Abstract:Objective To monitor the changes of resting energy expenditure in ventilated critically ill children,to compare the results of standard equations and indirect calorimetry (IC) in predicting energy expenditure,and to investigate the possible influence factors of the metabolic status of the critically ill children.Methods From September 2012 to September 2013,56 critically ill children on assisted ventilation and fitting the requirements of IC in pediatric intensive care unit of Shanghai Children's Medical Center were enrolled in this prospective study.IC measurements were performed using metabolic cart on day 1,4,7,10 after trachea intubation.General clinical data of these children were recorded.Results 130 IC measurements were performed in the 56 children.The measured resting energy expenditure (MREE) did not exhibit significant differences among day 1,4,7,and 10 (P =0.379).Although there were no significant differences between MREE and energy expenditure predicted with Schofield and WHO equations (P =0.917,P =0.995),the agreement was poor between the measured and predicted values (R2 =0.185,R2 =0.322).The metabolic status of the children on day 1 of ventilation was only correlated with age (P =0.000) and height (P =0.027),not with severity of underlying diseases or clinical outcomes.Conclusions MREE of IC method in ventilated critically ill children did not significantly change over time in this study.A poor agreement was observed between equationpredicted energy expenditure and MREE.IC measurement of resting energy expenditure is recommended for guiding individual nutritional support among critically ill children so as to improve clinical outcome.

Abstract:There is scarce data on energy expenditure in ill children with different degrees of malnutrition. This study aimed to determine resting energy expenditure (REE) trajectories in hospitalized malnourished children during and after hospitalization. We followed a cohort of children in Bangladesh and Malawi (2–23 months) with: no wasting (NW); moderate wasting (MW), severe wasting (SW), or edematous malnutrition (EM). REE was measured by indirect calorimetry at admission, discharge, 14-and-45-days post-discharge. 125 children (NW, n = 23; MW, n = 29; SW, n = 51; EM, n = 22), median age 9 (IQR 6, 14) months, provided 401 REE measurements. At admission, the REE of children with NW and MW was 67 (95% CI [58, 75]) and 70 (95% CI [63, 76]) kcal/kg/day, respectively, while REE in children with SW was higher, 79 kcal/kg/day (95% CI [74, 84], p = 0.018), than NW. REE in these groups was stable over time. In children with EM, REE increased from admission to discharge (65 kcal/kg/day, 95% CI [56, 73]) to 79 (95% CI [72, 86], p = 0.0014) and was stable hereafter. Predictive equations underestimated REE in 92% of participants at all time points. Recommended feeding targets during the acute phase of illness in severely malnourished children exceeded REE. Acutely ill malnourished children are at risk of being overfed when implementing current international guidelines.

Abstract:Objectives To determine the accuracy of resting energy expenditure (REE) calculated by using the Harris-Benedict(HB) equation, Food and Agriculture Organization/World Health Organization/ United Nations University (FAO/WHO/UNU) equations (FAO equations) , Shizgal-Rosa (SR) equation and the LIU equation in patients with short bowel syndrome(SBS). In addition, to explore the relationship between measured REE and body weight, fat free mass, body cell mass, fat mass and fat mass percent. Methods Fourty-one SBS patients including 30 male and 11 female, aged from 18 to 60 years admitted between January 2001 and October 2010 were enrolled in this study. All patients required long-term parenteral or enteral plus parenteral nutrition support Their mean age and mean stature were (37 ± 16) years and (164. 3 ± 9. 0) cm, and the average body weight and residual small intestine was (47.4 ± 9. 3) kg and (52 ±45) cm. Measured REEs and calculated REEs of SBS patients were estimated respectively by indirect calorimetry and REE equations, and then defined the difference of them. And body mass were metered by body composition analyzer. Results A significant correlation was found between measured REEs (1218 ± 293) Kcal and calculated REEs from the HB equation (r = 0. 588, P < 0.01), the SR equations (r = 0.591,P<0.01), the FAO equations (r=0.411 ,P<0.01) and the LIU equation (r=0.585,P<0.01).In the total sample, the paired t test between measured REEs and REEs derived from the HB equation,SR equation and FAO equation showed no significant difference (P > 0. 05). However, measured REEs were significantly higher than REEs calculated using the LIU equations by 14. 17% (P <0. 01). There was also a significant correlation between measured REEs and body weight, fat free mass and body cell mass (r = 0. 548,0. 641 and 0. 581). Conclusions Indirect calorimetry is preferred when an accurate REE estimate of SBS patients is necessary. However, if this machine is not available, SR equation is recommended to use and LIU equation must be avoided. Fat free mass may be more useful than body weight in REE calculation.
Study on Energy Metabolism in Perioperative Infants

Abstract:Objectives: To evaluate resting energy expenditure (REE) in surgical infants and define the effects of the degree of stress on its level. Subjects and Methods: Between June 1999 and June 2003 forty-five infants awaiting surgery (boys/girls: 32/13) from the Department of Pediatric Surgery were entered in the study. Indirect calorimetry was used to measure the REE of all the patients before operation and 1-7 days after operation. Clinical data were collected, including age, weight, height, diagnosis, operative method, site and duration, and first day postoperative C-reactive protein (CRP) levels. Results: The REE before operation did not differ significantly from the postoperative values (paired t-test, p > 0.05). The predicted REE was significantly higher than the measured REE even before and after the operation (p < 0.01). REE values in the first three postoperative days were lower in the mild stress group than in the severe surgical stress group. Conclusions: REE variation among the patients is relatively large. Changes in REE may be related to the degree of stress. Predicted REE values may be not a good reflection of measured REE.

Abstract:Objective To investigate nutritional treatment in the intensive care unit (ICU) of the mainland China. Methods A cross-sectional study was conducted in 116 ICUs of 118 mainland hospitals on April 26th, 2017. All patients of these ICUs were investigated at 0 o'clock on April 26th. Demographic and clinical parameters of those patients on April 25th (the investigation day) were recorded, including the dates of hospitalization, ICU admission and nutrition initiation and clinical outcome on 28 days after the investigation day. Results A total of 1953 patients were collected, including 631 females and 1306 males. The mean age was (64.1±19.3) years old (1950 cases). The means of Glasgow Coma Scale (GCS), Sequential Organ Failure Assessment (SOFA) and Acute Physiology and Chronic Health Evaluation Ⅱ (APACHEⅡ) scores were (10.76±4.35)(1749 cases), (5.65±3.52)(1783 cases), (17.14±7.31)(1792 cases), respectively. The outcomes of 28 days after the investigation day were 1483 survivors (75.9%), 312 non-survivors (16.0%) and 158 cases (8.1%) being lost to follow-up. There were no significant differences between the males and the females in age, severity of disease and clinical outcomes of 28 days but in height and weight. There were 73.7%(1440 cases) of patients with normal or mildly injured gastrointestinal function, 10.8%(210 cases) with moderately or severely injured function, 1.7%(33 cases) with gastrointestinal failure and 13.2%(258 cases) without evaluation. To the investigation day, enteral nutrition (EN) had been initiated in 69.4%(1356 cases) of patients and parenteral nutrition (PN) in 36.4%(711 cases) of patients. There were 1720(88.1%) patients with EN administration on the investigation day. The proportion of patients with nausea, vomit/regurgitation, aspiration, abdominal pain, abdominal distention and diarrhea was 4.8%(93 cases), 5.4%(105 cases), 0.9%(17 cases), 8.7%(170 cases), 27.5%(538 cases) and 4.3%(84 cases) respectively, while that of patients using EN was 3.1%(40 cases), 4.25%(54 cases), 0.79%(10 cases), 4.41%(56 cases), 26.85%(341 cases) and 5.43%(69 cases) correspondingly. The proportion of cases starting EN within 24, 48 and 72 hours after ICU entry was 22.4%(437/1953), 38.6%(754/1953) and 46.6%(911/1953), respectively. The proportion of cases receiving ≥80% estimated energy target (=past body weight ×25 kcal/kg.d) within 3, 7 and 14 days after ICU entry was 12.9%(78/607), 18.7%(189/1010) and 23%(305/1325) respectively, while that of cases with EN was 9.9%(60/607), 15.0%(151/1010) and 18.6%(246/1325) correspondingly. Conclusions Nowadays, most of patients in the mainland ICUs receive nutrition therapy and the EN usage rate is much higher than the PN rate. However, the time of EN initiation and the target-reaching rate of energy are suboptimal and an individualized plan of nutrition therapy is still missing. Does Resting Energy Expenditure Increase in Children with Newly Diagnosed Solid Malignant Tumor?

Abstract:Objective To gather observational data on resting energy expenditure(REE) of children with newly diagnosed malignant solid tumor,and to examine factors that might be relevant to their energy expenditure change.Methods It is a retrospective study.Data from sixty-one patients with newly diagnosed malignant tumor admitted to department of pediatric surgery in Xinhua Hospital were reviewed in this study.Resting energy expenditure was measured by indirect calorimetry.Patients with benign tumor admitted during the same period were used as the control group.Results The difference of percentage of patients with normal,decreased,or increased REE between benign and malignant groups was not significant(χ2 = 0.466,P = 0.792).Patients with one or more gastrointestinal symptoms had higher percent of Pre-REE compared with patients without any gastrointestinal symptoms,but no significant difference existed(119.8 ± 80.4 vs 91.1 ± 27.2,Z = 0.746,P = 0.456).Different tumor types were associated with different effects on REE.There was no significant difference in REE levels between patients with decreased diet intake and those without(Z = 0.528,P = 0.598).Conclusions It is confirmed that not all patients with malignant tumor have an increased REE.Moreover,the tumor site might be an independent factor affecting patients' REE level.
Postoperative Resting Energy Expenditure in Children with Congenital Heart Disease

Abstract:Objective To determine postoperative resting energy expenditure ( REE) in children with congenital heart disease ( CHD) requiring open heart surgery , to compare measured resting energy expenditure (MREE) with current predictive equations (PEE), and to investigate the possible influencing factors on the metabolic status of the postoperative CHD children .Methods From February 2015 to June 2015 , 150 ventila-ted children admitted to the cardiac intensive care unit after surgery for congenital heart disease in Shanghai Children's Medical Center were enrolled consecutively .Indirect calorimetry ( IC) measurements were performed using metabolic cart 4 hours after surgery .General clinical data were recorded .Results Totally 104 male and 46 female patients were enrolled, with a median age of 14 months (8.3-36.0 months).The non-protein re-spiratory quotient of patient was 0.79 ±0.20, MREE was (264.76 ±61.74) kJ/(kg· d), and PEE using Schofield equations was (278.51 ±93.42) kJ/(kg· d).Although there was no significant differences (P=0.096 ) between MREE and PEE , the agreement was poor between them ( R2 =0.119 ) .Multivariate stepwise regression analysis showed that MREE had significant positive correlation with risk adjustment in congenital heart surgery (RACHS-1) score (P=0.012) and negative correlation with age (P=0.010).Up to 97.33%of children (146/150) had lower energy intake compared with MREE on the 1st postoperative day .Conclu-sions MREE does not increase after surgery in CHD children;however , the substrate utilization is influenced . Factors influencing the postoperative REE include RACHS-1 score and age.The energy intake at 1st day after surgery is generally lower than REE in these children .
Resting energy expenditure in patients with liver cirrhosis: Indirect calorimetry vs. predictive equations

Abstract:Background and objectives: The objective of our study was to explore the accuracy of previously published prediction equations in predicting resting energy expenditure (REE) in patients with liver cirrhosis (LC). We also aimed to develop a novel equation to estimate REE for Chinese patients with LC. Methods and study design: In 90 patients with LC, the agreement between REE measured by Indirect calorimetry (IC) and predictive equations was quantified using paired T-test and visualized using a Bland-Altman Plot. Pearson correlation coefficient (R) was used to measure a linear correlation between REE measured by IC and different predictive equations. Stepwise multiple regression analysis was used to create a new REE equation. Results: The estimated REEs of previous equations were underestimated against REE measured by IC (1610 ± 334 kcal). Lean body mass (LBM) was positively correlated with REE measured by IC (r = 0.723, p < 0.01). The newly derived estimation equation for REE (kcal) was 1274.3 - 209.0 * sex - 5.73 * age + 3.69 * waist circumference + 22.89 * LBM. The newly derived estimation equation was found to have a Pearson-r value of 0.765 compared with REE measured by IC. Conclusions: REE in liver cirrhosis was underestimated by using predictive equations. The new predictive equation developed by using age, sex, waist circumference, and LBM may help estimate REE in Chinese patients with LC accurately and easily.
[Resting Energy Expenditure in Healthy Newborn Infants].

Abstract:OBJECTIVE:To establish the value of the resting energy expenditure (REE) in healthy newborns and evaluate relative factors of REE.METHODS:One hundred and fifty-four healthy newborns (75 boys, 79 girls; birth-weight 2,500-3,999 g) were enrolled in this study. The Apgar score at the 5th minute was equal to or more than 8; the postnatal age was equal to or more than 5 days. The newborns had no apparent defect. The mothers had no history of metabolic and endocrine diseases. REE was measured by Deltatrac II in child canopy mode for 30-45 minutes during asleep or quiet awake status.RESULTS:The average REE was (201.8 +/- 25.4) kJ/(kg.d), which was significantly lower than the predicted REE by Schofield formula[(226.1 +/- 4.8) kJ/(kg.d), P = 0.000], the predicted REE was 12.04% higher than the measured REE. There were no differences in sex and ways of delivery. The newborns whose birth-weight was between 2,500-2,999 g were measured in two modes: baby and child mode, and the REE values were significantly higher (122.6 +/- 25.0) kJ/(kg.d) and (208.8 +/- 26.4) kJ/(kg.d), respectively (P = 0.000).CONCLUSIONS:The prediction formula might be improper for calculating the REE in newborn infants. The indirect calorimetry was the better way to know the actual REE of newborns. The authors recommended that in child mode the measurement of REE in newborns would be the indirect calorimetry, and the REE in healthy newborns was (201.8 +/- 25.4) kJ/(kg.d).
Prediction of resting energy expenditure for adolescents with severe obesity: A multi‐centre analysis

Abstract:Summary Background and Objectives Resting energy expenditure (REE) assessments can help inform clinical treatment decisions in adolescents with elevated body mass index (BMI), but current equations are suboptimal for severe obesity. We developed a predictive REE equation for youth with severe obesity and obesity‐related comorbidities and compared results to previously published predictive equations. Methods Data from indirect calorimetry, clinical measures, and body composition per Dual x‐ray absorptiometry (DXA) were collected from five sites. Data were randomly divided into development (N = 438) and validation (N = 118) cohorts. A predictive equation was developed using Elastic Net regression, using sex, race, ethnicity, weight, height, BMI percent of the 95th%ile (BMIp95), waist circumference, hip circumference, waist/hip ratio, age, Tanner stage, fat and fat‐free mass. This equation was verified in the validation cohort and compared with 11 prior equations. Results Data from the total cohort (n = 556, age 15 ± 1.7 years, 77% female, BMIp95 3.3 ± 0.94) were utilized. The best fit equation was REE = −2048 + 18.17 × (Height in cm) – 2.57 × (Weight in kg) + 7.88 × (BMIp95) + 189 × (1 = male, 0 = female), R2 = 0.466, and mean bias of 23 kcal/day. Conclusion This new equation provides an updated REE prediction that accounts for severe obesity and metabolic complications frequently observed in contemporary youth.

Development of a new equation and validation of earlier resting energy expenditure predicting equations in adults living in Tehran

Abstract:Background: Predictive equations have been considered as a practical approach for estimating resting energy expenditure (REE) across multiple populations, but their accuracy for each community remains to be determined. Thus, the purposes of this study were to determine the validity of REE predictive equations and to develop a new REE predictive equation in adults living in Tehran. Methods: The study included 284 subjects (158 females) aged 18-60 years old from two cross-sectional studies conducted in Tehrani populations. Anthropometric measurements were assessed using standard protocols. REE was measured using indirect calorimetry (IC) and was estimated using preexisting equations. A new equation was also developed based on the REE from IC and variables such as age, sex, height, and weight. Measured REE was compared to new equation and preexisting predictive equations via correlation, linear regression, and Bland-Altman tests. Results: The new equation and the equations by Mifflin-St. Jeor, Livingston, Frankenfield, Nichols, Müller, and Ganpule demonstrated the best predictive value at a group level (mean percentage error=-2.2 to 2.4 %). At an individual level, the new equation and the equations by Mara, Frankenfield, Roza, Nikooyeh, and Harris & Benedict showed the greatest accuracies compared to measured REE (accuracy prediction=50-53%). Conclusion: This study highlights the importance of considering race when predicting REE. It also demonstrates that the newly developed equation is more appropriate in a clinical setting at group but not individual level. Thus, further research is needed to examine the new equation in an independent sample.
A Comparative Study of the Respiratory Indirect Calorimetry and HB Coefficient Method for the Guidance of the Target Energy in Patients with Sepsis

Abstract:Objective To explore the effects of nutritional supports following target energy measured by respiratory indirect calorimetry and HB coefficient method respectively on the nutrition and immune indexes of patients with sepsis. Methods A prospective comparative study of 60 patients with sepsis who was suitable for nutritional support and respiratory indirect calorimetry in the intensive care unit ( ICU) of the hospital from Jan-uary 2015 to October 2015 was conducted. Resting Energy Expenditure ( REE) was measured simultaneously by respiratory indirect calorimetry (n=30) and HB coefficient (Harris-Benedict formula×stress coefficient, n=30) in patients with sepsis and the nutritional support was given according to the MREE measured by the two methods. The nutritional and immune indexes were obtained at 0 day, 3 day, 7 day, and 14 day after nutritional support. The nutritional and immune indexes at 0 day were considered as the baseline and the differences in the nutritional and immune indexes between the baseline and 3 day, 7 day and 14 day were expressed as a "Δ". Values of Δ were compared between the two goups. Results With nutritional support for 3 days, Δalbumin,Δ prealbumin, Δthe total T lymphocytes, Δassist/induction of T cells and ΔIgG were higher in the respiratory indirect calorimetry group than in the HB coefficient group ( P<0. 05) . With nutritional support for 7 days, Δalbumin, ΔT lymphocytes, ΔIgM were higher in the respiratory indirect calorimetry group than in the HB coefficient group ( P<0. 05) . With nutritional support for 14 days, Δtransferrin, Δsuppression/cyto-toxic T cells, ΔIgG, ΔIgA, ΔC3 andΔC4 were higher in the respiratory indirect calorimetry group than in the HB coefficient group ( P<0. 05) . Conclusion Nutritional support with REE measured by respiratory in-direct calorimetry in patients with sepsis is more effective on nutritional and immune related indexes improve-ment have different degrees of improvement, thus respiratory indirect calorimetry is more suitable to guide the target energy of sepsis patients.

Abstract:Background: The main objective of the work was the analysis and description of data on body composition and resting energy expenditure (REE) values of selected groups of patients with obesity whose REE measurement results using indirect calorimetry reached a level below 95% of the predicted REE calculated using the Harris–Benedict (H–B) equation. The sub-goals were to describe the dependence of body composition on the size of the REE and to find out if the deviations between the number of the total measured REE and the REE calculated using H–B in the adapted group (patients with altered REE values, lower than expected caused by long caloric restriction) are significant. Methods: For the research, 71 (39 women and 32 men) patients treated in obesitology were selected. Patients underwent the measurement of resting metabolism using indirect calorimetry (IC) and body composition measurement on the bioimpedance device and, at the same time, the value of resting metabolism was calculated for everyone using the H–B equation. The whole group was divided into five groups according to the deviation of the measurement using IC and the calculation of the H–B equation. Results: In the total set of examined individuals, there were 32.4% with a reduced REE value compared to the REE calculation according to the H–B equation, which corresponds to 23 individuals. In the adapted group, the average measured REE was 2242 ± 616 kcal compared to the H–B calculation of 2638 ± 713 kcal. Statistically, these results were not significant, but a high case-to-case variation was found. The highest deviation from the H–B predictive calculation was −42% and +43% in the whole research group. The amount of muscle tissue in the adapted group averaged 44.3 ± 11.9 kg and the amount of fat-free mass (FFM) 77.9 ± 20.1 kg. When statistically testing the dependence of REE on FFM and muscle tissue in the adapted group, a strong correlation was found. Conclusions: The H–B equation alone is not suitable for setting a suitable diet therapy for an individual with obesity. In order to select and characterize a group of adapted individuals, it will be necessary to use other methods or a larger research sample, and preferably examine and divide patients with specific comorbidities or include their health status.

Abstract:Background: Nutritional problems are frequent among patients with neuromuscular diseases, who consequently need an adequate evaluation. Objective: to describe nutritional assessment and to estimate and measure body composition and energy requirement in children with neuromuscular diseases. Subjects and methods: We performed anthropometry, skinfold measurement and bioelectric impedance analysis (BIA) for estimate and measure, respectively, fat mass (FM). Resting energy expenditure (REE) was estimated by Schofield equations and measured by indirect calorimetry (IC). We compared actual energy intake with post-assessment recommendations. Results: We studied 40 patients, 13.6 ± 3.3 years old (Range: 5.8-19.3), 80% boys, diagnosed with Duchenne Muscular Dystrophy (n = 21), other dystrophies (7), Muscular Spinal Atrophy (7), myopathies (3) and others (2). According to body mass index (BMI) 22.5% were well nourished (zBMI -1 to +1), 17.5% overweight (zBMI +1 to +2), 17.5% obese (zBMI ≥ +2), and 42.5% undernourished (zBMI < -1). Estimated FM was 20.2% (3.6-46.3), lower than BIA measurement: 34.2% (9.6-60.5) p < 0.001. Estimated REE was higher than measured REE: 1325 (813-2244) vs. 1202 (900-2100) kcal/day, p = 0.002. Actual energy intake: 1452 (1033-2476) was higher than recommended: 1300 (900-1900) kcal/day, p < 0.001. Conclusion: Undernutrition and overweight are prevalent in this group of children with neuromuscular diseases. Clinical assessment underestimates FM and overestimates REE.
Energy Expenditure and Energy Intake in 10-12 Years Obese and Non-Obese Chinese Children in A Guangzhou Boarding School

Abstract:Objective: The objective of this study was to observe the variation of energy intake and energy expenditure in the obese Chinese children. Methods: Basic metabolic rate was measured by using open-circuit indirect calorimetry in 54 obese children and 60 non-obese children aged 10-12 years in a full-time boarding school in Guangzhou suburb, China. Total energy expenditure was estimated by using a factorial method. Dietary intake was surveyed by the weighed inventory method. Physical activity was determined using a 2-day activity diary. Results: Univariate analysis showed that basic metabolic rate was significantly higher (p<0.05) in obese children than in non-obese ones, but the significant difference disappeared after controlling for fat free mass and fat mass. Energy intake and total energy expenditure were significantly higher (p<0.05) in obese than in non-obese children. Obese children spent more time in sleeping and light physical activity, but less time in moderate physical activity and vigorous physical activity than non-obese children (p<0.05). Conclusions: Compared to non-obese children, both energy intake and expenditure were higher in obese Chinese children. It appears that an area for preventive strategies may be to encourage increased physical activity expenditure in this age group.

Abstract:Background: While indirect calorimetry (IC) is the gold standard used to calculate specific calorie needs in the critically ill, predictive equations are frequently utilized at many institutions for various reasons. Prior studies suggest these equations frequently misjudge actual resting energy expenditure (REE) in medical and mixed intensive care unit (ICU) patients; however, their utility for surgical ICU (SICU) patients has not been fully evaluated. Therefore, the objective of this study was to compare the REE measured by IC with REE calculated using specific calorie goals or predictive equations for nutritional support in ventilated adult SICU patients. Materials and Methods: A retrospective review of prospectively collected data was performed on all adults (n = 419, 18-91 years) mechanically ventilated for >24 hours, with an Fio 2 ≤ 60%, who met IC screening criteria. Caloric needs were estimated using Harris-Benedict equations (HBEs), and 20, 25, and 30 kcal/kg/d with actual (ABW), adjusted (ADJ), and ideal body (IBW) weights. The REE was measured using IC. Results: The estimated REE was considered accurate when within ±10% of the measured REE by IC. The HBE, 20, 25, and 30 kcal/kg/d estimates of REE were found to be inaccurate regardless of age, gender, or weight. The HBE and 20 kcal/kg/d underestimated REE, while 25 and 30 kcal/kg/d overestimated REE. Of the methods studied, those found to most often accurately estimate REE were the HBE using ABW, which was accurate 35% of the time, and 25 kcal/kg/d ADJ, which was accurate 34% of the time. This difference was not statistically significant. Conclusion: Using HBE, 20, 25, or 30 kcal/kg/d to estimate daily caloric requirements in critically ill surgical patients is inaccurate compared to REE measured by IC. In SICU patients with nutrition requirements essential to recovery, IC measurement should be performed to guide clinicians in determining goal caloric requirements.

Abstract:Background: Total energy expenditure (TEE) is the total energy expended by an individual to sustain life, activities, and growth. TEE is formed by four components: resting energy expenditure (REE), activity energy expenditure (AEE), growth-related energy expenditure (GEE), and the thermic effect of feeding (TEF). Some energy expenditure (EE) components may change throughout childhood and cannot be reliably estimated using prediction formulae. Objective: To summarize measured TEE components as reported in the literature in healthy and critically ill children. Methods: We searched MEDLINE, EMBASE, and CINAHL for studies published between 1946 and 7 September 2023. The primary outcome was energy expenditure. Included studies were published in English and measured one or more of TEE, AEE, GEE, and TEF with Indirect Calorimetry or Doubly Labeled Water in participants between 1 month and 18 years of age. We excluded studies reporting only REE or using predictive equations. Following abstraction, reported values were converted into kcal/kg/day or kcal/day as possible. Weighted mean values were calculated using median or means of EE measurements. Results: We found 138 studies, 8163 patients, and 16,636 eligible measurements. The median (IQR) study sample size was 20 (12, 35) patients. TEE was the most evaluated component. The median (IQR) TEE in infants was 73.1 (67.0, 76.5), in children 78.0 (66.0, 81.3), and in adolescents was 44.2 (41.8, 51.9) kcal/kg/day. Very few studies reported on GEE and TEF. Conclusions: This is one of the first studies that summarizes components of total energy expenditure in different pediatric age groups in healthy and critically ill children. Growth- and feeding-associated energy expenditure are poorly reported in healthy children, while all components of TEE (except REE) are poorly reported in critically ill children.
Resting Energy Expenditure Early after Cardiac Surgery and Validity of Predictive Equations: A Prospective Observational Study.

Abstract:Objective: Several predictive equations have been used to estimate patients' energy expenditure. The study aimed to describe the characteristics of resting energy expenditure (REE) in patients undergoing mechanical ventilation during early postoperative stage after cardiac surgery and evaluate the validity of 9 REE predictive equations. Methods: This was a prospective observational study. Patients aged 18-80 years old, undergone open-heart surgery, were enrolled between January 2017 and 2018. The measured REE (mREE) was evaluated via indirect calorimetry (IC). The predictive resting energy expenditure (pREE) was suggested by 9 predictive equations, including Harris-Benedict (HB), HB coefficient method, Ireton-Jones, Owen, Mifflin, Liu, 25 x body weight (BW), 30 x BW, and 35 x BW. The association between mREE and pREE was assessed by Pearson's correlation, paired t test, Bland-Altman method, and the limits of agreement (LOA). Results: mREE was related to gender, BMI, age, and body temperature. mREE was significantly correlated with pREE, as calculated by 9 equations (all p < 0.05). There was no significant difference between pREE and mREE, as calculated by 30 x BW kcal/kg/day (t = 0.782, p = 0.435), while significant differences were noted between mREE and pREE calculated by other equations (all p < 0.05). Taking the 30 x BW equation as a suitable candidate, most of the data points were within LOA, and the percentage was 95.6% (129/135). Considering the rationality of clinical use, accurate predictions (%) were calculated, and only 40.74% was acceptable. Conclusions: The 30 x BW equation is relatively acceptable for estimating REE in 9 predictive equations in the early stage after heart surgery. However, the IC method should be the first choice if it is feasible.
Influence Factors of Energy Metabolism and Nutritional Risk Screening in Patients with Newly Diagnosed Esophageal Cancer

Abstract:Objective To evaluate the possible factors that may influence the resting energy expenditure (REE) in patients with newly diagnosed esophageal cancer. Methods Totally, 40 patients with newly diagnosed esophageal cancer were prospectively collected from November 2008 to June 2009 in Xinhua Hospital. Nutritional risk screening 2002 (NRS 2002) was performed. REE and body composition were measured using indirect calorimetry and bioeletrical impedence method, respectively. Results Twenty-seven (67.5%) patients were found with nutrition risk, and NRS score was negatively correlated with prealbumin ( r = - 0.444, P = 0.004) and albumin levels (r = - 0.386, P = 0.014). Measured REE and predicted REE values were ( 6770 ± 1360) and (6021 ± 841 ) kJ/d, respectively (P ＜ 0.001 ). Among all 40 patients, 57.5% of them were hypermetabolic,30.0% were normal, and 12.5% were hypometabolic. Stepwise linear regression analysis showed that fat free mass was the only significant determinant variable for REE (P ＜ 0.001 ). Conclusion Fat free mass is a factor than can influence the energy metabolism in patients with newly diagnosed esophageal cancer.
Relative Validity of an Indirect Calorimetry Device for Measuring Resting Energy Expenditure and Respiratory Quotient.

Abstract:BACKGROUND AND OBJECTIVES Resting energy expenditure (REE) and respiratory quotient (RQ) are important for optimal nutritional care. The purpose of this study was to assess the accuracy and repeatability of an indirect calorimetry device (IIM-IC-100) in the measurement of REE and RQ in healthy Chinese adults. METHODS AND STUDY DESIGN A total of 38 healthy adults (19 male and 19 female) aged 18-52 years (25±6 years) were enrolled in this study. REE and RQ were measured by IIM-IC-100 and by VO2000, alternately and in duplicate. RESULTS There was a highly significant correlation between IIM-IC-100 REE and VO2000 REE (r=0.906, p<0.001), with mean IIM-IC-100 REE significantly higher than that of VO2000 (1475±269 vs 1394±313 kcal/d, p=0.002). Bland- Altman analysis revealed that the mean difference between IIM-IC-100 REE and VO2000 REE was 81.3 kcal/d, with limits of agreement of -185 to +347 kcal/d. There was no significant difference in RQ between the two devices. No significant differences were observed between the repeated measurements for both devices. Intrasubject coefficients of variation (CVs) of REE were smaller for IIM-IC-100 (5.8%) than for VO2000 (10.5%), while CVs of RQ were similar for IIM-IC-100 (7.2%) and VO2000 (6.9%). CONCLUSIONS These preliminary data indicated that the IIM-IC-100 showed promise as an accurate and precise tool in the assessment of REE and RQ in healthy Chinese adults.
