Abstract:Abstract Abstract 3054 Background: Disease relapse, major infection and graft-versus-host disease (GVHD) are the leading causes of death following allogeneic hematopoietic stem cell transplantation (allo-HSCT). Donor T lymphocytes play a critical role in alloimmune recognition and their ability to detect non-self-antigens can lead to GVHD or contribute to prevention of relapse and infection. Three effector pathways have been described for T-cell cytotoxicity: granzyme B/perforin, Fas/Fas ligand (FasL) and secreted molecules such as tumor necrosis factor (TNF)-alpha. Granzyme B is a proapoptotic serine protease. Fas is widely expressed in immunocompetent cells and non-lymphoid tissues. FasL is mainly expressed on activated cytotoxic T lymphocytes (CTLs). When CTLs recognize target cells, they become activated and express FasL which induces apoptosis of target cells. The Fas gene has been found to be silenced in many tumor types. The importance of those effector pathways for the development of acute GVHD (aGVHD) has been well established. In our previous study, we have reported donor and/or recipient TNF-alpha gene polymorphism influenced on the risk of aGVHD. Recently several important polymorphisms have been identified in granzyme B, Fas, and FasL genes and reported to be relevant to allograft rejection in solid organ transplant patients. However, such information is less available in allo-HSCT. This present study was designed to investigate the influence of donor and recipient genetic variations of effector pathway molecules on the abilities of T-cell alloimmune responses including GVHD, cytomegalovirus (CMV) infection within 100 days and disease relapse following allo-HSCT. Methods: The entire study population consisted of 138 pairs of recipients and their unrelated donors, who underwent HSCT in our Center from January 2001 to March 2009. Genomic DNA was extracted from peripheral blood samples obtained from recipients and donors before transplantation. Five single nucleotide polymorphisms (SNPs) including granzyme B +55(A/G), -295(C/T), Fas -1377(G/A), -670(T/C) and FasL -844 (T/C) were analyzed. Results: (1) Patients receiving stem cells from a donor with granzyme B +55 AA genotype developed aGVHD more frequently than those with mutated allele (AG or GG), which has been reported to be associated with low-producing of granzyme B by CTLs and incapable of inducing apoptosis. Multivariate analysis confirmed that donor with the granzyme B +55 AA genotype was an independent risk factor for aGVHD (RR=1.545, 95%CI: 1.012–2.357, P=0.044). (2) Donor with FasL -844 TT genotype, a genotype reported to a reduction in the expression of FasL on T cells, conferred protection against the risk of aGVHD (RR=2.981, 95% CI: 1.320–6.736, P=0.009), however was an independent risk factor for early CMV infection (RR: 3.658, 95% CI: 1.534–8.722, P=0.003). Patients experiencing aGVHD have a higher risk of developing early CMV infection (RR=1.931, 95% CI: 1.242–3.003, P=0.003), early CMV infection also was a significant factor impacting aGVHD (RR=1.630, 95%CI: 1.294–2.053, P<0.001). (3) Recipient with Fas -670 C- allele positive, a mutated allele with higher Fas expression, was associated with a significantly lower risk of relapse (RR=3.453, 95% CI: 1.699–7.091, P=0.001) as well as superior overall survival (84.2% vs 21.2%, P<0.001) in patients with acute myeloid leukemia (AML), but not in patients with other myelogenous or lymphoid malignancies. The polymorphic sites granzyme B -295(C/T), Fas -1377(G/A), did not correlate with the T-cell alloimmune reactivities. Conclusions: Genetic variation, other than HLA polymorphism, regulating the immune response may influence outcomes of patients undergoing allo-HSCT. Transplant patients would thus benefit from accurate risk assessment and individualization of immunosuppressive therapy, taking into account donor and recipient genetic data, as well as other risk factors. This study is the first report of relationship between donor or recipient genetic variations of effector pathway molecules and the abilities of T-cell alloimmune responses. According to our results, special targeting granzyme B and Fas/FasL pathways may be a promising therapeutic strategy in patients with aGVHD, CMV infection and leukemia relapse after allo-HSCT at least in AML patients harboring Fas -670 wild-type genotype. Myeloid-derived Suppressor Cells Mobilized by G-CSF in Allografts Correlate with Low Risk of Acute Graft-Versus Host Disease in Patients Undergoing Unmanipulated Allogeneic Blood and Marrow Transplantation (126.18)

Abstract:Introduction:The aim of this study was to investigate the association of donor myeloid-derived suppressor cells (MDSCs) in allografts with acute graft-versus host disease(aGVHD) in patients who had undergone granulocyte colony-stimulating factor (G-CSF) mobilized peripheral blood progenitor cell (G-PBPC) and G-CSF-primed bone marrow (G-BM) transplantation. Methods: Unmanipulated allografts from 71 patients were prospectively analyzed for MDSCs with respect to aGVHD, in which 23 patients got HLA-matched donors and 48 patients had haploidentical donors. Results: Median numbers (range) of monocytic/ granulocytic/ promyelocytic MDSCs were:3.43(0.31-10.65)×106/kg, 8.23(0.85-16.2 )×106/kg , 1.11(0.51-3.53)×107/kg ,respectively in G-BM and these of monocytic/ granulocytic MDSCs were:1.64(0.28-4.3)×107/kg , 1.96(0.62-6.2)×107/kg , respectively in G-PBPC. The incidence of grades II-IV aGVHD was 29.6%, and there was no difference between the HLA-matched and haploidentical transplantation group. Patients who received a higher dose of promyelocytic MDSCs(>0.82×107/kg, p<0.001) in G-BM or a higher total dose of monocytic MDSCs in G-BM and G-PBPC (>1.23×107/kg, p<0.001) exhibited a lower incidence of grades II-IV aGVHD. Conclusion: Our results suggest that a high number of MDSCs infused were associated with decreased risk of aGVHD in allogeneic blood and marrow transplantation, which may reflect the immune suppressive function of MDSCs in allogeneic transplantation.

Abstract:Ex vivo CD34+-selected T cell depletion (TCD) has been developed as a strategy to reduce the incidence of graft-versus-host disease (GVHD) after allogeneic (allo) hematopoietic stem cell transplantation (HSCT). Clinical characteristics, treatment responses, and outcomes of patients developing acute (aGVHD) and chronic GVHD (cGVHD) after TCD allo-HSCT have not been well established. We evaluated 241 consecutive patients (median age, 57 years) with acute leukemia (n = 191, 79%) or myelodysplastic syndrome (MDS) (n = 50, 21%) undergoing CD34+-selected TCD allo-HSCT without post-HCST immunosuppression in a single institution. Cumulative incidences of grades II-IV and III-IV aGVHD at 180 days were 16% (95% confidence interval [CI], 12 to 21) and 5% (95% CI, 3 to 9), respectively. The skin was the most frequent organ involved, followed by the gastrointestinal tract. Patients were treated with topical corticosteroids, poorly absorbed corticosteroids (budesonide), and/or systemic corticosteroids. The overall day 28 treatment response was high at 82%. The cumulative incidence of any cGVHD at 3 years was 5% (95% CI, 3 to 9), with a median time of onset of 256 days (range, 95 to 1645). The 3-year transplant-related mortality, relapse, overall survival, and disease-free survival were 24% (95% CI, 18 to 30), 22% (95% CI, 17 to 27), 57% (95% CI, 50 to 64), and 54% (95% CI, 47 to 61), respectively. The 1-year and 3-year probabilities of cGVHD-free/relapse-free survival were 65% (95% CI, 59 to 71) and 52% (95% CI, 45 to 59), respectively. Our findings support the use of ex vivo CD34+-selected TCD allograft as a calcineurin inhibitor-free intervention for the prevention of GVHD in patients with acute leukemia and MDS.
Peripheral Blood Stem Cell Mobilization in Healthy Donors by Granulocyte Colony-Stimulating Factor Causes Preferential Mobilization of Lymphocyte Subsets

Abstract:Background: Allogeneic hematopoietic stem cell transplantation is associated with a high risk of immune-mediated post-transplant complications. Graft depletion of immunocompetent cell subsets is regarded as a possible strategy to reduce this risk without reducing antileukemic immune reactivity. Study design and methods: We investigated the effect of hematopoietic stem cell mobilization with granulocyte colony-stimulating factor (G-CSF) on peripheral blood and stem cell graft levels of various T, B, and NK cell subsets in healthy donors. The results from flow cytometric cell quantification were examined by bioinformatics analyses. Results: The G-CSF-induced mobilization of lymphocytes was a non-random process with preferential mobilization of naïve CD4+ and CD8+ T cells together with T cell receptor αβ+ T cells, naïve T regulatory cells, type 1 T regulatory cells, mature and memory B cells, and cytokine-producing NK cells. Analysis of circulating lymphoid cell capacity to release various cytokines (IFNγ, IL10, TGFβ, IL4, IL9, IL17, and IL22) showed preferential mobilization of IL10 releasing CD4+ T cells and CD3-19- cells. During G-CSF treatment, the healthy donors formed two subsets with generally strong and weaker mobilization of immunocompetent cells, respectively; hence the donors differed in their G-CSF responsiveness with regard to mobilization of immunocompetent cells. The different responsiveness was not reflected in the graft levels of various immunocompetent cell subsets. Furthermore, differences in donor G-CSF responsiveness were associated with time until platelet engraftment. Finally, strong G-CSF-induced mobilization of various T cell subsets seemed to increase the risk of recipient acute graft versus host disease, and this was independent of the graft T cell levels. Conclusion: Healthy donors differ in their G-CSF responsiveness and preferential mobilization of immunocompetent cells. This difference seems to influence post-transplant recipient outcomes.

Abstract:Allogeneic peripheral blood stem cell transplantation (PBSCT) is increasingly used instead of bone marrow transplantation, particularly in HLA identical sibling pairs. Despite the presence of significantly increased numbers of T cells in the PBSC graft, acute graft-versus-host disease (GVHD) is not increased. We have investigated whether granulocyte-colony stimulating factor (G-CSF) administration to PBSCT recipients, both with and without donor G-CSF pretreatment, further modulates acute GVHD in a murine model of PBSCT. Recipients of G-CSF mobilized splenocytes showed a significantly improved survival (P<0.001) and a reduction in GVHD score and serum LPS levels compared with control recipients. G-CSF treatment of donors, rather than recipients, had the most significant effect on reducing levels of tumor necrosis factor (TNFalpha) 7 days after transplantation. As a potential mechanism of the reduction in TNFalpha, we demonstrate G-CSF decreased dendritic cells TNFalpha, and interleukin-12 production to lipopolysaccharide. In conclusion, G-CSF modulates GVHD predominantly by its effects on donor cells, reducing the production of TNFalpha. G-CSF treatment of bone marrow transplantation recipients, without pretreatment of the donor, does not have an impact on acute GVHD.

Abstract:Macrophages (MΦs) are an important immune cell population that are essential for tissue homeostasis and disease pathogenesis. MΦs are now classified as either M1, which produce pro-inflammatory cytokines, or M2, which produce antiinflammatory cytokines. The impact of granulocyte colony-stimulating factor (G-CSF) on MΦs in humans is unclear. Moreover, little is known about the association between MΦ subsets in allografts and the occurrence of acute graft-versus-host disease (aGVHD) in patients who undergo allogeneic hematopoietic stem cell transplantation (allo-HSCT). In the current study, we found that the M1/M2 ratio was markedly decreased in both G-CSF-treated bone marrow (post-BM) and G-CSF-treated peripheral blood from healthy donors. Post-BM MΦs exhibited reduced migration and increased phagocytosis. Moreover, post-BM MΦs reduced the percentage of Th1 and Tc1 lineages and increased the percentage of Th2, Tc2, and Treg lineages. Patients who received BM grafts with a higher M1/M2 ratio exhibited a higher incidence of grade 2–4 aGVHD. In summary, our data indicate that G-CSF decreases the M1/M2 ratio in BM grafts from healthy donors, which may contribute to preventing the occurrence of grade 2–4 aGVHD in patients after allo-HSCT.

Association of Natural Killer Cells in Allografts with Transplant Outcomes in Patients Receiving G-Csf-Mobilized Pbsc Grafts and G-Csf-Primed Bm Grafts from Hla-Haploidentical Donors

Abstract:The aim of this study was to investigate the effects of natural killer (NK) cells on transplant outcomes in patients receiving G-CSF-mobilized PBSC grafts and G-CSF-primed BM grafts from HLA-haploidentical donors. Forty-one haploidentical allogeneic hematopoietic SCT patients were analyzed according to the NK cell concentration in relation to acute GVHD (aGVHD), chronic GVHD (cGVHD), TRM and leukemia-free survival. The patients receiving a higher dose of CD56 bright NK cells (>1.9 × 10 6 /kg) showed a higher incidence of grades II–IV aGVHD (hazard risk (HR), 2.872; P =0.022) and cGVHD (HR, 2.884; P =0.039). A higher CD56 dim /CD56 bri NK cell ratio (>8.0) was correlated with a decreased risk of III–IV aGVHD (HR, 0.290; P =0.065) and TRM (HR, 0.072; P =0.012), thereby increasing the rate of leukemia-free survival (HR, 0.174; P =0.007) after haploidentical transplantation without in vitro T-cell depletion. Our results suggest that a high allograft CD56 dim /CD56 bright NK cell ratio (>8.0) plays an important role in improving transplant outcomes. A higher dose of CD56 bright NK cells might be a predictor for a higher incidence of GVHD.
Immune Tolerance Induced by Cytokines in Allogeneic Hematopoietic Stem Cell Transplantation

Abstract:Allo-HSCT is a potentially curative therapy for many hematological malignancies, the basis of which is graft-versus-leukemia (GVL) effect. However, acute graft-versus-host disease (GVHD) is responsible for 15% to 40% of mortality and is the major cause of morbidity after transplantation. Therefore, separation of GVHD and GVL partially or completely could improve the transplant outcomes. This study focuses on the effects of G-CSF on T cells in peripheral blood stem cell grafts and bone marrow grafts and its mechanisms after in vivo G-CSF application. The separation of GVHD and GVL effect and the mechanisms of which after in vivo G-CSF and interleukin-11 (IL-11) treatment of healthy donors were investigated. The main contributions of this research are listed as follows: (1) Immune tolerance of T cells was induced simultaneously in peripheral blood stem cell grafts and bone marrow grafts after in vivo G-CSF application; (2) T cell hyporesponsiveness and polarization of T cells from Th1 to Th2 were maintained after mixture of G-CSF-mobilized peripheral blood grafts (G-PB) and G-CSF-primed bone marrow grafts (G-BM) according to different proportions in vitro; (3) Treating donor with G-CSF and IL-11 decreased GVHD and retained GVL effect; (4) The incidence of acute GVHD was decreased after Allo-HSCT using G-PB and G-BM as allografts; (5) In combination with other techniques, the HLA barriers were overcomed using G-PB and G-BM as allografts; (6) The incidences of acute GVHD were significantly decreased and the GVL effects were retained or enhanced in relapsed patients after treatment by G-CSF-mobilized peripheral blood graft infusion compared with those received steady-state peripheral blood lymphocyte infusion, indicating that GVHD and GVL could be partially separated in clinical settings. Based on our results, we would conclude that the issues on the deficiency of donors are resolved, and novel strategies offered for the prophylaxis and treatment of patients with hematological malignancies who relapse after Allo-HSCT. Further studies on the mechanism of the separation of GVL and GVHD are warranted.
Recovery of Specific Subsets of Natural Killer and T Cells Highly Associated with Graft-Versus-Host Disease after Haploidentical Stem Cell Transplantation in Acute Leukemia

Abstract:Abstract Background In the allogeneic hematopoietic stem cell transplantation, recent studies showed that T cell and natural killer (NK) cells recovery are implicated in the graft-versus-host disease (GVHD) and graft versus leukemia (GVL) effects. However, the significance of specific subsets of NK and T cell recovery in relation to transplantation outcomes remains to be elucidated in the haploidentical stem cell transplantation (haploSCT). Methods Clinical data of patients with acute myeloid leukemia (n = 21) and acute lymphoblastic leukemia (n = 24) who underwent their first haploSCT between September 2009 and December 2017 were analyzed. Peripheral blood mononuclear cells obtained from 27 patients were examined by multiparametric flow cytometric analysis. PD-1 and Tim-3 expression were examined in CD4+ and CD8+ T-cells and NK cell receptor (NKG2D, NKG2A, NKG2C, DNAM1 and NKp46) expression were analyzed in NK cells, respectively, at the 3 determined times (immediate prior to conditioning therapy, 28 and 90 days after haploSCT). Results Median age at haploSCT was 38 years (range, 21-62) and median follow-up duration was 31.6 months. Myeloablative conditioning was used for 32% and reduced intensity regimen for 68% of patients. GVHD prophylaxis was based on post-transplant cyclophosphamide for 8 (18%) or on anti-thymocyte-globulin for 36 (82%) plus standard prophylaxis. Incidence of grade II-IV acute GVHD was 50%, gastrointestinal tract (GIT) GVHD was 55.6%, non-GIT acute GVHD 35.7%, and chronic GVHD was 52.4%. Longitudinal analysis of immune reconstitution after haploSCT showed that the incidence of acute GVHD was associated with a delayed expansion of the NK cell population and incidence of chronic GVHD was associated with the extent of CD4+ T cell reconstitution. The incidence of acute GVHD was significantly higher in patients with lower counts of CD56bright CD16neg cell (100% for patients with less than 30 cells/uL at day 28 vs 50% for patients with higher counts, P = 0.026), particularly in NKG2A (P = 0.002) and DNAM1 (P = 0.027)-positive NK cell subsets. In univariate analysis, early CMV replication (P < 0.001), chronic GVHD (P = 0.001), donor age ≥ 28years (P = 0.018), CD4/CD8 ratio of product ≥ 2.4 (P = 0.033), and dose of infused T cells ≥ 3.91 x 108 /kg (P = 0.022) were significantly associated with lower 3-year cumulative incidence of relapse after haploSCT. Donor age ≥ 28years was significantly associated with high incidence of chronic GVHD (P = 0.002). Dose of infused T cells ≥ 3.91 x 108 /kg (HR, 0.088; CI, 0.009 to 0.823; P = 0.033) were independent factors for reducing leukemia relapse after adjustment in multivariate analysis. Chronic GVHD was an independent prognostic factor for higher leukemia-free survival rate (72.7% versus 20.1%, P = 0.008). Longitudinal analysis of T cell reconstitution after haploSCT showed that the high dose of infused T cells was associated with the increased expansion of CD4+PD-1- T cells (P = 0.031 at day 28 and P = 0.017 at day 90). Of note, The incidence of chronic GVHD was significantly higher in patients with higher counts of CD4+ T cell at day 28 (100% for patients with over than 150 cells/uL at day 28 vs 38.8% for patients with lower counts, P = 0.008), particularly in CD4+PD-1- subsets (P = 0.008). Among CD4+ T cell, PD-1-/PD-1+ ratio over than 4.5 was significantly associated with increased chronic GVHD (P = 0.005). In 22 patients with chronic GVHD, GIT GVHD was adverse prognostic factor for overall survival (59.5 % in GIT GHVD vs 100% in patients without GIT GVHD, P = 0.063). The incidence of GIT GVHD was significantly higher in patients with lower CD4/CD8 ratio at day 28 (77.8% for patients with less than 1.5 vs 0% for patients with higher ratio, P = 0.045). Conclusions Our findings suggest that high CD56brightCD16neg NK cell count at day 28 after hasploSCT was significantly associated with decreased incidence of acute GVHD. High dose of infused T cells was associated with increased reconstitution of CD4+ PD-1- T cells and high CD4+ T cell counts, particularly in PD-1- subset, are associated with increased development of chronic GVHD. These findings should be further validated for elucidating the roles of these immune effectors cells in the development of GVHD and GVL effect in haploSCT for acute leukemia. Abstract:Background. Graft-versus-host disease (GVHD) is a serious complication after allogeneic hematopoietic stem cell transplantation (HSCT). We designed a functional assay for assessment of individual risk for acute GVHD. Study Design and Methods. Blood samples were collected from patients and donors before HSCT. Two groups of seven patients each were selected, one in which individuals developed acute GVHD grades II-IV and one in which none showed any clinical signs of GVHD. Peripheral blood mononuclear cells (PBMCs) isolated from donors were incubated in mixed lymphocyte cultures (MLCs) with recipient PBMCs. The cells were characterized by flow cytometry before and after MLC. Results. Samples from donors in the GVHD group contained significantly lower frequencies of naïve γδ T-cells and T-cells expressing NK-cell markers CD56 and CD94. Donor samples in this group also exhibited lower frequencies of naïve CD95+ T-cells compared to controls. After MLC, there were dissimilarities in the CD4/CD8 T-cell ratio and frequency of CD69+ T-cells between the two patient groups, with the non-GVHD group showing higher frequencies of CD8+ and CD69+ T-cells. Conclusion. We conclude that a thorough flow cytometric analysis of donor cells for phenotype and allogeneic reactivity may be of value when assessing pretransplant risk for severe acute GVHD.
Presence of innate lymphoid cells in allogeneic hematopoietic grafts correlates with reduced graft-versus-host disease

Abstract:Background: Allogeneic hematopoietic cell transplantation (HCT) can be devastating when graft-versus-host disease (GvHD) develops. GvHD is characterized by mucosal inflammation due to breaching of epithelial barriers. Innate lymphoid cells (ILCs) are immune modulatory cells that are important in the maintenance of epithelial barriers, via their production of interleukin (IL)-22 and their T cell suppressive properties. After chemo- and radiotherapy, ILCs are depleted, and recovery after remission-induction therapy and after allogeneic HCT is slow and incomplete in a significant number of patients, which is associated with an increased risk to develop acute GvHD. Objective: To investigate whether the presence of mature ILCs within G-CSF-mobilized HCT grafts is correlated with the development of acute GvHD after allogeneic HCT. Study design: We analyzed ILCs in a cohort of 36 patients who received allogeneic HCT for a hematologic malignancy, by flow-cytometric immune-phenotyping of prospectively collected, cryopreserved peripheral blood mononuclear cells (PBMCs) and donor-derived HCT grafts collected for the same patients. Biased analysis, with ILCs defined as CD3-lineage-CD45+CD127+CD161+ lymphocytes, was performed using FlowJo version 10 software. Unbiased analysis was done using FlowSOM, which uses a self-organizing map (SOM) with a minimal spanning tree (MST) to define and visualize different clusters present in the samples. Results: Remission-induction therapy significantly depleted ILCs from the blood, and patients who had a relatively low percentage of ILCs before allogeneic HCT were significantly more prone to develop acute GvHD, confirming previous findings in a separate cohort. Allogeneic HCT grafts, which were all obtained from the blood of G-CSF-mobilized healthy donors, contained ILCs at a frequency very similar to the peripheral blood of healthy individuals. The ILC subset composition was also comparable to that of the blood of healthy individuals, with the exception of NKp44+ ILC3s, which were significantly more abundant in HCT grafts. The relative ILC content of the graft tended to correlate with ILC reconstitution after allogeneic HCT, suggesting that peripheral expansion of transplanted mature ILCs may contribute to early ILC reconstitution after allogeneic HCT. Patients who received a relatively ILC-poor HCT graft had a significantly increased risk to develop acute GvHD, compared with patients who received relatively ILC-rich allogeneic HCT grafts. Unbiased phenotypic analysis with the FlowSOM algorithm confirmed that allogeneic HCT grafts of patients who developed acute GvHD contained a lower frequency of ILCs that clustered in NKp44+ ILC3 signature groups. Conclusion: The presence of ILCs in allogeneic HCT grafts is associated with a reduced risk to develop acute GvHD. These data suggest that enhancement of ILC reconstitution of ILC3s in particular, for example via adoptive transfer of ILCs, may prevent acute GvHD and has the potential to improve outcome of allogeneic HCT recipients.

Abstract:BACKGROUND Acute graft-versus-host disease (aGVHD) remains the most common and challenging complication after allogeneic hematopoietic stem cell transplantation (allo-HSCT). An important obstacle to the therapeutic effect of aGVHD is the inability to identify risk factors for an individual patient at the onset of symptoms. We performed a retrospective study with the aim of defining clinically meaningful pre-transplantation risk factors for grades II-IV aGVHD patients. MATERIAL AND METHODS To identify pre-transplantation risk factors for grades II-IV aGVHD after allo-HSCT, we performed a retrospective study in 292 patients who underwent allo-HSCT at our center from January 2010 to July 2015. RESULTS The cumulative incidence of grades II-IV aGVHD was 36.6±2.8%. The most common target organ of aGVHD was the skin (46.7%), followed by the gastrointestinal tract (29.9%). The risk factors we identified included HLA-mismatched related donors (odds ratio [OR], 3.21; 95% confidence interval [CI], 1.82-5.67) and conditioning regimens containing TBI but no ATG (OR, 2.66; 95% CI, 1.40-5.06). Simultaneously, transplantation with an identical sibling donor (OR, 0.31; 95% CI, 0.18-0.55) and the use of ATG in the conditioning regimen containing TBI (OR 0.37; 95% CI, 0.15-0.93) were two factors found to be associated with a decreased risk of grades II-IV aGVHD. CONCLUSIONS Our study suggests that pre-transplantation characteristics of donor and recipient play an important role in identifying patients at high risk for grades II-IV aGVHD, which provide a direction for the prevention and treatment of aGVHD in the future.

Abstract:The study was aimed to investigate the incidences and risk factors of acute and chronic graft-versus-host diseases (GVHD) and to clarify their effects on relapse and survival of recipients after allogeneic hematopoietic stem cell transplantation (allo-HSCT). The clinical data of 100 cases of allo-HSCT were retrospectively analyzed. The incidences and risk factors of aGVHD and cGVHD, relapse and survival were studied. The results showed that 31 cases developed aGVHD of II - IV grade (34.4%) and 14 cases developed aGVHD of III - IV grade (17.7%). HLA matched or mismatched did not show significant difference in the development of aGVHD of II - IV grade (p > 0.05). Previous occurrence of aGVHD was the risk factor for cGVHD (HR = 2.303, p = 0.088). The female was a favorable factor for cGVHD (HR = 0.401, p = 0.055). The relapse rate was lower in patients who developed cGVHD. The development of aGVHD of II - IV grade was the risk factor for overall survival (p < 0.05). The mortality of patients with aGVHD of III - IV grade and mortality of patients with aGVHD of 0 - I grade were 81.0% and 35.7% respectively, there was very significant difference between them (p = 0.000). In conclusion, till now GVHD and graft-versus-leukemia (GVL) effect can not be separated. The positive effect of GVL could be counteracted by GVHD-related mortality. It is necessary to prevent and control the development of severe aGVHD. The development of local cGVHD may be beneficial to the long-term disease-free survival of patients after allo-HSCT.

Abstract:Donor T lymphocytes play a critical role in alloimmune reactivities after allogeneic hematopoietic stem cell transplantation (allo-HSCT). CD28, inducible co-stimulator (ICOS) and cytotoxic T-lymphocyte antigen 4 (CTLA-4) belong to the same family of T-cell costimulatory molecules. After activation, three effector pathways have been described for T-cell cytotoxicity: granzyme B/perforin, Fas/Fas ligand (FasL) and secreted molecules such as TNF-alpha. Near recently, several important polymorphisms have been identified in those genes and reported to be associated with the risk of autoimmune diseases, malignancies and allograft rejection in solid organ transplantation patients. However, such information is less available in allo-HSCT. In the present study, we first investigated the influence of those genes polymorphic features on the abilities of T-cell alloimmune responses in allo-HSCT setting. We analyzed 10 single nucleotide polymorphisms (SNPs) in the CD28, ICOS, CTLA-4, Granzyme B, Fas and FasL genes in 138 pairs of recipients and their unrelated donors (URDs) and 102 pairs of recipients and their HLA-identical sibling donors. (1) We found two SNPs in donors influenced the risk of aGVHD. The association was particularly strong in the URD transplantation cohort. Unrelated donor with Granzyme B +55 mutated genotype (AA) was an independent risk factor for grades II-IV aGVHD (P = 0.024, RR = 1.811, 95%CI, 1.080-3.038). While unrelated donor with CTLA-4 CT60 mutated genotype (AA) was protective (P = 0.025, RR = 3.806, 95%CI, 1.187-12.204). (2) However, donor with CTLA-4 CT60 AA genotype was a risk of early CMV infection (P < 0.0001, RR = 0.383, 95%CI, 0.243-0.605) and relapse after allo-HSCT in AML patients (P = 0.047, RR = 2.792 95%CI, 1.013-7.696). Furthermore, AML patients with Fas -670 homogeneous mutated allele (TT genotype) also had a higher risk of relapse (P = 0.003, RR = 3.823 95%CI, 1.566-9.337). (3) The presence of those susceptible alleles in donor and/ or recipient (patients receiving CTLA-4 CT60 AA donor, patients receiving GranzymeB +55 AA donor, AML patients with Fas -670 TT genotype or with all) resulted in a reduced overall survival compared with those with wild-type genotypes (54.9% vs 69.5%, P = 0.029). The results of this study highlight the important effect of genetic variations in T-cell activation and effector pathways from the donors and recipients on the outcomes of allo-HSCT.

Abstract:Chronic graft- versus -host-disease (cGVHD) is one of the primary causes of morbidity and mortality for patients who undergo allogeneic hematopoietic stem cell transplantation (allo-HCT). In recent years, advancements in allo-HCT have allowed a broader range of patients to receive transplant, particularly older patients. We sought to assess the impact of cGVHD on outcomes in patients undergoing allo-HCT, for older patients as compared to their counterparts. We performed a retrospective analysis of all patients who underwent allo-HCT 1999–2018. Our results showed that those patients who developed cGVHD by D + 180 had an increased risk and incidence of NRM as compared to those patients without cGVHD. There was no significant difference in outcomes for those patients with cGVHD by age (≥60 years old [yo] and <60 yo). These findings suggest the significant morbidity of cGVHD, regardless of age.

Abstract:We investigated the effects of granulocyte colony‐stimulating factor (G‐CSF) on monocytic (M), promyelocytic (P), and granulocytic (G) myeloid‐derived suppressor cells (MDSCs) both in bone marrow and peripheral blood of 20 healthy donors and the association of MDSCs subgroups with acute and chronic graft‐versus‐host disease (aGvHD/cGvHD) in 62 patients who underwent haplo‐identical allogeneic hematopoietic stem cell transplantation (allo‐HSCT). Patients who received a higher absolute counts of M‐MDSCs or P‐MDSCs exhibited lower incidence of grade II–IV aGvHD (P = 0.001; P = 0.031) and extensive cGvHD (P = 0.011; P = 0.021). In the multivariate analysis, absolute counts of MDSCs in allografts emerged as independent factors that reduced the occurrence of grade II–IV aGvHD (M‐MDSCs: HR = 0.087, 95% CI = 0.020–0.381, P = 0.001; P‐MDSCs: HR = 0.357, 95% CI = 0.139–0.922, P = 0.033) and extensive cGvHD (M‐MDSCs: HR = 0.196, 95% CI = 0.043–0.894, P = 0.035; P‐MDSCs: HR = 0.257, 95% CI = 0.070–0.942, P = 0.04). Delayed M‐MDSC reconstitution was associated with aGvHD onset. The 3‐year cumulative incidence of transplant related mortality and relapse, 3‐year probability of disease‐free survival, and overall survival did not differ significantly between these subgroups. Abstract:Introduction The aim of this study was to investigate the association of donor CD4 + T cells expressing CD62L with transplant outcomes. Materials and Methods We report a prospective analysis of 31 patients who were treated with a Bu/Cy regimen, followed by unmanipulated blood and marrow transplantation. Results Median number (range) of CD4 + CD62L + , CD4 + CD45RA + CD62L + , and CD4 + CD45RO + CD62L + cells infused were 0.31(0.05–1.10)×10 8 /kg, 0.22(0.03–0.95)× 10 8 /kg, and 0.17(0.01–0.81)×10 8 /kg, respectively. The incidence of grades II to IV aGVHD was 36%. In a multivariate analysis, infusion of >0.22 × 10 8 CD4 + CD45RA + CD62L + cells infused/kg increased the risk of grades II to IV aGVHD (HR = 4.741, 95% CI = 1.037–21.662, P = 0.045). Thirteen of 31 patients experienced cGVHD, the risk of cGVHD was increased in patients receiving >0.45 × 10 8 CD4 + CD45RA + cells infused/kg (HR = 4.614, 95% CI = 1.265–16.829, P = 0.021). Conclusion Our results suggest that a high cell dose of CD4 + CD45RA + CD62L + cells increase the incidence of grades II–IV aGVHD. A high number of CD4 + CD45RA + cells infused were associated with increased risk of cGVHD in our transplant settings.
Regulatory T-Cells in the Graft and the Risk of Acute Graft-Versus-Host Disease After Allogeneic Stem Cell Transplantation

Abstract:BACKGROUND: FOXP3+ regulatory T-cells (Treg) are important regulators of allo-reactivity and may therefore represent an important predictor for the risk of graft versus-host disease (GVHD) after allogeneic stem cell transplantation.METHODS: To determine the clinical significance of Treg-content in stem cell grafts, we analyzed 58 human leukocyte antigen (HLA)-identical sibling donors (34 patients received myeloablative and 24 patients reduced intense conditioning regimens) and correlated the Treg frequency with clinical outcome after stem cell transplantation (SCT).RESULTS: A mean value of 9.1 x 10(6) CD4+ FOXP3+ Treg per kg body weight (bw) of the recipient was transplanted (ranging from 0.7 to 33.7 x 10(6) Treg/kg bw). Graft content of Treg correlated with mononuclear cells and CD3+ T-cells. Patients receiving low numbers of Treg (Treg(low)) after myeloablative conditioning for SCT had a significantly increased cumulative incidence of 76% for acute GVHD when compared with 23% for individuals receiving high numbers of Treg (Treg(high)). This observation, however, was not made in patients after reduced intense conditioning-SCT. Notably, relapse rate was not significantly different between Treg(low) and Treg(high) patients in either patient group and overall survival was even increased in Treg(high) patients after myeloablative SCT. Finally, low Treg graft levels represent an independent prognostic factor in multivariate analysis for the appearance of acute GHVD.CONCLUSION: Donor-derived Treg might be of particular significance for the development of acute GVHD after myeloablative SCT using HLA-identical sibling donors.

Abstract:Abstract 899 Introduction: The classic definition of acute (aGVHD) and chronic graft-versus-host disease (cGVHD) was based on a cut-off day 100 after transplantation, but this did not reflect that aGVHD can occur later and that symptoms of aGVHD and cGVHD can occur simultaneously. In 2005 a NIH consensus classification was proposed which included 1) classic aGVHD, occurring before day 100, 2) persistent, recurrent or late aGVHD occurring thereafter, 3) classic cGVHD and 4) an overlap syndrome with simultaneous features of aGVHD and cGVHD. Only few studies have evaluated this classification and no studies have determined the differential impact of reduced intensity (RIC) and myeloablative conditioning (MAC). Method: We retrospectively analyzed 202 AML patients who were transplanted between 1999 and 2008. 102 patients received RIC (generally 6×30 mg/m2 FLU, 4×4 mg/kg BU, 4×10 mg/kg ATG) and immunosuppression with CSA/MMF and 100 patients received MAC (generally 6×2 Gy TBI and 2×60 mg/kg CY) and CSA/MTX. Donors were HLA-matched related (n=82), -matched unrelated (n=88) or -mismatched (n=32). Result: Leukocyte recovery was faster after RIC than after MAC (14 vs. 19 days, P Conclusion: This study in AML patients shows that previously established GVHD risk factors remain valid for the new NIH classification. It also confirms the major impact of conditioning intensity on GVHD incidence, the negative prognostic impact of severe aGVHD and the benefit of moderate cGVHD. The new category late aGVHD may only include few patients but will allow more adequate allocation to therapies or clinical trials. Whether the subgroups classic and overlap cGVHD are clinically relevant remains to be determined. Disclosures: No relevant conflicts of interest to declare.

Abstract:BACKGROUND: Dendritic cells (DC) are central to the development of acute graft-versus-host disease (GVHD) following allogeneic hematopoietic stem cell transplantation (alloHSCT). We hypothesized that DC activation status determines the severity of GVHD and that activated DC may be detected in the circulation prior to clinical presentation of GVHD.METHODS: Following transplant, blood samples were obtained twice weekly from alloHSCT patients. Myeloid (CD11c+) and plasmacytoid (CD123hi) DC were enumerated by flow cytometry, and activated myeloid DC were identified using the CMRF-44 monoclonal antibody.RESULTS: Of 40 alloHSCT patients, 26 developed acute GVHD. Severity of GVHD was associated with low total blood DC counts (P=0.007) and with low myeloid and plasmacytoid DC numbers (P=0.015 and 0.003). The CMRF-44 antigen was expressed on blood CD11c+ DC in all cases prior to GVHD onset, whereas of the 14 patients without GVHD, seven had no CMRF-44+ CD11c DC. Patients with CMRF-44+ CD11c+ DC in more than 20% of samples were more likely to subsequently develop acute GVHD (P=0.001, odds ratio=37.1), while patients who developed grade 2-4 GVHD had prior higher percentages of CMRF-44+ CD11c+ DC compared to grade 0-1 GVHD patients (P=0.001). CMRF-44 expression on >7.9% CD11c+ DC predicted for subsequent development of GVHD with a sensitivity of 87.5% and specificity of 79.2%.CONCLUSIONS: Activation status, as assessed by CMRF-44 antigen expression, of blood CD11c+ DC is highly associated with acute GVHD and these cells may be targets for therapeutic intervention.
