Abstract:Purpose: To study the binocular alignment and ocular motility in patients with large-angle esotropia due to sixth nerve palsy treated with double augmented vertical recti transposition. Methods: This was a prospective interventional study. Fifteen patients with non-resolving sixth nerve palsy who underwent surgical correction were included in the study. Fourteen patients also underwent an additional medial rectus recession. Two patients with an associated small vertical deviation had a selective augmentation of one vertical rectus muscle. Binocular alignment, ocular motility, duction limitation, improvement in head posture, induced vertical deviations, and field of diplopia-free binocular single vision (when possible) were analyzed. Successful outcome was defined as a residual horizontal deviation of 10 prism diopters (PD) or less with no vertical deviation at final follow-up (6 months). Results: The double augmented Hummelsheim procedure improved esotropia from 58.3 ± 10.8 PD preoperatively to 7.2 ± 5.1 PD postoperatively (P = .001). Three (20%) patients had residual deviation of greater than 10 PD, of which 1 patient had diplopia and was treated with prisms. Postoperative binocular field of vision was performed in 6 patients, the mean of which was 20° for abduction and 45° for adduction. Three of 6 patients had elimination of face turn and the rest had residual head posture of less than 5°. Two patients had an induced vertical deviation of less than 4 PD. In patients who had selective augmentation, the vertical deviation was completely corrected. Conclusions: The patients operated on with double augmentation of the Hummelsheim procedure combined with medial rectus recession had reduced mean primary esotropia and improved diplopia-free field of vision postoperatively. Abstract:BECAUSE ONLY ABOUT 6.10%o of women with delayed menstruation spontaneously became pregnant, all sorts of methods have been tried to aid the remaining large number of infertile women to conceive. In 1926, Rubin' called attention to the fact that, because x-ray therapy was capable of regulating menstrual irregularity, it also was successful in producing favourable responses in cases of sterility associated with habitual amenorrhcea. In 1944, Mazer and Greenberg2 reported on 330 cases of amenorrhoea treated by low dosage irradiation; 71% responded favourably and an examination of the children born to these women indicated no deleterious effect on them. Drips,3 at the Mayo Clinic, also found irradiation effective and harmless for the treatment of female sterility. In 1951, Mazer and Israel4 advocated x-ray therapy for female sterility because it had proved successful in most instances where used, and because "no one has reported adverse effects on the patient and her immediate offspring". In a very complete and careful report, based upon his experience over a long period of time, Israel5 in 1952 stated that even though irradiation in the treatment of infertility was used on an empirical basis, it might safely be employed in properly selected cases and was also effective for the treatment of secondary amenorrhcea. Playfair and Booth6 also found x-ray therapy effective in the treatment of secondary amenorrhcea and at the same time stated that it has never been proved that, cytologically, x-rays have permanently damaged either the pituitary gland or the ovaries, provided the x-rays were properly applied. In 1952, Rubin7 again advocated irradiation for the treatment of female sterility, for in a follow-up of such treated women he had found no deleterious effect from the x-ray treatment in the women so treated, their offspring or the children of these offspring. Abstract:Background: Augmented transposition of the superior and inferior rectus muscles to the lateral rectus muscle is effective surgical treatment for esotropia in unilateral Duane syndrome. Medial rectus muscle recession in bilateral Duane syndrome may increase the risk of consecutive exotropia and cause limitation to adduction postoperatively. Vertical rectus muscle transposition may be useful in bilateral Duane syndrome with esotropia. Methods: We undertook a retrospective review of 11 patients with bilateral Duane syndrome and esotropia in primary position. All patients had vertical rectus muscle transpositions. Six patients had unilateral vertical rectus transpositions (2 eyes with and 4 without suture augmentation). Twelve eyes from 7 children (2 unilateral and 5 bilateral) had transpositions augmented with posterior fixation sutures. Posterior fixation suture were added to large deviations in patients without prior medial rectus recessions. Results: The preoperative esotropia at distance was 22.8 +/- 6.3 prism diopters (PD). It reduced to 2.0 +/- 6.7 PD postoperatively. (P < 0.001) Esotropia at near changed from 21.0 +/- 5.8 PD preoperatively to 1.2 +/- 8.1 PD postoperatively. (P < 0.001) One patient with a 10-degree face turn had complete resolution postoperatively. One patient had a small undercorrection and developed a vertical deviation requiring additional surgery. All patients had improvement in abduction. Nine of 11 patients did not develop any limitation to adduction. One patient developed a -1 adduction deficit 5 years later. Three patients achieved fusion with a mean stereovision of 67 seconds of arc (range, 80-40 seconds.). Follow-up averaged 22.2 months (range, 1-100 months). Conclusion: Vertical rectus muscle transposition in patients with bilateral Duane syndrome and esotropia is an effective procedure to improve ocular alignment and motility while preserving adduction.
Surgical Outcomes of Transposition Surgery for the Correction of Large-Angle Strabismus

Abstract:Background: Many potential surgical options exist to address large-angle deviations and head turns that result from various forms of paralytic strabismus. Muscle transposition surgeries serve as suitable alternatives to simple resection-recessions. Here, we report outcomes of augmented Hummelsheim and X-type transpositions for the correction of large-angle strabismus and provide insights for surgical planning. Methods: We performed a retrospective chart review of 40 consecutive patients with strabismus who were treated with an augmented Hummelsheim or X-type transposition surgery at a single academic medical center. Etiologies included cranial nerve palsies (n = 26), monocular elevation palsy (n = 3), Duane syndrome (n = 1), traumatic extraocular muscle damage (n = 8), and chronic progressive external ophthalmoplegia (n = 2). All patients were followed for a minimum of 2 months postsurgery. Logistic regression analyses were performed to assess for predictors of surgical outcome. Results: Forty consecutive patients were enrolled in our series. The median preoperative deviation was 46.5Δ (interquartile range [IQR] 35-70). The median postoperative deviation 2 months after surgery was 0.5Δ (IQR 0-9.5), which represented a significant improvement (P < 0.001). Thirty-three patients (82.5%) experienced an improvement in range and/or centration of binocular single vision (BSV). More patients who underwent an augmented Hummelsheim procedure and had a small overcorrection at postoperative day 3 had a favorable result on postoperative month 2 (79%) compared with those that were initially under-corrected (38%). Multiple logistic regressions found larger preoperative deviation (P < 0.005) and esotropia (P < 0.021) to be predictors of a less favorable surgical outcome (C-statistic = 0.83). Subgroup analysis revealed that less, favorable outcome after X-type transposition occurred most frequently in patients undergoing correction of an esodeviation. Conclusion: Augmented Hummelsheim transposition techniques offer effective treatment options for paralytic strabismus with esotropic deviations, whereas X-type transpositions are effective for exotropic deviations and deviations from severe inferior rectus damage. In addition to potentially providing a wider field of BSV, improved centration is often achieved.
Three-muscle surgery for large-angle esotropia in chronic sixth nerve palsy: comparison of two approaches

Abstract:Aims: To report the effect of two three-muscle surgeries, inferior rectus belly transposition plus augmented superior rectus transposition plus medial rectus recession (ISM) and modified vertical rectus belly transposition plus medial rectus recession (VM), in the management of large-angle esotropia in Chinese patients with chronic sixth nerve palsy. Methods: Twenty-eight consecutive patients with large-angle esotropia ≥50Δ were prospectively enrolled and underwent either ISM or VM. Main outcomes included preoperative and postoperative deviation in primary position, abduction limitation and complications. Follow-up was at least 6 months. Results: Of the included patients, 13 underwent ISM and 15 underwent VM. Preoperatively, ISM group displayed larger esotropia and more severe abduction limitation. 27 patients completed the follow-up. The postoperative horizontal deviation and abduction limitation were similar in both groups. At the last follow-up, ISM group demonstrated greater improvement of abduction limitation than VM group in both grading (group difference -2.1, p<0.001) and quantitation (group difference 2.6 mm, p=0.001). However, eight (30%) patients revealed an induced adduction limitation ≤-1. Of the 22 patients with unilateral palsy, more esotropia of 14.8Δ was corrected in ISM group, compared with VM group (p=0.003). Three patients (14%) developed vertical diplopia and three (14%) developed torsional diplopia. Unexpectedly, keratitis was observed in 4 of 27 (15%) patients, all with concurrent fifth and/or seventh nerve palsy. Three patients aggravated to corneal ulceration. Conclusions: Two three-muscle surgeries, ISM and VM were both effective for large-angle esotropia in Chinese patients with chronic sixth nerve palsy. However, attention should be paid to potential complications.
Modified Nishida Procedure Combined with Lateral Rectus Disabling for Duane Retraction Syndrome

Abstract:Duane retraction syndrome (DRS) is a complex congenital cranial dysinnervation disorder. The choice of surgical procedure in esotropic-DRS depends upon several factors that include: the amount of esotropia in the primary position, the presence and severity of palpebral fissure narrowing, globe retraction, presence of medial rectus muscle (MR) contracture, the likelihood of improving abduction, age of the patient, and the presence of binocularity and stereopsis. In the presence of MR contracture, MR recession is performed either alone (unilaterally or bilaterally) or in conjunction with Y splitting plus recession of the lateral rectus muscle (LR) for reducing globe retraction. MR recession, with or without adjustable sutures, may be simultaneously combined with partial thickness vertical rectus muscle transposition (VRT) or with superior rectus muscle transposition (SRT). We describe a novel combination of surgical procedures in the management of esotropic-DRS in two patients. In our first patient, following an initial MR recession combined with LR disinsertion and periosteal fixation (LRDAPF), a modified Nishida procedure was performed. In our second patient following a prior simultaneous MR recession and LR Y splitting with recession, we combined periosteal fixation of the LR with a modified Nishida procedure of the vertical rectus muscles.
Supramaximal Horizontal Rectus Recession–Resection Surgery for Complete Unilateral Abducens Nerve Palsy

Abstract:Purpose To review the surgical procedures and outcomes of supramaximal horizontal rectus recession–resection surgery for abduction deficiency and esotropia resulting from complete unilateral abducens nerve palsy. Methods A total of 36 consecutive cases diagnosed as complete abducens nerve palsy, receiving supramaximal medial rectus recession (8.5 ± 1.4 mm, range: 6–10) combined with a supramaximal lateral rectus resection (11.1 ± 1.7 mm, range: 8–14) as performed over the period from 2017 to 2020, were reviewed retrospectively. All surgeries were performed by a single surgeon. Pre- and post-operative ocular motility, ocular alignment, forced duction test, binocular vision, abnormal head posture, and surgical complications were assessed. Results Of these 36 cases, 23 (63.8%) were followed up for greater than 2 months (Mean ± SD = 8.4 ± 6.0, range: 2–24) after surgery and the collected data was presented. Mean ± SD age of these patients was 41.7 ± 14.4 (range: 12–67) years with 73.9% being female. Trauma (52.2%, 12/23) and cerebral lesions (21.7%, 5/23) were the primary etiologies for this condition. Esodeviation in primary position improved from 55.5 ± 27.2 prism diopters (PD) (range: +25 to +123) to 0.04 ± 7.3 PD (range: −18 to +12) as assessed on their last visit. Pre-operative abduction deficits of −5.6 ± 1.0 (range: −8 to −4) reduced to −2.4 ± 1.4 (range: −4 to 0) post-operatively. The mean dose-effect coefficient of 2.80 ± 1.20 PD/mm (range: 1.07–6.05) was positively correlated with pre-operative esodeviation. Rates of overcorrection and ortho were 69.6 and 26.1%, respectively, on the first day after surgery, while on their last visit the respective levels were 4.3 and 82.6%. Conclusion Supramaximal horizontal rectus recession–resection surgery is an effective treatment method for complete abduction deficiency. The dose-effect was positively correlated with pre-operative esodeviation. Overcorrection on the first day post-operatively is required for a long-term satisfactory surgical outcome.

Periosteal fixation of the medial and lateral recti for large-angle incomitant exotropia

Abstract:Purpose: To describe the evolution of a surgical technique for the correction of large-angle incomitant exodeviations. Methods: Retrospective review of an interventional case series from 2005 to 2019 in a single centre, with analysis of surgical procedure, prism diopter (PD) deviations and complications. Results: Thirty-one patients underwent surgery at an average age of 42 years (range 4-75 years) for minimal medial rectus function, mostly from oculomotor nerve palsy (23/31; 74%). The mean pre-operative exodeviation was 75 PD (range 30-200PD). Sixteen patients (52%) had undergone previous strabismus surgery. Thirty-eight operations were performed in which the medial rectus insertion was anchored to the periosteum of the posterior lacrimal crest via a retrocaruncular transconjunctival approach. The ipsilateral lateral rectus (LR) was disinserted and fixed to lateral orbital tissue in 29/38 (76%) operations, injected with botulinum toxin in 5, recessed in 2 and had already undergone maximal LR recession in 2. In all but the first 8 operations, temporary limbal sutures were passed through the eyelids to maximally adduct the globe post-operatively. At last follow-up (mean 24 months; range 2-130), the mean reduction in exodeviation was 49PD (range 10-80) and overall residual deviation was 26PD (range 80PD base-in to 14PD base-out). The 5 LR toxin procedures had a mean reduction of 22PD (range 10-40). Seven patients had persistent diplopia, one a transient corneal erosion and one caruncle suture exposure 4 years after surgery. Conclusion: Large-angle exodeviations can be markedly improved by bi-rectus fixation. This approach is both safe and effective and can be performed in complex patients with multiple previous procedures.
[Construction of a RU486 inducible recombinant adenoviral vector carrying murine interleukin-12 gene and experimental treatment of colonic carcinoma].

Abstract:OBJECTIVE To construct a RU486 inducible recombinant adenovirus of murine IL-12 protein and study its effect and safety on colonic cancer. METHODS The replication-defective recombinant adenovirus were produced after cotransfection of shutter vector pDC-RUmIL-12 and adenovirus DNA helper plasmid pBHGloxDeltaE1, 3Cre into HEK293 cells. The recombined adenovirus was purified by CsCl density gradient centrifugation and its titer was determined by end point dilution assay. Expression of this regulatable recombinant adenovirus vector in infected C26 colonic carcinoma cells was tested by ELISA kit in vitro. The tumor model was established by hypodermic inoculation of C26 cells. Sixty tumor-bearing mice were randomly divided into 4 groups: Ad-buffer group; Ad-RUmIL-12 group; Ad-RUmIL-12 + RU486 group and Ad-mIL-12 group, and the treatment effects and side effects were evaluated. RESULTS The adenoviral vector containing murine IL-12 gene was identify by PCR. The viral titer of Ad-RUmIL-12 was 4.62 x 10(10) pfu/ml. The expression of IL-12 protein was induced by the RU486 and the highest expression (516 +/- 43) pg/ml whereas no significant IL-12 protein was detected without inducer or getting rid of the inducer [(38 +/- 3) pg/ml and (42 +/- 5) pg/ml respectively]. The tumor size increased rapidly in group Ad-buffer and group Ad-RUmIL-12 (P > 0.05). Administration of Ad-RUmIL-12 + RU486 and Ad-mIL-12 were showed to delay markedly the growth of transplanted C26 tumor (P > 0.05). Significantly necrosis was observed in both Ad-mIL-12 and Ad-RUmIL-12 + RU486 experimental groups, but the level of the serum alanine transaminase and the rate of side effect was higher in Ad-mIL-12 group (4/15 and 10/15 respectively, P < 0.05). CONCLUSION A RU486 regulatable recombinant adenoviral vector containing IL-12 gene was successfully constructed. The expression of vector Ad-RUmIL-12, regulated by inducer RU486 in vivo, can obviously improve safety in tumor treatment and provide a good primer for further researches on in vivo gene therapy.
A Novel Technique Using Paretic Superior Rectus as a Globe Suspender for Monocular Elevation Deficiency

Abstract:Surgical innovations in strabismus provide opportunities to improve visual function, eye alignment, and cosmesis in rare pediatric ophthalmological conditions. Monocular elevation deficiency is a rare and multifactorial disease in which the affected eye is equally limited in terms of elevation during adduction and abduction. We aimed to present a novel procedure for the treatment of acquired monocular elevation deficiency using the paretic superior rectus muscle as a globe suspender to resolve hypotropia. We report the case of an eight-year-old girl with left eyelid ptosis and hypotropia two months after draining a left orbital abscess. Left inferior rectus muscle recession was performed at five years, with residual left hypotropia. Ophthalmological examination revealed a best-corrected visual acuity of 20/20 OD and 20/100 OS. Severe left eyelid ptosis and poor levator function were also observed. Extraocular motility showed left hypotropia of 40 prism diopters with the left superior rectus muscle under action (-4) in the adduction and abduction positions. A force duction test negative for restrictions on the inferior rectus muscle was performed intraoperatively. To reduce the risks of the Knapp procedure, the left superior rectus muscle was split into medial and temporal halves. Double-armed sutures were secured in half, and the halves were detached from the sclera. The medial and temporal halves were reattached anteriorly to the medial and lateral rectus insertions, respectively. Eight weeks after surgery, the patient had nine prism diopters of hypotropia in the primary gaze. Ten weeks after surgery, there was no change in visual acuity. In the cover test, the patient exhibited residual left hypotropia of nine prism diopters with a restriction (-4) of elevation in adduction and abduction. The parents were pleased with the satisfactory cosmetic outcomes, and postoperative clinical photographs of the patient showed improved hypotropia and persistent minimal elevation of the left eye during adduction and abduction. Superior rectus muscle splitting and vertical transposition to the medial and lateral rectus could be safer and simpler alternatives to the Knapp procedure and may offer a lower risk of anterior segment ischemia. Abstract:Background: Esotropia resulting from sixth cranial nerve palsy can substantially impact an individual's visual acuity and overall quality of life. If the condition does not resolve in 6-10 months, surgical intervention may be necessary. Various muscle surgeries may be considered, with vertical rectus muscle transposition emerging as the primary option for treatment of complete palsy. However, this technique carries the risk of anterior segment ischemia and post-surgery deviations. Herein, we present a successful treatment of chronic complete sixth nerve palsy using a modified Nishida procedure, without splitting or tenotomy, and an adjunct botulinum toxin A (BTA) injection in the ipsilateral medial rectus muscle. Case presentation: A 59-year-old woman with a history of traumatic sixth nerve palsy had previously undergone horizontal muscle strabismus surgeries. Following multiple left medial rectus recessions, lateral rectus resection, and BTA injections, esotropia persisted. The worsening of her condition led to emotional distress and impaired social interaction. Initial examination revealed marked esotropia and limited left eye abduction. Magnetic resonance imaging (SIGNA MR750w, GE Healthcare, Waukesha, WI, USA) of the left eye revealed a contracted medial rectus muscle and substantial atrophy of the left lateral rectus muscle. A modified Nishida procedure was performed with an injection of 3 units of BTA into the ipsilateral medial rectus muscle, resulting in improved ocular alignment and stable findings after nine postoperative months. Furthermore, we supported our successful outcome with a summary of similar reported cases of sixth nerve palsy managed using the modified Nishida procedure with or without adjunctive procedures. Conclusions: Following the modified Nishida procedure, the patient experienced a reduction in diplopia, improved ocular alignment and stability, and an increased binocular diplopia-free field. This case underscores the importance of an individualized approach to complex strabismus cases and highlights the modified Nishida procedure as a valuable tool in such circumstances. In the future, strabismus management will focus on refining personalized treatment and exploring innovative techniques for complex cases. Our success in using a combination of Nishida procedure and BTA injection should be further investigated in large-scale studies.
Novel superior oblique anterior fiber plication with or without adjustable sliding knot for extorsion

Abstract:Purpose: To report a novel surgical technique to correct excyclotropia, consisting of a superior oblique anterior fibers plication (SOAFP) with or without a hemihangback anterior knot, allowing access for postoperative adjustment. Methods: A retrospective interventional case series was conducted. Fourteen patients, 21-92 years of age, underwent SOAFP (18 eyes, 14 eyes on adjustable), at the Mayo Clinic in Rochester, Minnesota. SOAFP was the only procedure performed in 12 eyes; in 6 it was performed in conjunction with up to four horizontal rectus muscle recession, resection, and/or plication. Ocular alignment was assessed with prism and alternate cover and double Maddox rod tests; preoperatively, at initial and final (closest to 6-8 weeks) postoperative visits. Results: Preoperative torsion ranged from 2° to 30° of extorsion (mean, 10.14 ± 7.01). A SOAFP of 2-30 mm (mean, 8.93 ± 5.63) was performed. At the initial postoperative examination, mean intorsional shift was 11.18 ± 7.37, accounting for 1.86° ± 1.04° of correction per millimeter of plication. Three eyes were adjusted after the initial visit to obtain a stronger plication effect targeting of 5° intorsion. At the final visit, 61 ± 23 days postoperatively, mean extorsion was 1.21° ± 2.29°, ranging from 5° of extorsion to 3 of intorsion. Mean final intorsional shift was 9.14 ± 7.53°, accounting for a 1.16 ± 0.50° of correction per millimeter of plication. Of our 14 patients, 13 had improvement in diplopia. Conclusions: In our study cohort, SOAFP allowed for targeted and easily adjustable correction of extorsion.

Methods: This was a retrospective analysis of the outcomes from a single surgeon performing a plication of the inferior half of the medial rectus muscle in 17 consecutive patients with near exotropia unresponsive to medial rectus bupivacaine injection. Results: Thirteen of 17 (76%) patients were asymptomatic after surgery with no diplopia for near fixation and with either a normal or slightly reduced prism fusion range enabling them to have comfortable binocular single vision. There was one minor surgical overcorrection with distance diplopia that disappeared within 2 weeks of surgery. Of the 4 of 17 (24%) patients who required further intervention, 3 required one further surgical procedure and 1 required an injection of bupivacaine into the contralateral medial rectus muscle to obtain a satisfactory alignment and control of symptoms. No patient needed more than two total operations. Conclusions: The data show medial rectus inferior plication can produce excellent outcomes with minimal risk of overcorrection. Abstract:The steric factors that allow trivalent [(C(5)Me(5))(3)U] (1) to function as a three-electron reductant with C(8)H(8) to form tetravalent [{(C(5)Me(5))(C(8)H(8))U}(2)(μ-C(8)H(8))] (2) have been explored by examining the synthesis and reactivity of the intermediate, "[(C(5)Me(5))(2)(C(8)H(8))U]" (3), and the slightly less crowded analogues, [(C(5)Me(5))(C(5)Me(4)H)(C(8)H(8))U] and [(C(5)Me(4)H)(2)(C(8)H(8))U], that have, successively one less methyl group. The reaction of [{(C(5)Me(5))(C(8)H(8))U(μ-OTf)}(2)] (4; OTf=OSO(2) CF(3)) with two equivalents of KC(5)Me(5) in THF gave ring-opening to "[(C(5)Me(5))(C(8)H(8))U{O(CH(2))(4)(C(5) Me(5))}]" consistent with in situ formation of 3. Reaction of 4 with two and four equivalents of KC(5)Me(4)H generates two equivalents of [(C(5)Me(5))(C(5)Me(4)H)(C(8)H(8))U] (5) and [(C(5)Me(4)H)(2)(C(8)H(8))U] (6), respectively, which in contrast to 3 were isolable. Tetravalent 5 reduces phenazine and PhEEPh (E=S, Se, and Te) to form the tetravalent uranium reduction products, [{(C(5)Me(5))(C(8)H(8))U}(2)(μ-C(12)H(8)N(2))] (7), [{(C(5)Me(5))(C(8)H(8))U}(2)(μ-SPh)(2)] (8), [{(C(5)Me(5))(C(8)H(8))U}(2)(μ-SePh)(2)] (9), and [{(C(5)Me(5))(C(8)H(8))U}(2)(μ-TePh)(2)] (10), consistent with sterically induced reduction. In contrast, the less sterically crowded 6 does not react with these substrates.

Abstract:Purpose: The aim of this study was to report our postoperative results concerning the vertical rectus (VR) muscle union combined with lateral rectus (LR) plication for the treatment of large-angle esotropia (ET) in complete abducens nerve palsy. Methods: Medical records from 36 patients who had undergone the VR union procedure for ET treatment due to sixth-nerve palsy between July 2014 and July 2018 at Beyoglu Eye Research and Training Hospital were reviewed. One week before surgery, all patients underwent a 4-IU botulinum toxin A (btx) injection into the ipsilateral medial rectus (MR). All patients then underwent a VR muscle union procedure. A non-absorbable suture was inserted through the lateral muscular margin of each VR muscle at approximately 1/5 the width from the edge at 10 mm distance from the VR insertion. Both sutures were then tied to each other above the LR. Plication of the LR muscle using a non-absorbable suture was performed in all cases. Results: The study population consisted of 14 (38.9%) females and 22 (61.1%) males. The mean age was 36.31 ± 19.16 years. The mean preoperative deviation angle in primary gaze into distance was 47.77 ± 18.48 prism diopter (PD). The mean deviation angle 1 year after surgery was - 1.0 ± 6.62 PD. Abduction improved from - 4.27 ± 0.46 to - 1.88 ± 0.96. Conclusion: The VR muscle union in combination with LR plication appears to be an effective treatment procedure for complete abducens nerve palsy patients.

Abstract:Background: The combined superior oblique tendon tuck and inferior oblique recession is the method of choice for the treatment of congenital and acquired superior oblique muscle palsies with large vertical and cyclodeviations. An additional transposition of the insertions is indicated if the vertical deviation differs greatly from the excyclodeviation. Patients and methods: 38 patients who underwent combined oblique muscle surgery for congenital superior oblique muscle palsy were divided into 3 groups. Group A: 14 patients with small excyclodeviations in spite of large vertical deviations, even after diagnostic occlusion of 3 days. A superior oblique tendon tuck of 5 or 6 mm with transposition of the new insertion to the posterior margin of the original insertion was performed combined with a recession of the inferior oblique muscle of 5 or 6 mm with anterior displacement of 4-6 mm. Group B: 15 patients without a discrepancy between the vertical deviation and the cyclodeviation. They were treated with a conventional superior oblique tendon tuck of 5 or 6 mm combined with a recession of the inferior oblique muscle. Group C: 9 patients without a discrepancy between the vertical deviation and the cyclodeviation, but deviations smaller than in group B. A conventional superior oblique tendon tuck of 6 mm combined with a posterior tenotomy of the inferior oblique muscle was performed. The surgical effect was calculated as the difference between the preoperative deviation after diagnostic occlusion of 3 days and the postoperative deviations (1 week and 3 to 6 months). All angles of deviation were measured subjectively after dissociation of the binocular vision with a dark red filter in front of the fixating, not affected eye at 2.5 m distance using the Harms tangent screen. Results: 3 months postoperatively, the vertical deviation was diminished in primary position by 12.1 degrees/6.8 degrees/6.8 degrees in groups A/B/C, in 25 degrees adduction of the affected eye by 18.4 degrees/13.8 degrees/9.5 degrees. The reduction of the cyclodeviation in primary position was 6.4 degrees/8.0 degrees/5.6 degrees. As a side effect a clinically insignificant, consecutive Brown syndrome occurred in each group. In each group a significant positive correlation between the preoperative deviation and the surgical effect was found. Conclusions: The combined superior oblique tendon tuck and inferior oblique recession with transposition of the insertions is a suitable method for a selective treatment of the vertical deviation and the cyclodeviation in congenital superior oblique palsy.
Botulinum toxin augmented bilateral lateral rectus recession versus three muscles surgery in large-angle intermittent exotropia

Abstract:Aim: To report the surgical outcomes of correcting large angle intermittent exotropia in adult patients by bilateral lateral rectus muscle recession with intraoperative use of botulinum toxin and to compare the results with those of bilateral lateral rectus muscle recession and unilateral medial rectus resection. Methods: The medical records of patients who underwent surgical correction of large angle intermittent exotropia [exotropia >50 prism dioptre (PD)] were retrospectively reviewed. Two groups of patients were identified; Group I (21 patients) had bilateral lateral recti recession augmented with intraoperative botulinum toxin A (BTA) injection into the recessed muscles and group II (30 patients) were treated by bilateral lateral recti recession with unilateral medial rectus muscle resection. Preoperative data were extracted for age, gender, refraction, type of exotropia, angle of stereopsis and angle of deviation. The main outcome measures were the postoperative angle of deviation and stereoacuity angle by Titmus test measured at the end of one year of postoperative follow up. Results: By the end of the first postoperative year, 10 patients in group I (47.6%) and 20 patients in group II (66.7%) achieved esotropia/esophoria <5 PD or exotropia/exophoria <10 PD. The difference in surgical success rate was not statistically significant (P=0.1) but there was a statistically significant higher rate of undercorrection in group I (P=0.03). On the other hand, 3 patients in group I (14.3%) and 5 patients in group II (16.7%) had improved stereopsis; this difference in the sensory outcome was not statistically significant (P=0.8). In the BTA augmented surgery group, good stereoacuity and smaller preoperative angle of deviation were associated with significantly higher surgical success rate (P=0.004, 0.01 respectively). Conclusion: BTA augmented bilateral lateral recti recession is associated with higher rate of undercorrection as compared to bilateral lateral recti recession with unilateral medial rectus resection in the correction of large angle intermittent exotropia. The surgical success rate in BTA augmented surgery group is observed to be higher in patients with preoperative smaller angle of deviation and in patients with good stereoacuity.
Lateral Rectus Disabling and Simultaneous Modified Nishida Procedure for Exotropic Duane Retraction Syndrome

Abstract:Exotropic-Duane Syndrome (XT-DRS) is a congenital cranial nerve dysinnervation disorder characterized by exotropia, anomalous head posture (contralateral face turn), limited adduction, globe retraction, palpebral fissure narrowing, upshoot and downshoot in adduction, and varying levels of limitation of abduction. Patients with XT-DRS often have poor convergence because attempted convergence induces co-contraction of the lateral rectus muscle (LR) due to anomalous LR innervation. We describe two patients with XT-DRS who underwent simultaneous lateral rectus muscle (LR) disinsertion and periosteal fixation (LRDAPF) and modified Nishida procedure (MNP) and discuss their outcome. In one patient, we combined this procedure with a contralateral LR recession. Anomalous head posture, exotropia, globe retraction, and palpebral fissure narrowing were reduced. Upshoot and downshoot were also reduced. Convergence improved in both patients.
Lateral Rectus-Medial Rectus Union: A New Surgical Technique for Treatment of Complete Third Nerve Palsy

Abstract:Purpose: To describe a new surgical technique for the treatment of complete third nerve palsy. Methods: This study included patients with chronic complete third nerve palsy. The lateral rectus muscle was split, followed by disinsertion of the superior and inferior halves, which were passed between the sclera and superior and inferior rectus muscles, respectively. Then the medial rectus muscle was sutured as posteriorly as possible from its insertion and cut. Next, the distal stump of the medial rectus muscle was split into two halves and united with the superior and inferior halves of the lateral rectus muscle. Finally, the proximal portion of the medial rectus muscle was sutured back to its original insertion. In cases with hypotropia and a functional superior oblique muscle, superior oblique tenectomy was also performed. Success was defined as postoperative horizontal deviation of 10 prism diopters (PD) or less and vertical deviation of 5 PD or less. Results: Ten patients with a mean age of 32.4 ± 18.4 years had surgery using this technique; 2 of them had a history of strabismus surgery. Mean exotropia was 84 ± 14.9 PD, which reduced to 6.5 ± 8.2 PD. Mean vertical deviation was 16.5 ± 10 PD, which reduced to 2.5 ± 3.5 PD. Mean follow-up was 13.2 ± 7.9 months, and the success rate was 70% and 90% for horizontal and vertical deviations, respectively. Conclusions: The lateral rectus-medial rectus union technique is an effective and long-lasting procedure to overcome large-angle exotropia associated with complete third nerve palsy.
