Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Backgrounds and aims</h3><p>Iran has 2.1 and 1.8 percent of its 15‐64 years old population living with illicit substance and opioid use disorders, respectively. To address these problems, Iran has been developing a large and multi‐modality addiction treatment system, spanning the time before and after the Islamic revolution.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Iran's current drug treatment scene is a combination of services ranging from medical/harm reduction services to punitive/criminal justice programs. Included in this array of services are drop‐in centers providing low‐threshold harm reduction services, such as distribution of sterile needles and syringes; opioid maintenance treatment clinics providing methadone, buprenorphine and opium tincture; and abstinence‐based residential centers. We will review the evolution of this system in four phases.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>In 1980, Iran's revolutionary government shut down all voluntary treatment programs and replaced them with residential correctional programs. The first shift in the addiction treatment policies came 15 years later after facing the negative consequences. Addiction is viewed as a disease and new voluntary treatment centers offering non‐agonist medications and psychosocial services were established. With an increased number of people who inject drugs and HIV/AIDS epidemics, in the second shift, an extensive move towards harm reduction strategies and opioid‐maintenance programs have been implemented to reduce HIV‐related high‐risk behavior. The emergence of a methamphetamine use crisis creating an increased number of socially marginalized addicted people resulted in public and political demands for stricter policies and ended up in the third shift starting in 2010 with extended compulsory court‐based residential programs. Currently, there is a new shift toward reducing the severity of criminal penalties for drug use/sales and promoting proposals for opium legalization.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>Iran's evolutionary experience in developing a large addiction treatment program in a complex combination of medical/harm reduction and punitive/criminal justice addiction treatment can be examined in its political, clinical and pragmatic context.</p></section>

Abstract:Aim: Substance abuse has become a universal crisis in our modern age. Among illegal substances, opium and its derivatives have been ranked second in terms of usage after cannabis in the world. In many Asian regions, the use of opium enjoys a high social acceptance; hence, some common people and even medical practitioners believe that opium lowers blood glucose and pressure and treat dyslipidemia. How much this belief is scientifically justified? Method: The results of available studies on both humans and animals searched in different search engines up to mid-2016 were integrated (78 articles). Upon the findings we try to offer a more transparent picture of the effects of opium on the mentioned factors along with the probable underlying mechanisms of its action. Results: Taken together, a variety of evidences suggest that the consumption of opium has no scientific justification for amendment of these biochemical variables. The mechanisms proposed so far for the action of opium in the three above disorders are summarized at the end of the article. Short term effects seems to be mostly mediated through central nervous system (neural and hormonal mechanisms), but long term effects are often due to the structural and functional alterations in some body organs. Conclusion: Although opium may temporarily reduce blood pressure, but it increases blood glucose and most of blood lipids. Moreover its long term use has negative impacts and thus it aggravates diabetes, dyslipidemia and hypertension. Accordingly, it is necessary to inform societies about the potential disadvantages of unauthorized opium consumption.

Abstract:The emerging biomass-to-biofuel conversion industry has created an urgent need for identifying new applications for biorefinery lignin. This paper demonstrates a new route to producing dicarboxylic acids from biorefinery lignin through chalcopyrite-catalyzed oxidation in a highly selective process. Up to 95 % selectivity towards stable dicarboxylic acids was obtained for several types of biorefinery lignin and model compounds under mild, environmentally friendly reaction conditions. The findings from this study paved a new avenue to biorefinery lignin conversions and applications.

Abstract:Background Opium use disorder is a significant health problem in our country, leading to a considerable number of health issues. Opium has several detrimental effects on its consumers. However, the effect of Opium use disorder on intracerebral hemorrhage (ICH) has not been evaluated. This study aims to evaluate the relationship between Opium use disorder and ICH. Methods In this case–control study, 402 patients with ICH and 404 patients without ICH enrolled. Opium use disorder, other vascular risk factors including diabetes mellitus, hypertension, hyperlipidemia, and tobacco smoking was compared between these groups. Patients with ICH were divided into two groups; first group are patients with history of Opioid Use Disorder and second group are those patients without Opioid Use Disorder. ICH features including clinical and imaging characteristics and prognostic findings were compared between patients with and without Opium use disorder. Results This case–control study of 806 participants found that hypertension (OR = 6.84, 95% CI: 5.03–9.34, p -value: <0.001), Opium use disorder (OR = 4.23, 95% CI: 2.42–7.35, p -value: <0.001) and tobacco smoking (OR = 1.47, 95% CI: 1.01–2.16, p -value: 0.049) had a higher risk of ICH. Opium-addicted subjects had higher ICH scores (2.61 ± 1.27 vs. 2.11 ± 1.29, p -value: 0.005), were more likely to have infratentorial hemorrhage (22% vs. 12%, OR = 2.13, 95% CI: 1.06–4.28, p -value: 0.038), more likely to be intubated (66% vs. 54%, OR = 1.79, 95% CI: 0.98–3.27, p -value = 0.041) and had lower GCS scores (9.58 ± 3.60 vs. 8.25 ± 3.88, p -value: 0.01). The effect of Opium use disorder independently on ICH was also shown in logistic regression (adjusted OR = 3.15, p -value = 0.001). Conclusion This study is the first to evaluate the effect of Opium use disorder on ICH, identifying Opium use disorder as a new potential risk factor for ICH.

Abstract:The use of drugs predominantly among Iranian men leads to health and social problems. Iran has established methadone maintenance therapy centres but there is limited research exploring the experiences of men who make use of this therapy. The aim of this qualitative study was to explore the experiences of men on methadone maintenance therapy in Urmia, Iran. Sixteen opioid-dependent males on methadone syrup substitution therapy were interviewed. Transcribed data were subjected to thematic analysis. Three themes emerged from the data: (1) the role of family in drug addiction; (2) the role of environment in drug addiction; and (3) the support needed to overcome drug addiction. While drug rehabilitation programmes in Iran can help the person, their family is seen as the most important support that they need together with sociocultural acceptance to overcome stigmatisation.

Abstract:Background . Diabetes mellitus is recognized as one of the most common, serious, and costly chronic diseases. Opium addiction is also a common health problem in Iran. Given the high prevalence of opium use in South Khorasan Province and the increasing prevalence of opioid abuse in the community, this study was performed to investigate the effect of opioid abuse on the extent of disease in diabetic patients undergoing coronary angiography in the cardiology department of Vali-e-Asr Hospital in Birjand city, South Khorasan Province, Iran. Methods . This study recruited a total of 1051 diabetic patients who underwent coronary angiography in the cardiology department of Vali-e-Asr Hospital of Birjand city from 2011 to 2015. The collected data were analyzed using SPSS version 22.0 with the chi-square test and univariate regression analysis. P value <0.05 was considered as statistically significant. Results . Among opiate-addicted diabetics, the risk of coronary artery disease was 0.44 times higher than among nonaddicted diabetics (range 0.24–0.77, P = 0.004 ). The extent of coronary vessel involvement, when present, was not different between the two groups. Conclusion . Opiate-addicted diabetics appear to be more susceptible to CAD than their nonaddicted counterparts. The determinants and correlates of this interaction must be the subject of further study.

Abstract:<span><b>Background:</b> Substance abuse in women is a major public health problem, and despite the efforts to help women quit permanently, great numbers still end up in relapse. <b>Objectives:</b> The present study aimed to investigate the barriers to quitting addiction in Iranian women. <b>Methods:</b> In this descriptive qualitative research conducted in a university-affiliated hospital, Kerman, Iran, during 2016 - 2017, participants were selected using criterion sampling. The researchers interviewed 20 women with the mean age of 29.85 years (SD = 6.50), who were visiting two rehabilitation camps in the city of Kerman to quit the addiction. Transcripts were analyzed to find themes, which represented the shared consensus of the participants. The utilization of the qualitative analysis software MAXQDA 12 helped analyze and manage data of the study. <b>Results:</b> The participants of the study reported that easy access to drugs, keeping in touch with drug users, the stability of attitudes, lack of/insufficient social support, coercive treatment, and neglecting Narcotic Anonymous (NA) meetings were barriers to the process of quitting addiction permanently and escalating relapse. <b>Conclusions:</b> In order to facilitate the process of quitting addiction in women, access to drugs as well as keeping in touch with drug users must be restricted. Abstract:We characterized overdose prevalence and its correlates among people who inject drugs (PWID) in Iran. Our analysis included 2618 PWID with a recent history of drug injection, recruited from 11 cities across Iran using a respondent-driven sampling approach. The primary outcome was a lifetime history of drug-related non-fatal overdose. The nation-wide lifetime prevalence of overdose among PWID was 21.7%, with considerable variations across different cities. In the multivariable analysis, older age, being men, history of homelessness, younger age at injection initiation, recent stimulant injection, recent non-prescribed benzodiazepine use, lifetime suicide attempt, and HIV sero-positivity were positively associated with a history of non-fatal overdose. Our findings underscore the need for comprehensive and tailored approaches to overdose prevention and treatment in Iran, including increased access to take-home naloxone for PWID. The diverse risk profiles of PWID should also be considered in the development of substance use harm reduction policies in Iran, reflecting their varied needs in terms of mental, physical, and social well-being.

Abstract:Introduction Opioid use disorder is a pattern of problematic opioid use, leading to impaired functioning or clinically significant suffering. Morocco, a pioneer in the Arab world in the field of opiate substitution, is no exception to this rule, and has found itself confronted with a situation where opiate use is much more widespread in the north of the country. Morocco's geographical proximity to Europe and the multiple interactions fostered by migratory population flows undoubtedly contribute not only to the spread of hard drug use, particularly heroin, but also to the diversification of consumption methods (injection drugs) Objectives The main objective of our work was to study socio-demographic and clinical profile of opoid users in morocco, but also their quality of life after treatment in Morocco, before concluding with recommendations for improving the overall management of the patient. Methods We conducted a cross-sectional, analytical study in the Addictology Department at Ar-razi Hospital in Salé, which provides oral methadone substitution therapy for around 80 patients. Results The total number of patients responding to the questionnaire was 60 participants. The population of methadone-treated patients in our study was 83.33% (n = 50) male and 16.67% female (n = 10). The most common age group in our study was between 31 and 45 (71.67%). 36.67% were married (n=22), 80% (n=48) lived with their family, 83.34% (n=50) had a secondary school education or higher, while the vast majority 63.33% (n=38) had no fixed occupation. 96.7% (n=58) of participants were of Moroccan nationality, against only 3.3% who were foreigners (n=2). The main indication for methadone withdrawal in our patients was heroin use (66.67%), followed by Codeine, then Tramadol. The daily doses of methadone delivered ranged from 04 to 200 mg/patient, with an average of 75 mg. The main adverse effects reported by our patients were libido disturbance, constipation, fatigue and sleep disturbance. 63.33% (n=38) of patients continued to use other psychoactive substances on a regular basis, mainly tobacco, followed by cannabis. 13.33% (n=8) reported persistent craving, and the vast majority claimed to be supported by a family member (70%, n=40). Conclusions For several years, quality of life has been a major preoccupation of healthcare professionals in a bio-psycho-social approach. In this vision of care, quality of life should now be part of the clinical criteria for monitoring patients on methadone. Disclosure of Interest None Declared

Abstract:The Islamic Republic of Iran, in response to the highest rate of opium consumption in the world, has devoted substantial resources to reduce drug availability and address addiction-related problems. Demand-reduction activities have included residential rehabilitation centers, outpatient treatment centers, and support for addiction recovery mutual aid organizations such as Narcotics Anonymous. The growing role of voluntary nongovernmental organizations (NGOs) in supporting addiction recovery in Iran has not been fully described in the professional literature. This report describes one such NGO, Congress 60, which was founded in 1998 and has since grown to 38 branches in Iran with more than 20,000 members. Included in the review are the history, governance, philosophy, and recovery support methods of Congress 60.

Abstract:The effects of two beta-adrenoceptor antagonists, propranolol (0.2 mg/kg) and metoprolol (0.2 mg/kg) on some physiological and metabolic changes produced by maximal exercise in the horse were investigated. Both drugs reduced the elevation in heart rate seen immediately following exercise and reduced performance as was seen by the increased time taken to perform each gallop. The rise in plasma glucose, glycerol and lactate, and the fall in blood pH seen following exercise were attenuated by both drugs. However, a greater increase in plasma free fatty acids occurred. Exercise produced a nine- to 12-fold increase in plasma noradrenaline levels. Neither drug had any effect on resting levels of noradrenaline but after metoprolol the levels during exercise were increased.

Abstract:Background: Addiction is a chronic and relapsing disorder characterized by compulsive drug seeking and lasting changes in the brain. Low quality of life may influence the substance craving, which leads to relapsing. Therefore, the present study aimed to investigate the relationship between beliefs in substance craving and quality of life among narcotics anonymous. Methods: This cross-sectional study was performed on 202 narcotics anonymous in Kerman, southeastern Iran. Convenience sampling was used to select anonymous patients in the Narcotics Anonymous (NA) association in Kerman city. Narcotics anonymous completed the demographic questionnaire, the Craving Beliefs Questionnaire (CBQ), and the World Health Organization Quality of Life-BREF (WHOQOL-BREF). Multiple linear regression model was used to determine the predictors of craving beliefs. The data were then analyzed using SPSS 22. Results: The mean age of the participants was 38.48 ± 11.32 years. The majority of the samples were male (86.1%), married (65.4%), educated (93.6%) and urban (86.1%). The mean scores of craving beliefs and overall Quality of Life (QOL) were 77.58 ± 20.70 and 64.42 ± 23.13, respectively. Forty-two-point 1% had high level of craving beliefs. We found a significant negative and weak correlation between beliefs in substance craving, physical health domain (r = -0.16, p = 0.02), and overall quality of life (r = -0.15, p = 0.03). History of crack use, job, and physical health domain of QOL were predictors of beliefs in substance craving among the NAs. Conclusion: Based on the study results, the participants had a high level of substance craving, and some aspects of the quality of life had an impact on the beliefs of addiction. However, it is necessary to conduct more studies in this field; psychological interventions and programs to increase the quality of life may reduce the substance craving.

Abstract:Background: The long time use of opium has some effects on serums biochemical factors, the determination of this variation is a new approach in understanding off addiction and relive of drug abuser health. Hence in this study, these indicators in person who were withdrawing of opium have been studied. Methods: In this cross-sectional study bloods biochemical factors such as fasting blood sugar (FBS), sodium (Na), calcium (Ca), uric acid (UA), blood urea nitrogen (BUN), creatinine, cholesterol, total protein, and fibrinogen in three groups serum were studied: (1) Who had been permanent opium users more than 2 years (case). (2) Dependent person who has taken one month addiction withdrawal course (control). (3) A healthy group that had been demographically similar to the other groups. Findings: According to these study findings, FBS serum level in the case group is lower than control group. Serum level of Na, creatinine, and blood triglyceride (TG) in case study are higher than group control. Concentration of potassium, Ca, UA, BUN, cholesterol, total serum protein, fibrinogen, and thrombin time in case study and group control showed no significant difference. Also, in withdrawing case serum level of Na, Ca, UA, BUN, creatinine, and TG significantly increase and thrombin time decrease. Conclusion: According to this study not only the longtime use of opium but also opium with drawerin opium dependent people can change their serum biochemical factors. So recognition, treatment, and prevention of this change could be a new step in improving of health and condition of patients.
The Role of Endophytic Fungal Individuals and Communities in the Decomposition of Pinus massoniana Needle Litter

Abstract:The role of fungal endophytes (FEs) as “pioneer” decomposers has recently been recognized; however, the extent to which FEs contribute to litter loss is less well understood. The genetic and enzymatic bases of FE-mediated decomposition have also rarely been addressed. The effects of populations and individuals (with an emphasis on two dominant Lophodermium taxa) of FEs on needle-litter decomposition were assessed for Pinus massoniana, a ubiquitous pine in southern China. Data from in vivo (microcosm) experiments indicated that the percentage of litter-mass loss triggered by FEs was linearly correlated with incubation time and approached 60% after seven months. In vitro decomposition tests also confirmed that endophytic Lophodermium isolates caused 14–22% mass loss within two months. Qualitative analysis of exoenzymes (cellulase and laccase, important for lignocellulose degradation) revealed that almost all of the Lophodermium isolates showed moderate or strong positive reactions. Furthermore, partial sequences of β-glucosidase (glycoside hydrolase family 3, GH3), laccase, and cellobiohydrolase (GH7) genes were amplified from Lophodermium isolates as “functional markers” to evaluate their potential for lignocellulolytic activity. Three different genes were detected, suggesting a flexible and delicate decomposition system rich in FEs. Our work highlights the possibility that the saprophytism and endophytism of FEs may be prerequisites to initiating rapid decomposition and thus may be key in Fes’ contribution to litter decomposition, at least in the early stage. Potential indicators of the presence of core fungal decomposers are also briefly discussed.
