Abstract:A significant proportion of college students are adult children of an alcoholic parent (ACoA), which can confer greater risk of depression, poor self-esteem, alcohol and drug problems, and greater levels of college attrition. However, some ACoA are resilient to these negative outcomes. The goal of this study was to better understand the psychobiological factors that distinguish resilient and vulnerable college-aged ACoAs. To do so, scholastic performance and psychological health were measured in ACoA college students not engaged in hazardous alcohol use (resilient) and those currently engaged in hazardous alcohol use (vulnerable). Neural activity (as measured by functional magnetic resonance imaging) in response to performing working memory and emotion-based tasks were assessed. Furthermore, the frequency of polymorphisms in candidate genes associated with substance use, risk taking and stress reactivity were compared between the two ACoA groups. College ACoAs currently engaged in hazardous alcohol use reported more anxiety, depression and posttraumatic stress symptoms, and increased risky nicotine and marijuana use as compared to ACoAs resistant to problem alcohol use. ACoA college students with current problem alcohol showed greater activity of the middle frontal gyrus and reduced activation of the posterior cingulate in response to visual working memory and emotional processing tasks, which may relate to increased anxiety and problem alcohol and drug behaviors. Furthermore, polymorphisms of cholinergic receptor and the serotonin transporter genes also appear to contribute a role in problem alcohol use in ACoAs. Overall, findings point to several important psychobiological variables that distinguish ACoAs based on their current alcohol use that may be used in the future for early intervention.

Abstract:Disturbed or impoverished relationships have been identified as a predominant feature of adult children of alcoholics (ACOAs). Recently, the concept of co-dependency has been advanced to depict the interpersonal difficulties of these individuals, but lacks definitional rigor. The notion that childhood experiences with parents are 'carried forward' to affect attitudes and behaviors relevant to later intimate relationships is also consistent with attachment theory. This study examined the patterns of attachment of ACOAs as compared to adult children of non alcoholics (ACONAs). A cross-sectional sample of 203 individuals attending either a short term hospital based outpatient psychiatric program or a community based alcoholism treatment program was included in this study. Results supported the hypothesis that female ACOAs had a distinctive dysfunctional attachment profile. There were no significant differences in the attachment styles of male ACOAs as compared to ACONAs, or male substance abusers as compared to non-abusers.

Abstract:Alcoholism, medically referred to as alcohol use disorder, is a condition associated with harmful consumption of alcohol. Misuse of alcohol implies danger and harm, not only to oneself, but also rather extends to affect one's children. The household characteristics, parenting style, and the environment that parents with alcohol dependence disorder create for their children have a substantial effect on their psychological, behavioral, and developmental growth. The psychological impact on children of alcoholics (COAs) can be studied through personality traits of neuroticism, impulsivity, and sociability, also through the externalizing and internalizing behaviors. COAs are also at risk of developing psychological disorders such as oppositional defiant disorder, conduct disorder, and eating disorders. The development of these children is affected both biologically and psychologically, beginning at early pre-natal life to post-natal mortality. Additionally, there is a high risk of developing fetal alcohol syndrome, impaired physical development, and low intelligence quotient scores. Regarding the behavioral domain, COAs were found to be hyperactive and inattentive during school-age, tend to consume alcohol early in teenage years, and are more likely to develop poor coping mechanisms and impaired emotional venting. In addition, COAs were found to have a higher risk of developing personality problems and abusing substances, specifically alcohol. It is evident that adult COAs are at a greater risk of developing depression, and attempting suicide. Adult COAs also have a higher chance of failed relationships and tougher time finding jobs as compared to children of non-alcoholics. Moreover, it was noted that they have emotional expression difficulties, Intimacy issues, and inability to trust others or even themselves. As far as addressing COAs, identifying their need of help is the most crucial step. The primary approach to treatment is cognitive behavioral therapy.

Abstract:Background: Adverse childhood experiences (ACEs) increase the odds of alcohol use disorder (AUD). Aim: To study the ACEs, coping, and resilience in persons with AUD and their non-drinking siblings from high-density families. Methods: The study used a case-control study design. Using purposive sampling, 135 participants were selected; the sample consists of persons with AUD (n = 45), non-drinking siblings (n = 45), and healthy controls (n = 45), selected from out-patient and in-patient services from a government-run de-addiction centre in Bengaluru. Individuals were administered an ACEs questionnaire, Brief-COPE, and Connor-Davison Resilience scale. Descriptive statistics, Friedman's test, and Bonferroni's post-hoc test, Binary Logistic Regression were used for analysis. Results: ACEs, coping, and resilience significantly differ across the three groups. Persons with AUD and their non-drinking siblings are comparable in terms of ACEs and having dysfunctional family members. Non-drinking siblings and healthy controls have similar coping and resilience. None of the healthy controls had dysfunctional family members. Conclusion: ACEs are more prevalent and more frequent in persons with AUD. Individuals with AUD showed higher avoidant coping and lower resilience than their non-drinking siblings and healthy controls. Early identification of ACEs and interventions to build resilience and coping strategies could prevent individuals from developing AUD in high-density families.
The Relationship between Attachment Styles and Psychological Distress in Adult Children with Alcoholic Fathers

Abstract:Research suggests that a causal relation exists between alcoholism in fathers and the resulting personality and behavior of the child. Correlational research design was used to explore two variables and purposive sampling was considered the best sampling strategy for a sample of 202 males and 198 females. For the participants, age (18-25) and education (tenth grade as minimum level of education) were the two variables that were controlled. Any child of alcoholic father above or below the age bracket given and possessing lower than 10th grade of education was not selected. Demographic sheet, General health Questionnaire and Adult Attachment Questionnaire were filled out by the participants. A one-way Analysis of Variance revealed significant differences between all three groups. Those with secure attachment styles reported the least psychological distress, followed by the avoidant attachment style group. Those falling into the anxious/ambivalent attachment style reported the highest levels of psychological distress. The results were discussed in the light of the literature review. The alcoholic consumption of the parents and their abusive behavior towards the children can manifest as fear of intimacy, unstable relationships, and lack of confidence and self-worth for the victims. It is further proposed that these problems can carry through generations if left unchecked, creating a plethora of damaged individuals.

Abstract:This study examined the relation between adult attachment and drinking-related problems among college students and how maladaptive, insecure attachment-based coping strategies and alcohol consumption explained such link. A total of 257 college students completed research questionnaires. Results indicated that individuals with a history of family addiction reported significantly more alcohol consumption and drinking-related problems. In addition, findings indicated significant indirect effect paths from both attachment avoidance and attachment anxiety to maladaptive deactivating coping, to greater alcohol consumption, and to drinking-related problems. On the other hand, high attachment anxiety was associated with more alcohol consumption which in turn, led to more alcohol-associated problems, but maladaptive hyperactivating coping was not associated with either alcohol consumption or drinking-related problems. Implications of findings, limitations, and future research directions are discussed.

Abstract:Abstract Context: Children of alcoholics face risks of problem behavior during childhood and mental health problems in adult life. Identifying these problems at the earliest can significantly reduce the problems in future. Aim: We aimed to study psychological impact on adolescent children of parents with alcohol use disorder. Settings and Design: This is a cross-sectional study done at a tertiary care hospital in Maharashtra. Materials and Methods: Convenience sampling technique was used. One hundred adolescent children of alcohol use disorder patients were evaluated for problem behavior and perceived stress using the Child Behavior Checklist (CBCL) and Perceived Stress Scale, respectively. Current status of severity of alcohol use in parents was assessed using the Severity of Alcohol Dependence Questionnaire. Statistical Analysis Used: Data were analyzed using the Statistical Package for the Social Sciences Software Package (version 20.0). Results: In our study, we found that children whose fathers had severe alcohol dependence were exhibiting more externalizing behavior on CBCL and higher perceived stress and it was statistically significant. Conclusion: A positive correlation was found between severity of alcohol use in fathers and problem behavior in children.

Abstract:Alcohol problems have variable outcomes for marital relationships depending on whether drinking patterns are concordant or discordant among the members of the dyad; however, it is unclear what impact these variations in drinking patterns have on children. The current study was designed to explore several gaps and limitations in the parent heavy drinking literature. In particular, the prospective associations over 3years between parent heavy drinking, parenting, and child externalizing behavior were investigated in an integrated model to examine the influence of concordant and discordant drinking within couples on subsequent outcomes for their children. The study consisted of 180 couples recruited by mailings with children primarily between the ages of 4 and 11years old (52% male children). Parent-report of marital conflict, parenting, alcohol use, and child externalizing behavior were measured in a longitudinal study. Actor-Partner Interdependence Model analyses were conducted. Higher levels of maladaptive parenting were associated with higher externalizing for children of concordant drinking couples as opposed to discordant drinking couples. Implications for research and practice are discussed, including investigating mediators and moderators of the current findings such as quality of the parent-child relationship.

Abstract:Individuals consume alcohol for a variety of reasons (motives), and these reasons may be differentially associated with the types of drinking outcomes that result. The present study examined whether specific affect-relevant motivations for alcohol use (i.e., coping, enhancement) are associated with distinct types of consequences, and whether such associations occur directly, or only as a function of increased alcohol use. It was hypothesized that enhancement motives would be associated with distinct problem types only through alcohol use, whereas coping motives would be linked directly to hypothesized problem types. Regularly drinking undergraduates (N = 192, 93 female) completed self-report measures of drinking motives and alcohol involvement. Using structural equation modeling, we tested direct associations between coping motives and indirect associations between enhancement motives and eight unique alcohol problem domains: risky behaviors, blackout drinking, physiological dependence, academic/occupational problems, poor self-care, diminished self-perception, social/interpersonal problems, and impaired control. We observed direct effects of coping motives on three unique problem domains (academic/occupational problems, risky behaviors, and poor self-care). Both coping and enhancement motives were indirectly associated (through use) with several problem types. Unhypothesized associations between conformity motives and unique consequence types also were observed. Findings suggest specificity in the consequences experienced by individuals who drink to cope with negative affect versus to enhance positive affect, and may have intervention implications. Findings depict the coping motivated student as one who is struggling across multiple domains, regardless of levels of drinking. Such students may need to be prioritized for interventions.

Associations Between Transdiagnostic Cognitive-Affective Vulnerability Factors, Negative Reinforcement Drinking Motives, and Problematic Alcohol Use among Undergraduates

Abstract:Anxiety sensitivity, distress tolerance, and emotion regulation difficulties have each demonstrated significant individual associations with problematic alcohol use and negative reinforcement motives for alcohol use among college students. However, extant research has yet to examine these three factors simultaneously with regard to the possibility of differential associations with alcohol consumption, alcohol-related problems and coping and conformity motives for alcohol use. As such, the present study sought to examine whether such differential associations exist within a sample of undergraduates reporting past year alcohol use. Methods: Participants were 379 undergraduate students reporting alcohol use in the past year who completed self-report measures for course credit. Results: After controlling for the effects of sex, lifetime marijuana use status, and negative affectivity, greater anxiety sensitivity social concerns and difficulties with emotional awareness were associated with more alcohol-related problems. Greater anxiety sensitivity social concerns and impulse control difficulties were associated with greater conformity alcohol use motives, and greater impulse control and emotional clarity difficulties were associated with greater coping motives. Conclusions: These findings suggest that greater fears of anxiety symptoms because of their potential negative social consequences and certain emotion regulation difficulties (i.e., impulse control, emotional clarity, emotional awareness) may be particularly problematic because they are associated with alcohol-related problems and negative reinforcement motives for use among undergraduates.

Abstract:Adult children of alcoholics are adults who spent a part of, or their whole childhood in a dysfunctional family, where the biggest problem was alcohol addiction in one or both parents. In families with parental alcoholism, there is usually a lot of dysfunction in interpersonal relationships and in the upbringing of the children, which does not provide a healthy and optimal developmental environment for the child. There is often physical and psychological violence, and other forms of abuse and neglect that children perceive as traumatic. All this leaves the child with severe consequences, which they also struggle with in adulthood. Unresolved and traumatic childhood content often remains repressed and unprocessed and helps shape one’s functioning in adulthood, which is frequently emotionally and socially immature. Adult children of alcoholics often have problems in experiencing and regulating their emotions, as they had to carry many emotional burdens in a dysfunctional family, while they had no real opportunity for the healthy development of emotional regulation. The article will present research on the emotional experience of children with their alcoholic parents and how they recognize related consequences in their adult lives. Using the content analysis method, we analyzed 71 anonymous forum posts on the counselling forum on the topic “Adult children of alcoholics.” The directed approach to content analysis was used to validate forum posts by people who described their childhood experiences with an alcoholic parent. We identified parts of the content that fell into two predetermined categories: emotional experience in childhood with an alcoholic parent and the experience of its consequences in adulthood. The results showed that the adult children of alcoholics mostly experienced severe feelings of fear, shame, sadness and disgust with their alcoholic parents in their childhood, and that these feelings have remained unprocessed. In adulthood, they struggle with negative consequences in the personal sphere (e.g. poor self-esteem, inferiority, anxiety, depression), in interpersonal relationships (e.g. problems in partnerships, mistrust, social phobia, parental stress, complicated relationships with parents) and in everyday functioning (e.g. coping with one’s own addiction, dysfunctional behavioural patterns), but they also recognize that because of this experience they have managed to lay the foundations of their lives differently and better. The results confirm that children are hidden victims of parental alcoholism and justify the need for psychosocial and therapeutic support even in their adulthood.
Mortality after Inpatient Treatment for Severe Pneumonia in Children: a Cohort Study

Abstract:Abstract Background Although pneumonia is a leading cause of inpatient mortality, deaths may also occur after discharge from hospital. However, prior studies have been small, in selected groups or did not fully evaluate risk factors, particularly malnutrition and HIV. We determined 1‐year post‐discharge mortality and risk factors among children diagnosed with severe pneumonia. Methods A cohort study of children aged 1–59 months admitted to Kilifi County Hospital with severe pneumonia (2007–12). The primary outcome was death <1 year after discharge, determined through Kilifi Health and Demographic Surveillance System (KHDSS) quarterly census rounds. Results Of 4184 children (median age 9 months) admitted with severe pneumonia, 1041 (25%) had severe acute malnutrition (SAM), 267 (6.4%) had a positive HIV antibody test, and 364 (8.7%) died in hospital. After discharge, 2279 KHDSS‐resident children were followed up; 70 (3.1%) died during 2163 child‐years: 32 (95% confidence interval (CI) 26, 41) deaths per 1000 child years. Post‐discharge mortality was greater after admission for severe pneumonia than for other diagnoses, hazard ratio 2.5 (95% CI 1.2, 5.3). Malnutrition, HIV status, age and prolonged hospitalisation, but not signs of pneumonia severity, were associated with post‐discharge mortality. Fifty‐two per cent (95% CI 37%, 63%) of post‐discharge deaths were attributable to low mid‐upper arm circumference and 11% (95% CI 3.3%, 18%) to a positive HIV test. Conclusions Admission with severe pneumonia is an important marker of vulnerability. Risk stratification and better understanding of the mechanisms underlying post‐discharge mortality, especially for undernourished children, are needed to reduce mortality after treatment for pneumonia.
An Analysis of Drinking Motives in the Maintenance of Co-Occurring Obsessive–Compulsive Symptoms and Alcohol Misuse

Abstract:Objective: Despite the commonly observed co-occurrence of obsessive–compulsive symptoms (OCS) and substance use in clinical populations, few researchers have examined potential mechanisms which contribute to this relationship. This study assessed the mediating role of drinking motives in the relationship between OCS, alcohol consumption, and its resulting health risks among U.S. adults. Methods: Adult participants ( n = 1966; M age = 47.72, SD = 17.91; 76.30% female; 86.10% non-Hispanic White) sourced from Research Match answered an online questionnaire containing measures related to substance use and anxiety-related symptoms. Results: As hypothesized, there was a significant positive indirect path between OCS and both alcohol consumption and risky alcohol use through coping motives. However, there were also small indirect effects of OCS through social motivations for consumption and conformity motivations for risky alcohol use. Conclusions: These results add evidence that coping motives play a role in OCS and alcohol misuse and highlights the need to also consider drinking motives related to social facilitation and conformity in those with co-occurring OCS and alcohol misuse.

Risk of severe mood and anxiety disorders in the adult children of parents with alcohol use disorder: a nationwide cohort study

Abstract:Background Growing up with parental alcohol use disorder (AUD) is a risk factor for psychiatric disorders. This study investigated the risk of mood disorders and of anxiety disorders in the adult children of parents with AUD, adjusted for sociodemographic factors. Methods Individual-level register data on the total population were linked to follow children of parents with AUD from 1973 to 2018 to assess their risk of mood disorders and of anxiety disorders. AUD, mood disorders and anxiety disorders were defined with International Statistical Classification of Diseases and Related Health Problems codes from the National Patient Register. HRs of outcomes were calculated with Cox regression. Model 1 was adjusted for the child’s sex, parental education and death of a parent. Model 2 was adjusted for those factors and parental diagnosis of mood or anxiety disorder. Results Those with ≥1 parent with AUD (99 723 of 2 421 479 children) had a higher risk of mood disorder and of anxiety disorder than those whose parents did not have AUD (HR mood 2.32, 95% CI 2.26 to 2.39; HR anxiety 2.66, 95% CI 2.60 to 2.72). The risk remained elevated after adjustment for sociodemographic factors and parental psychiatric diagnosis (HR mood 1.67, 95% CI 1.63 to 1.72; HR anxiety 1.74, 95% CI 1.69 to 1.78). The highest risks were associated with AUD in both parents, followed by AUD in mothers and then in fathers. Conclusion Adult children of parents with AUD have a raised risk of mood and anxiety disorders even after adjustment for sociodemographic factors and parental mood or anxiety disorder. These population-level findings can inform future policies and interventions.

Abstract:Objective To identify latent classes of positive coping behaviors during the COVID-19 pandemic and examine associations with alcohol-related and mental health outcomes across participants with and without a history of alcohol use disorder (AUD). Methods Baseline data from 463 participants who were enrolled in the NIAAA COVID-19 Pandemic Impact on Alcohol (C19-PIA) Study were analyzed. Latent class analysis (LCA) was applied to five positive coping behaviors during COVID-19: taking media breaks, taking care of their body, engaging in healthy behaviors, making time to relax, and connecting with others. Latent class differences and the moderating role of history of AUD on six alcohol-related and mental health outcomes were examined using multiple regression models. Results LCA revealed two latent classes: 83.4% High Positive Coping and 16.6% Low Positive Coping. Low Positive Coping was associated with higher levels of perceived stress, anxiety symptoms, and loneliness. A history of AUD was consistently associated with higher levels of alcohol-related and mental health outcomes. Significant interactions between Coping Latent Classes and history of AUD indicated that the associations of Low Positive Coping with problematic alcohol use, depressive symptoms, and drinking to cope motives were either stronger or only significant among individuals with a history of AUD. Conclusions Individuals with a history of AUD may be particularly vulnerable to depressive symptoms and alcohol-related outcomes, especially when they do not utilize positive coping strategies. The promotion of positive coping strategies is a promising avenue to address alcohol-related and mental health problems during a public health crisis and warrants future research.

Abstract:Background: Recent evidence indicates that alcohol and other substance co-use, compared to alcohol-only use, might be more closely associated with negative reinforcement processes, and thus more likely during periods of increased stress. The present study examined this possibility by using data from an intensive longitudinal (daily) study of college student drinkers ( N = 1461, 54% women). We also examined individual differences in coping and enhancement drinking motives as predictors of alcohol and other substance co-use. Results: We used multilevel multinomial logistic regression to predict, relative to alcohol-only days, the likelihood of alcohol co-use with either cigarettes or marijuana, along with alcohol use with multiple substances and other substance-only use from daily interpersonal and academic stress, day-of-the-week, sex, and individual differences in coping and enhancement drinking motives. We found that, relative to alcohol-only, alcohol and marijuana co-use was more likely, and non-alcohol related substance use was less likely, on weekends. Alcohol and marijuana co-use was less likely, and other substance-only use was more likely, on days characterized by greater academic stress, whereas alcohol and cigarette co-use was more likely on days characterized by greater interpersonal stress. Individuals with higher levels of drinking to cope motivation were more likely to engage in alcohol and cigarette co-use, other substance-only use, and alcohol plus multiple substances, relative to alcohol-only. Individuals with higher levels of enhancement motives were more likely to engage in all types of alcohol and other substance co-use and other substance-only use relative to alcohol-only. Conclusions: Findings are discussed in terms of the complex nature of different patterns of co-use patterns when evaluating indicators of positive- and negative-reinforcement processes.

Abstract:While individuals with Bulimia Nervosa (BN) and Binge Eating Disorder (BED) often present with a higher rate of Alcohol Use Disorder (AUD) than the general population, it is unclear whether this extends to AN. This cross-sectional study examined differences in alcohol-related behaviours, measured using the Alcohol Use Identification Test (AUDIT), between AN participants (n = 58), recovered AN (rec-AN) participants (n = 25), and healthy controls (n = 57). Statistical models controlled for age and ethnicity. The relationship between alcohol-related behaviours with ED psychopathology and with depression was also assessed. The findings indicated that acute AN participants were not at greater risk of AUD than healthy controls. However, rec-AN participants displayed greater total audit scores than those with acute AN, and more alcohol-related behaviours than healthy controls. Acute AN participants consumed significantly less alcohol than both the healthy control group and rec-AN group. No associations were found between ED psychopathology and alcohol-related behaviours in the AN group or rec-AN. This highlights alcohol as a potential coping mechanism following AN recovery. Clinicians should consider assessments for AUD and targeted interventions aimed at encouraging healthy coping mechanisms in this group. Future studies should look at alcohol use as a moderating factor for AN recovery.
