Abstract:Interstitial and cornual pregnancies are dangerous, yet rare, forms of ectopic pregnancy, accounting for 2%-4% of all ectopic pregnancies. A 38-year-old female, para 1, gravida 3 had undergone another <i>in vitro</i> fertilisation (IVF) cycle (a salpingectomy performed elsewhere for treating a hydrosalpinx before a previous IVF attempt). Duration of pregnancy is 6 weeks and 5 days, a transvaginal ultrasound revealed an embryo with a positive foetal heartbeat, located in the left cornuum. As no conservative treatment option could be followed, we proceeded with laparoscopic removal of ectopic pregnancy through cornual resection. Since a specific surgical methodology has not yet been established, presenting more step-by-step surgical approaches that can be used in clinical practice is of high importance. We present a step-by-step surgical approach that we have implemented in cases of cornual pregnancy in our department.
Interstitial Ectopic Pregnancy: Laparoscopic Cornuostomy

Abstract:Study objective: To demonstrate and discuss the technique of cornuostomy for surgical management of interstitial ectopic pregnancy. Design: Stepwise demonstration of the technique with narrated video footage. Setting: Tertiary referral center in Manchester, United Kingdom. Intervention: Interstitial ectopic pregnancies are rare but are associated with a higher mortality rate than other ectopic pregnancies [1,2]. It occurs when the fertilized embryo implants in the interstitial portion of the fallopian tube traversing the vascularized myometrium. When undiagnosed they present late in the second trimester associated with rupture and catastrophic bleeding, with a mortality rate of 2% to 2.5%.2 Diagnosis requires a degree of vigilance from the ultrasound operator because it is commonly misdiagnosed as intrauterine pregnancies. Surgical management options include laparoscopic cornual resection or cornuostomy. There is no consensus on the optimal surgical technique but cornuostomy is a more conservative approach associated with less disruption to uterine anatomy and loss of myometrium [3,4]. A 22-year-old gravida 4 woman presented at 7 weeks' gestation with right iliac fossa pain. Initial serum human chorionic gonadotropin was 18 136 IU/L. Transvaginal ultrasound scan showed an empty endometrial cavity and an echogenic "donut"-shaped mass within the right interstitial space, within the uterine serosa but outside the endometrial cavity (Supplemental Video 1). At laparoscopy the diagnosis of a right interstitial ectopic pregnancy was confirmed (Supplemental Video 2). Vasopressin 20 IU diluted in 80 mL of normal saline was injected around the base of the ectopic pregnancy. Monopolar diathermy was used to incise the overlying serosa followed by hydrodissection to separate the ectopic gestational sac from the myometrial attachment. The resulting defect was inspected and closed in 2 layers. Total operating time was 46 minutes. Conclusion: Although there is no clear evidence to guide the management of all interstitial ectopic pregnancies, an individualized approach taking into account the woman's previous history and future fertility plans and wishes is essential. In this case, given the woman's previous contralateral salpingectomy and her wishes for a conservative approach, a laparoscopic cornuostomy was likely the best option.
Minimally-invasive Management of Intramural Ectopic Pregnancy: an Eight-Case Series and Literature Review

Abstract:Objective Intramural pregnancy (IMP) is a rare type of ectopic pregnancy and potentially fatal. Early diagnosis and management of IMP are important to preserve patient fertility. Here, we describe the use of minimally-invasive surgery for early IMP. Study design We retrospectively analyzed the clinical data of eight patients with IMP treated at our center (January 2010 to December 2018) and reviewed the literature describing minimally-invasive treatment of IMP. Results All eight patients had at least one risk factor for IMP. Two cases were confirmed by ultrasound, but ectopic pregnancy or gestational trophoblastic disease were initially suspected in the other cases. Surgery (laparoscopic in three patients, hysteroscopic in one patient, and laparoscopic combined with hysteroscopic in four patients) was successful in all cases, and all patients recovered well without complications. The literature review identified 14 articles describing 17 cases of IMP managed with minimally-invasive surgery. Laparoscopic surgery was used successfully as a sole treatment in 10 cases and after failure of hysteroscopic surgery in six cases. Only one case was treated with a combination of hysteroscopic surgery and methotrexate. Interestingly, one case at our center presented with a sinus connecting the gestational sac and uterine cavity and was treated successfully using hysteroscopic surgery during early pregnancy. Conclusions Laparoscopic surgery is a feasible management option for most cases of early IMP. Hysteroscopic surgery may be appropriate for cases where a sinus connects the gestational sac with the uterine cavity or when cornual ectopic pregnancy needs to be excluded.
Laparoscopic Management of Unruptured Cornual Ectopic Pregnancy in a 22-Year-Old Primigravida: A Case Report

Abstract:Cornual ectopic pregnancy, though rare, presents significant challenges in diagnosis and management. This case report details the clinical presentation and successful treatment of a 22-year-old primigravida experiencing symptoms of abdominal pain, nausea, and vomiting, ultimately diagnosed with an unruptured left cornual ectopic pregnancy. Employing a multidisciplinary approach involving clinical suspicion, beta-human chorionic gonadotropin (β-hCG) measurements, and transvaginal ultrasound findings, we underscored the importance of timely intervention to avert adverse outcomes. The patient underwent laparoscopic partial salpingectomy, resulting in minimal intraoperative blood loss and postoperative complications. Our experience highlights the effectiveness of laparoscopic intervention in managing cornual ectopic pregnancy and underscores the necessity of tailoring treatment strategies to individual patient circumstances. By adhering to established guidelines and advancing research efforts, we can further enhance outcomes for patients grappling with this challenging condition. This case emphasizes the critical role of early diagnosis, prompt intervention, and ongoing vigilance in the management of cornual ectopic pregnancies.
Macromastia and bilateralaxillary breast hypertrophy: a case report.

Abstract:BACKGROUND Breast hypertrophy presents at puberty or thereafter. It is a condition of abnormal enlargement of the breast tissue in excess of the normal proportion. Gland hypertrophy, excessive fatty tissue or a combination of both may cause this condition. Macromastia can be unilateral or bilateral. OBJECTIVE To present a case of massive bilateral gigantomastia with huge bilateral hypertrophy of the axillary breasts. METHODS Review of the prentation, clinical and investigative findings aswell as the outcome of surgical intervention of a young Nigerian woman with bilateral severe breast hypertrophy and severe hypertrophy of axillary breasts. RESULT The patient was a 26-year-old woman who presented with massive swelling of her breasts and bilateral axillary swellings, both of six years duration.. In addition to the breast pathology, she also suffered significant psychological problems. The breast ultrasonography confirmed only diffuse swellings, with no visible lumps or areas of calcifiCation. She had total bilateral excision of the hypertrophied axillary breasts, and bilateral breast amputation with composite nipple-areola complex graft of the normally located breasts.The total weight of the breast tissues removed was 44.8 kilogram. CONCLUSION Macromastia of this size is very rare. This case to date is probably the largest in the world literature. Surgical treatment of the condition gives a satisfactory outcome.
Cornual pregnancy as a rare entity of ectopic pregnancy: A case report

Abstract:Introduction and importance: Cornual pregnancy, an infrequently reported form of ectopic pregnancy occurring in the uterine horn, is inadequately documented in medical literature, with an incidence below 2 %. This condition poses a substantial risk to maternal health due to delayed diagnosis and the potential for life-threatening bleeding after rupture. Case presentation: We present a case report following the SCARE guidelines that details a 32-year-old woman with abdominal pain and abnormal uterine bleeding. Clinical examination, β-HCG levels, and endovaginal ultrasound confirmed the presence of a 4 cm unruptured right cornual pregnancy. The patient underwent a minilaparotomy, which revealed the ectopic pregnancy, followed by a successful cornuostomy and right salpingectomy. Postoperative recovery was uneventful. Clinical discussion: Cornual pregnancy, comprising around 2 % of ectopic pregnancies, is associated with increased risks of rupture and maternal morbidity. Major risk factors include a history of pregnancy termination, miscarriage, STIs, and smoking. Diagnosis is often delayed, leading to an increased risk of bleeding. Pelvic pain is a common presenting symptom, and sonographic findings aid in accurate diagnosis. Conclusion: Cornual pregnancy, though rare, represents a serious condition with a significant risk of maternal morbidity and mortality. A timely diagnosis is crucial for effective treatment, with ultrasound playing a pivotal role, complemented by the essential contribution of laparoscopy. This case underscores the importance of prompt intervention to mitigate the associated risks and improve patient outcomes.

Abstract:Study Objective: To investigate the effect of cornual suture at the time of laparoscopic salpingectomy on the incidence of interstitial pregnancy (IP) after in vitro fertilization (IVF). Design: Single-center. retrospective review (Canadian Task Force classification II-2). Setting: University hospital. Patients: Patients with hydrosalpinx who were treated with salpingectomy before IVF-embryo transfer and managed in our center were included in this study. Interventions: A total of 542 patients who underwent laparoscopic salpingectomy from April 2011 to March 2014 comprised group A. A total of 502 patients who underwent cornual suture at the time of laparoscopic salpingectomy from April 2014 to February 2016 comprised group B. Measurements and Main Results: The overall IP rate was significantly lower in group B (7/293, 2.39%) than in group A (27/373, 7.24%; p < .05). The intrauterine pregnancy and ongoing pregnancy/live birth rates were significantly higher in group B than in group A (both p < .05). All 34 patients with IP underwent laparoscopic cornuostomy and cornual repair. Seven of 11 patients with combined interstitial and intrauterine pregnancies carried the intrauterine pregnancy to term and delivered via cesarean section, whereas 4 patients underwent inevitable miscarriage. IP rupture occurred in 8 of 34 patients at a mean of 23.43 +/- 2.77 days after embryo transfer. The earliest time of rupture was on day 20 after embryo transfer. Conclusion: An optimized salpingectomy technique plays an important role in pretreatment before embryo transfer in patients with hydrosalpinx. Abstract:BackgroundInterstitial and cornual ectopic pregnancy is rare, accounting for 2–4% of ectopic pregnancies and remains the most difficult type of ectopic pregnancy to diagnose due to low sensitivity and specificity of symptoms and imaging. The classic triad of ectopic pregnancy—abdominal pain, amenorrhea and vaginal bleeding—occurs in less than 40% of patients. The site of implantation in the intrauterine portion of fallopian tube and invasion through the uterine wall make this pregnancy difficult to differentiate from an intrauterine pregnancy on ultrasound. The high mortality in this type of pregnancy is partially due to delay in diagnosis as well as the speed of hemorrhage.MethodsThree cases of interstitial pregnancy were retrospectively analyzed.ResultSuccessful laparoscopic cornuostomy and removal of products of conception were performed in two cases, while one case was successfully managed by local injection with KCL and methotrexate followed by systemic methotrexate.ConclusionEarly diagnosis and timely management are key to the management of interstitial and cornual ectopic pregnancy. With expertise in ultrasound imaging and advances in laparoscopic skills progressively, conservative medical and surgical measures are being used to treat interstitial and cornual ectopic pregnancy successfully.
Ruptured Left Cornual Ectopic Pregnancy: A Case Report

Abstract:Cornual ectopic pregnancies are rare with a mortality rate that is significantly higher than that of other ectopic pregnancy types. Due to the cornual region's location on the fallopian tube, rupture of a cornual gestation may lead to massive maternal hemorrhage resulting in hypovolemia and shock. Here, we report a 39-year-old female who presented to Hospital Corporation of America (HCA) Florida Healthcare's emergency department (ED) in a state of hypovolemic shock. She was six weeks pregnant based on an unknown and unsure last menstrual cycle. The diagnosis of a ruptured ectopic pregnancy was suspected based on a positive urine pregnancy test and a pelvic ultrasound that revealed an empty uterus and a copious amount of free fluid within the abdomen. Significant hematoperitoneum and hemodynamic instability required emergent exploratory laparotomy with findings of a ruptured left cornual ectopic pregnancy. A left cornual resection and repair was done with an uneventful postoperative period. With cornual ectopic pregnancies being a rare entity, our case emphasizes the importance of early detection and management to help prevent fatal complications.

Abstract:One case was managed by resection of the cornual pregnancy by laparotomy; the other was treated with ultrasonographically guided transvaginal injection of KCl into the ectopic gestation. Complete ablation of the cornual pregnancy. Both women had no complications during their pregnancies. Cesarean section was performed in different indications and allowed the birth of two living infants. Both surgical and conservative treatment of an ectopic pregnancy can be adopted in accordance with patient's condition. The both treatment permitted the development of the intrauterine pregnancy. The risk of heterotopic pregnancy is directly related to the number of embryos transferred.

Abstract:Interstitial or cornual ectopic pregnancies are a rare form of ectopic pregnancy which account for 2% to 4% of all ectopic pregnancies. Therefore, early recognition and accurate diagnosis of an interstitial or cornual ectopic pregnancy is critical for the safety and wellbeing of the patient especially due to the increased risk of massive hemorrhage. Surgical treatment of interstitial pregnancy without cornual resection: A case report

Abstract:Rationale: Interstitial pregnancy (IP) is a rare but extremely life-threatening form of ectopic pregnancy. The traditional surgical treatment for this anomaly is the resection of uterine cornua or fallopian tubes, which often damages their structural integrity, thereby compromising the reproductive potential for women who wish to preserve fertility. Patient concerns: A 33-year-old female was admitted to our hospital with suspected ectopic pregnancy, following a 4-weeks history of positive pregnancy tests after uterine evacuation. The patient was hemodynamically stable on arrival. Ultrasound revealed an empty uterus with an eccentric gestational sac located at the fundus and surrounded by a thin myometrium, indicative of a suspected interstitial pregnancy. Diagnosis and intervention: After failed attempt at medical management with a single dose of intramuscular methotrexate, the patient was arranged for hysteroscopy-assisted laparoscopy. In surgery, the uterine cavity appeared empty, and a 2 × 2 cm bulge with increased vascularity at the right uterine courna was identified upon examination. The gestational sac was aspirated through the vagina from the right ostium of the uterine tube using a suction curette pointing at the right ostium. Sutures were not needed afterward, and the myometrial anatomy was left undisrupted. The diagnosis of IP was confirmed by the postoperative histological report. Outcomes: Perioperative blood loss was approximately 10 ml and the operative time was 40 minutes. The patient had an uneventful postoperative recovery and was discharged after 3 days. Subsequent follow-ups showed a significant reduction in the patient serum beta hCG to 48IU/L within 5 days postoperation, and a negative result after 7 days. Lessons: This novel surgical technique is an alternative minimally-invasive approach for selected early diagnosed and hemodynamically stable IP patients. The technique represents a safe, quick, and simple approach combining the benefits of laparoscopy, such as allowing for immediate conversion of cornuectomy when uterus ruptures, and the benefits of suction curettage, such as shorter operative time and minimal blood loss. We believe patients with interstitial pregnancy who still have fertility wishes would benefit from this surgical technique to a larger extent in the future.

Abstract:Ruptured ectopic pregnancy is a leading cause of maternal morbidity and mortality worldwide. Cornual ectopic pregnancy is a specific type that occurs at the horn of the uterus. It often ruptures later that than the more common tubal ectopic pregnancies because of the more distensible myometrial tissues surrounding the gestational sac. The diagnosis of cornual pregnancy poses a lot of challenges to sonographers because of its closeness to the body of the uterus necessitating a lot of skill and expertise. Rupture of a cornual ectopic pregnancy is life threatening because of associated massive haemorrhage. Treatment is mostly by emergency laparotomy and cornual resection with salpingectomy or hysterectomy. Here, we report a case of cornual ectopic pregnancy at 11 weeks gestational age that was misdiagnosed as intra uterine gestation until it ruptured. Patient had laparotomy with right cornual resection and ipsilateral salpingectomy. Ruptured cornual ectopic pregnancy and its morbidities and mortalities should be prevented through detailed early first trimester examinations. Ultrasonography especially the transvaginal modality remains a very helpful but occasionally inconclusive tool in the diagnosis of this life-threatening condition.

Abstract:Objective To provide a comprehensive, step-by-step presentation of the laparoscopic resolution of ectopic pregnancy within a rudimentary uterine horn. Design A detailed demonstration of the laparoscopic technique presented through narrated video footage. Setting Pregnancy occurring in the rudimentary horn of a unicornuate uterus represents a rare form of ectopic pregnancy 1 . This condition is associated with a high risk of uterine rupture. Early detection is crucial for effective management and prevention of potential complications 2-3 . In this manuscript, we present a case study of a patient diagnosed with ectopic pregnancy in a rudimentary horn, who underwent successful laparoscopic resection. Interventions Ten main steps were identified and described in detail during the laparoscopic resection: Step 1: identification of the anatomy; Step 2: uterine mobilization; Step 3: Open Retroperitoneum; Step 4: Coagulation and section of left round ligament; Step 5: Bladder dissection; Step 6: Identification of vessels; Step 7: Coagulation and section of left utero-ovarian vessels; Step 8: Coagulation and section of uterine vessels; Step 9: Section of uterine septum; Step 10: Specimen removed. Conclusion This publication offers a detailed and instructive account of the laparoscopic resection of ectopic pregnancy within a rudimentary uterine horn. The stepwise approach demonstrated in the accompanying video contributes to a deeper understanding of this complex surgical technique.

Diagnosis of Cornual Pregnancy in the Emergency Room: a Rare Case Report

Abstract:Introduction: Ectopic or extrauterine pregnancy is a life-threatening medical condition in which a fertilized egg is implanted elsewhere than in the uterine cavity, commonly in the fallopian tubes. However, it can also occur in the peritoneal cavity, ovaries, and cervix. There are several etiological factors contributing to ectopic pregnancies, such as a history of pelvic inflammatory diseases, habitual abortions, and miscarriages, previous ectopic pregnancies, fertility procedures, as well as increased maternal age that has been correlated with a higher risk of carrying an ectopic pregnancy. Often, the first alerting sign in any ectopic pregnancy is a pain in the pelvic region, along with vaginal bleeding usually spotted between the 6th and 11th gestational week. Other symptoms include vomiting, nausea, tachycardia, and in severe cases hypovolemic shock due to internal bleeding induced by the rupture of the tubes. The key to diagnosing an ectopic pregnancy is transvaginal ultrasound, alongside the measurement of beta human chorionic gonadotropin hormone (beta-hCG) concentrations, which are usually found to be low in such cases. The management of ectopic pregnancy is either medical or surgical. Case report: We are reporting the case of a 32-year-old pregnant woman, who presented to the emergency department for diffuse abdominal pain and recurrent episodes of vomiting and diarrhea of one-week duration. The patient was stable on admission, but later on, she showed a rapid shift in her vital signs. Blood tests were obtained, and pelvic ultrasound was done, followed by a Computed Tomography (CT) scan that revealed the presence of a right cornual ectopic pregnancy. A rapid surgical intervention was carried out to save the patient’s life and alleviate the pain. Conclusion: Ectopic pregnancy is an urgent condition that needs to be treated immediately to avoid its fatal consequences. Several risk factors stand behind it, and a usual history and physical gynecological examinations are insufficient to detect it, hormone screening tests additionally to ultrasounds are highly recommended to decrease the incidence of mortality. A proper follow-up with a specialist could avoid the risk of an ectopic pregnancy as well as its undesirable outcomes.

Design Educational video with narration highlighting surgical management of a cervical ectopic pregnancy with a fertility sparing approach. Setting Single academic center. Patients or Participants 42-year-old female who presented at 8 weeks gestation by frozen embryo transfer who was diagnosed with a cervical ectopic pregnancy by ultrasound early in her prenatal care. She failed medical treatment with a multi-dose methotrexate regimen and was transferred for consultation by the Minimally Invasive Gynecologic Surgery (MIGS) team. She was counseled and desired fertility sparing surgical management. Interventions The patient underwent a diagnostic laparoscopy with several interventions aimed to minimize blood loss which are emphasized in this video. A bilateral temporary uterine artery ligation with laparoscopic hemoclips was performed, and a bladder flap was developed in case of need for an emergent hysterectomy. Vasopressin was injected into the cervix and a suction dilation and curettage was performed under laparoscopic visualization. Finally, a Foley balloon was inserted in the cervix overnight as a tamponade to prevent further bleeding. Measurements and Main Results There were no surgical complications. The patient was observed overnight and the Foley balloon tamponade was removed on postoperative day 1 with no further bleeding. Her beta hCG levels down-trended appropriately. Conclusion This video presents a stepwise approach for surgical management of a rare type of ectopic pregnancy. Cervical ectopic pregnancies are high risk for severe bleeding. Thus, it is important to have an organized approach to complete the procedure with minimal blood loss. The techniques emphasized in this video allow a fertility sparing procedure to be done safely and efficiently.

Abstract:Cornual pregnancy is a rare condition that accounts for approximately 2-4% of ectopic pregnancies worldwide. Herein, we report an unstable case of a 32-year-old woman with a history of oophorectomy and salpingectomy who was admitted for a ruptured cornual pregnancy in the left cornu, which was successfully managed by laparotomy.

Abstract:OBJECTIVE:To evaluate the therapeutic methods and their effects on patients with cornual pregnancies. METHODS:A retrospective study was performed on 83 patients, who were diagnosed as cornual pregnancy at Drum Tower Hospital from June 2010 to April 2015. The patients were divided into 5 groups: a laparoscope group, angle resection and uterine repair guided by laparoscope (n=16); a surgery group, operated with angle resection and uterine repair (n=49); an abortion group, guided by ultrasound or laparoscope (n=6); a drug group, treated by methotrexate and mifepristone (n=8) and a pregnancy bursal puncture group (n=4). We compared the general conditions, surgery circumstances and average days in hospital among the laparoscope group, the surgery group, and the abortion group. Moreover, we also investigated the outcomes of the drug group and pregnancy bursal puncture group. RESULTS:Compared with the surgery group, the intraoperative blood loss in the laparoscope group and abortion group was less (P<0.05). The length of procedure and average days in hospital between the surgery group and the laparoscope group were not statistically significant (P>0.05). The length of procedure in the abortion group was less than that in the laparoscope group or the surgery group (P<0.05). The cure rate was 100%. CONCLUSION:The therapeutic methods based on patient's condition can improve the curative effect and prognosis in cornual pregnancy, which can keep the integrity of generative organs with less injury. The laparoscope might be a main therapeutic method for cornual pregnancy due to its safety, effectiveness, and minimal invasion.

Abstract:Background and Objectives: Cornual pregnancies (CPs) are rare forms of ectopic pregnancy. When abortion does not occur, it can be a life-threatening condition for the mother and can also impair future fertility. We present our experience in the diagnosis and management of CPs. A systematic review was also conducted to investigate the reproductive outcomes after treatment. Materials and Methods: Between January 2010 and December 2022, we performed a retrospective, cross-sectional, single-center, and descriptive data collection and analysis (ClinicalTrial ID: NCT06165770). The search for suitable articles published in English was carried out using the following databases (PROSPERO ID: CRD42023484909): MEDLINE, EMBASE, Global Health, The Cochrane Library (Cochrane Database of Systematic Reviews, Cochrane Central Register of Controlled Trials, and Cochrane Methodology Register), Health Technology Assessment Database, Web of Science, and search register such as ClinicalTrial. Only studies describing the impact of CP treatment on fertility were selected. Results: Two studies were included in the systematic review. Seventeen patients suffering from CPs were selected. In our series, a pelvic ultrasound allowed for the diagnosis of a cornual localization in 35.30% of cases. Thirteen women (76.47%) underwent immediate surgical management. The laparoscopic approach was the most used (76.92%), with a laparotomic conversion rate of 30%. Four patients (23.52%) received medical treatment with methotrexate. After treatment, two patients managed to achieve pregnancy. Conclusions: CP is a rare form of ectopic pregnancy that can quickly become life-threatening for the mother. Ultrasound does not lead to a precise diagnosis in all cases. In the absence of complications and emergencies, laparoscopy is an approach that could be considered valid. For selected asymptomatic patients, medical treatment may be a valid alternative. The data from the studies included in the systematic review, although demonstrating a superiority of medical treatment in terms of future pregnancies, are heterogeneous and do not allow us to reach a definitive conclusion.
