Abstract:AIM To examine the patterns and correlates of mentholated cigarette smoking among adult smokers in the United States. DESIGN Cross-sectional data on adult current smokers (n = 63,193) were pooled from the 2003 and 2006/07 Tobacco Use Supplements to the Current Population Survey. MEASUREMENTS The associations between socio-demographic and smoking variables were examined with gender- and race/ethnicity-stratified multivariate logistic regression models predicting current use of mentholated cigarettes. FINDINGS Multivariate logistic regression analyses demonstrated that black smokers were 10-11 times more likely to smoke mentholated cigarettes than white smokers men: odds ratio (OR): 11.59, 99% confidence interval (CI): 9.79-13.72; women: OR: 10.12, 99% CI: 8.45-12.11). With the exception of American Indian/Aleut/Eskimo smokers, non-white smokers were significantly more likely to smoke mentholated cigarettes than were white smokers. Additional significant factors associated with mentholated cigarette smoking included being unmarried (never married: OR: 1.21, 99% CI: 1.09-1.34; divorced/separated: OR: 1.13, 99% CI: 1.03-1.23), being born in a US territory (OR: 2.01, 99% CI: 1.35-3.01), living in a non-metropolitan area (OR: 0.87, 99% CI: 0.80-0.96), being unemployed (OR: 1.24, 99% CI: 1.06-1.44) and lower levels of education. Race/ethnicity-stratified analyses showed that women were more likely than men to smoke mentholated cigarettes. Among black smokers, young adults (aged 18-24 years) were four times more likely to smoke mentholated cigarettes compared with individuals aged 65+. CONCLUSIONS Race/ethnicity, gender and age are significant correlates of mentholated cigarette smoking among current smokers. Given the importance of menthol in the cigarette market and the potential untoward health effects of this additive, continued surveillance of the prevalence and correlates of mentholated cigarette use among diverse socio-demographic groups is warranted to inform appropriate interventions.
A content analysis of illicit tobacco-related crimes reported in Australian media

Abstract:Introduction: Australian survey and seizure data suggest a growing illicit tobacco market. As an illicit product, accurately tracking trends in illicit tobacco growing, manufacture and sales is challenging. We examined trends in Australian illicit tobacco-related crimes using a content analysis of news articles. Methods: We analysed Australian news articles identified in the Factiva database and government press releases describing Australian illicit tobacco-related crimes reported between January 2000 and December 2023. Sources were coded for crime type, location, product type, dollar value of seized products, methods of distribution or storage, agencies involved, and other crimes involved. Results: We identified 447 crimes reported in 389 sources. The number of illicit-tobacco related crimes reported increased between 2000 and 2023. The most common crimes were possession of illicit tobacco (n=196/43.7%) and smuggling (n=187/41.8%), and the most common product type was 'illicit cigarettes' (n=197/44.1%). The most common distribution/storage method reported was via residential premises (n=98/21.9%). One-hundred and twenty incidents involved other crimes such as financial crimes involving money laundering (n=59/13.2%). Across all included news articles, the quantity of seized products totalled 827,529,307 cigarette sticks, 76,185 cartons, 668,687 packs, 239 hectares (of land growing tobacco plants), and 2,149,000 plants of illicit tobacco between 2000 and 2023. The median value (worth; AUD) of each seizure was $1,500,000 (range $43 to $67,000,000). Conclusions: Australian media reporting on illicit tobacco-related crimes increased over the past two decades, particularly since 2018. These findings highlight a need for improved border detection measures, investment in enforcement and other deterrent activities. Implications: This media analysis confirms trends identified in national survey data that indicate a growing illicit tobacco market in Australia since 2013 with a marked increase since 2018. While survey data suggests that Australian tobacco tax policy, which has included regular large tax increases since 2010, has decreased consumer demand for tobacco overall, it may have also incentivised criminal networks to supply illicit tobacco products due to it being a 'low risk' and 'high reward' activity. Controlling the Australian illicit tobacco market should be a policy priority.

Illicit trade and real prices of cigarettes in Chile

Abstract:Introduction: The tobacco industry claims that tobacco taxes are responsible for increased illicit trade in Chile, which they estimated at 37% in 2022. However, the evolution of cigarette consumption, estimated from population surveys, and of tax-paying cigarettes shows a decreasing penetration of illicit trade since 2018. Methods: A gap analysis was used to estimate the evolution of illicit trade based on an arithmetic identity stating that total national cigarette consumption over a given period is equal to the registered consumption as paying taxes plus the cigarettes that are consumed nationally without paying taxes. Results: Illicit trade penetration in Chile was around 10% in 2020, less than half of what the tobacco industry claimed. In addition, the evolution of real prices of cigarettes, calculated using tax collection data, indicates that real prices net of tobacco taxes increased significantly during 2015-2021, a period with no changes in tobacco taxation. The cheapest cigarettes, presumably competing with illicit cigarettes, registered the most significant price increase. Conclusions: Claims of increasing illicit trade penetration in Chile are unfounded and are not supported by data on consumption and tax-paying cigarettes.

Abstract:Data from the Australian Taxation Office and Australian Border Force show notable recent increases in illicit tobacco seizures across Australia. The illicit tobacco market results in substantial losses in tax revenue, funds organised crime, and perpetuates tobacco use, threatening to undermine Australia's ability to achieve its national commercial tobacco endgame goal of 5 % or less smoking prevalence by 2030. This commentary discusses recent trends in Australia's illicit tobacco trade, reasons why this is of concern, potential drivers of Australians' illicit tobacco use, and policy measures that could be implemented to mitigate increasing illicit tobacco trade such as implementing a track and trace system, increased investment in the Australian Border Force to enhance detection of illicit tobacco shipments at Australia's borders, and encouraging public tip-offs of illicit tobacco sales.

Abstract:Introduction: Illicit cigarettes because of their affordability could increase smoking prevalence, especially among young people. They also cause a large revenue loss for the government. This study aims to estimate illicit cigarette consumption and government revenue loss in Indonesia, a country with a very high smoking prevalence, especially among males. Methods: We estimated illicit cigarette trade in terms of volume and revenue loss. Illicit trade was estimated as the discrepancy between legal cigarette sales and domestic consumption recorded by national representative surveys. Data sources included Basic Health Research Survey, Global Adult Tobacco Survey, National Socioeconomic Survey, and data from Ministry of Finance. Results: We found that illicit cigarette consumption fluctuated from 19 billion sticks in 2007 to 14 billion sticks in 2013, and sharply increased to 59 billion sticks in 2018. Relative to cigarette consumption, illicit cigarettes were the lowest at 5% in 2013 and highest at 19% of consumption in 2018 (assuming 0% underreporting). The estimated government revenue loss ranged from IDR 24.2 to 42.0 trillion (US$ 1668 to 2897 million), which corresponds to 15.8% to 27.5% of cigarette excise revenue in 2018. Conclusions: In Indonesia, illicit cigarette consumption was found to be high and increasing, which contributed to a large government revenue loss (almost onethird of tobacco excise tax revenue). To reduce illegal cigarette production and smuggling, the government should increase resources to enforce the regulation on the excise tax system including stronger penalties, especially related to illicit cigarette production.
[The scale of border trade, tax-free import and tobacco smuggling to Norway]

Abstract:Background: There are no studies of the relative significance in Norway of registered sales, tax-free import, border trade or smuggling of tobacco. Material and methods: The estimated registered sales of tobacco are based on data from the Norwegian customs and excise authorities. The border trade and tax-free import estimates were based on nation-wide, representative surveys of daily smokers aged 16-74 carried out by Statistics Norway for the years 1990-1993 and 1997-2001. There are no detailed data on the scale of smuggling other than confiscation statistics compiled by the customs and excise authorities. It is assumed that confiscations amount to about a tenth of the total amount smuggled into the country. Results: The unregistered consumption of cigarettes and tobacco has been on the rise since the early 1990s; in the years 1997-2001 it accounted for about a quarter of total consumption. Broken down, the figures are as follows: 11% was purchased in Sweden, 5% in Denmark, 9% in other foreign countries; 1% was smuggled into the country. Interpretation: The rise in unregistered tobacco consumption is putting further pressure on the high Norwegian taxes on tobacco. But if taxes were cut, domestic demand would rise, and hence have little or even negative impact on revenue flowing to the government from the legal tobacco market and probably little impact on the levels of imported tobacco through tax-free arrangements or cross-border trade. Hence, although the price gap between Norway and neighbouring countries narrows, we must assume that the motivation to acquire tobacco will remain unaffected while Norwegians continue to travel to Sweden to stock up on inexpensive meat produce.
Odour management plans: a risk-based approach using stakeholder data.

Abstract:Annoyance is a familiar reaction to odours arising from wastewater treatment sites, a reaction which can result in complaints and prosecutions. Odour management plans can be used to assess the extent of nuisance caused and prioritise mitigation measures. Often, these plans have been developed purely on the basis of technical assessments of emission and dispersion, with the general aim of limiting odour concentrations to a specific threshold at some distance from the works. They have often been prepared with inconsistent communication between the site staff and consultant during the development process, and may not directly refer to the affected population. This paper describes a risk-based approach to developing odour management plans, in which mitigation measures are assessed and prioritised on the basis of the likely frequency and intensity of odour exposure. A key element of the approach is the use of knowledge from data gained from key stakeholder groups: customers, staff, and regulators. Emphasis is placed on the development of clear communications between these groups, which helps both in terms of raising awareness amongst operators as to the significance of odour problems, and also in managing the expectations of customers and regulators. The practical application of this approach is demonstrated by the development of odour management plans for wastewater treatment sites by Yorkshire Water Services Ltd. These have used data from staff, customers and regulators to develop risk grids, which are then used to prioritise remediation measures. Involving staff in the data collection operations has been successful in raising awareness of the significance of odours, and has in turn led to low-cost emission reductions through improved housekeeping. Improved communications between operators, customers and regulators has led to more realistic expectations with regard to odour problems, a situation which can reduce conflict and prevent the imposition of stringent and sometimes unrealistic odour standards.

Abstract:Objectives: To gain insight into nonclinical factors potentially influencing tracheostomy practice and determine whether a specialized consultation form impacts tracheostomy utilization. Design: Prospective, observational. Setting: Surgical intensive care unit (SICU). Patients: Patients requiring mechanical ventilatory support. Data abstracted from the Project Impact administrative database served as a practice benchmark. Interventions: Prospective data collection, completion of online survey, and implementation of specialized tracheostomy consultation form. Measurements and Main Results: Data were prospectively collected on 539 patients and 13 attending intensivists. Our SICU tracheostomy rate (54.2%) exceeded that of 18 comparable ICUs participating in Project Impact (13.9%, p < .001). We attempted to identify factors that might account for liberal tracheostomy use. In 41.5% (±0.6%) of patients undergoing tracheostomy, extubation had not occurred despite successful completion of spontaneous breathing trial on ≥1 occasion, a rate that varied significantly among attending intensivists responsible for decision making for this procedure (p < .001). Attending intensivists and postgraduate surgical trainees with SICU experience were surveyed to better understand perceptions of tracheostomy practice. Most respondents (96.1%) reported relying on spontaneous breathing trial to guide decision for extubation, 72.6% estimated that ≤25% of patients successfully completed spontaneous breathing trial but did not proceed to immediate extubation, 86.3% estimated that ≤25% of such patients undergo tracheostomy, and 58.8% felt an acceptable benchmark for this practice was ≤10%. In most survey domains, respondents’ perceptions underestimated actual practice. Implementation of a specialized tracheostomy consultation form did not impact tracheostomy utilization. Conclusions: We identified variation among clinicians with respect to tracheostomy practice as well as discrepancies between perceptions of this practice and actual utilization. These factors may underlie the liberal use of this procedure in our SICU. Processes for providing accurate physician feedback may assist in optimizing tracheostomy use.

Abstract:PURPOSE To examine the relative efficacy of cisplatin-based intraperitoneal (IP) therapy versus carboplatin-based IP therapy as salvage treatment of small-volume residual ovarian cancer. PATIENTS AND METHODS We retrospectively examined the surgically defined response rates of patients with ovarian cancer treated at the Memorial Sloan-Kettering Cancer Center on four organoplatinum-based salvage IP trials (cisplatin/etoposide, cisplatin/cytarabine, carboplatin/etoposide, carboplatin/etoposide + recombinant human erythropoietin). Additional criteria for inclusion in this analysis were: (a) small-volume residual disease (microscopic disease only or largest residual tumor mass < or = 0.5 cm) when IP therapy was initiated; (b) prior response to organoplatinum-based systemic therapy; (c) laparotomy evaluation for response to the IP salvage program. RESULTS The surgically documented complete response rate for patients with microscopic disease treated with cisplatin-based or carboplatin-based therapy was 46% (6/13) versus 38% (6/16), respectively (P > 0.25). In contrast, the surgically documented overall and complete response rates for patients with small-volume macroscopic disease treated with cisplatin or carboplatin were 71% (12/17) versus 32% (6/19) (P < 0.05, chi 2 test with Yates' correction), and 41% (6/17) versus 11% (2/19) (p < 0.1), respectively. CONCLUSION In agreement with experimental data demonstrating that the concentration of platinum within tumor is higher following equimolar doses of cisplatin, compared to carboplatin, we have observed, in this retrospective analysis, a higher surgically documented response rate for patients with small-volume residual macroscopic ovarian cancer receiving salvage cisplatin-based IP therapy. While a randomized trial will be required to definitively address the question of the relative effectiveness of the two commercially available organoplatinum agents for IP treatment of ovarian cancer, our data suggest that cisplatin is the superior agent for regional therapy in this disease.
The impact of tobacco control policies on illicit tobacco trade: A scoping review

Abstract:Introduction: There are concerns that progressively stringent tobacco control policies will result in increased illicit tobacco trade (ITT). We reviewed the international evidence base to identify studies examining the impact of tobacco control polices on ITT. Methods: Guided by JBI scoping review methodology, we searched six databases (PubMed, CINAHL, EMBASE, PsycINFO, Scopus, and Web of Science) in 2022 and 2024 for original research published in English since 2000. One reviewer screened titles and abstracts, and two reviewers screened full-text articles against inclusion criteria. We report the results according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews Checklist. Results: We identified 60 articles examining the impact of eight tobacco control policies or combination of policies on the ITT, including tobacco tax increases or decreases (n=43); menthol bans (n=5); decreasing allowable cigarette filter ventilation levels (n=1); standardised packaging (n=5); tobacco sales bans (n=1); a substantial reduction in the number of tobacco retailers (n=1); a very low nicotine content mandate (VLNC; n=2); and multicomponent policy interventions (n=4). Conclusions: The available evidence is predominantly focused on the impact of tobacco tax increases on the ITT, with equal numbers of studies describing an increase in the ITT or no impact on the ITT, countering claims that tobacco tax increases are the primary driver of increased ITT. Evidence on the impact of other tobacco control policies on the ITT is limited. Collaborative efforts to develop accurate and consistent methods to assess the ITT are needed.

Abstract:BACKGROUND: The tobacco industry claims that illicit cigarette trade in South Africa is high and rising. This is often used as an argument not to increase the tobacco excise tax or to regulate tobacco products.OBJECTIVES: To determine how the tobacco industry's estimates of the size of the illicit cigarette market have changed over time.METHODS: Published media articles were obtained from South African Press Cuttings; published articles and press releases were sourced from the internet. The period of interest is 1990-2012.RESULTS: Between 1990 and 2012 we found 90 newspaper articles and press statements that emphasised the tobacco industry's view on illicit trade. Articles that reported on action taken against illicit trade were excluded. Between 2006 and early 2011 the Tobacco Institute of Southern Africa, a body representing the interests of large cigarette companies, reported that South Africa's illicit cigarette market share was 20%. This share increased to 25% in late 2011 and 30% in 2012. In a 2012 presentation by Tobacco Institute of Southern Africa to National Treasury the illicit market share in 2008 was indicated as 7.9%, compared with claims in that year that the illicit market share was 20%. Industry findings that the illicit market decreased in 2007 and 2008 were not reported in the press.CONCLUSIONS: The tobacco industry has adjusted previous estimates of the illicit trade share downwards to create the impression that illicit trade is high and rising. If previous estimates by the tobacco industry were incorrect the credibility of current estimates should be questioned.

An Electron Microscopical Study of the Interaction of Bone With Growth Hormone Loaded Bone Cement

Abstract:Growth hormone was incorporated into Polymethylmethacrylate (PMMA) with a view to improving the quality of the bone-cement interface. Growth hormone was released from the PMMA and delivered to the cells at the bone-cement interface . The in vivo response to the growth hormone loaded cement was compared to the plain cement , using light microscopy, transmission electron microscopy and scanning electron microscopy, in a rabbit model. The results indicate that growth hormone released at the bone-cement interface stimulated osteogenesis and the reorganization of the tissue components. An advancing mineral front was observed in the direction of the bone cement with new bone formed in direct apposition to cement. This was compared to the interface with plain PMMA cement , which showed little organization of the tissue components , and spaces between the bone and the cement containing areas of fibrous tissue.

Abstract:Background: Illicit cigarette trade has significant economic and public health implications. It leads to governments tax revenue losses due to the evasion of tobacco taxes, and often these cigarettes are cheaper ones therefore increasing cigarette consumption. Objective: To estimate the Illicit cigarette trade and tax evasion in Zambia and establish its associated factors Methods and analysis: A cross sectional survey was used to collect empty cigarette packs from the retailers and street/bins in 25 districts covering 10 Provinces of Zambia. We used a descriptive analysis to calculate the proportion of illicit cigarette packs and other specific criteria. Logistic regression was used to model the factors associated with the prevalence of the illicit cigarette market in Zambia. Results: Conclusions: Our study demonstrated that 12.2% of the cigarettes sold on the Zambian market is illicit, with 10.1% evading tax. We found that cigarettes from Lusaka province, urban regions, border towns, and those that are imported had higher odds of being illicit. This finding underscores the fact that Zambia should ratify and implement the WHO Protocol on Illicit Tobacco Trade (ITP) to counter the supply of illicit cigarettes.

Abstract:Computer-driven cathode-ray tube (CRT) displays are becoming an important means of on-line man-machine communication, particularly for graphical input/output in laboratory investigations of computer-aided design techniques. Their operation, however, often requires so much of the computational resources of the associated computer that they are not yet considered economic or practical for general industrial use. This paper discusses the systems engineering problems in designing and using display systems, with emphasis on the hardware-software tradeoffs. As an example, a display specifically developed for computer-aided design applications is described which has unusual special-purpose computing capabilities for dynamic picture manipulations, including rotation, scaling, and translation of 3-dimensional images. it is concluded that there is much work ahead, and that the proper hardware-software organization for these complexes of computers, communication links, terminals, and men is a fertile field for the systems engineer.

Abstract:Objective To evaluate the common industry claim that higher tobacco taxation leads to higher levels of smuggling, particularly in a limited state capacity setting. Design This paper evaluates the effects of a tobacco tax increase in Sierra Leone on smuggling by using gap analyses. Its models are based on multiple rounds of the Demographic and Health Survey and customs data as well as newly collected data on cigarette prices. Results The paper shows that despite a substantial increase in cigarette taxation, and despite the absence of other formal tobacco control policies, smuggling has not increased in Sierra Leone. Its primary model shows a decrease in cigarette smuggling by 16.74% following the tax increase, alongside a decrease in cigarette consumption more widely and an increase in tax revenue. Conclusions By presenting a low income and lower enforcement capacity case study, this paper provides novel and critical evidence to the debate on the tax-smuggling link. Furthermore, it points to new questions on how states in these contexts can limit cigarette smuggling.

Improving estimates of the illicit cigarette trade through collaboration: lessons from two studies of Malaysia

Abstract:This paper critically analyses contrasting estimates of Malaysia’s illicit cigarette trade in 2011, 2015 and 2019 by Bui et al and Koya et al who previously produced independent estimates at about the same time using tax gap analysis. Collaboration between the two authors’ teams emerged due to the discrepancies in their results, generating this paper to explore the methodological issues identified and hence produce revised estimates of the rate of illicit. Key issues identified were: Bui et al ’s assessment of legally imported cigarettes impacting all years; their exclusion of ad valorem duty affecting the 2011 and 2015 estimates; Koya et al overlooked the value of cigarettes for export market in their ad valorem calculation and used the sales value of imported tobacco/tobacco products, not just cigarettes, both of which impact estimates for 2011 and 2015. Recalculations using Koya et al ’s consumption data reveal that in 2019, illicit cigarettes accounted for about 70% of the market, which is higher than Bui et al ’s estimate (38%) but slightly lower than Koya et al ’s (72%). For 2011 and 2015 where ad valorem applied, the corrected estimates show a share of the illicit cigarette market of approximately 41.1% and 52.7%, respectively, differing from Bui et al ’s 0% in 2011 and 29.6% in 2015, and Koya et al ’s 51% in 2011 and 55% in 2015. This paper provides essential lessons for addressing methodological issues between authors’ teams and updated estimates of Malaysia’s illicit cigarette trade, verifying that Malaysia faces a substantial illicit cigarette trade problem.

Abstract:Objective To estimate lost excise and value-added tax (VAT) revenue as a result of illicit cigarette trade from 2002 to 2022. Design Using gap analysis, we estimated the number of illicit cigarettes by calculating the difference between the number of self-reported cigarettes (derived from nationally representative surveys) and the number of legal (tax-paid) cigarettes (derived from government sources) from 2002 to 2022. We then calculated the excise and VAT revenue that the government lost through illicit trade, taking into account that some people would have quit or reduced their consumption if cigarette prices had been higher (ie, tax paid). Setting South Africa. Outcome measures Illicit trade estimates and lost revenue estimates. Results The illicit cigarette market comprised 5% of the market in 2009, peaked at 60% in 2021, and decreased to 58% in 2022. Accounting for the fact that some people would have reduced their consumption if cigarette prices had been higher (had the illicit marke not existed), the government lost R15 billion in excise revenue and R3 billion in VAT revenue in 2022. From 2002 to 2022, the government lost R119 billion (2022 prices) in excise and VAT revenue. The majority of the lost revenue occurred in the period 2010 to 2022, where R110 billion (2022 prices) in excise and VAT revenue was lost. A comprehensive sensitivity analysis indicates that the estimated lost revenue of R119 billion from 2002 to 2022 falls within the range of R65 billion to R130 billion (all 2022 prices). Conclusions The South African government has been losing a significant amount of revenue by not receiving excise and VAT from all cigarettes consumed in South Africa. This trend is likely to continue if the government does not secure the supply chain from the point of production to the point of sale.
