Abstract:Introduction This case report details the presentation of a 62-year-old Haitian female residing in a private residence with her son and separated son's father, with a history of paranoid schizophrenia and a psychiatric diagnosis of psychosis attributed to dysregulation due to noncompliance with medication. Emergency Medical Services initially transported the patient to the emergency room for bizarre behavior and paranoia noted by the son. Subsequently, she was referred to Psychiatric Emergency Services (PES) and later admitted to the inpatient unit for stabilization and safety. Upon initial evaluation, the patient was diagnosed with Capgras syndrome, characterized by delusional misidentification. She described an unknown man in her house, intermittently present, who she believed called the police while using her kitchen. Despite acknowledging living with her son's father, she could not identify this man as her son's father. During the assessment, she was observed wearing a cotton bud in each ear, attributing it to protection against mischievous entities called Jablou, associated with the Vodou religion. The patient's cultural background and religious beliefs, rooted in her upbringing in Haiti, prompt consideration of whether her auditory and visual experiences are manifestations of hallucinations or culturally influenced phenomena. The patient's belief in being cursed aligns with the cultural context, associating mental illness expressions with vodou curses. Despite acknowledging the curse, she denies having a mental illness, attributing her health issues to the curse, indicating a nuanced understanding. Methods This comprehensive case report extends its examination by contextualizing the patient's presentation within the broader framework of idioms of distress prevalent in Haitian culture. Drawing from research articles focusing on the Haitian population, the concept of "idioms of distress" emerges as a crucial lens to understand how diverse cultural groups express and experience distress. Results Two primary idioms of distress are delineated within Haitian culture, providing valuable insights into the societal responses to mental health challenges. The first idiom revolves around economic or interpersonal problems. This expression of distress is met with community support and lacks the stigma associated with the second idiom. In contrast, the second idiom, characterized by 'crazy' or 'moun fou,' is marked by a strong stigma. Individuals associated with this idiom are believed to be under a recent curse or have experienced a lapse in religious piety, resulting in community harassment and the belief that they will be confined to a restricted place. In the case context, the patient's behaviors during her hospital stay shed light on her unique manifestation of distress. Her fixation on a substantial sum of money, repeatedly expressing concern about its whereabouts, serves as a distinctive idiom of distress. This preoccupation becomes intertwined with her emotional state as she associates her moods with possessing this money. This narrative reflects the intricate interplay between cultural influences and psychiatric symptoms. Furthermore, discussions surrounding medication reveal additional layers of the patient's concerns. Fearful of being confined indefinitely, the patient expresses reluctance toward accepting medication, attributing her mental health issues to a Vodou demon curse. This underscores the impact of cultural beliefs on treatment adherence and highlights the importance of culturally sensitive interventions. Conclusions This case report underscores the necessity of recognizing and navigating the complexities of idioms of distress within a specific sub-context to improve outcomes, emphasizing the critical role cultural norms and beliefs play in shaping individuals' expressions of mental health challenges and their responses to treatment options. It also highlights the relevance of cultural factors to all aspects of mental health care, demonstrating the need for person-centered cultural formulations in diagnostic and treatment planning. Use of the questions from the Cultural formulation interview (CFI) can be used to bridge patient and provider understanding even in acute clinical settings and ensure patient centered care. Bringing in a patient's perspective of the symptoms of mental illness taking into consideration the cultural meanings of health, illness, and treatment will lead to a successful shared decision making and better outcome.
