Abstract:Triceps tendon ruptures are rare injuries and are frequently missed on initial presentation to the emergency department. In cases of complete rupture, surgical repair is recommended but no guidelines exist on the optimum reconstructive technique or rehabilitation. We present a surgical technique and rehabilitation programme for the management of these injuries. A midline posterior incision is performed, the ruptured triceps tendon is identified and mobilized, and the tendon footprint is prepared. The tendon is then repaired using bone suture anchors, with a parachute technique, and held in 40° of flexion. The rehabilitation programme is divided into five phases, over a period of 12 weeks. Range of movement is gradually increased in a brace for the first 6 weeks. Rehabilitation is gradually increase in intensity, progressing from isometric extension exercises to weight-resisted exercises, and finally plyometrics and throwing exercises. Our surgical technique provides a solid tendon repair without the need for further metalwork removal. The graduated rehabilitation programme also helps to protect the integrity of the repair at the same time as enabling patients to gradually increase the strength of the triceps tendon and ultimately return to sport activities.
Diagnosis and treatment of triceps tendon injuries: a review of the literature

Abstract:Objective: To elucidate mechanism of injury, nonoperative protocols, surgical techniques, rehabilitation schedules, and return to sports guidelines for partial and complete triceps tendon injuries. Data sources: The PubMed and OVID databases were searched in 2010 and peer-reviewed English language articles in 2011. Main results: After a fall on an outstretched hand, direct trauma on the elbow, or lifting against resistance, patients often present with pain and weakness of extension. Examination may reveal a palpable tendon gap, and radiographs may reveal a Flake sign. Acute partial injuries have positive outcomes with immobilization in 30-degree flexion for 4 to 6 weeks. Primary repair for complete rupture can restore normal extensor function after 3 to 4 months. Reconstruction returns normal extensor function up to 4 years. Most authors support postoperative immobilization for 2 to 3 weeks at 30- to 40-degree flexion, flexion block bracing for an additional 3 weeks, and unrestricted activity at 6 months. Athletes may be able to return to sports after 4 to 5 weeks of recovery from a partial injury, but return may be delayed if operative tendon repair is performed. Conclusions: Acute partial triceps tendon injuries may be managed conservatively at first and should be repaired primarily if this fails or if presentation is delayed. Reconstruction should first use the anconeus rotation technique. If the anconeus is devitalized, the Achilles tendon may be the allograft of choice.

Abstract:Triceps tendon avulsion is a rare but debilitating condition and the least frequent of all tendon injuries, but it is receiving increasing attention in the literature. The most common mechanism of injury is resisted extension, which is typically seen in a fall onto an extended hand. Such injuries are easily overlooked and should be considered a differential diagnosis in all patients who describe pain and swelling at the posterior aspect of the elbow following a traumatic event. Non-operative management is the general principle for partial rupture as opposed to a variety of surgical treatments for a complete avulsion. The goal of this meta-analysis is to analyse the current literature on triceps avulsion and provide a detailed overview of the occurrence, diagnosis, treatment options and outcomes, comparison of various repair techniques, and consequences of this injury.

Abstract:Rupture of the distal biceps brachii tendon is a rare but debilitating injury, resulting in significant loss of strength. Diagnosis of complete rupture is based on clinical history and physical tests, with surgical repair often recommended due to functional discomfort. Ultrasound (US) is a powerful tool for diagnosing and classifying ruptures, particularly partial ruptures, which is crucial for therapeutic decisions. A good understanding of the tendon's anatomical arrangement in two twisted bundles enables optimal analysis. If several surgical techniques are available, the minimally invasive approach to reinsert the tendon on the radial tuberosity is often preferred. Postoperative assessment includes radiographs, US, and sometimes magnetic resonance imaging, revealing postoperative changes with constant thickening of the repaired tendon. With accurate diagnosis and appropriate surgical technique, surgical repair has a low complication rate. Single-incision repairs show higher rates of re-tears and nerve injuries, and double-incision repairs are prone to heterotopic ossification. Article published online: 19 November 2024 © 2024. Thieme. Abstract:Introduction: Triceps tendon injuries are rare and often caused by direct trauma to the arm. There are no clear guidelines on the management of these and typically partial tears are treated conservatively whilst full thickness tears are treated with primary surgical repair. We aim to review the literature on the methods for triceps repair and propose a novel surgical technique. Methods: A “Medline” and “Embase” literature search of titles and abstracts combining “triceps brachii muscle,” “reconstruct/ed” or “reconstruction” alongside “reconstructive surgical procedures,” and further cross referenced with “repair/s/ed.” Excluded those related to brachial plexus injuries or general elbow trauma and removing duplicate results. 32 English results within 10 years were relevant and reviewed. Results: A 50-year-old gentleman with a 4-month-old full thickness triceps tear was repaired with a novel surgical technique of using an Achilles bone-tendon allograft fashioned into a “shark-fin” pyramidal shape and secured to the proximal ulnar in a lock and key type construct. The tendon was secured to the triceps remnant using a Krackow stitch. Complete radiological and clinical recovery was made by 18 months postoperatively with return to full physical activity. The literature review concluded no consensus in the method of treatment for delayed triceps reconstruction. Conclusion: The use of the bone-tendon allograft specifically shaped to fit congruently into an olecranon osteotomy site allows for direct bone-to-bone healing has not previously been mentioned in the literature. In this case report, we present a young adult female patient with a distal triceps tendon rupture from bouldering treated with open surgical repair technique using a modified bone tunnel and suture anchor fixation technique. A review of the current literature of biomechanical and clinical outcomes of various repair techniques is also presented.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 10_suppl3, October 2024. Injuries to the distal triceps brachii tendon are rare, but they can be challenging to diagnose accurately. While imaging radiographs are commonly used to confirm physical examination findings and guide treatment, magnetic resonance imaging (MRI) is considered the gold standard for diagnostic imaging. However, the accuracy of MRI in distinguishing between partial and full-thickness tears of the distal triceps tendon is not very reliable.A recent study conducted from 2011 to 2015 involving eight patients with nine triceps tendon tears aimed to assess MRI's accuracy in this differentiation. The study revealed that MRI correctly identified all three surgically confirmed complete tears, but its accuracy in detecting partial tears was less reliable, correctly identifying only four out of six cases. Notably, two cases were mistakenly identified as complete tears by MRI but were found to be partial tears during surgery.The characteristics of the distal triceps brachii insertion on the olecranon are fascinating and provide valuable insights into the diagnostic challenges faced by clinicians. Therefore, a recent study included fourteen fresh-frozen elbows were dissected, and a histologic examination was conducted on the distal triceps brachii insertion. Concurrently, T1-weighted elbow MR images of 102 patients without triceps pathology were analyzed.The study found that there are three distinct insertional areas of the distal triceps brachii tendon on the olecranon: capsular, deep muscular, and superficial tendinous. The superficial tendinous insertion exhibited a thickened portion known as the "central cord." MR analysis revealed a cleft between the superficial tendinous and deep muscular insertions, designated as the "lacuna," in some cases. Understanding this anatomy can help surgeons identify partial triceps injuries and avoid iatrogenic injury during surgery.While MRI is valuable in diagnosis, it is not always accurate, and false-positive results are not uncommon. Therefore, it is crucial to combine clinical examination with imaging for accurate diagnosis and treatment planning. These findings highlight the importance of a comprehensive approach that includes clinical assessment, with MRI serving as a valuable adjunctive tool in the diagnostic process and treatment planning for distal triceps tendon injuries.

Distal Triceps Tendon Repair in Strength Athletes Leads to Satisfactory Return to Sports: A Retrospective Analysis of 22 Cases

Abstract:Background: Distal triceps brachii tendon rupture (DTTR) is a relatively rare injury that is common in bodybuilding and high-intensity contact sports and can lead to significant functional impairment of the elbow joint. This study was conducted to evaluate clinical outcomes and the rate of return to sports among competitive bodybuilders and high-demand strength athletes after surgical repair of DTTR. Methods: This retrospective case series was performed in an institutional setting in tertiary health care. Return to sports of 22 competing or high-demand strength athletes (meaning three or more exercises per week) were analyzed pre- and postoperatively after surgical repair of DTTR using a hybrid technique of transosseous sutures and anchor fixation. Descriptive statistics were used to analyze demographic variables, and independent and paired t-tests were used to assess clinical outcomes. Results: The follow-up showed that from pre- to postoperatively, there was no deterioration in the number of sports disciplines (2.4 and 2.5 sporting activities per person, respectively; p = 0.540) or in the frequency of weekly training (4.1 and 4.1 times per person, respectively; p = 0.329). The postoperative visual analog scale for pain (from 6.0 to 1.6, p < 0.001), level of fitness (from 5.1 to 2.6, p = 0.002), and ability to train (from 5.2 to 1.3, p < 0.001) improved significantly. The time of return to sports was 1.5-3 months and 4-6 months after the surgery for ten patients each. The overall rate of return to sports was 95%, whereas 86% returned to the preinjury level of sporting activity. Conclusions: Repair of DTTR leads to high rates of return to sports in competitive athletes.

Abstract:The Achilles tendon is the thickest, strongest and largest tendon in the human body, but despite its size and tensile strength, it frequently gets injured. Achilles tendon ruptures (ATRs) mainly occur during sports activities, and their incidence has increased over the last few decades. Achilles tendon tears necessitate a prolonged recovery time, sometimes leaving long-term functional limitations. Treatment options include conservative treatment and surgical repair. There is no consensus on which is the best treatment for ATRs, and their management is still controversial. Limited scientific evidence is available for optimized rehabilitation regimen and on the course of recovery after ATRs. Furthermore, there are no universally accepted outcomes regarding the return to play (RTP) process. Therefore, the aim of this narrative review is to give an insight into the mechanism of injuries of an ATR, related principles of rehabilitation, and RTP.

Treating medication overuse headache: More than improving headache

Abstract:Medication overuse and medication overuse headache (MOH) are a major healthcare problem (1,2). In some tertiary headache centres, patients with MOH constitute up to 50% of the patients referred. The study by the COMOEASTAS group is a milestone for our understanding of the condition and the benefit of treatment. The multicentre, multinational prospective study recruited 694 patients with MOH and followed them for 6 months after ‘detoxification’ (3). In this context detoxification is misleading because the term implies that patients with MOH are ‘dependent’ on their medication. This might be true for patients overusing opioids or barbiturates, but not for patients overusing simple analgesics or triptans. Patients with overuse of opioids and or barbiturates were excluded from the present study. We still have no concept on the relationship between frequent intake of, for example, triptans and MOH: does the frequent intake of triptan lead to an increase in migraine frequency or are patients using more triptans because the headache becomes more frequent? The COMOESTAS study not only investigated the reduction of headache days after ‘detoxification’, but also the reduction in disability and the most important comorbidity of MOH, namely depression and anxiety. The study showed a highly significant and clinically relevant improvement in disability measured with the MIDAS score, and in depression and anxiety measured with the HADS score. What are the strengths of the paper? The study was multicentre and prospective. The patient number is high, the observation period of 6 months reasonable. Validated instruments such as the MIDAS (3,4) and HADS were used. The study was funded by the EU without involvement from industry. What are the shortcomings? The study had no control group with follow-up but without intervention. Therefore, we cannot exclude that part of the positive outcome is caused by spontaneous improvement or regression to the mean. Ideally, we would need a properly powered randomized trial. About 200 patients were lost to follow-up. The authors fail to provide information on whether patients with psychiatric comorbidity were treated for their condition. It is conceivable that many patients with depression and/or anxiety were not diagnosed prior to study entry and therefore not treated. Another problem is that we still do not know how improvement in headache relates to the improvement of psychiatric comorbidity. A multivariate analysis of baseline variables and outcomes would have been helpful. What are the practical consequences of the study? MOH needs to be identified and treated. My personal approach is to counsel patients with MOH. About onethird of them will be able to reduce the monthly intake of medication to treat acute migraine attacks below the critical threshold. The remaining patients are treated with topiramate or botulinum toxin in addition to behavioural therapy and exercise (5). Patients who fail this approach undergo a formal ‘withdrawal’ regimen in a 1-week day care setting. An important issue is the difference in MOH in the USA and the rest of the world. In the USA many patients with MOH overuse barbiturates and/or opioids (6,7). The prognosis of this group of patients is different from patients overusing simple analgesics or triptans. Therefore, the results of COMOESTAS do not necessarily apply in the USA.

Abstract:Nonunion of olecranon fractures is uncommon in simple fractures, and most cases come from complex injuries, including fracture dislocation. [ 1 ] It is hard to treat surgically because the anatomy of the elbow joint is destroyed with a sclerotic margin of the nonunion fragment, instability, and stiffness. [ 2 ] There is little literature on this topic, and surgical options for nonunion of olecranon fractures depend on various considerations including age, the amount and quality of bone stock, and the degree of articular damage. [ 1 ] Several methods have been previously mentioned. Painless fibrous nonunion with more than 90° range of motion (ROM) can be treated with no surgical treatment. Osteosynthesis with plates and bone grafts is the treatment of choice in young and active patients. Elbow arthroplasty should be used in elderly patients with severe arthritis. However, there are cases that do not fit into these categories, such as those with insufficient bone quality and elbow function to fix, and not much age to proceed with arthroplasty. For these cases, 1 technique that excises the nonunion fragment and reattaches the triceps tendon to the proximal ulna has been considered. Excision of the fragment with triceps advancement is used for older, lower-demand individuals with osteoporotic bone and more comminuted injuries. [ 3 ] In this study, we excised the nonunion fracture fragment with a triceps flap and reattached the tendon to the proximal ulna with suture anchors.

Acute Achilles Tendon Ruptures: An Update on Current Management Strategies

Abstract:Acute rupture of the Achilles tendon is a frequently encountered injury that occurs most commonly in people who participate in recreational athletics into their thirties and forties. For many years, the injury has been treated either nonsurgically or surgically using a standard open approach. More recently, a variety of new surgical techniques have emerged in the literature aiming to improve surgical outcomes while decreasing the potential complications associated with standard open repair. Nonsurgical treatment plans using early weight bearing and accelerated functional rehabilitation have also proven to be excellent management options. Achilles rupture treatment plans can include the use of orthobiologics, such as platelet-rich plasma, with mixed results. Accelerated functional rehabilitation protocols have shown to improve patient outcomes, whether used in conjunction with surgery or nonsurgical management modalities, and reduce the disparity between the injured tendon and the native Achilles.

Abstract:INTRODUCTION: Snapping triceps syndrome is a rare condition that may be misdiagnosed with ulnar nerve instability. It commonly affects young males who complain of painful snap on the medial side of the elbow. This snap appears when the elbow is being extended with resistance, such as during push-ups. Surgical treatment includes anterior transposition of the ulnar nerve and resection of the medial portion of triceps. AIM: To analyze the results of surgical treatment performed on wide-awake patients using local anesthetic without tourniquet. MATERIALS AND METHODS: Twenty-one patients were operated on 26 hands by a single surgeon between 2018 and 2023. Patients were assessed at least 6 months post-surgery via telephone calls, e-mails, and messaging apps. RESULTS: Eleven patients were reached for follow-up. Amount of revision surgeries is 8 in this series with maximum number of 5 in one patient for both hands. Two patients are still having different issues in their elbows. CONCLUSIONS: Snapping triceps syndrome is not easy to treat despite knowledge on this rare condition. The most common reason for revision surgery is persistent snapping even in those patients who were tested for active resisted extension during surgery. However, successful surgery may lead to full return to sport activities as none of our patients complained of loss of triceps power.

Introduction:A chronic patellar tendon rupture in adolescent is very rare but can happen in sport-related injury. Presentation at more than 6 weeks after injury might cause a significant disability. Prompt diagnosis and treatment are key factors to a good prognosis. In this case, we use an autologous semitendinosus tendon graft to repair the patellar tendon rupture.Case Report:A 17-year-old male presented with pain at left knee since 6 months ago. The pain started after he landed on his left knee, having jumped while playing volleyball. On physical examination of the left knee, edema and high patella were found. There was also a palpable gap at the inferior pole of the patella. The patient was unable to do active extension of the left lower leg. MRI evaluation of the left knee revealed a patellar tendon rupture, myositis ossificans and a partial ACL rupture. The patient was scheduled for an elective surgery, consisting of patellar tendon reconstruction using an autologous semitendinosus graft along with ossificans removal and debridement.Discussion:Risk factors of tendon rupture should be assessed with extensive evaluation to construct the appropriate surgical repair method. The decision on which method to use depends greatly on the patient's profile and nature of injury. Immobilization period and routine rehabilitation exercises are vital to an enhanced recovery and fully functional knee.Conclusion:An autologous semitendinosus graft provides a good result without the need of a steel wire, as demonstrated in our case. Cautious caregivers: gender stereotypes and the sexualization of men nurses' touch.

Abstract:AIM The aim of this research was to explore the experience of men nurses and the ways in which gender relations structure different work experiences for women and men in the same profession. BACKGROUND Men are now entering the nursing profession in record numbers and challenging the notion that men are inappropriate in caregiver roles or incapable of providing compassionate and sensitive care. A limitation of the current state of knowledge regarding caring and men nurses is that it is primarily focused on men nursing students, not practising nurses. Little is known about men nurses' practices of caring and how such practices reflect the gendered nature of nursing and nurses' caring work. METHODS The theme of men nurses as cautious caregivers emerged from data that were collected in two rounds of semi-structured interviews with eight men nurses practising in Nova Scotia, Canada. Thematic analysis, informed by feminist theory and masculinity theory, was used as the method for analysing the data. FINDINGS For men nurses, the stereotype of men as sexual aggressors is compounded by the stereotype that men nurses are gay. These stereotypes sexualize men nurses' touch and create complex and contradictory situations of acceptance, rejection and suspicion of men as nurturers and caregivers. They also situate men nurses in highly stigmatized roles in which they are subject to accusations of inappropriate behaviour. For men nurses, this situation is lived as a heightened sense of vulnerability and the continual need to be cautious while touching and caring for patients. Ultimately, this situation impacts on the ability of men nurses to do the caring work they came into nursing to do.

Abstract:The Achilles tendon is the most powerful and longest in our body. 20% of all serious tendon ruptures involve the Achilles tendon (AT). An AT rupture can be a career-ending injury. Proven risk factors predisposing and preventing tendon rupture are presented, as well as epidemiological data on the incidence of this disease in the population. In addition, it illustrates currently used diagnostic methods and presents the benefits they bring. Achilles tendon ruptures have a detrimental impact on athletic performance, and the most appropriate treatment method and return-to-play recommendations remain controversial. Recent studies show that operative intervention improves strength and functional results and is more effective compared to non-operative treatment combined with rehabilitation. Currently, according to the latest research, surgical intervention should be used, especially in athletes and very physically active people, because it shortens the time to return to full fitness and allows for better sports achievements in the future. However, you should remember about the complications that may result from such treatment. Nevertheless, the benefits of surgery outweigh the possible losses, especially to the athletic population. The purpose of this study is to comprehensively review the relationship between surgical repair of a ruptured AT and future athletic performance in an athlete population. The available research shows the effectiveness, side effects, complications, and benefits of surgical correction of a ruptured tendon in the athlete population, as well as the quality of performance they are able to achieve after recovery. Abstract:Novel tandem solar cell structure a-Si:H/HIT has been developed. The fabrication of hetero structure tandem cell (HSTC) has been combined an hydrogenated amorphous silicon (a-Si:H) p-i-n thin film solarp cell for a top cell and a heterojunction with an intrinsic thin layer (HIT) type silicon solar cell for a bottom cell. Using the novel tandem structure, an open circuit voltage of 1.43 V has been obtained. A tunnel recombination junction (TRJ) quality of an amorphous silicon based tandem solar cell can have a significant impact on its performance. Therefore, it is important to understand the TRJ characteristics.

Abstract:Acute rupture of the Achilles tendon (AT) is a common but debilitating injury that requires immediate diagnosis and effective management. Spontaneous bilateral AT rupture is rare; however, it can lead to severe disability for a significant period. This case report presents a 76-year-old patient who suffered a bilateral AT rupture while engaging in a non-strenuous activity. Upon confirmation of the diagnosis by physical examination and radiologic evaluation, conservative treatment was decided due to the presence of numerous comorbidities. A personalized rehabilitation protocol was implemented, allowing weight-bearing activities using Achilles boots at six weeks. Healing of both ATs was confirmed by an MRI at three months. Our case shows that non-operative treatment of these injuries can result in exceptionally favorable outcomes and should not be disregarded. However, thorough patient compliance and surveillance are prerequisites.
