Abstract:Neuroimaging studies demonstrate that vision loss in late-blind patients causes the primary visual cortex (V1) to respond to other sensory modalities. In a case-study, we characterized the effect of partial vision restoration with an electronic retinal prosthesis on cross-modal responses in V1. Following successful implantation with the Argus II epiretinal prosthetic system, two late-blind retinitis pigmentosa (RP) patients completed a series of MRI scans to determine: 1) the feasibility of acquiring images in the presence of the implant and 2) subjects’ BOLD response to three tactile tasks. Argus II subjects’ V1 tactile-evoked responses were acquired following counter-balanced periods of device use and no device use, and were further compared to the responses of 9 late-blind RP subjects and 9 sighted control subjects who previously completed the same tasks. Artifacts from the Argus II were limited to the orbit in which the device was implanted. No image distortion was evident in visual cortices for either structural or functional scans. For each subject and tactile task, the extent of the V1 tactile-evoked response increased following a period of not using their device. When compared to the 18 late-blind RP and sighted subjects, one Argus II subject who had been implanted for the shortest period of time (6 weeks) exhibited V1 tactile-evoked BOLD responses similar to RP subjects with only minimal light perception. The Argus II subject who had been implanted the longest (15 weeks) had a response extent similar to sighted control subjects and response strength similar to RP subjects with partial vision loss. Results from these two Argus II subjects indicate that long-term use of a retinal prosthesis may result in a reduction of tactile-evoked responses in V1. Our study also demonstrates that successful acquisition of structural and functional MR images is feasible in the presence of the electronic implant. Meeting abstract presented at VSS 2015
Electrophysiological and Histological Studies of Chronically Implanted Intrapapillary Microelectrodes in Rabbit Eyes.

Abstract:PURPOSE:To determine the safety and efficacy of transsclerally placed intrapapillary wire microelectrodes implanted chronically into the optic nerve head of rabbit eyes.METHODS:Four platinum wire microelectrodes were passed through the sclera and implanted into the optic nerve head of five rabbit eyes for 4-6 months. Color fundus photography, fluorescein angiography, electroretinograms (ERGs), and visually evoked potentials (VEPs) were used to monitor the retina. Electrically evoked potentials (EEPs) were elicited by bipolar electrical stimulation of the optic nerve axons by different combinations of the four electrodes immediately after the implantation and at 1-month intervals thereafter. The effects of the chronic implantation of the electrodes on the morphology of the optic nerve were evaluated by histological and immunohistochemical examinations at 4 and 6 months after the implantation.RESULTS:All of the electrodes remained stable in the implanted sites throughout the post-implantation period, except for one electrode that had pulled out of the optic nerve head at 1 month after implantation. No intraocular infection, inflammation, or vitreoretinal proliferation was observed in any eye. EEPs could be elicited from each pair of electrodes at all testing times. The mean threshold currents (charge densities) to evoke EEPs increased from 19.3 +/- 9.2 microA (6.0 +/- 2.9 microC/cm2) on the implantation day to 78.8 +/- 31.9 microA (24.6 +/- 10.0 microC/cm2) at 1 month after implantation, but did not change significantly thereafter. The implicit time and amplitude of the a- and b-waves of the ERGs and of P1 of the VEPs did not change significantly throughout the post-implantation period. Histological evaluation of the optic nerve head revealed slight tissue encapsulations surrounding the electrode and increased expression of glial fibrillary acidic protein near the surface of the optic nerve.CONCLUSIONS:Implantation of transscleral intrapapillary microelectrodes appears to be safe and effective. These findings indicate that the implantation of microelectrodes in the optic nerve head should be considered for an optic nerve-based prosthesis.

Modelling in biomechanics: an overview.

Abstract:The state of the art in biomechanical modelling is reviewed with specific reference being made to physical and mathematical idealizations of the human body and its segments. Scale models and anthropomorphic dummies which fall within the physical category are discussed briefly. Mathematical models which are much more prevalent and virtually synonymous with computer simulation models are then considered in two major classifications: deformable element and rigid body representations. The former include lumped parameter, discrete parameter, finite element and continuum models and are commonly used to represent the vertebral column. Examples of rigid body models of the total body response and inertial parameter variety are outlined. Finally, topics relating directly to the construction of sport and locomotion models are discussed. These include considerations for developing a static or dynamic model, utilizing Newtonian or Lagrangian equations of motion, and dealing with indeterminate systems.

Abstract:Purpose: To assess the feasibility of a second-generation (44-channel) suprachoroidal retinal prosthesis for provision of functional vision in recipients with end-stage retinitis pigmentosa (RP) over 2.7 years. Design: Prospective, single-arm, unmasked interventional clinical trial. Participants: Four participants, with advanced RP and bare-light perception vision. Methods: The 44-channel suprachoroidal retinal prosthesis was implanted in the worse-seeing eye. Device stability, functionality, and adverse events were investigated at approximately 12-week intervals up to 140 weeks (2.7 years) postdevice activation. Main outcome measures: Serious adverse event (SAE) reporting, visual response outcomes, functional vision outcomes, and quality-of-life outcomes. Results: All 4 participants (aged 39-66 years, 3 males) were successfully implanted in 2018, and there were no device-related SAEs over the duration of the study. A mild postoperative subretinal hemorrhage was detected in 2 recipients, which cleared spontaneously within 2 weeks. OCT confirmed device stability and position under the macula. Improvements in localization abilities were demonstrated for all 4 participants in screen-based, tabletop, and orientation and mobility tasks. In addition, 3 of 4 participants recorded improvements in motion discrimination and 2 of 4 participants recorded substantial improvements in spatial discrimination and identification of tabletop objects. Participants reported their unsupervised use of the device included exploring new environments, detecting people, and safely navigating around obstacles. A positive effect of the implant on participants' daily lives in their local environments was confirmed by an orientation and mobility assessor and participant self-report. Emotional well-being was not impacted by device implantation or usage. Conclusions: The completed clinical study demonstrates that the suprachoroidal prosthesis raises no safety concerns and provides improvements in functional vision, activities of daily living, and observer-rated quality of life. Severe aortic regurgitation as a late complication of temporal arteritis.

Abstract:Two patients with a remote history of pathologically documented giant cell arteritis developed severe regurgitation. The first patient developed severe aortic regurgitation five years after the pathologic documentation of giant cell arteritis of the temporal arteries. Giant cell arteritis involvement of the aortic root was confirmed. The second patient developed aortic regurgitation seven years after pathologic documentation of giant cell arteries of the temporal arteries. Although pathologic confirmation of the aortic root process was not obtained, this case strengthens the clinical association between giant cell arteritis of the temporal arteries and subsequent aortic root dilatation and severe aortic regurgitation. Observation for signs of de novo severe aortic regurgitation is indicated in follow-up of patients with temporal arteritis.

Abstract:Retinal prosthesis can restore partial vision in patients with retinal degenerative diseases such as retinitis pigmentosa and age-related macular degeneration. Epiretinal prosthesis is one of three therapeutic approaches, which received regulatory approval several years ago. The thresholds of an epiretinal stimulation is partly determined by the size of the physical gap between the electrode and the retina after implantation. Precise positioning of epiretinal stimulating electrode array is still a challenging task. In this study, we demonstrate an approach to positioning epiretinal prostheses for an optimal response at the cortical output by monitoring both the impedance at the electrode-retina interface and the evoked-potential at the cortical level. We implanted a single-channel electrode on the epiretinal surface in adult rats, acutely, guided by both the impedance at the electrode-retina interface and by electrically evoked potentials (EEPs) in the visual cortex during retinal stimulation. We observe that impedance monotonously increases with decreasing electrode-retina distance, but that the strongest cortical responses were achieved at intermediate impedance levels. When the electrode penetrates the retina, the impedance keeps increasing. The effect of stimulation on the retina changes from epiretinal paradigm to intra-retinal paradigm and a decrease in cortical activation is observed. It is found that high impedance is not always favorable to elicit best cortical responses. Histopathological results showed that the electrode was placed at the intra-retinal space at high impedance value. These results show that monitoring impedance at the electrode-retina interface is necessary but not sufficient in obtaining strong evoked-potentials at the cortical level. Monitoring the cortical EEPs together with the impedance can improve the safety of implantation as well as efficacy of stimulation in the next generation of retinal implants.

Abstract:Abstract Background To describe the outcomes of patients with retinitis pigmentosa (RP) who received the Argus II Retinal Prosthesis System. Methods This retrospective, interventional case series evaluated 10 consecutive patients who received the Argus II retinal implant and underwent visual function tests with the system on and system off. The main outcome measures were safety (the number, seriousness, and relatedness of adverse events), and visual function measured by computer-based objective tests, including square localization (SL) and direction of motion (DOM). Secondary measures included functional vision performance, including orientation and mobility (O&M) tasks. Results There were no intraoperative complications and all prostheses remained implanted at the end of follow up. The mean patient age was 41.3 years; mean duration of the implant in vivo was 2.1 years. One patient had a suture exposure over the coil suture tab and over the inferior case suture tab at 2 years postoperatively, which was managed successfully. One patient developed mild vitreous hemorrhage that resolved spontaneously. One patient developed high intraocular pressure postoperatively due to a tight scleral band (SB) that was managed successfully. Patients performed significantly better with the Argus II system on than off on all tasks. Conclusion Patients who received the Argus II had a safety profile out to 4 years post-implantation that was markedly better than that observed in the pre-approval phase of the Argus II. In this population of RP patients, the Argus II retinal prosthesis provided useful visual function over several years that likely translates into improved quality of life. Visual Cortical Thickness Increases with Prolonged Artificial Vision Restoration

Abstract:The Argus II retinal prosthesis restores visual perception to late blind patients. It has been shown that structural changes occur in the brain due to late-onset blindness, including cortical thinning in visual regions of the brain. Following vision restoration, it is not yet known whether these visual regions are reinvigorated and regain a normal cortical thickness or retain the diminished thickness from blindness. We evaluated the cortical thicknesses of ten Argus II Retinal Prostheses patients, ten blind patients, and thirteen sighted participants. The Argus II patients on average had a thicker left Cuneus Cortex and Lateral Occipital Cortex relative to the blind patients. The duration of the Argus II use (time since implant in active users) significantly partially correlated with thicker visual cortical regions in the left hemisphere. Furthermore, in the two case studies (scanned before and after implantation), the patient with longer device use (44.5 months) had an increase in the cortical thickness of visual regions, whereas the shorter-using patient did not (6.5 months). Finally, a third case, scanned at three time points post-implantation, showed an increase in cortical thickness in the Lateral Occipital Cortex between 43.5 and 57 months, which was maintained even after 3 years of disuse (106 months). Overall, the Argus II patients cortical thickness was on average significantly rejuvenated in two higher visual regions and, patients using the implant for a longer duration had thicker visual regions. This research raises the possibility of structural plasticity reversing visual cortical atrophy in late-blind patients with prolonged vision restoration.

Feasibility Assessment of an Optically Powered Digital Retinal Prosthesis Architecture for Retinal Ganglion Cell Stimulation

Abstract:Clinical trials previously demonstrated the notable capacity to elicit visual percepts in blind patients affected with retinal diseases by electrically stimulating the remaining neurons on the retina. However, these implants restored very limited visual acuity and required transcutaneous cables traversing the eyeball, leading to reduced reliability and complex surgery with high postoperative infection risks. To overcome the limitations imposed by cables, a retinal implant architecture in which near-infrared illumination carries both power and data through the pupil to a digital stimulation controller is presented. A high efficiency multi-junction photovoltaic cell transduces the optical power to a CMOS stimulator capable of delivering flexible interleaved sequential stimulation through a diamond microelectrode array. To demonstrate the capacity to elicit a neural response with this approach while complying with the optical irradiance limit at the pupil, fluorescence imaging with a calcium indicator is used on a degenerate rat retina. The power delivered by the laser at the permissible irradiance of 4 mW/mm2 at 850 nm is shown to be sufficient to both power the stimulator ASIC and elicit a response in retinal ganglion cells (RGCs), with the ability to generate of up to 35 000 pulses per second at the average stimulation threshold. This confirms the feasibility of generating a response in RGCs with an infrared-powered digital architecture capable of delivering complex sequential stimulation patterns at high repetition rates, albeit with some limitations.

Abstract:Purpose: Extraocular electrical stimulation is known to provide neuroprotection for retinal cells in retinal and optic nerve diseases. Currently, the treatment approach requires patients to set up extraocular electrodes and stimulate potentially weekly due to the lack of an implantable stimulation device. Hence, a minimally-invasive implant was developed to provide chronic electrical stimulation to the retina, potentially improving patient compliance for long-term use. The aim of the present study was to determine the surgical and stimulation safety of this novel device designed for neuroprotective stimulation. Methods: Eight normally sighted adult feline subjects were monocularly implanted in the suprachoroidal space in the peripheral retina for 9–39 weeks. Charge balanced, biphasic, current pulses (100 μA, 500 μs pulse width and 50 pulses/s) were delivered continuously to platinum electrodes for 3–34 weeks. Electrode impedances were measured hourly. Retinal structure and function were assessed at 1-, 2-, 4-, 6- and 8-month using electroretinography, optical coherence tomography and fundus photography. Retina and fibrotic thickness were measured from histological sections. Randomized, blinded histopathological assessments of stimulated and non-stimulated retina were performed. Results: All subjects tolerated the surgical and stimulation procedure with no evidence of discomfort or unexpected adverse outcomes. The device position was stable after a post-surgery settling period. Median electrode impedance remained within a consistent range (5–10 kΩ) over time. There was no change in retinal thickness or function relative to baseline and fellow eyes. Fibrotic capsule thickness was equivalent between stimulated and non-stimulated tissue and helps to hold the device in place. There was no scarring, insertion trauma, necrosis, retinal damage or fibroblastic response in any retinal samples from implanted eyes, whilst 19% had a minimal histiocytic response, 19% had minimal to mild acute inflammation and 28% had minimal to mild chronic inflammation. Conclusion: Chronic suprathreshold electrical stimulation of the retina using a minimally invasive device evoked a mild tissue response and no adverse clinical findings. Peripheral suprachoroidal electrical stimulation with an implanted device could potentially be an alternative approach to transcorneal electrical stimulation for delivering neuroprotective stimulation.

Abstract:In addition to epiretinal and subretinal areas, the optic nerve (ON) is also a candidate location for implanting visual prosthesis to restore vision of patients with retinitis pigmentosa (RP). Since the ON receives all the signals from the retina, stimulating the ON may potentially evoke phosphenes over a wider range of visual field. In this study, we designed a 9-channel microelectrode array and implanted it between the dura mater and pia mater of rabbit ONs by lateral orbitotomy. We recorded the current thresholds and evaluated the efficacy of the array using electrically evoked potentials (EEPs). Spatial discrimination of approximately 20° was verified by EEP maps over visual cortex. A large area of the visual field (over 130° along horizontal meridian) could be activated by this microelectrode array. Visual evoked potentials (VEPs) and different pathological examinations were used to examine potential damage of ONs. One year post implantation, we did not notice significant damages to either the ONs or the microelectrode arrays. EEPs were successfully recorded up to 6months post implantations. However, further studies are still needed to reduce fibrous encapsulation of the microelectrode array, which resulted in a gradual elevation of current thresholds to elicit EEPs.

Abstract:In patients with atrophic age-related macular degeneration, subretinal photovoltaic implant (PRIMA) provided visual acuity up to 20/440, matching its 100μm pixels size. Next-generation implants with smaller pixels should significantly improve the acuity. This study in rats evaluates removal of a subretinal implant, replacement with a newer device, and the resulting grating acuity in-vivo. Six weeks after the initial implantation with planar and 3-dimensional devices, the retina was re-detached, and the devices were successfully removed. Histology demonstrated a preserved inner nuclear layer. Re-implantation of new devices into the same location demonstrated retinal re-attachment to a new implant. New devices with 22μm pixels increased the grating acuity from the 100μm capability of PRIMA implants to 28μm, reaching the limit of natural resolution in rats. Reimplanted devices exhibited the same stimulation threshold as for the first implantation of the same implants in a control group. This study demonstrates the feasibility of safely upgrading the subretinal photovoltaic implants to improve prosthetic visual acuity.

Abstract:Traditional continuous-variable teleportation can only approach unit fidelity in the limit of an infinite (and unphysical) amount of squeezing. We describe a new method for continuous-variable teleportation that approaches unit fidelity with finite resources. The protocol is not based on squeezed states as in traditional teleportation but on an ensemble of single photon entangled states. We characterize the teleportation scheme with coherent states, mesoscopic superposition states, and two-mode squeezed states and we find several situations in which near-unity teleportation fidelity can be obtained with modest resources.

Abstract:Visual prostheses based on a stimulating microelectrode array to restore vision offer a promising approach for the blind and has become a rapidly growing scientific field in neurorehabilitation engineering. A number of research groups from major developed countries lead the research activities in this field. The goal of the C-Sight Project is to develop an implantable microelectronic medical device that will restore useful vision to blind patients. Retinitis pigmentosa (RP) and age-related macular degeneration are the two leading causes for blindness, for which there have been no effective treatments, both surgically and biologically, until now.The ultimate goal of our project is to develop a completely implantable visual prosthesis based on a penetrating microelectrode array at the optical nerve; to implement this, it requires many technical advances. However, in our study, an implantable microcamera, the wearable information processor, and the multichannel neurostimulator are investigated. Electrophysiological experiments were also performed to provide evidence for the feasibility of our approach. Some related psychophysical studies including simulated phosphene positioning and recognition of pixelized images are also reported in this article.
Compound subretinal prostheses with extra-ocular parts designed for human trials: successful long-term implantation in pigs

Abstract:Background: Subretinal implants aim to replace photoreceptor function in patients suffering from degenerative retinal disease like retinitis pigmentosa by topically applying electrical stimuli in the subretinal space. This study-as a last step before upcoming human trials-explored a newly developed surgical technique for permanent implantation of complex subretinal implants with extra-ocular parts. Methods: The implant consisted of a microphoto-diode array (MPDA) with 1550 electrodes and a 4x4 array of gold electrodes for direct electrical stimulation; both were mounted onto a polyimide foil for transscleral placement into the subretinal space. The foil carried connection lanes to a silicone cable that was implanted under the skin and led to a stimulator box in the animal's neck. Surgery was performed in 11 domestic pigs. Improved vitreo-retinal surgical technique consisted of a 180 degrees peripheral retinotomy and use of diathermy to penetrate the choroid in order to avoid choroidal haemorrhage. Subretinal forceps were used to place the implant safely onto the retinal pigment epithelium before the retina was flattened, peripheral laser photocoagulation was applied and the eye was filled with silicon oil. The implant was stabilized by a scleral fixation patch, use of a metal clamp with bone screws on the animal's skull and a tissue ring under the animal's skin in the neck. Behaviour was observed in the freely moving animals after direct subretinal electrical stimulation and funduscopy, optical coherence tomography, fluorescein angiography and histology were performed. Results: All implants were successfully placed subretinally. In three animals a proliferative vitreo-retinopathy was observed after approximately 2 weeks. Otherwise, funduscopy and OCT demonstrated complete retinal attachment and FA showed no retinal vascular abnormalities over and around the implant. The animals showed clear behavioural reactions to electrical stimulation over the whole examination period. Histological examination failed to show any voltage-induced alteration in the cellular architecture of the retina overlying the stimulation electrodes. Conclusions: This study demonstrates the feasibility of a new surgical procedure for highly safe and controlled implantation of complex subretinal devices with extra-ocular parts. The new implant design proved to be safely implantable in free-moving pigs for an observation period of 4 weeks.

Abstract:Abstract Objective. Retinal prostheses hold the potential for artificial vision in blind patients suffering from outer retinal dystrophies. The optimal number, density and coverage of the electrodes that a retinal prosthesis should have to provide adequate artificial vision in daily activities is still an open question and an important design parameter needed to develop better implants. Approach. To address this question, we investigated the interaction between the visual angle, the pixel number and the pixel density without being limited by a small electrode count. We implemented prosthetic vision in a virtual reality environment in order to simulate the real-life experience of using a retinal prosthesis. We designed four different tasks simulating: object recognition, word reading, perception of a descending step and crossing a street. Main results. The results of our study showed that in all the tasks the visual angle played the most significant role in improving the performance of the participant. Significance . The design of new retinal prostheses should take into account the relevance of the restored visual angle to provide a helpful and valuable visual aid to profoundly or totally blind patients.

Abstract:Purpose: To make sure the biocompatibility and to get the permission to use the EPIRET3-device in a clinical trial according to German laws and to European ISO standards. To characterize the systems and the interaction with the retinal cells of RP-subjects after explantation. Methods: We tested the finally designed wireless epiretinal implants preclinically in vitro. The systems were directly seeded with murine L929 fibroblast cells. Stainings for testing the integrity of the cell membranes were perfomed. Indirect tests were used to detect potentially toxical substances in the cell suspension. After implantation in 6 RP-subjects, the system remained for 28 days. Explantation was done, and the electrode devices were characterized using scanning electron microscopy (SEM/REM). Results: The finally designed systems showed no signs of toxicity in the direct cell tests and in the indirect tests of the cell suspension. After implantation, remaining and explantation of the systems we found small groups of retinal cells on the epiretinally located electrode device. The cells were mostly identified as retinal glial (Muller) cells. Conclusions: The EPIRET3-System is a biocompatible and safe retinal implant system, successfully implanted in RP-individuals, and explanted after 28 days.
