Abstract:OBJECTIVES The aim of this study was to evaluate optical coherence tomography (OCT) and intravascular ultrasound (IVUS) versus coronary angiography in the assessment of target lesion calcification and its effect on stent expansion.BACKGROUND IVUS is more sensitive than angiography in the detection of coronary artery calcium, but the relationship among IVUS, OCT, and angiography has not been studied.METHODS Overall, 440 lesions (440 patients with stable angina) underwent OCT- and IVUS-guided stent implantation. Coronary calcification was evaluated using: 1) angiography; 2) IVUS (maximum calcium angle and the surface pattern); and 3) OCT (mean and maximum calcium angle, calcium length, and maximum calcium thickness).RESULTS Median patient age was 66 years, and 82.5% were men. Among 440 lesions, calcium was detected by angiography in 40.2%, IVUS in 82.7%, and OCT in 76.8%. The maximum calcium angle, maximum calcium thickness, and calcium length by OCT or IVUS increased in relation to the increasing severity of angiographically visible calcium. In 13.2% of lesions with IVUS-detected calcium, calcium was either not visible or was underestimated (> 90 degrees smaller maximum arc) by OCT mostly due to superficial OCT plaque attenuation. In 21.6% of lesions with IVUS calcium angle > 180 degrees, angiography did not detect any calcium; these lesions had thinner and shorter calcium deposits as assessed using OCT, and final minimum stent area was larger compared to those with angiographically visible calcium. In lesions with thinner calcium deposits by OCT, IVUS detected a smooth surface with reverberations whereas thick calcium deposits were associated with an irregular surface without reverberations.CONCLUSIONS Angiographic detection of target lesion coronary calcium (compared to intravascular imaging) has not changed in the past 2 decades, and angiographically invisible calcium (only detectable by IVUS or OCT) did not appear to inhibit stent expansion. Experimental Study on Anastomotic Hemodynamics Using Tomographic Particle Image Velocimetry

Abstract:Fluid mechanics is an important factor to the failure of the graft anastomosis surgery. In order to better understand the mechanism of restenosis and graft failure of the anastomosis from the perspective of fluid mechanics, an end-toside silicone anastomosis model with 45° anastomotic angle was used for research. The anastomosis model was connected to a continuous flow loop. where the flow rate can be adjusted during the experiment to simulate several conditions. The tomographic particle image velocimetry (TPIV) was applied to obtain the internal three-dimensional and three-component (3D-3C) flow field. The hemodynamics inside the anastomosis model was quantitatively analyzed based on the averaged 3D-3C flow field. The results present that there are phenomena such as recirculation, spiral flow, flow stagnation, and large vortex structures inside the anastomotic flow field. The oscillation of the recirculation zone size and flow stagnation point indicate the fluid stimulation to the endothelial cells. Also, the double spiral vortex structure could cause high local shear stress, which in turn cause trauma to the red blood cells and lead to local coagulation. Besides, thanks to the 3D-3C flow field, the wall shear stress (WSS) on the entire model wall was obtained, which enables us to predict the mechanical influence to the intimal hyperplasia. The local low WSS was found near the graft toe and recirculation regions, while the local high WSS was found on the host vessel bottom where the jet directly impinges. The abnormal flow phenomena can explain the high-risk regions in the anastomosis, where the stenosis and intimal hyperplasia are prone to appear. The 3D-3C flow field based on the in vitro experiment can strengthen the understanding of the anastomotic restenosis pathology and provide further reference for the surgery improvement.

Abstract:Ascorbic acid treatmentL of animals exposed to cold, and enhances the survival of animals prevents the adrenal hypertrophy typicalexposed to this stressor (Dugal and Therien, 1949). It was observed that ascorbic acid pretreatment prevented the eosinopenia and the depletion of adrenal cholesterol typical of acute epinephrin stress (Bacchus and Toompas, 1951). This paper reports data which indicate that ascorbic acid prevents the eosinophil response to epinephrin, but not to corticotrophin. Male albino mice were used in these experiments. Groups I and III consisted of mice which received pretreatment with ascorbic acid 12 hours prior to being stressed. Abstract:Abstract The structural morphology of coronary stents (e.g. stent expansion, lumen scaffolding, strut apposition, tissue protrusion, side branch jailing, strut fracture), and the local hemodynamic environment after stent deployment are key determinants of procedural success and subsequent clinical outcomes. High-resolution intracoronary imaging has the potential to enable the geometrically accurate three-dimensional (3D) reconstruction of coronary stents. The aim of this work was to present a novel algorithm for 3D stent reconstruction of coronary artery stents based on optical coherence tomography (OCT) and angiography, and test experimentally its accuracy, reproducibility, clinical feasibility, and ability to perform computational fluid dynamics (CFD) studies. Our method has the following steps: 3D lumen reconstruction based on OCT and angiography, stent strut segmentation in OCT images, packaging, rotation and straightening of the segmented struts, planar unrolling of the segmented struts, planar stent wireframe reconstruction, rolling back of the planar stent wireframe to the 3D reconstructed lumen, and final stent volume reconstruction. We tested the accuracy and reproducibility of our method in stented patient-specific silicone models using micro-computed tomography (μCT) and stereoscopy as references. The clinical feasibility and CFD studies were performed in clinically stented coronary bifurcations. The experimental and clinical studies showed that our algorithm (1) can reproduce the complex spatial stent configuration with high precision and reproducibility, (2) is feasible in 3D reconstructing stents deployed in bifurcations, and (3) enables CFD studies to assess the local hemodynamic environment within the stent. Notably, the high accuracy of our algorithm was consistent across different stent designs and diameters. Our method coupled with patient-specific CFD studies can lay the ground for optimization of stenting procedures, patient-specific computational stenting simulations, and research and development of new stent scaffolds and stenting techniques.

Abstract:BackgroundCoronary obstruction (CO) is a rare but devasting complication during transcatheter aortic valve replacement (TAVR).ObjectivesWe aim to demonstrate that the predicted distance between the coronary ostia and the closest structure derived with patient-specific computer simulation is associated with CO risk during TAVR.MethodsWe retrospectively analysed 14 aortic stenosis patients who underwent TAVR through finite element simulation. The frame deformation predicted with patient-specific computer simulation was qualitatively and quantitatively compared to the post-operative device deformation. The minimum distance between each coronary ostium and the closest structure was calculated and compared in patients who developed CO, at high risk of CO, and at no risk of CO.ResultsFour patients experienced CO during TAVR, 5 patients were at high risk of CO, and the remaining 5 patients had no risk of CO. A high coefficient of determination was obtained for all measurements extracted from the simulated device and the post-operative device (≥0.95). Simulations predicted shorter distance between the coronary ostium and the closest structure in patients who experienced CO, compared to patients at high risk of CO or who did not experience this complication (right coronary: 5.9 vs. 6.8 vs. 8.8 mm, left coronary: 3.0 vs. 3.3 vs. 6.5 mm respectively).ConclusionsThe distance between the coronary ostium and the closest structure was lower in patients who experienced CO during TAVR through patient-specific computational simulation. This technology enables coronary obstruction analysis before TAVR in the future.

Abstract:The structural morphology of coronary stents and the local hemodynamic environment following stent deployment in coronary arteries are crucial determinants of procedural success and subsequent clinical outcomes. High-resolution intracoronary imaging has the potential to facilitate geometrically accurate three-dimensional (3D) reconstruction of coronary stents. This work presents an innovative algorithm for the 3D reconstruction of coronary artery stents, leveraging intravascular ultrasound (IVUS) and angiography. The accuracy and reproducibility of our method were tested in stented patient-specific silicone models, with micro-computed tomography serving as a reference standard. We also evaluated the clinical feasibility and ability to perform computational fluid dynamics (CFD) studies in a clinically stented coronary bifurcation. Our experimental and clinical studies demonstrated that our proposed algorithm could reproduce the complex 3D stent configuration with a high degree of precision and reproducibility. Moreover, the algorithm was proved clinically feasible in cases with stents deployed in a diseased coronary artery bifurcation, enabling CFD studies to assess the hemodynamic environment. In combination with patient-specific CFD studies, our method can be applied to stenting optimization, training in stenting techniques, and advancements in stent research and development.

Abstract:Coronary stenting or percutaneous coronary intervention (PCI) is widely used to treat coronary artery disease. Improper deployment of stents may lead to post-PCI complication, in-stent restenosis, stent fracture and stent thrombosis. Intravascular optical coherence tomography (OCT) with micron-scale resolution provides accurate in vivo assessment of stent apposition/malapposition and neointima coverage. However, manual stent analysis is labor intensive and time consuming. Existing automated methods with intravascular OCT mainly focused on stent struts with thin tissue coverage. We developed a deep learning method to automatically analyze stents with both thin (≤0.3mm) and very thick tissue coverage (>0.3mm), and an algorithm to accurately analyze stent area for vessels with multiple stents. 25203 images from 56 OCT pullbacks and 41 patients were analyzed. Three-fold cross-validation demonstrated that the algorithm achieved a precision of 0.932±0.009 and a sensitivity of 0.939±0.007 for stents with ≤0.3mm tissue coverage, and a precision of 0.856±0.019 and a sensitivity of 0.874±0.011 for stents with >0.3mm tissue coverage. The correlation between the automatically computed and manually measured stent area is 0.954 (p<0.0001) for vessels with a single stent, and is 0.918 (p<0.0001) for vessels implanted with multiple stents. The proposed method can accurately detect stent struts with very thick tissue coverage and analyze stent area in vessels implanted with multiple stents, and can effectively facilitate the evaluation of stent implantation and post-stent tissue coverage.

Holistic Structural Visualization of Coronary Arteries with Single-catheter Dual-modality Intravascular Imaging Combining IVUS and OFDI

Abstract:Summary Objectives To test the imaging performance and practical use of a novel dual-modality intravascular imaging system combining intravascular ultrasound (IVUS) and optical coherence tomography (OCT) into a single catheter. Background IVUS enables assessing coronary plaque burden, a robust metric for patient prognosis, while OCT and OFDI provide high-resolution images of coronary microstructure and detailed assessment of stent implantation. Owing to their complementary strengths, co-registering IVUS and OFDI provides a more comprehensive assessment of coronary lesions during PCI. Methods We developed a 2.6Fr imaging catheter integrating both IVUS and OFDI, interfacing to a dual-modality imaging console through a fast interchange connector. A novel algorithm fuses the IVUS and OFDI signals into a single combined image. We verified the performance of the two modalities and their visualization by imaging of cadaveric coronary arteries and tested practical imaging in the catheterization laboratory in swine in vivo . Results Coronary atherosclerotic lesions of cadaver hearts revealed complementary optical and acoustic image features. Spatial co-registration of the modalities was confirmed by high correlation of measured lumen areas. Fused into a combined visualization, dual-modality imaging offers quantitative characterization of lesions, including plaque-burden and fibrous cap thickness. Imaging in vivo does not add procedural complexity compared to conventional single modality imaging. Conclusion Dual-modality IVUS/OFDI imaging with fused visualization provides improved assessment of coronary atherosclerotic lesions and is compatible with a routine clinical setting. Combining the strength of the two modalities offers unique opportunities both as a powerful research instrument and to improve clinical management of patients undergoing PCI. Visual abstract Highlights ▪ A catheter-based imaging system integrating ultrasound and optical frequency domain imaging enables real time intravascular holistic structural visualization (IV-HSV) of coronary arterial lesions with a single pullback during cardiac catheterization procedures. ▪ A computational method to automatically fuse images from both modalities generates a single complete map of vessel wall structures - the IV-HSV image, which offers holistic investigation of coronary plaque key features, such as fibrous cap thickness, calcification, and plaque burden. ▪ The visualization of plaque morphology along the entire extent of the coronary arterial wall reveals vital information for guiding percutaneous coronary interventions and for advancing our understanding of the pathophysiology of coronary artery diseases. Condensed abstract To facilitate a holistic investigation of coronary arterial lesions for both basic research and clinical interventions, this work presents a catheter-based imaging system integrating IVUS and OFDI, as well as a new rendering method that computationally fuses the intrinsically co-registered images from both modalities into a single cross-sectional map of vessel structures. Imaging human cadaveric coronary arteries shows the benefit of this system by revealing near-lumen microstructures and spatially correlated macrostructures deep inside the vessel wall. The co-registration accuracy and operation in a clinical setting of this system was demonstrated through swine catheterization in vivo .

Abstract:As experts continue to debate the optimal surgery practice for coronary disease - percutaneous coronary intervention (PCI) or coronary aortic bypass graft (CABG) - computational tools may provide a quantitative assessment of each option. Computational fluid dynamics (CFD) has been used to assess the interplay between hemodynamics and stent struts; it is of particular interest in Bioresorbable Vascular Stents (BVS), since their thicker struts may result in impacted flow patterns and possible pathological consequences. Many proofs of concept are presented in the literature; however, a practical method for extracting patient-specific stented coronary artery geometries from images over a large number of patients remains an open problem. This work provides a possible pipeline for the reconstruction of the BVS. Using Optical Coherence Tomographies (OCT) and Invasive Coronary Angiographies (ICA), we can reconstruct the 3D geometry of deployed BVS in vivo. We illustrate the stent reconstruction process: (i) automatic strut detection, (ii) identification of stent components, (iii) 3D registration of stent curvature, and (iv) final stent volume reconstruction. The methodology is designed for use on clinical OCT images, as opposed to approaches that relied on a small number of virtually deployed stents. The proposed reconstruction process is validated with a virtual phantom stent, providing quantitative assessment of the methodology, and with selected clinical cases, confirming feasibility. Using multimodality image analysis, we obtain reliable reconstructions within a reasonable timeframe. This work is the first step toward a fully automated reconstruction and simulation procedure aiming at an extensive quantitative analysis of the impact of BVS struts on hemodynamics via CFD in clinical trials, going beyond the proof-of-concept stage.

Abstract:Detection of stent struts imaged in vivo by optical coherence tomography (OCT) after percutaneous coronary interventions (PCI) and quantification of in-stent neointimal hyperplasia (NIH) are important. In this paper, we present a new computational method to facilitate the physician in this endeavor to assess and compare new (drug-eluting) stents. We developed a new algorithm for stent strut detection and utilized splines to reconstruct the lumen and stent boundaries which provide automatic measurements of NIH thickness, lumen and stent area. Our original approach is based on the detection of stent struts unique characteristics: bright reflection and shadow behind. Furthermore, we present for the first time to our knowledge a rotation correction method applied across OCT cross-section images for 3D reconstruction and visualization of reconstructed lumen and stent boundaries for further analysis in the longitudinal dimension of the coronary artery. Our experiments over OCT cross-sections taken from 7 patients presenting varying degrees of NIH after PCI illustrate a good agreement between the computer method and expert evaluations: Bland-Altmann analysis revealed a mean difference for lumen cross-section area of 0.11±0.70mm2 and for the stent cross-section area of 0.10±1.28mm2.

Abstract:Intracoronary optical coherence tomography (OCT) and intravascular ultrasound (IVUS) are two popular techniques for the detection and determination of atherosclerosis. IVUS allows visualization of plaques while also providing a large penetration depth to determine plaque volume. Intracoronary OCT provides the ability to capture microscopic features associated with high risk plaque. Traditionally to utilize the benefits of both modalities, separate probes and systems had to be used one at a time to image a vessel. We present work required to create a combined OCT IVUS system capable of simultaneous imaging to detect atherosclerotic plaques. A novel integrated probe of size 0.69 mm OD featuring sequential placement of components was created to acquire co-registered images within small coronary vessels. By utilizing commercial graphics processing units (GPUs) real time visualization of acquired data is possible up to a maximum 48 frames per second per channel. In vitro studies on human coronary artery samples as well as in vivo studies in rabbits and pigs show various plaque buildups in both OCT and IVUS images which match histology results, demonstrating the capabilities of the system.
Fully automated construction of three-dimensional finite element simulations from Optical Coherence Tomography

Abstract:Despite recent advances in diagnosis and treatment, atherosclerotic coronary artery diseases remain a leading cause of death worldwide. Various imaging modalities and metrics can detect lesions and predict patients at risk; however, identifying unstable lesions is still difficult. Current techniques cannot fully capture the complex morphology-modulated mechanical responses that affect plaque stability, leading to catastrophic failure and mute the benefit of device and drug interventions. Finite Element (FE) simulations utilizing intravascular imaging OCT (Optical Coherence Tomography) are effective in defining physiological stress distributions. However, creating 3D FE simulations of coronary arteries from OCT images is challenging to fully automate given OCT frame sparsity, limited material contrast, and restricted penetration depth. To address such limitations, we developed an algorithmic approach to automatically produce 3D FE-ready digital twins from labeled OCT images. The 3D models are anatomically faithful and recapitulate mechanically relevant tissue lesion components, automatically producing morphologies structurally similar to manually constructed models whilst including more minute details. A mesh convergence study highlighted the ability to reach stress and strain convergence with average errors of just 5.9% and 1.6% respectively in comparison to FE models with approximately twice the number of elements in areas of refinement. Such an automated procedure will enable analysis of large clinical cohorts at a previously unattainable scale and opens the possibility for in-silico methods for patient specific diagnoses and treatment planning for coronary artery disease.

Abstract:Intravascular ultrasound (IVUS)-based reconstructions have been traditionally used to examine the effect of endothelial shear stress (ESS) on neointimal formation. The aim of this analysis is to compare the association between ESS and neointimal thickness (NT) in models obtained by the fusion of optical coherence tomography (OCT) and coronary angiography and in the reconstructions derived by the integration of IVUS and coronary angiography. We analyzed data from six patients implanted with an Absorb bioresorbable vascular scaffold that had biplane angiography, IVUS and OCT investigation at baseline and 6 or 12 months follow-up. The IVUS and OCT follow-up data were fused separately with the angiographic data to reconstruct the luminal morphology at baseline and follow-up. Blood flow simulation was performed on the baseline reconstructions and the ESS was related to NT. In the OCT-based reconstructions the ESS were lower compared to the IVUS-based models (1.29 ± 0.66 vs. 1.87 ± 0.66 Pa, P = 0.030). An inverse correlation was noted between the logarithmic transformed ESS and the measured NT in all the OCT-based models which was higher than the correlation reported in five of the six IVUS-derived models (−0.52 ± 0.19 Pa vs. −0.10 ± 0.04, P = 0.028). Fusion of OCT and coronary angiography appears superior to IVUS-based reconstructions; therefore it should be the method of choice for the study of the effect of the ESS on neointimal proliferation.
Finite element analysis of stent implantation in a three-dimensional reconstructed arterial segment

Abstract:Endovascular stent deployment is a mechanical procedure used to rehabilitate a diseased arterial segment by restoring blood flow in occluded regions. The success or failure of the stent implantation depends on the stent device and the deployment technique. The optimal stent deployment can be predicted by investigating the factors that influence this minimally invasive procedure. In this study, we propose a methodology which evaluates the alterations in the arterial environment caused by stent deployment. A finite element model of a reconstructed right coronary artery with a stenosis was created based on anatomical information provided by intravascular ultrasound and angiography. The model was used to consider placement and performance after intervention with a commercially available Leader Plus stent. The performance of the stent, within this patient-specific arterial segment is presented, as well as the induced arterial deformation and straightening. The arterial stress distribution is analyzed with respect to possible regions of arterial injury. Our approach can be used to optimize stent deployment and to provide cardiologists with a valuable tool to visually select the position and deploy stents in patient-specific reconstructed arterial segments, thereby enabling new methods for optimal cardiovascular stent positioning.

Abstract:Angioplasty with stent placement is a widely used treatment strategy for patients with stenotic blood vessels. However, it is often challenging to predict the outcomes of this procedure for individual patients. Image-based computational fluid dynamics (CFD) is a powerful technique for making these predictions. To perform CFD analysis of a stented vessel, a virtual model of the vessel must first be created. This model is typically made by manipulating two-dimensional contours of the vessel in its pre-stent state to reflect its post-stent shape. However, improper contour-editing can cause invalid geometric artifacts in the resulting mesh that then distort the subsequent CFD predictions. To address this limitation, we have developed a novel shape-editing method that deforms surface meshes of stenosed vessels to create stented models.

Abstract:Zika virus (ZIKV) is an emerging mosquito-transmitted flavivirus that shares a considerable degree of homology with dengue virus (DENV). Here, we examined longitudinal antibody response against ZIKV during natural infection in 2 convalescent individuals. By decomposing the antibody recognition into DI/DII and DIII of the E glycoprotein, we showed their development in humans followed a spatiotemporal hierarchy. Plasma binding to DI/DII appeared to peak and wane during early infection with extensive cross-reactivity with DI/DII of DENV. Binding to DIII, however, peaked early but persisted months into the infection without detectable cross-reactivity with DIII of DENV. A clear trend of increase in DIII-specific neutralizing activity was observed over the course of infection. mAbs isolated during early infection are largely DI/DII specific, weakly neutralizing, and highly cross-reactive with DENV, while those from later infection are more diverse in recognition, potently neutralizing, and ZIKV specific. The most potent neutralizing mAb targeting the DIII provided 100% protection in mice from lethal ZIKV infection and could therefore serve as a promising candidate for antibody-based therapy and prevention. The dynamic features unveiled here will assist us to better understand the pathogenesis of ZIKV infection and inform rational design of vaccines.
Value of Combined Optical Coherence Tomography and Optical Flow Ratio Measurements after Percutaneous Coronary Intervention

Abstract:Background: Optical flow ratio (OFR) is a novel computational fractional flow reserve derived from optical coherence tomography (OCT). However, the impact of combining post-stenting morphology (OCT) and physiology (OFR) remains largely unknown. Method: OCT and OFR were analyzed at the independent core laboratory. Target lesion failure (TLF) was defined as the composite of cardiac death, target lesion myocardial infarction and target lesion revascularization. Suboptimal stent deployment was identified with at least one TLF-related OCT or OFR characteristics. Results: A total of 448 ACS patients (459 vessels) were assessed. Stent expansion<80%, MSA<4.5 mm2 and stent edge lipid-rich plaque and OFR<0.90 were independent predictors of TLR (all p value<0.001). Patients with OCT-suboptimal [adjusted hazard ratio (HR): 7.88, 95% CI: 2.73-22.72, p<0.001] or OFR-suboptimal (adjusted HR: 5.78, 95% CI: 2.54-13.14, p<0.001) stent deployment showed significantly higher risk of TLF compared to those with optimal stent deployment with a significant interaction (pinteraction<0.001). OCT and OFR both-suboptimal stent deployment was confirmed as an independent predictor of TLF (adjusted HR: 9.39, 95% CI: 4.25-20.76, p<0.001). Conclusion: Combined OCT and OFR conferred an optimal reclassification of stent deployment, which may aid in decision-making regarding a tailored PCI strategy for optimal stent deployment.

Abstract:Aim: In endovascular ICAD treatment, an individualized patient-specific treatment pathway may be very helpful. Patient selection for endovascular treatment based on anatomical markers has not been shown to be effective in past clinical trials. This pilot study combines CT perfusion (CTP) analysis and computational fluid dynamics to identify hemodynamically significant patients for a reliable pathway for intervention planning. Methods: We assessed patients from two stroke centers with 50-99% ICAD lesions. Using a novel technique, CTP scans generated acute vascular territory maps for major cerebral arteries (ACA,MCA,PCA) with perfusion Time-to-peak (TTP) maps. Patient-specific blood flow simulations using physiology-informed boundary conditions calculated stenotic pressure ratios for both the patient’s acute and a population-based healthy scenario. The changes in affected territory volumes and pressure ratio between healthy and acute conditions and the acute TTP delays were presented to an expert reader who stratified cases into three clinical groups (A) non-hemodynamic, (B) hemodynamically hazardous with insufficient collateral support and (C) hemodynamically stable due to collateralisation. These results were compared against patient outcomes of recurrent ischemic events attributed to the target stenosis using the commonly used 70% threshold. Results: (95%CI in brackets) Patients (n=47, age: 73±11Y, 23F/24M) were stratified across the three categories A) 30, B) 15, C) 2. Of the patient groups with follow-up (n=31), categorisation of hemodynamic ICAD against recurrent ipsilateral stroke in territory (n=8) yielded a sensitivity of 75% [34-97] and specificity of 91% [72-98]. Mean±standard deviation of stenosis degree (SD) 55±14% for patients across each cohort was A) 50±11%, B) 70±9%, C) 70%. This is compared to using 70% stenosis threshold for dichotomising patients against risk of recurrent stroke, which yielded a sensitivity of 62.5% [24-91], and specificity of 87% [66-97]. Conclusions: We report on a CTP derived method for stratifying hemodynamically significant ICAD. The methodology has potential in both in assessing symptomatic chronic ICAD and identifying cohorts who would benefit from endovascular intervention.

Abstract:Abstract The three-dimensional (3D) representation of the bifurcation anatomy and disease burden is essential for better understanding of the anatomical complexity of bifurcation disease and planning of stenting strategies. We propose a novel methodology for 3D reconstruction of coronary artery bifurcations based on the integration of angiography, which provides the backbone of the bifurcation, with optical coherence tomography (OCT), which provides the vessel shape. Our methodology introduces several technical novelties to tackle the OCT frame misalignment, correct positioning of the OCT frames at the carina, lumen surface reconstruction, and merging of bifurcation lumens. The accuracy and reproducibility of the methodology were tested in n = 5 patient-specific silicone bifurcations compared to contrast-enhanced micro-computed tomography (μCT), which was used as reference. The feasibility and time-efficiency of the method were explored in n = 7 diseased patient bifurcations of varying anatomical complexity. The OCT-based reconstructed bifurcation models were found to have remarkably high agreement compared to the μCT reference models, yielding r 2 values between 0.91 and 0.98 for the normalized lumen areas, and mean differences of 0.005 for lumen shape and 0.004 degrees for bifurcation angles. Likewise, the reproducibility of our methodology was remarkably high. Our methodology successfully reconstructed all the patient bifurcations yielding favorable processing times (average lumen reconstruction time < 60 min). Overall, our method is an easily applicable, time-efficient, and user-friendly tool that allows accurate and reproducible 3D reconstruction of coronary bifurcations. Our technique can be used in the clinical setting to provide information about the bifurcation anatomy and plaque burden, thereby enabling planning, education, and decision making on bifurcation stenting.

Abstract:Optical coherence tomography (OCT) is an emerging high-resolution intravascular imaging modality that can provide physicians with critical information, thereby enabling precise characterization of plaque morphology and luminal geometry and facilitating pre-intervention lesion assessment. As OCT has a higher sensitivity for lipid-rich plaque characterization than intravascular ultrasound, vulnerable plaque detection by OCT has thus been investigated. By evaluating both the calcium thickness and arc, OCT can be the ideal method for determining both the indication and endpoint of rotational atherectomy for calcified lesions prior to stent implantation. OCT has become applicable for the optimization of stent implantation with immediate and semi-automatic quantification of stent apposition and expansion to achieve potentially better clinical outcomes. In bifurcation lesions, OCT allows the visualization of the stent-link location overhanging the side-branch ostium and the guidewire recrossing point prior to the final kissing balloon inflation through three-dimensional reconstructed OCT images, providing us with deep insights into the mechanical optimization of stent struts. Furthermore, recent studies have reported several OCT-derived predictors of adverse clinical events. Important limitations of OCT, including the excessive contrast volume needed and observation of aorto-ostial lesions, may partially be overcome through the use of low-molecular-weight dextran and a guide extension catheter. The clinical applications of OCT have been expanding, and evidence on its clinical utility has been accumulating.
