Abstract:Among various test methods for different human joints, the use of robot systems has attracted major interest and inherits the potential to become a gold standard in biomechanical testing in the future. A key issue associated with those robot-based platforms is the accurate definition of parameters, e.g., tool center point (TCP), length of tool or anatomical trajectories of movements. These must be precisely correlated to the physiological parameters of the examined joint and its corresponding bones. Exemplified for the human hip joint, we are creating an accurate calibration procedure for a universal testing platform by using a six degree-of-freedom (6 DOF) robot and optical tracking system for recognition of anatomical movements of the bone samples.

Design and validation of a general purpose robotic testing system for musculoskeletal applications

Abstract:Orthopaedic research on in vitro forces applied to bones, tendons, and ligaments during joint loading has been difficult to perform because of limitations with existing robotic simulators in applying full-physiological loading to the joint under investigation in real time. The objectives of the current work are as follows: (1) describe the design of a musculoskeletal simulator developed to support in vitro testing of cadaveric joint systems, (2) provide component and system-level validation results, and (3) demonstrate the simulator's usefulness for specific applications of the foot-ankle complex and knee. The musculoskeletal simulator allows researchers to simulate a variety of loading conditions on cadaver joints via motorized actuators that simulate muscle forces while simultaneously contacting the joint with an external load applied by a specialized robot. Multiple foot and knee studies have been completed at the Cleveland Clinic to demonstrate the simulator's capabilities. Using a variety of general-use components, experiments can be designed to test other musculoskeletal joints as well (e.g., hip, shoulder, facet joints of the spine). The accuracy of the tendon actuators to generate a target force profile during simulated walking was found to be highly variable and dependent on stance position. Repeatability (the ability of the system to generate the same tendon forces when the same experimental conditions are repeated) results showed that repeat forces were within the measurement accuracy of the system. It was determined that synchronization system accuracy was 6.7+/-2.0 ms and was based on timing measurements from the robot and tendon actuators. The positioning error of the robot ranged from 10 microm to 359 microm, depending on measurement condition (e.g., loaded or unloaded, quasistatic or dynamic motion, centralized movements or extremes of travel, maximum value, or root-mean-square, and x-, y- or z-axis motion). Algorithms and methods for controlling specimen interactions with the robot (with and without muscle forces) to duplicate physiological loading of the joints through iterative pseudo-fuzzy logic and real-time hybrid control are described. Results from the tests of the musculoskeletal simulator have demonstrated that the speed and accuracy of the components, the synchronization timing, the force and position control methods, and the system software can adequately replicate the biomechanics of human motion required to conduct meaningful cadaveric joint investigations.
Validation of a novel biomechanical test bench for the knee joint with six degrees of freedom

Abstract:A novel biomechanical test bench has been developed for in-vitro evaluation of the knee joint. The test bench allows the kinematics of the knee joint to be studied in all six degrees of freedom. Flexion-extension knee movements are induced by quadriceps and hamstring muscle forces simulated by five pneumatic cylinders. The kinematics of the knee and the actively applied muscle forces are measured simultaneously. The aim of this study was to validate the sensitivity and reproducibility of this novel test bench. Four fresh frozen human knees were tested three times, each with seven flexion-extension cycles between 5° and 60°. After the native knees had been tested, the posterior cruciate ligament and then the lateral collateral ligament were dissected. The injured knees were tested in identical conditions [3×(7×5°-60°)] in order to evaluate whether the test bench is capable of detecting differences in knee kinematics between a native state and an injured one. With regard to reproducibility, the novel test bench showed almost perfect agreement for each specimen and for all states and flexion angles. In comparison with the native knees, the injured knees showed significant differences in knee kinematics. This validated novel test bench will make it possible to investigate various knee pathologies, as well as current and newly developed treatment options.
New methodology for multi-dimensional spinal joint testing with a parallel robot

Abstract:Six degree-of-freedom (6DOF) robots can be used to examine joints and their mechanical properties with the spatial freedom encountered physiologically. Parallel robots are capable of 6DOF motion under large payloads making them ideal for joint testing. This study developed and assessed novel methods for spinal joint testing with a custom-built parallel robot implementing hybrid load-position control. We hypothesized these methods would allow multi-dimensional control of joint loading scenarios, resulting in physiological joint motions. Tests were performed in 3DOF and 6DOF. 3DOF methods controlled the forces and the principal moment within +/-10 N and 0.25 N m under combined bending and compressive loads. 6DOF tests required larger tolerances for convergence due to machine compliance, however expected motion patterns were still observed. The unique mechanism and control approaches show promise for enabling complex three-dimensional loading patterns for in vitro joint biomechanics, and could facilitate research using specimens with unknown, changing, or nonlinear load-deformation properties.

Abstract:The mechanism for copper loading of the antioxidant enzyme copper, zinc superoxide dismutase (SOD1) by its partner metallochaperone protein is not well understood. Here we show the human copper chaperone for Cu,Zn-SOD1 (hCCS) activates either human or yeast enzymes in vitro by direct protein to protein transfer of the copper cofactor. Interestingly, when denatured with organic solvents, the apo-form of human SOD1 cannot be reactivated by added copper ion alone, suggesting an additional function of hCCS such as facilitation of an active folded state of the enzyme. While hCCS can bind several copper ions, metal binding studies in the presence of excess copper scavengers that mimic the intracellular chelation capacity indicate a limiting stoichiometry of one copper and one zinc per hCCS monomer. This protein is active and unlike the yeast protein, is a homodimer regardless of copper occupancy. Matrix-assisted laser desorption ionization-mass spectrometry and metal binding studies suggest that Cu(I) is bound by residues from the first and third domains and no bound copper is detected for the second domain of hCCS in either the full-length or truncated forms of the protein. Copper-induced conformational changes in the essential C-terminal peptide of hCCS are consistent with a “pivot, insert, and release” mechanism that is similar to one proposed for the well characterized metal handling enzyme, mercuric ion reductase.
A Framework for Analytical Validation of Inertial-Sensor-Based Knee Kinematics Using a Six-Degrees-of-Freedom Joint Simulator

Abstract:The success of kinematic analysis that relies on inertial measurement units (IMUs) heavily depends on the performance of the underlying algorithms. Quantifying the level of uncertainty associated with the models and approximations implemented within these algorithms, without the complication of soft-tissue artefact, is therefore critical. To this end, this study aimed to assess the rotational errors associated with controlled movements. Here, data of six total knee arthroplasty patients from a previously published fluoroscopy study were used to simulate realistic kinematics of daily activities using IMUs mounted to a six-degrees-of-freedom joint simulator. A model-based method involving extended Kalman filtering to derive rotational kinematics from inertial measurements was tested and compared against the ground truth simulator values. The algorithm demonstrated excellent accuracy (root-mean-square error ≤0.9°, maximum absolute error ≤3.2°) in estimating three-dimensional rotational knee kinematics during level walking. Although maximum absolute errors linked to stair descent and sit-to-stand-to-sit rose to 5.2° and 10.8°, respectively, root-mean-square errors peaked at 1.9° and 7.5°. This study hereby describes an accurate framework for evaluating the suitability of the underlying kinematic models and assumptions of an IMU-based motion analysis system, facilitating the future validation of analogous tools.

Abstract:This paper presents a methodology and generalized motion framework for quantifying passive biomechanical stability and Range of Motion of human cadaveric specimens, using a position-controlled industrial robot and a wrist-mounted force/torque sensor. Many biomechanical studies on diarthrodial joints using human cadaveric specimens are published in the literature, using various test protocols and machines to apply the loading conditions. In these studies, laxity or mobility of the joints are quantified by measuring the magnitude of translations and rotations with respect to force and torque. The protocols and anatomical motions of the specimens are usually described high-level, textually, and from a medical perspective to a broad audience. The present paper aims to describe, from a technical perspective to a robotics audience, our method to perform biomechanical studies and how existing protocols can be replicated through parameterization using the existing textual descriptions. To accomplish this, we propose a generalized task space motion framework for performing biomechanical studies on diarthrodial joints. The generalization is made by defining the robot Tool Center Point at the cadaveric joint rotation center and aligning the specimen so the anatomical motions can be modeled in world frame or tool frame. The framework was successfully evaluated in a technical pilot study on the shoulder, using one cadaveric shoulder specimen and an established protocol from the literature. The specimen was tested in the intact state and in an injury state, with increased passive instability observed for the injury state compared to intact state.

Abstract:Intraoperative measurement of hip posture is the basis for assessing hip range of motion (ROM) and predicting postoperative functional limits allowable for activities of daily living. Although computer navigation for total hip arthroplasty (THA) has improved the accuracy of intraoperative ROM evaluation, it has not gained widespread popularity due to its complex and time-consuming protocol. We therefore developed an inertial measurement unit-based hip smart trial system (IMUHST) for intraoperative monitoring of hip posture. An in vitro validation experiment was conducted using bone models with a three-dimensional measurement model as the reference standard. The absolute mean error, Bland - Altman analysis and intra-class correlation coefficient demonstrated that the validity and reliability of this system meets the requirement for clinical application. Given that monitoring posture is the basis for evaluating the direction(s) of potential impingement, subluxation and dislocation, the IMUHST is a promising development direction of computer assisted surgery in THA.

Abstract:Robotics technologies have been modified to control and measure both the force and position of synovial joints for the study of joint kinematics. One such system was developed to perform kinematic testing of a human joint. A 6-axis articulated robotic manipulator with 6 degrees of freedom (DOF) of motion was designed and constructed; a mathematical description for joint force and position was devised; and hardware and software to control forces applied to the joint, as well as position of the joint, were developed. The new methodology was utilized to simulate physiological loading conditions and to perform an anterior-posterior (A-P) translation test on a human cadaveric knee. Testing showed that this new system can simulate complex loading conditions and also measure the resulting joint kinematics.

Abstract:Abstract Background The Opti_Knee system, a marker-based motion capture system, tracks and analyzes the 6 degrees of freedom (6DOF) motion of the knee joint. However, the validation of the accuracy of this gait system had not been previously reported. The objective of this study was to validate and the system. Two healthy subjects were recruited for the study. Methods The 6DOF kinematics of the knee during flexion–extension and level walking cycles of the knee were recorded by Opti_Knee and compared to those from a biplanar fluoroscopy system. The root mean square error (RMSE) of knee kinematics in flexion–extension cycles were compared between the two systems to validate the accuracy at which they detect basic knee motions. The RMSE of kinematics at key events of gait cycles (level walking) were compared to validate the accuracy at which the systems detect functional knee motion. Pearson correlation tests were conducted to assess similarities in knee kinematic trends between the two systems. Results In flexion–extension cycles, the average translational accuracy (RMSE) was between 2.7 and 3.7 mm and the average rotational accuracy was between 1.7 and 3.8°. The Pearson correlation of coefficients for flexion–extension cycles was between 0.858 and 0.994 for translation and 0.995-0.999 for angles. In gait cycles, the RMSEs of angular knee kinematics were 2.3° for adduction/abduction, 3.2° for internal/external rotation, and 1.4° for flexion/extension. The RMSEs of translational kinematics were 4.2 mm for anterior/posterior translation, 3.3 mm for distal/proximal translation, and 3.2 mm for medial/lateral translation. The Pearson correlation of coefficients values was between 0.964 and 0.999 for angular kinematics and 0.883 and 0.938 for translational kinematics. Conclusion The Opti_Knee gait system exhibited acceptable accuracy and strong correlation strength compared to biplanar fluoroscopy. The Opti _Knee may serve as a promising portable clinical system for dynamic functional assessments of the knee.

Abstract:The knee joint plays a pivotal role in mobility and stability during ambulatory and standing activities of daily living (ADL). Increased incidence of knee joint pathologies and resulting surgeries has led to a growing need to understand the kinematics and kinetics of the knee. In vivo, in silico, and in vitro testing domains provide researchers different avenues to explore the effects of surgical interactions on the knee. Recent hardware and software advancements have increased the flexibility of in vitro testing, opening further opportunities to answer clinical questions. This paper describes best practices for conducting in vitro knee biomechanical testing by providing guidelines for future research. Prior to beginning an in vitro knee study, the clinical question must be identified by the research and clinical teams to determine if in vitro testing is necessary to answer the question and serve as the gold standard for problem resolution. After determining the clinical question, a series of questions ( What surgical or experimental conditions should be varied to answer the clinical question, what measurements are needed for each surgical or experimental condition, what loading conditions will generate the desired measurements, and do the loading conditions require muscle actuation? ) must be discussed to help dictate the type of hardware and software necessary to adequately answer the clinical question. Hardware (type of robot, load cell, actuators, fixtures, motion capture, ancillary sensors) and software (type of coordinate systems used for kinematics and kinetics, type of control) can then be acquired to create a testing system tailored to the desired testing conditions. Study design and verification steps should be decided upon prior to testing to maintain the accuracy of the collected data. Collected data should be reported with any supplementary metrics (RMS error, dynamic statistics) that help illuminate the reported results. An example study comparing two different anterior cruciate ligament reconstruction techniques is provided to demonstrate the application of these guidelines. Adoption of these guidelines may allow for better interlaboratory result comparison to improve clinical outcomes. Abstract:Accurate measurements of hip joint kinematics are essential for improving our understanding of the effects of injury, disease, and surgical intervention on long-term hip joint health. This study assessed the accuracy of conventional motion capture (MoCap) for measuring hip joint center (HJC) location and hip joint angles during gait, squat, and step-up activities while using dynamic biplane radiography (DBR) as the reference standard. Twenty-four young adults performed six trials of treadmill walking, six body-weight squats, and six step-ups within a biplane radiography system. Synchronized biplane radiographs were collected at 50 images per second and MoCap was collected simultaneously at 100 images per second. Bone motion during each activity was determined by matching digitally reconstructed radiographs, created from subject-specific CT-based bone models, to the biplane radiographs using a validated registration process. Errors in estimating HJC location and hip angles using MoCap were quantified by the root mean squared error (RMSE) across all frames of available data. The MoCap error in estimating HJC location was larger during step-up (up to 89.3 mm) than during gait (up to 16.6 mm) or squat (up to 31.4 mm) in all three anatomic directions (all p < 0.001). RMSE in hip joint flexion (7.2°) and abduction (4.3°) during gait was less than during squat (23.8° and 8.9°) and step-up (20.1° and 10.6°) (all p < 0.01). Clinical analysis and computational models that rely on skin-mounted markers to estimate hip kinematics should be interpreted with caution, especially during activities that involve deeper hip flexion.

Abstract:The presence of normal upper limb arm swing movement is considered an important gait movement that affects the selective coordinated locomotor control of the shoulder, elbow, and wrist joint movements. However, in patients with neurological disorders, flexor synergy is characterized by decreased selective neuromuscular control and reciprocal disinhibition of the wrist flexor muscles associated with spasticity or shortness. This research aimed to demonstrate the reliability, validity, and feasibility of the progressive exoskeletal robotic shoulder joint kinematics system. The robotic shoulder joint kinematics system comprises a gait function-retraining robot designed to provide arm swing. The changes in the shoulder joint angle between ImageJ motion analysis software and robotic shoulder joint kinematics system were compared in this research to investigate the reliability and validity of the latter. The linear regression analysis revealed good correlation between the measured angles and the shoulder angle data ( R2=0.943 ). Furthermore, the test–retest reliability test demonstrated excellent reliability ( R2=0.978 ). The robotic shoulder joint kinematics system generated successful arm swing during the locomotion and range of motion training of the shoulder.

Abstract:HIP International, Ahead of Print. Background:The growing adoption of robotic-assistance during total hip arthroplasty (THA) has provided novel means through which a patient's anatomy and dynamic spinopelvic relationship can be incorporated into surgical planning. However, the impact of enhanced technologies on intraoperative decision-making and changes to component positioning has not yet been described.Methods:A multicentre, prospective study included 105 patients (52% women) patients who underwent robotic-assisted THA with the integration of software that incorporates a patient's pelvic tilt (PT) and virtual range-of-motion (VROM) for impingement modeling. The primary outcome of the study was the percentage of patients who underwent changes to the preoperative plan for cup position after incorporating the data from the software.Results:Utilising the intraoperative VROM information, the preoperative plan for cup position was changed from the default (40° inclination and 20° anteversion) in 82/105 (78%) cases. When stratifying by spinopelvic mobility, 64% were considered normal (change ⩾ 10° and ⩽30°), 27% were stiff (change 30°). For all cohorts, the majority of cases (78%) deviated from the 40° inclination and 20° version target. When evaluating the proportion of cases within the Lewinnek and Callanan safe zones based on spinopelvic mobility, 19% of cases within the normal group were planned outside of both zones compared to 39% of stiff cases and 10% of hypermobile cases.Conclusions:Utilising the latest version of robotic-assisted THA software, the preoperative plan for cup position was changed in the vast majority (78%) of patients, causing substantial deviations from traditional, generic cup targets.

Validation of a Prototype Augmented Reality Navigation System for Total Hip Replacement

Abstract:Introduction: Assistive technologies are becoming more common in total hip replacement (THR) procedures, improving surgeons' abilities to achieve target implant orientations. These systems can be large, absorbing limited space in the operating theatre, and they can add complexity to surgery. Materials and methods: We developed a small footprint prototype system that can assist in the accurate placement of implant components using augmented reality (AR) technology into preoperatively planned positions. This technology augments the 3D pelvis and the cup in its target position and displays the real-time position of instruments. The accuracy of the developed prototype system was assessed through a cadaveric study, comparing the achieved implant positions to the preoperative target. All cadavers received preoperative 3D planning to identify the target cup position and orientation. Cadaveric surgeries were completed using the AR system to achieve the target cup placement. Postoperative computed tomography (CT) was used to measure the achieved component position for each hip. Results: The mean absolute deviation (range) from target acetabular placement to the achieved acetabular placement was 2.9° (-8.7 to 3.3°), 3.0° (-5.7 to 7°) and 1.6mm (-1.2 to 3.5mm) for inclination, anteversion, and depth, respectively. Sixty-six percent of results were within +/-5° of the preoperative target orientation. Conclusion: We present a cadaver validation study on a small footprint prototype system using augmented reality to enable accurate cup placement and provide additional information intraoperatively. Our results are comparable with reported results for image-based navigation from the literature.

Abstract:The robotic arm-assisted system (MAKO) has pioneered a transformative approach to hip joint restoration, excelling in reinstating innate hip joint anatomy and biomechanics. This case represents the initial application of the system in revision surgery following a cut-out complication. A 69-year-old female, previously independent and mobile, presented with left hip pain. X-rays revealed a cut-out complication of the proximal nail, necessitating revision to total hip arthroplasty due to the patient's prior activity level and implant prominence. Utilizing the robotic system, preoperative planning accurately identified limb length discrepancy and guided implant sizing. The system facilitated precise acetabular reaming and optimal component placement. The patient regained functional independence. This report also underscores the system's potential for accurate acetabulum component placement and restoration of hip joint anatomy and biomechanics in revision cases. Future advancements in this technology may expand its role in complex reconstructions and revisions, further enhancing patient outcomes in hip arthroplasty.
A Novel 3-DOF HIP Exoskeleton for Kinematically Establishing Animal Model of Avascular Necrosis of the Femoral Head.

Abstract:This paper outlines a detailed description of the design on a 3-DOF hip robotic device for kinematically establishing animal model of avascular necrosis (AVN) of the femoral head. In order to realize the hip extension/flexion and abduction/adduction movements of rabbit subjects, a novel 2-DOF hip motion apparatus based on a pair of cardan joints is investigated. 2 electric grounded motors are used to drive rabbits passively in terms of predefined trajectories repeatedly with an additional periodical impact loading on the femoral head during motions. It is ideal to simulate usual forceful and repeated actions in sports, such as gymnastics and acrobatics, and initiated a new way to establish animal femoral head AVN models by means of kinematically simulating the real motion procedure rather than traditional medical or physiological operation method. With sets of experiments, the characterization of the exoskeleton system is discussed.
Design and Experimental Evaluation of a Haptic Robot-Assisted System for Femur Fracture Surgery

Abstract:In the face of challenges encountered during femur fracture surgery, such as the high rates of malalignment and X-ray exposure to operating personnel, robot-assisted surgery has emerged as an alternative to conventional state-of-the-art surgical methods. This paper introduces the development of Robossis, a haptic system for robot-assisted femur fracture surgery. Robossis comprises a 7-DOF haptic controller and a 6-DOF surgical robot. A unilateral control architecture is developed to address the kinematic mismatch and the motion transfer between the haptic controller and the Robossis surgical robot. A real-time motion control pipeline is designed to address the motion transfer and evaluated through experimental testing. The analysis illustrates that the Robossis surgical robot can adhere to the desired trajectory from the haptic controller with an average translational error of 0.32 mm and a rotational error of 0.07 deg. Additionally, a haptic rendering pipeline is developed to resolve the kinematic mismatch by constraining the haptic controller (user hand) movement within the permissible joint limits of the Robossis surgical robot. Lastly, in a cadaveric lab test, the Robossis system assisted surgeons during a mock femur fracture surgery. The result shows that Robossis can provide an intuitive solution for surgeons to perform femur fracture surgery.

Intraoperative Technology Use Improves Accuracy of Functional Safe Zone Targeting in Total Hip Arthroplasty

Abstract:Background: Targets of acetabular inclination and anteversion have been suggested based on a patient's spinopelvic mobility. Current methods in total hip arthroplasty (THA) include manual instrumentation, computer-assisted navigation, and robotic-assisted surgery. This study aims to compare the accuracy of these 3 methods in targeting the functional safe zone. Methods: This is a prospective multicenter study including a series of 251 consecutive primary posterior THA patients from April 2019 to January 2021. Preoperative lateral standing and sitting spinopelvic radiographs were obtained. Each patient was classified using the Hip-Spine Classification. A functional safe zone plan was determined. Surgeons used their preferred method (manual instrumentation, computer-assisted, or robotic-assisted). Postoperative anteversion and inclination was measured and compared to the preoperative plan. Mean differences between preoperative and postoperative values were calculated. Welch's t-test was used to assess significant between-group differences with P < .05 considered significant. Results: Of the 249 patients, there were 63 manual instrumentation, 68 computer-assisted navigation, and 118 robotic-assisted surgery. Robotic-assisted surgery (rTHA) was significantly more accurate in targeting anteversion (1 ± 2) compared to manual instrumentation (mTHA; 7 ± 6, P < .001) and computer-assisted navigation (cTHA; 6 ± 6, P < .001). rTHA was also significantly more accurate in targeting inclination (1 ± 1) compared to mTHA (8 ± 7, P < .001) and cTHA (6 ± 7, P < .001). Although cTHA had greater accuracy in targeting both inclination and anteversion compared to mTHA, these differences were not statistically significant. Conclusion: Robotic-assisted surgery was more accurate than both computer-assisted navigation and manual instrumentation in targeting the functional safe zone in primary THA. Further research is needed to evaluate the effect of improved accuracy on dislocation rates.
