Abstract:Objectives We hypothesized that autonomic atrial remodeling can be reversed by low-level (LL) vagosympathetic nerve stimulation (VNS).Background Previously, we showed that VNS can be antiarrhythmogenic.Methods Thirty-three dogs were subjected to electrical stimulation (20 Hz) applied to both vagosympathetic trunks at voltages 10% to 50% below the threshold that slowed sinus rate or AV conduction. Group 1 (n = 7): Programmed stimulation (PS) was performed at baseline and during 6-h rapid atrial pacing (RAP). PS allowed determination of effective refractory period (ERP) and AF inducibility measured by window of vulnerability (WOV). LL-VNS was continuously applied from the 4th to 6th hours. Group 2 (n = 4): After baseline ERP and WOV determinations, 6-h concomitant RAP+LL-VNS was applied. Sustained AF was induced by injecting acetylcholine (ACh) 10 mM into the anterior right ganglionated plexus (Group 3, n = 10) or applying ACh 10 mM to right atrial appendage (Group 4, n = 9).Results Group 1: The ERP progressively shortened and the Sigma WOV (sum of WOV from all tested sites) progressively increased (p < 0.05) during 3-h RAP then returned toward baseline during 3-h RAP+LL-VNS (p < 0.05). Group 2: 6-h concomitant RAP+LL-VNS did not induce any significant change in ERP and Sigma WOV. Group 3 and Group 4: AF duration (AF-D) and cycle length (AF-CL) were markedly altered by 3-h LL-VNS (Group 3: baseline: AF-D = 389 +/- 90 s, AF-CL = 45.1 +/- 7.8 ms; LL-VNS: AF-D = 50 +/- 15 s, AF-CL = 82.0 +/- 13.7 ms [ both p < 0.001]; Group 4: baseline: AF-D = 505 +/- 162 s, AF-CL = 48.8 +/- 6.6 ms; LL-VNS: AF-D = 71 +/- 21 s, AF-CL = 101.3 +/- 20.9 ms [ both p < 0.001]).Conclusions LL-VNS can prevent and reverse atrial remodeling induced by RAP as well as suppress AF induced by strong cholinergic stimulation. Inhibition of the intrinsic cardiac autonomic nervous system by LL-VNS may be responsible for these salutary results. [Myocardial Protective Effect of Acetylcholine Against Ischemia/reperfusion Injury and Its Underlying Mechanism].

Abstract:OBJECTIVE:To determine whether the caidioprotection of acetylcholine (ACh) against ischeniia/reperftision (I/R) injury is re-kited to mitochondrial permeability transition pore (MEW) and mitochondrial AW-sensitive potassium channel (mitoK(ATP)).METHODS:Male Sprague-Dawley rats were used for Langendorif isolated bean perkision. The hearts were subjected to global ischemia for 30 mm followed by 120 rein of reperfusion and the left ventricular hemodynaniic parameters were measured. Formazan, a product of 2,3, 5-triphenyl-tetrazolium chloride (TTC), which is proportional to myocardial viability, was measured at 490 nm, and the level of lactate dehydrogenase (LDH) in the coronary effluent was measured to evaluate the cardiac injury.RESULTS:The pretreatment with ACh (0.1 mol/L, 5 mm) before I/R markedly increased myocardial formazan content, reduced LDH release, improved the recovery of the left veritficular developed pressure, +/- dP/dtmax, and rate pressure product (left ventricular developed pressure multiplied by hean rate) and attenuated the decrease of coronary flow during reperfusion. The opener of MPTP, atiractyloside (20 mmoL/L) or the inhibitor of mitoK(ATP), 5-hydroxydecanoate (100 micromol/L) abolisbed the beneficial effect of ACh.CONCLUSION:In the isolated rat bean, ACh protects myocardium against ischemia/reperfusion injury via inhibiting the opening of MPTP and increasing the opening of mitoKATP in heart.
Detection of drusen of the optic disc in children by B-scan ultrasonography.

Abstract:Distinguishing between anomalous elevation of the optic disc caused by buried drusen and true papilledema can often be clinically difficult. In a prospective study of 18 children with drusen of the optic disc B-scan ultrasonography with a lowered sensitivity was found to be useful in the uncooperative child. A rational approach to the investigation of elevation of the optic disc in children is offered.

Abstract:In order to investigate the mechanisms responsible for the inotropic effects of muscarinic acetylcholine receptor stimulation by high concentrations of muscarinic receptor agonists, we studied the effects of carbachol at 30–300 μM on the electrically induced [Ca2+]i transient of rat isolated ventricular myocytes. Carbachol at this dose range increased the amplitude and duration of the electrically induced [Ca2+]i transient time and dose dependently. It also increased the resting fluorescence ratio and time to 80% decline of amplitude from the peak. At 100–300 μM the increase in [Ca2+]i transient was followed by a cluster of Ca2+ oscillations in 50–83% of the cells studied. The effects were blocked by atropine, but not pertussis toxin. Depletion of Ca2+ from sarcoplasmic reticulum by ryanodine, which itself reduced the amplitude of the [Ca2+]i transient and increased resting fluorescence, abolished the effect of carbachol on the [Ca2+]i transient without affecting its effect on resting fluorescence ratio. The caffeine-induced [Ca2+]i transient was unaffected by prior addition of carbachol in a Ca2+ free and low Na+ solution. Inhibition of Ca2+ influx by the L-type Ca2+ channel blocker, verapamil, which itself reduced the amplitude of the [Ca2+]i transient without affecting the resting fluorescence ratio, attenuated the augmentation of the amplitude of the [Ca2+]i transient elicited by carbachol. Ni2+, a non-specific Ca2+ channel blocker and an inhibitor of Na+Ca2+ exchange, abolished the effects of carbachol on both [Ca2+]i transient and resting fluorescence ratio. Low external Na+, which increased the resting fluorescence ratio due to its inhibitory effect on Na+Ca2+ exchange, also abolished the effects of carbachol. The results indicate that the inotropic effect of muscarinic acetylcholine receptor stimulation by high concentrations of a muscarinic receptor agonist may be due to an increase in the electrically induced [Ca2+]i transient in ventricular myocytes via a process which is not pertussis toxin sensitive. The increase in the electrically induced [Ca2+]i transient may result from increases in Na+Ca2+ exchange and influx of Ca2+ via voltage-gated Ca2+ channels, and mobilization of Ca2+ from the intracellular store. The mobilization of Ca2+ from the intracellular store is a secondary event. The study has provided evidence for the first time that muscarinic acetylcholine receptor stimulation by high concentrations of carbachol increases Ca2+ influx via the Ca2+ channel and mobilization of Ca2+ from its intracellular store. The study has also demonstrated for the first time the occurrence of Ca2+ oscillations induced by high concentrations of carbachol.

Abstract:Introduction: Electrical stimulation of the cervical vagus nerve is neuroprotective after MCAO in rats; decrease in infarct volume ranges from ~40% (using surgically implanted electrodes) to ~30% (using clinically applicable non-invasive devices). The exact mechanism of vagus nerve stimulation (VNS)-induced neuroprotection is not known, but so far cerebral blood flow-related mechanism has been eliminated. VNS attenuates systemic inflammation via activation of alpha7 nicotinic acetylcholine receptors (α7 nAChR) on macrophages. We hypothesized that α7 nAChR -mediated anti-inflammatory effect plays a mechanistic role in VNS-induced neuroprotection after cerebral ischemia. Methods: Focal ischemia was induced by 2 hour transient MCAO in Wistar rats and verified by >60% drop in baseline cerebral blood flow. Methyllycaconitine (MLA; 10 mg/kg, IP) was used to block α7 nAChRs. Animals were randomly assigned to one of four experimental groups: sham stimulation/vehicle, sham stimulation/MLA, VNS/vehicle, VNS/MLA (n=6 in each). VNS was initiated 30 minutes after MCAO and delivered for 1 hour via implanted electrodes, using previously published stimulation parameters. Animals were euthanized 24 hours later to measure infarct volume by TTC. In separate cohorts, immunohistochemistry was used to assess the changes in brain levels of α7 nAChR-related cytokines, TNF-α and high mobility group box protein 1 (HMGB1), in sham-stimulated and VNS groups. Results: Mean infarct volume in sham-stimulated and VNS groups were 43.83 ± 1.53% and 25.33 ± 3.39%, respectively (p<0.05). MLA abolished the neuroprotective effect of VNS; after MLA treatment mean infarct volume in sham-stimulated and VNS groups were 45.24 ± 4.78% and 48.58 ± 2.78%, respectively (p=n.s.). MLA alone had no effect on ischemic outcome; there was no difference in infarct volume between MLA- treated and untreated sham-stimulated animals (p=n.s.). Ischemia-induced elevation in TNF-α and decrease in HMGB1 levels were completely reversed by VNS as early as 3 hours after ischemia. Conclusion: This study shows that anti-inflammatory mechanisms via nicotinic acetylcholine receptor-mediated signaling play a role in protective effect of VNS against ischemic tissue damage.

Anatomical and functional organization of cardiac fibers in the porcine cervical vagus nerve allows spatially selective efferent neuromodulation

Abstract:Cardiac disease progression reflects the dynamic interaction between adversely remodeled neurohumoral control systems and an abnormal cardiac substrate. Vagal nerve stimulation (VNS) is an attractive neuromodulatory option to dampen this dynamic interaction; however, it is limited by off-target effects. Spatially-selective VNS (sVNS) offers a promising solution to induce cardioprotection while mitigating off-target effects by specifically targeting pre-ganglionic parasympathetic efferent cardiac fibers. This approach also has the potential to enhance therapeutic outcomes by eliminating time-consuming titration required for optimal VNS. Recent studies have demonstrated the independent modulation of breathing rate, heart rate, and laryngeal contraction through sVNS. However, the spatial organization of afferent and efferent cardiac-related fibers within the vagus nerve remains unexplored. By using trial-and-error sVNS in combination with micro-computed tomography fascicle tracing, we show the significant spatial separation of cardiac afferent and efferent fibers (179±55° SD microCT, p<0.05 and 200±137° SD, p<0.05 sVNS – degrees of separation across a cross-section of nerve) at the mid-cervical level. We also show that cardiac afferent fibers are located in proximity to pulmonary fibers consistent with recent findings of cardiopulmonary convergent neurons and circuits. We demonstrate the ability of sVNS to selectively elicit desired scalable heart rate decrease without stimulating afferent-related reflexes. By elucidating the spatial organization of cardiac-related fibers within the vagus nerve, our findings pave the way for more targeted neuromodulation, thereby reducing off-target effects and eliminating the need for titration. This, in turn, will enhance the precision and efficacy of VNS therapy in treating cardiac pathology, allowing for improved therapeutic efficacy.

Long-Term Effects of Ivabradine on Cardiac Vagal Parasympathetic Function in Normal Rats

Abstract:Background: The complex interactions that exist between the pacemaker current, I f , and the parasympathetic nervous system could significantly influence the course of patients undergoing chronic therapy with the I f blocker ivabradine. We thus aimed to assess the effects of chronic ivabradine therapy on autonomic modulation and on the cardiovascular response to in situ and in vitro parasympathetic stimulation. The right atrial expression of HCN genes, encoding proteins for I f , was also evaluated. Methods: Sympathetic and parasympathetic heart rate variability parameters and right atrial HCN (1-4) RNA levels were analyzed in 6 Control and 10 ivabradine-treated male Wistar rats (IVA; 3 weeks, 10 mg/kg/day). The heart rate (HR) and systolic blood pressure (SBP) responses to in situ electrical stimulation of the vagus nerve (2–20 Hz) were assessed in 6 additional Control and 10 IVA rats. The spontaneous sinus node discharge rate (SNDR) response to in vitro cholinergic receptors stimulation using carbamylcholine (10 −9 –10 −6 mol/L) was also assessed in these later rats. Results: Ivabradine significantly increased vagal modulation and shifted the sympatho-vagal balance toward vagal dominance. In Control, in situ vagus nerve stimulation induced progressive decrease in both the SBP ( p = 0.0001) and the HR ( p < 0.0001). Meanwhile, in IVA, vagal stimulation had no effect on the HR ( p = 0.16) and induced a significantly lower drop in SBP ( p < 0.05). IVA also displayed a significantly lower SNDR drop in response to carbamylcholine ( p < 0.01) and significantly higher right atrial HCN4 expression ( p = 0.02). Conclusion: Chronic ivabradine administration enhanced vagal modulation in healthy rats. In addition, ivabradine reduced the HR response to direct muscarinic receptors stimulation, canceled the cardioinhibitory response and blunted the hemodynamic response to in situ vagal stimulation. These data bring new insights into the mechanisms of ivabradine-related atrial proarrhythmia and suggest that long-term I f blockade may protect against excessive bradycardia induced by acute vagal activation.

Abstract:Attenuated cardiac vagal activity is linked to ventricular arrhythmogenesis, and is associated with mortality in patients with chronic heart failure (CHF). However, the sites and mechanisms responsible for decreased cardiac vagal activity are still poorly understood. Our previous studies showed that expression of N-type Ca++ channel α-subunits (Cav2.2-α) and N-type Ca++ currents were reduced in intracardiac ganglion neurons from coronary artery ligation-induced CHF rats. Rat intracardiac ganglia are divided into the atrioventricular ganglion (AVG) and sinoatrial ganglion. Ventricular myocardium only receives projection of neuronal terminals from the AVG. Atrial GIRK Channels Mediate the Effects of Vagus Nerve Stimulation on Heart Rate Dynamics and Arrhythmogenesis

Abstract:Diminished parasympathetic influence is central to the pathogenesis of cardiovascular diseases, including heart failure and hypertension. Stimulation of the vagus nerve has shown promise in treating cardiovascular disease, prompting renewed interest in understanding the signaling pathway(s) that mediate the vagal influence on cardiac physiology. Here, we evaluated the contribution of G protein-gated inwardly rectifying K+ (GIRK/Kir3) channels to the effect of vagus nerve stimulation (VNS) on heart rate (HR), HR variability (HRV), and arrhythmogenesis in anesthetized mice. As parasympathetic fibers innervate both atria and ventricle, and GIRK channels contribute to the cholinergic impact on atrial and ventricular myocytes, we collected in vivo electrocardiogram recordings from mice lacking either atrial or ventricular GIRK channels, during VNS. VNS decreased HR and increased HRV in control mice, in a muscarinic receptor-dependent manner. This effect was preserved in mice lacking ventricular GIRK channels, but was nearly completely absent in mice lacking GIRK channels in the atria. In addition, atrial-specific ablation of GIRK channels conferred resistance to arrhythmic episodes induced by VNS. These data indicate that atrial GIRK channels are the primary mediators of the impact of VNS on HR, HRV, and arrhythmogenesis in the anesthetized mouse.

Abstract:The aberration of programmed cell death including cell death associated with autophagy/mitophagy, apoptosis, necroptosis, pyroptosis, and ferroptosis can be observed in the development and progression of doxorubicin-induced cardiotoxicity (DIC). Vagus nerve stimulation (VNS) has been shown to exert cardioprotection against cardiomyocyte death through the release of the neurotransmitter acetylcholine (ACh) under a variety of pathological conditions. However, the roles of VNS and its underlying mechanisms against DIC have never been investigated. Forty adults male Wistar rats were divided into 5 experimental groups: (i) control without VNS (CSham) group, (ii) doxorubicin (3 mg/kg/day, i.p.) without VNS (DSham) group, (iii) doxorubicin + VNS (DVNS) group, (iv) doxorubicin + VNS + mAChR antagonist (atropine; 1 mg/kg/day, ip, DVNS + Atro) group, and (v) doxorubicin + VNS + nAChR antagonist (mecamylamine; 7.5 mg/kg/day, ip, DVNS + Mec) group. Our results showed that doxorubicin insult led to left ventricular (LV) dysfunction through impaired cardiac autonomic balance, decreased mitochondrial function, imbalanced mitochondrial dynamics, and exacerbated cardiomyocyte death including autophagy/mitophagy, apoptosis, necroptosis, pyroptosis, and ferroptosis. However, VNS treatment improved cardiac mitochondrial and autonomic functions, and suppressed excessive autophagy, apoptosis, necroptosis, pyroptosis, and ferroptosis, leading to improved LV function. Consistent with this, ACh effectively improved cell viability and suppressed cell cytotoxicity in doxorubicin-treated H9c2 cells. In contrast, either inhibitors of muscarinic (mAChR) or nicotinic acetylcholine receptor (nAChR) completely abrogated the favorable effects mediated by VNS and acetylcholine. These findings suggest that VNS exerts cardioprotective effects against doxorubicin-induced cardiomyocyte death via activation of both mAChR and nAChR.

Abstract:Background With chronic ischemia after myocardial infarction, the resulting scar tissue result in electrical and structural remodeling vulnerable to an arrhythmogenic substrate. The cholinergic anti‐inflammatory pathway elicited by vagal nerve via α7 nicotinic acetylcholine receptors (α7‐n AC hR) can modulate local and systemic inflammatory responses. Here, we aimed to clarify a novel mechanism for the antiarrhythmogenic properties of vagal nerve during the ischemic cardiomyopathy ( ICM ). Methods and Results Left anterior descending artery of adult male Sprague‐Dawley rats was ligated for 4 weeks to develop ICM . Western blot revealed that eliciting the cholinergic anti‐inflammatory pathway by nicotine treatment showed a significant reduction in the amounts of collagens, cytokines, and other inflammatory mediators in the left ventricular infarcted border zone via inhibited NF ‐ κB activation, whereas it increased the phosphorylated connexin 43. Vagotomy inhibited the anti‐inflammatory, anti‐fibrosis, and anti‐arrhythmogenic effect of nicotine administration. And immunohistochemistry confirmed that the nicotine administration‐induced increase of connexin 43 was located in intercellular junctions. Furthermore nicotine treatment suppressed NF ‐κB activation in lipopolysaccharide‐stimulated RAW 264.7 cells, and α‐bungarotoxin (an α7‐n AC hR selective antagonist) partly inhibited the nicotine‐treatment effect. In addition, 4‐week nicotine administration slightly improved the cardiac function, increased cardiac parasympathetic tone, decreased the prolonged QT c, and decreased the arrhythmia score of programmed electric stimulation‐induced ventricular arrhythmia. Conclusions Eliciting the cholinergic anti‐inflammatory pathway exerts anti‐arrhythmogenic effects against ICM ‐induced ventricular arrhythmia accompanied by downregulation of cytokines, downgenerating of collagens, decrease in sympathetic/parasympathetic ratio, and prevention of the loss of phosphorylated connexin 43 during ICM . Our findings may suggest a promising therapy for the generation of ICM ‐induced ventricular arrhythmia by eliciting the cholinergic anti‐inflammatory pathway.

Abstract:Background: Recent studies have indicated that various components of cardiac nerve network may play important role in atrial fibrillation (AF). Vagal stimulation shortens atrial effective refractory period, predominantly via the activation of the acetylcholine-activated potassium channel (IKACh) that shortens action potential duration and increases the vulnerability to AF. Vasoactive intestine polypeptide (VIP), a neural polypeptide co-released with acetylcholine (ACh) largely from intrinsic cardiac neurons during vagal stimulation, was shown to contribute to the vulnerability to AF. However, the underlying mechanism remains unclear. The aim of this study is to investigate the effect of VIP on IKACh in the presence of ACh in rat atrial myocytes. Methods: The effects of VIP on IKACh in isolated adult rat atrial myocytes were investigated with voltage patch-clamp recording techniques. IKACh were recorded as an inwardly rectifying current activated by muscarinic agonist carbachol (CCH, 1μM). Furthermore, the post-synaptic signal pathway was investigated with Rp-cAMP (10 μM), a competitive antagonist of cyclic AMP (cAMP), H-89 (0.1μM), PKA inhibitor, and forskolin (0.1μM), an activator of adenylyl cyclase. Results: VIP (1μM) potentiates IKACh at a holding potential of −115 mV (VIP: −19.34 ± 5.22 pA/pF, n=12, vs. baseline: −15.85 ± 2.66 pA/pF, n=14, p<0.05). The enhancement of IKACh by VIP could be mitigated by Rp-cAMP (VIP plus Rp-cAMP: −8.24 ± 1.44 pA/pF n= 6; p<0.05 vs. baseline), and could be inhibited by H-89 (VIP plus H-89: −13.41 ± 2.95 pA/pF n=6; p=0.59 vs. baseline). Furthermore, these enhancements could be mimicked by an activator of adenylyl cyclase, forskolin (−27.35 ± 4.82pA/pF n= 5; p<0.01 vs. baseline). Conclusion: VIP enhanced IKACh in atrial myocytes which might contribute to vagal-induced APD shortening and is likely mediated by a cAMP-PKA dependent pathway.
Cerebellar fastigial nucleus electrostimulation attenuates inflammation in a Post-Infarction rat model by activating cholinergic anti-inflammatory pathway

Abstract:There are negative correlations between indices of heart rate variability (HRV) and markers of inflammation. The inflammation plays an important role in myocardial damages after myocardial infarction (MI). Our previous study found that fastigial nucleus electrostimulation (FNS) improved abnormal HRV in a rat model of MI. Whether it can reduce inflammation and improve cardiac function after MI and the underlying mechanisms remain unknown. 66 Sprague Dawley rats were randomly divided into 4 groups as follows: i) Sham group (sham operation); ii) MI group (left anterior descending coronary artery ligation); iii) FNS + MI group (left fastigial nucleus electrostimulation plus MI); iv) FNL + FNS + MI group (left fastigial nucleus lesion plus FNS plus MI). The serum expressions of acetylcholine (ACh), pro-inflammatory cytokines tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6), and anti-inflammatory cytokines IL-10 were measured by ELISA. Subsequently, the infarct size, the infiltration of inflammatory cells, the fibrotic area, and cardiac function were also evaluated. Additionally, the expressions of the cholinergic anti-inflammatory pathway (CAP)-related proteins in infarct tissue, such as nuclear factor kappa B (NF-κB) and singal transducers and activators of transcription 3 (STAT3), were determined by Western blot. We found that FNS significantly increased ACh and IL-10 levels in serum, and decreased TNF-α and IL-6 levels. FNS significantly attenuated inflammatory cell infiltration, reduced infarct size, decreased fibrosis, increased left ventricular ejection fraction, and reduced mortality. Besides, the ratios of phosphorylated-STAT3/STAT3 and phosphorylated-NF-κB/NF-κB in infarct tissue significantly elevated after MI. FNS reduced the ratios of p-STAT3/STAT3 and p-NF-κB/NF-κB in infarct tissue. The protective effects of FNS were partially reversed by the fastigial nucleus lesion. Our data suggested that FNS can alleviate the inflammation after MI, and its cardiac neuroprotective mechanism may be achieved by increasing vagal tone, releasing ACh, and further activating the CAP via α7 nicotinic acetylcholine receptor. The precise mechanism remains to be elucidated.

Abstract:SCN10A/Na(V)1.8 may be associated with a lower risk of ventricular fibrillation in the setting of acute myocardial infarction (AMI), but if and by which mechanism Na(V)1.8 impacts on ventricular electrophysiology is still a matter of debate. The purpose of this study was to elucidate the contribution of Na(V)1.8 in ganglionated plexi (GP) to ventricular arrhythmias in the AMI model. Twenty beagles were randomized to either the A-803467 group (n = 10) or the control group (n = 10). NaV1.8 blocker (A-803467, 1 mu mol/0.5 mL per GP) or DMSO (0.5 mL per GP) was injected into four major GPs. Ventricular effective refractory period, APD90, ventricular fibrillation threshold, and the incidence of ventricular arrhythmias were measured 1 h after left anterior descending coronary artery occlusion. A-803467 significantly shortened ventricular effective refractory period, APD90, and ventricular fibrillation threshold compared to control. In the A-803467 group, the incidence of ventricular arrhythmias was significantly higher compared to control. A-803467 suppressed the slowing of heart rate response to high-frequency electrical stimulation of the anterior right GP, suggesting that A-803467 could inhibit GP activity. SCN10A/Na(V)1.8 was readily detected in GPs, but was not validated in ventricles by quantitative RT-PCR, western blot and immunohistochemistry. While SCN10A/Na(V)1.8 is detectible in canine GPs but not in ventricles, blockade of NaV1.8 in GP increases the incidence of ventricular arrhythmias in AMI hearts. Our study shows for the first time an influence of SCN10A/Na(V)1.8 on the regulation of ventricular arrhythmogenesis via modulating GP activity in the AMI model.
Optogenetic Therapy on Cardiac Vagal Dysfunction-Related Ventricular Arrhythmia in Type 2 Diabetes

Abstract:Withdrawal of cardiac vagal activity is associated with ventricular arrhythmias-related sudden cardiac death and high mortality in T2DM patients. Although vagal nerve stimulation (VNS) has emerged as a promising therapy for ventricular arrhythmias, VNS-induced off-target side effects due to a lack of organ specificity severely limit its prescription in the clinic. To avoid the limitations of the VNS, we employed an optogenetic technique that combines with a miniaturized bio-optoelectronic implant with genetic targeting strategies to achieve cardiac-specific vagal activation in T2DM rats. We hypothesize that optogenetic activation in cardiac vagal postganglionic (CVP) neurons can restore vagal control of cardiac function, and further reduce susceptibility to ventricular arrhythmias in T2DM. Rat T2DM was induced by a high-fat diet plus streptozotocin injection. AAV-channelrhodopsin-2 (ChR2, 2 μl, 5x1012 vg/ml), an excitatory light-sensitive opsin gene, was in vivo transfected into CVP neurons located in the atrioventricular ganglion (AVG). At three weeks after opsin gene (i.e., AAV-ChR2) transfection, continual optogenetic stimulation in CVP neurons was applied twice daily (10 Hz, 50% duty cycle, 5 mW for 1 hour) by illuminating a LED probe that is controlled and powered wirelessly in conscious rats. Our data from immunofluorescence staining showed that microinjection of AAV-ChR2 into the AVG induced expression of ChR2-mcherry in almost all CVP neurons. Optogenetic activation of CVP neurons resulted in a negative inotropic reaction on the left ventricular systolic pressure in a frequency-dependent manner in anesthetized rats. Data from spectral analysis of heart rate variability demonstrated that optogenetic stimulation gradually restored T2DM-reduced high-frequency power (an index of cardiac vagal activation) from one to three days after optogenetic therapy in vivo, whereas it has no effect on low-frequency power (an index of cardiac sympathetic activation). Additionally, data from 24-hour continuous ECG recording in conscious rats demonstrated that optogenetic stimulation in CVP neurons improved the T2DM-impaired heterogeneity of ventricular electrical activity, which was measured by evaluating ventricular arrhythmia-related ECG parameters at three days after optogenetic therapy in vivo. These data suggested that optogenetic activation in CVP neurons might be an effective intervention against cardiac vagal dysfunction-related ventricular arrhythmias in the T2DM state. Abstract:Background: Ischemic stroke is one of the leading causes of chronic disability worldwide, and stroke-induced heart damage can lead to death. According to research, patients with a variety of brain disease have good clinical results after vagus nerve stimulation (VNS). After ischemic stroke, mast cells (MCs) degranulate and release a large number of mediators, which may cause systemic inflammation. Chymase secreted by MCs can increase the levels of pathological angiotensin II (AngⅡ), which plays a crucial role in the deterioration of heart disease. Our goal was to develop a minimally invasive, targeted, and convenient VNS approach to assess the impact of VNS and to clarify the relationship between VNS and MCs in the prognosis of patients with myocardial atrophy after acute ischemic stroke. Methods: In this study, we verified the role of VNS in the treatment of myocardial atrophy after stroke and its molecular mechanism using a rat model of middle cerebral artery occlusion (MCAO/r). Behavioral studies were assessed using neurobehavioral deficit scores. Enzyme-linked immunosorbent assays, immunofluorescence staining, Western blotting and qRT-PCR were used to analyze the expression levels of myocardial atrophy, MC and inflammatory markers in rat hearts. Results: VNS improved myocardial atrophy in MCAO/r rats, inhibited MC activation, reduced the expression of chymase and AngⅡ, and inhibited the expression of proinflammatory factors. The chymase activator C48/80 reversed these effects of VNS. Chymase activation inhibited the effect of VNS on myocardial atrophy in MCAO/r rats, increased AngⅡ expression and aggravated inflammation and autophagy. The myocardial atrophy of MCAO/r rats was improved after chymase inhibition, and AngⅡ expression, inflammation and autophagy were reduced. Our results suggest that VNS may reduce the expression of chymase and AngⅡ by inhibiting MC activation, thereby improving myocardial atrophy and reducing inflammation and autophagy in MCAO/r rats. Inhibition of MC activation may be an effective strategy for treating myocardial atrophy after stroke. Conclusions: VNS inhibits MC activation and reduces the expression of chymase and AngII, thereby alleviating myocardial atrophy, inflammation and autophagy after stroke.

Abstract:The inhibitory effect of safrazine, an antidepressant monoamine oxidase inhibitor containing a hydrazine terminal, on phenytoin behavior was investigated using isolated rat hepatocytes and rat liver microsomes. Biotransformation of phenytoin to form its oxidation metabolite, 5-phenyl-5-(p-hydroxyphenyl)hydantoin, was greatly inhibited in the liver cells. Examination of microsomes showed that the inhibition mode was a mixed type.
