Abstract:This study utilizes guidance from the United States Environmental Protection Agency (USEPA) and the Agency for Toxic Substances and Disease Registry (ATSDR) to calculate the cancer risk to United States Marines who were exposed to carcinogens in drinking water at Marine Corps Base Camp Lejeune. Camp Lejeune is a 233-square-mile Marine Corps training facility in North Carolina. From 1953 to 1987, nearby dry cleaners, landfills, and underground storage tanks contaminated drinking water systems that served Camp Lejeune (ATSDR, 2017). Some of the most toxic contaminants found in the drinking water modeled by ATSDR include benzene, tetrachloroethylene (PCE), trichloroethylene (TCE), trans-1,2-dichloroethylene (DCE), and vinyl chloride (VC). ATSDR utilized MODFLOW and EPANET modeling software to determine the level of contamination in the three main drinking water systems at Camp Lejeune: Tarawa Terrace, Holcomb Boulevard, and Hadnot Point. This paper presents an application of methodology to quantify cancer risk for the Marines who lived and served at Camp Lejeune from 1953 to 1987 using ATSDR's health assessment, chemical contaminant modeling, and USEPA methodology. While VC and TCE were found to be the main risk drivers, benzene and PCE also contributed to the cumulative cancer risk. This analysis shows (1) That the cancer risk was greatest during the 1970s and 1980s and (2) that the inhalation exposure pathway had the greatest contribution to overall cancer risk followed by ingestion, with the smallest contribution from dermal absorption.

Abstract:Background: Drinking water at U.S. Marine Corps Base (MCB) Camp Lejeune, North Carolina was contaminated with trichloroethylene and other industrial solvents from 1953 to 1985. Methods: A cohort mortality study was conducted of Marines/Navy personnel who, between 1975 and 1985, began service and were stationed at Camp Lejeune (N = 159,128) or MCB Camp Pendleton, California (N = 168,406), and civilian workers employed at Camp Lejeune (N = 7,332) or Camp Pendleton (N = 6,677) between October 1972 and December 1985. Camp Pendleton's drinking water was not contaminated with industrial solvents. Mortality follow-up was between 1979 and 2018. Proportional hazards regression was used to calculate adjusted hazard ratios (aHRs) comparing mortality rates between Camp Lejeune and Camp Pendleton cohorts. The ratio of upper and lower 95% confidence interval (CI) limits, or CIR, was used to evaluate the precision of aHRs. The study focused on underlying causes of death with aHRs ≥ 1.20 and CIRs ≤ 3. Results: Deaths among Camp Lejeune and Camp Pendleton Marines/Navy personnel totaled 19,250 and 21,134, respectively. Deaths among Camp Lejeune and Camp Pendleton civilian workers totaled 3,055 and 3,280, respectively. Compared to Camp Pendleton Marines/Navy personnel, Camp Lejeune had aHRs ≥ 1.20 with CIRs ≤ 3 for cancers of the kidney (aHR = 1.21, 95% CI: 0.95, 1.54), esophagus (aHR = 1.24, 95% CI: 1.00, 1.54) and female breast (aHR = 1.20, 95% CI: 0.73, 1.98). Causes of death with aHRs ≥ 1.20 and CIR > 3, included Parkinson disease, myelodysplastic syndrome and cancers of the testes, cervix and ovary. Compared to Camp Pendleton civilian workers, Camp Lejeune had aHRs ≥ 1.20 with CIRs ≤ 3 for chronic kidney disease (aHR = 1.88, 95% CI: 1.13, 3.11) and Parkinson disease (aHR = 1.21, 95% CI: 0.72, 2.04). Female breast cancer had an aHR of 1.19 (95% CI: 0.76, 1.88), and aHRs ≥ 1.20 with CIRs > 3 were observed for kidney and pharyngeal cancers, melanoma, Hodgkin lymphoma, and chronic myeloid leukemia. Quantitative bias analyses indicated that confounding due to smoking and alcohol consumption would not appreciably impact the findings. Conclusion: Marines/Navy personnel and civilian workers likely exposed to contaminated drinking water at Camp Lejeune had increased hazard ratios for several causes of death compared to Camp Pendleton.

Abstract:Background Maternal solvent exposure has been suspected to increase offspring cancer risk. The study aimed to evaluate the associations between maternal residential exposure to solvents from industrial pollution during pregnancy and childhood cancer. Methods The present study included 15,744 cancer cases (aged 0–19 years at diagnosis) identified from California Cancer Registry and 283,141 controls randomly selected from California Birth Registry (20:1 frequency-matched by birth year: 1998–2016). We examined industrial releases of tetrachloroethylene and 1,1,1-trichloroethane within 3 km of the birth address, while we used a 5 km buffer for carbon disulfide. We calculated the total exposure from all linked Toxic Release Inventory sites during each index pregnancy and assigned "ever/never" and "high/low exposed/unexposed" exposure, using median values. We performed quadratic decay models to estimate cancer risks associated with maternal solvent exposure in pregnancy. Results 1,1,1-Trichloroethane was associated with rhabdomyosarcoma (adjusted Odds Ratio (aOR): 1.96; 95% Confidence Interval (CI): 1.16, 3.32) in the "ever exposed" group. Ever exposure to carbon disulfide was associated with increased risks of medulloblastoma (OR = 1.85, 95% CI 1.01, 3.40) and ependymoma (OR = 1.63, 95% CI 0.97, 2.74). Conclusions Overall, our findings suggested maternal residential exposure to solvents from industrial sources might be associated with elevated childhood cancer risks.

Abstract:Purpose: To evaluate whether residential proximity to air emissions of correlated industrial carcinogens, both singly and in combination, are associated with breast cancer incidence. Methods: Using the US Environmental Protection Agency's Toxics Release Inventory, we estimated the 10-year annual average air releases of 26 known or probable carcinogens within 3, 5, and 10 km of Sister Study participants' baseline residences (n = 50,343, 2003–2009). We used Cox proportional hazards regression to estimate adjusted hazard ratios (HR) and 95% confidence intervals (CI) for the association between ambient concentrations of each individual compound and incident breast cancer. To assess mixtures, we applied an exposure continuum mapping (ECM) framework to identify latent mixture profiles via a self-organizing map and assessed whether these profiles were related to incident breast cancer by estimating a joint exposure-response function with generalized additive models. Results: During follow-up (mean = 11.6 years), 4,282 breast cancer cases were identified. The exposure prevalence for compounds emitted within 3-km of participants' residences varied from median vs. no exposure = 2.61, 95% CI: 1.37–5.09). Our application of ECM identified 49 profiles that explained 79% of the variance in emission patterns observed within 3-km of participant residences. Profiles revealed that relatively high levels of exposure to several compounds were rare (<1%) and most participants resided in locations defined by low emissions patterns. Estimation of a joint exposure-response surface indicated that breast cancer was not significantly related to changing patterns of the composition of emission mixtures (P = 0.31). Conclusions: Preliminary results suggest that breast cancer incidence may be related to emissions of certain individual industrial carcinogens, particularly asbestos. Examination of complex exposure scenarios revealed that high simultaneous exposure to many compounds was rare, and that our identified latent patterns of these emissions were not associated with breast cancer.

Abstract:Environmental exposures to chemicals are suspected risk factors for non-Hodgkin lymphoma (NHL), but few studies have assessed historic environmental risk factors. In this study, we estimated the associations between NHL and 1) historic environmental pollutant emissions from the Risk Screening Environmental Indicators (RSEI) model, which uses a database from the Environmental Protection Agency of toxic release emissions to air, water, and land, and 2) chemical mixtures measured in house dust (groups of PCBs, PAHs, and two mixtures of pesticides) for study participants enrolled in the NCI-SEER population-based case-control study (1998-2000) at four SEER centers - Detroit, Iowa, Los Angeles County, and Seattle. We assigned 11 years of annual temporally-varying historic environmental exposure scores by intersecting residential locations from participants' residential histories with a fine grid from the RSEI model and by performing inverse distance weighting between facilities releasing specific carcinogenic chemicals and residential locations for spatially-precise exposure assignments. We used Bayesian index low-rank kriging multiple membership models to identify important lag times for RSEI scores, cumulative effects of RSEI scores, and specific carcinogenic chemical releases into the environment. We found a significant positive association between RSEI scores and NHL at the maximum time lag of 11 years (OR = 1.17, 95% CI (1.06, 1.32)) and a significant cumulative RSEI score effect (OR = 1.30, 95% CI (1.02, 1.84)) for long-term residents in Detroit, where benzene and trichloroethylene were the most important chemicals driving this association. Additionally, we identified significant inverse associations for two study centers and time lags that did not persist in cumulative exposure models. Large weights for dichloromethane and pentachlorophenol in models of cumulative exposure also support evidence for their association with NHL risk. These results underscore the importance of considering historic and cumulative environmental exposures and using residential histories for diseases with long latency periods such as NHL.
Carcinogenic industrial air pollution and postmenopausal breast cancer risk in the National Institutes of Health AARP Diet and Health Study

Abstract:Background: Chemicals emitted from industrial facilities include known or suspected mammary carcinogens and endocrine disruptors, but epidemiologic studies are limited. We evaluated associations between air emissions of multiple carcinogenic chemicals and postmenopausal breast cancer risk in a large prospective U.S. Methods: We used the U.S. Environmental Protection Agency's Toxics Release Inventory to estimate historical airborne emissions (1987-1995) of 19 known and probable carcinogens for participants enrolled (1995-1996) in the NIH-AARP Diet and Health Study. Among 170,402 women, 15,124 breast cancers were diagnosed through 2018. We constructed inverse distance- and wind-weighted average emissions metrics within 1, 2, 5, and 10 km of the enrollment address for each chemical. We estimated multivariable adjusted HRs and 95 % CIs for categories (quartiles, tertiles, medians) of each chemical in association with breast cancer overall and separately by type (invasive, ductal carcinoma in situ) and estrogen receptor (ER) status. Results: We observed an association between benzene emissions and breast cancer risk that was strongest at 1 km (HRQ4 vs. non-exposed = 2.06, 95 %CI: 1.34-3.17; p-trend = 0.001). The magnitude of the association weakened with increasing distance (2 km HRQ4 vs. non-exposed = 1.17, 95 %CI=0.92-1.49; p-trend = 0.19; 5 km HRQ4 vs. non-exposed = 1.05, 95 %CI=0.94-1.16; p-trend = 0.37; 10 km HRQ4 vs. non-exposed = 0.95, 95 %CI=0.89-1.02; p-trend = 0.19) and appeared to be most relevant for invasive rather than intraductal disease. Overall risk was also elevated for vinyl chloride at 5 km (HR≥median vs. non-exposed = 1.20, 95 %CI=1.01-1.43; p-trend = 0.04), but not 2 km or 10 km. We observed suggestive associations for asbestos, trichloroethylene, and styrene in different subgroup analyses, but risk patterns were not clear across distances. Associations with other chemicals were generally null, with limited evidence of heterogeneity by disease type or ER status. Conclusions: An increased risk of breast cancer associated with relatively high levels of industrial benzene emissions warrants additional study, particularly among participants with diverse sociodemographic characteristics that live in areas with higher density of industrial facilities.

Abstract:Purpose: In the U.S. industrial pollution disproportionately burdens under-resourced communities, but little is known about the amount of potential exposure to carcinogenic industrial air pollutants and their associations with cancer risk, especially for female cancers with known racial disparities. We assessed exposures to airborne carcinogenic emissions by race/ethnicity, education, and neighborhood deprivation. Methods: We used a U.S. Environmental Protection Agency nationwide database of regulated chemical emissions from industrial facilities to estimate historical exposures (1987-1995) to 21 known and 30 probable carcinogens for participants in the NIH-AARP Diet and Health Study. This prospective cohort includes 177,806 women enrolled in 1995-1996. We estimated chemical emissions from facilities within 2km of the enrollment residence, adjusted for distance from the home and wind direction. We compared exposure prevalence by race/ethnicity, educational attainment, and a study-specific census tract-level neighborhood deprivation index derived from 20 measures of socioeconomic status (SES) by calculating proportions exposed from each population group. We included these variables in the same ordinal logistic regression model to estimate odds ratios (ORs) and 95% confidence intervals (CIs) comparing the highest category of emissions (highest tertile (T3) or >90th percentile) to the referent group (zero emissions) for all sociodemographic characteristics, additionally adjusted for urbanicity and state of residence. Results: The majority identified as non-Hispanic White (89%), followed by non-Hispanic Black (6%), Hispanic/Latina (2%) and Asian (1%). Despite comprising a smaller proportion of the cohort, Black women were nearly twice as likely (20%) as White women (11%) to live within 2km of an exposure source, whereas proportions among Hispanic/Latina (13%) and Asian women (12%) were similar to those among White women. Similar patterns were observed with low educational attainment (≤high school vs. college educated) and living in areas with high (T3) compared to low (T1) deprivation. In mutually adjusted models, compared to White women, Asian (OR=1.4, CI 1.2-1.5), Black (OR=1.4, CI 1.3-1.5), and Hispanic/Latina (OR=1.5, CI 1.4-1.6) women had 1.4-1.6 times greater exposure burden to any known or probable carcinogen, with similar estimates for benzene, cadmium, dichloromethane, lead, perchlorate, and styrene. Low educational attainment and high neighborhood deprivation were associated with up to 2-fold higher odds of being heavily exposed. Conclusions: We demonstrated notable disparities in environmental exposure to airborne carcinogens by race/ethnicity and individual and neighborhood-level measures of SES. Existing etiologic and mechanistic evidence supports the role of some of these chemicals as carcinogens for sites with known disparities. Future efforts to evaluate associations of these exposures with cancer risk are warranted, particularly in populations with sufficient power to evaluate risk separately by race/ethnicity and SES. Abstract:Abstract The United States Environmental Protection Agency designates superfund sites as regions contaminated with hazardous waste. These sites have previously been associated with adverse health outcomes, including an increased incidence of cancer. Historically, communities with racial and ethnic minorities and people of low socioeconomic status have served as prime locations for these sites, as these communities have lacked the ability to advocate against this land use. Here, we ask whether racial and ethnic minorities are disproportionately exposed to these sites or other regions of hazardous waste in our catchment area at the Case Comprehensive Cancer Center, and in the state of Ohio. Further, we are examining whether areas of high exposure are associated with adverse health outcomes including higher cancer incidence and mortality. We utilized the Environmental Justice Index (EJI) database from the Centers for Disease Control and Prevention to assess, on a census tract level, the percent of racial and ethnic minorities in a region, defined as all persons except white, non-Hispanic individuals. We also queried hazardous and toxic waste sites including regions with a national priority list site, a toxic release inventory site, treatment, storage, and disposal sites, risk management plan sites, coal mines, and lead mines. We intersected these data and found that census tracts with a higher percentage of minority persons were more likely to be exposed to hazardous and toxic sites. Our current studies are assessing whether this high environmental burden is contributing to disproportionately worse cancer burden for these individuals. We will test this by comparing EJI data with data from the Ohio Cancer Incidence Surveillance System database to assess cancer incidence and mortality in Ohio. This will also provide an avenue for identifying specific exposures that may contribute to different cancer types. Further, we plan to assess if persons with low socioeconomic status or those living with disabilities are disproportionately affected by environmental burden. Overall, understanding environmental burden and health outcomes will facilitate the prioritization of communities at greatest need, contributing to greater health equity. Citation Format: Jessica R. We analyzed the effect of different JEM dimensions on the exposure-response relationships between occupational silica exposure and lung cancer risk to investigate how these variations influence estimates of exposure by a quantitative JEM and associated health endpoints. Methods: Using SYN-JEM, and alternative SYN-JEM specifications with varying dimensions included, cumulative silica exposure estimates were assigned to 16 901 lung cancer cases and 20 965 controls pooled from 14 international community-based case-control studies. Exposure-response relationships based on SYN-JEM and alternative SYN-JEM specifications were analyzed using regression analyses (by quartiles and log-transformed continuous silica exposure) and generalized additive models (GAM), adjusted for age, sex, study, cigarette pack-years, time since quitting smoking, and ever employment in occupations with established lung cancer risk. Results: SYN-JEM and alternative specifications generated overall elevated and similar lung cancer odds ratios ranging from 1.13 (1st quartile) to 1.50 (4th quartile). In the categorical and log-linear analyses SYN-JEM with all dimensions included yielded the best model fit, and exclusion of job-specific estimates from SYN-JEM yielded the poorest model fit. Additionally, GAM showed the poorest model fit when excluding job-specific estimates. Conclusion: The established exposure-response relationship between occupational silica exposure and lung cancer was marginally influenced by varying the dimensions of SYN-JEM. Optimized modelling of exposure-response relationships will be obtained when incorporating all relevant dimensions, namely prior rating, job, time, and region. Quantitative job-specific estimates appeared to be the most prominent dimension for this general population JEM.
Long-term exposure to ambient fine particulate matter and risk of liver cancer in the NIH-AARP Diet and Health Study

Abstract:Background: Fine particulate matter (PM2.5) exposure has been associated with liver cancer incidence and mortality in a limited number of studies. We sought to evaluate this relationship for the first time in a U.S. cohort with historical exposure assessment. Methods: We used spatiotemporal prediction models to estimate annual average historical PM2.5 concentrations (1980-2015) at residential addresses of 499,729 participants in the NIH-AARP Diet and Health Study, a cohort in 6 states (California, Florida, Louisiana, New Jersey, North Carolina, and Pennsylvania) and 2 metropolitan areas (Atlanta, Georgia, and Detroit, Michigan) enrolled in 1995-1996 and followed up through 2017. We used a time-varying Cox model to estimate the association for liver cancer and the predominant histologic type, hepatocellular carcinoma (HCC), per 5 µg/m3 increase in estimated outdoor PM2.5 levels, incorporating a 5-year average, lagged 10 years prior to cancer diagnosis and adjusting for age, sex, race/ethnicity, education level and catchment state. We also evaluated PM2.5 interactions with hypothesized effect modifiers. Results: We observed a non-significantly increased risk of liver cancer associated with estimated PM2.5 exposure (Hazard ratio [HR] = 1.05 [0.96-1.14], N = 1,625); associations were slightly stronger for HCC, (84 % of cases; HR = 1.08 [0.98-1.18]). Participants aged 70 or older at enrollment had an increased risk of liver cancer versus other age groups (HR = 1.50 [1.01-2.23]); p-interaction = 0.01) and risk was elevated among participants who did not exercise (HR = 1.81 [1.22-2.70]; p-interaction = 0.01). We found no evidence of effect modification by sex, smoking status, body mass index, diabetes status, or alcohol consumption (p-interaction > 0.05). Conclusions: Our findings in this large cohort suggest that residential ambient PM2.5 levels may be associated with liver cancer risk. Further exploration of the variation in associations by age and physical activity are important areas for future research.
Sociodemographic inequities in the burden of carcinogenic industrial air emissions in the United States

Abstract:Abstract Background Industrial facilities are not located uniformly across communities in the United States, but how the burden of exposure to carcinogenic air emissions may vary across population characteristics is unclear. We evaluated differences in carcinogenic industrial pollution among major sociodemographic groups in the United States and Puerto Rico. Methods We evaluated cross-sectional associations of population characteristics including race and ethnicity, educational attainment, and poverty at the census tract level with point-source industrial emissions of 21 known human carcinogens using regulatory data from the US Environmental Protection Agency. Odds ratios and 95% confidence intervals comparing the highest emissions (tertile or quintile) to the referent group (zero emissions [ie, nonexposed]) for all sociodemographic characteristics were estimated using multinomial, population density–adjusted logistic regression models. Results In 2018, approximately 7.4 million people lived in census tracts with nearly 12 million pounds of carcinogenic air releases. The odds of tracts having the greatest burden of benzene, 1,3-butadiene, ethylene oxide, formaldehyde, trichloroethylene, and nickel emissions compared with nonexposed were 10%-20% higher for African American populations, whereas White populations were up to 18% less likely to live in tracts with the highest emissions. Among Hispanic and Latino populations, odds were 16%-21% higher for benzene, 1,3-butadiene, and ethylene oxide. Populations experiencing poverty or with less than high school education were associated with up to 51% higher burden, irrespective of race and ethnicity. Conclusions Carcinogenic industrial emissions disproportionately impact African American and Hispanic and Latino populations and people with limited education or experiencing poverty thus representing a source of pollution that may contribute to observed cancer disparities.

A systematic review and meta-analysis of haematological malignancies in residents living near petrochemical facilities

Abstract:Abstract Background The petrochemical industry is a major source of hazardous and toxic air pollutants that are recognised to have mutagenic and carcinogenic properties. A wealth of occupational epidemiology literature exists around the petrochemical industry, with adverse haematological effects identified in employees exposed to ‘low’ concentrations of aromatic hydrocarbons (benzene, toluene, ethylbenzene, and xylene). Releases from the petrochemical industry are also thought to increase the risk of cancer incidence in fenceline communities. However, this emerging and at times inconclusive evidence base remains fragmented. The present study’s aim was to conduct a systematic review and meta-analysis of epidemiological studies investigating the association between incidences of haematological malignancy and residential exposure to the petrochemical industry. Methods Epidemiological studies reporting the risk of haematological malignancies (Leukaemia, Hodgkin’s lymphoma, Non-Hodgkin’s lymphoma, and Multiple myeloma) were included where the following criteria were met: (i) Cancer incidence is diagnosed by a medical professional and coded in accordance to the International Classification of Diseases; (ii) A clear definition of fenceline communities is provided, indicating the proximity between exposed residents and petrochemical activities; and (iii) Exposure is representative of normal operating conditions, not emergency events. Two investigators independently extracted information on study characteristics and outcomes in accordance with PRISMA and MOOSE guidelines. Relative risks and their 95% confidence intervals were pooled across studies for the four categories of haematological malignancy, using a random effects meta-analysis. Results The systematic review identified 16 unique studies, which collectively record the incidence of haematological malignancies across 187,585 residents living close to a petrochemical operation. Residents from fenceline communities, less than 5 km from a petrochemical facility (refinery or manufacturer of commercial chemicals), had a 30% higher risk of developing Leukaemia than residents from communities with no petrochemical activity. Meanwhile, the association between exposure and rarer forms of haematological malignancy remains uncertain, with further research required. Conclusions The risk of developing Leukaemia appears higher in individuals living near a petrochemical facility. This highlights the need for further policy to regulate the release of carcinogens by industry. Graphical abstract
