Abstract:Background: An estimated 159 million children under five are affected by malnutrition, with an additional 101 million children under five being underweight in sub-Saharan Africa. In Namibia, 24% of children under age 5 are stunted and 8% are severely stunted. The Kavango region has the highest rate of stunting (38.8%) in the country. Objective: The purpose of the study was to determine and describe the nutritional knowledge and practices of mothers with malnourished children regarding feeding at Rundu Intermediate Hospital, Kavango East Region, Namibia. Materials and methods: A quantitative cross-sectional research design was used. A convenience sampling technique was used to select 199 mothers with malnourished children who were admitted to the pediatric ward. Data were collected via self-administered questionnaires with closed-ended questions. Results: 51.8% of the mothers had correct nutritional knowledge regarding breastfeeding for six months before giving other food, and 74.4% believed that newborns should be initiated to breastfeeding within an hour after birth, while the same number breastfed their babies on demand. Furthermore, 35.6% of the participants followed appropriate practices regarding feeding. Conclusions: Most mothers (51.8%) had appropriate nutritional knowledge. However, only a minority (35.6%) of the mothers had the appropriate practices regarding feeding their children. Hence, there was a knowledge/practice mismatch regarding the significance of exclusive breastfeeding during the first six months, and generally the vitality of breast milk to the child. These findings may be used to develop strategies and target interventions to create awareness among mothers regarding effective breastfeeding practices.
Feeding Behaviour and Nutritional Status among Children Aged 6 to 23 Months Old in Bahi District in Tanzania

Abstract:Background: The survival, well-being, and development of young children depend on optimal infant and young child feeding (IYCF) practices. It is imperative to assess nutrition status and feeding behaviour practices in order to develop interventions. The objective of this study is to assess nutrition status, feeding behaviour, and its association with nutrition status among children aged 6 to 23 months old in Bahi district, Tanzania. Methods: A community-based, cross-sectional study was employed. A multistage sampling technique was used. Bahi district council, wards, and village were randomly selected, and then a systematic random sampling method was used to select participants. Anthropometric measurements were used to determine the nutrition status of children. Process for the Promotion of Child Feeding (ProPAN) manual guided assessment of feeding behaviours associated with nutrition status. Z-score was used to determine the level of malnutrition; the chi-square test and logistic regression were used as descriptive and inferential statistical tests, respectively, to determine the association between nutrition status and feeding behaviour. Results: Out of 395 children aged between 6 and 23 months, 62.8% were stunted, 25.3% were underweight, and 6.6% were wasted. In the multivariable analysis, the results reveal that stunting was significantly associated with perception of exclusive breastfeeding (number of months) (AOR 4.24, 95%CI: 2.012-8.284) and number of feeds per day (AOR 2.02, 95%CI: 1.00-6.314). With regard to perception of exclusive breastfeeding (number of months), the children whose mothers perceived that children supposed to be breastfed for more than six months were four times more likely to be not stunted compared to those who were perceived to be breastfed less than three months, and the children whose mothers' fed them twice to three times were two times more likely to be not stunted compared to the children who were fed only two times. Conclusion: Bahi district council has a higher prevalence rate of stunting, underweight, and wasting among children aged 6 to 23 months. Feeding on the first colostrum, exclusive breastfeeding, and breastfeeding for more than six months are the main determinants of nutrition status.

Abstract:Background: Inadequate breastfeeding and complementary feeding practices are key determinants of child malnutrition and impact one-third of the under-five mortality rate worldwide. The district of Namuno in Cabo Delgado (Mozambique) has been long registering a high prevalence of acute malnutrition. To date, no data exists about Infant and Young Child Feeding (IYCF) practices in the area. The current pilot study explores the feeding practices among children aged 0-23 months in Namuno and sets out its main drivers. Methods: This cross-sectional study was realized between August and September 2019 and drew on questionnaires for mothers/caregivers of children aged 0 to 23 months. We computed IYCF indicators and correlated them with mothers'/caregivers' school attendance, delivery setting, and distance between home and the place where livelihood activities took place (workplace), using univariate and multivariate (age-adjusted) logistic regression analysis. Results: The survey was data derived from a sample of 632 mothers/caregivers. 62% of 0-23-month-old children received colostrum whilst only 31% of 0-5 months babies were on exclusive breastfeeding (EBF). Among 6-23 months old children, 17% consumed foods and beverages from at least five out of eight defined food groups, 31% received a minimum frequency of meals, and 23% had a minimum acceptable diet. Data revealed a positive correlation between early initiation of breastfeeding and delivery in a maternity ward (OR 6.9, CI 3.2-16.1, p-value < 0.001). No difference in the IYCF indicators between female and male babies was detected. Conclusions: In the Namuno district, IYCF practices did not fulfill WHO/UNICEF's indicators and recommendations. This suggests that efforts should be focused on EBF-enabling interventions to improve children's dietary consumption patterns.

Abstract:BackgroundRwandan children's nutritional status is characterized by higher undernutrition rates. Infant and young child feeding practices (IYCF) have a direct impact on how well-nourished youngsters under two years old are.ObjectivesThis study aimed to assess feeding practices and nutritional status among children aged six to 23 months in Nyabihu district.MethodsA cross-sectional study was conducted in 164 households having children aged six to 23 months in December 2020. A structured questionnaire was used to collect infants' and young children's feeding practices. Using SPSS version 20.0, data were entered and analysed. Binary logistic regression was utilized to identify association between feeding practices and nutritional status.ResultsBreastfeeding initiation within the first hour of life was 94.5%, and exclusive breastfeeding was 79.9%. Complementary foods were given timely to 73.2% of children, 47.6% had minimum meal frequency (MMF), 23.2% had minimum dietary diversity (MDD), and 15.9% were fed a minimally acceptable diet (MAD). However, 4.3% of children were wasted, 6.7% were underweight, and 23.2% were stunted. There was no significant association when variables were passed to a binary logistic regression-adjusted model.ConclusionComplementary feeding practices remain suboptimal. Adequate Dietary Intake and Consumption of Indigenous Fermented Products Are Associated with Improved Nutrition Status among Children Aged 6–23 Months in Zambia

Abstract:Agroecological food systems and socioeconomic characteristics are known to influence household food security and food consumption patterns and consequently have an impact on child nutritional status. The present study examined food consumption patterns among children aged 6–23 months in two geographic regions of Zambia, with special focus on consumption of fermented products, and its association with illnesses and nutritional status. The cross-sectional survey enrolled a total of 213 children from Namwala and Mkushi districts of Zambia. A 24 h recall and food frequency questionnaire (FFQ) were used to determine the number of food groups consumed and consequently dietary diversity scores and food consumption patterns, respectively. Determinants of child’s linear growth as measured by Height-for-Age Z-scores (HAZ) were assessed via multiple linear regression analysis. In total, 54% of the children met the minimum dietary diversity by consuming food from at least 5+ food groups. Maize meal porridge, Mabisi (fermented milk), Chibwantu and Munkoyo (fermented beverages based on cereals) and groundnuts were among the frequently consumed foods. A higher consumption of fermented beverages was observed in Namwala compared to Mkushi district. A significant association was observed between HAZ score (rho = 0.198, p = 0.004), Weight-for-Age Z-score (WAZ) (rho = 0.142, p = 0.039) and consumption of mabisi. Dietary intake had a positive association with child nutritional status. The frequent consumption of traditional non-alcoholic cereal and milk-based fermented foods underpinned their contribution to the children’s dietary intake. Moreover, the trend would be viewed as an indicator to nutrition and policy actors on possible unoptimized potential of indigenous fermented foods’ influence in nutritional and health status among children at regional and national levels. Although Zambia has a wide range of traditional non-alcoholic fermented food products, their prospects in provision of macro- and micronutrients along with microbiota benefits remain scanty despite global efforts increasingly advocating for the inclusion of such traditional foods in food-based recommendations.
Breastfeeding practices and social norms in Kinshasa, Democratic Republic of the Congo: A qualitative study

Abstract:Introduction: Breastfeeding has many benefits for both mothers and children. The World Health Organization recommends exclusive breastfeeding for the first six months of life. However, in the Democratic Republic of the Congo, slightly under half of children under six months are exclusively breastfed. This study aimed to describe breastfeeding practices and to explore perceived social norms regarding breastfeeding among mothers in Kinshasa. Materials and methods: A qualitative descriptive study was conducted in Kinshasa from June to July 2013. This study purposively sampled 54 mothers of infants aged 6 to 12 months, who participated in six focus group discussions. Based on the Theory of Planned Behaviour, the discussion guide explored infant feeding in the first six months, knowledge of breastfeeding, perception of the feasibility of exclusive breastfeeding, and perception of the social norms with regard to exclusive breastfeeding. The content analysis approach was used to analyse data. Results: Mothers had good breastfeeding knowledge; however, few of them had practised exclusive breastfeeding as recommended during the first six months. Exclusive breastfeeding was considered unfeasible in their context. Barriers to exclusive breastfeeding were reported as baby's cries, social pressure, warm climate, and poor maternal diet. Social norms were supportive of breastfeeding but unfavourable to exclusive breastfeeding. Conclusion: In Kinshasa, mothers have a good knowledge of breastfeeding. However, few practise exclusive breastfeeding. Social pressure plays an important role in the cessation of exclusive breastfeeding before six months. In order to improve the practice of exclusive breastfeeding in this context, social and behaviour change programmes should target the entire population rather than mothers only.

Abstract:Background: Inappropriate infant and young child complementary feeding practices related to a lack of maternal knowledge contributes to an increased risk of malnutrition, morbidity, and mortality. There is a lack of data regarding the effect of nutrition education on maternal knowledge, feeding, and hygiene practices as part of a supplementary feeding intervention targeting infants and young children with moderate acute malnutrition in low-income countries like Uganda. Objective: To determine whether nutrition education improves knowledge, feeding, and hygiene practices of mothers with infants and young children diagnosed with moderate acute malnutrition. Methods: A cross-sequential study using a pretest–posttest design included 204 mother–infant pairs conveniently sampled across 24 randomly selected clusters. Weekly nutrition education sessions were embedded in a supplementary porridge intervention for 3 months. Mean scores and proportions for knowledge, feeding, and hygiene practices were determined at baseline and end line. The difference between mean scores at the 2 time points were calculated with the paired t test analysis, while the proportions between baseline and end line were calculated using a z test analysis. Results: Mean scores for knowledge, dietary diversity, and meal frequency were higher at end line compared to baseline ( P < .001). Handwashing did not improve significantly ( P = .183), while boiling water to enhance water quality improved ( P < .001). Conclusion: Nutrition education in conjunction with a supplementary feeding intervention targeting infants and young children with moderate acute malnutrition improved meal frequency, dietary diversity and water quality.

Abstract:Abstract Objectives Efforts to improve infant and young child (IYC) feeding practices include the use of nutrition social behaviour change communication (SBCC) among caregivers of children under 5y. We assessed the association between monthly participation in community-level nutrition group meetings on caregiver health and nutrition knowledge, attitudes and practices (KAPs). Methods Data from community-based cross-sectional survey conducted in Eastern and Southern Highland Zones of Tanzania, were used. Indices were developed for caregivers’ knowledge on nutrition, health-and-childcare, household and young child dietary diversity, and vitamin A intakes. Impact of the number of nutrition meeting attendance on caregiver knowledge and practices scores were examined using multiple regression analyses controlling for potential caregiver socio-demographic confounders such as maternal age, marital status, education, employment, household size, and household wealth index. Results Out of 547 caregivers surveyed, 49.7% attended nutrition group meetings and received information on nutrition SBCC while 50.3% did not. Overall, 28% of participating women had a moderate level of nutrition knowledge, 62% had high level of vitamin A knowledge, and 57% had high level of health-and-childcare knowledge. Participation in nutrition group meetings was significantly associated with health-and-childcare knowledge score, household and young child dietary diversity scores, and household and young child vitamin A intake; magnitude of the associations were greater for caregivers who attended at least four meetings. Conclusions Our findings emphasize the need for programmes that seek to address the issues present in the use of nutrition SBCC at community level to improve maternal/caregiver KAPs and subsequently to improve nutrition status of infant and young children. Abstract:Background Breastfeeding is the basis for appropriate nutrition for infants and is strongly supported by the Ministry of Health in China. However, there are differences in infant feeding practices in different areas of the country. The aim of this study was to compare the infant feeding practices and the prevalence of determinants of initiation and continuing to breastfeed until six months of age in city, suburb and rural areas in Zhejiang Province, PRC. Methods A longitudinal cohort study of infant feeding practices was undertaken in city, suburban and rural areas in 2004/2005. Mothers were recruited and interviewed before discharge from hospitals. A total of 1520 mothers were recruited into the study. Follow-up interviews were administered at 1, 3 and 6 months after birth to obtain details of infant feeding practices. Results 'Any breastfeeding' rates were high before discharge at all three locations, 96.5% in city, 96.8% in suburb and 97.4% in the rural area. The 'exclusive breastfeeding' rates in the city, suburban and rural areas before discharge were 38.0%, 63.4% and 61.0% respectively. By sixth months the 'any breastfeeding' rates had declined to 62.8%, 76.9% and 83.6% and the 'exclusive breastfeeding' rates had fallen to 0.2%, 0.5% and 7.2% in city, suburb and rural areas respectively. There were differences in feeding practices between the three locations, including the use of prelacteal feeds and the introduction of supplementary feeds. Conclusion Mothers who lived in the city were least likely to be 'exclusive breastfeeding' at discharge. At six months the city infants also had lower rates of 'any breastfeeding' and 'exclusive breastfeeding'.

Abstract:Abstract Background Through the health extension package, Ethiopia had practiced infant and young child feeding. However, infant and young child feeding (IYCF) practice has been poor. Hence, in this study, the knowledge, attitude, and practice of the mothers/caregivers on infant and young child feeding were assessed. Methods A cross-sectional study was carried out among 486 mothers/caregivers from Assosa Districts in the Assosa Zone of Benshangul Gumuz Region, Western Ethiopia. A semi-structured interviewer-administered questionnaire was used. To isolate independent predictors for good knowledge, good practice, and favorable attitude of the mothers/caregivers related to child feeding, multivariable logistic regression analyses were performed. Results Out of 486 study participants, 456 (93.8 %) of mothers had good knowledge, 432 (88.9 %) had a positive attitude, and 380 (78.2 %) mothers had good practice of IYCF practice recommendations. Furthermore, age of mothers, educational status of the mother, place of delivery, father’s educational status, father’s involvement & support, previous knowledge about IYCF, discussion with their husband about IYCF, and ANC follows up were significantly associated with mother’s knowledge on IYCF recommendation. Conclusions Overall mothers had good knowledge and a positive attitude about IYCF practices. To support IYCF practices, behavior change communications intervention strategies should be introduced in mothers to bridge the gap between knowledge and practices.

[Quality of Human Liquid Specimens Appropriate for Laboratory Tests and Analyses--Chairmen's Introductory Remarks].

Abstract:One of the major concerns for laboratory medicine communities is the quality of human specimens. Thanks to previous efforts in laboratory medicine field, routine quality control is almost guaranteed. Evidence is continually accumulating, and this evidence keeps the analytical levels in the clinical laboratory high. There is still, however, insufficient scientific evidence when handling and storing routine specimens, especially liquid specimens. Data based on poor quality specimens affects designs of patients' diagnostic and therapeutic strategies. Furthermore, such poor data also affect the outcomes of clinical trials. The only solution to avoid this situation is to acquire specimens of favorable quality. In this symposium, six experts involved in basic science to the clinical frontline gave unique presentations to share their expertise with us, and their concerns about specimen quality.

Abstract:Despite complementary feeding practices among mothers with children aged 6 – 24 months, death and disease infection rates are still escalating regardless of the nutritional status of children within that age gap. Complementary feeding practices plays a key role in providing the much-needed nutrients as it enhances successive development and general wellbeing of children, 6-24 months. A few research studies have been conducted in Kisumu County, more specifically, Kisumu County Referral Hospital on complementary feeding practices and nutritional status among children aged 6-24 months which is part of the critical window in the life of a child. The study assumed that most mothers with children between the age of 6-24 months within the Sub County attended Kisumu County Referral Hospital which was the area of study. The current study sought to establish the influence of complementary feeding practices and the nutritional status of children 6-24 months A sample selection of 79 participants was done through purposive sampling. Data was harvested through questionnaires. The study instruments were piloted and adjusted using participants who were not part of the sample. Analysis of data was done using descriptive statistics; mean and percentages. The study reveals that there are poor complementary feeding practices among mothers due to harmful cultural beliefs and low social-economic status. The study also report that malnourished children were 55% male and 45% female respectively. The study recommends that health aid programs should be more focused on homes with low economic background, counseling and sensitization to improve the mother’s knowledge on complementary feeding practices and nutritional status.

Abstract:The aim was to assess maternal feeding practices of children one to three years. A descriptive observational design was employed. The sample consisted of mothers-child dyads. A validated structured questionnaire was used. Data was analyzed using SPSS version 26.0. The nutrition status of the children at birth indicated 11.6% underweight as compared to the time of the study (7.2%), 7.9% were stunted increased to 38.0%, while wasting decreased from 11.4%–2.4%. Early cessation of breastfeeding and inappropriate complementary feeding practices were the factors influencing growth. The prevalence of underweight and wasting were low while stunting and overweight were high.

A qualitative analysis of infant and young child feeding practices in rural Rwanda

Abstract:Objective: To explore and gain an in-depth understanding of the factors influencing child feeding practices among rural caregivers in Rwanda. Design: In-depth semi-structured qualitative interviews were conducted. Purposive sampling was used to recruit participants. Interviews were audio-recorded, transcribed verbatim and coded. Data were analysed inductively using thematic analysis. Setting: Rutsiro District, Western Province, Rwanda. Participants: Participants included twenty-four mothers (median age 32 years) with children 6-23 months old. Results: We identified five key themes: (i) breast-feeding practices and role in food supply; (ii) family v. children's food preparations; (iii) food classification systems and their influence on child feeding decisions; (iv) child feeding during diarrhoeal episodes and (v) influence of poverty on child feeding practices and child care. Conclusions: Mothers' infant and young child feeding decisions are informed by information both from health workers and from traditional/own knowledge. Navigating through this information sometimes creates conflicts which results in less than optimal child feeding. A nutrition educational approach that is cognisant of maternal perceptions should be employed to improve child feeding practices. Efforts to improve child feeding practices must be complemented by programmes that enhance household economic opportunities and access to foods.

Aim: This study aims to assess midwives' nutrition knowledge and to what extent they integrate maternal nutrition in services provided at health facilities. Setting: This study included hospitals and clinics within the three selected districts in Botswana. Methods: A cross-sectional study design with an analytical component was employed. Direct observation through a checklist and a structured interviewer-administered questionnaire were used. Data were analysed using SPSS IBM version 26. Results: A sample of 102 midwives participated, resulting in a response rate of 82%. Most of the participants were females (89.2%). Maternal nutrition knowledge was found to be variable but decreased with midwives' age. A statistically significant correlation coefficient of p < 0.005 at r = -0.278 was observed between maternal nutrition knowledge and age. Similarly, there was a statistically significant negative correlation between maternal nutrition knowledge, practices and maternal nutrition course attended using Pearson correlation (r = -0.217 p < 0.028). Conclusion: Midwives had adequate to variable but declining maternal nutrition knowledge and practices with age. There is a need to provide midwives with refresher courses, as their nutrition knowledge and practices were related with courses attended. Contribution: The study contributes to provide the literature concerning nutrition knowledge and practices of midwives. The results will assist in addressing the gaps encountered and lead to the improvement of maternal nutrition and pregnancy outcomes.
