Abstract:Both major depressive disorder (MDD) and childhood trauma have been linked with brain structural changes. As childhood trauma is more highly prevalent in MDD patients, previous morphometric findings in MDD therefore might have been confounded by childhood trauma. This study aimed to differentiate the impact of childhood trauma from the influence of MDD diagnosis on gray matter volume (GMV). Seventy-eight subjects were recruited into four study groups (n = 16, MDD patients with childhood trauma exposures, CTE/MDD; n = 14, MDD patients without CTE, non-CTE/MDD; n = 24, healthy controls with CTE, CTE/HC; and n = 24, HCs without CTE, non-CTE/HC). All participants underwent high-resolution structural magnetic resonance scans. Voxel-based morphometry was used to investigate GM alterations, and a 2 × 2 analysis of variance was performed to identify the main effects of diagnosis, childhood trauma, and their interactions. The main effects of diagnosis displayed abnormal GMV located in the left superior parietal lobule (MDD < HC) and right middle occipital gyrus (MDD > HC). While the left dorsolateral prefrontal cortex (DLPFC) volume revealed a significant main effect of childhood trauma, as shown by decreased GMV of the left DLPFC in subjects with CTE, regardless of diagnosis. A negative correlation was also found between the left DLPFC volume and emotional neglect in individuals reporting CTE. The present findings suggest that decreased GMV of the left DLPFC is a function of childhood trauma rather than MDD, which may represent the biological risk for developing MDD.

Abstract:Childhood trauma (CT) is a major risk factor for psychiatric disorders. Emotional and cognitive functions are often affected in many psychiatric conditions, and these functions are mediated by the limbic system. However, previous research has primarily focused on patient populations. Therefore, we aim to examine the impact of CT on the limbic brain structure in healthy individuals. We enrolled 48 individuals in health, evenly split into two groups: 24 healthy participants with CT (HP-CT) and 24 healthy participants without CT (HP-nCT). They underwent scale assessments and MRI data acquisition. Comparisons between the two groups were performed after subcortical subregion volume segmentation using FreeSufer. Lastly, we examined correlations between volume changes and scale scores. We found that HP-CT group had smaller volumes in several subregions of the hippocampus, amygdala, and cortical limbic structures, including the subiculum (Sub) head and body, cornu ammonis (CA)1 head, molecular layer (ML) head, granule cell layer of the dentate gyrus (GC-ML-DG) body, CA4 body, fimbria, hippocampus-amygdala transition area (HATA), whole hippocampus head and body, whole hippocampus, basal nucleus (Ba), accessory basal nucleus (AB), cortico-amygdaloid transition area (CAT), paralaminar nucleus (PL) of the left hemisphere; and hippocampal tail, presubiculum (PreSub) body, and basal forebrain of the right hemisphere. Volume changes in the CA4 body and GC-ML-DG body were correlated with sexual abuse. Changes in the volume of the right basal forebrain were linked to emotional neglect. However, these findings were not significant after correction for multiple comparisons. CT impacts multiple structures of the limbic system, including the hippocampus, and amygdala. This also suggests that region-specific changes within the limbic system can serve as clinical biomarkers supporting cross-diagnostic psychiatric illnesses.
Altered Spontaneous Neuronal Activity of Visual Cortex and Medial Anterior Cingulate Cortex in Treatment-Naïve Posttraumatic Stress Disorder.

Abstract:Background: Although no more traumatic stimuli exists, a variety of symptoms are persisting in chronic Posttraumatic Stress Disorder (PTSD) patients. It is therefore necessary to explore the spontaneous brain activity of treatment-naive PTSD patients during resting-state.Method: Seventeen treatment-naive PTSD patients and twenty traumatized controls were recruited and underwent a resting functional magnetic resonance imaging (Rs-fMRI) scan. The differences of regional brain spontaneous activity between the participants with and without PTSD were measured by Amplitude of Low-frequency fluctuation (ALFF). The relationship between the altered brain measurements and the symptoms of PTSD were analyzed.Result: Compared to traumatized controls, the PTSD group showed significantly altered ALFF in many emotion-related brain regions, such as the medial anterior cingulate cortex (MACC), dorsolateral prefrontal cortex (DLPFC), insular (IC), middle temporal gyms (MTG), and ventral posterior cingulate cortex (VPCC). Interestingly this is the first report of a hyperactive visual cortex (V1/V2) during resting-state in treatment-naive PTSD patients. There were significant positive correlations between ALFF values in the bilateral visual cortex and re-experiencing or avoidance in PTSD. Negative correlation was observed between ALFF values in MACC and avoidance.Conclusion: This study suggested that the visual cortex and the MACC may be involved in the characteristic symptoms of chronic PTSD, such as re-experiencing and avoidance. Future studies that focus on these areas of the brain are required, as alteration of these areas may act as a biomarker and could be targeted in future treatments for PTSD. (C) 2014 Elsevier Inc. Abstract:Objective To investigate the possible changes of brain structural morphometry in patients with post-traumatic stress disorder (PTSD).Methods Twelve patients with PTSD caused by a fire disaster were involved, and 12 gender-, age-matched healthy persons who exposed the same fire disaster were selected as controls. Brain structural images including T1 and 3D images were gained by magnetic resonance imaging scan. Then structural images were dealt with voxel-based morphometry using the Statistical Parametric Mapping software (SPM2) to compare the differences in gray and white matter between the two groups.Results In comparison with controls, the PTSD patients had less gray-matter density in left hippocampus, left anterior cingulate cortex (Brodmann′s area 32), and bilateral insulars (Brodmann′s area 13) (P<0.001). The intensities in other gray matter regions and the white matter regions did not show any significant differences between two groups.Conclusion The results suggest that there may be morphometric changes in brain areas including left hippocampus, left anterior cingulate cortex and bilateral insulars in patients with PTSD.
Posttraumatic Stress Disorder and Depression Symptom Severities Are Differentially Associated With Hippocampal Subfield Volume Loss in Combat Veterans

Abstract:Background Two decades of human neuroimaging research have associated volume reductions in the hippocampus with posttraumatic stress disorder. However, little is known about the distribution of volume loss across hippocampal subfields. Recent advances in neuroimaging methods have made it possible to accurately delineate 10 gray matter hippocampal subfields. Here, we apply a volumetric analysis of hippocampal subfields to data from a group of combat-exposed Veterans. Method Veterans (total, n = 68, posttraumatic stress disorder, n = 36; combat control, n = 32) completed high-resolution structural magnetic resonance imaging. Based on previously validated methods, hippocampal subfield volume measurements were conducted using FreeSurfer 6.0. The Clinician-Administered PTSD Scale assessed posttraumatic stress disorder symptom severity; Beck Depression Inventory assessed depressive symptom severity. Controlling for age and intracranial volume, partial correlation analysis examined the relationship between hippocampal subfields and symptom severity. Correction for multiple comparisons was performed using false discovery rate. Gender, intelligence, combat severity, comorbid anxiety, alcohol/substance use disorder, and medication status were investigated as potential confounds. Results In the whole sample, total hippocampal volume negatively correlated with Clinician-Administered PTSD Scale and Beck Depression Inventory scores. Of the 10 hippocampal subfields, Clinician-Administered PTSD Scale symptom severity negatively correlated with the hippocampus–amygdala transition area (HATA). Beck Depression Inventory scores negatively correlated with dentate gyrus, cornu ammonis 4 (CA4), HATA, CA2/3, molecular layer, and CA1. Follow-up analysis limited to the posttraumatic stress disorder group showed a negative correlation between Clinician-Administered PTSD Scale symptom severity and each of HATA, CA2/3, molecular layer, and CA4. Conclusion This study provides the first evidence relating posttraumatic stress disorder and depression symptoms to abnormalities in the HATA, an anterior hippocampal region highly connected to prefrontal-amygdala circuitry. Notably, dentate gyrus abnormalities were associated with depression severity but not posttraumatic stress disorder symptoms. Future confirmatory studies should determine the extent to which dentate gyrus volume can differentiate between posttraumatic stress disorder- and depression-related pathophysiology.

Abstract:The hippocampus and amygdala are important structures in the posttraumatic stress disorder (PTSD); however, the exact relationship between these structures and stress or PTSD remains unclear. Moreover, they consist of several functionally distinct subfields/subregions that may serve different roles in the neuropathophysiology of PTSD. Here we present a subregional profile of the hippocampus and amygdala in 145 survivors of a major earthquake and 56 non‐traumatized healthy controls (HCs). We found that the bilateral hippocampus and left amygdala were significantly smaller in survivors than in HCs, and there was no difference between survivors with (n = 69) and without PTSD (trauma‐exposed controls [TCs], n = 76). Analyses revealed similar results in most subfields/subregions, except that the right hippocampal body (in a head‐body‐tail segmentation scheme), right presubiculum, and left amygdala medial nuclei (Me) were significantly larger in PTSD patients than in TCs but smaller than in HCs. Larger hippocampal body were associated with the time since trauma in PTSD patients. The volume of the right cortical nucleus (Co) was negatively correlated with the severity of symptoms in the PTSD group but positively correlated with the same measurement in the TC group. This correlation between symptom severity and Co volume was significantly different between the PTSD and TCs. Together, we demonstrated that generalized smaller volumes in the hippocampus and amygdala were more likely to be trauma‐related than PTSD‐specific, and their subfields/subregions were distinctively affected. Notably, larger left Me, right hippocampal body and presubiculum were PTSD‐specific; these could be preexisting factors for PTSD or reflect rapid posttraumatic reshaping.

Anterior cingulate cortex grey matter volume abnormalities in adolescents with PTSD after childhood sexual abuse

Abstract:Adverse childhood experiences (ACE) substantially increase the risk of later psychiatric and somatic pathology. While neurobiological factors are likely to play a mediating role, specific insights are lacking. The scarce neuroimaging studies in traumatised pediatric populations have provided inconsistent results, potentially due to the inclusion of different types of trauma. To further improve our understanding of the neurobiology of pediatric psychotrauma, this study seeks to investigate abnormalities in grey matter volume (GMV) in a homogeneous group of adolescents with posttraumatic stress disorder (PTSD) due to childhood sexual abuse (CSA) and the relationship between GMV and symptom severity. We performed a voxel based morphometry (VBM) analysis in 21 adolescents with CSA-related PTSD and 25 matched non-traumatised, non-clinical adolescents. Hippocampus, amygdala, anterior cingulate cortex (ACC), medial PFC (mPFC) and superior temporal gyrus (STG) were chosen as regions of interest (ROIs). Trauma symptomatology was measured with the Trauma Symptom Checklist for Children (TSCC) and dissociation symptoms with the Adolescent Dissociative Experiences Scale (A-DES). The ROI analysis showed that the CSA-related PTSD group had significant smaller volumes of the dorsal ACC as compared to healthy controls. However, no correlations were found between GMV and scores on the TSCC and A-DES. The smaller ACC volume is partly in line with previous studies in traumatised youth and is a consistent finding in traumatised adults. Taken together our results suggest that the dorsal ACC is implicated in the neurobiological sequelae of CSA, potentially associated with an altered evaluative processing of emotion, but not directly with PTSD severity.

Abstract:Posttraumatic stress disorder (PTSD) is a heterogeneous disorder, and symptom severity varies over time. Neurobiological factors that predict PTSD symptoms and their chronicity remain unclear. This study investigated whether the volume of the hippocampus and its subfields, particularly cornu ammonis (CA) 1, CA3, and dentate gyrus, are associated with current PTSD symptoms and whether they predict PTSD symptom changes over 2 years. We examined clinical and structural magnetic resonance imaging measures from 252 trauma-exposed post-9/11 veterans (159 with Time 1 PTSD diagnosis) during assessments approximately 2 years apart. Automated hippocampal subfield segmentation was performed with FreeSurfer Version 7.1, producing 19 bilateral subfields. PTSD symptoms were measured at each assessment using the Clinician-Administered PTSD Scale–IV (CAPS). All models included total intracranial volume as a covariate. First, similar to previous reports, we showed that smaller overall hippocampal volume was associated with greater PTSD symptom severity at Time 1. Notably, when examining regions of interest (CA1, CA3, dentate gyrus), we found that smaller Time 1 hippocampal volumes in the bilateral CA1-body and CA2/3-body predicted decreased PTSD symptom severity at Time 2. These findings were not accounted for by combat exposure or treatment history. Additionally, both Time 1 CA1-body and CA2/3-body volume showed unique associations with changes in avoidance/numbing, but not with changes in reexperiencing or hyperarousal symptoms. This supports a more complex and nuanced relationship between hippocampal structure and PTSD symptoms, where during the posttrauma years bigger may not always mean better, and suggests that the CA1-body and CA2/3-body are important factors in the maintenance of PTSD symptoms. Abstract:The concept of acute stress disorder (ASD) was introduced as a diagnostic entity to improve the identification of traumatized people who are likely to develop posttraumatic stress disorder (PTSD). Neuroanatomical models suggest that changes in the prefrontal cortex, amygdala, and hippocampus play a role in the development of PTSD. Using voxel-based morphometry, this study aimed to investigate the predictive power of gray matter volume (GMV) alterations for developing PTSD. The GMVs of ASD patients (n = 21) were compared to those of PTSD patients (n = 17) and healthy controls (n = 18) in whole-brain and region-of-interest analyses. The GMV alterations seen in ASD patients shortly after the traumatic event (T1) were also correlated with PTSD symptom severity and symptom clusters 4 weeks later (T2). Compared with healthy controls, the ASD patients had significantly reduced GMV in the left visual cortex shortly after the traumatic event (T1) and in the left occipital and prefrontal regions 4 weeks later (T2); no significant differences in GMV were seen between the ASD and PTSD patients. Furthermore, a significant negative association was found between the GMV reduction in the left lateral temporal regions seen after the traumatic event (T1) and PTSD hyperarousal symptoms 4 weeks later (T2). Neither amygdala nor hippocampus alterations were predictive for the development of PTSD. These data suggest that gray matter deficiencies in the left hemispheric occipital and temporal regions in ASD patients may predict a liability for developing PTSD.

Abstract:Different subterritories of anterior cingulate cortex (ACC) and adjacent ventromedial frontal cortex have been shown to serve distinct functions. This scheme has influenced contemporary pathophysiologic models of psychiatric disorders. Prevailing neurocircuitry models of post-traumatic stress disorder (PTSD) implicate dysfunction within pregenual ACC and subcallosal cortex (SC), as well as amygdala and hippocampus. In the current study, cortical parcellation of magnetic resonance imaging data was performed to test for volumetric differences in pregenual ACC and SC, between women with PTSD and trauma-exposed women without PTSD. The PTSD group exhibited selectively decreased pregenual ACC and SC volumes. These results are consistent with contemporary schemes regarding functional and structural dissection of frontal cortex, and suggest specific regional cortical pathology in PTSD.

Abstract:To the Editor: By controlling multiple confounding factors, the important article of Qureshi and colleagues implies that a causal link exists between posttraumatic stress disorder (PTSD) and late-life dementia.1,2 This letter reviews the authors' response to it and explores possible cerebral functional and structural evidence to support a relationship between the two diseases. Establishing the relationship between PTSD and dementia heavily relies on cerebral functional abnormalities and cerebral atrophy. It would provide an improvement in clinic decision-making, such as early prevention, better prediction of progress, and prognosis in dementia. However, mainly based on a twin study,3 a recent opinion claimed that no causal relationship seemed to exist between PTSD and dementia.4 Because the opinion and the article of Qureshi and colleagues ignore the PTSD-induced functional abnormalities that are commonly accepted and some debate still exists regarding the structural studies, it is important to separate the two abnormalities and provide some critical opinions and experiments related to the twin study to reveal the underlying link between the two diseases. Much previous research has found robust evidence for PTSD-induced functional abnormalities, including hyperresponsive amygdala, hypoactive medial prefrontal cortex (MPFC), and anterior cingulate cortex (ACC) dysfunction.5,6 Emotional Stroop research on PTSD first highlighted diminished medial prefrontal and anterior cingulate activation, which neuroimaging research on patients with PTSD exposed to trauma-related pictures, sounds, or traumatic imagery scripts further supported. Because abnormalities of these areas can predict the onset and progress of dementia, these studies suggest that a link exists between the two diseases. Poorer functional activity is not always associated with lower volume that is closely related to dementia, so more evidence from structural imaging studies is needed, but some debate still exists on PTSD-induced neurotoxicity, and the limitations of these experiments are obvious. For example, genetic studies require large sample sizes to detect small to medium gene effects, but subject numbers in twin neuroimaging studies are always small.7 Moreover, the present main drawback of voxel-based morphometry analysis is the potential poor sensitivity to detect regional changes. A great number of animal studies strongly support the hypothesis that stress can decrease dendritic branching and neurogenesis in MPFC volume and damage hippocampal formation.8 MPFC and pregenual ACC are highly interconnected with the amygdala, and structural or functional abnormalities in these regions have been reported to markedly impair fear extinction in human. Some researchers emphasize that not only has research in humans found that ACC volumes in PTSD are smaller than in trauma-exposed individuals without PTSD, but has also found that ACC volume correlates with severity of PTSD symptoms.5 One of the most important types of genetic neuroimaging studies in human is twin studies. One twin study revealed that combat veterans with PTSD and their identical twins who did not have PTSD had smaller hippocampus than combat veterans without PTSD, suggesting that a small hippocampus is a risk factor for rather than a result of PTSD.2 Nevertheless, some scientists criticized the twin study and called for a replication of this research. The same group reported inconsistent results in different papers and the reason is unclear. In addition, a small hippocampus was not an extraordinarily strong predictor for the enormous overlap in the volume data. They believed that hippocampus volume and stress could affect each other.9 In contrast to the previous twin study, another twin study emphasized that there was no evidence of regional differences related to genetic risk for PTSD and that most regional changes correlated with PTSD. Meanwhile, it suggested that pregenual ACC is also significantly associated with PTSD and combat exposure.10 In conclusion, correlation, even post hoc, does not equal causation, but a large number of consistent studies indicate that causation is likely to exist between the two diseases. Further genetic neuroimaging studies and pathological research on the frontal amygdala circuit and hippocampus shall contribute to the causation. Abstract:Previous neuroimaging studies suggest that childhood maltreatment (CM) can affect brain function and structure and constitutes a potent risk of developing depression. The present study attempts to differentiate the effect of CM from the impact of depression diagnosis on brain structure. Eighty-four patients with first-episode depression and 84 controls participated in this study. All subjects underwent a structural magnetic resonance imaging (MRI) scan. Voxel-based morphometric analysis of variance (ANOVA), between-group comparison, and regression analyses were performed on a whole-brain level to identify the unbiased effects of CM and depression on brain structure. Our data showed different main effects of CM and depression on brain structure separately: the significant main effect of CM on fronto-limbic areas, visual cortex and cerebellum, and the main effect of depression on orbitofrontal regions and bilateral temporal lobes. The maltreatment-by-diagnosis interaction effect was found located on the bilateral prefrontal cortex. The CM severity was negatively related with the inferior occipital volume and positively with volumes in the left dorsolateral prefrontal cortex, right caudate, and left middle temporal gyrus. This study suggested that early life stress like CM is associated with structural abnormalities of the fronto-limbic regions that are commonly regarded as the psychopathological consequence of depression. The effect of CM on brain structure should be considered in future neuroimaging research of depression.

Abstract:The vast majority of individuals experience trauma within their lifetime. Yet, most people do not go on to develop clinical levels of psychopathology. Recently, studies have highlighted the potential protective effects of having larger amygdala and hippocampal volumes, such that larger volumes may promote adaptive functioning following trauma. However, research has not yet elucidated whether certain subregions of these stress-sensitive structures have specific protective effects. Herein, we examined the mediating effects of amygdala and hippocampal subregions on the relationship between traumatic exposure and concurrent or longitudinal changes in psychiatric symptom levels in typically developing youth (9-15 years of age). Using high-resolution T1-and T2-weighted structural MRI scans, we found that the volume of the right basolateral complex of the amygdala mediated associations between trauma exposure and internalizing symptoms. Specifically, greater levels of childhood trauma related to larger volumes, and larger volumes were associated with fewer internalizing symptoms. The volume of the right CA4/dentate gyrus (DG) of the hippocampus yielded similar mediation results, such that greater trauma was related to larger volumes, which in turn were associated with decreases in internalizing symptoms across time. These findings provide initial support for potentially protective effects of larger right amygdala and hippocampal subregion volumes against internalizing symptomology concurrently and longitudinally during adolescence.

Abstract:Nitric oxide (NO) has been proposed as a mediator of postnatal mammary gland development. We investigated the immunohistochemical localization of three isoforms of nitric oxide synthase (NOS) enzymes, neuronal NOS (nNOS), inducible NOS (iNOS) and endothelial NOS (eNOS), in the mouse mammary gland during reproductive cycle. The NOS isoforms detected in normal mouse mammary glands were the constitutive forms of NOS (nNOS and eNOS). nNOS was localized to the alveoli, myoepithelia, lactiferous ducts and blood vessel endothelia, while eNOS was localized in the alveoli, lactiferous ducts and blood vessel endothelia. The strongest immunoreactivity for both constitutive NOS isoforms was observed in pregnant mice. The differential staining intensity of NOS enzymes in the mammary gland led us to conclude that nitric oxide in the mouse mammary gland is mainly synthesized by constitutive NOS isoforms, and suggest that NO has functional roles in post-pubertal growth and differentiation of the mammary gland.

Fibrobroncho-scopy is help only in endobronchial forms; they don't reach 10% of cases. Computed Tomography and transparietal biopsy can be diagnostic. In the uncertain cases and in presence of symptoms, thoracotomy with exeresis of tumor is justified.
Increased Inhibition of the Amygdala by the Mpfc May Reflect a Resilience Factor in Post-traumatic Stress Disorder: A Resting-State Fmri Granger Causality Analysis

Abstract:Purpose: To determine whether effective connectivity of the amygdala is altered in traumatized subjects with and without post-traumatic stress disorder (PTSD).Materials and Methods: Resting-state functional MRI data were obtained for 27 patients with typhoon-related PTSD, 33 trauma-exposed controls (TEC), and 30 healthy controls (HC). Effective connectivity of the bilateral amygdala was examined with Granger causality analysis and then compared between groups by conducting an analysis of variance.Results: Compared to the HC group, both the PTSD group and the TEC group showed increased effective connectivity from the amygdala to the medial prefrontal cortex (mPFC). The TEC group showed increased effective connectivity from the mPFC to the amygdala relative to the HC group. Compared to the TEC group, the PTSD group showed increased effective connectivity from the amygdala to the supplementary motor area (SMA), whereas decreased effective connectivity was detected from the SMA to the amygdala. Both the PTSD group and the TEC group showed decreased effective connectivity from the superior temporal gyrus (STG) to the amygdala relative to the HC group. Compared to the HC group, the TEC group showed increased effective connectivity from the amygdala to the dorsolateral prefrontal cortex (dlPFC), while both the PTSD group and the TEC group showed decreased effective connectivity from the dlPFC to the amygdala. The PTSD group showed decreased effective connectivity from the precuneus to the amygdala relative to both control groups, but increased effective connectivity from the amygdala to the precuneus relative to the HC group.Conclusion: Trauma leads to an increased down-top excitation from the amygdala to the mPFC and less regulation of the amygdala by the dlPFC. The results suggest that increased inhibition of the amygdala by the mPFC may reflect a resilience factor, and altered amygdala-SMA and amygdala-STG effective connectivity may reflect compensatory mechanisms of brain function. These data raise the possibility that insufficient inhibition of the amygdala by the mPFC might lead to PTSD in those who have been exposed to traumatic incidents, and may inform future therapeutic interventions.

Abstract:Previous studies have revealed morphologic alterations in patients with major depressive disorder (MDD) with experiences of childhood trauma. However, the underlying neural mechanisms remain largely unknown. This study aims to explore the brain structural changes and their possible mediation role in the relationship between childhood maltreatment and psychological resilience in drug-naïve adolescents with first-episode MDD. A total of 57 adolescents with first-episode MDD and 36 healthy controls (HCs) completed the T1-weighted magnetic resonance imaging scan. The adverse childhood experiences and current psychological resilience were assessed using the Childhood Trauma Questionnaire-Short Form and the Connor Davidson Resilience Scale, respectively. The voxel-based morphometry approach was applied to examine changes in the gray matter volume (GMV). Compared with the HCs, adolescents with MDD had significantly reduced GMV volumes in the left fusiform gyrus, right orbitofrontal gyrus, right superior temporal gyrus, right calcarine cortex, right middle frontal gyrus, left angular gyrus, right precuneus, right posterior cingulate gyrus, and right posterior central gyrus, as well as significantly increased GMV volumes in the left lenticular putamen and right lenticular pallidum. The GMV of the right calcarine cortex was found to be negatively correlated with the severity of emotional abuse and positively correlated with the level of psychological resilience. Moreover, the GMV of the right calcarine cortex might partially mediate the relationship between childhood maltreatment and psychological resilience. The present study provided further evidence for structural impairments in adolescents with MDD. Our findings also confirmed the important role of depression-related GMV changes in childhood growth experiences and psychological resilience characteristics during adolescent brain maturation.

Abstract:Background: As mood dysregulation and hyperarousal are overlapping and prominent features of posttraumatic stress disorder (PTSD), and mood disorders (MD) including bipolar disorder (BD), we aimed to clarify the role of trauma and MD on the resting state functional connectivity (RSFC) of amygdala in MD youth with or without trauma exposure, and healthy controls (HC).Methods: Of 23 subjects, 21 completed the magnetic resonance imaging (MRI) protocol, 5 were excluded for subject motion, leaving final sample size of 16: nine subjects with MD (5/9 with trauma), and 7 HC. Youth were assessed with Schedule for Affective Disorders and Schizophrenia for School Aged Children—Present and Lifetime Version (K-SADS-PL), and other behavioral measures including Young Mania Rating Scale (YMRS). Imaging data were acquired using functional MRI in 3-T scanner. Imaging included T1-weighted structural MRI and 6-min resting state acquisition.Results: In between group analysis, the average correlation coefficients between left anterior cingulate cortex (Acc) and left insula cortex with left amygdala regions were significantly larger in HC compared to the patient population. Connectivity between left amygdala and left cingulate cortex shows a significant negative correlation with YMRS severity.Conclusions: In this preliminary study, MD with trauma youth had more manic symptoms and difficulties regulating anger. While MD youth showed reduced RSFC of left amygdala with left acc and left insula, no significant difference between the subgroups of children with MD was observed. However, when looking at both clinical groups together, we observed a significant correlation of RSFC of left amygdala to left acc, and YMRS scores.

Abstract:Studies of posttraumatic stress disorder (PTSD) report volume abnormalities in multiple regions of the cerebral cortex. However, findings for many regions, particularly regions outside commonly studied emotion-related prefrontal, insular, and limbic regions, are inconsistent and tentative. Also, few studies address the possibility that PTSD abnormalities may be confounded by comorbid depression. A mega-analysis investigating all cortical regions in a large sample of PTSD and control subjects can potentially provide new insight into these issues. Given this perspective, our group aggregated regional volumes data of 68 cortical regions across both hemispheres from 1379 PTSD patients to 2192 controls without PTSD after data were processed by 32 international laboratories using ENIGMA standardized procedures. We examined whether regional cortical volumes were different in PTSD vs. controls, were associated with posttraumatic stress symptom (PTSS) severity, or were affected by comorbid depression. Volumes of left and right lateral orbitofrontal gyri (LOFG), left superior temporal gyrus, and right insular, lingual and superior parietal gyri were significantly smaller, on average, in PTSD patients than controls (standardized coefficients = −0.111 to −0.068, FDR corrected P values < 0.039) and were significantly negatively correlated with PTSS severity. After adjusting for depression symptoms, the PTSD findings in left and right LOFG remained significant. These findings indicate that cortical volumes in PTSD patients are smaller in prefrontal regulatory regions, as well as in broader emotion and sensory processing cortical regions.
