Collateral and Cerebrovascular Reserve Predict the Developement of Hyperperfuson Syndome after Carotid Endarterectomy in Asymptomatic Patients

Abstract:Introduction - In some patients after carotid endarterectomy hyperperfusion syndrome developes. It is characterized by two fold increase in flow on medial cerebral artery associated with hypertension, headache, seizures and in more severe forms transient neurological deficiency , perioperative stroke or hemorrhage. Methods - Research included 97 patients, who were operated from significant asymptomatic extracranial carotid stenosis. Preoperative assesment included: duplex ultrasound ; magnetic resonance angiography (MRA) for estimation of Circle of Willis (CoW) morphology (complete and incomplete CoW) ; transcranial doppler ultrasound (TCD) to estimate collateral flow; measurement of Breath Holding Index (BHI) for determination of cerebrovascular reactivity . Presence of hyperpefrusion syndrome was established by former critera. Binary logistic regression was used to create a model of prediction of hyperperfusion syndome. Results - From 97 patients operated under general anesthesia, 22(22,4%) patients developed hyperperfusion syndrom and transient neurological defciency with complete regression was registred in 4 patients. No perioerative stroke or death was registered. Incomplete CoW, low BHI,no TCD signs of collateral flow, contralateral stenosisocclusion, low compliance to antihypertensive therapy had positive predicive value. Contrary operative technique and time of crossclamping had no effect on developement of hyperperfusion syndome . Derived binary model of prediction p(y=1)= 1/(1+eˆ-(-3.921 +0,357 x presence of diabetes + 0,648 x presence of hypertension + 0.900 x low compliant hypertension+0.57 x degree of stenosis >85% + 1,49 x crossclamping longer than 25 min + 1.231 x no TCD signs on collateral flow + 1.647 x BHI <0,69 + 3.828 x incomplete CoW + 0.118 x intraluminal shunt usage + 2,741 x contralateral occlusion) was statisticaly significant χ2(6)=40,5, p<0,0001 and it correctly classified 54,3% of cases. Conclusion - Hyperperfusion syndrome developes after carotid endarterectomy more frequently in patients with incomplete CoW, low cerebrovascular reactivity and untreated hypertension.
