Abstract:Cardiovascular diseases (CVDs) are the number one health threat globally. Adverse indoor thermal environments are associated with excess mortality caused by CVDs in the cold season. While many studies have focused on the impact of indoor temperature on CVDs, none has considered the fluctuation of indoor temperature. To quantify the effect of indoor temperature on blood pressure and the effect of indoor temperature fluctuation on blood pressure variability (BPV), 172 middle-aged and elderly people in areas that experience both hot summers and cold winters in China completed a household survey regarding their characteristics and living habits. A hierarchical linear model (HLM) was used to analyze the impact of indoor temperature on home blood pressure. A multiple linear model was used to analyze the effect of indoor temperature fluctuation on day-to-day home blood pressure variability. The results showed that there was a significant negative correlation between morning temperature below 18 °C and blood pressure, especially systolic blood pressure (SBP). At the same time, morning temperature fluctuations have an independent influence on BPV, and a deviation of morning temperature fluctuation greater than 1.1 °C significantly increased BPV. Morning temperature and its fluctuation threshold for the rise of SBP and its variability of middle-aged and elderly people were clarified, which can provide a basis for the design, operation, and evaluation of residential thermal environmental health performance for the middle-aged and elderly population in this area, thereby reducing the cardiovascular health risk of the corresponding population.
Susceptibility of Hypertensive Individuals to Acute Blood Pressure Increases in Response to Personal-Level Environmental Temperature Decrease

Abstract:Background: Environmental temperature is negatively associated with blood pressure (BP), and hypertension may exacerbate this association. The aim of this study is to investigate whether hypertensive individuals are more susceptible to acute BP increases following temperature decrease than non-hypertensive individuals. Methods: The study panel consisted of 126 hypertensive and 125 non-hypertensive (n = 251) elderly participants who completed 940 clinical visits during the winter of 2016 and summer of 2017 in Beijing, China. Personal-level environmental temperature (PET) was continuously monitored for each participant with a portable sensor platform. We associated systolic BP (SBP) and diastolic BP (DBP) with the average PET over 24 h before clinical visits using linear mixed-effects models and explored hourly lag patterns for the associations using distributed lag models. Results: We found that per 1 degrees C decrease in PET, hypertensive individuals showed an average (95 % confidence interval) increase of 0.96 (0.72, 1.19) and 0.28 (0.13, 0.42) mmHg for SBP and DBP, respectively; and nonhypertensive participants showed significantly smaller increases of 0.28 (0.03, 0.53) mmHg SBP and 0.14 (-0.01, 0.30) mmHg DBP. A lag pattern analysis showed that for hypertensive individuals, the increases in SBP and DBP were greatest following lag 1 h PET decrease and gradually attenuated up to lag 10 h exposure. No significant BP change was observed in non-hypertensive individuals associated with lag 1-24 h PET exposure. The enhanced increase in PET-associated BP in hypertensive participants (i.e., susceptibility) was more significant in winter than in summer. Conclusions: We found that a decrease in environmental temperature was associated with acute BP increases and these associations diminished over time, disappearing after approximately 10 hours. This implies that any intervention measures to prevent BP increases due to temperature drop should be implemented as soon as possible. Such timely interventions are particularly needed for hypertensive individuals especially during the cold season due to their increased susceptibility.
Personal-level Exposure to Environmental Temperature is a Superior Predictor of Endothelial-Dependent Vasodilatation Than Outdoor-Ambient Level

Abstract:Environmental temperatures influence cardiovascular physiology. However, the majority of time is spent indoors, making outdoor-ambient temperatures inaccurate estimates of true exposures encountered by most individuals. We evaluated in 50 healthy adults the associations between previous 7-day outdoor-ambient (four occasions) and prior 24-hour personal-level (two occasions) environmental temperature exposures with blood pressure, heart rate variability, sleep parameters, and endothelial-dependent vasodilatation (brachial flow-mediated dilatation [FMD]) using generalized estimating equations. Participants (34 females; age, 32.1 +/- 9.6 years) had normal blood pressures (107.8 +/- 13.3/70.2 +/- 9.4 mm Hg), FMD (7.4 +/- 2.8%), as well as sleep and heart rate variability parameters. Mean 7-day outdoor-ambient (4.6 +/- 9.7 degrees C) differed from personal-level temperature exposures (22.0 +/- 3.0 degrees C). Colder outdoor-ambient temperatures (per -10 degrees C) over the previous 1-6 days (rolling averages) were associated with decreases in FMD: -0.57% (95% confidence interval [CI]: -1.14% to 0.01%, P = .055) to -0.62% (95% CI: -1.07% to -0.18%, P = .006). However, a 10 degrees C decrease in personal-level temperature during the prior 24 hours was associated with a greater decrement in FMD: -2.44% (95% CI: -4.74% to -0.13%, P = .038). Both were also linearly related to FMD during all seasons and without a threshold temperature. Other end points were not significantly related to either temperature level in this study. Short-term exposures to colder environmental temperatures reduced endothelial-dependent vasodilatation, supporting the epidemiologic associations with heightened cardiovascular risk. We show here for the first time that temperature exposures characterized at the personal level may be more robust predictors of endothelial function than outdoor-ambient levels. Abstract:Abstract Introduction Evening and nocturnal exposure to cold and warm temperature can respectively blunt or enhance vasodilation needed to initiate sleep. However, the effects of temperature exposure several hours prior to bedtime on sleep have not been well studied. We examined the impact of colder and warmer daytime temperatures on nighttime sleep in men and women. Methods In a 7 to 13-day inpatient study (NCT01568671), the metabolism of twelve men (23.2 ± 4.9 yrs; 23.4 ± 1.6 kg/m2) and fifteen women (25.5 ± 4.0 yrs; 22.1 ± 2.2 kg/m2) was measured in a room calorimeter during exposure to a randomly assigned temperature between 16-31 ̊C from 0800-1300 each day while wearing standardized clothing. Thereafter, ambient temperature was controlled at a thermal neutral temperature (23-25 ̊C), including during nighttime sleep. Diet and physical activity each day were also standardized. Sleep parameters were assessed using the wrist actigraphy on the non-dominant hand (ActiGraph wGT3X-BT; 80Hz) and compared amongst thermal neutral (24.3 ± 0.4 ̊C), coldest (19.1 ± 1.2 ̊C), and warmest (30.4 ± 0.7 ̊C) daytime temperatures. Whole-body thermal insulation was calculated as the inverse of the slope of metabolism versus ambient temperature below the thermal neutral zone. Linear mixed effects models were used to predict each sleep parameter with sex and temperature as fixed effects and each subject as a random effect. Results Over all temperatures, women spent longer time in bed (460.1 ± 58.6 vs. 405.1 ± 68.9 min, p=0.01) and slept longer (427.3 ± 54.1 vs. 372.3 ± 62.0 min p=0.007). No significant interaction was observed between sex and temperature for the sleep parameters. Sleep efficiency after cold exposure was significantly correlated with insulation (R=0.45, p=0.03), which was greater for women than men (0.38 ± 0.04 vs. 0.34 ± 0.04°C/W/m2, p< 0.04). No differences in sleep parameters were observed after warm exposure. Conclusion Our findings suggest that daytime cold exposure may influence nocturnal sleep in a relationship mediated by individual thermal insulation. Future studies will need to assess sleep architecture and skin and core temperature regulations of men and women in response to daytime ambient temperature. Support (if any)

Effect of a Brief Heat Exposure on Blood Pressure and Physical Performance of Older Women Living in the Community—A Pilot-Study

Abstract:Global climate change is affecting health and mortality, particularly in vulnerable populations. High ambient temperatures decrease blood pressure (BP) in young and middle aged adults and may lead to orthostatic hypotension, increasing the risk of falls in older adults. The aim of this study was to evaluate the feasibility of a test protocol to investigate BP response and aerobic capacity of older adults in a hot indoor environment. BP response and aerobic capacity were assessed in 26 community-dwelling older women (median age 75.5 years) at a room temperature of either 20 °C or 30 °C. The protocol was well tolerated by all participants. In the 30 °C condition systolic and diastolic BP (median difference 10 and 8 mmHg, respectively) and distance walked in 6 min (median difference 29.3 m) were lower than in the 20 °C condition (all p < 0.01). Systolic BP decreased after standing up from a lying position in the 30 °C (17.4 mmHg) and 20 °C (14.2 mmHg) condition (both p < 0.001). In conclusion, the protocol is feasible in this cohort and should be repeated in older adults with poor physical performance and impaired cardio-vascular response mechanisms. Furthermore, aerobic capacity was reduced after exposure to hot environmental temperatures, which should be considered when recommending exercise to older people during the summer months.

Abstract:BACKGROUND: Blood pressure (BP) is generally higher in cold than in warm seasons. This seasonal BP change is largely attributable to outdoor temperature changes. However, if such a typical seasonal change is observed in BP measured under a standardized indoor condition remains unclear.METHODS: Resting supine BPs and heart rate (HR) were measured under a standardized room temperature during summer and the next winter in 104 untreated men (38.1 ± 4.4 years). Subjects were classified as having normotension (NT group: n = 79) or mildly elevated BP (ME group: n = 25) according to the summer measurements. Seasonal variation was defined as the difference from winter to summer measurements. We also examined body composition, endocrine parameters, and renal function.RESULTS: Age did not differ in the two groups (37.6 ± 4.2 versus 39.1 ± 4.9 years). The mean seasonal change in systolic BP was 2.7 ± 1.1 mmHg for the NT group and -4.6 ± 1.9 mmHg for the ME group (p = 0.001). Laboratory and outdoor temperatures did not differ between the two groups in either season. HR, noradrenaline, and estimated glomerular filtration rate were significantly higher during winter in the NT group but not in ME group.CONCLUSIONS: Typical seasonal change in BP may be masked in mildly elevated BP measured under a standardized indoor condition. The mechanisms are multifactorial. Our data suggest that out-of-office BP measurements are necessary for correctly understanding seasonal BP change especially in individuals with mildly elevated BP.

Abstract:BACKGROUND AND AIM: Excess wintertime cardiovascular mortality has been observed in populations globally and is partly attributed to cold-induced elevated blood pressure (BP). An inverse relationship between outdoor temperature and BP is well documented. The influence of indoor environmental conditions (e.g., heating, insulation) and indoor temperature on BP is less clear. We examined the effects of indoor and outdoor temperature on wintertime BP and central hemodynamics among older adults in peri-urban Beijing, China. METHODS: We enrolled 1,285 older adults into a panel study conducted in two winter seasons (2018-19 and 2019-2020). Staff traveled to participant homes to administer a questionnaire and measure their brachial and central systolic and diastolic BPs, pulse pressures, and pulse pressure amplification. Indoor temperature was measured within the 5-minutes prior to BP measurements for all participants and 12-month indoor temperature measurements were obtained in a 37% (1st season) and 32% (2nd season) random subsample. Village-level outdoor temperature was estimated from regional meteorological stations. Multivariable mixed-effects regression models were used to estimate the within- and between-individual effects of indoor and outdoor temperature on BP, allowing for effect measure modification. RESULTS:Indoor and outdoor winter temperatures ranged from 0.0 to 28.0˚C and -14.3 to 6.4˚C, respectively. Those in colder homes had a lower wealth score and less home insulation. We observed an inverse linear relationship between indoor temperature and BP (mmHg per 1˚C increase in temperature: −0.4 to −0.5 systolic, −0.3 diastolic, and −0.2 pulse pressure). We found little evidence of any effect of outdoor temperature with BP, or that outdoor temperature modified the effect of indoor temperature. CONCLUSIONS:We found an inverse association between indoor temperature and BP, but no relationship of BP with outdoor temperature. This suggests a stronger effect on BP of indoor compared to outdoor temperature. Improving insulation and household heating may help lower BP and prevent excess winter mortality. We used multivariable mixed-effects regression models to estimate the within-individual and between-individual effects of indoor and outdoor temperatures on BP. Results: Indoor and outdoor temperatures ranged from 0.0 to 28 °C and −14.3 to 6.4 °C, respectively. In adjusted models, a 1 °C increase in indoor temperature was associated with decreased SBP [−0.4 mmHg, 95% confidence interval (CI): −0.7 to −0.1 (between-individual; brachial and central BP); −0.5 mmHg, 95% CI: −0.8 to −0.2 (within-individual, brachial BP); −0.4 mmHg, 95% CI: −0.7 to −0.2 (within-individual, central BP)], DBP [−0.2 mmHg, 95% CI:−0.4 to −0.03 (between-individual); −0.3 mmHg, 95% CI: −0.5 to −0.04 (within-individual)], and within-individual pulse pressure [−0.2 mmHg, 95% CI: −0.4 to −0.04 (central); −0.3 mmHg, 95% CI: −0.4 to −0.1 (brachial)]. Between-individual SBP estimates were larger among participants with hypertension. There was no evidence of an effect of outdoor temperature on BP. Conclusion: Our results support previous findings of inverse associations between indoor temperature and BP but contrast with prior evidence of an inverse relationship with outdoor temperature. Wintertime home heating may be a population-wide intervention strategy for high BP and cardiovascular disease in China.
Hourly Personal Temperature Exposure and Heart Rate Variability: A Multi-Center Panel Study in Populations at Intermediate to High-Risk of Cardiovascular Disease

Abstract:Background: Several studies reported temperature exposure was associated with altered cardiac automatic function, while this effect of temperature on hourly heart rate variability (HRV) among populations with cardiovascular risks was seldom addressed.Methods: We conducted this panel study in four Chinese cities with three repeated visits among 296 participants at intermediate to high-risk of cardiovascular disease (CVD). Real-time temperature level and 24-h ambulatory electrocardiogram were monitored during each seasonal visit. Linear mixed-effects models were used to investigate associations between individual temperature and HRV parameters, and the seasonal effects and circadian effect were also evaluated.Results: We found the overall downward trend of hourly HRV associated with acute exposure to higher temperature. For each 1 degrees C increment in temperature of 1-3 h prior to HRV measurements (lag 1-3 h), hourly standard deviation of normal-to-normal intervals (SDNN) decreased by 0.38% (95% confidence interval [CI]: 0.22, 0.54), 0.28% (95% CI: 0.12, 0.44), and 0.20% (95% CI: 0.04, 0.36), respectively. Similar inverse associations between temperature and HRV were observed in stratified analyses by temperature level. Inverse associations for cold and warm seasons were also observed, despite some effects gradually decreased and reversed in the warm season as lag times extended. Moreover, HRV showed a more significant reduction with increased temperature during daytime than nighttime. Percent change of hourly SDNN was -0.41% (95% CI: -0.62, -0.21) with 1 degrees C increment of lag 1 h during daytime, while few obvious changes were revealed during nighttime.Conclusions: Generally, increasing temperature was significantly associated with reduced HRV. Inverse relationships for cold and warm seasons were also observed. Associations during daytime were much more prominent than nighttime. Our findings clarified the relationship of temperature with HRV and provided evidence for prevention approaches to alleviate cardiac automatic dysfunction among populations at intermediate to high-risk of CVD.
Ambient temperature and ambulatory blood pressure: An hourly-level, longitudinal panel study

Abstract:Background: Variations of blood pressure (BP) related to air temperature have been reported previously; however, no evidence is available regarding the association of hourly ambient temperature with ambulatory blood pressure.Methods: We conducted a longitudinal panel study among 1895 patients from an outpatient department who received repeated ambulatory blood pressure monitoring in Urumqi, China between July 2020 and December 2021. We obtained hourly ambient temperature from the nearest monitoring station to the residential address, and measured 4 ambulatory blood pressure indicators. Linear mixed-effect model combined with distributed lag models were applied to investigate the cumulative associations of hourly temperature with BP.Results: A total of 97,466 valid blood pressure measurements were evaluated. We observed almost linear and monotonically decreasing relationships between temperature and blood pressure. The effects occurred in the same hour, attenuated thereafter and became insignificant approximately 36 h. A 10 degrees C decrease in temperature was significantly associated with increments of 0.84 mmHg in systolic blood pressure, 0.56 mmHg in diastolic blood pressure, 1.38 mmHg in mean arterial pressure, and 0.66 mmHg in pulse pressure over lag 0 to 36 h. Stronger associations were found among patients of female sex, age between 18 and 65 years, overweight or obesity, minority, less education or in the cold season, as well as those without hypertension or with coronary heart disease, or did not take anti- hypertension medication.Conclusion: Our study provides robust evidence that hourly ambient temperature is inversely associated with ambula-tory blood pressure. It also highlights a linear relationship between decreased ambient temperature and elevated BP, which may have implications for the prevention and management of hypertension in susceptible populations.
The acute effects of outdoor temperature on blood pressure in a panel of elderly hypertensive patients

Abstract:Higher level of blood pressure (BP) in winter than in summer has been observed, but the association between temperature and BP and its potential modifiers with adjustment of individual confounders and time trends was rarely explored. We aimed to investigate the association between outdoor temperature and BP and its potential modification factors in a longitudinal panel study in Shanghai, China. From January 2011 to December 2012, we scheduled 54 follow-ups for BP measurements per subject via home visit every other week for 50 elderly hypertensive patients. We applied linear mixed-effect models to analyze the association between temperature and BP after controlling for individual characteristics, antihypertensive medication, comorbidities, and time trends. We evaluated the potential effect modifiers by stratification analyses. For a 1 °C decrease in the average temperature on concurrent day and previous day, systolic BP increased by 0.19 mmHg (95 % confidence interval = 0.06, 0.31) and diastolic BP increased by 0.12 mmHg (95 % confidence interval = 0.03, 0.21). The effect of temperature on BP was stronger among those with older age, female sex, low socioeconomic status, and obese physique. The effect was weak and even null for those taking the angiotensin receptor blockers, angiotensin-converting enzyme inhibitor, or its combination with calcium antagonists. Further, the effect was almost restricted within those having chronic comorbidities. Our results demonstrated that an acute decrease in outdoor temperature was significantly associated with a rise in BP among elderly hypertensive patients, in Shanghai, China. Individual characteristics, antihypertensive medications, and comorbidities may modify this effect.

Abstract:Background: Limited evidence is available regarding the potential heterogeneity of ambient temperature on blood pressure (BP) in various climate zones. Objectives: To explore the effect of daily ambient temperature on BP in various climate zones across 31 provinces in China. Methods: A representative population sample (n = 451,770) were obtained from the China Hypertension Survey study (CHS) from October 2012 to December 2015. Survey seasons were divided into warm and cold seasons. Survey sites were divided into three climate zones as subtropical, temperate monsoon and temperate continental zones. Results: After adjustment for confounders, an overall 10 degrees C decrease in ambient temperature was statistically associated 0.74 mmHg (95% CI: 0.69, 0.79) and 0.60 mmHg (95% CI: -0.63, -0.57) rise for SBP and DBP, respectively. In the warm season. U-shaped exposure-response curves were observed between ambient temperature and BP in temperate monsoon and continental zones, and liner curves in other zones. A greater SBP change due to a lower temperature was observed in subtropical zones, so did DBP in temperate continental zones, especially for cold season. Female, rural and elderly populations were more susceptible to cold weather than their counterparts. Discussion: There is a geographical disparity between temperature and BP across different climate zones, which should been taken into consideration when maintaining stable blood pressure levels. Association Between Ambient Temperature and Blood Pressure and Blood Pressure Regulators: 1831 Hypertensive Patients Followed Up for Three Years

Abstract:Several studies have suggested an association between ambient air temperature and blood pressure. However, this has not been reliably confirmed by longitudinal studies. Also, whether the reaction to temperature stimulation is modified by other factors such as antihypertensive medication is rarely investigated. The present study explores the relationship between ambient temperature and blood pressure, without and with antihypertensive medication, in a study of 1,831 hypertensive patients followed up for three years, in two or four weekly check ups, accumulating 62,452 follow-up records. Both baseline and follow-up blood pressure showed an inverse association with ambient temperature, which explained 32.4% and 65.6% of variation of systolic blood pressure and diastolic blood pressure (P<0.05) respectively. The amplitude of individual blood pressure fluctuation with temperature throughout a year (a 29 degrees centigrade range) was 9.4/7.3 mmHg. Medication with angiotensin converting enzyme inhibitor benazepril attenuated the blood pressure fluctuation by 2.4/1.3 mmHg each year, though the inverse association of temperature and blood pressure remained. Gender, drinking behavior and body mass index were also found to modify the association between temperature and diastolic blood pressure. The results indicate that ambient temperature may negatively regulate blood pressure. Hypertensive patients should monitor and treat blood pressure more carefully in cold days, and it could be especially important for the males, thinner people and drinkers.

Abstract:Introduction: Associations of ambient temperature with cardiovascular morbidity and mortality have been well documented in epidemiological studies. We investigated whether inflammation, coagulation, oxidative stress, antioxidant activity and endothelial function may be among the mechanistic pathways through which ambient temperature influences cardiovascular health. Methods: A panel of 40 college students underwent repeated blood collections for 12 times under three exposure scenarios in Beijing, China in 2010-2011. Ambient temperature data were measured using a HOBO pro V2 logger. A total of 15 plasma circulating biomarkers were measured in the laboratory. We created health indices to indicate the overall levels of inflammation, coagulation, oxidative stress, antioxidant activity and endothelial function based on different combinations of circulating biomarkers. We used linear mixed-effects models to estimate exposure effects of temperature on health indices and circulating biomarkers controlling for air pollution and other potential confounders. Results: We found significant and consistent negative associations between ambient temperature and different health indices. There were increases of 2.1% [95% confidence interval (CI): 0.3, 4.0], 1.6% (95% CI: 0.1, 3.1), 2.8% (95% CI: 0.6, 5.1), 5.0% (95% CI: 3.1, 6.9) and 3.0% (95% CI: 1.3, 4.8) in indices of inflammation, coagulation, oxidative stress, antioxidant activity and endothelial function, respectively, for a 10 °C decrease in daily temperature at 2-d moving average. Most associations between temperature and individual biomarkers tended to be positive but were less consistent in magnitude and strength. Conclusions: Low temperature is associated with increased levels of inflammation, coagulation, oxidative stress, antioxidant activity and endothelial function in healthy adults. Our findings provide novel insights into the mechanistic pathways through which ambient temperature may influence cardiovascular health.
The effect of a change in ambient temperature on blood pressure in normotensives

Abstract:The aim of this study was to investigate the influence of ambient temperature on blood pressure (BP). BP measurements were taken in 20 normotensive volunteers who stayed in Greenland for a 6-week period. Measurements of systolic (SBP), diastolic (DBP) and heart rate (HR) were taken before (3 sessions), during (7–8 sessions) and after the journey (3 sessions). Each session consisted of five BP measurements in the supine position after at least 5 min rest. All five readings were averaged. Temperature data (mean ± s.d.), collected from meteorological services, before, during and after Greenland were 15.7 ± 0.6, 0.5 ± 1.5 and 8.2 ± 0.8°C. SBP values were 116 ± 7.0, 122 ± 7.6 and 116 ± 7.4 and DBP 63 ± 5.2, 66 ± 5.8 and 65 ± 6.5 mm Hg, respectively. HR amounted to 58 ± 7.4, 61 ± 6.7 and 60 ± 7.4 bpm. Significant differences existed between, before and during for SBP and DBP and between, during and after for SBP. Readings were grouped in four categories based on the temperature at the time of reading. For SBP as well as DBP a clear dose-response relationship was demonstrated between low temperature and high BP, although for DBP only a few correlations were statistically significant. Mean correlation coefficients for SBP and DBP against temperature were −0.44 (P < 0.001) and −0.27 (P < 0.005), respectively. our results are in favour of a moderate, but both significant and relevant increase in sbp and dbp when moving from higher to lower ambient temperature.

Abstract:Seasonal variation in cardiovascular functions (CVFs) associated with climatic changes is an important emerging public health issue. The objectives of this study were to demonstrate seasonal variation in CVFs by comparing intra-individual differences between winter and summer among people working in a forest environment and to discuss the possible mechanisms accounting for the health effects of seasonal variation in cardiovascular hemodynamics. A total of 72 staff members of the Experimental Forest of National Taiwan University were recruited for continuous health monitoring during two seasons to investigate the intra-individual seasonal variation in CVFs, complete blood counts, and biochemical examinations. CVFs were assessed by measuring the arterial pressure waveform by a cuff sphygmomanometer using an oscillometric blood pressure device, and aortic stiffness was measured by brachial-ankle pulse wave velocity (baPWV). The results showed that cholesterol levels, white and red blood cell counts, and platelet counts were higher in winter than in summer. Subjects showed not only higher vascular stress, as indicated by higher levels of brachial systolic and diastolic blood pressure (SBP and DBP), central end-SBP and DBP, systemic vascular resistance (SVR), and baPWV, but also lower cardiac activities, including lower levels of heart rate, left ventricular contractility, and cardiac output in winter than in summer. The central and brachial BP, cardiac output, SVR, and baPWV were significantly associated with temperature changes in seasonal variation after controlling related confounding factors. This study provides evidence of higher vascular stress and susceptibility to atherothrombosis during winter compared with summer.

Abstract:We collected data from Kailuan cohort study from 2006 to 2011 to examine whether short-term effects of ambient temperature on heart rate (HR) and blood pressure (BP) are non-linear or linear, and their potential modifying factors. The HR, BP and individual information, including basic characteristics, life style, socio-economic characteristics and other characteristics, were collected for each participant. Daily mean temperature and relative humidity were collected. A regression model was used to evaluate associations of temperature with HR and BP, with a non-linear function for temperature. We also stratified the analyses in different groups divided by individual characteristics. 47,591 residents were recruited. The relationships of temperature with HR and BP were “V” shaped with thresholds ranging from 22 °C to 28 °C. Both cold and hot effects were observed on HR and BP. The differences of effect estimates were observed among the strata of individual characteristics. The effect estimate of temperature was higher among older people. The cold effect estimate was higher among people with lower Body Mass Index. However, the differences of effect estimates among other groups were inconsistent. These findings suggest both cold and hot temperatures may have short-term impacts on HR and BP. The individual characteristics could modify these relationships.
Seasonal Fluctuations of Blood Pressure and Heart Rate in Healthy People: A Meta-Analysis of Panel Studies

Abstract:A meta-analysis was performed to study the seasonal fluctuations in blood pressure (BP) and heart rate (HR) and their dependence on gender, age, and regional climate characteristics. Data on seasonal dynamics of BP and HR in healthy people were taken from 24 panel studies conducted in 23 regions of the globe. The regions were in different climatic zones from subarctic to tropical. The meta-analysis of the data on daily BP values in humans showed that both systolic BP (SBP) and diastolic BP (DBP) were higher in colder seasons compared with warmer seasons. The differences between winter and summer averaged 3.42 [2.00, 4.84] mm Hg for SBP and 2.86 [0.98, 4.74] mm Hg for DBP. Daily HR was maximum in autumn and minimum in spring and summer; the difference averaged no more than 2 bpm. There was no significant difference between office and ambulatory SBP measurements, while seasonal changes in DBP were evident in the case of office measurements, but not in the case of ambulatory measurement. Nighttime values of BP and HR did not display significant seasonal changes. Significant differences in the amplitude of seasonal changes in BP and HR were not observed between men and women or between different age groups of healthy people. Greater seasonal changes in BP were found in regions with lower annual fluctuations in air temperature and more southern regions. Thus, the meta-analysis confirmed the seasonal dynamics in BP and HR in healthy people. Fluctuations are probably caused by the changes that occur in neurohumoral regulation under the influence of weather conditions.

Abstract:Objectives: Mean blood pressure varies moderately with outdoor air temperature in many western populations. Substantial uncertainty exists, however, about the strength of the relationship in other populations and its relevance to age, adiposity, medical treatment, climate and housing conditions.Methods: To investigate the relationship of blood pressure with season and outdoor temperature, we analysed cross-sectional data from the China Kadoorie Biobank study of 506 673 adults aged 30-79 years recruited from 10 diverse urban and rural regions in China. Analyses related mean blood pressure - overall and in various subgroups - to mean local outdoor temperature.Results: The mean difference in SBP between summer (June-August) and winter (December-February) was 10 mmHg overall, and was more extreme, on average, in rural than in urban areas (12 vs. 8 mmHg; P for interaction <0.0001). Above 5 degrees C, SBP was strongly inversely associated with outdoor temperature in all 10 areas studied, with 5.7 (SE 0.04) mmHg higher SBP per 10 degrees C lower outdoor temperature. The association was stronger in older people and in those with lower BMI. At lower temperatures, there was no evidence of an association among participants who reported having central heating in their homes.Conclusion: Blood pressure was strongly inversely associated with outdoor temperature in Chinese adults across a range of climatic conditions, although access to home central heating appeared to remove much of the association during the winter months. Seasonal variation in blood pressure should be considered in the clinical management of hypertension.

Abstract:Seasonal effects on subclinical cardiovascular functions (CVFs) are an important emerging health issue for people living in urban environment. The objectives of this study were to demonstrate the effects of seasonal variations of temperature, relative humidity, and PM2.5 air pollution on CVFs. A total of 86 office workers in Taipei City were recruited, their arterial pressure waveform was recorded by cuff sphygmomanometer using an oscillometric blood pressure (BP) device for CVFs assessment. Results of paried t-test with Bonferroni correction showed significantly increased systolic and diastolic BP (SBP, DBP), central end-systolic and diastolic BP (cSBP, cDBP) and systemic vascular resistance, but decreased heart rate (HR), stroke volume (SV), cardio output (CO), and cardiac index in winter compared with other seasons. After controlling for related confounding factors, SBP, DBP, cSBP, cDBP, LV dp/dt max, and brachial-ankle pulse wave velocity (baPWV) were negatively associated with, and SV was positively associated with seasonal temperature changes. Seasonal changes of air pollution in terms of PM2.5 were significantly positively associated with DBP and cDBP, as well as negatively associated with HR and CO. Seasonal changes of relative humidity were significantly negatively associated with DBP, and cDBP, as well as positively associated with HR, CO, and baPWV. This study provides evidence of greater susceptibility to cardiovascular events in winter compared with other seasons, with ambient temperature, relative humidity, and PM2.5 as the major factors of seasonal variation of CVFs.
