Abstract:Objectives: To study iron metabolism and its association with thyroid function in infants with congenital hypothyroidism (CH).Study design: One hundred and twenty-three patients were recruited and matched with 110 controls. Serum levels of thyroid-stimulating hormone (TSH), thyroxine (T-4), free T-4 (FT4), ferritin (SF) and soluble transferrin receptor (STfR), were measured. The STfR/lgSF ratio (the serum transferrin receptor-ferritin index, TfR-F index) was calculated. Between-group differences in laboratory parameters and the correlation of parameters of iron metabolism with T-4 were studied.Results: The CH group showed the following serum levels: SF=218.72 +/- 103.45 mu g/L, STfR=5.19 +/- 1.58 mg/L, STfR/lgSF=2.29 +/- 0.72 and T-4=38.39 +/- 19.11 nmol/L. The control group serum levels were: SF=182.44 +/- 96.17 mu g/L, STfR=4.39 +/- 1.70 mg/L, STfR/lgSF=2.03 +/- 0.83 and T-4=111.63 +/- 17.98 nmol/L. To adjust for an age mismatch between CH patients and controls, a subgroup analysis of all infants aged <= 28 days at blood sampling was performed and showed: SF=214.26 +/- 99.43 mu g/L, STfR=5.22 +/- 1.53 mg/L, STfR/lgSF=2.31 +/- 0.72 and T-4=36.90 +/- 17.11 nmol/L for CH patients and SF=188.50 +/- 98.30 mu g/L, STfR=4.56 +/- 1.72 mg/L, STfR/lgSF=2.10 +/- 0.87 and T-4=112.27 +/- 23.87 nmol/L for controls, respectively. STfR was significantly elevated in CH patients. Significant negative correlations were found for STfR and TfR-F index with T-4 in CH patients only.Conclusions: Infants with CH did not show laboratory signs of iron deficiency. The observed elevation of STfR and its negative association with the severity of CH, suggest the presence of a self-regulating mechanism.
Correlation Between Hashimoto's Thyroiditis–Related Thyroid Hormone Levels and 25-Hydroxyvitamin D

Abstract:Objective: The purpose of this study was to further clarify the association of Hashimoto's thyroiditis (HT) and vitamin D deficiency, and to seek the connection between them and related influencing factors.Methods: Data were obtained from subjects who underwent health examinations from January 2018 to December 2018. The diagnosis of HT was based on: antithyroid peroxidase antibody (TPO-Ab) levels >35 IU/ml and/or antithyroglobulin antibody (Tg-Ab) levels >40 IU/ml. Based on the Endocrine Society guidelines, 25-hydroxyvitamin D [25(OH)D] levels ≥30.0 ng/ml were classified as a vitamin D sufficiency; those between 20 and 29.9 ng/ml, as an insufficiency; and those <20 ng/ml, as a deficiency. All statistical analysis was performed by software R.Results: Of a total of 75,436 individuals who were physically examined, 5,656 of them had 25(OH)D levels tested at the same time; 5,230 were enrolled. The level of 25(OH)D in the non-HT group was higher than that in the HT group. Multiple regression analysis showed that HT was statistically significantly correlated with being male, body mass index (BMI), waist circumference, and thyroid-stimulating hormone (TSH). TSH levels in the insufficiency group and deficiency group were higher than those in the sufficiency group. Free triiodothyronine (FT3) and thyroxine (FT4) levels in the insufficiency group and deficiency group were lower than those in the sufficiency group. 25(OH)D increased by 1 ng/ml at the normal reference level, with an increase of 2.78 ng/dl in FT4 concentration and a decrease of 0.17 mIU/L in TSH.Conclusions: Patients with HT present with a reduced 25(OH)D level, and TSH is an independent risk factor for HT. TSH is negatively correlated with 25(OH)D level. FT3 and FT4 levels were positively correlated with 25(OH)D levels.

Abstract:Objective. This study adopts the method of retrospective analysis to collect general information and laboratory results of physical examination population, hoping to clarify the correlation between uric acid and thyroid hormone. Methods. The subjects of the study were healthy subjects who underwent physical examination at the Sir Run Run Shaw Hospital affiliated to the Medical College of Zhejiang University from January 2016 to December 2018. Demographic information and medical history of all subjects were recorded through an electronic health system. Serum uric acid (SUA) was grouped by quartiles. Statistical analyses were performed with R version 3.5.1. Results. A total of 48,526 subjects were included in the analysis. Gender ratio, age, BMI, waist circumference, systolic blood pressure, diastolic blood pressure, FBG, HbA1c, TG, HDL-C, ALT, AST, FT3, FT4, and TSH were significantly different among the uric acid groups. The regression coefficients of SUA in the TSH, FT3, and FT4 regression models were B=1.000 (95% CI 1.000-1.000, p=0.009), B=0.999 (95% CI 0.999-0.999, p<0.001), and B=1.001 (95% CI 1.001-1.001, p<0.001), respectively. There was a significant dose-dependent relationship between FT4, FT3, and SUA gradient. Conclusions. Under normal thyroid function, there were significant differences in TSH, FT3, and FT4 between groups with different uric acid levels. Uric acid levels were linearly correlated with FT3 and FT4, but not with TSH.

Abstract:We have established a method for quantitative analysis of the deuterium contents (D/H) at the phenyl, methine, benzyl, N-methyl and methyl groups of l-ephedrine/HCl, d-pseudoephedrine/HCl and methamphetamine/HCl by 2H NMR spectroscopy. Comparison of the 5 position-specific D/H values of l-ephedrine/HCl and d-pseudoephedrine/HCl prepared by three methods (chemical synthesis, semichemical synthesis, and biosynthesis) showed that chemically synthesized ephedrines and semisynthetic ephedrines have highly specific distributions of deuterium at the methine position and at the benzyl position, compared with the other positions. The classification of several methamphetamine samples seized in Japan in terms of the D/H values at these two positions clearly showed that the methamphetamine samples had been synthesized from ephedrines extracted from Ephedra plants or semisynthetic ephedrines but not from synthetic ephedrine. This isotope ratio analysis method should be useful to trace the origins of seized methamphetamine in Southeast Asia.
Correlation between thyroid disorders and blood lipid level

Abstract:Objective: To investigate the relationship between thyroid dysfunction and blood lipid level. Methods: Two hundred patients with thyroid function problems admitted during April 2013 to March 2015 were enrolled for the study, including 100 cases each with hyperthyroidism or hypothyroidism, and 80 participants undergoing health maintenance examination during the same period served as control. The levels of thyroid hormones and blood lipids in all subjects were measured, and the correlation between their levels was analyzed.Results: By univariate analysis, significant difference was found in all the studied thyroid hormones and blood lipids among the three populations(all P0.05). Correlation analysis in patients with abnormal thyroid function showed that the level of thyroid stimulating hormone(TSH) had a positive correlation with the levels of total cholesterol(TC), low density lipoprotein cholesterol(LDL-C), and apolipoprotein B100(Apo B10)(r=0.695, 0.714, and 0.632); the level of free triiodothyronine(FT3) had a negative correlation with the levels of TC, LDL-C and Apo B100(r=–0.769, –0.705 and –0.689); the level of free thyroxine(FT4) had a negative correlation with the levels of TC, LDL-C and Apo B100(r=–0.728, –0.676 and –0.625)(all P0.05).Conclusion: Thyroid dysfunction may cause blood lipid metabolic abnormity, which deserves attention in clinical practice.

Abstract:OBJECTIVE: The aim of this study is to assess serum levels of thyroid hormones (TH) and complement C3, C4, CH50 in patients with multiple sclerosis (MS) and neuromyelitis optica (NMO), and investigate whether there is a correlation between serum TH and complement factors in these patients. METHODS: One hundred and eighty-eight MS patients and sixty-five NMO patients were included in this study. The levels of serum TH were measured by magnetic antibody enzyme linking immunassay and complement C3, C4 and CH50 were determined by immunoturbidimetry. RESULTS: The levels of TT4, complement C4 and CH50 in the serum were significantly higher in NMO patients than that in the MS. There were no significant differences in serum TT3, FT3, FT4 concentrations between NMO and MS patient. In MS patients, significant correlations between mean EDSS levels and serum C3 (r=0.223, p=0.014), and serum C4 (r=0.216, p=0.017) were found, and serum TT3 was positively correlated with serum CH50 (r=0.342, p=0.01). In NMO patients, the positive association between serum FT4 and C4 (r=0.533, p=0.006) were also found. CONCLUSIONS: These results indicate that NMO patients has different serum TH and complement C3, C4 and CH50 levels from MS patients, and that serum TH levels is correlated with CH50 and C4 in these patients, suggesting TH may play a different role in modulating the complement activation in MS and NMO.

Abstract:Background: Graves' disease is characterized by a diffuse hyperfunctional goiter that develops suddenly and is linked to an immune thyroid stimulating factor. Graves' disease hyperthyroidism is caused by circulating IgG antibodies binding to and activating the G-protein–coupled thyrotropin receptor. This activation causes thyroid enlargement by stimulating follicular hypertrophy and hyperplasia, as well as increases in thyroid hormone synthesis. Objectives: This study aims to assess the Determination the concentration of vitamin D in patients with graves diseases, Measurement T3, T4, FT3, FT4 and TSH hormones and Determination the correlation between immunological markers by the (SPSS). Materials and Methods: This research included a total of (90) people of various sexes and ages (30 men and 60 females), including (60) Graves' disease patients and (30) healthy people. Patients with Graves' disease were seen at Alsader Medical City (Najaf Center for Diabetes and Endocrinology). Between November 2021 and January 2022. Results: In this research, the levels of (T3, T4, TSH, FT3 and FT4) hormones and Vitamin D revealed a very significant association (P.value> 0.005) across the groups tested. A non-significant association was also found between vitamin D and other variables. According to the patients' group, the FT3 revealed a strong negative connection with vitamin D in comparison to the other parameters. Conclusion: The Thyrotropin receptor antibody (TRAb) test and ultrasonography of the thyroid gland are useful diagnostic tools for autoimmune Graves' disease. Grave's disease is connected with decreased vitamin D levels in the blood and could be made worse by high serum levels of T3, T4, FT3, FT4, and TRAb-positive and a low serum level of TSH.

Association between blood manganese levels and depressive symptoms among US adults: A nationally representative cross-sectional study

Abstract:Background: Recent studies indicated that manganese (Mn) levels were inconsistently associated with the prevalence of depression. We aimed to evaluate whether blood Mn concentrations were associated with the risk of depression among US adults. Methods: Using the NHANES 2011-2019 datasets, we conducted a cross-sectional study in 16,572 eligible participants with complete data on blood Mn concentrations and depression diagnosis. A weighted multivariable logistic model and restricted cubic spline model were applied to explore the association and dose-response relationship of blood Mn concentrations with depression risk in the total population and subgroups. Results: In the total population, compared with the lowest reference group of blood Mn, participants in the second, third, and fourth quartile had an OR of 0.84 (95%CI: 0.66, 1.07), 0.93 (95%CI: 0.73, 1.19) and 0.91 (95%CI: 0.71, 1.15) for depression (ptrend = 0.640). In subgroup analyses, doubling of blood Mn concentrations was associated with a 0.83-fold (95%CI: 0.67, 1.02), 0.30 -fold (0.14, 0.65) decreased risk of depression in females and other ethnic groups, respectively. Significant modification effects of ethnicity on the association of blood Mn concentrations with depression risk were observed. Limitations: cross-sectional study design and self-reported depressive symptoms. Conclusions: Elevated blood Mn concentrations were associated with decreased depression risk in females and other specific subgroups. Mn supplementation could be a potential pathway for intervention and prevention of depression.

Abstract:Objective To analyze the correlation between serum thyrotropin (TSH) level and thyroid hormone, blood glucose and blood lipid metabolism in patients with hyperthyroidism. Methods A total of 64 patients with hyperthyroidism treated in Zhumadian Central Hospital from July 2017 to June 2019 were divided into group A (n=30)and B group(n=34) according to their TSH level. The patients with TSH ≤ 0.02 μIU/ml were set as group A, and patients with TSH>0.02 μIU/ml as group B. The thyroid function, blood glucose and blood lipid metabolism of all the patients were examined. Results Serum total triiodothyronine (TT3), total thyroid hormone (TT4), free thyroid hormone (FT4) in group A were slightly higher than those in group B, but the difference were not statistically significant (P>0.05). The level of FT3 in group A was significantly higher than that in group B (P 0.05). Conclusions There is a certain correlation between serum TSH level and thyroid hormone, blood glucose and blood lipid metabolism in patients with hyperthyroidism. Abstract:To clarify the clinical value of serums total triiodothyronine (TT3), total thyroxine (TT4), free triiodothyronine (FT3), and free thyroxine (FT4) and provide a more eligible and economic strategy to assess thyroid function. A total of 2,673 participants (500 patients with hyperthyroidism, 500 patients with hypothyroidism, and 1,673 healthy people) were involved in our study. Serums TT3, TT4, FT3, and FT4 and thyrotropin (TSH) were measured with VIDAS fluorescent enzyme immunoassay. The Pearson correlation between TT3, TT4, FT3, and FT4 and TSH was determined to identify the most important indicator for thyroid function besides TSH. The correlation of TT4, and FT4 with TSH was statistically significant in healthy individuals ( P < 0.01), and the R -values were −0.065 and −0.152, respectively. The correlation of TT4, FT4, TT3, and FT3 with TSH was statistically significant in patients with hyperthyroidism, and the R -values were −0.241, −0.225, −0.195, and −0.176, respectively. The correlation of TT4, FT4, TT3, and FT3 with TSH was statistically significant in patients with hypothyroidism, and the R -values were −0.322, −0.262, −0.179, and −0.136, respectively. In our opinion, TSH and FT4 are the most valuable indicators in assessing thyroid function in a healthy population, and TSH and TT4 are the most meaningful in hyperthyroidism and hypothyroidism.

An alpha-1,3-mannosyltransferase of Cryptococcus neoformans.

Abstract:Cryptococcus neoformans is a pathogenic fungus, distinguished by an elaborate polysaccharide capsule that is essential for its virulence. As part of an effort to understand the biosynthesis of this important structure, we initiated purification of an alpha-1,3-mannosyltransferase with appropriate specificity for a role in building the main capsule polysaccharide, glucuronoxylomannan. A pool of proteins that was 5,000-fold enriched in this activity included several polypeptides, which acted potentially as the catalytic protein. These were analyzed using sequence information and double-stranded RNA interference. Interference that targeted a sequence corresponding to part of a 46 kDa protein in the enriched fraction abolished the activity of interest and reduced the capsule on the affected cells. This gene was cloned and expressed in active form in Saccharomyces cerevisiae to confirm function, and was termed CMT1, for cryptococcal mannosyltransferase 1. CMT1 has no confirmed homologs in GenBank other than CAP59, a cryptococcal gene encoding a protein of unknown function that is required for capsule synthesis and virulence. The Cmt1p protein also co-purifies with a homolog of CAP64, a gene whose product has similarly been implicated in capsule synthesis and virulence. A strain disrupted in CMT1 was generated in C. neoformans; this had no effect on virulence in an animal model of cryptococcosis.
The correlation between thyrotropin and dyslipidemia in a population-based study.

Abstract:This study investigated the relationship between serum thyrotrophin levels and dyslipidemia in subclinical hypothyroid and euthyroid subjects. A total of 110 subjects with subclinical hypothyroidism and 1,240 euthyroid subjects enrolled in this study. Patients with subclinical hypothyroidism had significantly lower high density lipoprotein cholesterol (HDL-C) levels than those who were euthyroid. The lipid profiles were each categorized and mean thyrotrophin levels were higher in subjects in the dyslipidemia subclasses than subjects in the normal subclasses. Thyrotrophin was positively associated with serum triglyceride and negatively associated with serum HDL-C in women. Thyrotrophin was also positively associated with total cholesterol (TC) in the overweight population along with TC and LDL-C in overweight women. In the euthyroid population, thyrotrophin was positively associated with TC in the overweight population. In conclusion, serum thyrotrophin was correlated with dyslipidemia in subclinical hypothyroid and euthyroid subject; the correlation was independent of insulin sensitivity.
Leptin Concentration and the Zn/Cu Ratio in Plasma in Women with Thyroid Disorder

Abstract:We investigated the possible correlation between the leptin concentration and the Zn/Cu ratio in the plasma of women with thyroid disorder. Forty women with hypothyroidism (n=20) or hypothyroidism (n=20) and 20 euthyroid controls were recruited. The results showed that the women with thyroid disorder (hypothyroidism or hyperthyroidism) had higher plasma leptin concentrations than the normal controls (p<0.05). Moreover, the plasma leptin concentration had no correlation with plasma thyroid hormone levels in the separate groups, nor among all the participants considered together. A strong correlation (p<0.005) between leptin and adiposity was only observed in euthyroid women. Plasma values of Zn and Cu and the Zn/Cu ratio were not markedly different among women with altered thyroid status. However, a weak correlation (r=0.28, p=0.032) between leptin and the Zn/Cu ratio was found from the pooled data of all participants and retained after adjustment for adiposity. We suggest that there may exist an interaction between the plasma leptin level and thyroid hormone-induced abnormality for selected minerals.
Thyroid Dysfunction and its Correlation with Section Metabolic Syndrome among Perimenopausal and Postmenopausal Women Attending a Tertiary Care Hospital in Karnataka: A Cross-sectional Study

Abstract:Introduction: Thyroid dysfunctions are more common among women than men and increase near menopause. These are also associated with metabolic syndrome, increasing cardiovascular risk. These cardiovascular events and cerebrovascular events increase women’s morbidity and mortality, especially after menopause. Though many researchers in the past few decades have tried to highlight this problem, majority of them have focused on the elderly population. Studies on women of the perimenopausal age group are still not frequently found. Aim: To assess the correlation between thyroid dysfunction and metaoblic syndrome among perimenopausal and postmenopausal women. Materials and Methods: A cross-sectional study was conducted among women attending the Medicine Outpatient Department (OPD) of Adichunchanagiri Institute of Medical Sciences, Nagara BG, Karnataka, India, from December 2017 to May 2019. Total 100 participants were included in the study, of which 50 were perimenopausal and 50 were postmenopausal women. Clinical history, examination and relevant investigations {Free T3 or Total T3, Free T4 or Total T4, Thyroid Stimulating Hormone (TSH)} were recorded. Association was studied using Chi-square test, groups were compared using Analysis of Variance (ANOVA) test, and correlation was estimated using Pearson’s correlation test. Results: Mean age of the study participants was 55.14±8.05 years. Thyroid dysfunction was observed in 17% of the participants, of whom 14% had subclinical hypothyroidism, 2% had overt hypothyroidism and 1% had hyperthyroidism. Prevalence of metabolic syndrome was 45%. There was a significant association of hypothyroidism with Fasting Blood Sugar (FBS) (p-value=0.005) and a positive correlation between FBS and TSH level and High Density Lipoprotein (HDL) with T3 and T4 levels. Conclusion: Prevalence of hypothyroidism and metabolic syndrome were high among perimenopausal women and as FBS increases or HDL decreases, the chances of hypothyroid increases. Therefore, early identification and reduction of these components of metabolic syndrome decreases hypothyroidism and cardiovascular events.

Abstract:It has been reported that bone is the primary organ for manganese (Mn) accumulation, but the association between manganese and bone loss remains debatable. Therefore, this study aimed to evaluate the relationship between blood manganese and bone mineral density/bone mineral content (BMD/BMC) by using a representative sample from the National Health and Nutrition Examination Survey (NHANES). A total of 9732 subjects over the age of 18 with available data were enrolled in this study. The relationship between blood manganese and BMD/BMC of the total body, spine and femoral regions was evaluated using multivariate linear regression models. Subgroup analyses were also performed. We observed a negative association between blood manganese and BMD/BMC in the femoral neck and total body in the fully adjusted model, especially femoral neck BMD in women aged 50–70 years. In brief, people exposed to manganese should be aware of the increased risk of osteopenia or osteoporosis. Besides, due to the lack of available data, there are no definite values for the tolerable upper intake level (UL), average requirement (AR) and population reference intake (PRI) of manganese. The results of our study may provide some references for the establishment of AR, PRI and UL of Mn.

Inverse association between serum iron levels and Hashimoto's thyroiditis in United States females of reproductive age: analysis of the NHANES 2007-2012

Abstract:Purpose: Hashimoto's thyroiditis (HT) is a significant public health concern, particularly among females. While existing studies have explored the correlation between serum iron levels and HT, limited research has specifically focused on this association in reproductive-age females. Our study aims to investigate the relationship between serum iron and HT. Methods: Using data from the National Health and Nutrition Examination Survey (NHANES) database (2007-2012), we employed weighted multivariate logistic regression models, an XGBoost model, and smooth curve fitting. We assessed the correlation between serum iron and HT and examined linear and non-linear relationships with thyroid peroxidase antibodies (TPOAb) and thyroglobulin antibodies (TgAb). Results: Among 2,356 participants, each unit increase in serum iron was associated with a 43% reduced risk of HT (Odds Ratios (OR) 0.574; 95% Confidence Interval (CI) 0.572, 0.576). Quartile analysis confirmed these effects. The XGBoost model identified serum iron as the most significant variable correlated with HT. Smooth curves revealed a linear association between log2-transformed serum iron and HT. Additionally, log2-transformed serum iron inversely correlated with TPOAb levels (β -15.47; 95% CI -25.01, -5.92), while a non-linear relationship was observed with TgAb. Conclusion: Our study reveals that in reproductive-age women, every unit increase in serum iron is associated with a 43% lower risk of HT, demonstrating an inverse relationship. Additionally, serum iron exhibits a negative correlation with TPOAb and a non-linear association with TgAb.

Abstract:BackgroundManganese (Mn) is an essential trace metal element required for optimal human health. However, few studies have assessed the Mn status in hypertensive patients, especially in China. Moreover, factors associated with Mn status have not yet been thoroughly explored. Therefore, we aimed to assess the serum Mn status of adults with hypertension in China and its association with demographic factors.MethodsAn observational, cross-sectional study was conducted to assess serum Mn concentrations in 14 provinces of China. A total of 2597 patients with hypertension were randomly identified by sex, age, and district, and serum Mn concentrations were quantified using inductively coupled plasma mass spectrometry (ICP-MS).ResultsIn our study population, the median serum Mn levels were 1.60 (interquartile range (IQR), 0.94–2.85) µg/L for males and 1.51 (IQR, 0.86–2.69) µg/L for females. In adjusted linear regression models, significantly higher serum Mn concentrations were found in summer (compared with spring, β, 1.06 µg/L, 95% CI: 0.62 to 1.50), and those living in Guangxi (compared with Heilongjiang, β, 0.81, 95% CI: 0.06 to 1.56), Shanxi (compared with Heilongjiang, β, 0.75, 95% CI: 0.01 to 1.50), and Liaoning (compared with Heilongjiang, β, 1.65, 95% CI: 0.91 to 2.38), and significantly lower serum Mn concentrations were found in patients who aged 60–70 years (compared with those aged < 60 years, β, − 0.40 μg/L, 95% CI: − 0.76 to − 0.05).ConclusionOur findings observed high serum Mn status among Chinese adults with hypertension, and revealed the association between terms of age, region, and season with serum Mn.

Abstract:Background: Thyroid hormone metabolism is linked to iron metabolism. Thyroperoxidase, a key enzyme in the biosynthesis of thyroid hormones, is iron-dependent. Thus, iron deficiency might be the primary cause of hypothyroidism. Copper is another trace element that has been linked to thyroid status. Copper regulates excessive thyroxine (T4) absorption and reduces cell damage during thyroid hormone synthesis. The present study clarified the possible correlations between iron and copper levels as well as total iron binding capacity (TIBC) and triiodothyronine (T3), T4, as well as thyroid-stimulating hormone (TSH) levels in healthy and hypothyroid subjects.Materials and methods: Thirty-five healthy subjects and 35 hypothyroid subjects were included in this study. Serum T3, T4, and TSH levels were measured using the enzyme linked fluorescence assay. Serum iron levels and TIBC were estimated using ferrozine/magnesium carbonate method, while serum copper levels were estimated using colorimetric method.Results: Copper levels were not significantly different between healthy and hypothyroid subjects. Iron and T4 levels were significantly lower in hypothyroid subjects compared with those in healthy subjects, while TIBC and TSH levels were significantly higher. There was no significant correlation between copper levels and T3, T4, and TSH levels.Conclusion: There were inverse correlations between TIBC and T4 as well as iron levels, and there was no significant correlation between copper levels and all thyroid function parameters. Routine examination of iron levels and thyroid function is highly recommended for early diagnosis and therapy of hypothyroidism.Keywords: total iron binding capacity, iron, copper, hypothyroidism
Iliopsoas Tenotomy at the Lesser Trochanter Versus at the Pelvic Brim in Ambulatory Children With Cerebral Palsy

Abstract:Background: Progressive hip flexion deformity is a common problem in ambulatory children with spastic cerebral palsy, causing static and dynamic deformity. The iliopsoas muscle is recognized as a major deforming force in the development of this problem. Many clinicians address this problem by lengthening the iliopsoas, either in an intramuscular location at the pelvic brim or by complete tenotomy at the lesser trochanter. The goal of this study was to compare the outcomes of patients with ambulatory cerebral palsy who had intramuscular lengthening at the pelvic brim to those who underwent complete release of the iliopsoas tendon at the level of the lesser trochanter. Methods: Twenty patients were included in the study, 11 of whom had iliopsoas release at the lesser trochanter (group 1) and 9 of whom had intramuscular lengthening at the pelvic brim (group 2). All patients had physical examinations, plus kinematic and kinetic analyses in our gait laboratory before and 1 year after surgery. Results: Hip flexion contracture was decreased significantly only in group 1, although there was a trend of decrease in group 2. There was a significant increase in maximum hip extension in terminal stance and a reciprocal decrease in maximum swing phase hip flexion ingroup 1, with a similar trend that did not reach significance in group2. Stride length increased significantly in both groups. There was no significant change in power generation of hip flexion during the swing phase in either group. Conclusions: We found improved static and dynamic parameters of hip extension after iliopsoas lengthening and did not detect any adverse kinematic or kinetic change in hip function after surgery. The improvement was more robust in the group who underwent release at the lesser trochanter. Because there are no adverse effects of iliopsoas release from the lesser trochanter and the improvement in hip extension is greater, this approach should be considered in ambulatory patients with spastic diplegia when a hip flexor weakening procedure is considered.
