Abstract:Background To measure the iodine nutritional status of island adults in Zhoushan, China.Methods A comparison study was carried out in eight villages. These were selected from iodized salt and non-iodized salt districts of Zhoushan island by random sampling. The Mann-Whitney test was used to compare the urinary iodine concentration and dietary iodine intake between the two districts.Results The median of urinary iodine concentration in the non-iodized salt group was 90 mug/l, which was lower than 194 mug/l in the iodized salt group (u = 14.673; p < 0.000), whereas the median of daily dietary iodine intake in the two groups was 128 and 147 mu g, respectively (u = 1.847; p = 0.065). There was no significant correlation between dietary iodine intake and urinary iodine concentration (p = 0.095).Conclusions Salt iodization is necessary. Special characteristics of the island diet should also be considered.

Abstract:Zhejiang introduced universal salt iodization (USI) programme in 1995 and has achieved the goal of elimination of iodine deficiency disorders (IDD) since 2011. However, no systematical data of iodine nutritional status in population in pregnancy is available. In this cross-sectional study, pregnant women were interviewed to complete questionnaires in addition to handing in samples of urine and household table salt between March 2016 to February 2017. Date of birth, age of pregnancy, ethnicity and dietary iodine habits were recorded. The overall median urinary iodine concentration in 8561 pregnant women was 130.47 µg/L, which was lower than the cut-off value of iodine sufficiency of 150 µg/L recommended by the WHO. Participants using non-iodized salt, taking non-iodine-containing supplements, in coastal and in Han group were independently associated with iodine deficiency. The current USI programme did not supply Zhejiang pregnant women with sufficient iodine intake. They are generally iodine deficient, which have great public health importance since even mild IDD in pregnancy have adverse effects on fetal neurodevelopment. We strongly recommend urgent measures to improve iodine intake in pregnancy.

Abstract:OBJECTIVE:To evaluate the iodine nutritional status and its relation to iodized salt intake in child islanders.METHODS:A comparing study was carried out in 4 townships selected by random sampling from Dinghai (iodized salt) and Daishan(non-iodized salt) of Zhoushan island and total 592 of children were included in the study. The Mann-Whitney test was used to compare the urinary iodine concentration and dietary iodine intake of two groups. The correlation of urinary iodine concentration and dietary iodine intake were examined by Spearman correlation test. Ordinal regression was used to analyse the dependent variables of urinary iodine concentration.RESULTS:The urinary iodine concentration of non-iodized salt district was lower than that in iodized salt district (87 microg/L compared with 150 microg/L, u=7.296, P=0.000) ,whereas the amount of daily iodine intake in the two groups was 34.5 microg/d and 62.3 microg/d (u=6.925, P=0.000). The urinary iodine concentration of 58.6 % children in non-iodized salt district was below 100 microg/L. Age and iodized-salt intake were significant factors in the final regression model (P<0.05) with the OR of 1.119 and 3.238, respectively.CONCLUSION:The daily dietary iodine intake for children in Zhoushan island is insufficient, the iodized salt prophylaxis is necessary.

There was no significant correlation between daily iodine intake and urinary iodine concentration (r(s) = 0.052, P = 0.095).CONCLUSIONS:In Zhoushan islands, the daily iodine intake did not meet the daily need (150 micro g/day) suggesting that iodized salt supplement was necessary. However, side effect due to overdose should be brought into attention.

Abstract:We aimed to assess dietary iodine intake and sources in Zhejiang Province a decade after a reduction in iodine concentration in iodized salt. Three-day 24 h dietary recall and household weighing were used, complemented by "Chinese Food Composition" data. Household water and salt samples were collected from 5890 residents and analyzed. Differences in iodized salt consumption rates were observed across the following regions: inland (84.20%), subcoastal (67.80%), and coastal (37.00%) areas. The median (P25, P75) iodine concentration in water and diet were 2.2 (0.9, 4.0) μg/L and 142.05 (58.94, 237.11) μg/d, respectively, with significant regional differences in dietary concentration (inland [185.61 μg/d], subcoastal [153.42 μg/d], and coastal [75.66 μg/d]). Males (149.99 μg/d) and iodized salt consumers (191.98 μg/d) had a significantly higher dietary iodine intake than their counterparts. Regions were ranked as follows based on the proportions of individuals meeting the recommended dietary iodine intake: inland (69.40%), subcoastal (56.50%), and coastal (34.10%) areas. Dietary sources included salt (48.54%), other foods (32.06%), drinking water (8.84%), laver (4.82%), kelp (3.02%), and other seafood (2.32%). The qualified iodized salt consumption rate was significantly lower than the national standard. Zhejiang Province should continue implementing measures to control iodine deficiency through salt iodization, education efforts, and increasing the qualified iodized salt consumption rate.

Abstract:The median urinary iodine concentration (UIC) of school-aged children has been commonly used as a surrogate to assess iodine status of a population including pregnant women. However, pregnant women have higher iodine requirements than children due to increased production of thyroid hormones. The aim of the study was to evaluate the iodine status of pregnant women and children as well as their household salt iodine concentration (SIC) in Quzhou, Zhejiang Province, China. Eligible pregnant women and children from all six counties of Quzhou in 2021 were recruited into the study. They were asked to complete a socio-demographic questionnaire and provide both a spot urine and a household table salt sample for the determination of UIC and SIC. A total of 629 pregnant women (mean age and gestation weeks of 29.6 years and 21.6 weeks, respectively) and 1273 school-aged children (mean age of 9 years and 49.8% of them were females) were included in the study. The overall median UIC of pregnant women and children in our sample was 127 (82, 193) μg/L and 222 (147, 327) μg/L, respectively, indicating sufficient iodine status in children but a risk of mild-to-moderate iodine deficiency in pregnant women. Distribution of iodine nutrition in children varied significantly according to their sex and age ( P < 0.05). The rate of adequately household iodised salt samples (18–33 mg/kg) provided by pregnant women and children was 92.4% and 90.6%, respectively. In conclusion, our results indicated a risk of insufficient iodine status in pregnant population of China, but iodine sufficiency in school-aged children. Our data also suggested that median UIC of children may not be used as a surrogate to assess iodine status in pregnant women.

Abstract:We aim to analyze the changes in dietary iodine intake and the contribution rates of various foods to it after the reduction in salt iodine concentration in Zhejiang. We used data from two cross-sectional nutrition surveillance surveys conducted by the Zhejiang Provincial Center for Disease Control and Prevention in 2010 (9798 residents) and 2022 (5980 residents). In both surveys, multi-stage stratified and systematic sampling were adopted, and uniformly trained investigators conducted the dietary surveys using a 24 h dietary review and weighing record methods for 3 consecutive days. From 2010 to 2022, the median salt iodine concentration and the consumption rate of qualified iodized salt in Zhejiang households dropped from 28.80 to 22.08 mg/kg and from 76.65% to 64.20%, respectively. Moreover, the residents' median dietary iodine intake decreased from 277.48 to 142.05 μg/d. Significant interregional differences in dietary iodine intake were found in 2010 and 2022 (H = 639.175, p < 0.001; H = 588.592, p 0.05). From 2010 to 2022, the proportion of residents with dietary iodine intake below the estimated average requirement increased from 15.10% to 34.80%, while that of residents with intake above the tolerable upper limit decreased from 15.00% to 2.90%. The contribution rate of salt to dietary iodine intake among residents in Zhejiang decreased from 74.92% to 48.54%, showing an apparent overall downward trend despite the dietary intake being generally adequate (markedly inadequate in coastal regions). The salt iodine concentration and the consumption rate of qualified iodized salt in households in Zhejiang showed downward trends. Salt remained the main source of dietary iodine; however, its contribution decreased significantly. Zhejiang may need to reverse the trend of the continuous decline in the consumption rate of qualified iodized salt to protect the health of its residents.

Abstract:BACKGROUND: Both insufficient and excess iodine may produce thyroid disease. After salt iodization in China, the median urine iodine concentration (UIC) of children aged 8-10 years appeared adequate. However, it is unknown whether dietary changes due to rapid economic development in Shanghai have affected whole population iodine nutrition.OBJECTIVE: To assess dietary iodine intake, UIC and the prevalence of thyroid disease in the general population of Shanghai.DESIGN: A cross-sectional survey was conducted with general participants aged 5-69 years (n = 7,904) plus pregnant and lactating women (n = 380 each) selected by stratified multistage sampling. The iodine concentrations in their salt, drinking water and urine were measured. Daily iodine intake was estimated using the total diet study approach. Serum thyroid hormone concentrations and thyroid-related antibodies were measured and thyroid ultrasonography was performed.RESULTS: The median iodine concentration in salt was 29.5 mg/kg, and 12.8 µg/L in drinking water. Iodized salt, used by 95.3% of participants, contributed 63.5% of total dietary iodine. Estimated daily iodine intake was 225.96 µg. The median UIC of general participants was 146.7 µg/L; UIC <100 µg/L (iodine insufficiency) was seen in 28.6%; UIC >300 µg/L (iodine excess) in 10.1%. Pregnant women had a median UIC of 135.9 µg/L, with UIC <150 µg/L in 55.4%. Thyroid nodules and subclinical hypothyroidism were found in 27.44% and 9.17%, respectively.CONCLUSIONS: According to published criteria, the current dietary iodine intake in Shanghai was generally sufficient and safe, but insufficient in pregnant women. Thyroid nodules and subclinical hypothyroidism were the commonest thyroid diseases identified.

Abstract:Background: Despite the implementation of the universal salt iodization (USI) program for correction of iodine deficiency in China for ∼20 years, the actual iodine nutrition status of Chinese residents and the prevalence of iodine deficiency and iodine excess are issues that need to be addressed. This nationally representative cross-sectional study was conducted across all 31 provinces of mainland China to gather extensive data on iodine nutrition status and the influential factors. Methods: This study included 78,470 participants, aged 18 years or older, who were interviewed and asked to answer a questionnaire. Urine iodine concentration (UIC) was measured by the inductively coupled plasma mass spectrometry method, and goiter was examined by thyroid ultrasonography. In addition, sixty 9-11 years old school children in each province were randomly selected to evaluate the UIC and thyroid ultrasonography. The iodine nutrition status was determined according to the World Health Organization guidelines. Results: The iodized salt coverage was 95.37%. The median urine iodine (MUI) was 177.89 μg/L (interquartile range [IQR], 117.89-263.90 μg/L) and goiter prevalence was 1.17% (confidence interval [95% CI 0.95-1.43]) in the adult population. The MUI was 199.75 μg/L (IQR, 128.41-303.37 μg/L) in school-age children, and goiter prevalence was 3.50% [95% CI, 2.93-4.13]. The percentage of individuals with UIC <50 μg/L was 3.43%, <20%. Analysis indicated that sex, age, geographic factors, body mass index, and smoking habits influence the iodine nutrition level. Conclusion: The mandatory USI program has successfully eliminated iodine deficiency disorders, and the findings indicate that the iodine nutrition level in the general population is within the safe range.

Abstract:Abstract Background/Aims: Iodine deficiency is a public health problem over the world. China has achieved the iodine deficiency disorders (IDD) elimination goal through a mandatory universal salt iodization (USI) program. The aim of this study is to assess the current iodine nutritional status and thyroid volume of school-aged children in Wuhan, central of China. Methods The cross-sectional study was conducted among 8–10 years old children from 5 districts in Wuhan. A total of 1000 students were investigated. Salt iodine concentrations, urine iodine concentrations were determined using the standardized methods recommended by WHO/ICCIDD. An ultrasonography of thyroid and anthropometric measurements were performed in all subjects. Results The median urinary iodine concentration(MUI) of students was 247.50 μg/L with interquartile range of 147.00 to 384.00 μg/L. Two districts were classified as regions with adequate iodine nutrition, two districts as regions above the requirement, and one district as a region excessive according to the MUI. The prevalence of goiter was 3.0% and 7.8% according to Chinese national and WHO reference values, respectively, and the difference was not observed among different iodine nutrition groups(P༞0.05). The median thyroid volume(Tvol) of all students was 2.56 ml and the P97 of thyroid volume of both male and female in our study was generally higher than that reported by WHO/ICCIDD in 2007 as a function of age and BSA. The Tvol was in significant association with age, height, weight and BSA by Spearman’s correlation analysis(P < 0.001),and the result of multivariate linear regression analysis showed that only BSA were found to have a significant effect on thyroid volume after log conversion (t = 13.437, P < 0.001). Conclusion The present study indicated that the iodine nutrition among school-aged children in Wuhan was generally adequate with a relatively low goiter rate, while some of them have excess iodine nutrition. Thyroid volume in our study was higher than the reference of WHO and showed a significant correlation with BSA. So further researches were needed to evaluate the thyroid function in children with iodine excessive and the applicability of WHO reference in goiter diagnose. It was also highly recommended to establish the local reference values for thyroid volume that might be applicable to precisely define goiter prevalence in Wuhan.
The Effect of Iodine Status on the Risk of Thyroid Nodules: A Cross-Sectional Study in Zhejiang, China

Abstract:Objective. The aim of this study was to explore whether iodine nutrition is associated with the risk of thyroid nodules among adult population in Zhejiang Province, China. Methods. A cross-sectional study was conducted in the general population aged 18 years or older. A total of 2,710 subjects received physical examination, questionnaires, and thyroid ultrasonography. Urinary iodine concentration (UIC) and thyroid hormone levels were measured and documented for each subject. 4 multiple logistic regression models adjusted for other risk factors were applied to analyze the association between iodine nutrition and thyroid nodules. Results. The prevalence of thyroid nodules was 15.5% among all adults. As indicated by all 4 models, subjects with UIC varying from 200 μg l−1 to 399 μg l−1 had lower risk of thyroid nodules compared with those with relatively low UIC (<100 μg l−1), with approximately 37–57 percent reduction in risk. Moreover, subjects with UIC between 100 and 199 μg l−1 had a decreased risk of thyroid nodules in model 1 and 2 (OR = 0.75, 95% CI, 0.58–0.97; OR = 0.75, 95% CI, 0.58–0.97, respectively). However, there was no significant difference of risk in thyroid nodules between subjects with high UIC (≥400 μg l−1) and low UIC (<100 μg l−1). Furthermore, intake of iodized salt was inversely associated with risk of thyroid nodules, with approximately 69–77 percent reduction in risk. Conclusion. The relationship between UIC and the risk of thyroid nodules is U-shaped. Consumption of noniodized salt is an independent risk factor of thyroid nodules.
Adequate iodine nutrition and higher salt intake in Chinese adults aged 18–59 years recommended by international organizations

Abstract:Iodine deficiency and excessive salt intake have adverse health effects. This study evaluated the iodine level and salt intake in Chinese adults aged 18–59 years after implementing the salt reduction program and compared with both the World Health Organization (WHO) and Chinese recommendations. Adults aged 18–59 years were randomly selected using multi-stage stratified random sampling in coastal urban area (CUA), non-coastal urban area (Non-CUA), coastal rural area (CRA), and non-coastal rural area (Non-CRA) of Fujian Province, China. Iodine, sodium, and creatinine concentrations in spot urine samples were measured. Knudsen equation was used to determine 24-h urinary iodine and sodium excretion. The median urinary iodine concentration (mUIC) and urinary sodium concentration (mUNaC) among adults ( n = 3513) were 132.0 μg/L and 4.0 g/d, respectively. The mUIC and median daily iodine intake in CUA, Non-CUA, CRA and Non-CRA were 112.1, 127.5, 128.5, 167.5 μg/L and 189.6, 182.5, 199.4, 236.0 μg/d, respectively. The mUNaC and median daily salt intake (mDSI) in these four areas were 2.4, 2.8, 2.9, 2.9 g/L and 9.8, 10.4, 10.4, 10.6 g/d, respectively. The mUIC and DII of residents were higher in the Non-CRA than in the other three areas ( P < 0.05). The UNaC and DSI of residents were lower in the CUA than in the other three areas ( P < 0.05). The logistic regression demonstrated that the people living in CUA and Non-CUA consumed less salt compared with those in Non-CRA. Except for Non-CUA, the DII was lower (< 150 μg/d) among women of childbearing age in the low–salt intake group (< 5 g/d) compared with the high–salt intake group (≥ 5 g/d) ( P < 0.05). Iodine nutrition in Chinese adults aged 18–59 years was sufficient, but the salt intake was substantially higher than the WHO and Chinese recommendations. Further policy implementation is needed to reduce salt intake and improve the monitoring of iodine levels in Chinese adults, especially in women of childbearing age.

Methods: A comparative study was carried out in 12 classes which were selected from Dinghai(iodized salt district) and Daishan(non-iodized salt district) of Zhoushan Islands by random sampling. Mann-Whitney test was used to compare the urinary iodine concentration and dietary iodine intake of the two groups. Spearman correlation was used to look for the correlation between urinary iodine concentration and dietary iodine intake in the two groups. Ordinal regression was used to analyze the factors that affect the urinary iodine concentration. Poisson regression was used to analyze the risk factors of goiter.Results: The urinary iodine concentration in non-iodized salt district was 129 μg/L,which was lower than 169 μg/L in iodized salt district(u=6.302,P=0.000) , whereas the daily iodine intake in the two groups was 105 μg/d and 184 μg/d respectively, the difference was also significant(u=4.710,P=0.000). The influence of sex and iodized-salt intake was significant in the final regression model (P0.05), the value of OR was 0.765 and 1.933 respectively. There was no significant difference of daily iodine intake quantity and urinary iodine concentration in the goiter students and normal students (P0.05). The effect of sex and iodized-salt intake on the development of goiter was significant in the Poisson model (P0.05), the values of OR is 2.25, 3.52 respectively.Conclusion: In Zhoushan Island, the special pattern of island diet should be considered. The problem of excessive intake of iodine in island areas should be monitored.
What Iodine Intervention Measures Should Be Taken in Different Water Iodine Areas? Evidence from a Cross-sectional Chinese Survey

Abstract:AbstractThe aim of this study was to assess the population’s iodine nutrition and thyroid diseases in different water iodine areas and to offer suggestions to the governments of different countries to adjust the present policy in different water iodine areas. A cross-sectional survey was conducted in different water iodine areas in China. Urinary iodine, water iodine, salt iodine and thyroid function were determined. The thyroid volumes and nodules were measured by ultrasound. Upon categorization by water iodine level for the 10.0 ~ 39.9 μg/L, 40.0 ~ 100.0 μg/L and 100.1 ~ 150.0 μg/L areas, in adults, the prevalence of subclinical hypothyroidism was 9.28%, 5.35% and 11.07%, and the median urinary iodine (MUI) was 153.7 μg/L, 189.8 μg/L and 317.0 μg/L; in children of the three areas, the prevalence of goitre was 3.83%, 4.47% and 16.02%, and the MUI was 164.1 μg/L, 221.0 μg/L and 323.3 μg/L; in pregnant women of those areas, the MUI was 148.6 μg/L, 176.9 μg/L and 324.9 μg/L. Logistic regression results indicated that low iodine intake was a risk factor for developing hypothyroxinaemia in pregnant women. The iodine status of pregnant women is insufficient in areas with a median water iodine level of 10.0 ~ 39.9 μg/L. Low iodine intake increases the risk of developing hypothyroxinaemia in pregnant women. The iodine status of adults and children is excessive, and the iodine status of pregnant women is above the requirements in areas with a median water iodine concentration of 100.1 ~ 150.0 μg/L. Iodized salt, especially for pregnant women, should be supplied in areas with a median water iodine concentration of 10.0 ~ 39.9 μg/L to improve the iodine status of pregnant women. Supplying non-iodized salt is not enough to protect local residents from the harm from excess iodine in areas with a median water iodine concentration above 100.0 μg/L.

Abstract:Due to great variation in the natural background levels of iodine found across China,the universal salt iodization(USI) policy,which lacks sufficient consideration on this regional variation,is not the most proper strategy for preventing and controlling iodine deficiency disease,and even may increase potential health risk caused by excessive iodine intake.Recently,the Ministry of Health(MOH) of China has amended the USI policy.However,it is still in urgent needs to evaluate and analyze this policy from a perspective of health risk assessment.In this paper,the iodine status of Chinese residents was assessed,and potential morbidity of subclinical hypothyroidism caused by excessive iodine intake in China was determined by integrating the distribution of urinary iodine concentrations of Chinese 8-10 year-old children,dose-response curves and threshold levels.Then the region-specific recommended iodine upper limits in edible salt were calculated based on the 5% benchmark dose,combined with the results of national dietary survey.Results showed that urinary iodine concentrations of national 8-10 year-old children varied greatly in different regions.The geometric mean value and geometric standard deviation were 168.17 and 2.24 μg·L-1,respectively.The morbidity of subclinical hypothyroidism induced by excessive iodine intake reached 4.00%.Recommended iodine upper limit in edible salt for regions with water iodine concentrations below 150 μg·L-1 was 29.62 mg·kg-1.However,for regions with water iodine concentrations above 150 μg·L-1,adequate iodine status has been achieved through intake pathways other than iodized salt,and thus there is no need to consume iodized salt for residents in those regions.These conclusions stay in support of the current salt iodization policy that region-specific iodine content in edible salt should be established according to iodine status of local residents.
Repeated Cross-Sectional Studies on Urinary Iodine and Iodine Content of Salt among School-Aged Children from 2012 to 2014 in Yuhuan County, Zhejiang Province

Abstract:OBJECTIVE:To investigate the iodine status and its change among school-aged children in their morning urine and eating salt from 2012 to 2014 in Yuhuan County, Zhejiang Province, China.METHODS:Three repeated cross-sectional studies were carried out at a same primary school in 2012, 2013 and 2014, respectively. Three classes were randomly selected from each of 3 to 5 grade by the cluster-stratified sampling every year. Totally, 1343 out of 1350 eligible children aged 8 to 10 years old were involved into this study. Their morning urine and salt eating at home were collected and tested.RESULTS:The overall median of urine iodine was 116.0 μg/L, and no significant change was found over year. The overall proportions of subjects with urine iodine < 50 μg/L, < 100 μg/L, and ≥ 300 μg/L were 4.8%, 38.6% and 3.5%, respectively, and there were little differences over year, gender and grade of children. An overall median of iodine from 1343 salt samples was 0.0 mg/kg and no year difference was statistically observed. The proportions of subjects consumed iodized salt significantly decreased from 25.1% in 2012 to 21.8% in 2013 and to 14.2% in 2014. There was a significant difference in urine iodine between subjects taken iodized salt or not and also a weak positive correlation between salt iodine and urine iodine.CONCLUSION:The nutritional status of iodine is overall stable, proper and safety in recent 3 years among school children in Yuhuan County. The coverage rate of iodized salt is very low.

Abstract:Background: A mandatory universal salt iodization (USI) has been implemented in Sarawak, Malaysia since 2008. Objectives: The aim of this study is to assess the current iodine status among school-aged children (SAC) and pregnant women (PW) after 10 years of USI implementation in Sarawak. Methods: This cross-sectional survey among school-aged children and pregnant women was conducted between July and October 2018 in Sarawak. The multistage proportionate to population size sampling technique was used to select 30 schools and 30 maternal and child health care clinics. A total of 1200 children aged 8 to 10 years and 750 first-trimester pregnant women were randomly selected to participate in the study. Iodine excretion level in urine was determined according to the World Health Organization classification. Results: A total of 988 children and 677 PW participated in the study with a response rate of 82.3% and 90.2%, respectively. The overall median urinary iodine concentration (UIC) level among the children was 126.0 μg/L (interquartile range [IQR], 71.0-200.9 μg/L) and classified as adequate iodine status. The median UIC among PW was 123.9 μg/L (IQR, 56.5-192.1μg/L) indicating inadequate iodine status. Conclusion: The present findings indicate that despite adequate iodine status in children, the majority of PW still showed inadequate iodine status. Thus, comprehensive monitoring of the iodine deficiency disorder problem among PW is warranted.

Abstract:OBJECTIVE:To understand the level of urinary iodine and iodine content of salt among school children aged 8 to 10 years in Yuhuan County, Zhejiang Province. METHODS:A cross-sectional study was conducted in a primary school of Yuhuan County purposely selected in 2012. Totally, 450 children aged 8 to 10 years were involved by the cluster-stratified sampling. All subjects received a free physical examination and morning urine and salt using in the household were collected. Urinary iodine and the iodine content of salt the children were tested. RESULTS:Totally, 449 of 450 children provided the morning urine enough to be tested. The median of children's urinary iodine was 114.0 microg/L, and the proportions of them with the urinal iodine < 50 microg/L, < 100 microg/L and > or = 300 microg/L were 4.4%, 37.9% and 2.7%, respectively. Among 447 salt samples collected, the median of iodine content was 0.0 mg/kg. The coverage rate of iodized salt was 27.3%, the qualified rate of iodized salt was 92.6%, and the intake rate of qualified iodized salt was 25.3%. The median of urinary iodine of children with iodized salt was significantly higher than that of those without iodized salt (Z = 3.47, P = 0.001). CONCLUSION:Current nutritional status of iodine is overall proper and safety in the population of school children aged 8-10 years old in Yuhuan County.
Iodine status and knowledge among pregnant and lactating women in Large City in Northern China after nearly 30 years of the universal salt iodization

Abstract:Background: With the implementation of the universal salt iodization, the iodine nutrition for children and adults has been appropriate, but pregnant women are still at risk of iodine deficiency. It is of great public health significance to explore the iodine status and knowledge, and influence factors and the appropriate health education methods among pregnant and lactating women. Methods: From January 2022 to December 2023, at least 50 pregnant women and 50 lactating women were randomly selected from the resident population annually in 16 districts of Tianjin, North China. A total of 1671 pregnant women and 1658 lactating women were recruited. All participants' households salt and random urine samples were collected. A questionnaire was conducted to collect data on iodine related knowledge and behaviors as well as needs of health education from all participants. Logistic regression models were constructed to analyze the factors affecting the iodine related knowledge level. We used the Rasch model and the quadrantal graph to analyze the participants' knowledge level on different iodine-related questions and their needs for health education. Results: The median urine iodine concentration (UIC) of pregnant and lactating women in Tianjin were 152.40 μg/L and 124.60 μg/L. In some districts, the median UIC of pregnant and lactating women below the appropriate range. The iodized salt coverage rate of pregnant and lactating women in Tianjin was 76.12% and 77.40%, respectively. In pregnant and lactating women who did not actively supplement with iodine, the median UIC in those who consumed non-iodized salt were significantly lower than that in those who consumed iodized salt (139.26 μg/L and 154.40 μg/L, P = 0.044; 94.60 μg/L and 123.80 μg/L, P < 0.001). Compared with the low knowledge score group, pregnant women in the high knowledge score group had a higher proportion of iodized salt consumption (71.25% and 78.05%, P = 0.003), and pregnant and lactating women in the high knowledge score group had a higher proportion of actively supplement iodine (44.61% and 55.34%, P < 0.001; 39.26% and 49.78%, P < 0.001). Health education may be the main factor affecting the iodine related knowledge scores for pregnant and lactating women, with adjusted odds ratios (OR) and 95% confidence intervals (CI) of 2.89 (2.30, 3.62) and 2.46 (1.97, 3.07), respectively. Pregnant and lactating women are most expected to acquire knowledge through healthcare professionals (72.11%) and wechat/website (74.91%), respectively. Conclusions: Pregnant and lactating women in some areas of Tianjin are at risk of iodine deficiency. Iodized salt consumption is an important way to ensure iodine nutrition of the population, and the lack of iodine related knowledge is an important factor affecting the consumption of iodized salt. Health education in different ways can be carried out for different people to improve the acceptance and efficiency of health education.
