Abstract:Background and objectives: Chronic nonspecific back pain is a common clinical disease typically treated by ultrasound-guided spinal injection. This minimally invasive treatment targets the posterior ramus of the spinal nerve (PRSN). The target of the medial branch is clear, but there is unclear target for the intermediate and lateral branches. This study attempted to observe the distribution of PRSN in the dorsal region of transverse process to provide a more detailed anatomical basis for treating spinal pain. Methods: The present study was conducted on 16 transverse processes of six adult male embalmed corpses. The dorsal area of the transverse process was divided into three equal zones, which are zone I, zone II and zone III from inside to outside. The origin, distribution, quantity, transverse diameter, and relationship with the bone structure of the PRSN on the transverse process were observed. Results: Sixty PRSNs were found in the lumbar of six cadavers, of which 48 were divided into three branches, and 12 PRSNs were divided into two branches. The intermediate branch is mainly distributed in zone I, and the lateral branch is mainly distributed in zone II. Twenty-nine communicating branches were found in 48 adjacent segments of six specimens, all of which originated from the intermediate branch of the previous segment and connected with the lateral branch of the next segment. Conclusion: This anatomical study describing the PRSN may have important clinical significance for spinal surgeons. Understanding the bony localization targets of the PRSN and the links between the PRSNs may benefit patients with low back pain who receive spinal injections.

Abstract:Background: Knowledge of neural anatomy is fundamental for safe, efficacious use of regional anesthesia. Spinal column procedures, such as a facet joint block, require an accurate understanding of neural pathways relative to anatomic structure. Since Bogduk's report it has been known that human lumbar posterior ramus of the spinal nerve (PRSN) comprises three, equally sized primary branches. However, inconsistencies and controversy remain over the exact locations and pathways of the peripheral portions of the PRSN branches. In this study, the authors investigated the detailed anatomy of the human PRSN. Methods: The authors performed ventral dissection in seven cadavers to determine the layout of the PRSN between T10 and L4 spinal segments. They captured three-dimensional images with a laser scanner. For fine detail analysis, specimens from another cadaver were subjected to a modified Spalteholz technique to render all nonnerve tissue transparent. Computer graphics were used to create a three-dimensional structural model. Results: All three PRSN branches emanated from an ipsilateral origin and passed posterior to the transverse process. The medial PRSN branch consistently passed between the mammillary and accessory processes under the mammilloaccessory ligament. The intermediate branch passed between the longissimus and iliocostalis muscles and extended to the skin. The lateral branch traveled far lateral from the origin. Conclusions: The authors created a 3D model of the PRSN in the lumbar segment, which may be useful for planning surgical approaches to dorsal areas of the vertebral column. In addition, this knowledge may improve the accuracy of procedures involving the spinal column, particularly radiofrequency neurolysis of the facet joint.

Abstract:Objective:To observe the therapeutic effect of patients with temperature-sensitive mild moxibustion combined with fire needle at Ashi points on patients with the third lumber vertebrae transverse process syndrome and to probe into a new therapy.Methods:82 cases of patients with the third lumber vertebrae transverse process syndrome,these cases were randomly divided into two groups:the treatment group(42 cases)which received treatment of temperature-sensitive mild moxibustion combined with fire needle at Ashi points therapy,the control group(40 cases)which received treatment of acupuncture combined with cupping therapy.Then observe the clinical effect of patients with the third lumber vertebrae transverse process syndrome with the two different therapy,and compare the difference of therapeutic effect and course of treatment between the two groups.Results:The cure rate of treatment group and control group were 83.33%、 52.50% according to efficacy.There was significant deviation(P0.05)in the score of subjective symptom.It indicated that the treatment of temperature-sensitive mild moxibustion combined with fire needle at Ashi points therapy were more effective to treat patients with the third lumber vertebrae transverse process syndrome than the treatment of acupuncture combined with cupping therapy.Conclusion:The treatment of temperature-sensitive mild moxibustion combined with fire needle therapy were more effective to treat patients with The third lumber vertebrae transverse process syndrome than the treatment of acupuncture combined with cupping therapy.And it can become a new and high-performance treatment for patients with the third lumber vertebrae transverse process syndrome.

Abstract:Purpose: Spinal column procedures require an accurate understanding of neural pathways relative to the anatomic structure. Since Bogduk's report in 1982, it has been known that the human lumbar posterior ramus of the spinal nerve (PRSN) comprise not two but three primary branches at least in some lumbar segments. The purpose of the current study was to examine the existence of the three primary branches in the thoracic and lumbar segments. Methods: In this study, we investigated the anatomy of the human PRSN in the thoracic and lumbar segments. Ventral dissection was performed in eight cadavers to determine the anatomy of the PRSN between T1 and L5. Results: At the distal end of a given PRSN, the PRSN divided into three primary branches-medial, intermediate and lateral-in 196 out of 272 segments in the thoracic and lumbar regions in eight cadavers. The medial branch supplied the spinalis compartment, and reached the skin. The lateral branch supplied the iliocostalis muscle compartment, and reached skin. The intermediate branch supplied the longissimus muscle and the area between the medial and the lateral branch, which was a seemingly shorter branch. Conclusion: The triplication of the primary branch of the PRSN is considered not uncommon. The third branch should be recognized in the literature and in textbooks.

Abstract:Objective: Lumbar medial branch denervation is commonly used to treat chronic facetogenic low back pain. Controversy exists regarding risk to adjacent neural structures. The objectives of this cadaveric study were to: (1) dissect, digitize, and model in 3D the branches of the first (L1) to fifth (L5) lumbar dorsal rami located near the junction of the transverse process and lateral neck of the superior articular process; and (2) quantify the minimal distance between the lateral/intermediate and medial branches at the anterior quarter and midpoint of the lateral neck of the superior articular process. Design: Eighteen formalin-embalmed specimens were dissected, digitized and modeled in 3D. The high-fidelity 3D models were used to compare branching patterns and quantify the mean minimal distance between the lateral/intermediate and medial branches of the lumbar dorsal ramus at the anterior quarter and midpoint of the lateral neck of the superior articular process. A Two-way ANOVA was performed to determine if difference of mean distances was significant. Results: There was variability in the branching pattern of the lumbar dorsal rami. In 46 cases (51.1%) the lumbar dorsal ramus divided into 2 branches, in 41 cases (45.6%) into 3, and in 3 cases (3.3%) 4. The mean minimal distance between the lateral/intermediate and medial branches was significantly greater at the midpoint (3.2 ± 2.5 mm) than the anterior quarter (1.2 ± 1.8 mm) of the lateral neck of superior articular process. Conclusion: Minimal distance measurements between the branches of the lumbar dorsal rami at the anterior quarter and midpoint of the lateral neck of the superior articular process were computed. When placing the distal end of the needle tip at the anterior quarter of the lateral neck of the superior articular process, the smaller mean minimal distance between the branches suggests there is a greater risk for inadvertent denervation of the lateral/intermediate branches. Further anatomical and clinical investigations are required.

Abstract:In the needle insertion of epidural anesthesia with the paramedian approach, the needle can pass through the longissimus muscle in the dorsum of the patients. When the needle touches a nerve in the muscles, the patients may experience pain in the back. Obviously, the needle should avoid the nerve tract. To provide better anesthetic service, analysis of the structure and where the concerned nerves lie in that region is inevitable. Material and method: We studied five cadavers in this study. Two cadavers were fixed with Thiel's method. With these cadavers, we studied the nerve running of the posterior rami of the spinal nerve from the nerve root to the distal portion. Three of them were used for the study of transparent specimen, with which we studied the course and size of the nerve inside the longissimus muscle. Results: We observed there were three branches at the stem of the posterior rami of the spinal nerves between the body segment T3 and L5, i.e. medial branch, medial branch of the lateral branch and lateral branch of the lateral branch. The medial branch of the lateral branch supplied to the longissimus muscle. With the transparent specimen, we found that there were different nerve layouts between the upper thoracic, lower thoracic, upper lumbar, and lower lumbar segments in the medial branch of the lateral branch in the longissimus muscle. In the lower thoracic and upper lumbar segments, the medial branch of the lateral branch of the upper lumbar segments produced layers nerve network in the longissimus muscle. L1 and L2 nerves were large in size in the muscle. Conclusion: In the upper lumbar segments the medial branch of the lateral branch of the posterior rami of the spinal nerve produced dense network in the longissimus muscle, where the epidural needle has high possibility to touch the nerve. Anesthetists have to consider the existence of the medial branch of the lateral branch of the posterior rami of the spinal nerve when they insert the needle in the paramedical approach to the spinal column.
Peripheral Nerve Discharge Elicited by Manual Acupuncture at Zusanli (ST 36) Regulates Blood Pressure in Anesthetized Rats

Abstract:Objective: To investigate target organ response by recording mean arterial blood pressure (MAP) fluctuation corresponding to nerve-tract discharges from the nerve innervating acupoint of Zusanli (ST 36) in the hind limb evoked by MA in anesthetized rats. Methods: Male SD rats anesthetized with chloral hydrate were randomly divided into 3 groups which were treated with manual acupuncture (MA), injection of lidocaine followed by MA and injection of normal saline (NS) followed by MA, respectively. The right carotid artery was canulated for persistent measurement of the blood pressure and meanwhile nerve discharges from the nerve-tract were recorded for analysis with amplitude spike counts for every 5 s. Results: The results showed that there were significant nerve discharges recorded from the nerve-tract when applying MA at Zusanli (ST 36) and simultaneous decrease in the MAP, while there was no response when inserting a needle into the Zusanli (ST 36) without manipulation (P<0.05). Furthermore, the reduction of MAP during MA could be completely abolished after blockade of peripheral nerve discharges with an injection of lidocaine into the tissue around Zusanli (ST 36) but not with that of normal saline (NS). Conclusion: These results indicate that MA at Zusanli (ST 36) can elicit the peripheral nerve discharges from the nerve innervating the acupoint; such kind of nerve discharges may contain acupuncture signal regulating blood pressure via somato-cardiovascular reflex.
A systematic review and coordinate-based meta-analysis of fMRI studies on acupuncture at LR 3

Abstract:Objectives: The acupoint LR3 (Taichong) is frequently utilized in clinical acupuncture. However, its underlying neural mechanisms remain not fully elucidated, with speculations suggesting its close association with specific brain activity patterns. Methods: A comprehensive literature search was undertaken across several online databases, such as PubMed, Web of Science, Embase, Cochrane Library, CNKI (China National Knowledge Infrastructure), Wanfang Database, VIP Database, and the Chinese Biomedical Database. Two independent researchers handled the study selection, quality assessment, and data extraction processes. Using the seed-based d-mapping meta-analysis approach, we evaluated the brain regions activated by LR3 acupuncture in healthy subjects. Subsequent subgroup analysis was stratified by fMRI types, and regression analyses were performed considering the duration of acupuncture, depth of needle insertion, and needle diameter. The identified active brain regions were then intricately projected onto large-scale functional networks. Results: A total of 10 studies met the criteria for inclusion, encompassing 319 healthy right-handed participants. The meta-analysis indicates that acupuncture at the LR3 activates regions such as the right postcentral gyrus, left thalamus, left middle frontal gyrus, and right superior frontal gyrus. Additionally, meta-regression analysis highlights that increased acupuncture duration correlates with progressively intensified activation of the right superior frontal gyrus. Subgroup analysis posits that variations in the type of fMRI employed might account for heterogeneity in the pooled results. Concurrently, functional network analysis identifies the primary activated regions as aligning with the Basal ganglia network, Auditory network, Left executive control network, Posterior salience network, Right executive control network, and Sensorimotor networks. Conclusion: Acupuncture at the LR3 in healthy subjects selectively activates brain regions linked to pain perception, emotional processing, and linguistic functions. Extending the needle retention duration intensifies the activation of the right superior frontal gyrus. These findings enrich our comprehension of the neurobiological underpinnings of acupuncture's role in pain mitigation and emotional regulation.

Abstract:Acupuncture has a long history and definite effectiveness in traditional Chinese medicine. Manual acupuncture and electroacupuncture stimulate specific acupoints on meridians to obtain local and/or systemic biological effects, but the mechanism behind these effects remains unclear. In the present study, we used three-dimensional tissue-clearing technology to study four specific acupoints on the Ren meridian of mice, which is located within the anatomic landmark of the linea alba. The distribution, density, branching, and relationships between blood vessels and nerves were quantitated. The acupoints and meridian before and after electroacupuncture were visualized and analyzed together with the surrounding structures using topological Mapper methods. We found that capillary branching and sympathetic neurovascular interactions (SNIs) were denser in the CV 4 acupoint (Guanyuan) compared with surrounding non-acupoints. Furthermore, SNIs in CV 4 indicated capabilities in topological amplification, which was confirmed by high resolution in vivo dynamic vascular imaging. After electro or manual acupuncture, CV 4 showed different degrees of increased blood flow detected with NIR-II probe of LZ-1105, compared with little increased in non-acupoints. Abstract:This report compares the morbidity, health care and drug utilization, and health status of random samples of HMO-enrolled Medicare beneficiaries ages 65-79, and 80 and over. The population represented 3,683 person-years of Kaiser Permanente eligibility (59.2 percent female). Those 80 and over were 20 percent of the person-years (64.1 percent female). The effects of age and sex on morbidity and utilization were analyzed using log linear models that controlled for eligibility, and using two-way analyses of variance of rates. Few differences were found in the morbidity experiences and utilization rates of the two age groups. Indicators of self-reported health status did differ. The findings support the idea that noninstitutionalized very old persons are the healthy survivors of their cohort. Their health care needs may not be much different than younger old persons in terms of the types and amounts of health care services needed.

Abstract:OBJECTIVE To observe changes of discharges of the sciatic nerve branch and mast cell activities and collagen fibers in the acupoint area during manual acupuncture stimulation of "Zusanli"(ST 36), so as to reveal the relationship between peripheral nerve and mast cell activities. METHODS A total of 30 male SD rats were divided into normal, acupuncture control (an acupuncture neidle was inserted into ST 36 without manipulation), manual acupuncture (MA), disodium cromoglycate (DSCG, suppressing mast cell activity) plus acupuncture (MA + DSCG) and col lagenase (dissolving the collagen fibers) plus acupuncture (MA+ collagenase) groups (6 rats/group). After dissection of a branch of the sciatic nerve innervating ST 36 region in the left hind-limb under anesthesia, the ipsilateral ST 36 was stimulated by manipulating the acupuncture needle for 20 min. Discharges of the sciatic nerve branch were recorded by using a pair of metal electrodes and data acquisition system (Power Lab). Skin and muscle tissues of ST 36 area were sampled, sliced and stained with Toluidine Blue for detecting the number of degranulated mast cells. RESULTS Compared with the control group, the mean power spectrum of d ischarges of the sciatic nerve and the mean rates of the degranulated mast cells in "Zusanli" (ST 36) area in the MA group were significantly increased (P<0.01). Whereas the mean power spectrum of discharges of the sciatic nerve and the mean degranulation rates of mast cells were considerably lower in the MA + DSCG group and MA+ collagenase group than in the MA group (P<0.01). No significant differences were found between the normal and control groups, and between the MA+NDSCG and MA+collagenase groups in the mean power density and degranulation rates of mast cells (P>0.05). CONCLUSION Manual acupuncture stimulation of Zuai"ST 36 can significantly potentiate the discharge activity of the sciatic nerve and induce degranulation of mast cells at the same time, suggesting an involvement of mast cells in initiating acupuncture signals by peripheral sensory nerve.
Changes in Acupuncture-Induced Specific Acupoint Neurotransmitters are Possibly Related to Their Physiological Functions in Rats

Abstract:This study investigated changes in neurotransmitters induced by the application of electroacupuncture (EA) at Zusanli (ST36) and Neiguan (PC6). A total of 30 rats were divided into five groups: sham, ST (EA at bilateral ST36 and ST37), ScT (ST plus previous neurectomy of the bilateral sciatic nerves), ScS (sham plus previous neurectomy of the bilateral sciatic nerve), and PC (EA at bilateral PC6 and PC7). The P2X2 receptor expression was stronger in the sham group than in the ST and PC groups (both p < 0.05) but similar between the sham and ScT groups (p > 0.05). Dopamine levels in the extracellular fluid surrounding the acupoints were higher in the PC group than in the sham and ST groups during the postacupuncture period (both p < 0.05). Glutamate levels in the extracellular fluid surrounding the acupoints were higher in the ST group than in the sham group during the acupuncture period (p < 0.05) and higher in the ST group than in the sham and PC groups during the postacupuncture period (both p < 0.05). Serum adrenaline and noradrenaline levels were higher in the PC group than in the sham, ST, and ScT groups (all p < 0.05). Glutamate levels in the CSF were higher in the ST group than in the sham, ScS, and PC groups (all p < 0.05). GABA levels in the CSF were higher in the ST group than in the sham, ScT, and PC groups (all p < 0.05). EA at ST36 and ST37 and PC6 and PC7 exerted an analgesic effect, EA at PC6 and PC7 can enhance heart function, and EA at ST36 and ST37 modulates the cerebral cortex. However, the study needs an evaluation of direct pain behavior, heart function, and brain function in the future.
Brain Activities Responding to Acupuncture at ST36 (zusanli) in Healthy Subjects: A Systematic Review and Meta-Analysis of Task-Based fMRI Studies

Abstract:Purpose: Stomach 36 (ST36, zusanli ) is one of the important acupoints in acupuncture. Despite clinical functional magnetic resonance imaging (fMRI) studies of ST36 acupuncture, the brain activities and the neural mechanism following acupuncture at ST36 remain unclear. Methods: Literature searches were conducted on online databases, including MEDLINE, Embase, Cochrane Library, Web of Science, China National Knowledge Infrastructure, Wanfang database, WeiPu database, and China Biology Medicine, for task-based fMRI studies of acupuncture at ST36 in healthy subjects. Brain regions activated by ST36 acupuncture were systematically evaluated and subjected to seed-based d mapping meta-analysis. Subgroup analysis was conducted on control procedures, manual acupuncture, electrical acupuncture (EA), and acupuncture-specific activations. Meta-regression analysis was performed to explore the effects of needle retention time on brain activities following ST36 acupuncture stimulation. The activated brain regions were further decoded and mapped on large-scale functional networks to further decipher the clinical relevance of acupuncturing at ST36. Results: A total of sixteen studies, involving a total of 401 right-handed healthy participants, that satisfied the inclusion criteria were included in the present meta-analysis. Meta-analysis showed that acupuncturing on ST36 positively activates the opercular part of the right inferior frontal gyrus (IFG.R), left superior temporal gyrus (STG.L), and right median cingulate/paracingulate gyri (MCG.R) regions. Needle retention time in an acupuncture session positively correlates with the activation of the left olfactory cortex, as shown in meta-regression analysis. Subgroup analysis revealed that EA stimulation may be a source of heterogeneity in the pooled results. Functional network mappings showed that the activated areas were mapped to the auditory network and salience network. Further functional decoding analysis showed that acupuncture on ST36 was associated with pain, secondary somatosensory, sound and language processing, and mood regulation. Conclusion: Acupuncture at ST36 in healthy individuals positively activates the opercular part of IFG.R, STG.L, and MCG.R. The left olfactory cortex may exhibit positive needle retention time-dependent activities. Can Electroacupuncture Affect the Sympathetic Activity, Estimated by Skin Temperature Measurement? A Functional MRI Study on the Effect of Needling at GB 34 and GB 39 on Patients with Pain in the Lower Extremity.

Abstract:The Purpose of this study was to investigate the possible needling effect on sympathetic activity by using functional MRI (fMRI). Twelve patients with left lower extremity pain were enrolled in our study. Each was given deep needling at left GB34 (yanglingquan) and GB39 (xuanzhong) points simultaneously. All patients got the strong DeQi sensation by manipulating the needles, and then electroacupucture (EA) was given and lasted for thirty minutes before fMRI scan. Then the fMRI scan was performed by scanning the whole brain with five blocks lasting 2 minutes each. The patients' palm skin temperatures were tested every five minutes as indication of the sympathetic activity from the beginning of EA to the end of our fMRI scan. Functional images were processed by using FEAT software at different levels including the simple single subject analysis, multi-subjects group mean analysis, and multi-subjects unpaired two difference analysis. Finally, 9 of 12 patients' palm temperatures increased gradually but the other three decreased. In comparison of two different palm skin temperature change groups, more significant activation over bilateral caudate head, right lentiform and periaqueductal gray (PAG) were found in the temperature-increased group, but palm temperature-decreased patients revealed more significant activation over bilateral anterior cingulated cortex (ACC), insula, primary somatosensory gyrus (SI), orbitofrontal cortex, occipital cortex, hippocampus and amygdala formation. Our study suggested that needling at analgesic points may modulate the sympathetic activity and such evident difference on brain responses may correlate with the clinical analgesia effects.
Differential Temporal Neural Responses of Pain-Related Regions by Acupuncture at Acupoint ST36： a Magnetoencephalography Study

Abstract:Previous neuroimaging studies primarily focused on the spatial distribution of acupuncture needling stimulation. However, a salient feature of acupuncture was its long-lasting effect. This study attempted to detect the spatial-temporal neural responses evoked by acupuncture at an analgesia acupoint ST36 by using magnetoencephalography. To further verify its functional specificity, we also adopted acupuncture at Pericardium 6 and nonacupoint as separated controls.Forty-two college students, all right-handed and acupuncture naïve, participated in this study. Every participant received only one acupoint stimulation, resulting in 14 subjects in one group. Both magnetoencephalography data (151-channel whole-head system) and structural functional magnetic resonance imaging data (3D sequence with a voxel size of 1 mm(3) for anatomical localization) were collected for each subject. All processing procedures were performed in BrainStorm Toolbox.Acupuncture at ST36 showed a significantly time-varied brain activities with different onset time. Our results presented that acupuncture at different acupoints (or comparing with nonacupoint) can specifically induce neural responses in different brain areas-acupuncture at ST36 can specifically induce the neural responses of pain-inhibition areas, while acupuncture at PC6 can specifically induce the activities of the insula and amygdala.In the present study, we attempted to detect the temporal neural responses underlying the functional specificity of acupuncture at ST36, using acupoint belonging to different meridians and non-acupoint with efficacy-irreverent as separate controls. The specific neural substrates involving acupuncture at different acupoints may be related to its functional specificity in clinical settings.

Abstract:Objective: This research was conducted to increase precision of point location and standardize acupoint targets in relation to the facial nerve (CN VII). Materials and methods: A literature review, cadaver dissection, and electrostimulation of the CN VII were performed, focusing on the anatomical locations of the acupoints along the facial-nerve trajectory. The results were contrasted against established acupoint locations described in the 4th edition of Chinese Acupuncture and Moxibustion. Results: Triple Energizer (TE)-17 targets the facial nerve at its cranial exit; Qian Zheng at the facial nerve trunk; Tai Yang at the frontotemporal branch; Gallbladder (GB)-3 at the zygomatic branch; Small Intestine (SI)-18 at the buccal branch; Stomach (ST)-5 at the marginal mandibular branch; SI-17 at the cervical branch; TE-18 at the posterior auricular branch; TE-19 at the posterior auricular nerve; TE-20 at the superior auricular nerve; and TE-22 at the anterior auricular nerve. Conclusions: This study demonstrated the specificity with which acupuncture points are located in relation to the facial nerve. Standardization of facial acupuncture points to distinct branches of the facial nerve will facilitate reproducible research designs and interpractitioner reproducibility.

Abstract:Study Design: Anatomical study Objective: This study aimed to elaborate on the anatomical characteristics of the medial branch of the lumbar dorsal rami and to discuss its possible clinical significance. Summary of Background Data: Radiofrequency ablation targeting the medial branch of the lumbar dorsal rami has been increasingly used in the clinical management of facetogenic low back pain (FLBP). Nonetheless, attention is also being given to complications such as atrophy of the lumbar soft tissues and muscles. Therefore, a more detailed understanding of the innervation pattern on the facet joint may improve the precision of nerve ablation therapy for FLBP. Methods: An anatomical study of 8 human specimens was carried out. The anatomic characteristics of the medial branch were observed and recorded. Results: The medial branch originates from the lumbar dorsal rami, running close to the root of the posterolateral side of the superior articular process of the inferior cone. When passed through the mamillo-accessory ligament, it turns direction to the medial and caudal side, running in the multifidus muscle. In our study, each medial branch sent out 2-5 branches along the way. All the medial branches in L1-L4 gave off 1-2 small branches when crossing the facet joint and innervated the joint of the lower segment. Nineteen medial branches (23.75%) gave off recurrent branches to innervate the joint at the upper segment. Conclusion: The anatomical features of the medial branch remain similar in each lumbar segment. There are two types of joint branches, including the articular fibers that emanate from the medial branch as it runs along the medial border of the facet joint, and the recurrent branch from the medial branch that innervates the upper facet joint. Moreover, an anastomotic branch was found in the medial branches between different segments.

Abstract:Background: Accumulating evidence from anatomical, physiologic, and neuroimaging research shows that Classical acupuncture points stimulate nerve trunks or their branches in the head, trunk, and extremities. The first part of this series revealed that phenomenon in the extremities. Principal meridian distributions mirror those of major peripheral nerves there and Classical acupuncture points are proximate to peripheral nerves there. These relationships were shown to be consistent with the linear neuroembryologic development of the extremities. The second part of this series revealed that, in the trunk, a neuroanatomical basis for the Principal meridians exists consistent with lateral folding in early fetal neuroembryologic development. Objective: The aim of this Part is to provide anatomical data that corroborates a neuroanatomical basis for the Principal meridians in the head and neck, which is consistent with the longitudinal and lateral folding that occurs in early fetal neuroembryologic development. Methods: Adobe Photoshop software was used to apply Classical acupuncture points and Principal meridians as layers superimposed on neuroanatomic images of the head and neck, allowing demonstration of their anatomical relationships. Results: The Principal meridian distributions in the head and region can be conceptualized as connecting branches of the cranial and/or cervical spinal nerves. Conclusions: Anatomical data support the conceptualization of acupuncture Principal meridians in the head and neck as connecting branches of the cranial and/or cervical spinal nerves and are consistent with neuroembryologic development. Overall, the acupuncture Principal meridians can be conceptualized to have a neuroanatomical substrate that is corroborated by developmental neuroembryology.

Abstract:Nerve and mast cells are densely distributed around acupoints in connective tissue. To explore the internal relations between them in acupuncture effect, we examined dorsal root potential (DRP) response to acupuncture at Zusanli (ST36) under sodium cromoglicate (DSCG, a mast cell stabilizer) intervention in anesthetized Sprague-Dawley (SD) rats. We used single unit nerve recording techniques to collect nerve signals from DRP afferent nerves for a 45-minute period that includes 4 stages, that is, base, drug absorption, acupuncture, and recovery stages. We analyzed the recorded signals from time-domain and frequency-domain perspectives. The results showed that once acupuncture needle was inserted, twisting needle excited more nerves discharges than those at base discharges in ACU (from 35.1 ± 7.2 to 47 ± 9.2 Hz, P = 0.004), and there existed the same trend in Saline + ACU group (from 23.8 ± 2.6 to 29.8 ± 4.2 Hz, P = 0.059). There was no change of nerve discharges under twisting needle with injection of DSCG (from 34.8 ± 5.3 to 34.7 ± 4.4 Hz, P = 0.480). We conclude that acupuncture manipulation promotes neural signal production and DSCG could partly inhibit nerve discharges.
