Abstract:The hairy elbows syndrome (HES) is a rare congenital phenotype characterized by an abnormal increase in long hairs localized on the upper limbs extensor surfaces. This feature is often associated with short stature, facial asymmetry, dysmorphisms, intrauterine growth retardation (IUGR), and mental and speech delay. We report a case with hypertricosis cubiti associated with infantile spasms, behaviour disorders and cerebral hemisphere asymmetry. Although these findings have not been previously described we are uncertain whether they are unusual or underestimated. However, it is likely that these neurological findings are strongly interrelated leading to a more severe phenotype of the syndrome.
Expression of constitutively active alpha-PAK reveals effects of the kinase on actin and focal complexes

Abstract:The family of p21-activated protein kinases (PAKs) appear to be present in all organisms that have Cdc42-like GTPases. In mammalian cells, PAKs have been implicated in the activation of mitogen-activated protein kinase cascades, but there are no reported effects of these kinases on the cytoskeleton. Recently we have shown that a Drosophila PAK is enriched in the leading edge of embryonic epithelial cells undergoing dorsal closure (N. Harden, J. Lee, H.-Y. Loh, Y.-M. Ong, I. Tan, T. Leung, E. Manser, and L. Lim, Mol. Cell. Biol. 16:1896-1908, 1996), where it colocalizes with structures resembling focal complexes. We show here by transfection that in epithelial HeLa cells alpha-PAK is recruited from the cytoplasm to distinct focal complexes by both Cdc42(G12V) and Rac1(G12V), which themselves colocalize to these sites. By deletion analysis, the N terminus of PAK is shown to contain targeting sequences for focal adhesions which indicate that these complexes are the site of kinase function in vivo. Cdc42 and Rac1 cause alpha-PAK autophosphorylation and kinase activation. Mapping alpha-PAK autophosphorylation sites has allowed generation of a constitutively active kinase mutant. By fusing regions of Cdc42 to the C terminus of PAK, activated chimeras were also obtained. Plasmids encoding these different constitutively active alpha-PAKs caused loss of stress fibers when introduced into both HeLa cells and fibroblasts, which was similar to the effect of introducing Cdc42(G12V) or Rac1(G12V). Significantly dramatic losses of focal adhesions were also observed. These combined effects resulted in retraction of the cell periphery after plasmid microinjection. These data support our previous suggestions of a role for PAK downstream of both Cdc42 and Rac1 and indicate that PAK functions include the dissolution of stress fibers and reorganization of focal complexes.
Sleep Disturbances and Risk of Falls in an Old Chinese Population-Rugao Longevity and Ageing Study.

Abstract:Background: To explore the relationship between sleep disturbances and falls in an elderly Chinese population. Methods: Data from 1726 individuals aged 70-87 years from the Rugao Longevity and Ageing Study were used. The Pittsburgh Sleep Quality Index (PSQI) was used to assess sleep variables. Outcomes were falls >= 1 time per year and falls >= 2 times per year. Results: A total of 22.7% of the participants experienced >= 1 fall, and 9.8% experienced >= 2 falls per year. Poor sleep quality was associated with >= 1 fall (OR 1.08, 95% CI 1.05-1.12; OR 1.27, 95% CI 1.14-1.41) and >= 2 falls (OR 1.08, 95% CI 1.03-1.14; OR 1.28, 95% CI 1.10-1.48), with an increase per PSQI score and SD PSQI score, respectively. In addition, sleep quality, sleep latency, sleep efficiency, and sleep disturbance subcomponents were associated with an increased risk of >= 1 fall with ORs of 1.44 (95% CI, 1.21-1.72), 1.23 (95% CI, 1.09-1.40), 1.12 (95% CI, 1.01-1.23) and 1.70 (95% CI, 1.35-2.14), respectively, and were associated with an increased risk of >= 2 falls with ORs 1.54 (95% CI, 1.22-1.96), 1.21(95% CI, 1.02-1.44), 1.17 (95% CI 1.02-1.33), and 1.78 (95% CI, 1.31-2.44), respectively. Further, participants slept <= 5 h per night had an increased risk of >= 1 fall (OR 2.34; 95% CI, 1.59-3.46) and >= 2 falls (OR 2.19; 95% CI, 1.30-3.69). Conclusions: Poor sleep quality and several subcomponent sleep symptoms were consistently associated with increased risk of falls >= 1 time and >= 2 times in Chinese elderly. The identification of sleep disturbances may help identify high-risk Chinese elders who may benefit from fall prevention education.

Abstract:Falls constitute a leading cause of unintentional injury deaths among older adults. This study aimed to examine the comprehensive characteristics of fatal falls among older individuals in Yunnan Province, China, to highlight the challenges faced in elderly care. A total of 22,798 accidental fall-related deaths were extracted from China's National Disease Surveillance Points System aged 60 and above between 2015 and 2019. Quantitative and textual data were analyzed to assess the incidence rates of initiating factors, locations, symptoms, and overall survival (OS) outcomes after falling. Hypertension emerged as the most significant intrinsic factor, especially among individuals aged between 70 and 79, female older adults, and urban residents ( P < 0.001). Home was identified as the most common location where fatal falls occurred (61.19%). The head was the most commonly injured body region (58.75%). The median of OS for all fatal falls was 2 days (0.13, 30), of which deaths occurred within 24 h [9287 (49.36%)]. There were instances where timely discovery after falling did not occur in 625 cases; their median of OS was significantly shorter compared to those discovered promptly after falling ( P < 0.001). Targeted interventions focusing on fall prevention and post-fall care are equally crucial for the well-being of older adults.

Abstract:Objective To survey the status and related risk factors for elderly falls among nursing home residents and to provide evidence for the development of specific measures.Methods Totally 370 elderly residents aged over 60 years old from 5 nursing homes in two districts were selected and the situation and relevant factors to falls occurred from November 2012 to October 2013 were collected by face-to-face interview with self-designed questionnaire.Results The incidence of falls was 12.97％ and the main places of occurrence were bedroom and stairs/passage way,the main self-reasons for falls were weak legs,dizzy and not keep balance while the main environmental reasons were slippery uneven ground.Of them,5.95％ elder residents injured by falls and the main injured body parts were lower limbs and head,17.24％suffered from fracture,24.14％ to be hospitalized and 10.34％ were disabled.The risk factors included age (OR =1.044),osteoporosis (OR =18.488),vertigo (OR =5.571),poor health (OR =1.608),fall history (OR =20.173),restricted activities for fear of falling (OR =2.192).Osteoporosis (OR =19.394) and fall history (OR =26.751) were the main harmful factors.Conclusions Since falls threaten the physical and mental health of nursing home residents,integrated intervention measures should be done to reduce the occurrence of falls.

Abstract:Objective: To understand the epidemiological characteristics and influencing factors of elderly unintentional fall-related injuries (EUFI) in Jiangsu from 2006 to 2021 and provide a scientific basis for the prevention of falls in the elderly and the formulation of related policies. Methods: Data on EUFI was collected from the first diagnosed cases in the injury surveillance hospitals in Jiangsu from 2006 to 2021, and epidemiological characteristics of injuries were analyzed. logistic regression was performed to identify the risk factors for EUFI. Results: The number of EUFI increased from 2006 to 2021, ranked as the first cause of elderly unintentional injuries during these years. The proportion of EUFI among elderly unintentional injuries increased in the same period. The male-to-female ratio is 1∶1.4. Falls mainly occurred during 8:00-10:00 am (32.7%). The most common place where falls occurred was at home (63.6%). From 2006 to 2014, leisure activities (62.1%), housework/study (23.1%), and work (5.2%) were the top three activities when injuries occurred. During 2015-2021, leisure activities (41.7%), housework/study (22.6%) and walking (20.5%) were the top three activities when injuries occurred. The major fall-related injuries were fractures (39.9%), with the injured parts at low limbs (32.5%), of which most were mild injuries (60.3%), and the primary outcome was hospitalization after treatment (74.1%). logistic regression showed that the risk factors of EUFI were female, old age, immigrants, winter, public residence, housework/study, and leisure activities. Higher education, occupation of household and production and transportation equipment operators were protective factors (all P<0.05). Conclusions: Fall is the leading cause of unintentional injuries in the elderly in Jiangsu, especially the elderly female population. Corresponding measures should be taken to prevent and intervene in falls in the elderly in the community according to the distribution characteristics.
The Incidence of Falls and Related Factors among Chinese Elderly Community Residents in Six Provinces.

Abstract:This cross-sectional study classified the factors related to falls among residents ≥ 60 years old in China in order to provide evidence for the prevention of falls in the elderly. A total of 2994 participants were enrolled, and the correlations between social demography, physical health, self-health management awareness, family support, and fall risk were analyzed. Factors influencing falls were classified by location, cause, and the activity during falls. Suffering from osteoarthropathy (OR = 1.761, 95% CI: 1.234–2.513, p < 0.05), depression or anxiety (OR = 1.896, 95% CI: 1.331–2.700, p < 0.001), household size > 2 (OR = 1.806, 95% CI: 1.042–3.130, p < 0.05), and poor self-assessed health (OR = 1.478, 95% CI: 1.107–1.972, p < 0.01) were risk factors. Higher participation in community health programs (OR = 0.522, 95% CI: 0.298–0.912, p < 0.05) and spousal support (OR = 0.909, 95% CI: 0.841–0.981, p < 0.05) were protective factors. Falls were divided into the following categories: stairs/hallway (vision, attention problems), bath/toilet (vision, attention problems, slipping), indoor housework (dizziness, leg weakness), and outdoor activities (attention, surface problems). While acknowledging that the personal physical and mental health of the elderly may lead to falls, community support, accelerated transformation of housing, and the construction of barrier-free environments play an important role in reducing the risk of falls.
Falls and Fall-Related Consequences among Older People Living in Long-Term Care Facilities in a Megacity of China

Abstract:Introduction: Falls are currently the top safety problem in long-term care facilities (LTCFs) in China. Due to the increasing number of residents living in LTCFs, more evidence is needed to give a foundation for fall prevention. Objective: This study aimed to explore the epidemiological characteristics of falls in LTCFs in central Shanghai. Methods: The study was conducted in 21 LTCFs in a central district in Shanghai, with a capacity of 3,065 residents. A two-stage sampling method was applied in participant recruitment. Falls were recorded by LTCF staff over a 12-month period. Details of falls were obtained by face-to-face interviews. The χ2 test was used in data analyses. Results: The incidence of falls was 13.5%; 64.0% falls resulted in injuries, with 32.0% involving fractures. Women had a significantly higher incidence of injurious falls than men (χ2 = 4.066, p = 0.044). Residents aged 80–89 years or in level 1 care had the highest incidence of falls with severe consequences. The incidence of falls was significantly higher at small- or medium-sized LTCFs, public LTCFs, and LTCFs with higher environmental risk levels compared to their counterparts. Most falls occurred when walking on a flat floor (28.9%) and rising up or sitting down (24.0%); 40.9% occurred during the night. Of those injured, 54.8% were treated in hospitals, and only 53.7% completely recovered. Conclusions: Though the average incidence of falls in LTCFs in Shanghai was relatively low, great variation was observed between LTCFs, and severe consequences occurred frequently. Fall prevention programmes should be evidence-based with applicable devices and individualized care services and supports. The roles of personal and institutional factors on falls warrant further study.
Epidemiological Pattern of Fall-Related Injuries among Residents in Shanghai, China

Abstract:[Objective] To investigate the epidemiologic features of fall-related injuries, and its relationships with income, education level, and senior residents in Shanghai, China.[Methods] A multi-stage stratified cluster sampling approach with probability proportional to size was used to select 45 857 residents in Shanghai for home visits.Mortality and demographic data were collected from Shanghai Vital Registration System from 2001 to 2011.Records on fall-related hospitalized cases and fatalities from 164 hospitals in Shanghai were also analyzed.Logistic regression was used to control confounding from age, gender, education levels and income.[Results] During 2001 to 2011, the average annual income increased by 206.63% in Shanghai; while the fall specific mortality rate increased from 10.63 per 100 000 in 2001 to 14.76 per 100 000 in 2011.The morbidity of fall was 14.70 per 1 000 in the survey.Those with family average monthly incomes lower than 500 Yuan RMB were 6.5 times as likely as to suffer from fall-related injuries than those with higher than 5 000 Yuan RMB.The residents with an education level of primary school and under showed the highest fall specific mortality and morbidity rates, 57.85 per 100 000(73.23%) and 33.96 per 1 000(49.24%), 22 times and 5.5 times compared with those with college degree or above, respectively.The Luwan and Jing'an districts showed the highest mortality rates(24.10 per 100 000 and 19.59 per 100 000 respectively) with leading proportions of senior residents.The mortality rate of fall in the children 5 years was the highest among children aged 0-14 years.The mortality and morbidity of fall increased dramatically with age among those aged 50 years or above, among which the 50-54 age group accounted for the largest proportion and the morbidity accelerated.The female showed higher mortality and morbidity rates of fall than the male.Combined head injuries accounted for about half of the total fatalities of fall.The relative risk of fall-related injuries was higher in lower income, lower education level, and higher age groups, with odds ratios of 1.39, 1.27, and 1.21, respectively.[Conclusion] The prevention and control of fall-related injuries confronts a huge challenge.Income, education level, and age are the main risk factors for fall-related injuries.Populations under 5 years old, over 50 years old, with an education level lower than primary school, and with lower income should be the targets for intervention.More efforts are needed, such as installing comprehensive fall-related injury prevention programs to the new responsibility system of family doctors.

Abstract:Hospitalised patients were proven to be poor at recognising their risk of falls. Measurement of patients' self-perceived and health professionals' assessment of fall risk should be conducted to evaluate the disparity. This study provides a solid foundation to raise medical staff's awareness of the targeted population, identify the underlying factors and implement tailored fall prevention strategies and education.

Abstract:Background: Unintentional falls are known to be a leading cause of injury mortality among older Chinese adults, yet we lack data on the most recent trends in related mortality. To address this, we used the latest nationally representative data from China to examine trends in elderly unintentional fall mortality by place (urban/rural), sex (men/women), and age group (65-69, 70-74, 75-79, 80-84, and ≥85 years) from 2010 to 2021. Methods: We retrieved mortality data from the Chinese Health Statistical Yearbook (2010-21) and population data from the Chinese Population Census (2010, 2020). Using line graphs, we examined mortality trends over time. We fitted a joinpoint regression model to detect periods experiencing significant changes and calculated the average and specific annual percentage change of mortality rates to quantify significant changes in the mortality of the elderly due to unintentional falls. Results: Between 2010 and 2021, the age-standardised mortality rate from unintentional falls increased from 45.7 to 67.8 per 100 000 population among Chinese adults aged 65 years and older. Subgroup analyses by sex and place showed similar changing patterns to the overall mortality trends. The joinpoint regression identified certain recent periods that saw significant increases in mortality among adults aged 65-69, 70-74, 75-79, and 80-84 years. During the study period, men and individuals living in rural areas generally had higher unintentional fall mortality rates than women and people living in urban areas (mortality rate ratios: 1.09-1.21 for men vs. women and 1.01-1.27 for rural areas vs. urban areas). Notably, the differences between urban and rural areas, and those between men and women, were consistent across the three younger age groups (65-69, 70-74, and 75-79 years) studied, but reduced in the two oldest age groups (80-84 and ≥85 years). Conclusions: The age-standardised mortality rate from unintentional falls increased between 2010 and 2021 among Chinese adults aged 65 years or older, with wide variations across years. Unintentional fall mortality has recently increased among adults aged 65 to 84 years. Differences between urban and rural areas, as well as between men and women, deserve the attention of injury researchers and policymakers.
Clinical characteristics and influencing factors of serious fall injuries among older inpatients: A secondary analysis of multicenter cross-sectional administrative data

Abstract:Background Inpatient falls are common adverse events in hospitals, particularly among older adults. However, current research on falls in the older population is limited by small sample sizes and reliance on single-center designs without distinguishing the different levels of fall injuries. Objective To examine the circumstances of falls in older inpatients and investigate the factors influencing serious fall injuries. Design A secondary analysis of multicenter cross-sectional administrative data from the China National Database of Nursing Quality (CNDNQ). Setting(s) 67 hospitals in Hunan, China. Participants 5143 fall incidents from 5048 older inpatients were included from January 2019 to December 2022. Methods Descriptive statistics (means, standard deviations, frequencies, and percentages) were used to describe fall characteristics. Given the multilevel data structure, a Generalized Linear Mixed Model (GLMM) with a Logistic link function was employed, treating hospitals and wards as random effects. Univariate and multivariate analyses were conducted. Results The prevalences of fall-related injuries and serious fall injuries in this study were 63.3 % and 25.0 %, respectively. Nurse-to-patient ratio at the time of fall ≤ 0.04 ( aOR = 1.522, 95 % CI: 1.297–1.785), female gender ( aOR = 1.304, 95 % CI: 1.137–1.497), a lengthy hospital stay ( aOR = 1.009, 95 % CI: 1.001–1.018) were associated with an increased risk of serious fall injuries. Surgical ( aOR = 0.655, 95 % CI: 0.482–0.889) and orthopedic wards ( aOR = 0.649, 95 % CI: 0.449–0.939) exhibited lower risks of serious fall injuries than internal medicine wards. The time period from 8:00 to 15:59 was associated with a higher risk of serious fall injuries compared to the period from 0:00 to 7:59 ( aOR = 1.416, 95 % CI: 1.171–1.713). Compared to lying, standing ( aOR = 2.594, 95 % CI: 1.733–3.883), getting in/out of bed ( aOR = 1.403, 95 % CI: 1.009–1.951), walking or rehabilitating ( aOR = 3.039, 95 % CI: 2.194–4.208), toileting ( aOR = 1.411, 95 % CI: 1.038–1.917), and showering ( aOR = 2.170, 95 % CI: 1.391–3.385) were associated with an increased risk of serious fall injuries. Conclusions Fall-related injuries and serious fall injuries are highly prevalent among older inpatients. The study provides insights for targeted fall prevention strategies, improving nursing management and patient safety. Registration Not registered.

Abstract:ObjectiveThis study aimed to investigate temporal trends and regional differences of both falls and injurious falls among Chinese older adults and identify the associated risk factors. MethodsWe conducted a retrospective analysis using the 2011, 2013, 2015 and 2018 waves of China Health and Retirement Longitudinal Study. Our sample included 35 613 people aged 60 years or above. We analysed two binary outcome variables that were collected at each wave, including whether a respondent had any falls in the last 2 or 3 years, and if so, whether a respondent experienced any injurious falls that warranted seeking medical treatment. The explanatory variables included individual-level sociodemographic characteristics, physical function and health status. We conducted both descriptive and multivariate logistic analyses. ResultsWe found no significant trend for falls after adjusting for individual-level factors, while significant regional variations in falls existed with higher fall prevalence in the central and western areas, compared with the eastern area. We detected a significant descending trend of injurious falls between 2011 and 2018 and identified the northeastern region with the lowest rates of injurious falls during the study period. Our study also revealed significant risk factors for falls and injurious falls, such as chronic conditions and function limitations. ConclusionsOur results indicated no temporal trend of falls, a declined trend of injurious falls, and significant regional variations in the prevalence of falls and injurious falls in 2011-2018. These findings have important implications for prioritising areas and subpopulations to prevent falls and injuries among China's elderly population.

Abstract:Introduction: As a developing country with the largest older adult population in the world, strengthening the research on falls among the older adults is undoubtedly an urgent item in China. This study aimed to explore the prevalence and risk factors associated with falls and injury from falls among community-dwelling older adults in Guangzhou, China, particularly focusing on their associations with chronic diseases. Methods: A total of 1,629 participants aged 65 years and above were selected from 11 counties in Guangzhou by the multi-stage stratified random sampling method in 2021. Socio-demographic characteristics, health and lifestyle factors, the status of falls, and injury from falls were measured by structured questionnaires through face-to-face interviews. Chi-square tests and logistic regression analysis were used to identify factors associated with falls and injury from falls. Chord diagrams were used to explore their associations with chronic diseases. Results: A total of 251 participants (15.41%, 95% CI: 13.98%−17.25%) reported falls, and 162 participants (9.46%, 95% CI:7.72%−11.55%) indicated an injury from falls. Logistic regression analysis showed the results as follows: female patients (adjusted OR = 1.721, 95% CI: 1.681–1.761) aged ≥80 years (1.910, 1.847–1.975), unemployed (1.226, 1.171–1.284), uninsured (1.555, 1.448–1.671), average monthly household income of 2,001–4,000 CNY (1.878, 1.827–1.930), number of services provided by the community health center ≥13 times per year (1.428, 1.383–1.475), illness within 2 weeks (1.633, 1.595–1.672), high-intensity physical activity (2.254, 2.191–2.32), sedentary (1.094, 1.070–1.117), and number of chronic disease illnesses ≥3 (1.930, 1.870–1.993). Meanwhile, those risk factors were also associated with injury from falls. The older adults with medium-intensity physical activity were at lower risk (0.721, 0.705–0.737) of falls and higher risk (1.086, 1.057–1.117) of being injured from falls. Chord diagrams showed the correlations between chronic diseases and falls and injury from falls among community-dwelling older adults in Guangzhou, China. Conclusion: The high prevalence of falls is found among community-dwelling older adults in Guangzhou, China, which is related to multiple factors such as demographic variables, lifestyle, and health status, especially for chronic diseases. Therefore, targeted interventions should be developed and implemented urgently.

Cross-sectional study on prevalence and risk factors for falls among the elderly in communities of Guangdong province, China

Abstract:Objective This study aims to investigate the prevalence and risk factors of falls among the elderly in Guangdong, China. Methods A cross-sectional study was conducted in six communities of Guangdong province. People over 60 years old were selected with multistage random-cluster sampling. Data on falls within the previous 12 months and fall-related risk factors were collected through a face-to-face interview. Results The prevalence of falls among older adults was 11.9% (95% CI: 11.0% to 12.8%) among 5374 interviewees. The common injuries caused by falls were bruises/scrapes (40.0%) and fractures (15.5%), and most people fall while doing housework (35.0%). Univariate analysis showed that 14 factors were associated with falls among older adults, including gender, age, residence, occupation, education level, balance ability, situation of cognition, disease, depression, living arrangement, marital status, the behaviour of exercise, drinking and drug use (p<0.05). Multivariate analysis showed that the associated factors of falls among older adults included woman (OR=1.68, 95% CI: 1.40 to 2.02), age from 70 to 79 years (OR=1.31, 95% CI: 1.09 to 1.58), age over 80 (OR=1.63, 95% CI: 1.25 to 2.13), impaired balance ability (OR=1.45, 95% CI: 1.20 to 1.75), exercise several times per month (OR=1.69, 95% CI: 1.13 to 2.53), polypharmacy (OR=1.54, 95% CI: 1.19 to 2.00), cognition impairment (OR=1.35, 95% CI: 1.08 to 1.69), mild depression (OR=1.89, 95% CI: 1.47 to 2.45) and moderate depression (OR=3.07, 95% CI: 1.99 to 4.73). Conclusions The hazards caused by falls to the elderly in China cannot be ignored. A multidimensional customised fall prevention programme should be considered to reduce the risk of falls among the elderly based on the results above.

Epidemiologic Characteristics of Fall in the Elderly in Urban and Rural Areas in Shanghai

Abstract:Objective To understand the epidemiologic characteristics of fall in the elderly in Shanghai,as well as the differences between urban and rural areas,and provide evidence for targeted fall prevention and intervention.Methods From January to March in 2017,a questionnaire survey was conducted in the elderly aged 60 or above selected from 7 urban communities and 6 rural communities in Shanghai to understand the epidemiologic characteristics of fall in the elderly and analyze the gender and urban-rural differences.Results In urban area,a total of 3 386 elderly people were surveyed,in whom 441 (13.0％) had fall and 261 (7.7％) were injured after fall.In rural area,a total of 2 826 elderly people were surveyed,in whom 320 (11.3％) had fall and 169 (6.0％) were injured after fall.Fall risk in women were higher than that in men in both urban and rural areas with OR of 1.62 (95％CI:1.42-1.86) and 1.16 (95％CI:1.38-1.98) respectively,but the differences of fall related injury were not significant.Compared with urban areas,fall risk and fall related injury risk were both lower in rural areas with OR of 0.86 (95％ CI:0.73-0.99) and 0.74 (95％ CI:0.56-0.99).Compared with urban areas,men had lower risk for fall,and women had lower risk for fall related injury with OR of 0.68 (95％CI:0.51-0.90) and 0.66 (95％CI:0.47-0.93) respectively.Fall mainly occurred at home.Fall in urban area more frequently occurred on stairs,and fall in rural area more frequently occurred during farming.More than 60％ of the falls had environmental risk factors.Slippery ground and uneven ground were main reasons.The incidence of fracture resulted from fall was high indicated by 89 fracture cases in urban areas (28.2％) and 64 fracture cases in rural areas (36.1％).Conclusions The risk for fall in Shanghai had gender and urban-rural differences.Targeted intervention should be conducted according to the characteristics of fall in the elderly.
Fall status and risk factors in older Chinese adults: A cross-sectional study

Abstract:Aim: To analyse the risk factors and incidence of falls in geriatric outpatients in a university hospital ward in Hangzhou, China. Methods: From May 2020 to August 2022, 1712 geriatric outpatients in a university hospital ward in Hangzhou, China, were screened using a socio-demographic questionnaire (e.g. gender, age, living arrangement, etc.) and assessment scales. The correlation between each factor and falls was preliminarily analysed by chi-squared tests. Finally, binary logistic regression analysis was conducted to further analyse the risk factors of falls. The STROBE checklist was used in reporting this study. Results: Of the 1712 geriatric outpatients recruited, 1626 participants (60-79 and ≥ 80 years old) with complete questionnaire and assessment data were included. The occurrence of falls for those in the 60-79 age group was 8.4%, and for those in the ≥80 age group it was 13.4%. Age (p = .007), use of a walking assistance device (p < .001), the Stay Independent Brochure Questionnaire (SIB) (OR = 7.751, 95% CI = 5.089-11.806, p < .001), living arrangement (p = .004), timed up and go test (TUGT) (p = .007) and three diseases or above (OR = 2.496, 95% CI = 1.358-11.4.586, p = .003) reached statistical significance. Conclusions: Older people have a high incidence of falls. In this study, age, disease history, SIB scores (≥4 points), living arrangement, TUGT and walking assistance device increased the probability of falls in older Chinese adults. Personalised interventions should be carried out according to the specific situation of older people to effectively reduce their incidence of falls and improve their quality of life. Relevance to clinical practice: The basic characteristics and fall risk factors of the older can help nurses identify fall risk, and early intervention by caregivers can reduce fall-related injuries, which has practical significance for promoting healthy aging. Patient or public contribution: The subjects of this study were older patients ≥60 years old, and the demographic characteristics and fall-related information of patients were obtained by questionnaire. The team worked closely with a team of experts in the field of health care. Some researchers collect data and rewrite them, while other researchers analyse the information and write a paper. Trends of falls mortality among older adults in urban and rural China, 1987-2021.

Abstract:BACKGROUND:Falls in older age pose a major public health concern, with unclear urban-rural patterns of falls mortality in China. This study examines the trends of late-life falls mortality in urban and rural China over a 35-year period. METHODS:Falls mortality data were sourced from China's National Health Commission. Joinpoint regression analysis was used to examine changes in trends and age-period-cohort modelling to estimate age, period and cohort effects on fall-related mortality from 1987 to 2021. Net drift, local drift, longitudinal age curves and period relative risks were also calculated. RESULTS:The age-standardised falls mortality in older age showed a long-term trend of initial decline prior to 2003, followed by a steep increase thereafter, with notable distinctions between urban and rural patterns. The rise in rural populations, particularly among older males, was more conspicuous. In rural areas, the decline in falls mortality diminished with age, contrary to the urban trend. Falls mortality increased with age in both urban and rural older populations, peaking in the group aged 85-89. The period effect curves of falls mortality in urban and rural areas both approximated a U-shaped pattern while there were minor variations in early cohorts. CONCLUSIONS:China has experienced a consistent rise in late-life falls mortality in recent years. Notably, there are significant urban-rural disparities in age, period and cohort effects of fall-related mortality among older adults. Rural residents, males and older age groups have potential higher fatal-falls risk. Targeted strategies should be implemented to prevent late-life falls.
Population Ageing and Injurious Falls among One Million Elderly People Who Used Emergency Medical Services from 2010 to 2017 in Beijing, China: a Longitudinal Observational Study

Abstract:Objectives To access the epidemiological characteristics of elderly people using emergency medical services (EMS) in Beijing, as a consequence of injurious falls, and the association between an ageing population and injurious falls. Design A longitudinal observational study based on Beijing EMS data. Setting All citizens aged 60 years or above who used EMS from 2010 to 2017 in Beijing, China. Participants During 2010 to 2017, 2516 128 people used EMS in Beijing. Of these, 1528 938 people aged under 60 years were excluded and the remaining 987 190 people were included in our study. Methods Data were from the emergency dispatch database of Beijing’s Emergency Medical Centre. We described the proportion of elderly people using EMS in Beijing due to injurious falls with regard to region, time, sex, age and proportion of the elderly population. Results Among the 987 190 participants who used EMS, 82 694 (8.38%) had had a fall. The proportion of falls rose from 7.12% in 2010 to 9.45% in 2017, and was higher in urban (8.62%) than in suburban (7.80%) regions. Elderly people were more likely to use EMS due to injurious falls during September to November (8.99%) than during December to February (7.68%) (adjusted OR (aOR)=1.19), and during 08:00 to 09:59 (10.02%) than during 00:00 to 01:59 (4.11%) (aOR=2.52). Elderly people in districts with high (8.92%, aOR=1.15) and medium (8.23%, aOR=1.09) proportions of an elderly population were more likely to use EMS due to injurious falls than those in districts with a low proportion of elderly population (7.81%). Conclusions In Beijing, the proportion of elderly people using EMS due to injurious falls increased with age, and was positively correlated with the proportion of the elderly population. Taking care of elderly people and preventing injurious falls should be one of the key issues to be handled in a society with an ageing population.

Abstract:Older adults have a high prevalence of falls due to a decline in physiological functions and various chronic diseases. This study aimed to investigate the prevalence of and risk factors for falls among older individuals in the Chinese Longitudinal Healthy Longevity Survey (CLHLS). We collected information from 9737 older individuals (average age=84.26 years) from the CLHLS and used binary logistic regression analysis to explore the independent risk factors and protective factors for falls. The logistic regression analysis results are reported as adjusted odds ratios (aORs) and 95% confidence intervals (95%CIs). The prevalence of falls among older adults in China was 21.6%, with women (24.6%) having a higher prevalence than men (18.1%). Logistic regression analysis revealed that never (or rarely) eating fresh fruit, difficulty with hearing, cataracts, and arthritis were the common independent risk factors for falls in older Chinese men and women. Among men, age ≥80 years (aOR=1.86), never doing housework (aOR=1.36), and dyslipidemia (aOR=1.47) were risk factors, while eating milk products once a week was a protective factor. Alcohol consumption (aOR=1.40), physical labor (aOR=1.28), and heart disease (aOR=1.21) were risk factors for falls in women, while a daily sleep duration of 6-12 h and garlic consumption once a week were protective factors. The prevalence of falls among older adults in China is 21.6% and is greater in women than in men. These risk and protective factors can be used to formulate reasonable recommendations for living habits, diet, and chronic disease control strategies.
