Abstract:AbstractOceanic melting at the base of the floating Antarctic ice shelves is now thought to be a more significant cause of mass loss for the Antarctic ice sheet than iceberg calving. In this study, a 10-km horizontal-resolution circum-Antarctic ocean–sea ice–ice shelf model [based on the Regional Ocean Modeling System (ROMS)] is used to study the delivery of ocean heat to the base of the ice shelves. The atmospheric forcing comes from the ERA-Interim reanalysis (~80-km resolution) and from simulations using the polar-optimized Weather Research and Forecasting Model (30-km resolution), where the upper atmosphere was relaxed to the ERA-Interim reanalysis. The modeled total basal ice shelf melt is low compared to observational estimates but increases by 14% with the higher-resolution winds and just 3% with both the higher-resolution winds and atmospheric surface temperatures. Abstract:Bumetanide is a potent 'loop' diuretic for the treatment of oedema associated with congestive heart failure, hepatic and renal diseases, acute pulmonary congestion and premenstrual syndrome and in forced diuresis during and after surgery. Bumetanide may be given orally, intravenously or intramuscularly and produces a rapid and marked diuresis, and increased urinary excretion of sodium, chloride and other electrolytes (within 30 minutes) which persists for 3 to 6 hours. Its principal site of action is on the ascending limb of the loop of Henle, with a secondary action on the proximal tubule. Pharmacologically, bumetanide is about 40-fold more potent than frusemide (furosemide), with the exception of its effects on urinary potassium excretion where its potency is lower. Studies in patients with oedema due to congestive heart failure, pulmonary oedema or hepatic disease show that oral or intravenous bumetanide 0.5 to 2 mg/day produces results comparable to those with frusemide 20 to 80 mg/day. In acute pulmonary oedema, intravenous bumetanide produces a very rapid diuresis. Higher doses of bumetanide may be required (up to 15 mg/day) in patients with chronic renal failure or nephrotic syndrome. In these patients muscle cramps are not uncommon with bumetanide, but glomerular filtration rates are unaffected. In most studies, diuretic effects were accompanied by decreased bodyweight, abdominal girth and improvements in a variety of haemodynamic parameters. Comparison of bumetanide with frusemide at a dose ratio of 1 : 40 reveals no significant differences in clinical response with the exception of renal disease, where patients with oedema appear to respond better to bumetanide. Combination with thiazide diuretics enhances the clinical response to bumetanide. Potassium supplements and spironolactone may be beneficial additions to bumetanide where patients at risk of hypokalaemia can be identified. Clinically important side effects are infrequent, with audiological impairment occurring to a lesser extent than with frusemide. Bumetanide thus offers an important alternative to frusemide when a 'loop' diuretic is indicated.

Abstract:Cyclophilin B (CyPB) is secreted in biological fluids such as blood or milk and binds to a specific receptor present on the human lymphoblastic cell line Jurkat and on human peripheral blood lymphocytes. This study was intended to specify the areas of CyPB that are involved in the interaction with the receptor. A synthetic peptide corresponding to the first 24 N-terminal amino acid residues of CyPB was shown to specifically recognize the receptor. Moreover, modification of Arg18 of CyPB by p-hydroxyphenlglyoxal led to a dramatic loss of affinity for the receptor. However, when this residue was replaced by an alanine residue using site-directed mutagenesis, no modification of the binding properties was found, suggesting that Arg18 is not directly involved but is sufficiently close to the interaction site to interfere with the binding when modified. Competitive binding experiments using a chimaeric protein made up of the 24 N-terminal amino acid residues of CyPB fused to the cyclophilin A core sequence confirmed the involvement of this region of CyPB in receptor binding.
SAT-Based ATPG for Zero-Aliasing Compaction

Abstract:Aliasing in the test response compaction is an important source of fault coverage loss. Methods to avoid the aliasing generally require modification of the compactor to some extent. This can lead to a higher compactor complexity and consequently to higher area overhead, longer signal propagation delays, etc.We propose a novel method, the Zero-aliasing ATPG (ZATPG), which is able to reduce the aliasing without need of designing new compactors. ZATPG works by augmenting the SAT-based ATPG process to constrain test pattern generation to produce no aliasing in the compactor. The method is general enough to be applicable to any compactor design.We demonstrate our method on a LFSR-based MISR compactors, using the Single Stuck-At fault model. Our method is able to find a test with zero aliasing and complete fault coverage for smaller compactors than conventional, unguided ATPG. Thus, the area overhead of the compactor can be reduced, while the complete fault coverage is preserved.

Abstract:assess this case report for the efficacy of ciclosporin due to the multimodal treatment given to the dog. Co-therapy with antibiotics, home-made topical glucocorticoid preparations and azathioprine was used, and the azathioprine was continued after the cessation of ciclosporin; it is therefore unclear whether the ciclosporin was responsible for remission. The use of home-made preparations is also unlicensed and its efficacy difficult to measure. The fact that it only involves one dog and there is no comparison possible also makes assessments on the efficacy of the treatment method difficult. Lesional scores were recorded but only as percentages, rather than a recognised scoring system.

Abstract:Eleven plasma variables have been studied in 18 elderly patients with cardiac oedema treated with bendrofluazide, frusemide and bumetanide, using a fully balanced, cross-over trial design. Bumetanide caused the least disturbance of plasma potassium and bendrofluazide the greatest. Frusemide produced an intermediate effect. The difference between the changes in plasma potassium produced by bumetanide and bendrofluazide was significant (P less than 0.02).

Abstract:In an open, parallel, randomized study, bumetanide was compared with furosemide in the long-term management of patients with severe renal edema. The doses used were: bumetanide, 1 mg; furosemide, 40 mg. Graded increases were administered to daily maxima of 10 mg bumetanide and 400 mg furosemide. The efficacy of parameters measured were: weight, recumbent and standing pulse and blood pressure, abdominal girth, and estimated degree of malaise or fatigue when present. Safety parameters included: physical examination, CBC and platelets, blood chemistry, chemical and microscopic urinalysis, creatinine clearance, air audiometry, and ophthalmic examination. Weight loss and reduction of edema and of mean arterial pressure occurred in both groups of patients. In the bumetanide-treated group, weight loss was statistically significant at most observation times during the first eight weeks, and reduction in edema was significant throughout the trial (P less than 0.05). Reduction in these parameters in the furosemide-treated patients was not statistically significant. However, the differences between the diuresis and other changes produced by the two drugs were not significant. While reduction in the mean arterial pressure of patients on bumetanide was significant (P less than 0.05), the difference with that in the furosemide treatment group was not significant. Serum electrolyte abnormalities were infrequent. All patients tolerated the drug trial well. No deterioration in audiometry occurred in any patients, including 12 who entered the trial with abnormal air audiograms. Both drugs appear to be equally safe. While a daily dose of 4.2 mg bumetanide is highly effective in severe renal edema, the design of the study did not warrant definitive conclusion regarding comparative efficacy with furosemide.

Abstract:The longitudinal dependence of the standing wave in a complex-coupled DFB laser is experimentally investigated. We have observed localized clamping of the luminescence for unstable operation conditions such as multi-mode emission. We present measurements of the variation in luminescence as a function of position along the laser cavity length for various relative phases between facets and grating. We impose these conditions by systematically depositing layers of Ge to change the relative facet-to-grating distance in an InGaAsP MQW complex coupled DFB laser. A model based on standing-wave considerations is used to explain the experimental findings.
Bumetanide in refractory ascites of cirrhosis of the liver: a comparison with furosemide

Abstract:Bumetanide and furosemide were compared in the treatment of resistant ascites in patients with liver disease of alcoholic origin. In a prospective randomized trial, no significant difference was found in the ability of these two agents to promote natriuresis or diuresis. Abstract:Corrosion is a natural process converting refined metals to their more stable oxide. Chemical reactions with the environme t cause gradual destruction of materials (mostly metals). Corrosion is the most im portant and dangerous degradation process for metallic structures used fo r different applications and various installations appearing as uniform, pitting , grooving or weld metal corrosion or in high stress areas or under insulation. Undete ct d corrosion will lead to catastrophic failure of structures and consequently to enormous economic losses. Overall direct costs associated with metallic corro sion were calculated for US in 1998 with $276 Billion (approx. 3.2% of the US gros s domestic product) mainly related to fields like infrastructure, production, manufacturing or transportation. Detection of corrosion on time during inspection an d maintenance is very important in all industrial fields. Often corrosion is invisi bly, especially when occurring inside a component or because of different types of coatin g or insulation covering the outer surface. Acoustic emission testing is a non-invasiv e and integral testing method, used successfully during decades for inspection in industry. Furthermore, AT enables the detection of active corrosion inside or outside of components or under insulation applying only a few sensors on the surfa ce. The paper will summarize fundamental lab experiment s of EC funded projects, where the basics for detection and evaluation of co rrosion could be checked and verified. The results demonstrate the abilities but also the limits of the methodology. Case studies from different structures and industri al fields will be demonstrated: • Pressurized gas storage tanks in a refinery • Pressurised gas pipeline • Flat bottomed storage tanks for natural oil or HC liquids • Tankships for oil products • Storage tanks for LPG Furthermore, an outlook will be given about future possibilities for corrosion detection by AE monitoring. The methodology include s monitoring the start and different steps of material degradation over time. In combination with risk based inspection, stops and repairs can be performed on t ime before a hazardous situation will occur. Abstract:Key Points Oral torsemide was not superior to furosemide in measures of renal tubular delivery or duration of action. A dose equivalence of approximately 40 mg oral furosemide:10 mg oral torsemide resulted in similar natriuresis. The two-fold higher doses of torsemide did not improve fluid status due to the kidney’s compensation. Background Torsemide is proposed to have clinically important pharmacokinetic and pharmacodynamic advantages over furosemide. However, clinical outcomes did not differ in the Torsemide Comparison with Furosemide for Management of Heart Failure (TRANSFORM) randomized trial. Methods We conducted a multicenter mechanistic substudy of patients with heart failure randomized to oral furosemide or torsemide (TRANSFORM-Mechanism trial). At baseline and 30 days, participants underwent detailed assessments of pharmacokinetic and pharmacodynamic parameters. Results The TRANSFORM-Mechanism trial enrolled 88 participants. Kidney bioavailability, or the proportion of dose delivered to the tubular site of action, was significantly less with torsemide compared with furosemide (median, 17.1% [interquartile range, 12.3%–23.5%] versus 24.8% [16.6%–34.1%], P < 0.001). Furosemide had a longer duration of kidney drug delivery and natriuresis ( P ≤ 0.004 for both). Prescribed doses of furosemide and torsemide in the TRANSFORM-Mechanism trial were similar to the TRANSFORM trial, with clinicians on average using a 2:1 dose equivalence conversion between drugs. However, these doses resulted in a substantially greater natriuresis with torsemide ( P < 0.001). A dose equivalence of approximately 4:1 resulted in similar natriuresis. Higher diuretic doses in the torsemide group resulted in mild perturbations in kidney function and significant increases in renin, aldosterone, and norepinephrine ( P < 0.05 for all). Plasma volume ( P = 0.52) and body weight ( P = 0.89) did not improve with torsemide versus furosemide. Conclusions We observed no meaningful pharmacokinetic/pharmacodynamic advantages for torsemide versus furosemide. The greater natriuresis from higher diuretic doses in the torsemide group was offset by greater neurohormonal activation and kidney dysfunction. Clinical pharmacology of the loop diuretics furosemide and bumetanide in neonates and infants

Abstract:The loop diuretics furosemide and bumetanide are used widely for the management of fluid overload in both acute and chronic disease states. To date, most pharmacokinetic studies in neonates have been conducted with furosemide and little is known about bumetanide. The aim of this article was to review the published data on the pharmacology of furosemide and bumetanide in neonates and infants in order to provide a critical analysis of the literature, and a useful tool for physicians. The bibliographic search was performed electronically using PubMed and EMBASE databases as search engines and March 2011 was the cutoff point. The half-life (t(½)) of both furosemide and bumetanide is considerably longer in neonates than in adults and consequently the clearance (CL) of these drugs is reduced at birth. In healthy volunteers, plasma t(½) of furosemide ranges from 33 to 100 minutes, whereas in neonates it ranges from 8 to 27 hours. The volume of distribution (V(d)) of furosemide undergoes little variation during neonate maturation. The dose of furosemide, administered by intermittent intravenous infusion, is 1 mg/kg and may increase to a maximum of 2 mg/kg every 24 hours in premature infants and every 12 hours in full-term infants. Comparison of continuous infusion versus intermittent infusion of furosemide showed that the diuresis is more controlled with fewer hemodynamic and electrolytic variations during continuous infusion. The appropriate infusion rate of furosemide ranges from 0.1 to 0.2 mg/kg/h and when the diuresis is <1 mL/kg/h the infusion rate may be increased to 0.4 mg/kg/h. Treatment with theophylline before administration of furosemide results in a significant increase of urine flow rate. Bumetanide is more potent than furosemide and its dose after intermittent intravenous infusion ranges from 0.005 to 0.1 mg/kg every 24 hours. The t(½) of bumetanide in neonates ranges from 1.74 to 7.0 hours. Up to now, no data are available on the continuous infusion of bumetanide. Extracorporeal membrane oxygenation (ECMO) is used for a variety of indications including sepsis, persistent pulmonary hypertension, meconium aspiration syndrome, cardiac defects and congenital diaphragmatic hernia. There are two studies of furosemide in neonates undergoing ECMO and only one on the pharmacokinetics of bumetanide under ECMO. When ECMO was conducted for 72 hours, the total amount of furosemide administered was 7.0 mg/kg, and the urine production in the 3 days of treatment was about 6 mL/kg/h, which is the target value. The t(½) of bumetanide in neonates during ECMO was extremely variable. CL, t(½), and V(d) were 0.63 mL/min/kg, 13.2 hours, and 0.45 L/kg, respectively. Furosemide may be administered by inhalation and inhibits the bronco-constrictive effect of exercise, cold air ventilation and antigen challenge. However, inhaled furosemide is not active in infants with viral bronchiolitis and its effect on broncho-pulmonary dysplasia is still uncertain. Furosemide does not significantly increase the risk of failure of patent ductus arteriosus closure when indomethacin or ibuprofen have been co-administered. Infants with low birth weight treated long-term with furosemide are at risk for the development of intra-renal calcification. Furosemide therapy above 10 mg/kg bodyweight cumulative dose had a 48-fold increased risk of nephrocalcinosis. The use of furosemide in combination with indomethacin increased the incidence of acute renal failure. The maturation of the kidney governs the pharmacokinetics of furosemide and bumetanide in the infant. CL and t(½) are influenced by development, and this must be taken into consideration when planning a dosage regimen with these drugs.
Differential effects of human atrial natriuretic peptide and furosemide on glomerular filtration rate and renal oxygen consumption in humans

Abstract:Objective: Imbalance in the renal medullary oxygen supply/demand relationship can cause hypoxic medullary damage and ischemic acute renal failure. Human atrial natriuretic peptide (h-ANP) increases glomerular filtration rate in clinical acute renal failure. This would increase renal oxygen consumption due to increased tubular load of sodium. Loop diuretics are commonly used in acute renal failure. Data on the effects of loop diuretics on glomerular filtration rate and renal oxygen consumption in humans are, however, controversial. We evaluated the effects of h-ANP and furosemide on renal oxygen consumption, glomerular filtration rate, and renal hemodynamics in humans. Design and setting: Prospective two-agent interventional study in a university hospital cardiothoracic ICU. Patients: Nineteen uncomplicated, mechanically ventilated postcardiac surgery patients with normal renal function. Interventions: h-ANP (25 and 50 ng/kg per minute, n=10) or furosemide (0.5 mg/kg per hour, n=9). Measurements and results: Renal plasma flow and glomerular filtration rate were measured using the infusion clearance technique for (51)Cr-labeled EDTA and paraaminohippurate, corrected for by renal extraction of PAH. h-ANP increased glomerular filtration rate, renal filtration fraction, fractional excretion of sodium, and urine flow. This was accompanied by an increase in tubular sodium reabsorption (9%) and renal oxygen consumption (26%). Furosemide infusion caused a 10- and 15-fold increase in urine flow and fractional excretion of sodium, respectively, accompanied by a decrease in tubular sodium reabsorption (-28%), renal oxygen consumption (-23%), glomerular filtration rate and filtration fraction (-12% and -7%, respectively). Conclusions: The filtered load of sodium is an important determinant of renal oxygen consumption. h-ANP improves glomerular filtration rate but does not have energy-conserving tubular effects. In contrast, furosemide decreases tubular sodium reabsorption and renal oxygen consumption and thus has the potential to improve the oxygen supply/demand relationship in clinical ischemic acute renal failure.

Abstract:Objective To study the change of the plasma renin activity(PRA) after 40mg intravenousinjection of furosemide. Methods The changes of PRA of 22 cases with refractory hypertension were examined before and after 10min, 30min injecting furosimide 40mg by vein.Meanwhile the changes of blood pressure and heart rate were monitored. Results Compared the changes of PRA before and after 10min,30min injection of furosemide by vein,it is 3.45:7, 11:5.68ng/ml/h respectively,P<0.05.The peak is at the 10min.The ATⅡ also has the same change but not so obvious. Conclusion Injecting furosemide 40mg by vein can stimulate the increase of PRA quickly,the peak is at 10min, there are obvious age and sex differences, the ATⅡ also has the same change but not obvious.

Abstract:Abstract Single injections of dihydroergocristine (dhe) (25, 50 and 100 μg, i.p.) did not depress the systemic arterial pressure but antagonized the pressor effects of (-)-noradrenaline (100 μg, s.c.) in unanaesthetized rats. The effects of these doses of dhe on water diuresis were minimal: slight K retention, a rise in urinary Na:K and a slight but significant fall in the clearance of p-aminohippuric acid were noted. Single injections of phentolamine mesylate (10, 20 and 40 μg, i.p.) greatly depressed systemic arterial pressure and reduced the glomerular filtration rate and p-aminohippuric acid clearance and the excretion of water and Na proportionately, but markedly. The Na:K in the urine rose. Chemical denervation of the kidneys either with dhe, 32 μg, or phentolamine, 2 mg, thrice daily for 5–7 days decreased, but for 14 days increased, the juxtaglomerular index of the kidneys. Parallel changes were found in the extractable renin. Continued treatment with dhe (32 μg thrice daily for 2 weeks) raised, and hyperduric adrenaline (250 μg twice daily for 1 week) lowered the stores of growth hormone in the adenohypophysis. Continued treatment with dhe, 32 μg thrice daily produced antidiuresis and retention by the third day lasting to the fifth. This condition had reversed by the eighth day.

Abstract:Background Limited evidence exists regarding efficacy and safety of diuretic regimens in ambulatory, congestion-refractory, chronic heart failure (CHF) patients. Objective To compare the potency and safety of commonly used diuretic regimens in CHF patients. Methods A prospective, randomized, open-label, crossover study conducted in NYHA class II-IV CHF patients, treated in an ambulatory day-care unit. Each patient received 3 different diuretic regimens: intravenous (IV) furosemide 250mg; IV furosemide 250mg plus oral metolazone 5mg; and IV furosemide 250mg plus IV acetazolamide 500mg. Treatments were administered once a week, in one of six randomized sequences. The primary endpoint was total sodium excretion, and the secondary was total urinary volume excreted, both measured for 6 hours post-treatment initiation. Results A total of 42 patients were recruited. Administration of furosemide plus metolazone resulted in the highest weight of sodium excreted, 4691 mg (95% CI: 4153-5229) compared to furosemide alone 3835 mg (95% CI: 3279-4392), P =0.015 and to furosemide plus acetazolamide 3584 mg (95% CI: 3020-4148), P =0.001. Furosemide plus metolazone resulted in 1.84 liters of urine (95% CI: 1.63-2.05), compared to 1.58 liters (95% CI: 1.37-1.8) P =0.039 collected following administration of furosemide plus acetazolamide and 1.71 liters (95% CI 1.49-1.93) following furosemide alone. The incidence of worsening renal function (WRF) was significantly higher when adding metolazone (41%) to furosemide compared to furosemide alone (17%) and to furosemide plus acetazolamide (2.6%), P <0.001. Conclusions In ambulatory CHF patients, furosemide plus metolazone resulted in a significantly higher natriuresis compared to IV furosemide alone or furosemide plus acetazolamide.

Prevalence and Specificity of the Abnormal Niacin Response: A Potential Endophenotype Marker in Schizophrenia

Abstract:The skin flush response to niacin is abnormally blunted among a subset of patients with schizophrenia (SZ), preferentially associates with SZ compared to other mental illnesses, occurs frequently in nonpsychotic members of SZ-affected families, appears heritable, and shows evidence of genetic association. The niacin response abnormality (NRA) may prove to be a useful SZ endophenotype. Using a laser Doppler flowmeter, we undertook this study to estimate the prevalence of NRA in SZ (n = 70), bipolar disorder (BP, n = 59), and healthy control (HC, n = 87) groups, and to estimate its specificity for the illness. From the dose-response curves, we calculated the concentration of methylnicotinate required to elicit a half-maximal blood flow (MBF) response (EC50 value) and MBF value for each subject. The median log10EC50 of the SZ was above the third quartile of log10EC50 of either the HC or BP groups, whereas the MBF was significantly lower in the SZ than in the HC or BP groups. With a definition of NRA of having both EC50 above the ninetieth percentile of the control samples and MBF response below the sixtieth percentile for the control range, the NRA predicted SZ with 31% sensitivity and 97% specificity. Moreover, the NRA was not influenced by age, gender, race, and cigarette smoking. In summary, the NRA may define a SZ subtype with a clinically significant phospholipid signaling defect. Understanding its molecular origins may shed light on the pathophysiology of SZ and suggest new tools for its early diagnosis and treatment.
