Abstract:Literature can offer a wealth of information about epilepsy: from complex narratives to children's picture books, it can help broaden people's understanding, show what it is like to live with epilepsy and provide a medium to which people with epilepsy (PWE) can relate. The latter being particularly important in such cases where seizure experiences are highly subjective, such as those associated with 'focal seizures', a common seizure type, which are known for their variable and hard-to-describe symptoms, causing complications with diagnosis as many of the symptoms overlap with those of other psychological health conditions.Literature, however, has more to offer than acting as a source for demystifying epilepsy. On a disciplinary level, literature is surrounded by different frameworks for linguistic analysis which, importantly, are also applicable to real-life discourse. In particular, the well-established discipline, cognitive stylistics, provides ample theory for analysing the different facets of literature, from narratological and storyworld level, to the intricacies of characterisation revealing the structure behind the presentation of fictional characters' experiences, attitudes and personalities. Such methods have the potential to transform and decode complex, subjective experiences into manageable pieces of information. This, then, holds great potential for shedding light on the experiences of real-life seizure narratives to the extent that the identified seizure's linguistic 'profiles' can be used to aid real-life situations. Therefore, the present study calls to attention the potential evoked through the convergence between literature, linguistic analysis, fictional characters, PWE and seizure narratives. Extrapolating the qualities of these converging strands can enrich our understanding of the seizure experience, as well as bring to awareness the areas of risk that surround aspects of the diagnosis process.

Abstract:Literature primarily reflects and affirms existing attitudes and conceptions regarding the epilepsy population. In the fiction of many nations for many centuries, individuals with epilepsy have emerged as evil or saintly, as geniuses, or as objects of pity whose lives were not worth living. The character who had epilepsy was outside the realm of personal identification--too far beneath or too far above us. Nevertheless, there were exceptions. Literature, as well as film and television, especially in the last 2 decades, has shown both readers and audiences that there are human beings who have epilepsy (rather than who are epileptics). These human beings are simply part of the continuum of what we know as normal: no more and no less than merely human.

Dissociation in patients with epilepsy and functional seizures: A narrative review of the literature

Abstract:Dissociation is a "disruption of the usually integrated functions of consciousness, memory, identity or perception of the environment" according to DSM-5. It is commonly seen in psychiatric disorders including primary dissociative disorders, post-traumatic stress disorder, depression, and panic disorder. Dissociative phenomena are also described in the context of substance intoxication, sleep deprivation and medical illnesses including traumatic brain injury, migraines, and epilepsy. Patients with epilepsy have higher rates of dissociative experiences as measured on the Dissociative Experiences Scale compared to healthy controls. Ictal symptoms, especially in focal epilepsy of temporal lobe origin, may include dissociative-like experiences such as déjà vu/jamais vu, depersonalization, derealization and what has been described as a "dreamy state". These descriptions are common in the setting of seizures that originate from mesial temporal lobe epilepsy and may involve the amygdala and hippocampus. Other ictal dissociative phenomena include autoscopy and out of body experiences, which are thought to be due to disruptions in networks responsible for the integration of one's own body and extra-personal space and involve the temporoparietal junction and posterior insula. In this narrative review, we will summarize the updated literature on dissociative experiences in epilepsy, as well as dissociative experiences in functional seizures. Using a case example, we will review the differential diagnosis of dissociative symptoms. We will also review neurobiological underpinnings of dissociative symptoms across different diagnostic entities and discuss how ictal symptoms may shed light on the neurobiology of complex mental processes including the subjective nature of consciousness and self-identity.
Writing autohistoria-teoría: agency and illness in German life narratives by Evelyne Leandro and Mely Kiyak

Abstract:Health concerns by migrants have been neglected in the German healthcare system, and they are impacted by discriminating discourses of othering. By analysing two autobiographical illness narratives by immigrants in contemporary Germany, this article exposes limitations in existing discourses of migration health and argues for more relational and affirmative theories of illness and care. Evelyn Leandro's diary The Living Death: The Struggle with a Long-Forgotten Illness (2017) describes her own drawn-out therapy against leprosy as a Brazilian in Berlin. In Mr Kiyak Thought That the Best Part of His Life Will Start Now (2013), the Turkish-German journalist Mely Kiyak narrates her father's experience with advanced lung cancer in a German hospital. Drawing on medical anthropology, postcolonial theory and material (eco)feminism, I argue that these narratives establish migrant health and agency in transnational assemblages that include chemotherapy, lungs and skin, family networks, healthcare providers, food cultures and health policies. These assemblages of illness are connected with the narratives' hybrid and relational aesthetics and politics: similar to Gloria Anzaldúa's practice of autohistoria-teoría, I show how Kiyak's and Leandro's life writing combines personal and communal storytelling with critical theorising to include diverse voices, languages, histories and identities. By transgressing identities of self and other, German and foreign, patient and physician, human and non-human, the narratives inspire a greater sense of the extent to which (all) bodies, histories, cultures, technology and medicine are entangled in a dense network of relations. This article envisions a relational and hybrid ontology and aesthetics of migration health and thereby intervenes into the growing field of transcultural medicine and medical humanities.

Abstract:This essay examines constructions of deafness in medieval culture, exploring how deaf experience disrupts authoritative discourses in three textual genres: medical treatise, literary fiction, and autobiographical writing. Medical manuals often present deafness as a physical defect, yet they also suggest how social conditions for deaf people can be transformed in lieu of treatment protocols. Fictional narratives tend to associate deafness with sin or social stigma, but they can also imagine deaf experience with a remarkable degree of sympathy and nuance. Autobiographical writing by deaf authors most vividly challenges diagnostic models of disability, exploring generative forms of perception that deafness can foster. In tracing the disruptive force that deaf experience exerts on perceived notions of textual authority, this essay reveals how medieval culture critiqued the diagnostic power of medical practitioners. Deafness does not simply function as a symptom of an individual problem or a metaphor for a spiritual or social condition; rather, deafness is a transformative capacity affording new modes of knowing self and other.

Abstract:Our subject is a dialogue with six literary works, understood in the publishing market as biographies that consider first and second person narratives on the relationship with situations of chronic, rare or complex disease. As a theoretical essay, we try to build an argument: these biographies are public forms of people's construction and presentation; they represent the possibility of assigning a place of visibility to moral experiences, of great public appeal, but which must be recognized in health care and training as political tools for reflection on practices. Thus, they become testimonies, providing less personal life histories and more biographical paths, with an interactionist dialogue among meanings, places, people, positions, stigma, right violations and discrimination.

Abstract:Questions of illness experience and identity are discussed, based on the analysis of a story told by the breast-cancer activist Audre Lorde. Displacing Parsons’ conceptualization of illness as a sick role, I understand the ill person as a narrative subject, defined by discursive possibilities. Three discourses of illness are proposed: the medical institutional discourse, the discourse of illness experience, and the pink-ribbon discourse. Each has its preferred narratives. These discourses overlap and mutually affect each other. Problems with the Foucauldian conceptualization of the subject are considered, and a dialogical imagination of relations of governmentality is proposed.
Emotion recognition and theory of mind after temporal lobe epilepsy surgery: A systematic review

Abstract:In order to navigate in our complex social world successfully, it is crucial to maintain and practice cognitive skills that are dedicated to adaptive social functioning. Emerging evidence suggests that besides deficits in declarative memory, executive functions, and language, impairments in social cognition (SC, e.g., emotion recognition, theory of mind) are also present in temporal lobe epilepsy (TLE). The organic and psycho-social consequences of epilepsy surgery might have additional implications regarding this deficit. Here we qualitatively synthesize longitudinal and cross-sectional findings on SC after TLE surgery. A literature search using PubMed and Scopus identified 275 potential articles. Studies were eligible if they (1) included patients with a diagnosis of TLE, (2) included a healthy comparison group, (3) reported original research, (4) were published in peer-reviewed journals and in English language, (5) reported the intervention of epilepsy surgery. Articles that (1) were case studies, (2) did not focus on SC abilities, (3) used interviews or self-report questionnaires to examine SC functions were excluded. A total of 16 original studies assessing emotion recognition (ER) and/or theory of mind (ToM) matched our criteria. The literature suggests that neither ER nor ToM abilities change after surgery: post-surgery patients show similar impairment patterns to pre-surgery patients. Nevertheless, individual improvement or decline could be masked by group comparisons and results should be considered in light of methodological heterogeneity among studies.

Abstract:Minimally invasive surgical techniques, such as MR-guided laser interstitial thermal therapy (LITT), have emerged as promising alternatives to open disconnective surgeries in drug-resistant epilepsy (DRE). This review synthesizes current literature on the application of LITT for corpus callosal disconnection and functional hemispheric disconnection. Studies highlight LITT's effectiveness for achieving seizure control and functional outcomes, often with reduced complications compared to traditional open procedures. Challenges include technical limitations to achieving total disconnection and adequate assessment of disconnection postoperatively. The literature is largely composed of observational studies and there is a need for rigorous, multi-center trials to establish robust guidelines and improve generalizability in clinical practice. There is also a need for a more robust exploration of how patient-specific factors contribute to response or nonresponse to intervention.

The effect of tuberculin on the respiratory rate of skin from sensitised guinea-pigs.

Abstract:ONE of the classical distinctions usually drawn between the tuberculin or delayed type of hypersensitivity and the urticarial or immediate type of response is the fact that tuberculin has a cytotoxic action in vitro on the tissues of sensitised animals (Rich and Lewis, 1932; Moen and Swift, 1936). This has led to a widespread belief that the tuberculin reaction in vivo occurs as the result of a necrosing effect of tuberculin on the skin. A previous study however (Cruickshank, 1951) showed that sensitised skin slices cultivated in vitro by the method of Medawar (1948) showed no abnormality in the presence of tuberculin. Since then Baldridge and Kligman (1951) have failed to repeat Rich's findings and Everett, Livingood, Pomerat and Funan (1952) have shown that tuberculin and chemical sensitising agents have no toxic effects on tissue cultures of human epidermal cells from subjects showing sensitivity to skin tests. Histological studies have recently led Gell and Hinde (1954) to advance the idea that the tuberculin reaction in vivo is essentially proliferative. In vitro however there is no histological evidence of proliferation of mesenchymal cells of the dermis. Since it was considered that histological examination of the skin after cultivation might not be a sufficiently sensitive method to reveal small changes of either a toxic or proliferative nature further experiments have been made in which the oxygen consumption of the skin cultures has been used as a measure of their biological activity. With conventional apparatus for measuring oxygen consumption the period of observation is usually limited to some 4 hr. This is an insufficient period on which to base a decision concerning the effects of tuberculin, as, in vivo, the reaction does not apparently begin until 4 hr. have elapsed and is usually maximal at 24 hr. Accordingly a special differential capillary microrespirometer has been devised which will permit continuous observation of skin cultures for prolonged periods.

Abstract:Each collective trauma holds its own particularities and forms of horror. When the violence is exerted by the government responsible for the care of the population it is termed state terrorism. The traumatic experience and its subsequent negation create a profound dissociation between two narratives: the explicit, which conceals the true facts, and the implicit, which remains unconscious and unbridgeable. In the gap between the two, life becomes suspended. From a Jungian perspective, this can be understood as the interruption of the process of translation and integration (terms that I will explore in some depth) from implicit sensory phenomena to an explicit representational narrative. This profoundly affects the development of the ego-self axis. In turn, it creates a special challenge for analytic technique that calls for new ways of listening to, and meeting the patient in, that non-verbal, unrepresented territory. Drawing upon clinical material, an embodied perspective of Jungian clinical work is offered to show how the inclusion of the body of patient and analyst enables access to the non-represented, though implicitly encoded, traumatic affective memories stored in the somatic unconscious.
Fabrication of Cationic Exchange Polystyrene Nanofibers for Drug Delivery

Abstract:Purpose: To prepare polystyrene nanofiber ion exchangers (PSNIE) with surface cation exchange functionality using a new method based on electrospinning and also to optimize crosslinking and sulfonation reactions to obtain PSNIE with maximum ion exchange capacity (IEC). Method: The nanofibers were prepared from 15% w/v polystyrene solution in dimethylacetamide (DMAc) containing 0.025 %w/v tetrabutylammonium bromide (TBAB) using electrospinning technique, followed by crosslinking with sulfuric acid/formaldehyde in a ratio ranging from 100/0 to 50/50 v/v and sulfonation in sulfuric acid. Degree of crosslinking was determined as the amount of fibers that remained in dichloromethane. The morphology and diameter of the fibers were evaluated by scanning electron microscopy (SEM) while IEC of PSNIE was performed by salt splitting titration. Results: PSNIE crosslinked with a sulfuric acid/formaldehyde ratio of 90/10 with 0.1 %w/v silver sulfate for 10 min at 70  C and sulfonated in 98 % sulfuric acid with 0.2 %w/v silver sulfate as the catalyst at 100  C for 30 min showed a maximum IEC of 3.21 meq/g-dry-PSNIE. Increase in sulfonation temperature caused the IEC of PSNIE to increase due to faster sulfonation. It was observed that the higher the temperature the faster the rate of sulfonation reaction. The diameter of the fibers after sulfonation was 404 ± 42 nm. Conclusion: These results indicate that PSNIE can be successfully prepared by electrospinning. Furthermore, cationic drug can be loaded onto the novel PSNIE for controlled release delivery.

Abstract:Boundary effects are investigated for fluids with internal orientational degrees of freedom such as molecular liquids, thermotropic and lyotropic liquid crystals, and polymeric fluids. The orientational degrees of freedom are described by the second rank alignment tensor which is related to the birefringence. We use a standard model to describe the orientational dynamics in the presence of flow, the momentum balance equations, and a constitutive law for the pressure tensor to describe our system. In the spirit of irreversible thermodynamics, boundary conditions are formulated for the mechanical slip velocity and the flux of the alignment. They are set up such that the entropy production at the wall inferred from the entropy flux is positive definite. Even in the absence of a true mechanical slip, the coupling between orientation and flow leads to flow profiles with an apparent slip. This has consequences for the macroscopically measurable effective velocity. In analytical investigations, we consider the simplified case of an isotropic fluid in the Newtonian and stationary flow regime. For special geometries such as plane and cylindrical Couette flow, plane Poiseuille flow, and a flow down an inclined plane, we demonstrate explicitly how the boundary conditions lead to an apparent slip. Furthermore, we discuss the dependence of the effective viscosity and of the effective slip length on the model parameters.

Abstract:Graphic medicine, an interdisciplinary field situated at the crossroads of comics and healthcare, operates as a medium through which the intricate nature of experiences with illness can be articulated, challenging orthodox medical dogmatism in an engaging and accessible way. Combining the affordances of comics and the narrative power of storytelling, graphic medicine elucidates the socio-cultural stigmatization of dementia influenced by a multitude of discourses. Diverging from existing discourses that depict individuals with Alzheimer's disease (AD) as zombies, brain-dead, or empty shells, graphic memoirs reconstruct these reductive notions and represent them as imaginative, productive, and perceptive. Taking these cues, the present paper close reads some sections of Dana Walrath's (2016) Aliceheimer's: Alzheimer's Through the Looking Glass in order to demonstrate how graphic medicine reconceptualizes the preeminent hallucinatory experiences of her AD-afflicted mother, Alice, as visions. Walrath deploys collage art to epitomize Alice's ordeal with AD. In particular, Walrath deploys thought-provoking fragments from Lewis Caroll's Alice in Wonderland, strategically to proximate Alice's experiences with AD and tackle the problem of dementia and sociality. Additionally, the paper explores how the text fosters interdependence, respect, and trust to recognize and restore Alice's personhood. The paper concludes by discussing how Aliceheimer's operates as an alternative paradigm beyond the confines of biomedical and cultural models of dementia through the use of lexical puissance.

Abstract:This article explores a literary tradition situated at the intersection of scientific reports, memoirs, and creative writing, termed "clinical narrative." This genre offers a profound approach to the painful aspects of conscious experience, particularly the phenomenological states associated with mental illness and brain disease, seen as unsettling landscapes of phenomenal experience. Through case studies providing multifaceted viewpoints – first-person, second-person, and third-person perspectives – we argue that clinical narratives are valuable resources for a transepistemic study of consciousness. By examining clinical conundrums such as somatic and nihilistic delusions, and anosognosia, we highlight the importance of detailed phenomenological, hermeneutic, and narrative accounts while acknowledging the significance of subpersonal, mechanistic models from cognitive and affective neuroscience. The tradition embodies the tension between the diverse perspectives in the field of mental health, including stories that directly challenge the medical discourse. However, the narrative arts can act as mediators or even peacemakers, by fostering an understanding between the opposing views. Stories are open to multiple interpretations, preserving the diversity of discourses on human meaning and avoiding the imposition of monolithic versions of our humanness.
