Abstract:In this paper, a finite element model of human head was established based on CT scanning on a 40-year-old and 50 percentile Chinese male volunteer, and the model was verified with the experiment conducted by Verschueren and skull fracture was investigated during the collision. The frontal of head was impacted with different velocities during the impact tests. A break-deletion element process was represented to simulate the pathological phenomena of skull fracture.The results showed that the simulation results and experimental results were in a good consistency on both mechanics and pathology.

Abstract:To simulate skull-CSF-brain interaction relations, a simple finite element head model is established, based on ALE (Arbitrary Lagrangian-Eulerian) and overlapping mesh methods. The responses of head under impact was simulated with this model. The numerical results are coincidence well with the experimental results conducted by Nahum et al. What’s more, it is found that the skull-brain relative displacement and brain injury may be predicted better with the ALE method.

Abstract:In this study, we investigate the effects of modelling choices for the brain–skull interface (layers of tissues between the brain and skull that determine boundary conditions for the brain) and the constitutive model of brain parenchyma on the brain responses under violent impact as predicted using computational biomechanics model. We used the head/brain model from Total HUman Model for Safety (THUMS)—extensively validated finite element model of the human body that has been applied in numerous injury biomechanics studies. The computations were conducted using a well-established nonlinear explicit dynamics finite element code LS-DYNA. We employed four approaches for modelling the brain–skull interface and four constitutive models for the brain tissue in the numerical simulations of the experiments on post-mortem human subjects exposed to violent impacts reported in the literature. The brain–skull interface models included direct representation of the brain meninges and cerebrospinal fluid, outer brain surface rigidly attached to the skull, frictionless sliding contact between the brain and skull, and a layer of spring-type cohesive elements between the brain and skull. We considered Ogden hyperviscoelastic, Mooney–Rivlin hyperviscoelastic, neo–Hookean hyperviscoelastic and linear viscoelastic constitutive models of the brain tissue. Our study indicates that the predicted deformations within the brain and related brain injury criteria are strongly affected by both the approach of modelling the brain–skull interface and the constitutive model of the brain parenchyma tissues. The results suggest that accurate prediction of deformations within the brain and risk of brain injury due to violent impact using computational biomechanics models may require representation of the meninges and subarachnoidal space with cerebrospinal fluid in the model and application of hyperviscoelastic (preferably Ogden-type) constitutive model for the brain tissue.

Abstract:Introduction: Maxillofacial trauma accounts for a high percentage of patients reporting to the emergency medicine department and being admitted in the hospital. The purpose of this study was to form a direct association between maxillofacial fractures and traumatic brain injury (TBI).Methods: Ninety patients with maxillofacial fractures that reported to/were referred to the Department of Oral and Maxillofacial Surgery were observed for features indicative of TBI based on clinical presentation and radiological interpretations. Parameters such as loss of consciousness, vomiting, dizziness, headache, seizures and the requirement for intubation, cerebrospinal fluid rhinorrhoea and otorrhoea were also assessed. Appropriate radiographs for the diagnosis of the fracture were taken followed by a computed tomography (CT) scan when indicated in accordance to the Canadian CT Head Rule. These scans were then assessed for contusion, extradural haemorrhage, subdural haemorrhage, subarachnoid haemorrhage, pneumocephalus and cranial bone fracture.Results: A total of 90 patients were evaluated, of which 91.1% were males and 8.9% were females. Association between the occurrence of head injury and different maxillofacial bone fractures using the Chi-square test showed a statistical significance of <0.001 in patients with naso-orbito-ethmoid as well as frontal bone fractures. There was a clear association between fractures present in the upper as well as the middle third of the face and traumatic head injury (<i>P</i> ≤ 0.001).Discussion: Patients with the frontal bone and zygomatic bone fractures have a high prevalence of TBI. Patients with the upper and middle third of the face injury are more prone to traumatic head injury and importance should be given to patients with the same and prevent poor prognosis.

Abstract:Objective: The purpose of this study was to establish the constructive of craniofacial suture in the three-dimensional finite element model of craniofacial complex in child, to analyse the respose precedure of the zygomatic impact, to explore the biomechanics characteristic of the children craniofacial trauma. Methods: A 7-year-old female was adopted for study The complex cranial geometry was measured from a series of two-dimensional CT images. The multy-lay spiral CT scans were transformed with a self-developed preprocessor into a finite element mesh. The craniofacial sutures were constructed through the MSC Patran program. Identical impact and boundary conditions were used for the zygomatic impact simulations. Results: It has been shown that the finite element model(FEM) exhibited fine morphological and mechanical comparability. The higher stresswas showed in the zygomatic regions and atlas occipital articular in 3d FEM. The maximum von Mises stress was found at the zygomatic regions and atlas occipital articular. Conclusion: The concentrations of shear stress and tension stress in the suture and articular would increase the risk of injury in this area. But the conduction of the stress might be weakening in the suture of child skull.
Influence of Skull Fracture on Traumatic Brain Injury Risk Induced by Blunt Impact

Abstract:This study is aimed at investigating the influence of skull fractures on traumatic brain injury induced by blunt impact via numerous studies of head-ground impacts. First, finite element (FE) damage modeling was implemented in the skull of the Total HUman Model for Safety (THUMS), and the skull fracture prediction performance was validated against a head-ground impact experiment. Then, the original head model of the THUMS was assigned as the control model without skull element damage modeling. Eighteen (18) head-ground impact models were established using these two FE head models, with three head impact locations (frontal, parietal, and occipital regions) and three impact velocities (25, 35, and 45 km/h). The predicted maximum principal strain and cumulative strain damage measure of the brain tissue were employed to evaluate the effect of skull fracture on the cerebral contusion and diffuse brain injury risks, respectively. Simulation results showed that the skull fracture could reduce the risk of diffuse brain injury risk under medium and high velocities significantly, while it could increase the risk of brain contusion under high-impact velocity.

Abstract:Traumatic head injuries remain a leading cause of death and disability worldwide. Although skull fractures are one of the most common head injuries, the fundamental mechanics of cranial bone and its impact tolerance are still uncertain. In the present study, a strain-rate-dependent material model for cranial bone has been proposed and implemented in subject-specific Finite Element (FE) head models in order to predict skull fractures in five real-world fall accidents. The subject-specific head models were developed following an established image-registration-based personalization pipeline. Head impact boundary conditions were derived from accident reconstructions using personalized human body models. The simulated fracture lines were compared to those visible in post-mortem CT scans of each subject. In result, the FE models did predict the actual occurrence and extent of skull fractures in all cases. In at least four out of five cases, predicted fracture patterns were comparable to ones from CT scans and autopsy reports. The tensile material model, which was tuned to represent rate-dependent tensile data of cortical skull bone from literature, was able to capture observed linear fractures in blunt indentation loading of a skullcap specimen. The FE model showed to be sensitive to modeling parameters, in particular to the constitutive parameters of the cortical tables. Nevertheless, this study provides a currently lacking strain-rate dependent material model of cranial bone that has the capacity to accurately predict linear fracture patterns. For the first time, a procedure to reconstruct occurrences of skull fractures using computational engineering techniques, capturing the all-in-all fracture initiation, propagation and final pattern, is presented.

Abstract:Abstract Post-mortem computed tomography (PMCT) enables the creation of subject-specific 3D head models suitable for quantitative analysis such as finite element analysis (FEA). FEA of proposed traumatic events is an objective and repeatable numerical method for assessing whether an event could cause a skull fracture such as seen at autopsy. FEA of blunt force skull fracture in adults with subject-specific 3D models in forensic pathology remains uninvestigated. This study aimed to assess the feasibility of FEA for skull fracture analysis in routine forensic pathology. Five cases with blunt force skull fracture and sufficient information on the kinematics of the traumatic event to enable numerical reconstruction were chosen. Subject-specific finite element (FE) head models were constructed by mesh morphing based on PMCT 3D models and A Detailed and Personalizable Head Model with Axons for Injury Prediction (ADAPT) FE model. Morphing was successful in maintaining subject-specific 3D geometry and quality of the FE mesh in all cases. In three cases, the simulated fracture patterns were comparable in location and pattern to the fractures seen at autopsy/PMCT. In one case, the simulated fracture was in the parietal bone whereas the fracture seen at autopsy/PMCT was in the occipital bone. In another case, the simulated fracture was a spider-web fracture in the frontal bone, whereas a much smaller fracture was seen at autopsy/PMCT; however, the fracture in the early time steps of the simulation was comparable to autopsy/PMCT. FEA might be feasible in forensic pathology in cases with a single blunt force impact and well-described event circumstances.

Abstract:Although the finite element head model (FEHM) has been widely utilized to analyze injury locations and patterns in traumatic brain injury, significant controversy persists regarding the selection of a mechanical injury variable and its corresponding threshold. This paper aims to determine an objective injury threshold for maximum principal strain (MPS) through a novel data-driven method, and to validate and apply it. We extract the peak responses from all elements across 100 head impact simulations to form a dataset, and then determine the objective injury threshold by analyzing the relationship between the combined injury degree and the threshold according to the stationary value principle. Using an occipital impact case from a clinical report as an example, we evaluate the accuracy of the injury prediction based on the new threshold. The results show that the injury area predicted by finite element analysis closely matches the main injury area observed in CT images, without the issue of over- or underestimating the injury due to an unreasonable threshold. Furthermore, by applying this threshold to the finite element analysis of designed occipital impacts, we observe, for the first time, supra-tentorium cerebelli injury, which is related to visual memory impairment. This discovery may indicate the biomechanical mechanism of visual memory impairment after occipital impacts reported in clinical cases.
A QCT-Based Nonsegmentation Finite Element Head Model for Studying Traumatic Brain Injury

Abstract:In the existing finite element head models (FEHMs) that are constructed from medical images, head tissues are usually segmented into a number of components according to the interior anatomical structure of the head. Each component is represented by a homogenous material model. There are a number of disadvantages in the segmentation-based finite element head models. Therefore, we developed a nonsegmentation finite element head model with pointwise-heterogeneous material properties and corroborated it by available experiment data. From the obtained results, it was found that although intracranial pressures predicted by the existing (piecewise-homogeneous) and the proposed (pointwise-heterogeneous) FEHM are very similar to each other, strain/stress levels in the head tissues are very different. The maximum peak strains/stresses predicted by the proposed FEHM are much higher than those by the existing FEHM, indicating that piecewise-homogeneous FEHM may have underestimated the stress/strain level induced by impact and thus may be inaccurate in predicting traumatic brain injuries.
Analysis of finite element models for head injury investigation: reconstruction of four real-world impacts

Abstract:Previous studies have shown that both excessive linear and rotational accelerations are the cause of head injuries. Although the head injury criterion has been beneficial as an indicator of head injury risk, it only considers linear acceleration, so there is a need to consider both types of motion in future safety standards. Advanced models of the head/brain complex have recently been developed to gain a better understanding of head injury biomechanics. While these models have been verified against laboratory experimental data, there is a lack of suitable real-world data available for validation. Hence, using two computer models of the head/brain, the objective of the current study was to reconstruct four real-world crashes with known head injury outcomes in a full-vehicle crash laboratory, simulate head/brain responses using kinematics obtained during these reconstructions, and to compare the results predicted by the models against the actual injuries sustained by the occupant. Cases where the occupant sustained no head injuries (AIS 0) and head injuries of severity AIS 4, AIS 5, and multiple head injuries were selected. Data collected from a 9-accelerometer skull were input into the Wayne State University Head Injury Model (WSUHIM) and the NHTSA Simulated Injury Monitor (SIMon). The results demonstrated that both models were able to predict varying injury severities consistent with the difference in AIS injury levels in the real-world cases. The WSUHIM predicted a slightly higher injury threshold than the SIMon, probably due to the finer mesh and different software used for the simulations, and could also determine regions of the brain which had been injured. With further validation, finite element models can be used to establish an injury criterion for each type of brain injury in the future.
Child head injury criteria investigation through numerical simulation of real world trauma

Abstract:Finite element modelling has been used for decades in the study of adult head injury biomechanics and determination of injury criteria. Interest is recently growing in investigation on pediatric head injury which requires elaboration of biofidelic models that take into account child's head particularities in terms of size, geometry, and mechanical properties. In this study, a finite element model of a 3-year-old child head is proposed. The model is reconstructed from real CT scan images and mechanical properties are extracted from available data in the literature. A large number of real accidents (25 falls) are reconstructed with proposed model using different brain constitutive relationships in order to investigate their influence on model response. Mechanical output parameters (HIC, pressure, shearing stress) are calculated from these simulations. Statistical analysis was performed in order to evaluate predictive capability of the parameters. Von Mises stress appears to be clearly the most predictive parameters, allowing clear distinction between injured and non-injured cases. To the authors' knowledge, this study proposes for the first time a statistically based neurological injury criterion for a pediatric population using finite element modelling.

Abstract:In this study, using computational biomechanics models, we investigated influence of the skull-brain interface modeling approach and the material property of cerebrum on the kinetic, kinematic and injury outputs. Live animal head impact tests of different severities were reconstructed in finite element simulations and DAI and ASDH injury results were compared. We used the head/brain models of Total HUman Model for Safety (THUMS) and Global Human Body Models Consortium (GHBMC), which had been validated under several loading conditions. Four modeling approaches of the skull-brain interface in the head/brain models were evaluated. They were the original models from THUMS and GHBMC, the THUMS model with skull-brain interface changed to sliding contact, and the THUMS model with increased shear modulus of cerebrum, respectively. The results have shown that the definition of skull-brain interface would significantly influence the magnitude and distribution of the load transmitted to the brain. With sliding brain-skull interface, the brain had lower maximum principal stress compared to that with strong connected interface, while the maximum principal strain slightly increased. In addition, greater shear modulus resulted in slightly higher the maximum principal stress and significantly lower the maximum principal strain. This study has revealed that using models with different modeling approaches, the same value of injury metric may correspond to different injury severity.

A modified human head model for the study of impact head injury

Abstract:A recently published finite element (FE) head model is modified to consider the viscoelasticity of the meninges, the spongy and compact bone in the skull. The cerebrospinal fluid (CSF) is simulated explicitly as a hydrostatic fluid by using a surface-based fluid modelling method, which allows fluid and structure interaction. It is found that the modified model yields smoother pressure responses in a head impact simulation. The baseline model underestimated the peak von Mises stress in the brain by 15% and the peak principal stress in the skull by 33%. The increase in the maximum principal stress in the skull is mainly caused by the updation of the material's viscoelasticity, and the change in the maximum von Mises stress in the brain is mainly caused by the improvement of the CSF simulation. The study shows that the viscoelasticity of the head tissue should be considered, and that the CSF should be modelled as a fluid, when using FE analysis to study head injury due to impact.

Abstract:In the real world crashes, brain injury is one of the leading causes of deaths. Using isolated human head finite element (FE) model to study the brain injury patterns and metrics has been a simplified methodology widely adopted, since it costs significantly lower computation resources than a whole human body model does. However, the degree of precision of this simplification remains questionable. This study compared these two kinds of methods: (1) using a whole human body model carried on the sled model and (2) using an isolated head model with prescribed head motions, to study the brain injury. The distribution of the von Mises stress (VMS), maximum principal strain (MPS), and cumulative strain damage measure (CSDM) was used to compare the two methods. The results showed that the VMS of brain mainly concentrated at the lower cerebrum and occipitotemporal region close to the cerebellum. The isolated head modelling strategy predicted higher levels of MPS and CSDM 5%, while the difference is small in CSDM 10% comparison. It suggests that isolated head model may not equivalently reflect the strain levels below the 10% compared to the whole human body model.
Finite element simulations of the head–brain responses to the top impacts of a construction helmet: Effects of the neck and body mass

Abstract:Traumatic brain injuries are among the most common severely disabling injuries in the United States. Construction helmets are considered essential personal protective equipment for reducing traumatic brain injury risks at work sites. In this study, we proposed a practical finite element modeling approach that would be suitable for engineers to optimize construction helmet design. The finite element model includes all essential anatomical structures of a human head (i.e. skin, scalp, skull, cerebrospinal fluid, brain, medulla, spinal cord, cervical vertebrae, and discs) and all major engineering components of a construction helmet (i.e. shell and suspension system). The head finite element model has been calibrated using the experimental data in the literature. It is technically difficult to precisely account for the effects of the neck and body mass on the dynamic responses, because the finite element model does not include the entire human body. An approximation approach has been developed to account for the effects of the neck and body mass on the dynamic responses of the head–brain. Using the proposed model, we have calculated the responses of the head–brain during a top impact when wearing a construction helmet. The proposed modeling approach would provide a tool to improve the helmet design on a biomechanical basis.

Abstract:This paper reviews the traditional areas of impact biomechanics as they relate to brain injury caused by blunt impact. These areas are injury mechanisms, human response to impact, human tolerance to impact and the use of human surrogates. With the advent of high-speed computers, it is now possible to add computer models to the list of human surrogates that used to be limited to animals and human cadavers. The advantages and shortcomings of current computer models are discussed. One of the computer models was used to predict the pressures and shear stresses developed in the brain and the extent of stretch of the bridging veins in the brains of American football players who sustained severe helmet-to-helmet head impact during the game. It was found that increases in intracranial pressure were more dependent on translational acceleration while the primary determinant for the development of shear stresses in the brain is rotational acceleration. Although the current head injury criterion is based almost entirely on translational acceleration, it is recommended that any new criterion should reflect the contribution of both translational and rotational acceleration.

A Semi-Automatic Method of Generating Subject-Specific Pediatric Head Finite Element Models for Impact Dynamic Responses to Head Injury

Abstract:To account for the effects of head realistic morphological feature variation on the impact dynamic responses to head injury, it is necessary to develop multiple subject-specific pediatric head finite element (FE) models based on computed tomography (CT) or magnetic resonance imaging (MRI) scans. However, traditional manual model development is very time-consuming. In this study, a new automatic method was developed to extract anatomical points from pediatric head CT scans to represent pediatric head morphological features (head size/shape, skull thickness, and suture/fontanel width). Subsequently, a geometry-adaptive mesh morphing method based on radial basis function was developed that can automatically morph a baseline pediatric head FE model into target FE models with geometries corresponding to the extracted head morphological features. In the end, five subject-specific head FE models of approximately 6-month-old (6MO) were automatically generated using the developed method. These validated models were employed to investigate differences in the head dynamic responses among subjects with different head morphologies. The results show that variations in head morphological features have a relatively large effect on pediatric head dynamic response. The results of this study indicate that pediatric head morphological variation had better be taken into account when reconstructing pediatric head injury due to traffic/fall accidents or child abuses using computational models as well as predicting head injury risk for children with obvious difference in head size and morphologies.

Abstract:In this study, a statistical model of cranium geometry for 0- to 3-month-old children was developed by analyzing 11 CT scans using a combination of principal component analysis and multivariate regression analysis. Radial basis function was used to morph the geometry of a baseline child head finite element (FE) model into models with geometries representing a newborn, a 1.5-month-old, and a 3-month-old infant head. These three FE models were used in a parametric study of near-vertex impact conditions to quantify the sensitivity of different material parameters. Finally, model validation was conducted against peak head accelerations in cadaver tests under different impact conditions, and optimization techniques were used to determine the material properties. The results showed that the statistical model of cranium geometry produced realistic cranium size and shape, suture size, and skull/suture thickness, for 0- to 3-month-old children. The three pediatric head models generated by morphing had mesh quality comparable to the baseline model. The elastic modulus of skull had a greater effect on most head impact response measurements than other parameters. Head geometry was a significant factor affecting the maximal principal stress of the skull (p = 0.002) and maximal principal strain of the suture (p = 0.021) after controlling for the skull material. Compared with the newborn head, the 3-month-old head model produced 6.5% higher peak head acceleration, 64.8% higher maximal principal stress, and 66.3% higher strain in the suture. However, in the skull, the 3-month-old model produced 25.7% lower maximal principal stress and 11.5% lower strain than the newborn head. Material properties of the brain had little effects on head acceleration and strain/stress within the skull and suture. Elastic moduli of the skull, suture, dura, and scalp determined using optimization techniques were within reported literature ranges and produced impact response that closely matched those measured in previous cadaver tests. The method developed in this study made it possible to investigate the age effects from geometry changes on pediatric head impact responses. The parametric study demonstrated that it is important to consider the material properties and geometric variations together when estimating pediatric head responses and predicting head injury risks.
