Abstract:Scandinavian Journal of Public Health, Ahead of Print. Aims:Connecting cohorts with biobanks is a Finnish biobank collaboration, creating an infrastructure for the study of healthy ageing. We aimed to develop a model for data integration and harmonisation between different biobanks with procedures for joint access.Methods:The heart of the collaboration is the integrated datasets formed by using data from three biobanks: (a) Arctic Biobank, hosting regional birth cohorts and cohorts of elderly; (b) hospital-affiliated Borealis Biobank of Northern Finland; and (c) THL Biobank, hosting population-based cohorts. The datasets were created by developing a data dictionary, harmonising cohort data and with a joint pseudonymisation process.Results:The connecting cohorts with biobanks resource at its widest consists altogether of almost 1.4 million individuals from collaborating biobanks. Utilising data from 107,000 cohort participants, we created harmonised datasets that contain attributes describing metabolic risk and frailty for studies of healthy ageing. These data can be complemented with medical data available from Biobank Borealis and with samples taken at hospital settings for approximately 38,000 cohort participants. In addition, the harmonised connecting cohorts with biobanks datasets can be expanded with supplementary data and samples from the collaborating biobanks.Conclusions:The connecting cohorts with biobanks datasets provide a unique resource for research on ageing-related personalised healthcare and for real-world evidence studies. Following the FAIR principles on findability, accessibility, interoperability, and reusability, the reused and harmonised datasets are findable and made accessible for researchers. The same approach can be further utilised to develop additional datasets for other research topics.

Abstract:Biobanks facilitate genome-wide association studies (GWASs), which have mapped genomic loci across a range of human diseases and traits. However, most biobanks are primarily composed of individuals of European ancestry. We introduce the Global Biobank Meta-analysis Initiative (GBMI)-a collaborative network of 23 biobanks from 4 continents representing more than 2.2 million consented individuals with genetic data linked to electronic health records. GBMI meta-analyzes summary statistics from GWASs generated using harmonized genotypes and phenotypes from member biobanks for 14 exemplar diseases and endpoints. This strategy validates that GWASs conducted in diverse biobanks can be integrated despite heterogeneity in case definitions, recruitment strategies, and baseline characteristics. This collaborative effort improves GWAS power for diseases, benefits understudied diseases, and improves risk prediction while also enabling the nomination of disease genes and drug candidates by incorporating gene and protein expression data and providing insight into the underlying biology of human diseases and traits.

Abstract:Recommendations and outlines for standardization in biobanking processes are presented by a research team with long-term experience in clinical studies. These processes have important bearing on the use of samples in developing assays. These measurements are useful to document states of health and disease that are beneficial for academic research, commercial healthcare, drug development industry and government regulating agencies. There is a need for increasing awareness within proteomic and genomic communities regarding the basic concepts of collecting, storing and utilizing clinical samples. Quality control and sample suitability for analysis need to be documented and validated to ensure data integrity and establish contexts for interpretation of results. Standardized methods in proteomics and genomics are required to be practiced throughout the community allowing datasets to be comparable and shared for analysis. For example, sample processing of thousands of clinical samples, performed in 384 high-density sample tube systems in a fully automated workflow, preserves sample content and is presented showing validation criteria. Large studies will be accompanied by biological and molecular information with corresponding clinical records from patients and healthy donors. These developments position biobanks of human patient samples as an increasingly recognized major asset in disease research, future drug development and within patient care.

Abstract:Biobank samples and data from studies of large prospective cohorts (LPC) represent an invaluable resource for health research. Efficient sharing and pooling of samples and data is a central pre-requisite for new advances in biomedical science. This requirement, however, is not compatible with the present scattered and traditional access governance structures, where legal and ethical frameworks often form an obstacle for effective sharing. Moreover, the EU General Data Protection Regulation (GDPR) is demanding increasingly rigorous administration from all those organisations processing personal data. The BBMRI-LPC project (Biobanking and Biomolecular Research Infrastructure - Large Prospective Cohorts) assembled 21 LPCs from 10 countries and two EU-wide multinational cohort networks with a key objective to promote collaborative innovative transnational research proposed by external researchers on the broad field of common chronic diseases, and analyze the gaps and needs involved. BBMRI-LPC organized three scientific calls to offer European investigators an opportunity to gain free of charge transnational access to research material available in the participating cohorts. A total of 11 high-quality research proposals involving multiple prospective cohorts were granted, and the access process in the individual projects carefully monitored. Divergent access governance structures, complex legal and ethical frameworks and heterogeneous procedures were identified as currently constituting substantial obstacles for sample and data transfer in Europe. To optimize the scientific value and use of these research resources, practical solutions for more streamlined access governance in collaborative projects are urgently needed. A number of infrastructure developments could be made to improve time-efficiency in access provision.

Abstract:Currently, researchers have to apply separately to individual biobanks if they want to carry out studies that use samples and data from multiple biobanks. This article analyzes the access governance arrangements of the original five biobank members of the Biobank Standardisation and Harmonisation for Research Excellence in the European Union (BioSHaRE-EU) project in Finland, Germany, the Netherlands, Norway, and the United Kingdom to identify similarities and differences in policies and procedures, and consider the potential for internal policy "harmonization." Our analysis found differences in the range of researchers and organizations eligible to access biobanks; application processes; requirements for Research Ethics Committee approval; and terms of Material Transfer Agreements relating to ownership and commercialization. However, the main elements of access are the same across biobanks; access will be granted to bona fide researchers conducting research in the public interest, and all biobanks will consider the scientific merit of the proposed use and it's compatibility with the biobank's objectives. These findings suggest potential areas for harmonization across biobanks. This could be achieved through a single centralized application to a number of biobanks or a system of mutual recognition that places a presumption in favor of access to one biobank if already approved by another member of the same consortium. Biobanking and Biomolecular Resources Research Infrastructure-European Research Infrastructure Consortia (BBMRI-ERIC), a European consortium of biobanks and bioresources with its own ethical, legal, and social implications (ELSI) common service, could provide a platform by developing guidelines for harmonized internal processes.

Abstract:Population-based biobanking is an essential element of medical research that has grown substantially over the last two decades, and many countries are currently pursuing large national biobanking initiatives. The rise of individual biobanks is paralleled by various networking activities in the field at both the national and international level, such as BBMRI-ERIC in the EU. A significant contribution to population-based biobanking comes from large cohort studies and national repositories, including the United Kingdom Biobank (UKBB), the CONSTANCES project in France, the German National Cohort (NAKO), LifeLines in the Netherlands, FinnGen in Finland, and the All of Us project in the U.S. At the same time, hospital-based biobanking has also gained importance in medical research. We describe some of the scientific questions that can be addressed particularly well by the use of population-based biobanks, including the discovery and calibration of biomarkers and the identification of molecular correlates of health parameters and disease states. Despite the tremendous progress made so far, some major challenges to population-based biobanking still remain, including the need to develop strategies for the long-term sustainability of biobanks, the handling of incidental findings, and the linkage of sample-related and sample-derived data to other relevant resources.

Abstract:Large biobanks have set a new standard for research and innovation in human genomics and implementation of personalised medicine. The Estonian Biobank was founded a quarter of a century ago, and its biological specimens, clinical, health, omics, and lifestyle data have been included in over 800 publications to date. What makes the biobank unique internationally is its translational focus, with active efforts to conduct clinical studies based on genetic findings, and to explore the effects of return of results on participants. In this review we provide an overview of the Estonian Biobank, highlight its strengths for studying the effects of genetic variation and quantitative phenotypes on health-related traits, development of methods and frameworks for bringing genomics into the clinic, and its role as a driving force for implementing personalized medicine on a national level and beyond.

Abstract:Biobanks are essential for the realization of P4-medicine, hence indispensable for smart health. One of the grand challenges in biobank research is to close the research cycle in such a way that all the data generated by one research study can be consistently associated to the original samples, therefore data and knowledge can be reused in other studies. A catalogue must provide the information hub connecting all relevant information sources. The key knowledge embedded in a biobank catalogue is the availability and quality of proper samples to perform a research project. Depending on the study type, the samples can reflect a healthy reference population, a cross sectional representation of a certain group of people (healthy or with various diseases) or a certain disease type or stage. To overview and compare collections from different catalogues, we introduce visual analytics techniques, especially glyph based visualization techniques, which were successfully applied for knowledge discovery of single biobank catalogues. In this paper, we describe the state-of-the art in the integration of biobank catalogues addressing the challenge of combining heterogeneous data sources in a unified and meaningful way, consequently enabling the discovery and visualization of data from different sources. Finally we present open questions both in data integration and visualization of unified catalogues and propose future research in data integration with a linked data approach and the fusion of multi level glyph and network visualization.

Abstract:This socio-economic study is based on the widely held view that there is an inadequate supply of human biological samples that is hampering biomedical research development and innovation (RDI). The potential value of samples and the associated data are thus not being realized. We aimed to examine whether the financing of biobanks contributes to this problem and then to propose a national solution. We combined three methods: a qualitative case study; literature analysis; and informal consultations with experts. The case study enabled an examination of the complex institutional arrangements for biobanks, with a particular focus on cost models. For the purposes of comparison, a typology for biobanks was developed using the three methods. We found that it is not possible to apply a standard cost model across the diversity of biobanks, and there is a deficit in coordination and sustainability and an excess of complexity. We propose that coordination across this diversity requires dedicated resources for a national biobanking distributed research infrastructure. A coordination center would establish and improve standards and support a national portal for access. This should be financed centrally by public funds, possibly supplemented by industrial funding. We propose that: a) sample acquisition continues to be costed into projects and project proposals to ensure biobanking is driven by research needs; b) core biobanking activities and facilities be supported by central public funds distributed directly to host public institutions; and c) marginal costs for access be paid for by the user.

Effect of Side Feed Pressurization on the Performance of Shimmed Foil Gas Bearings-Part II : Model , Predictions and Comparisons to Rotordynamic Measurements

Abstract:Oil-free micro turbomachinery implementing gas foil bearings (GFBs) has improved mechanical efficiency and reliability. Adequate thermal management for operation in high temperature environments is an issue of importance in applications such as in gas turbines and turbochargers. GFBs often need a cooling gas flow, axially fed through one end of the bearings, to transport the heat conducted from a hot turbine, for example. Side gas pressurization has also a paramount effect on reducing amplitudes of motion, synchronous and subsynchronous. In a comparison 2007 TRC report, shaft motion measurements in a test rotor supported on GFBs show this remarkable effect. A computational gas film model implementing the evolution of gas circumferential velocity as a function of the imposed side pressure is advanced. For tested GFBs, predicted direct stiffnesses and damping coefficients increase as the magnitude of feed pressure raises, while the difference in cross-coupled stiffnesses, directly related to rotor-bearing system stability, decreases. Predictions of threshold speed of instability and whirl frequency ratio are in close agreement with laboratory measurements. A model for GFBs with shims is introduced. The shimmed GFB, whose film clearance resembles a three lobe bearing, generates significantly larger hydrodynamic pressures than those in a conventional GFB. Shimmed GFBs operate with a smaller journal eccentricity and attitude angle than conventional GFB, and with increased direct stiffness and damping coefficients. A linear finite element rotordynamic analysis (XLTRC 2) models the test rotor supported on GFBs and predicts the system rotordynamic stability and synchronous rotor responses, phase angle and amplitude. Predicted synchronous responses are in very good agreement with the test measurements, in particular for small to moderate imbalances. As a final observation, although external pressurization aids to improve GFB rotordynamic performance; a too large supplied gas flow rate for cooling an actual high temperature application may penalize sensibly the efficiency and performance of the turbomachinery.

Abstract:We explored the social shaping of science at the micro‐level reality of face‐to‐face interaction in one of the traditional places for scientific activities—the scientific lab. We specifically examined how doctoral students' perception of their: (i) interaction with doctoral mentors (MMI) and (ii) lab social environment (LSE) influenced productivity. Construed as the production of peer‐reviewed articles, we measured productivity using total number of articles (TOTAL), number of articles with impact factor greater than or equal to 4.00 (IFGE4), and number of first‐authored articles (NFA). Via face‐to‐face interviews, we obtained data from n = 210 molecular biology Ph.D. students in selected universities in Japan, Singapore, and Taiwan. Additional productivity data (NFA) were obtained from online bibliometric databases. To summarize the original 13 MMI and 13 LSE semantic‐differential items which we used to measure students' perceptions, principal component (PC) analyses were performed. The results were smaller sets of 4 MMI PCs and 4 LSE PCs. To identify which PCs influenced publication counts, we performed Poisson regression analyses. Although perceived MMI was not linked to productivity, perceived LSE was linked: Students who perceived their LSE as intellectually stimulating reported high levels of productivity in both TOTAL and IFGE4, but not in NFA. Our findings not only highlight how students' perception of their training environment factors in the production of scientific output, our findings also carry important implications for improving mentoring programs in science. © 2016 by The International Union of Biochemistry and Molecular Biology, 45(2):130–144, 2017.

Abstract:Biobanks are important resources and central tools for translational medicine, which brings scientific research outcomes to clinical practice. The key purpose of biobanking in translational medicine and other medical research is to provide biological samples that are integrated with clinical information. In 2008, the Shanghai Municipal Government launched the "Shanghai Tissue Bank" in an effort to promote research in translational medicine. Now a sharing service platform has been constructed to integrate clinical practice and biological information that can be used in diverse medical and pharmaceutical research studies. The platform collects two kinds of data: sample data and clinical data. The sample data are obtained from the hospital biobank management system, and mainly include the donors' age, gender, marital status, sample source, sample type, collection time, deposit time, and storage method. The clinical data are collected from the "Hospital-Link" system (a medical information sharing system that connects 23 tertiary hospitals in Shanghai). The main contents include donors' corresponding medication information, test reports, inspection reports, and hospital information. As of the end of September 2014, the project has a collection of 16,020 donors and 148,282 samples, which were obtained from 12 medical institutions, and automatically acquired donors' corresponding clinical data from the "Hospital-Link" system for 6830 occurrences. This project will contribute to scientific research at medical institutions in Shanghai, and will also support the development of the biopharmaceutical industry. In this article, we will describe the significance, the construction phases, the application prospects, and benefits of the sample repository and information sharing service platform.
Basic principles of biobanking: from biological samples to precision medicine for patients

Abstract:Abstract The term “biobanking” is often misapplied to any collection of human biological materials (biospecimens) regardless of requirements related to ethical and legal issues or the standardization of different processes involved in tissue collection. A proper definition of biobanks is large collections of biospecimens linked to relevant personal and health information (health records, family history, lifestyle, genetic information) that are held predominantly for use in health and medical research. In addition, the International Organization for Standardization, in illustrating the requirements for biobanking (ISO 20387:2018), stresses the concept of biobanks being legal entities driving the process of acquisition and storage together with some or all of the activities related to collection, preparation, preservation, testing, analysing and distributing defined biological material as well as related information and data. In this review article, we aim to discuss the basic principles of biobanking, spanning from definitions to classification systems, standardization processes and documents, sustainability and ethical and legal requirements. We also deal with emerging specimens that are currently being generated and shaping the so-called next-generation biobanking, and we provide pragmatic examples of cancer-associated biobanking by discussing the process behind the construction of a biobank and the infrastructures supporting the implementation of biobanking in scientific research.

Abstract:Biomedical data are generated and collected from various sources, including medical imaging, laboratory tests and genome sequencing. Sharing these data for research can help address unmet health needs, contribute to scientific breakthroughs, accelerate the development of more effective treatments and inform public health policy. Due to the potential sensitivity of such data, however, privacy concerns have led to policies that restrict data sharing. In addition, sharing sensitive data requires a secure and robust infrastructure with appropriate storage solutions. Here, we examine and compare the centralized and federated data sharing models through the prism of five large-scale and real-world use cases of strategic significance within the European data sharing landscape: the French Health Data Hub, the BBMRI-ERIC Colorectal Cancer Cohort, the federated European Genome-phenome Archive, the Observational Medical Outcomes Partnership/OHDSI network and the EBRAINS Medical Informatics Platform. Our analysis indicates that centralized models facilitate data linkage, harmonization and interoperability, while federated models facilitate scaling up and legal compliance, as the data typically reside on the data generator's premises, allowing for better control of how data are shared. This comparative study thus offers guidance on the selection of the most appropriate sharing strategy for sensitive datasets and provides key insights for informed decision-making in data sharing efforts.

Abstract:Precision medicine has the potential to provide more accurate diagnosis, appropriate treatment and timely prevention strategies by considering patients' biological makeup. However, this cannot be realized without integrating clinical and omics data in a data-sharing framework that achieves large sample sizes. Systems that integrate clinical and genetic data from multiple sources are scarce due to their distinct data types, interoperability, security and data ownership issues. Here we present a secure framework that allows immutable storage, querying and analysis of clinical and genetic data using blockchain technology. Our platform allows clinical and genetic data to be harmonized by combining them under a unified framework. It supports combined genotype-phenotype queries and analysis, gives institutions control of their data and provides immutable user access logs, improving transparency into how and when health information is used. We demonstrate the value of our framework for precision medicine by creating genotype-phenotype cohorts and examining relationships within them. We show that combining data across institutions using our secure platform increases statistical power for rare disease analysis. By offering an integrated, secure and decentralized framework, we aim to enhance reproducibility and encourage broader participation from communities and patients in data sharing.

Abstract:Biological samples are known to be essential for clinical diagnosis of various diseases. They are also extremely valuable for subsequent scientific studies to unravel the mystery of illnesses. Translational medicine is an interdisciplinary field to discover new diagnostic tools and treatments by combining clinical science with basic biomedical research. Biological studies in translational medicine are increasingly dependent on large numbers of relevant specimens. Biobanks are established to collect, store, and deliver such complex specimens to meet clinical and research needs. Biobanks are biorepository facilities capable of storing large numbers of specimens. Most domestic and foreign biobanks utilize different degrees of standardization and automation to index and organize the large numbers of biological samples. It remains a challenge to accurately store various biological samples while maintaining their original biological characteristics to meet the demands of precision medicine and basic scientific research. This article focuses on current efforts in the construction and operation of biobanks to maintain accuracy and serve the complex requirements of clinical diagnosis and basic research. Firstly, the importance of sample collection, transportation, processing, storage, and information management are discussed. We emphasize the integration of clinical information and scientific research data as an essential part of biobanks. Secondly, we summarize how to maintain the biological integrity of various biological samples such as arterial and venous blood and various tissue samples. More specifically, (1) peripheral blood samples are important sources of material for liquid biopsies, while blood samples during surgeries are often collected from arteries of patients. In addition, there are significant differences in both cellular and non-cellular components between blood from arteries and veins. Therefore, it is important to differentiate these two blood samples. (2) Tissue samples are notoriously heterogenic, in particular for tumors and symmetrical organs. Thirdly, we discuss biological activities of specimens to support chimeric antigen receptor T cell immunotherapies, organoid establishment, exosome functional studies, and patient-derived tumor xenograft (PDX) animal models. Finally, we try to explore a multi-dimensional utilization of biological samples and aim to achieve "One specimen for multiple purposes" and "Multiple specimens for one purpose" management models as an important part of the new discipline of sample science and cryobiology. The ultimate goal is to utilize biobanks in an accurate and precise manner to aid the advancement of precision medicine and basic biological research efforts. The large number of human biological specimens managed by biobanks is poised to be an integral part of future translational medicine and fundamental biomedical research.
Automated Sample Annotation for Diabetes Mellitus in Healthcare Integrated Biobanking

Abstract:Healthcare integrated biobanking describes the annotation and collection of residual samples from hospitalized patients for research purposes. The central idea of the current work is to establish an automated workflow for sample annotation, selection and storage for diabetes mellitus.This is challenging due to incomplete data at the time of sample selection. The study evaluates a machine learning (ML) and natural language processing (NLP) based two-step procedure for timely and precise sample annotation for diabetes mellitus.Electronic health record data of 785 persons were extracted from the hospital information system. In the first step, a conditional inference forest (CIF) model was trained and tested based on laboratory values from the first 72 hours of the hospital stay using test- (n=550) and training data sets (n=235). Performance was compared with a simple laboratory cut-off classifier (LCC) and a logistic regression (LR) model. Algorithms based on laboratory values, ICD-10 codes or information from discharge summaries extracted by a natural language processing software (NLP-DS) were evaluated as a second (review) step designed to increase the precision of annotations.For the first step, recall/precision/F1-score/accuracy were 71%/86%/0.78/0.82 for CIF and 77%/70%/0.74/0.75 for LR compared to 73%/68%/0.70/0.72 for LCC. NLP-DS was the best performing second (review) step (93%/100%/0.97/0.97). Combining first-step models with NLP-DS increased precision to 100% for all procedures (66%/100%/0.80/0.85 for CIF&NLP-DS 72%/100%/0.84/87.2 for LR&NLP-DS and 66%/100%/0.80/0.85 for LCC&NLP-DS). The number of samples removed by the NLP-DS analysis was higher for LR&NLP-DS and LCC&NLP-DS(removal rate 35% and 38% of initially selected samples) compared to CIF&NLP-DS (removal rate of 20%).The developed two-step procedure is an efficient implementable method for timely and precise annotation of samples from diabetic hospitalized patients. Abstract:Big data and artificial intelligence are key elements in the medical field as they are expected to improve accuracy and efficiency in diagnosis and treatment, particularly in identifying biomedically relevant patterns, facilitating progress towards individually tailored preventative and therapeutic interventions. These applications belong to current research practice that is data-intensive. While the combination of imaging, pathological, genomic, and clinical data is needed to train algorithms to realize the full potential of these technologies, biobanks often serve as crucial infrastructures for data-sharing and data flows. In this paper, we argue that the ‘data turn’ in the life sciences has increasingly re-structured major infrastructures, which often were created for biological samples and associated data, as predominantly data infrastructures. These have evolved and diversified over time in terms of tackling relevant issues such as harmonization and standardization, but also consent practices and risk assessment. In line with the datafication, an increased use of AI-based technologies marks the current developments at the forefront of the big data research in life science and medicine that engender new issues and concerns along with opportunities. At a time when secure health data environments, such as European Health Data Space, are in the making, we argue that such meta-infrastructures can benefit both from the experience and evolution of biobanking, but also the current state of affairs in AI in medicine, regarding good governance, the social aspects and practices, as well as critical thinking about data practices, which can contribute to trustworthiness of such meta-infrastructures.
