Abstract:Background: Disrupted metacognition is implicated in development and maintenance of negative symptoms, but more fine-grained analyses would inform precise treatment targeting for individual negative symptoms. Aims: This systematic review identifies and examines datasets that test whether specific metacognitive capacities distinctly influence negative symptoms. Materials & methods: PsycINFO, EMBASE, Medline and Cochrane Library databases plus hand searching of relevant articles, journals and grey literature identified quantitative research investigating negative symptoms and metacognition in adults aged 16+ with psychosis. Authors of included articles were contacted to identify unique datasets and missing information. Data were extracted for a risk of bias assessment using the Quality in Prognostic Studies tool. Results: 85 published reports met criteria and are estimated to reflect 32 distinct datasets and 1623 unique participants. The data indicated uncertainty about the relationship between summed scores of negative symptoms and domains of metacognition, with significant findings indicating correlation coefficients from 0.88 to -0.23. Only eight studies investigated the relationship between metacognition and individual negative symptoms, with mixed findings. Studies were mostly moderate-to-low risk of bias. Discussion: The relationship between negative symptoms and metacognition is rarely the focus of studies reviewed here, and negative symptom scores are often summed. This approach may obscure relationships between metacognitive domains and individual negative symptoms which may be important for understanding how negative symptoms are developed and maintained. Conlclusion: Methodological challenges around overlapping participants, variation in aggregation of negative symptom items and types of analyses used, make a strong case for use of Individual Participant Data Meta-Analysis to further elucidate these relationships.
On Metacognition: Overconfidence in Word Recall Prediction and Its Association with Psychotic Symptoms in Patients with Schizophrenia

Abstract:A two-factor account has been proposed as an explanatory model for the formation and maintenance of delusions. The first factor refers to a neurocognitive process leading to a significant change in subjective experience; the second factor has been regarded as a failure in hypothesis evaluation characterized by an impairment in metacognitive ability. This study was focused on the assessment of metacognition in patients with schizophrenia. The aims of the study were to measure the overconfidence in metacognitive judgments through the prediction of word list recall and to analyze the correlation between basic neurocognition (memory and executive function) and metacognition through a metamemory test and the severity of psychotic symptoms. Method: Fifty-one participants with a diagnosis of schizophrenia were evaluated. The Positive and Negative Syndrome Scale (PANSS) was used to assess the severity of psychiatric symptoms, and the subtest of metamemory included in the Executive Functions and Frontal Lobe-2 battery (BANFE-2) was used to evaluate overconfidence and underestimation errors, intrusion and perseverative response, total volume of recall, and Brief Functioning Assessment Scale (FAST) for social functioning. Results: The strongest correlation is observed between overconfidence errors and the positive factor of the PANSS (r = 0.774, p < 0.001). For the enter model in the multiple linear regression (r = 0.78, r2 = 0.61; F = 24.57, p < 0.001), the only significant predictor was overconfidence errors. Conclusion: Our results highlight the relevance of a metacognitive bias of overconfidence, strongly correlated with psychotic symptoms, and support the hypothesis that metacognitive defects contribute to the failure to reject contradictory evidence. From our perspective, these findings align with current mechanistic models of schizophrenia that focus on the role of the prefrontal cortex.

Abstract:Abstract Background Healthy metacognition involves several capacities, including the ability to integrate information about the self and others in order to formulate ways of coping with social challenges and psychological distress. Multiple studies have demonstrated that reduced general metacognitive capacity is predictive of the development and persistence of overall negative symptom burden. However, there have been no published analyses investigating how specific sub-components of metacognition influence the expression of individual negative symptoms. We aggregated individual participant data from studies reporting measures of subtypes of metacognitive functioning and examined the strength of association with specific negative symptoms. Methods PsycINFO, EMBASE, MEDLINE, Cochrane Library and grey literature databases were searched for eligible studies. Forwards and backwards citation searching and contacting of study authors revealed additional datasets not identified in the original search. Included studies assessed negative symptoms and metacognition using interview-based measures in participants aged 16 years or older. Selection was restricted to quantitative research, excluding case studies, and only English language publications were screened. Experimental and observational studies were screened sequentially at title, abstract and full-text level to determine whether they met search criteria. A second reviewer independently screened a proportion of records to check the reliability of inclusion/exclusion judgements (Cohen’s Kappa = 0.74). Participant data and metadata of included studies were extracted and compiled combining original author and report information for all pre-specified outcomes where available. The proposed plan for the systematic review and meta-analyses was also pre-registered on PROPSERO (CRD42019130678). Results 97 unique reports were identified, of which 30 included negative symptom specific hypotheses. Samples overlapped substantially across publications with these 97 reports corresponding to 30 unique datasets. The raw individual participant level data for 23 of the 30 unique datasets was obtained. Preliminary analyses investigated the relationship between components of metacognition measured with the MAS-A (Lysaker et al., 2005), and the original negative symptoms subscale score of the PANSS (Kay et al., 1987) to maximise available data. We will discuss the results, which suggest that there are distinct relationships between subscales of metacognition and negative symptoms. We will also discuss the limitations of these results including a limited scope for analysing covariates due to the computational complexity of the models used, and difficulties in handling the diversity of data present in the meta-analysis. We will also discuss why high heterogeneity might be present, and provide further support for analysis investigating the relationship between individual negative symptoms and components of metacognition. Discussion The data suggest there is complexity in the relationship between components of metacognition and individual negative symptoms. It is for subsequent analyses to determine whether individual negative symptoms have distinct relationships with each metacognitive capacity, and whether the variation in the strength of these associations could explain the high heterogeneity observed.

Abstract:Introduction: In this study we assessed the contribution of psychopathology, including the two domains of negative symptoms (motivational deficit and expressive deficit), processing speed as an index of neurocognition, and emotion recognition, as an index of social cognition, to poor functional outcomes in people with schizophrenia. Methods: The Positive and Negative Syndrome Scale was used to evaluate positive symptoms and disorganization and the Brief Negative Symptom Scale to assess negative symptoms. The Symbol Coding and the Trail Making Test A and B were used to rate processing speed and the Facial Emotion Identification Test to assess emotion recognition. Functional outcome was assessed with the Personal and Social Performance Scale (PSP). Regression analyses were performed to identify predictors of functional outcome. Mediation analyses was used to investigate whether social cognition and negative symptom domains fully or partially mediated the impact of processing speed on functional outcome. Results: One hundred and fifty subjects from 8 different European centers were recruited. Our data showed that the expressive deficit predicted global functioning and together with motivational deficit fully mediated the effects of neurocognition on it. Motivational deficit was a predictor of personal and social functioning and fully mediated neurocognitive impairment effects on the same outcome. Both motivational deficit and neurocognitive impairment predicted socially useful activities, and the emotion recognition domain of social cognition partially mediated the impact of neurocognitive deficits on this outcome. Conclusions: Our results indicate that pathways to functional outcomes are specific for different domains of real-life functioning and that negative symptoms and social cognition mediate the impact of neurocognitive deficits on different domains of functioning. Our results suggest that both negative symptoms and social cognition should be targeted by psychosocial interventions to enhance the functional impact of neurocognitive remediation.

Abstract:CD1d molecules are MHC class I-like molecules that present lipid Ags to NKT cells. Although we have previously shown that several different cell signaling molecules can play a role in the control of Ag presentation by CD1d, a defined mechanism by which a cell signaling pathway regulates CD1d function has been unclear. In the current study, we have found that the Rho kinases, Rho-associated, coiled-coil containing protein kinase (ROCK)1 and ROCK2, negatively regulate both human and mouse CD1d-mediated Ag presentation. Inhibition of ROCK pharmacologically, through specific ROCK1 and ROCK2 short hairpin RNA, or by using dendritic cells generated from ROCK1-deficient mice all resulted in enhanced CD1d-mediated Ag presentation compared with controls. ROCK regulates the actin cytoskeleton by phosphorylating LIM kinase, which, in turn, phosphorylates cofilin, prohibiting actin fiber depolymerization. Treatment of APCs with the actin filament depolymerizing agent, cytochalasin D, as well as knockdown of LIM kinase by short hairpin RNA, resulted in enhanced Ag presentation to NKT cells by CD1d, consistent with our ROCK inhibition data. Therefore, our overall results reveal a model whereby CD1d-mediated Ag presentation is negatively regulated by ROCK via its effects on the actin cytoskeleton.
Negative global metacognitive biases are associated with depressive and posttraumatic stress disorder symptoms and improve with targeted or game-based cognitive training.

Abstract:Objective: Metacognition is disrupted in several clinical populations. One aspect of metacognition, global metacognitive bias (difference between objective and self-reported abilities), has shown to be particularly relevant to clinical functioning. However, previous studies of global metacognitive biases in populations with elevated depressive/posttraumatic stress disorder (PTSD) symptoms have not measured objective and self-reported abilities relative to normative samples, limiting the quantification of biases. Additionally, few studies have examined whether cognitive interventions can improve metacognitive biases or how this relates to depressive/PTSD symptom severity. Method: A total of 84 participants with mild traumatic brain injury (77% veterans) performed PTSD and depression assessments along with self-reported and objective measures of global cognition. Age-adjusted norm-based z scores were used for self-reported and objective cognition, and bias was calculated by subtracting objective minus self-report scores. Participants then received 13 weeks of targeted cognitive training or entertainment games training (both providing performance feedback). Participants were assessed at baseline, immediately posttraining, and 3 months posttraining. Results: We found large negative metacognitive biases in those with clinically significant severity of depressive symptoms (z score difference = −1.77), PTSD symptoms (−1.47), and depressive + PTSD symptoms (−2.29). Metacognitive biases improved after both targeted and entertainment training and was associated with reductions in depressive/PTSD symptom severity ( r = −.41/−.42, respectively), led by the entertainment training group ( r = −.54/−.46, respectively). Conclusions: These findings show that clinically significant severity of depressive/PTSD symptoms is associated with substantial negative global metacognitive biases and preliminarily suggests that cognitive training may improve these biases and depressive/PTSD symptom severity. Abstract:<h3 class="article-section__sub-title section1"> Aim</h3><p>In the clinical high‐risk (CHR) state for psychosis, both negative symptoms and lower cognitive function have been associated with poorer daily functioning. Recent evidence suggests that negative symptoms share overlapping variability with cognition and may partially mediate the relationship between cognition and functioning. However, the nature of this overlap is unknown, and the reverse mediation model remains untested leaving the precise nature of these associations unclear.</p><h3 class="article-section__sub-title section1"> Methods</h3><p>In order to clarify these relationships, a sample of community‐dwelling youth meeting CHR criteria was collected from a specialty CHR clinic (n = 91, mean age = 21, 63% male). Bootstrapping methods were then applied in a mediation analysis to test both negative symptoms and cognition as independent variables and mediating variables predicting social and role functioning in CHR individuals. Canonical correlation analysis was used to characterize the overlapping variability between negative symptoms and cognition.</p><h3 class="article-section__sub-title section1"> Results</h3><p>Support for a primary role of negative symptoms in predicting functioning and cognition was observed. Canonical correlation revealed a single dimension of overlap between the two symptom types (<i>r</i> = .62), represented by a strong correlation between negative symptoms in general and tasks involving verbal working memory, vigilance and social cognition specifically. A single cognitive factor composed primarily of these tasks was found to predict role functioning (adjusted <i>R</i><sup><i>2</i></sup> = .04). </p><h3 class="article-section__sub-title section1"> Conclusions</h3><p>The results highlight the importance of considering specific cognitive mechanisms overlapping with negative symptoms in research and rehabilitative practice in CHR populations, as well as the primary importance of targeting negative symptoms.</p>

Abstract:BACKGROUND: Negative symptoms are a significant barrier to function and may have a range of etiological roots and links to outcome. A previous study identified a subgroup of patients with schizophrenia who had both higher levels of negative symptoms and relatively poorer attentional function who had uniquely lower self-esteem and greater internalized stigma.AIMS: To determine whether participants previously classified as having High Negative/Poorer Attention would continue to have lower self-esteem, higher self-stigma, and also higher levels of anxiety and avoidant coping 5 months later.METHOD: Participants were 77 (77.8%) of the original 99 participants who completed follow-up procedures.RESULTS: The High Negative/Poorer Attention group had significantly poorer social functioning, lower appraisal of their competence, higher levels of anxiety, and a higher preference for ignoring stressors five months after classification.CONCLUSIONS: Negative symptoms with concomitant attention deficits may lead to more social and psychological dysfunction than negative symptoms or attention deficits alone. Individuals with both high levels of negative symptoms and poor attention may represent a meaningful subgroup with unique psychosocial difficulties that persist over time.

Abstract:I n the July 15 issue of Pediatric Blood & Cancer, MarecBerard et al. [1] reported the official, updated recommendations of the French National Federation of Cancer Centers regarding the use of erythropoietic stimulating agents (ESA) in children with cancer. These recommendations are updates of ones previously developed and published in 1999 and 2004. This is an important and timely statement given the recent FDA restrictions regarding approved use of these agents in adults in the US. The procedure used for this update started with a literature search for relevant publications regarding use of ESAs between November 2003 and February 2007. A multidisciplinary working group of experts critically reviewed the publications and draft guidelines were developed that included levels of evidence to support them, including expert opinion. This document was then peer-reviewed by independent experts, whose comments were incorporated into the final version of this document in December 2007. The recommendations were said to be based on evidence synthesis and ‘‘transparent’’ expert opinion. Four randomized controlled trials RCTs of ESA in children were identified [2–5]. These reports were evaluated for the number of patients studied, the follow-up period, the primary oncologic diagnoses, the baseline or study entry hemoglobin level, the dose and schedule of the ESA used, and the end points used to evaluate ESA effectiveness (hemoglobin changes, number of transfusions while on study drug, quality of life, toxicities, and survival). The authors concluded from evaluation of these trials, in combination with data analyzed previously from four earlier trials, that ESA use in anemic children with cancer: (1) promotes increased levels of hemoglobin; (2) reduces the need for blood transfusions; (3) does not appear to be associated with a significant risk of toxicity; (4) does not appear to improve the quality of life of such children; and (5) has no conclusive effect on overall survival. This led them to recommend that the systematic administration of ESAs is not recommended for the prevention or treatment of anemia in children with cancer and that the decision of whether to use an ESA must be made on a case-by-case basis, when there is a relative or absolute contraindication for transfusion. There are a number of weaknesses to this report, including: (1) some methods merely referenced or available only in French; (2) several published RCTs of ESA use in children with cancer that were apparently overlooked [6–10]; (3) inclusion of data from two reports that concern the same study (3,5); (4) a puzzling assignment of level of evidence for several of the published reports that are not in line with their own defined levels of evidence; and (5) a recommendation of the universal administration of ESAs intravenously (as apposed to subcutaneously) without any supporting data. Our initial response to some of their recommendations was one of surprise, and it stimulated us to review all the available data on this topic. In discussing their conclusions, we will group our comments according to some practical issues regarding ESA use in pediatric oncology: Is the hemoglobin level really consistently elevated with ESA use? Is the incidence of red cell transfusions truly reduced? Is the quality of life improved? Is the use of ESAs cost effective? Is there any evidence for increased tumor progression or relapse in children treated with ESAs?
Predictors of functioning in treatment-resistant schizophrenia: the role of negative symptoms and neurocognition

Abstract:Introduction: Predictors of functioning are well-studied in schizophrenia, but much less so in treatment-resistant schizophrenia (TRS). In this study, we aim to investigate contributions of schizophrenia symptom domains and neurocognition to predict functioning in a TRS population (n = 146). Methods: Participants were assessed on the Positive and Negative Syndrome Scale (PANSS), to calculate scores for five symptom factors (Positive, Negative, Cognitive, Depressive and Hostility) and two negative symptom constructs (Diminished Expressivity (DE), and Social Anhedonia (SA) as part of the Motivation and Pleasure-related dimension), based on a previously validated model, modified in accordance with EPA guidelines on negative symptoms assessment. Neurocognition was assessed with symbol coding and digit sequencing tasks from the Brief Assessment of Cognition in Schizophrenia (BACS). Functioning was assessed with the Social and Occupational Functioning Assessment Scale (SOFAS), employment status and World Health Organization Disability Assessment Schedule 2.0 (WHODAS 2.0). Multiple regression analyses were performed on psychopathology scores and BACS scores against all three measures of functioning, controlling for age and sex. For WHODAS, regression with PANSS scores of significant symptom factors were also performed. Results: A lower severity of negative symptoms in the SA dimension was the strongest predictor of higher functioning across all three functioning measures. Neurocognition, in particular processing speed and attention assessed on the symbol coding task, predicted employment. A lower severity of somatic concerns and depressive symptoms was associated with lesser self-reported disability on WHODAS. Discussion: This study represents a first attempt at elucidating significant predictors of functioning in TRS. We highlight negative symptoms and neurocognition as important treatment targets to improve functioning in TRS, consistent with previous studies in general schizophrenia.

Abstract:Dysfunctional metacognitive beliefs ("metacognitions") and executive control are important factors in mental disorders such as depression and anxiety, but the relationship between these concepts has not been studied systematically. We examined whether there is an association between metacognitions and executive control and hypothesized that decreased executive control statistically predicts increased levels of metacognitions. Two hundred and ninety-nine individuals recruited from the general population and outpatient psychiatric clinics completed the Metacognitions Questionnaire-30 and three subtests from the Cambridge Neuropsychological Test Automated Battery corresponding to the three-component model of executive functions. Controlling for current depression and anxiety symptoms, decreased ability to shift between mental sets was associated with increased negative beliefs about the uncontrollability and danger of worry and beliefs about the need to control thoughts. The results suggest a basic association between metacognitions and executive control. Individual differences in executive control could prove important in the personalization of metacognitive therapy.

Metacognitive training for patients with schizophrenia: Preliminary evidence for a targeted, single-module programme

Abstract:Objective: Metacognitive training is an eight-module, group-based treatment programme for people with schizophrenia that targets the cognitive biases (i.e. problematic thinking styles) thought to contribute to the genesis and maintenance of delusions. The present article is an investigation into the efficacy of a shorter, more targeted, single-module metacognitive training programme, administered individually, which focuses specifically on improving cognitive biases that are thought to be driven by a ‘hypersalience of evidence-hypothesis matches’ mechanism (e.g. jumping to conclusions, belief inflexibility, reasoning heuristics, illusions of control). It was hypothesised that a more targeted metacognitive training module could still improve performance on these bias tasks and reduce delusional ideation, while improving insight and quality of life. Method: A sample of 28 patients diagnosed with schizophrenia and mild delusions either participated in the hour-long, single-session, targeted metacognitive training programme ( n = 14), or continued treatment as usual ( n = 14). All patients were assessed using clinical measures gauging overall positive symptomology, delusional ideation, quality of life and insight, and completed two cognitive bias tasks designed to elucidate the representativeness and illusion of control biases. Results: After a 2-week, post-treatment interval, targeted metacognitive training patients exhibited significant decreases in delusional severity and conviction, significantly improved clinical insight, and significant improvements on the cognitive bias tasks, relative to the treatment-as-usual controls. Performance improvements on the cognitive bias tasks significantly correlated with the observed reductions in overall positive symptomology. Patients also evaluated the training positively. Conclusions: Although interpretations of these results are limited due to the lack of an optimally designed, randomised controlled trial and a small sample size, the results are promising and warrant further investigation into targeted versions of the metacognitive training programme.

Abstract:Background Metacognitive Training for Depression in older adults (MCT-Silver; www.uke.de/mct-silver ) is a cognitive-behavioral based group intervention that aims at reducing depression by targeting (meta)cognitive beliefs and rumination. In the present study, it was examined whether negative cognitive beliefs, positive metacognitive beliefs and/or rumination may be implicated as mediators of MCT-Silver’s effects on depression. Materials and methods We conducted a secondary analysis of a randomized controlled trial comparing MCT-Silver to an active control intervention (cognitive remediation) including 66 older adults (60 years and older) with complete baseline data. Clinician-rated (Hamilton Depression Rating Scale) and self-reported (Beck Depression Inventory-II) depression, negative cognitive beliefs (Dysfunctional Attitudes Scale-18B), positive metacognitive beliefs (positive beliefs subscale; Metacognition Questionnaire-30) and rumination (10-item Ruminative Response Scale) were assessed before (pre) and after 8 weeks of treatment (post), as well as 3 months later (follow-up). It was examined whether change in depression (pre- to follow-up) was mediated by change in negative cognitive beliefs, positive metacognitive beliefs and/or rumination (pre- to post-assessment). Results Mediation results differed for self-reported vs. clinician-rated depression. The effect of MCT-Silver on reduction in clinician-rated depression was mediated by a reduction in self-reported rumination, whereas reduction in self-reported depression was mediated by a reduction in negative cognitive beliefs. Positive metacognitive beliefs were not a significant mediator for either outcome. Conclusion The current study provides initial evidence for the roles of negative cognitive beliefs and rumination in the treatment of depression in later life with MCT-Silver. Given the divergence of findings and lack of causal precedence, mechanisms of change for MCT-Silver cannot yet be equivocally identified.

Elective orthopaedic surgery for inhibitor patients

Abstract:Summary. We report on a series of 108 elective orthopaedic surgical procedures. It includes 88 radiosynoviortheses and 20 major orthopaedic procedures, performed on 51 patients at nine centres worldwide. The average age of patients was 28.5 years (range 5–40 years), and the average follow‐up time was 2 years (range 1–5 years). There were 82 good results, 15 fair and 11 poor. In the synoviorthesis group (41 patients, 88 synoviortheses) the average age was 14.3 years (range 5–40 years) and the average follow‐up was 6.5 years (range 1–10 years). There were 66 good results, 14 fair and eight poor. There were no complications. In the group of major orthopaedic procedures, the average age of the 10 patients was 32.5 years (range 27–40 years), and the average follow‐up was 2.3 years (range 1–5 years). There were 16 good results, one fair and three poor. Postoperative bleeding complications occurred in three of the 20 major orthopaedic procedures performed (15% complications rate). They occurred in three patients treated with insufficient doses of recombinant activated factor VII. Despite such complications, the study has shown that haemophilic patients with inhibitors requiring elective orthopaedic surgery (EOS) can undergo such procedures with a high expectation of success. In other words, EOS is now possible in haemophilic patients with inhibitors, leading to an improved quality of life for these patients. Thorough analysis of each case as part of a multidisciplinary team will allow us to perform elective orthopaedic procedures in patients with inhibitors.

Abstract:Background: Metacognition is the ability to monitor and self-assess cognitive performance. It can be impaired in neurodegenerative diseases, with implications for daily function, and the ability of patients to reliably report their symptoms to health professionals. However, metacognition has not been systematically assessed in early-mid stage Parkinson's disease (PD) and REM sleep behavioral disorder (RBD), a prodrome of PD. Objectives: This study aimed to evaluate metacognitive accuracy and self-confidence in PD and RBD patients across various cognitive tasks. Methods: We conducted detailed computerized cognitive assessments with 19 cognitive tasks within an established PD and RBD cohort. Participants self-rated their performance post-task. Metacognitive accuracy was calculated by comparing these ratings against objective performance and further analyzed against clinical and mental health factors. Results: PD and RBD patients' metacognitive accuracy aligned with control subjects. However, they exhibited lower confidence across cognitive domains, reflecting their reduced cognitive performance. A notable inverse correlation was observed between their confidence and MDS-UPDRS I and II scales and HADS anxiety and depression scores. Conclusion: Our findings indicate that patients with early to mid-stage PD and RBD are generally aware of their cognitive status, differing from other neurological disorders. The inverse relationship between patient confidence and symptoms of depression, anxiety, and daily life challenges underscores the impact of emotional and functional difficulties on their self-perception of cognitive abilities. This insight could be significant for understanding how these conditions affect mental health, aiding clinicians in developing more effective patient care strategies.

Diagnosis of adenomyosis: a review.

Abstract:This article reviews the various preoperative diagnostic methods for uterine adenomyosis. MEDLINE and PubMed were searched using the keywords adenomyosis and adenomyosis diagnosis. Reviews, case-controlled studies and reports published from 1949 through March 2005 and written, or at least abstracted, in English were analyzed. Transvaginal sonography is superior to the transabdominal route, but its diagnostic sensitivity is still suboptimal, ranging from 50% to 87%. Magnetic resonance imaging is most effective for both diffuse and focal adenomyosis, with sensitivity and specificity that are comparable to or even better than those of sonography as it depicts contrasts between low-intensity lesions and surrounding tissue. Computed tomography has poor diagnostic value due to similar images portrayed by foci and normal myometrium. Myometrial biopsy of the posterior uterine wall, as pursued by some authorities in recent years, is superior to sonography, but its routine use is not recommended. Although various methods were added to the clinician's armamentarium over the last 2 decades, the preoperative diagnosis of adenomyosis remains challenging.

Abstract:Aim Schizophrenia is a leading cause of disability worldwide; early detection and intervention are critical. Early in their illness, individuals at clinical high‐risk (CHR) for psychosis have subthreshold psychotic symptoms that are often derogatory and self‐directed. Abstract:Abstract Deficits in metacognitive capacity in schizophrenia can be conceptualized as existing along a spectrum from more discrete to more synthetic activities. These capacities may be of great importance in schizophrenia research given their potential to mediate and moderate the impact of illness-related factors on outcome. To explore this possibility this review summarizes research on synthetic metacognition using a paradigm in which metacognitive capacity is rated on the basis of spontaneously produced personal narratives. Evidence from a review of the literature shows that these deficits are detectable in patients with schizophrenia and are related to, but not reducible to, symptom severity and poorer neurocognitive function. Independent of symptoms and neurocognition, deficits in synthetic metacognition, which are likely linked to the brain’s ability to integrate information, are related to a range of outcomes including functional competence, learning potential, and insight. These deficits may also play a role in long term psychosocial functioning via their impact on the ability to sustain social functions.

Abstract:Patients with first-episode schizophrenia show significant cognitive impairments even at this early phase of their illness. Antipsychotic medication improves clinical symptomatology, but the effectiveness of this treatment on neuropsychological deficits remains unclear. We investigated clinical symptom and neuropsychological performance change in 34 unmedicated first-episode psychotic patients (17 males, 17 females) from the time prior to treatment until 33.6+/-11.3 days after treatment initiation at which time patients demonstrated meaningful recovery from psychosis. Twenty-four matched healthy subjects were also studied. Performance in most neuropsychological functions (language skills, attention, nonverbal learning and reasoning, motor speed) remained stable for the group as a whole. However, reduction in negative symptoms was significantly correlated with performance increases in verbal fluency and attention. Higher negative symptom recovery was associated with improvement of cognitive performance to levels approaching those of healthy subjects, whereas low or no negative symptom improvement was associated with stable or decreased cognitive performance. Reduction in positive symptoms was not associated with change in cognitive abilities. These findings suggest a linkage between early, treatment-induced improvements in negative symptoms and reductions in distinct cognitive deficits.
