Abstract:PURPOSE: For the topical anesthetic, transcriptional transactivator peptide (TAT) modified lidocaine (LID) loaded nanostructured lipid carriers (TAT-NLCs-LID) were prepared and then used for improving transdermal delivery of local anesthetic drug.METHODS: In this study, TAT was conjugated with Distearoyl phosphatidylethanolamine-(polyethylene glycol)2000-maleimide (DSPE-PEG2000-Mal) to obtain TAT-PEG2000-DSPE. TAT-NLCs-LID were successfully prepared and characterized by determination of their particle size, morphology, drug encapsulation efficiency and in vitro drug release behavior. The skin permeation of LID-LNPs was examined using a Franz diffusion cell mounted with depilated mouse skin in vitro and in vivo anesthesia effect was evaluated on mice.RESULTS: The results showed that TAT-NLCs-LID have substantially small mean diameter (157.9 nm) and high encapsulation efficiency (81.8%). From the in vitro skin permeation results, transdermal flux of TAT-NLCs-LID was about several times higher than that of LID solution and NLCs-LID. In vivo anesthesia effect evaluation illustrated that TAT-NLCs-LID can enhance the transdermal delivery of LID by reducing the pain threshold in mice.CONCLUSION: These results indicate that the novel TAT containing drug delivery system is very useful for overcoming the barrier function of the skin and could deliver anesthetic through the skin. TAT-NLCs-LID could function as promising topical anesthetic system.

Optimized Preparation of Naloxone Microemulsion and Its Transdermal Delivery Effect

Abstract:OBJECTIVE To prepare W/O microemulsion formulation containing naloxone and to investigate its transdermal delivery effect in vitro.METHODS The microemulsion was prepared with the system contained of isopropyl myristate-water-lecithin-propanol according to the central composite design(CCD).The steady permeation rate of naloxone hydrochloride was determined in vitro using Franz diffusion cell with the skin of SD male rats.The concentration of naloxone hydrochloride was measured by HPLC.RESULTS The optimal formulation contained lecithin(29%),propano(l25%) and IPM(35%).The steady permeation rate of naloxone microemulsion was 4.33 μg·h-1·cm-2,much higher than that of the aqueous solution(0.20 μg·h-1·cm-2).CONCLUSION Microemulsion enhanced the permeation of naloxone hydrochloride across the skin of SD male rats.
Transdermal Delivery of Lidocaine-Loaded Elastic Nano-Liposomes with Microneedle Array Pretreatment

Abstract:This study aimed to improve the transdermal delivery of lidocaine hydrochloride (LidH) using elastic nano-liposomes (ENLs) and microneedle (MN) array pretreatment. LidH-containing ENLs were prepared using soybean phosphatidylcholine and cholesterol, with Span 80 or Tween 80, using a reverse-phase evaporation method. The ENL particle size, stability, and encapsulation efficiency (EE) were characterized and optimized based on the component ratio, pH, and type of surfactant used. In vitro transdermal diffusion study was performed on MN-pretreated mouse skin using Franz diffusion cells. The anesthetic effects of LidH in various formulations after dermal application were evaluated in vivo in rats by measuring the tail withdrawal latency after photothermic stimulation. Stable LidH-loaded Tween 80 or Span 80 ENLs were obtained with particle sizes of 115.8 and 146.6 nm and EEs of 27% and 20%, respectively. The formulations did not exert any cytotoxicity in HaCaT cells. Tween 80 and Span 80 ENL formulations showed enhanced LidH delivery on pretreated mice skin in vitro and prolonged the anesthetic effect in vivo compared to that by LidH application alone. LidH-loaded ENLs applied to MN-pretreated skin can shorten the onset time and prolong the anesthetic effect safely, which merits their further optimization and practical application.
Matrix Effect-Free On-Line Pass-Through Cleanup Procedure for the Fast Determination of Local Anesthetic Drug by LC-MS/MS

Abstract:A novel nano-titania modified covalent organic frameworks (NTM-COFs) has been synthesized and characterized by transmission electron microscopy (TEM) and scanning electron micrographs (SEM). Besides, NTM-00Fs, as efficient sorbent, has also been evaluated in the on-line pass-through cleanup procedure prior to the analysis of local anesthetic drugs (lidocaine, bupivacaine and tetracaine) in human plasma by liquid chromatography-tandem quadrupole mass spectrometry (LC-MS/MS). Under optimum conditions, the level of matrix effects can be dramatically reduced by the NTM-00Fs based on-line pass-through cleanup procedure, with acceptable recoveries ranging from 88.8% to 103%. Satisfactory trueness and precision of the proposed method were also obtained, with the within- and between-run RSDs less than 7.0% and relative error (REs) less than 12%. The limits of detections (LODs) of lidocaine, bupivacaine and tetracaine were 0.029 mu g1(-1), 0.027 mu g.L-1 and 0.016 mu gl(-1), respectively. The analytical method has been successfully applied for the determination of the plasma concentrations of bupivacaine in five parturients who received an epidural administration of bupivacaine hydrochloride during vaginal delivery. Results demonstrate the applicability of the developed NTM-COFs on-line pass-through cleanup procedure coupled with LC-MS/MS method to clinical studies.
Polymeric Nanocapsules Loaded with Lidocaine: A Promising Formulation for Topical Dental Anesthesia

Abstract:Lidocaine is the most commonly used local anesthetic worldwide, known for its rapid onset and moderate duration of anesthesia. However, it is short-lived and does not effectively promote effective topical anesthesia in the oral cavity when used alone. Our aim was to investigate whether an approximate 50% encapsulation of lidocaine in poly(ε-caprolactone) nanocapsules (LDC-Nano) would be able to increase its permeation and analgesic efficacy and reduce cytotoxicity. In this study, we characterized LDC-Nano and conducted MTT tests with HaCaT cells to assess their in vitro cytotoxicity. Additionally, in vitro permeation assays across the pig esophageal epithelium and the anesthetic efficacy of the hind paw incision model in rats were performed. Plain lidocaine (LDC) was compared with LDC-Nano and lidocaine hydrochloride plus epinephrine (LDC-Epi). The physicochemical characteristics of LDC-Nano were satisfactory (pH: 8.1 ± 0.21; polydispersity index: 0.08 ± 0.01; mean diameter (nm): 557.8 ± 22.7; and encapsulation efficiency (%): 51.8 ± 1.87) and remained stable for up to 4 months. LDC-Nano presented similar in vitro cytotoxicity to LDC but was higher than LDC-Epi (LD50: LDC = 0.48%; LDC-Nano = 0.47%; and LDC-Epi = 0.58%; p < 0.0001). Encapsulation increased the permeability coefficient about 6.6 times and about 7.5 the steady-state flux of lidocaine across the mucosal epithelium. Both encapsulation and epinephrine improved anesthesia duration, with epinephrine demonstrating superior efficacy (100% of animals were anesthetized up to 100, 30, and 20 min when LDC-Epi, LDC-nano, and LDC were used, respectively). Although LDC-Epi demonstrated superior in vivo anesthetic efficacy, the in vitro permeation and cytotoxicity of LDC-Nano indicate promising avenues for future research, particularly in exploring its potential application as a topical anesthetic in the oral cavity.

Study on Percutaneous Permeation of Naloxone Ethosomes

Abstract:OBJECTIVE To investgate the possiblility of percutaneous transport of naloxone which is carried by ethosomes, and the effect of the concentrations of ethanol and naloxone content on the permeation rate. METHODS Naloxone ethosomes was prepared by injection method. With the skin of SD male rat, the penetration experiments in vitro were performed on modified Franz diffusion cells. The amount of drug penetration concentrations of naloxone was determined by HPLC. The accumulation of drug-penetration, the steady penetration rate for different concentration of ethanol and naloxone content were calculated. RESULTS Keeping the concentrations of naloxone at 5 mg·mL-1,the steady rate of naloxone ethosomes with 20%,30%,40%,50% ethanol were (54.3±3.6),(92.0±8.8),(111.4±22.6) and (139.8±7.6)μg·h-1·cm-2, respectively. The value was 1.76,2.98,3.61 and 4.52 higher than that of naloxone aqueous solution [(30.9±3.4)μg·h-1·cm-2].The order of drug amount accumulated in skin was :50% 40%30%20% naloxone ethosomesdrug aqueous solution. The steady rate of naloxone ethosomes with 0.625,1.25,2.5,5,7.5 and 10 mg·mL-1 naloxone were (2.8±1.3),(15.1±2.4),(43.8±5.0),(92.0±8.8),(145.2±4.6) and (193.8±5.7)μg·h-1·cm-2, respectively. CONCLUSION Ethosomes can strikingly improve permeation, and increase the skin accumulation of drug. The steady state flux of naloxne increased with increasing the concentration of ethanol and naloxone content.

Abstract:PURPOSE: Topical anesthesia analgesic therapy has diverse applicability in solving the barrier properties of skin and unfavorable physicochemical properties of drugs. Lidocaine (LID) combined with prilocaine (PRI) has been used as a topical preparation for dermal anesthesia for treatment of conditions such as paresthesia.MATERIALS AND METHODS: In this study, for combination anesthesia and overcoming the drawbacks of LID and PRI, respectively, LID- and PRI-loaded solid lipid nanoparticles (SLNs) and nanostructured lipid carriers (NLCs) were prepared and characterized by determination of their particle size, drug loading capacity, stability, in vitro drug release behavior and in vitro cellular viability. Ex vivo skin permeation and in vivo anesthesia analgesic efficiency of these two systems were also evaluated and compared.RESULTS: Results revealed that combination delivery of the dual drugs exhibited more remarkable efficiency than signal drug-loaded systems. SLN systems have better ex vivo skin permeation ability than NLCs. NLC systems revealed a stronger in vivo anesthesia analgesic effect than SLN systems.CONCLUSION: It can be concluded that SLNs and NLCs have different advantages, and that both carriers are promising dual drug delivery systems for topical anesthetic analgesic therapy.
Improvement of lidocaine skin permeation by using passive and active enhancer methods

Abstract:Lidocaine is generally recognized and preferred for local anaesthesia, but in addition, studies have described additional benefits of lidocaine in cancer therapy, inflammation reduction, and wound healing. These properties contribute to its increasing importance in dermatological applications, and not only in pain relief but also in other potential therapeutic outcomes. Therefore, the purpose of our study was to enhance lidocaine delivery through the skin. A stable nanostructured lipid carrier (NLC), as a passive permeation enhancer, was developed using a 23 full factorial design. The nanosystems were characterized by crystallinity behaviour, particle size, zeta potential, encapsulation efficiency measurements, and one of them was selected for further investigation. Then, NLC gel was formulated for dermal application and compared to a traditional dermal ointment in terms of physicochemical (rheological behaviour) and biopharmaceutical (qualitative Franz diffusion and quantitative Raman investigations) properties. The study also examined the use of 3D printed solid microneedles as active permeation enhancers for these systems, offering a minimally invasive approach to enhance transdermal drug delivery. By actively facilitating drug permeation through the skin, microneedles can complement the passive transport achieved by NLCs, thereby providing an innovative and synergistic approach to improving lidocaine delivery.

Abstract:Analgesic creams find widespread application as adjuncts for localized anesthesia prior to surgical procedures. Nevertheless, the onset of analgesic action is protracted due to the skin barrier's inherent characteristics, which necessitates prolonged intervals of patient and clinician waiting, consequently impinging upon patient compliance and clinician workflow efficiency. In this work, a biodegradable microneedles (MNs) patch was introduced to enhance the intradermal administration of lidocaine cream to achieve rapid analgesia through a minimally invasive and conveniently accessible modality. The polylactic acid (PLA) MNs were mass-produced using a simple hot-pressing method and served the purpose of creating microchannels across the skin's surface for rapid absorption of lidocaine cream. Optical and electron microscopes were applied to meticulously scrutinize the morphology of the fabricated MNs, and the comprehensive penetration tests involving dynamometer tests, evaluation on porcine cadaver skin, artificial film, optical coherence tomography (OCT), transepidermal water loss, and analysis on rats' skins, demonstrated the robust mechanical strength of PLA MNs for successful intradermal penetration. The behavioral pain sensitivity tests on living rats using Von Frey hair filaments revealed that the MN-assisted lidocaine treatment expeditiously accelerated the onset of action from 40 to 10 min and substantially enhanced the efficacy of localized anesthesia. Furthermore, different treatment protocols encompassing the sequence of drug application relative to MN treatment, MN dimensions, and the frequency of MN insertions exhibited noteworthy influence on the resultant local anesthesia efficacy. Together, these results demonstrated that the lidocaine cream followed by diverse PLA MN treatments would be a promising strategy for rapid clinical local anesthesia with wide-ranging applications.

Abstract:Objective: To evaluate the efficacy and safety of the tropically applied transdermal lidocaine that is enhanced by NS-TP-5B type painless mirconeedle patch. Methods: Healthy volunteers (n = 30) were enrolled in a randomized, double-blind and self-controlled clinical trial. Each volunteer's two arms were randomly selected to apply either test or control patch. The test patch was NS-TP-5B type painless mirconeedle patch and the control patch was sham microneedle patch. Then pain scores were determined by a VAS method after tropic administration of 2% lidocaine solution. Results: Twenty minutes after the treatment with painless mirconeedle patch, the pain scores of test group were significantly reduced as compared to baseline scores on a scale of 0 ∼ 10(4.7 ± 1.5 vs 6.1 ± 1.5, P < 0.001), while the pain scores of control group 20 min after the treatment with sham microneedle patch were similar to the baseline. Conclusion: The painless microneedle patch can enhance the permeability of topically applied transdermal lidocaine.
The structure of hobo transposable elements and their insertion sites

Abstract:The hobo transposable elements of Drosophila form a family of 3.0‐kb elements and their deletion derivatives. Their distribution is consistent with the model that 3.0‐kb elements are functionally complete but that smaller hobos are defective and require complete elements in trans for transposition. The sequence of one 3.0‐kb element is presented; it has several interesting features, including a 1.9‐kb open reading frame downstream from potential TATA and CAT sequences. Comparison of 11 independent insertion sites shows that in every case the hobo element has integrated at and duplicated either the sequence NNNNNNAC or CTTTNNNN. There is evidence that an eight nucleotide sequence internal to hobo that matches both of these sequences has been used as an insertion site for a second hobo element, as the first step in the creation of an internal deletion derivative. Structural similarities between hobo and the eukaryotic transposable elements P, Ac, 1723, and Tam3, found in widely divergent host organisms, suggest that they all transpose by a common mechanism.
Lidocaine–Liposomes—A Promising Frontier for Transdermal Pain Management

Abstract:(1) Background: We aim to develop novel gel formulations for transdermal drug delivery systems in acute and inflammatory pain therapy. (2) Methods: We induced inflammation by the injection of λ-carrageenan on the hind paw of 80 Wistar male rats. The animals were randomized into eight groups of 10 rats each: C (placebo gel), E (EMLATM), L (lidocaine 2%), L-CD (lidocaine + cyclodextrin 2.5%), L-LP (lidocaine + liposomes 1.7%), L-CS (lidocaine + chitosan 4%), L-CSh (lidocaine + chitosan hydrochloride), and L-CS-LP (lidocaine + chitosan + liposomes). The behavior response was determined with a hot plate, cold plate, and algesimeter, each being performed at 30, 60, 120, 180, and 240 min after pain induction. At the end of the experiment, tissue samples were collected for histological assessment. (3) Results: L-LP had the greatest anesthetic effects, which was proven on the cold plate test compared to placebo and EMLATM (all p ≤ 0.001). L-CS-LP had a significant effect on cold plate evaluation compared to placebo (p ≤ 0.001) and on hot plate evaluation compared to EMLATM (p = 0.018). (4) Conclusions: L-LP is a new substance with a substantial analgesic effect demonstrated by the cold plate in the first 120 min. Further studies with more animals are needed to determine the maximum doses that can be applied for a better analgesia with minimum side effects.

Abstract:Lidocaine hydrochloride (LIDH) as an anesthetic is widely used in local anesthesia. Dissolving microneedles (MNs) have great application value in the field of skin anesthesia. However, the limited drug-loading of dissolving MNs is an existing challenge that affects clinical use. In this study, we have screened isomaltulose (ISO) as the proper matrix material for the MNs by using molecular dynamics (MD) simulation. Our findings indicate that ISO has good compatibility with LIDH, and the LIDH-loaded ISO MNs (LI-MNs) have high drug-loading capacity. The drug-loading capacity of LI-MNs could reach 80%, and it could effectively puncture the skin. In addition, the preparation method of customized LI-MNs was established based on three-dimensional (3D) printing technology. It was shown that the administration time of LI-MNs could be controlled within 3 min. Also, the LI-MNs were able to provide the local anesthetic efficacy within 2 min and sustained for more than 2 h. Significantly, LI-MNs had more efficient drug efficacy compared to the topical creams and the majority of existing LIDH-loaded dissolving MNs. They even provided a longer duration of action than the injections. Overall, the LI-MNs with high drug-loading have a promising application prospect.

Fabrication of lidocaine-loaded polymer dissolving microneedles for rapid and prolonged local anesthesia

Abstract:There is an urgent need for research into effective interventions for pain management to improve patients' life quality. Traditional needle and syringe injection were used to administer the local anesthesia. However, it causes various discomforts, ranging from brief stings to trypanophobia and denial of medical operations. In this study, a dissolving microneedles (MNs) system made of composite matrix materials of polyvinylpyrrolidone (PVP), polyvinyl alcohol (PVA), and sodium hyaluronate (HA) was successfully developed for the loading of lidocaine hydrochloride (LidH). The morphology, size and mechanical properties of the MNs were also investigated. After the insertion of MNs into the skin, the matrix at the tip of the MNs was swelled and dissolved by absorption of interstitial fluid, leading to a rapid release of loaded LidH from MNs' tips. And the LidH in the back patching was diffused into deeper skin tissue through microchannels created by MNs insertion, forming a prolonged anesthesia effect. In addition, the back patching of MNs could be acted as a drug reservoir to form a prolonged local anesthesia effect. The results showed that LidH MNs provided a superior analgesia up to 8 h, exhibiting a rapid and long-lasting analgesic effects. Additionally, tissue sectioning and in vitro cytotoxicity tests indicated that the MNs patch we developed had a favorable biosafety profile.

Abstract:The aim of this research was to investigate the stability of a lidocaine-loaded nanostructured lipid carrier dispersion at different temperatures, formulate a nanostructured lipid carrier gel, and test the penetration profile of lidocaine from the nanostructured lipid carrier gel using different skin penetration modeling methods. The formulations were characterized by laser diffraction, rheological measurements and microscopic examinations. Various in vitro methods were used to study drug release, diffusion and penetration. Two types of vertical Franz diffusion cells with three different membranes, including cellulose, Strat-M®, and heat separated human epidermis were used and compared to the Skin-parallel artificial membrane permeability assay (PAMPA) method. Results indicated that the nanostructured lipid carrier dispersion had to be gelified as soon as possible for proper stability. Both the Skin-PAMPA model and Strat-M® membranes correlated favorably with heat separated human epidermis in this research, with the Strat-M® membranes sharing the most similar drug permeability profile to an ex vivo human skin model. Our experimental findings suggest that even when the best available in vitro experiment is selected for modeling human skin penetration to study nanostructured lipid carrier gel systems, relevant in vitro/in vivo correlation should be made to calculate the drug release/permeation in vivo. Future investigations in this field are still needed to demonstrate the influence of membranes and equipment from other classes on other drug candidates.

Dissolving Microneedles for Rapid and Painless Local Anesthesia

Abstract:Microneedles are emerging drug delivery methods for painless treatment. The current study tested dissolving microneedles containing lidocaine (Li-DMN) for use in local anesthesia. An Li-DMN patch was fabricated by centrifugal lithography with carboxymethyl cellulose as a structural polymer and assessed for physical properties by optical microscopy and a fracture force analyzer. The biocompatibility was evaluated by a histology section in vitro and by ear thickness in vivo. The efficacy of the Li-DMN patch was assessed by electrophysiological recordings in primary cultured sensory neurons in vitro and a von Frey test on rats’ hind paws in vivo. The physical properties of the microneedle showed enough rigidity for transdermal penetration. The maximal capacity of lidocaine-HCl in the Li-DMN patch was 331.20 ± 6.30 μg. The cytotoxicity of the dissolving microneedle to neuronal cells was negligible under an effective dose of lidocaine for 18 h. Electrophysiological recordings verified the inhibitory effect of the voltage-gated sodium channel current by the Li-DMN patch in vitro. A skin reaction to the edema test and histologic analysis of the rats’ ears after application of the Li-DMN patch were negligible. Also, the application of the Li-DMN patch reduced the nocifensive behavior of the rats almost immediately. In conclusion, the dissolving microneedle patch with carboxymethyl cellulose is a promising candidate method for the painless delivery of lidocaine-HCl.

Abstract:Twelve patients were studied by means of Magnetic Resonance Imaging (MRI) in order to demonstrate either loco-regional recurrence of central chondrosarcomas or degenerative evolutions of exostoses. MRI findings were compared with plain film, scintigraphy, and Computed Tomography. MRI showed loco-regional recurrences by demonstrating their site, extent, and relationship with adjacent structures. In showing the degenerative evolution of exostoses MRI--confirmed at surgery--was superior to plain film in 3 cases out of 7, and to Computed Tomography in 2 cases. The authors discuss MRI findings: chondrosarcomas have low-intensity signal in T1-weighted sequences and high-intensity signal in T2-weighted. Thanks to its high contrast resolution MRI always allowed the detection of the chondrosarcoma. Nevertheless, MRI did not allow the tumor grading--which is due either to a difficult evaluation of the morphology of neoplastic calcifications, or to non-specific-intensity signal.
Local anesthetic effects of lidocaine-loaded carboxymethyl chitosan cross-linked with sodium alginate hydrogels for drug delivery system, cell adhesion, and pain management

Abstract:Research highlights • CCS-SA hydrogel were successfully prepared and characterized. • CCS-SA hydrogel exhibit good LID drug loading and encapsulation efficiency. • LID-loaded CCS-SA hydrogel exhibited prolonged in vitro release and non-toxicity. • CCS-SA/LID system promising skin drug-delivery system for local anesthetic drug. • In vivo results demonstrate that the LID-loaded CCS-SA hydrogel has no significant toxicity and excellent biocompatibility. The development of hydrophilic carriers for drug encapsulation represents a current challenge. In this work lidocaine (LID) loaded with carboxymethyl chitosan (CCS) cross-linked with sodium alginate (SA) hydrogels were prepared to achieve the controlled release of LID drug. The mechanical characteristics of the CCS-SA hydrogels were examined via the swelling ratio, in vitro degradation, and rheological analysis. The as-prepared CCS-SA hydrogels were characterized by FTIR, XRD, and SEM analysis. The morphology of the CCS-SA hydrogels composites was confirmed porous structure. Satisfactory encapsulation and release characteristics were obtained for the LID-loaded CCS-SA hydrogels. Cytotoxicity assays test on Balb-c 3T3 cells confirmed that the LID-loaded CCS-SA hydrogels reduced its toxicity, relative to the free LID drug. The live/dead cell assay showed that the LID-loaded CCS-SA hydrogels using cell encapsulation study are nontoxic and cytocompatible toward Balb-c 3T3 cells. The anesthetic efficacy of LID-loaded CCS-SA hydrogels was evaluated using Balb/c mice. When administered in inflamed areas, LID-loaded CCS-SA hydrogels emerged as having stronger and longer analgesic effects. The measurement of CCS-SA hydrogels loaded with LID intravenously to tumor-bearing mice demonstrated active targeting in tumor tissues 24 h after injection. Based on these results, it appears that LID-loaded CCS-SA hydrogels have a local anesthetic effect that can effectively reduce injection times for postoperative pain management and limit the adverse effects of anti-inflammatory drugs. Effect of Lidocaine Solid Lipid Nano-particles on Sciatic Nerve Anesthesia Block in Rats

Abstract:This research aimed at exploring the effect of lidocaine solid lipid nano-particles on sciatic nerve anesthesia block in rats. Firstly, we prepared liposomes encapsulated with lidocaine (nano-lidocaine).Then, we stimulated sciatic nerve of rats injected with narcotic drugs, and evaluated their sensory nerve block and motor nerve block. Finally, we observed their systemic toxic reaction and inflammatory infiltration. We found that compared with rats injected with free lidocaine, the sciatic nerve block time of those injected with nano-lidocaine was longer, and its blocking effect on sensory and motor nerve was stronger than that of those injected with free lidocaine. No systemic toxic reaction was observed in the experimental group. Histological examination manifested that there was mild or moderate inflammatory cell infiltration in the free lidocaine group, while there was no obvious cell infiltration in the nano-lidocaine group. Thus, we believe that lidocaine encapsulated with nano-liposomes has a small particle size, which can produce extra long sciatic nerve block in rats without systemic toxicity and local tissue damage. So, it has satisfactory anesthetic effect and safety.
Capsaicin-loaded Nanolipoidal Carriers for Topical Application: Design, Characterization, and in Vitro/in Vivo Evaluation

Abstract:Capsaicin has been used in clinical applications for the treatment of pain disorders and inflammatory diseases. Given the strong pungency and high oil/water partition coefficient of capsaicin, capsaicin-loaded nanolipoidal carriers (NLCs) were designed to increase permeation and achieve the analgesic, anti-inflammatory effect with lower skin irritation. Capsaicin-loaded NLCs were prepared and later optimized by the Box-Behnken design. The physicochemical characterizations, morphology, and encapsulation of the capsaicin-loaded NLCs were subsequently confirmed. Capsaicin-loaded NLCs and capsaicin-loaded NLCs gel exhibited sustained release and no cytotoxicity properties. Also, they could significantly enhance the penetration amount, permeation flux, and skin retention amounts of capsaicin due to the application of NLCs. To study the topical permeation mechanism of capsaicin, 3,3'-dioctadecyloxacarbocyanine perchlorate (Dio) was used as a fluorescent dye. Dio-loaded NLCs and Dio-loaded NLCs gel could effectively deliver Dio up to a skin depth of 260 and 210 μm, respectively, primarily through the appendage route on the basis of version skin sections compared with Dio solution, which only delivered Dio up to 150 μm. In vivo therapeutic experiments demonstrated that capsaicin-loaded NLCs and capsaicin-loaded NLCs gel could improve the pain threshold in a dose-dependent manner and inhibit inflammation, primarily by reducing the prostaglandin E2 levels in the tissue compared with capsaicin cream and capsaicin solution. Meanwhile, skin irritation was reduced, indicating that application of NLCs could decrease the irritation caused by capsaicin. Overall, NLCs may be a potential carrier for topical delivery of capsaicin for useful pain and inflammation therapy.
