Clinical Child and Family Psychology Review

Abstract:This paper presents the clinical/developmental framework underlying a new diagnostic observational tool, the Disruptive Behavior Diagnostic Observation Schedule (DB-DOS). The special importance of observation for clinical assessment during the preschool period is delineated. The developmental rationale for a multi-dimensional assessment of disruptive behavior in young children, including problems in modulation of negative affect and low competence is discussed. The ways in which the DB-DOS will elucidate distinctions between normative and atypical behavior during this developmental period via (a) the integration of qualitative and quantitative dimensions of behavior within a clinically-sensitive coding system, (b) the observation of child behavior both within, and outside of, the parent–child context and (c) the use of specially designed tasks to “press” for clinically salient behaviors are addressed. The promise of this new method for yielding a more precise, developmentally based description of the phenotype of early onset disruptive behavior problems and for providing a standardized clinical tool for observational assessment of disruptive behavior in young children is presented. Large-scale validation of the measure is currently underway.

Abstract:Background/Objectives: Research on early risk factors for disruptive behaviors (DBs: hyperactivity–impulsivity/HI, non-compliance/NC, or physical aggression/PA) has predominantly focused on individual DBs in silos (i.e., HI, NC, or PA) or the broader category of externalizing, reporting mostly common risk factors among them. However, studies addressing DB comorbidity, i.e., the simultaneous occurrence of more than one DB, showed differences in risk factors among DB comorbid profiles. Aiming to clarify this discrepancy, the present study compared the early risk factors associated with different longitudinal patterns (i.e., trajectories) of single-DBs (HI, NC, PA) with risk factors associated with monomorbid (HIonly, NConly, PAonly) and comorbid (HI + NC, NC + PA, HI + NC + PA) joint-DBs trajectories during the preschool period. Methods: In a population-based birth cohort (N = 2045), parents' pre-conception characteristics, pregnancy and perinatal conditions, and age 5 months child and family characteristics were used to compare children following single-DB and joint-DBs high trajectories to children following low or moderate trajectories. The DB trajectories were derived from mother ratings at ages 11⁄2, 21⁄2, 31⁄2, 41⁄2, and 5 years. Results: More risk factors were identified for single-DB high trajectories than for joint-DBs high trajectories. On average, children on a single-DB high trajectory shared only 44.2% of their risk factors with children on a related joint-DBs high trajectory. Moreover, high trajectories of single-DBs shared a larger proportion of their risk factors than did high trajectories of joint-DBs. The findings show that categories of DBs include different subgroups of children based on their comorbidity patterns across DBs, which are differentially linked to early risk factors. Conclusions: Addressing comorbidity when investigating early risk factors of preschool DBs may improve our understanding of the etiological processes leading to these distinct but related behaviors and increase our ability to intervene upstream to prevent the earliest forms of potentially life-altering psychopathological conditions.

Context-Specific Associations Between Harsh Parenting and Peer Rejection on Child Conduct Problems at Home and School

Abstract:Although harsh parenting and peer rejection are independently associated with childhood conduct problems (CP), these patterns are often informant specific, suggesting that their associations across contexts (i.e., home and school) should be considered. In a sample of 142 children with and without attention-deficit/hyperactivity disorder (ADHD; ages 5-10; 66% male), we used structural equation modeling to evaluate the structure of multi-informant (parent, teacher) and multimethod (semi-structured interview, questionnaire) rated aggressive, rule-breaking, and oppositional behavior. Next, we explored context-specific associations by modeling harsh parenting and peer rejection as simultaneous and independent predictors of home and school CP. We observed several key findings: (a) the structure of parent- and teacher-reported CP was best accounted by context-specific CP (i.e., home vs. school) and a second-order general CP factor; (b) harsh punishment and peer rejection each independently predicted the second-order general CP factor; and (c) peer rejection was uniquely associated with school CP, whereas harsh punishment was associated only with the second-order general CP factor and did not exhibit specificity with home CP. Whereas harsh parenting and peer rejection were each independently associated with generalized CP, peer rejection showed an additional, unique context-specific association with CP exclusively expressed at school. We discuss potential explanatory mechanisms underlying context-specific associations of CP, as well as address etiological and clinical implications for understanding informant-discrepancies in CP.

Abstract:Behavior Modification, Ahead of Print. Background:Children with Down syndrome often engage in contextually inappropriate social behavior, which researchers suggest may function to escape from difficult activities to preferred social interactions. Caregivers may reinforce the behavior, perceiving it only as evidence of the child's social strength, when, in fact, the pattern may also prevent or slow the development of critical skills. Unlike overt forms of challenging behavior, contextually inappropriate social behavior had never been subjected to experimental analysis.Aims:The purpose of the current study was to identify and demonstrate functional control of contextually inappropriate social behavior to caregiver-informed contingencies.Method and Procedures:We interviewed caregivers and subjected contextually inappropriate social behavior to functional analyses for nine young children with Down syndrome.Outcomes and Results:We found sensitivity to the caregiver-informed contingencies for all nine participants with strong functional control and large effect sizes for most.Conclusions and Implications:Caregivers may not perceive contextually inappropriate social behavior as problematic, yet patterns of contextually inappropriate and other problem behaviors suggest decreased engagement and poor task persistence. Assessments that lead to intervention decisions may be more informative when they include questions about social topographies of behavior not typically considered as problematic. Once caregivers are aware of the pattern, they may be better prepared to intervene.

Abstract:Abstract Objectives Identification of clinically significant irritability in preschool age is important to implement effective interventions. However, varying informant and measurement methods display distinct patterns. These patterns are associated with concurrent and future mental health concerns. Patterns across multi‐informant methods in early‐childhood irritability may have clinical utility, identifying risk for impaired psychosocial functioning. Methods Using data from the Multidimensional Assessment of Preschoolers Study ( N = 425), latent profile analysis identified irritability patterns through the parent‐reported Multidimensional Assessment Profile Scales–Temper Loss (MAPS‐TL), parent‐reported interviewer‐rated Preschool Age Psychiatric Assessment (PAPA), and observer‐rated Disruptive Behavior Diagnostic Observation Schedule (DB‐DOS). These profiles were characterized on protective factors, global functioning, and mental health syndromes, concurrently and at early school age and preadolescent follow‐up. Results Fit indices favored a five‐class model: Low All, High Observation with Examiner (high DB‐DOS Examiner Context), High All, High Parent Report (high MAPS‐TL/PAPA), and Very High Parent Report (very high MAPS‐TL/PAPA). Whereas Low All and High Observation with Examiner exhibited strong psychosocial functioning, remaining profiles showed impaired psychosocial functioning, with the Very High Parent Report group showing higher impairment at follow‐ups, d s = 0.37–1.25. Conclusions Multi‐informant measurements of irritability may have utility for clinical prediction, and future studies should test utility for diagnostic precision.

Predictors of Later Psychopathology in Young Children with Disruptive Mood Dysregulation Disorder

Abstract:Objective: This study aimed to identify childhood factors that predict later psychiatric problems in children with disruptive mood dysregulation disorder (DMDD). Methods: The sample consisted of 36 6-year-old children who met criteria for DMDD who were followed up at 9 years of age. Child psychopathology was assessed at age 6 using the Preschool Age Psychiatric Assessment (PAPA) and at age 9 using the Kiddie-Schedule for Affective Disorders and Schizophrenia. We compared children with DMDD at age 6 who continued to have a psychiatric diagnosis at age 9 (n = 17) to children with DMDD at age 6 with no psychiatric diagnosis at age 9 (n = 19) across several age 6 predictors: child psychopathology, irritability and temperament, parenting, and maternal psychopathology. In addition, we examined whether children with DMDD at age 6 and no psychiatric diagnosis at age 9 continued to experience elevated psychiatric symptoms and impairment at age 9 compared to children with a non-DMDD diagnosis at age 6 and no psychiatric diagnosis at age 9 (n = 44) and children with no psychiatric diagnosis at age 6 or 9 (n = 266). Results: The following variables predicted which children with DMDD at age 6 would have a psychiatric diagnosis at age 9: higher levels of externalizing symptoms, anger/frustration, headstrong/hurtful behaviors, functional impairment, and temperamental surgency and negative affect; lower levels of effortful control/executive functioning; and maternal depression. However, children with DMDD at age 6 and no psychiatric diagnosis at age 9 continued to demonstrate greater disruptive behavior disorder symptoms and impairment at age 9 compared to children with no psychiatric diagnosis at age 6 or 9. Conclusions: These findings identify factors predicting later psychopathology in children with DMDD. In addition, we found that the subgroup of children with DMDD at age 6 but no psychiatric diagnosis at age 9 continued to evidence symptomatology and impairment 3 years later.

Abstract:Intrusive parenting has been traditionally considered a negative parenting style and includes actions that are overly directive and controlling of children's behavior. However, current research aims to contextualize this parenting behavior. This study examined the relation between intrusive parenting and early childhood behavior problems or emotion regulation, as well as the moderating role of race/ethnicity and developmental delay. Participants (n = 511; 47.4% male) were part of a larger, longitudinal Early Head Start study, entitled Baby FACES, with a racially and ethnically diverse sample (40.1% White, 18.7% Black, and 41.2% Hispanic). Behavior problems and developmental delay were assessed via parent report, while parent intrusiveness and emotion regulation were rated by observers. More use of intrusive parenting at age 2 was associated with greater behavior problems and less emotion regulation at age 3. However, this relation was moderated by race/ethnicity and developmental level. Greater intrusiveness was related to increased behavior problems and poorer emotion regulation for White and Hispanic/Latino families; they were unrelated in Black families. Similarly, although intrusive parenting and behavior problems were positively associated at all developmental levels, the relation was strongest for children with typical development. Further, fewer maternal risk factors, being female, and being Hispanic/Latino were associated with higher emotion regulation, and more risk factors were associated with more behavior problems. These findings highlight the importance of considering how a child's context and development interact with parenting style. Abstract:Self-regulation (SR) difficulties are implicated in a wide range of disorders which develop in childhood, including attention deficit hyperactivity disorder (ADHD), oppositional defiance disorder (ODD), anxiety and depression. However, the integration of the existing research evidence is challenging because of varying terminology and the wide range of tasks used, as well as the heterogeneity and comorbidity within and across diagnostic categories. The current study used the Research Domain Criteria (RDoC) framework to guide the examination of different SR processes in young children showing a wide range of symptomatology. Children (aged 4–8) referred by teachers for moderate-to-high conduct, hyperactivity and/or emotional problems at school (assessed using the Strengths and Difficulties Questionnaire (SDQ) subscales; n = 212), and children in SDQ typical ranges ( n = 30) completed computerised cognitive control and decision-making tasks. Parents completed questionnaires to assess ADHD, ODD, anxiety and depression symptoms ( n = 191). Compared to children with no teacher-reported difficulties, those with moderate-to-high problems showed poorer visuomotor control and decision-making. A factor analysis revealed that task variables adhered to RDoC dimensions and predicted variance in specific disorders: difficulties in cognitive control predicted ADHD symptoms, low reward-seeking was associated with depression and high reward-seeking was associated with ODD. This study highlights how the assessment of cognitive processes positioned within the RDoC framework can inform our understanding of disorder-specific and transdiagnostic difficulties in SR which are associated with diverse clinical symptoms in children.

Abstract:Oppositional defiant problems (ODP) and obsessive-compulsive problems (OCP) may co-occur in children, though the way they interact is not known. The aim of the study was to examine longitudinal associations between executive functions at age 3 and ODP, ODP dimensions, and OCP at age 6. The sample consisted of 622 preschoolers (50% were boys) from the general population. Executive functions were assessed by teachers using the Behavior Rating Inventory of Executive Functioning – Preschool version questionnaire when children were 3 years old, and ODP and OCP were informed by parents and teachers at the age of 6 years. Multiple linear regression analyses indicated that higher Inhibit and Emotional Control and lower Shift deficits were associated with higher ODP reported by teachers, while higher Shift but lower Inhibit deficits were related to higher OCP. Moreover, ODP and OCP shared difficulties on the Flexibility Index, which means that the capacity to modulate emotions and behavior according to contextual and environmental demands is compromised in both disorders. The findings inform etiology and prevention, pointing out not only the executive function specificities related to each problem, but also common cognitive challenges related to Flexibility. Young children could benefit from training and programs designed to improve executive function processes at an early age to prevent later behavioral difficulties.

Abstract:Inhibitory control plays an important role in children's cognitive and socioemotional development, including their psychopathology. It has been established that contextual factors such as socioeconomic status (SES) and parents' psychopathology are associated with children's inhibitory control. However, the relations between the neural correlates of inhibitory control and contextual factors have been rarely examined in longitudinal studies. In the present study, we used both event-related potential (ERP) components and time-frequency measures of inhibitory control to evaluate the neural pathways between contextual factors, including prenatal SES and maternal psychopathology, and children's behavioral and emotional problems in a large sample of children (N = 560; 51.75% females; M age = 7.13 years; Range age = 4–11 years). Results showed that theta power, which was positively predicted by prenatal SES and was negatively related to children's externalizing problems, mediated the longitudinal and negative relation between them. ERP amplitudes and latencies did not mediate the longitudinal association between prenatal risk factors (i.e., prenatal SES and maternal psychopathology) and children's internalizing and externalizing problems. Our findings increase our understanding of the neural pathways linking early risk factors to children's psychopathology.

Abstract:Examined the hypothesis that distinct parenting practices may be associated with type and profile of a child's disruptive behavior problems (e.g., oppositional, aggressive, hyperactive). Parents of 631 behaviorally disruptive children described the extent to which they experienced warm and involved interactions with their children and the extent to which their discipline strategies were inconsistent and punitive and involved spanking and physical aggression. As expected from a developmental perspective, parenting practices that included punitive interactions were associated with elevated rates of all child disruptive behavior problems. Low levels of warm involvement were particularly characteristic of parents of children who showed elevated levels of oppositional behaviors. Physically aggressive parenting was linked more specifically with child aggression. In general, parenting practices contributed more to the prediction of oppositional and aggressive behavior problems than to hyperactive behavior problems, and parenting influences were fairly consistent across ethnic groups and sex.

Abstract:Oppositional Defiant Behavioral (ODD) deals problems controlling emotions and behaviors. According to the Diagnostic Manual (DSM-V), the main characteristics of ODD are the persistence of irritated/angry moods, hostile and defiant behavior, and revenge against others. The prevalence of oppressive defiant disorder ranges from 1% to 11% (American Psychiatric Association, 2013), and uncooperative, opposing, and hostile behaviors toward parents, teachers, other authoritarian figures, and their peers. The article is compiled by collecting data from the literature about the specifics of this disorder. In this way, detailed information is provided about the history, diagnostic criteria, various causes that may affect the occurrence of this disorder, as well as information about various therapeutic interventions and how they affect this disorder. This paper was developed through a qualitative method, specifically with a case study, where a deliberate sample was selected, which meets the three pre-established criteria: to have been diagnosed with the oppressive defiant disorder; be around the age of 4-6 years; as well as pursue therapeutic interventions based on the behavioral approach. The paper raises two research questions regarding the impact of this disorder on the daily life of a 4-year-old and the impact that therapeutic intervention with a behavioral approach has on improving this daily life. The study was conducted through information obtained about the case through the method of triangulation of resources (parents, therapist, educator), as well as from direct activity environments. High number psychosocial and biological risk factors have been identified to influence the occurrence of this disorder. Studies have also been conducted on the best ways to intervene and treat this disorder, where behavioral approaches were noted to be amongst the most used interventions.
Comparing Self-Regulation-Associated Event Related Potentials in Preschool Children with and without High Levels of Disruptive Behavior

Abstract:<h3 class="a-plus-plus">Abstract</h3> <p class="a-plus-plus">Deficient self-regulation plays a key role in the etiology of early onset disruptive behavior disorders and signals risk for chronic psychopathology. However, to date, there has been no research comparing preschool children with and without high levels of disruptive behavior using Event Related Potentials (ERPs) associated with specific self-regulation sub-processes. We examined 15 preschool children with high levels of disruptive behavior (35 % female) and 20 peers with low disruptive behavior (50 % female) who completed a Go/No-go task that provided emotionally valenced feedback. We tested whether 4 ERP components: the Error Related Negativity, the Error Positivity, the Feedback Related Negativity, and the No-go N2, differed in preschool children with and without high levels of disruptive behavior. Preschoolers with high levels of disruptive behavior showed less differentiation between the Error Positivity and corresponding waveforms following correct responses at posterior sites. Preschoolers with high and low disruptive behavior also showed differences in Go/No-go N2 waveform amplitudes across electrodes. These findings suggest that preschool children with high levels of disruptive behavior may show abnormal brain activity during certain self-regulation sub-processes, informing potential advances in conceptualizing and treating early disruptive behavior.</p>

Abstract:We examined associations between child inhibitory control, harsh parental discipline and externalizing problems in 120 4 year-old boys and girls in the US, China, and Japan. Individual differences in children’s inhibitory control abilities, assessed using behavioral tasks and maternal ratings, were related to child externalizing problems reported by mothers. As predicted, both child inhibitory control and maternal harsh discipline made significant contributions to child externalizing problems in all three countries. Across countries, child inhibitory control and maternal harsh discipline made significant independent contributions to early externalizing problems, suggesting an additive model of association. Our findings supported the cross-cultural generalizability of child inhibitory control and parental harsh punishment as key contributors to disruptive behavior in young children.

Abstract:Investigated 45 children between the ages of 3 and 6 years—ranging widely in degree and type of problem behavior—via parent ratings, teacher ratings, and objective observations of social behavior in play groups. The goal was to ascertain cross-informant correspondence for preschool-aged children, with a separate examination of internalizing versus externalizing, behavior patterns. Regarding internalizing behavior, parent ratings (but not teacher ratings) independently predicted observed isolation and withdrawal. As for externalizing behavior, however, teacher ratings were far more strongly associated with play group observations than were parent reports; only the teacher information independently predicted observed noncompliance and aggression. These differential patterns of cross-informant convergence across internalizing versus externalizing domains are discussed in light of the need for accurate identification of behavioral/emotional risk in preschoolers.

Abstract:A proportion of children within the autism spectrum is at risk for severe deregulation of thought, emotion and behaviour resulting in (symptoms of) psychotic disorders over the course of development. In an attempt to identify this subgroup, children with PDD-NOS, subtype MCDD (n = 24) were compared to children with PDD-NOS (n = 23) on executive function (EF) skills. Significant differences emerged, always to the disadvantage of the children with PDD-NOS, subtype MCDD on various EF measures. The findings suggest compromised attention regulation and impaired inhibitory control in children with MCDD, which may help explain high levels of thought problems which are frequently observed in these children. Our findings provide evidence for recognizing a PDD subcategory of MCDD that is of specific interest with regard to long-term developmental risks involved.

Abstract:The developmental association between disruptive behaviors (DBs: hyperactivity-impulsivity, non-compliance, physical aggression) and internalizing problems in early childhood is not well understood and has generated competing hypotheses and mixed results. Using a person-centered strategy, the present study aimed to examine concurrent trajectories of DBs and trajectories of internalizing problems from age 1.5 to 5 years in a population-representative sample (N = 2057; 50.7% boys). Six trajectories of DBs and three trajectories of internalizing problems, based on parent reports and obtained via latent growth modeling across five periods of assessment, were used as longitudinal indicators of each type of behaviors. Children following low or moderate trajectories served as the reference class. Compared to children in the reference class, those in trajectory classes characterized by high levels of co-occurring DBs (OR = 6.60) and, to a lesser extent, those in single high DB classes (OR = 2.78) were more likely to follow a high trajectory of internalizing problems simultaneously. These results support a multiple problem hypothesis regarding the association between DBs and internalizing problems, consistent with a developmental perspective that includes a general factor underpinning different psychopathologies. These findings highlight the importance of considering the co-occurrence between DBs and internalizing problems when studying either construct in children.
Emotional Regulation and Executive Function Deficits in Unmedicated Chinese Children with Oppositional Defiant Disorder

Abstract:OBJECTIVE: This study aims to explore the feature of emotional regulation and executive functions in oppositional defiant disorder (ODD) children.METHODS: The emotional regulation and executive functions of adolescents with ODD, as well as the relationship between the two factors were analyzed using tools including Adolescent Daily Emotional Regulation Questionnaire (ADERQ), Wisconsin Card Sorting Test (WCST) and Cambridge Neuropsychological Test Automated Battery (CANTAB), in comparison with attention deficit hyperactivity disorder (ADHD) children without behavioral problem and healthy children; the ADERQ assessed emotional regulation ability and others were used to assess executive function.RESULTS: Compared to normal children, the ODD group displayed significant differences in the scores of cognitive reappraisal, rumination, expressive suppression, and revealing of negative emotions, as well as in the score of cognitive reappraisal of positive emotions. WCST perseverative errors were well correlated with rumination of negative emotions (r=0.47). Logistic regression revealed that the minimum number of moves in the Stocking of Cambridge (SOC) test (one test in CANTAB) and negative emotion revealing, were strongly associated with ODD diagnosis.CONCLUSION: Children with ODD showed emotion dysregulation, with negative emotion dysregulation as the main feature. Emotion dysregulation and the lack of ability to plan lead to executive function deficits. The executive function deficits may guide us to understand the deep mechanism under ODD.
