanoma is poor, with a five-year survival rate of 25% versus 80% for cutaneous melanoma. Advanced age, surgically unresectable disease, and lymph node involvement are all poor prognostic markers. There is no standard protocol for treatment. Surgery is the only curative treatment for the resectable disease. Adjuvant chemotherapy, radiation, and immunotherapy have an established role in cutaneous melanoma but there is only limited data on adjuvant systemic therapy with mucosal melanoma. Further research is imperative to establish proper management guidelines for this rare disease entity.
Gene mutations and clinical prognosis of mucosal melanoma in different locations of head and neck

Abstract:Background Mucosal melanoma is the second most common subtype of melanoma in China and head and neck region is one of the main sites of this disease. Aims/Objectives Analyzed the phenotypes of C-Kit, NRAS, PDGFRA and BRAF genes in patients with in different locations to explore the characteristics of gene mutations. Material and Methods 96 patients were included in this study. C-Kit (exons 9, 11, 13, 17 and 18), NRAS (exons 1 and 2), PDGFRA (exons 12, 14 and 18) and BRAF (exons 11 and 15) were analyzed by PCR amplification and Sanger sequencing. Results 14 (14.58%) patients had C-Kit mutation, 6 (6.25%) had BRAF mutation, 23 (23.96%) had PDGFRA mutation, and 12 (12.50%) had NRAS mutation. The NRAS mutation (p = 0.037, 95%CI: 1.050-4.572) was an independent factor affecting distant metastasis and was most commonly found in the nasal cavity/paranasal sinuses (p = .043) while the BRAF mutation was more common in locations other than the nasal cavity/paranasal sinus (p = .008) and was associated with local recurrence. Conclusions and Significance Gene phenotypes of mucosal melanoma in different locations has differences. Lesions in the nasal cavity and paranasal sinus should be assessed separately from other parts such as the nasopharynx.

Abstract:e21588 Background: Primary mucosal melanoma arising in the urinary tract is rare and poorly characterized. Methods: The records of patients with urological mucosal melanoma who presented to the department of Renal Cancer and Melanoma of Peking University Cancer Hospital between September, 2004 and April, 2019 were reviewed. Available clinicopathological and molecular characteristics were summarized, including pathological parameters, gene mutation, primary surgical intervention, systemic treatment and clinical course. The rates of local recurrence rate, loco-regional lymph node metastasis and distant metastasis were assessed. American Joint Committee on Cancer (AJCC) TNM Staging System for bladder cancer/renal pelvis and ureter cancer/urethral carcinoma (8th ed., 2017) were adopted for staging. Results: Fifty-eight patients were involved in the study with a median age of 62.5 years (range: 32-82). The anatomic sites of the primary urological mucosal melanomas were from the urethra (89.7%), bladder (6.9%), ureter (0%) and kidney (0%), and the left (4.4%) were from multiple loci. At initial diagnosis, 75.9% (n=44) were stage I/II disease, 1.7% (n=1) stage III, and 22.4% (n=13) stage IV. There was 3.4% incidence of CKIT mutation and 1.7% of BRAF mutation. After median follow-up of 22.6 mo, 31.4% (16/51) relapsed locally after organ-preserved surgery. 21.6% (11/51) and 39.2% (20/51) developed metastases to reginal lymph nodes and distance, respectively. The median relapse free survival and median overall survival were 12.2 (95%CI: 7.9-16.4) mo and 33.9 (95%CI: 19.2-48.6) mo, respectively. Univariate Cox analysis showed that TNM stage and systemic adjuvant therapy were the prognostic factors of OS, while no association was found with Breslow thickness, miotic rate, ulceration and gender. Conclusions: Urological mucosal melanoma predominantly arises from lower urinary tract with rare BRAF and CKIT mutation. AJCC TNM Staging System for urothelial carcinoma is proved practical for urothelial melanoma, which should be validated in larger population. Future research is required to identify adjuvant treatment approaches to improve disease outcomes.

Abstract:9569 Background: Mucosal melanoma is rare and associated with extremely poor prognosis. Little is known about its outcome and prognostic analysis. In this study, we evaluated prognostic factors among mucosal melanomas. Methods: The survival rates, Relapse Free Survival (RFS), Overall Survival (OS) and prognostic factors were compared for 706 mucosal melanomas at different anatomical sites. Results: Mucosal melanoma from nasal pharyngeal and oral (268 pts), upper and lower gastrointestinal (GI) (221 pts), gynecological and urological (196 pts) had a similar survival with a 1-y survival rate (88%, 83%, 86%), 2-y survival rate (66%, 57%, 61%), 5-y survival rate (27%, 16%, 20%), respectively. Multivariate analysis revealed that Depth of Invasion (p < 0.001), Lymph node metastases (p < 0.001), Distant metastases (p < 0.001) were three independent prognostic factors for OS among 706 pts. Anatomical site (p = 0.031), Depth of Invasion (p < 0.001), Lymph node metastases (p < 0.001) were three independent prognostic factors for RFS among 543 pts. KPS status, Depth of Invasion, Lymph node metastases, Distant metastases were independent factors for OS among nasal pharyngeal and oral pts. Depth of Invasion, Lymph node metastases, CKIT Mutation were independent factors for RFS among nasal pharyngeal and oral pts. Gender, Lymph node metastases, Distant metastases were independent factors for OS among GI pts. Gender, Depth of Invasion, Lymph node metastases were independent factors for RFS among GI pts. Lymph node metastases, Distant metastases were independent factors for OS among Gynecological and Urological pts. Depth of Invasion, Lymph node metastases were independent factors for RFS among Gynecological and Urological pts. Conclusions: This is the first prognostic analysis for mucosal melanoma with the largest sample size for the first time. with few exceptions, It revealed that Depth of Invasion, Lymph node metastases, Distant metastases were independent prognostic factors for OS, Depth of Invasion and Lymph node metastases were independent prognostic factors for RFS. These results should be incorporated into the establishment of stage system and design of future clinical trials involving patients with mucosal melanoma.

Abstract:Abstract Background: As a highly malignant tumor type, the mucosal type is the second most common pathological subtype in China. In this study, we analyzed the results and clinical data for C-KIT, NRAS, PDGFRA and BRAF genotypes in patients, to explore the characteristics of gene mutation in head and neck mucosal melanoma and the correlation between the four common gene mutation types and prognosis.Methods: C-KIT (exons 9, 11, 13, 17 and 18), NRAS (exons 1 and 2), PDGFRA (exons 12, 14 and 18) and BRAF (exons 11 and 15) were analyzed by PCR amplification and Sanger sequencing in patients. The Chi-squared test, Log rank test and Cox regression model were used to analyze the risk factors. Results: In total, 96 patients were included in the study. 14 (14.58%) patients had the C-KIT mutation, 6 (6.25%) had the BRAF mutation, 23 (23.96%) had the PDGFRA mutation, and 12 (12.50%) had the NRAS mutation. The NRAS mutation (P = 0.037, 95%CI: 1.050–4.572) was an independent factor affecting postoperative distant metastasis. The mutation types of different primary tumor sites were different. The BRAF mutation was more common in the nasopharynx and other sites (P = 0.008), whereas the NRAS mutation was more common in the nasal cavity/sinus (P = 0.043).Conclusion: The specificity of this genotype may have some guiding significance for treatment of patients with head and neck mucosal melanoma, but this study did not find any correlation with survival time, and this needs further analysis with a larger dataset.

Abstract:Acral and mucosal melanoma are uncommon variants of melanoma. Acral melanoma has an age-adjusted incidence of approximately 1.8 cases per million individuals per year, accounting for about 2% to 3% of all melanoma cases. On the other hand, mucosal melanoma, with an incidence of 2.2 cases per million per year, makes up around 1.3% of all melanoma cases. These melanomas, in addition to being biologically and clinically distinct from cutaneous melanoma, share certain clinical and pathologic characteristics. These include a more aggressive nature and a less favorable prognosis. Furthermore, they exhibit a different mutational pattern, with KIT mutations being more prevalent in acral and mucosal melanomas. This divergence in mutational patterns may partially account for the relatively poorer prognosis, particularly to immune checkpoint inhibitors. This review explores various aspects of acral and mucosal melanoma, including their clinical presentation, pathologic features, mutational profiles, current therapeutic approaches, outcomes associated with systemic therapy, and potential strategies to address resistance to existing treatments.

Abstract:ObjectivesMelanomas arising from mucosa are rare and associated with a poor prognosis. This study aims to provide an analysis of metastatic pathways, time intervals, factors influencing metastatic spread and organs for distant metastases.MethodsA total of 116 patients with mucosal melanomas of different sites were included. The mean follow-up interval was 47 ± 52 months. Patients were assigned to two different metastatic pathways, either presenting loco-regional lymph node metastases as first spread or direct distant metastases. The distribution of distant metastases was assessed.ResultsTwenty-six patients presented with a pre-existing metastatic spread and were not assigned to pathways. Of the included patients, 44 developed metastases after treatment of the primary tumour; 25 patients directly developed distant metastases; 16 patients developed regional lymph node metastases prior to distant metastases. Location of the primary tumour in the upper airway or GI tract and advanced T stage were significant risk factors of direct distant metastases. Distant metastases are mainly located in the lung, the liver and non-regional lymph nodes.ConclusionsMucosal melanomas show a high rate of direct distant metastases rather than regional lymph node metastases. Thus the follow-up should always include a whole-body cross-sectional imaging in high-risk tumours.Key points• Mucosal melanomas show a high rate of direct distant metastases.• T stage and primary location are predictors for direct distant metastases.• Distant metastases were mainly found in lung, liver and lymph nodes.• Follow-up of a high-risk mucosal melanoma should include whole-body imaging.
