Abstract:Context: The collegiate athletic setting has been described as having high workloads and working demands. The extensive time commitment required of athletic trainers working in this setting has been identified as a precursor to work-family conflict (WFC) and work-family guilt (WFG). Although individualized, experiences in the work-life interface can largely be affected by organizational factors (ie, elements specific to the workplace). Staff size and patient load may influence the athletic trainer's feelings of WFC and WFG, yet these factors have not been directly studied. Objective: To examine organizational factors and experiences of WFC and WFG among collegiate athletic trainers. Design: Cross-sectional study. Setting: Collegiate setting. Patients or other participant(s): A total of 615 (females = 391, gender variant or nonconforming = 1, males = 222, preferred not to answer = 1) athletic trainers responded to an online survey. The average age of participants was 33 ± 9 years, and they were Board of Certification certified for 10 ± 8 years. A total of 352 participants (57.2%) worked in National Collegiate Athletic Association Division I, 99 in Division II (16.1%), and 164 in Division III (26.7%). Main outcome measure(s): Participants responded to demographic and workplace characteristic questions (organizational infrastructure, staff size, and number of varsity-level athletic teams). They completed WFC and WFG scales that have been previously validated and used in the athletic trainer population. Results: Work-family conflict and WFG were universally experienced among our participants, with WFC scores predicting WFG scores. Participants reported more time-based conflict than strain- or behavior-based conflict. No differences in WFC and WFG scores were found among organizational infrastructures. Weak positive correlations were present between staff size and WFC scores and WFG scores. The number of athletic teams was not associated with WFC or WFG scores. Conclusions: Organizational factors are an important component of the work-life interface. From an organizational perspective, focusing on improving work-life balance for the athletic trainer can help mitigate experiences with WFC and WFG.

Abstract:The athletic training research agenda identifies a need for exploration on strategies for improved work–life balance. One unexplored area is self-care practices used to maintain work–life balance. Fourteen (six males, eight females) athletic trainers employed in the secondary school setting participated in a qualitative research study. All interviews were recorded, transcribed, and analyzed using an inductive approach as prescribed by phenomenological approach. Data saturation guided recruitment and peer review and intercoder reliability was used for credibility. Secondary school athletic trainers are engaging in self-care practices regularly and connect those activities to their work–life balance. Self-care practices are designed to create “me time” and are geared toward physical and social self-care activities. In addition, our results address the interchangeable thoughts of self-care practices, stress relief, and work–life balance.

Work-Life Balance of Faculty in Professional Physical Therapist Education: A Mixed-Methods Study

Abstract:Introduction: The purpose of this mixed-methods study was to explore perceptions of work-life balance (WLB) of full-time faculty in professional physical therapist (PT) education programs. Review of literature: Studies show that faculty are at risk of diminution of WLB and subsequent burnout due to job expectations and changing educational landscapes. Limited evidence exists to describe perceived WLB of faculty in professional PT education. Subjects: Full-time faculty in professional PT education programs (n = 239) responded to an online survey and 16 participated in focus group discussions. Methods: The Work/Nonwork Interference and Enhancement Survey (W/NW Scale), faculty characteristic prompts, and open-ended questions were disseminated electronically to all accredited professional PT education programs within the United States. Focus groups were conducted to further explore nuances of WLB. Frequencies and percentages were calculated for survey categorical variables with means and standard deviations for continuous variables. Differences in W/NW Scale and faculty characteristics were analyzed using independent-samples Mann-Whitney U tests. Open coding and an iterative process of thematic analysis was used to analyze the qualitative data. Results: Medians for all W/NW Scale dimensions were between 2 and 3 out of 5. W/NW Scale dimension scores differed significantly in each of the faculty characteristics of gender, defined workload, and faculty job searching (Z = -2.399 to -3.517, P ≤ .016). Qualitative data yielded 3 main themes: 1) implicit and explicit job duties; 2) leadership; and 3) individual boundaries. Ebb and flow of WLB emerged as an overarching theme. Discussion and conclusion: This study offers a portrayal of the lived experience of faculty regarding WLB. Results offer useful information about faculty demographics, work-related factors, and personal responsibilities. One limitation was that cultural, community, and religious demands were not explored. Findings may facilitate improved understanding of faculty WLB perceptions and can guide leadership to optimize the balance between faculty's paid work and personal life.

Abstract:Context: Athletic training is a multifaceted profession characterized by interpersonal relationships and a team approach to care. Collaborative relationships, by nature, open the door to conflict, which has been reported frequently in the collegiate athletic setting. However, secondary school athletic trainers' (ATs') experiences with conflict and pressure in their role are not readily understood. Objective: To measure the extent and sources of stress, pressure, and conflict within the secondary school athletic training setting and determine if differences exist across employment characteristics. Design: Cross-sectional study. Setting: Secondary school athletics. Patients or other participants: Secondary school ATs (n = 725, age = 39.8 ± 10.5 years, years certified = 16.7 ± 9.7, years in current role = 10.6 ± 7.8). Main outcome measures: Participants were asked to reply to an online questionnaire with quantitative measures pertaining to organizational conflict and workplace dynamic. Employment type (school district employee, school district teacher, medical or university facility, independent contractor) and status (full time, part time) served as independent variables. Likert-scale scores (1 = strongly agree to 5 = strongly disagree; 1 = always to 5 = never) and perceived sources of stress, pressure, and conflict were the dependent variables. Analyses consisted of Kruskal-Wallis tests with Mann-Whitney U post hoc tests and odds ratios to assess associations between variables of interest. Results: We obtained a 15.3% response rate (725/4745). Although the ATs reported experiencing conflict and pressure, these experiences were relatively infrequent and not universal. Compared with part-time ATs, full-time ATs described higher ratings of strong relationships with coaches (P = .003) and principals (P = .002). The most frequently identified sources of conflict were parents (59%) and coaches (53.9%), followed by athletes (32.6%). Full-time ATs were 1.6 times more likely to report experiencing conflict with a coach than part-time ATs (odds ratio = 1.550, 95% CI = 1.037, 2.317; P = .040). Conclusions: Secondary school ATs' experiences regarding organizational conflict were relatively positive. Instances of pressure and conflict were noted, though relatively infrequently, and these experiences were largely uninfluenced by employment type.

Abstract:Context: The literature in athletic training has consistently demonstrated evidence of work-family-life conflict and the potential consequences of that conflict among athletic trainers (ATs) employed in the clinical setting. Parental responsibilities have been suggested to increase the conflict among work, family, and life. The emotions that occur because of this conflict have received little attention and warrant further study. Objective: To investigate perceptions and antecedents of work-family guilt (WFG) among secondary school ATs with children. Design: Phenomenological qualitative study. Setting: Secondary schools. Patients or other participants: Twenty (13 women, 7 men) ATs with children (range = 1-3) employed in the secondary school setting. All but 3 were married (n = 17) at the time of the study. Their average age was 37 ± 11 years, and they were certified as ATs for 14 ± 11 years. Data collection and analysis: Participants completed one-on-one semistructured phone interviews. Multiple-analyst triangulation and peer review were used to establish data credibility. Results: General inductive analysis revealed that men and women participants experienced feelings of WFG despite having supportive work environments. The guilt for both sexes stemmed from work interfering with family and an altruistic mindset. Women indicated they felt pressure from their husbands that contributed to feelings of guilt. Conclusions: Secondary school ATs experienced WFG. Trying to balance parental and athletic training duties can cause an emotional response, and ATs' giving and caring nature may be a precursor to guilt.

J Strength Cond Res 37(11): 2213-2221, 2023-Monitoring accumulated workloads, acute:chronic workload ratios (ACWR), and training monotony (TM) are practical methods for monitoring athlete physical stress. Performance testing provides useful information about the changing nature of physical abilities. Therefore, the purpose was to examine differences in accumulated workloads based on session type, explore seasonal trends in ACWR and TM, and assess changes in performance assessments in collegiate women's lacrosse athletes. Athletes, who were identified as starters ( n = 12), wore positional monitoring technology during training sessions ( n = 61) and games ( n = 17) and completed preseason and postseason assessments of speed, agility, power (jump tests), strength, aerobic capacity, and body composition. Separate 1-way analyses of variance were used to determine differences in accumulated workloads for session type and differences in performance assessments from preseason to postseason ( p < 0.05). When compared with games, practice sessions elicited greater ( p < 0.001) accumulated total distance, player load, repeated high-intensity efforts, accelerations, change of direction, explosive efforts, high-speed efforts ( p = 0.002), and high-speed distance ( p = 0.002). Throughout the season, ACWR and TM ranged from 0.16 to 1.40 AU and 0.68-1.69 AU, respectively. The 40-yd sprint ( p < 0.001) and pro-agility ( p < 0.001) improved from preseason to postseason, whereas no changes in aerobic capacity, lower-body power, or strength were observed ( p > 0.05). The monitoring of accumulated loads, ACWR and TM, and performance tests revealed novel information about the seasonal demands of collegiate women's lacrosse. Women lacrosse players are able to improve speed and agility throughout the season, while maintaining strength, power, and endurance, with minimal reductions in fat-free mass.

Abstract:ABSTRACT Context Athletic trainers (ATs) face organizational-professional conflict (OPC), often surrounding return-to-sport decisions. To prioritize patient safety and establish a healthy work environment, OPC must be mitigated, yet little research has determined how ATs manage conflicts with stakeholders. Objective To explore ATs’ experiences with OPC in the secondary school setting. Design Qualitative study Setting Telephone interviews Patients or Other Participants 16 ATs (9 females, 7 males; age = 43±11 years; years certified = 17±9; years in their current positions = 9±6) Data Collection and Analysis We digitally recorded telephone interviews and had them professionally transcribed. Data saturation guided recruitment efforts, and was met. To ensure rigor and trustworthiness of the data, we completed basic member checks along with multiple analyst triangulation. We analyzed the qualitative data using an interpretive phenomenological approach. Results Four themes emerged: effective communication, professional relationships, stakeholder education and professional experience. Participants used effective communication described as frequent, open, and direct, during interactions with stakeholders to manage OPC. OPC was reduced when ATs built professional relationships with stakeholders centered on trust and respect. Participants used stakeholder education as a primary strategy for managing OPC by educating stakeholders about prognosis and return-to-sport timelines post-injury and providing rationale for decisions made. Additionally, years of experience served as a mitigating factor of conflict, in that as ATs gained experience and confidence, they perceived less OPC. Conclusions Participants suggested various interpersonal relationship development strategies that can be implemented to manage OPC, especially when starting a new position or building rapport with stakeholders. Specifically, educating various stakeholders on reasons for clinical decisions via effective communication and developing strong professional relationships built on mutual respect assisted in avoiding OPC. Since professional experience appears to alleviate conflict, OPC management strategies should be taught during professional preparation and used early during transition to autonomous practice.

Abstract:Nickel-catalyzed Suzuki-Miyaura coupling of heteroaryl ethers with arylboronic acids was described. Selective activation of the phenol C-O bonds was achieved by converting them into the corresponding aryl 2,4-dimethoxy-1,3,5-triazine-6-yl ethers, in which aryl C-O bond could be selectively cleaved with inexpensive, air-stable NiCl2(dppf) as a catalyst. Coupling of these readily accessible heteroaryl ethers proved tolerant of extensive functional groups.

Abstract:Student-athlete mental health is an important and complex issue necessitating examination of variables that may explain changes in key outcomes during training and competition periods. Further, in-season changes in mental health may be predicted by student-athlete training load or psychological resilience and should be examined further. This longitudinal study assessed changes in mental health symptoms of depression, anxiety, and subjective well-being across a competitive season and examined potential interactive effects of resilience and training load in predicting mental health symptoms over time. Participants were 160 NCAA Division I student-athletes between 18–23 years old (mean age = 19.9 ± 1.3). We measured mental health symptoms and resilience at 3 timepoints (across 12 weeks) throughout a competitive season with psychometric questionnaires. Daily training loads were also collected and then used to calculate average acute to chronic workload ratios (ACWR). Positive mental health symptoms did not change across student-athletes; whereas, negative symptoms changed over time with the highest levels of symptoms occurring in the middle of the season. Resilience was positively associated with subjective well-being and negatively associated with anxiety and depression across the study period. ACWR was not associated with depression or anxiety symptom scores but a significant interaction between ACWR and resilience was found for subjective well-being symptoms. Specifically, resilience buffered against ACWR's negative effect on positive mental health. Results suggest the importance of student-athlete resilience during the competition season and evaluation of depression and anxiety at pre-season with mid-season re-evaluation.

An Examination of Athletic Trainers' Occupational Recovery Experiences During Time After Work

Abstract:Context: Although researchers have directed scholars toward investigating the effectiveness of the nonwork personal time of athletic trainers (ATs), no one has characterized the occupational recovery experiences of ATs. Objective: To examine the reliability and validity of the Recovery Experience Questionnaire (REQ) for use in AT populations. Design: Cross-sectional study. Setting: Web-based survey. Patients or other participants: A total of 144 ATs (71 men, 73 women) working in a variety of National Collegiate Athletic Association sports across all levels of competition. Main outcome measure(s): The REQ was administered to assess the AT recovery experience. Results: Preliminary evidence emerged for the reliability (ω = 0.80-0.90) and validity of the REQ for use in AT populations. Weak linear relationships were identified between stress and perceptions of psychological detachment (r = -0.314, P < .001), mastery (r = -0.179, P = .32), control (r = -0.284, P = .001), and relaxation (r = -0.157, P = .06). Conclusions: Our results support measuring and applying occupational recovery for AT stress and work-life balance. Given that occupational recovery as a construct was only weakly related to stress, it is clearly a unique and distinct variable worth considering within the work-life balance line of inquiry.
Post-Pandemic Changes to Employment and Employment Satisfaction in Early Career and Career Advancing Athletic Trainers

Abstract:ABSTRACT Context The labor market suffered a mass exodus of employees, including healthcare workers, due to the COVID-19 pandemic. Previous research highlights the vulnerability of young professional athletic trainers (YPATs) to attrition. Investigating pandemic-related employment changes and their impact is essential for developing strategies to improve the retention of YPATs. Objective to determine impact of COVID-19 on YPATs’ employment and effect of COVID-19 and employment setting migration on outcome measures of personal and financial well-being, job and career satisfaction, and optimism about staying in the profession. Design Cross-sectional, descriptive study. Setting Web-based survey. Participants 1,111 participants responded to survey for an 11.5% response rate (n=1,111/12,810). Partial responses were included; thus, number of respondents varied by question. Data Collection and Analysis A web-based survey comprised of 34 closed-ended questions was disseminated to NATA members in November 2022. Descriptive statistics including counts and frequencies for each surveyed item in addition to chi-square tests were used to analyze responses. Results 30.2% (n=335/1,111) of YPATs changed employers and 25.7% (n=286/1,111) reported a change in employment setting. Notably, YPATs migrated from high school, collegiate, and professional athletics to clinic, industrial, or sales settings. Overall, the pandemic decreased job satisfaction, career satisfaction, and optimism about professional longevity. Changes in employment had a positive impact on personal well-being in addition to job and career satisfaction, but not optimism regarding staying in profession. Salary and work-life balance were reported as important predictors of short and long-term retention for YPATs. Conclusions The pandemic brought changes to employers and employment settings for many YPATs. To reduce turnover and attrition, administrators must begin to prioritize employee well-being and satisfaction. In addition to increasing salary and reducing workload, employers should recognize the importance of administrative support in promoting well-being together with satisfaction and professional optimism in early professional and career-advancing athletic trainers.

Self-Care Strategies of Athletic Trainers

Abstract:Purpose: Strategies for reducing burnout and improving work-life balance have been recommended for both organizations and individual athletic trainers. Little is known about what types of self-care (SC) strategies are already being used by athletic trainers. The purpose of this study was to explore and describe the number and type of SC strategies used by athletic trainers on a weekly basis. Methods: We used a cross-sectional design with web-based survey to explore the SC strategies used by athletic trainers. The survey instrument consisted of a SC checklist which asked the participants to select all the SC strategies they engage in weekly with 17 potential options, including an opportunity to describe intensity of exercise and also an “other” option with the ability to describe additional strategies. Results: Participants were mostly female, in their mid-career (age = 33 ± 9 y; experience = 10 ± 8 y), predominantly White, working in the college/university setting, holding a professional master’s degree. Participants engage in 6.9±2.8 SC strategies per week (range=1-15; mode=6). They most often engage in positive social relationships (n=522, 72.5%), humor (n=477, 66.3%), laughter (n=476, 66.1%), hydration (n=472, 65.6%), and moderate exercise (n=418, 58.1%). Participants most often selected physical (2.6±1.1; range=1- 4; mode=3, n=681, 94.6%), interpersonal (2.6±1.0; range=1-3; mode=3, n=665, 92.4%) or contemplative and meditative (2.1±1.3; range=1-5; mode=1, n=641, 89.0%) SC strategies to use on a weekly basis. Creative strategies were used the least frequently (1.0±0.2; mode=1, n=341, 47.4%). Only 25 (3.5%) participants indicated they engaged in no SC strategies weekly. There were no statistically or clinically significant differences between groups of gender identities or early and late career athletic trainers when compared to weekly use of contemplative and meditative, physical, interpersonal or creative SC strategies. Conclusions: Physical and interpersonal SC strategies are most popular among athletic trainers. Athletic trainers should continue to engage in SC strategies, as wellbeing positively impacts connections to the workplace. Organizations should implement workplace wellness and employee assistance programs that embrace common SC strategies.

Abstract:BACKGROUND Autoimmune hepatitis (AIH) is a chronic inflammatory condition of the liver of unknown etiology. Its epidemiological and anatomoclinical characteristics and its outcome were unknown in Tunisia. AIM To analyse epidemiological, anatomoclinical, immunological and histological aspects of AIH and to determine factors predicting relapse after treatment and death of this disease in Tunisia. METHODS Patients presenting with AIH between January 1996 and December 2004 were evaluated in retrospective multicentric study. The diagnosis of AIH was established according to the criteria of the revised score of the international autoimmune hepatitis group (1999) RESULTS Eighty three patients were identified (70 female; mean age=49+17.9 years). 63% presented probable AIH and 37% presented definite AIH. Thirty two percent presented with the acute pattern. Eighty three per cent of cases were type I AIH and 5 % of cases were type II HAI. Fifty seven percent of the patients were cirrhotic at presentation. Associated autoimmune diseases was seen in 27 patients, dominated by diabetes, autoimmune thyroiditis and Sjögren's syndrome. An overlap syndrome was diagnosed in 25% of cases; primary biliary Cirrhosis-AIH in 20% of cases and primary sclerosing cholangitis-AIH in 5% of cases. Fifty patients were treated by glucocorticoids as monotherapy or in combination with azathioprine. Complete remission was achieved in 90% of cases. Fourteen percent relapsed within a median time of 12 months. Factors associated with relapse were: treatment with Azathioprine<18 months, absence of lobular necrosis and anti-nuclear antibody (+) profile. Mortality was observed in 17 % of cases. Factors associated with death were encephalopathy as an independent factor and treatment with Azathioprine<18 months. CONCLUSION In Tunisia, epidemiological and clinical characteristics of AIH were similar to those reported in the literature but with a higher frequency of cirrhosis at presentation. Treatment with Azathioprine < 18 months was the main factor associated with relapse and represented with encephalopathy a factor associated with death.
Professional Master's Athletic Training Students' Career Influences Part I: Perceptions of Athletic Training

Abstract:Context As athletic training education transitions to a professional master's degree, understanding the perceptions professional master's athletic training students have of athletic training is important. Objective To examine second-year professional master's students' perceptions of the athletic training profession, a career in the profession, and identify the factors that influence their perceptions. Design Convergent mixed methods. Setting Online surveys and individual phone interviews. Patients or Other Participants A total of 80 second-year professional master's students (63 female, 13 male, 4 no response, age = 24.63 ± 2.29 years), who were enrolled in the final semester of their program in the spring of 2019, completed the online survey. Ten survey respondents completed the follow-up phone interviews. Data Collection and Analysis A survey instrument and a semistructured interview guide were developed to answer the research questions. Both were validated externally by 3 independent researchers using a content-validity indexing tool. The data from the survey and interviews were woven together and merged to provide 1 comprehensive results section. Trustworthiness was established using triangulation, member checks, memos, and peer debriefing. Results Three higher-order themes emerged from the data: (1) perceptions of the athletic training profession: lack of appreciation and awareness for the profession from others, rewarding profession, and dynamic profession; (2) perceptions of a career in athletic training: low pay, long hours and inconsistent schedules, and inability to have work-life balance; (3) factors influencing perceptions: clinical experiences and interactions with athletic trainers. Conclusions Second-year professional master's athletic training students developed positive and negative perceptions about the profession and a career in the profession during their professional education experiences. The factors identified were professional socializing agents and should be considered when designing clinical education experiences.
Seasonal Accumulated Workloads in Collegiate Women’s Soccer: A Comparison of Starters and Reserves

Abstract:Research quantifying the unique workload demands of starters and reserves in training and match settings throughout a season in collegiate soccer is limited. Purpose: The purpose of the current study is to compare accumulated workloads between starters and reserves in collegiate soccer. Methods: Twenty-two NCAA Division III female soccer athletes (height: 1.67 ± 0.05 m; body mass: 65.42 ± 6.33 kg; fat-free mass: 48.99 ± 3.81 kg; body fat %: 25.22 ± 4.78%) were equipped with wearable global positioning systems with on-board inertial sensors, which assessed a proprietary training load metric and distance covered for each practice and 22 matches throughout an entire season. Nine players were classified as starters (S), defined as those playing >50% of playing time throughout the entire season. The remaining 17 were reserves (R). Goalkeepers were excluded. A one-way ANOVA was used to determine the extent of differences in accumulated training load throughout the season by player status. Results: Accumulated training load and total distance covered for starters were greater than reserves ((S: 9431 ± 1471 vs. R: 6310 ± 2263 AU; p < 0.001) and (S: 401.7 ± 31.9 vs. R: 272.9 ± 51.4 km; p < 0.001), respectively) throughout the season. Conclusions: Starters covered a much greater distance throughout the season, resulting in almost double the training load compared to reserves. It is unknown if the high workloads experienced by starters or the low workloads of the reserves is more problematic. Managing player workloads in soccer may require attention to address potential imbalances that emerge between starters and reserves throughout a season.
