Abstract:Improving performance is an imperative for most healthcare systems in industrialised countries. This article considers one such system, the UK's National Health Service (NHS). Recent NHS reforms and strategies have advocated improved healthcare productivity as a fundamental objective of policy and professional work. This article explores the construction of productivity in contemporary NHS discourse, analysing it via the Foucauldian concept of governmentality. In this manner it is possible to investigate claims that the commodification of health work constitutes a threat to autonomy, and counter that with an alternative view from a perspective of neoliberal self-governance. Contemporary policy documents pertaining to NHS productivity were analysed using discourse analysis to examine the way in which productivity was framed and how responsibility for inefficient resource use, and possible solutions, were constructed. Data reveals the notion of productivity as problematic, with professionals as key protagonists. A common narrative identifies traditional NHS command/control principles as having failed to engage professionals or having been actively obstructed by them. In contrast, new productivity narratives are framed as direct appeals to professionalism. These new narratives do not support deprofessionalisation, but rather reconstruct responsibilities, what might be called 'new professionalism', in which productivity is identified as an individualised professional duty.

Examining presenteeism and productivity losses among nursing workers: a cross-sectional study

Abstract:Objective: To characterize presenteeism and productivity losses among nursing professionals in public and private health services. Method: A cross-sectional study was conducted with 123 participants from a public hospital and 159 from a private hospital in Manaus, Brazil. The Sociodemographic Health Questionnaire was utilized to assess presenteeism, while the Work Limitations Questionnaire measured productivity losses. Data were analyzed using non-parametric methods. Results: Health-related work loss and presenteeism were reported by 50.41% of public sector professionals and 39.62% of private sector professionals. Despite this, presenteeism was more prevalent in the private sector (93.65%) compared to the public sector. Common health issues included musculoskeletal (26.49%), mental/behavioral (19.21%), respiratory (17.22%), neurological (16.56%), and gastrointestinal (5.96%) conditions. Additionally, 54.24% of private sector professionals and 44.23% of public sector professionals did not seek treatment. The private sector exhibited greater limitations in receiving care due to time management (40.34%), mental-interpersonal tasks (49.95%), production tasks (52.54%), and physical tasks (61.30%), resulting in higher productivity losses among nurses (13.46%) and nursing technicians (15.82%). High-complexity sectors demonstrated the greatest productivity losses. Conclusion: The study identified significant differences in the characteristics of presenteeism and productivity losses between nursing professionals in the public and private health sectors. These results point to the need to improve management and occupational safety and regulatory measures to solve workers' health problems and mitigate presenteeism and productivity losses in the public and private health sectors.
On the intrinsic regulation of neuropeptide Y release in the mammalian suprachiasmatic nucleus circadian clock

Abstract:Timing of the circadian clock of the suprachiasmatic nucleus (SCN) is regulated by photic and non‐photic inputs. Of these, neuropeptide Y (NPY) signaling from the intergeniculate leaflet (IGL) to the SCN plays a prominent role. Although NPY is critical to clock regulation, neither the mechanisms modulating IGL NPY neuronal activity nor the nature of regulatory NPY signaling in the SCN clock are understood, as NPY release in the SCN has never been measured. Here, microdialysis procedures for in vivo measurement of NPY were used in complementary experiments to address these questions. First, neuronal release of NPY in the hamster SCN was rhythmic under a 14L : 10D photocycle, with the acrophase soon after lights‐on and the nadir at midday. No rhythmic fluctuation in NPY occurred under constant darkness. Second, a behavioral phase‐resetting stimulus (wheel‐running at midday that induces IGL serotonin release) acutely stimulated SCN NPY release. Third, bilateral IGL microinjection of the serotonin agonist, (±)‐2‐dipropyl‐amino‐8‐hydroxyl‐1,2,3,4‐tetrahydronapthalene (8‐OH‐DPAT) (another non‐photic phase‐resetting stimulant), at midday enhanced SCN NPY release. Conversely, similar application of the serotonin antagonist, metergoline, abolished wheel‐running‐induced SCN NPY release. IGL microinjection of the GABA agonist, muscimol, suppressed SCN NPY release. These results support an intra‐IGL mechanism whereby behavior‐induced serotonergic activity suppresses inhibitory GABAergic transmission, promoting NPY activity and subsequent phase resetting. Collectively, these results confirm IGL‐mediated NPY release in the SCN and verify that its daily rhythm of release is dependent upon the 14L : 10D photocycle, and that it is modulated by appropriately‐timed phase‐resetting behavior, probably mediated by serotonergic activation of NPY units in the IGL.

Abstract:The complexity and incessant change in the corporatised health care workplace has influenced nurses' work choices, morale, quality of work-life and the wellbeing of patients. Thus, there is an urgent moral imperative to improve the quality of work-life for nurses. To this end, it is crucial to re-define progress beyond the sole economic markers of success and profit in the health care workplace. This paper argues for the identification of ethical markers and indicators of organisational success based on bridging and linking social capital which could be used to re-organise health care organisations, hence crafting inclusive moral spaces where nurses can safely work and provide quality care for patients. Social and ethical evaluation is well suited to examine current workplace dilemmas from a psychosocial perspective and provide a framework for best practice in building capacity in effective social relations and family friendly, ethical workplaces.

Abstract:Abstract Importance: Understanding burnout among practitioners in skilled nursing facilities (SNFs) and assisted living facilities (ALFs) while considering contextual factors may lead to practices that enhance therapist and patient satisfaction as well as quality of care. Objective: To examine productivity standards and burnout in the context of setting and role, as reported by therapy practitioners in geriatric settings, and to explore relationships between productivity standards and perceived ethical pressures. Design: Cross-sectional online survey with descriptive data. Participants: Practitioners (N = 366) included occupational therapists, physical therapists, speech-language pathologists, and occupational and physical therapy assistants working in SNFs and ALFs in the United States. A survey integrating the Maslach Burnout Inventory: Human Services Survey for Medical Personnel (MBI–HSS) and questions addressing demographics and contextual factors was distributed via social media. Results: Of 366 practitioners, 20.5% were burned out, exhibiting extreme scores for Emotional Exhaustion (EE), Depersonalization (DP), and Personal Accomplishment (PA) on the MBI–HSS. Significant relationships between productivity requirements and EE, DP, and PA, after accounting for covariates, were evident. Significant relationships between productivity standards and five of the six ethically questionable behaviors existed. Role affected productivity requirements, specifically between therapists and assistants, whereas setting did not. Conclusions and Relevance: Productivity standards and related pressures are associated with concerning aspects of burnout among practitioners working in geriatric settings. Advocating for change in defining productivity and incorporating positive support in the work environment may assist in reducing burnout and turnover rates and improve patient satisfaction and care. Plain-Language Summary: This research highlights the prevalence of burnout and perceived pressures related to productivity requirements among occupational therapy practitioners working in skilled nursing and assisted living facilities.

Abstract:ABSTRACT Objective: to understand the factors that intensify work from the perspective of nursing professionals working in medical and surgical clinical units. Method: this is a descriptive, exploratory study, with a qualitative approach, carried out with 18 nursing professionals working at a university hospital in southern Brazil. Data production took place between April and June 2021 through semi-structured interviews. Data processing occurred through textual analysis, with the aid of IRaMuTeQ software, with the results being subjected to content analysis. The project followed the recommendations for research involving human beings, approved by the Research Ethics Committee. Results: based on similarity analysis and Descending Hierarchical Classification, data were organized into seven classes: I - Weaknesses in physical structure and lack of materials and/or equipment; II - Complexity of patients’ clinical condition; III - Weaknesses in training, qualification and number of professionals; IV - Work rhythms and demands; V - Work process organization; VI - Weaknesses in management; and VII - Lack of interprofessional collaboration. Conclusion: work intensification is present in study participants’ daily lives, being related to the lack of materials and equipment, severity of patients’ clinical condition, weaknesses in professional training and qualification, intensified pace and increase in work demands, weaknesses in management, and lack of interprofessional collaboration.
Psychosocial risks in the practice of healthcare professionals: from the culture of stoicism to occupational suicide.

Abstract:Introduction It is well known that healthcare professionals, in a somewhat generalized manner, work in stressful contexts that embrace emotional overload, highly hierarchical environments, and not always sensitive to the vulnerabilities that arise. Chronic professional stress in institutions, associated with the perception of low control and emotional exhaustion, acts as a trigger for eminently deleterious consequences, significantly affecting the most dedicated and perfectionist professionals. Objectives This work aims, through a non-systematic literature review, to analyze the psychosocial risks associated with the practice of healthcare professionals, as well as the mitigation strategies whose practical implementation may depend on and maintenance of a positive and protective occupational environment. Methods For the purpose of literature review, a search was conducted on search engines such as Google Scholar, Research Gate, and PubMed, with no date limitations, using the following terms (or combinations): "occupational psychiatry"; "psychosocial risks AND healthcare professionals"; "mitigation strategies"; "occupational risk management." Results Healthcare sector professionals are the ones reporting exposure to higher levels of workload intensity, including parameters related to work speed (under time pressure), combined with prominent emotional demands and psychological suffering. In addition to the most commonly analyzed occupational stressors (workload, job fatigue, particularly draining emotional interactions, marked cognitive demands, complex decision-making, conflicts of a deontological nature), other relevant contextual factors emerge. Among these, predisposing personality traits (such as neuroticism), a sense of personal sacrifice with neglect of self-care, vicarious trauma, which is intimately related to compassion fatigue in the face of frequent and prolonged exposure to traumatic experiences (of various kinds) of the patients they accompany, and occupational violence, which can manifest as verbal or behavioral threats, mobbing, physical harm, and/or sexual abuse based on a tendentially gender-based and deeply hierarchical structure. Conclusions Undeniable consequences such as job dissatisfaction, psychological distress, the development of anxiety, depression, burnout, and post-traumatic stress disorder translate into a loss of capacity to perform occupational functions, with a higher risk of medical/clinical errors, conferring risks that should never be neglected to the safety of the users of healthcare institutions. On the darker side of this panorama, and in the face of chronic depletion, occupational suicide emerges. It is therefore urgent to requalify the work environment, aiming at creating and maintaining a positive occupational environment, or alternatively, a preventive approach to the risk of mental health problems originating or exacerbated in the workplace. Abstract:Technology change, rising international trade and investment, and increased competition are changing the organization, distribution and nature of work in industrialized countries. To enhance productivity, employers are striving to increase innovation while minimizing costs. This is leading to an intensification of work demands on core employees and the outsourcing or casualization of more marginal tasks, often to contingent workers. The two prevailing models of work and health – demand-control and effort-reward imbalance – may not capture the full range of experiences of workers in today's increasingly flexible and competitive economies. To explore this proposition, we conducted a secondary qualitative analysis of interviews with 120 American workers [6]. Our analysis identifies aspects of work affecting the quality of workers' experiences that are largely overlooked by popular work-health models: the nature of social interactions with customers and clients; workers' belief in, and perception of, the importance of the product of their work. We suggest that the quality of work experiences is partly determined by the objective characteristics of the work environment, but also by the fit of the work environment with the worker's needs, interests, desires and personality, something not adequately captured in current models.

Abstract:The health sector is one of the major fields of economic, social, scientific, and technological development in Brazil, and has been the arena of dispute between capitalist interests that regard health as a commodity and advocates of universal access who regard health as an essential good. The Brazilian Federal Constitution of 1988 determined that health is a fundamental right and the State's responsibility, and since then the country has made progress with public policy, but has suffered setbacks and blockages due to its status as a peripheric country, historically subordinated to the interests of core countries, which see Brazil as a broad consumer market. These external interests associated with the internal bourgeoisie have been taking advantage of the Brazilian State since the 1960s, when the foundation of business groups expanded, dominating various health segments, especially since the neoliberal policies of the 1990s. These aspects are much explored in publications in Public Health, but this text seeks a new approach, using the Marxist Theory of Dependency as a reference to analyze, albeit in a preliminary way, the situation of political, economic, and technological dependence that has distanced health policy from the ideals of a public and universal system, defended in the Brazilian Health Reform.

Abstract:Background: Nursing students on clinical placements as part of their professional training are routinely faced with situations involving ethical conflicts. The initial act of perceiving a situation as causing an ethical dilemma is the result of both the students’ personal values, drawn from their culture and families, and of the professional knowledge and values that they have acquired through training and experience. Objectives: Nursing students’ experiences on clinical placements in primary care settings were investigated in order to identify situations that they perceived as involving ethical conflict and describe the elements they took into consideration during their decision-making processes in these situations. Methods: The research design was qualitative descriptive case study. Around 50 students from three different intakes to a nursing degree answered a questionnaire and discussed it in focus groups. Ethical considerations: The study was designed in accordance with the principles guiding research with human beings and was approved by the Human Research Ethics Committee. Results: Synthesised into two principal axes: (a) ethical conflicts in primary care, linked with the domains of working processes, professional nursing ethics and human and social rights and (b) students’ decision-making processes – realisation, reflection and intervention. Conclusion: The student nurses saw themselves both as actors and spectators in situations involving ethical problems and demanding moral deliberation, demonstrating the ability to base their arguments soundly. They tended to emphasise the possibilities offered by dialogue and that different ethical values must be respected to find fair solutions to ethical problems.

Abstract:This critical-reflective essay seeks to discuss the medicalization of performance based on reflections on flexibility in the world of work and the phenomenon of self-entrepreneurship. In a context of economy financialization, productive restructuring and State's weakening as guarantor of social rights, precarization and informality are increasingly prevailing, where the promotion of labour flexibilization occurs in line with political-ideological strategies of neoliberal inspiration. There emerge ways of individualization tied to the construction of a multifunctional worker, available to develop multiple abilities, which becomes a potential enterprise and has as a principle the same market dynamics as this enterprise. The entrepreneur of the self needs to invest in itself to keep being valuable and having great functioning conditions, in order to avoid, to the fullest, its own decapitalization. Therefore, in order to enable conditions to sustain the image of success and of author of its own story, the usage of medicines, stimulants and multivitamins has been used as a strategy in search of high performance. To reflect on "the entrepreneur of the self", the new ways of subjectivity and the suffering derived from those, as well as the apparatus that produces them, is necessary and consists in a challenge for community health.

Abstract:Nursing Ethics, Ahead of Print. BackgroundMoral distress is a phenomenon that can lead to an imbalance of the mind and body. There are many coping strategies to overcome the obstacles that lead the subject to this condition. Some coping strategies are capable of being achieved through the cultivation of moral resilience.AimThe aim is to identify the strategies of moral resilience in the nursing management of University Hospitals in Brazil.Research designThe research design is the qualitative study with discursive textual analysis.Participants and research context: 44 nurse managers and nurses in leadership positions participated in a total of 30 University Hospitals in Brazil. Data were collected online, using a questionnaire with open questions.Ethical considerationsThe Ethics Committee approved the study. Participants received information about the research, agreed to respond to the questionnaire, and were guaranteed anonymity.FindingsPersonal adaptive strategies (intrapersonal and interpersonal) and organizational collaborative strategies (intrinsic and transformational management) emerged from this process. The intrapersonal strategies involved elements of rationality, flexibility, rebalancing practices, moral courage, and detachment. The interpersonal strategies addressed support networks, team involvement, and dialog. Organizational strategies dealt with actions which reorient ethical infrastructure, ethical education, and psychological protection, as well as fostering dialogical relationships, empowerment, and cooperation.ConclusionFrom the perspective of social historical construction, it is understood that developing personal and organizational strategies is essential to cultivating moral resilience.

Abstract:Abstract Objective: To grasp the perceptions and experiences of professionals in the primary health care as teamwork in the Family Health Strategy (FHS) in a medium-sized municipality in the southern of Brazil. Method: Qualitative research carried out, between April and July 2016, with 13 health professionals (nurses, doctors, dentists, community health agents, nursing technicians and oral health aide) workers in FHS. Data were collected through semi-structured interviews, transcribed and submitted to content analysis. Results: From the analysis emerged three thematic categories: Teamwork from the perspective of the FHS health professionals; Strategies for maintaining teamwork out of daily routine; and Challenges of the insertion of Oral Health in the FHS. Conclusions and implications for a practice: Participants demonstrated that they understood the importance of teamwork and interpersonal relationships, and recognize the inherent challenges of this process. They revealed weaknesses in the relationships among dentistry and other members of the FHS, marked by a sense of exclusion and disinterest in participating.

Abstract:The morbidity and mortality caused by bacterial meningitis remains significant despite advances in antimicrobial therapy and supportive care. Prevention of meningitis by routine immunization of infants, who are at greatest risk, offers the only practical way of reducing the incidence of this disease. Widespread use of the recently developed protein conjugate vaccines against Haemophilus influenzae type b by itself could reduce the incidence of bacterial meningitis in the U.S. by more than half. To prevent disease caused by the other pathogens, an effective vaccine against the group B meningococcus must be developed, and the immunogenicity of the pneumococcal and quadrivalent meningococcal vaccines should be improved. Until such time that universal immunization of infants with highly immunogenic vaccines is possible, continued efforts must focus on targeting immunization at high-risk individuals and using chemoprophylaxis to prevent secondary disease where indicated. Addendum: On October 4, 1990, the U.S. Food and Drug Administration licensed the praxis Haemophilus influenzae type b-protein conjugate vaccine (Hboc) for use in infants at 2, 4, and 6 months of age with a booster dose at 15-18 months. Physicians are directed to statements by the Immunizations Practices Advisory Committee and the American Academy of Pediatrics for official recommendations concerning its use.

Abstract:ABSTRACTThis study aims to describe and analyze the implications of the neoliberal model in the dynamics of hospital work, in the view of the nursing workers. It is a qualitative, descriptive and exploratory research. The scenario is a teaching hospital, and the subjects are 17 nursing workers. The collection took place in July and August 2011, through a semi-structured interview. The data treatment technique was the content analysis. The results show that neoliberalism in hospital nursing work entails several negative impacts, which focus on the inappropriate distribution of material resources, as well as the heavy use of technologies in the process of care. There is also a high turnover of workers contracted due to low wages and precarious employment conditions. We conclude that the neoliberal model has both negative and positive effects. It is considered important to approach this subject in training courses of future nursing staff.
