Abstract:All residencies and fellowships are required by the Accreditation Council for Graduate Medical Education (ACGME) and local institutional policy to conduct an annual program review of educational effectiveness. However, a number of family medicine residencies were cited for having an inadequate annual review or for failing to document the review in 2008. The ACGME and university offices of graduate medical education provide program directors some guidance on conducting and documenting annual program reviews, but few articles describe a detailed process for such a review. In this article, the authors describe the systematic process their program uses to conduct and document an annual program review and argue that the annual program review is an excellent way of modeling quality improvement to residents and showing residents and faculty that their input helps improve the residency and the residents' educational experiences. The article also describes metrics included in the process and tells how resident and faculty participation is integrated in the review. Specific outcomes of the process are also described. The authors believe that other residency programs and fellowship programs can adapt this process to conduct annual reviews that improve educational and clinical outcomes.
[Construction, genetic stability and protective efficacy of recombinant fowlpox virus expressing hemagglutinin gene of H5N1 subtype avian influenza virus].

Abstract:Construction, genetic stability and protective efficacy of a recombinant fowlpox virus expressing hemagglutinin (HA) gene (rFPV-HA) of H5 subtype avian influenza virus were conducted. The results indicated that the recombinant fowlpox virus was obtained and purified by blue plaque selection. HA expressed by rFPV-HA was identified by indirect immunofluorescence array. The rFPV-HA could express reporter gene LacZ and HA gene after fifteen successive passages in chicken embryo fibroblast. The rFPV-HA induced completely protection of chickens against lethal challenge with homologous avian influenza virus. The results show that rFPV-HA has good genetic stability and antigenicity and should be a useful tool in HPAI control program by preventing illness and death in chickens when the disease spreads widely.
Evaluation of the Quality of the Pre-ARCP (Pre-Annual Review of Competency Progression) Corporate Report for Postgraduate Doctors (Core Trainees) in Relation to Their Postgraduate Teaching Attendance and Audit Involvement

Abstract:Aims This evaluation was done with the focus to improve the quality of pre-ARCP corporate report for core-trainees. The pre-ARCP corporate report is a document compiled by the medical education department guided by other departments to capture certain competencies. The area of interest was how the report could be up-to-date and accurate. This would ultimately lead to a fair and objective summation of facts for the Psychiatric and Educational Supervisor report which will be reviewed by the ARCP panel. Methods This evaluation included all full-time core trainees within the Northern training scheme of the Trust who have undergone ARCP in January and July 2022. Exclusion criteria included less-than full-time trainees and core trainees who did not take part in an ARCP panel in January and July 2022. The questionnaire was designed by the Project Team and approved by the Trust Audit Team prior to data collection. The data were collected from 11th July to 31st July 2022. Electronic questionnaires were sent out to 33 postgraduate doctors. Results A total of 12 postgraduate doctors responded (36%). Out of the 12 doctors that responded, 11 (92%) had taken part in the ARCP panel in July 2022. 11/12 (92%) reported having received their pre-ARCP corporate report prior to the ARCP and in adequate time. Similarly, 11/12 (92%) of postgraduate doctors agreed with the record of both the RCPsych teaching attendance and audit involvement recorded on their pre-ARCP corporate report. In relation to capturing locality teaching attendance, 58% of postgraduate doctors reported the information as accurate. Inaccurate capturing of leaves, on-calls and/or rest days were pointed out by respondents as reasons for the discrepancy in attendance. Of those who contacted medical education, 10/10 (100%) reported that the issue was resolved before the portfolio submission date for ARCP. Of those who did not take action, 50% (1/2) reported the reason as being “I did not see the need to take action”. Conclusion We found that core trainees felt that capturing accurate RCPsych teaching attendance as well as accurate audit involvement before ARCP are areas that required improvement. There is room for improvement regarding recording locality teaching attendance and absences due to leaves, on-calls, and compensatory rest. It brings us back to reflect on the time spent by each affected postgraduate doctor to clarify their records when discrepancies are noted. Results were discussed with Medical Education department and suggestions for improvement implemented. A re-evaluation is scheduled to take place in July 2023.

Setting a direction for continuing professional development in anesthesiology: A synthesis of the outcome from an Utstein style meeting

Abstract:Background An Utstein style meeting of key stakeholders from the existing collaboration surrounding post‐graduate training was arranged to set a direction for continuing professional development (CPD) of anesthesiologists in Denmark. A 2‐day meeting was planned to guide discussions about competencies in anesthesiology, facilitate the development of a blueprint for a portfolio‐based CPD program and provide examples of how a portfolio can be used in practice. Methods The meeting agenda was based on an adaptation of Kern's six‐step approach to curriculum development. Twenty‐four participants from the university hospitals in Denmark were invited. Prior to the meeting participants were informed of the objectives and the Utstein style process. Results Participants acknowledged a need for a more structured approach to CPD, preferably within the current organizational set up at the departmental level, and with a portfolio‐based, individualized curriculum. It was recognized that CPD should contain an array of possibilities to accommodate needs and wants of both the individual and the department. It was emphasized that, while anesthesiologists are used to give feedback to trainees, many are less familiar in providing the same to peers, and psychological safety was identified as a prerequisite to support a culture where specialists can reflect openly on each other's performance. Conclusion The results provide an insight into the attitudes, opportunities, and challenges of anesthesiologists in relation to continuing professional development in Denmark. Generally, participant suggestions are in line with the shift in medical education toward workplace‐based learning, feedback and lifelong learning.

Abstract:Introduction Advance care planning (ACP) refers to a legal process and documentation that outlines one's wishes regarding their health care to take effect when they no longer have the capacity to make decisions for themselves. It involves designating a medical power of attorney and completing an advance directive (or living will). Despite strong recommendations for people to complete this process, and evidence of benefits to patients and caregivers/loved ones, it remains under-utilized with some estimates that only about one third of older adults have engaged in ACP. Psychiatrists are in a unique position to assist patients with these difficult conversations, particularly those with neurocognitive and other psychiatric conditions that introduce additional complexity and challenges. There are both mental health advance directives and dementia-specific advance directives. However, psychiatrists do not often formally engage patients in advance care planning, and trainees have limited experience in these discussions. In this poster, we describe initiatives to improve resident knowledge and comfort with ACP discussions in psychiatry and other specialty training programs. Methods A literature review was conducted in databases (PubMed, PsycInfo) for relevant publications regarding advance care planning in psychiatry, with a focus on psychiatric and neurocognitive disorders. Additionally, we review published studies of initiatives to promote ACP for resident physicians or detailing their perspectives and barriers to ACP. Results Advance care planning is underutilized generally, but also notably in neurocognitive disorders. There are numerous physician- and patient- related barriers to ACP. For physicians, these are often logistical, including time constraints and lack of continuity with patients. Other barriers include discomfort with the conversation due to lack of experience and feeling unsure when to initiate or revisit these discussions. Published initiatives generally target residents in Family Medicine and Internal Medicine, with few involving residents in Psychiatry, specifically. Interventions take the form of interactive and dynamic lectures or workshops with roleplay exercises, having residents complete ACP documentation themselves, and some with discussions with patients and caregivers for their perspective. Conclusions Advance care planning is an important and generally beneficial tool for patients. Psychiatrists can play an important role in promoting ACP with their patients, particularly in geriatric psychiatry. A variety of methods are used to train resident physicians, though there is limited literature on this topic, especially for psychiatry residents. We discuss the potential role of psychiatrists in ACP and recommendations for training.

Abstract:Abstract Around 60% of dermatological conditions are managed by nondermatology specialist doctors (Lowell BA, Froelich CW, Federman DG, Kirsner RS. Dermatology in primary care: prevalence and patient disposition. J Am Acad Dermatol 2001; 45: 250–5). Despite this, a survey of junior doctors found that only 8% of doctors felt confident in initiating basic therapy for a skin complaint (Hussain W, Hafiji J, Stanley AG, Khan KM. Dermatology and junior doctors: an evaluation of education, perceptions and self-assessed competencies. Br J Dermatol 2008; 159: 505–6). This highlights that there is a need to evaluate and improve current ‘hands-on’ dermatology undergraduate training. A quality-improvement project was done looking at fourth-year medical students on clinical placement in dermatology at a UK university hospital. Previous feedback showed that while attending dermatology clinics, 50% of students reported they did not have the opportunity to take any histories, and only 41% received feedback from clinicians. Students were asked to rate their satisfaction of their placement on a scale of 1 to 5; the average student score was 2.6. Based on this feedback, we decided to commit more clinicians to have a specific educational role as part of their job plan. Only one out of the existing nine consultants expressed an interest. This led to us organizing for one existing and four new senior dermatology colleagues, who had recently joined the department, to be assigned as ‘clinical teachers’ in their job plans. They were allocated 1–2 h per week dedicated to education (0.25–0.5 SPA – supporting programmed activity). Funding was from the existing dermatology undergraduate teaching budget. The clinical teachers were assigned to tutor a couple of students a week and to give them personalized feedback. Students were asked to assess the quality of this feedback through their online platform ‘My Knowledge Map’. The students also logged the number of histories they were able to take in clinic and with which clinician; this allowed us to organize a prize for the most accommodating clinician per block. The clinical teachers were also expected to deliver dermatology teaching sessions in a 1/7 rota, and to be involved in assessments. Through these changes, the percentage of students who were able to take a history during their dermatology week increased to 77% and 91% of these students received feedback on their skills. Overall student satisfaction in clinics increased from 2.3 to 3.4 out of 5. Our findings show that assigning a specific ‘clinical teacher’ role in the job plans of dermatology clinicians increased students’ hands-on experience, the amount of personalized feedback they received and overall student satisfaction.

Abstract:BackgroundAs the pandemic wanes, there is an opportunity to reevaluate resultant changes in graduate medical education (GME), particularly from the viewpoints of those affected most. We aimed to assess both trainee and faculty perceptions on the educational changes and innovations resulting from the pandemic to inform future educational planning.MethodsWe surveyed trainees and core education faculty at three New York City children’s hospitals. Surveys assessed perceived changes to educational activities, skills, scholarship, effectiveness of virtual teaching, future desirability, and qualitative themes.ResultsThe survey was completed by 194 participants, including 88 (45.4%) faculty and 106 (54.6%) trainees. Trainees were more likely to report a negative impact of the pandemic compared with faculty (75.5% vs. 50%, p < 0.01). Most respondents reported a decrease in formal educational activities (69.8%), inpatient (77.7%) and outpatient (77.8%) clinical teaching. Despite this, most perceived clinical and teaching skills to have stayed the same. Most (93.4%) participated in virtual education; however, only 36.5% of faculty taught virtually. Only 4.2% of faculty had extensive training in virtual teaching and 28.9% felt very comfortable teaching virtually. In the future, most (87.5%) prefer a hybrid approach, particularly virtual didactic conferences and virtual grand rounds. Faculty themes included challenges to workflows and increased empathy for trainees, while trainee themes included increased work/life balance and support, but increased burnout.ConclusionMany changes and innovations resulted from the pandemic. Hospital systems and GME programs should consider this data and incorporate viewpoints from trainees and faculty when adapting educational strategies in the future.

Abstract:Suboptimal training for healthcare students is a recognized barrier to successful completion of advance care planning (ACP) with patients and families. Our study sought to enhance ACP knowledge and communication skills for interprofessional healthcare students. During academic year 2017-2018, 46 students (19-medicine, 16-nursing, and 11-social work), received three training modules delivered by interprofessional faculty. Students subsequently observed a clinical ACP encounter attended by a patient and their family, a clinical social worker, and an internal medicine resident. Three surveys (pre-training T1, post-training T2, and post-clinical encounter T3) evaluated change in student knowledge, communication self-efficacy, ACP self-efficacy, and interprofessional teamwork (using SPICE-R). A randomized waitlist approach was used to test the effects of the clinical ACP training. Student attendance and engagement were high. Relative to baseline, all outcomes differed at all data collection intervals (p < 0.05), except for the SPICE-R from T2 to T3 (p > 0.05). ACP self-efficacy scores declined at T2 before improving at T3. Communication self-efficacy was lower at T2 but improved at T3. Teamwork improved with a medium-large effect (ES = 0.75) at T2 and a large effect (ES = 1.00) at T3. Participant knowledge of ACP improved overall (p < 0.001) as well as for each discipline (p < 0.05). Preliminary findings indicate the interprofessional training experience enhanced student communication skills, ACP knowledge, and appreciation for team-based care. T2 findings demonstrate decrease in communication and ACP self-efficacy, perhaps suggesting students initially underestimated ACP complexity and overestimated their ability to communicate about ACP. T3 findings further suggest that students ultimately benefited from the training experience with meaningful improvements on all key outcomes.
Academic Half Day Improves Resident Perception of Education Without Compromising Patient Safety

Abstract:Background Residency programs are required to offer a didactic curriculum and protect resident time for education. Our institution implemented an Academic Half Day (AHD) in the 2021-2022 academic year to address issues related to the standard noon conference series. Objective Determine the impact of AHD implementation on education, patient safety, and workflow. Methods This was a prospective, single-site educational intervention study. Pre- and post-implementation surveys and ACGME surveys assessed changes in trainee and faculty attitudes and behaviors. Patient safety and workflow were evaluated by comparing the number of safety event reports, rapid response team activations, time to admission from the ED, and time of discharge on AHD days compared to other weekdays. Results Survey response rates were: residents 68%/48%, fellows 42%/35%, and faculty 59%/29%. AHD was associated with a significant, positive change in resident attitudes and experiences and on ACGME survey items. On AHDs compared with other weekdays, there were no significant differences in safety event report rates (p=0.98), nor in rapid response team activation rates (p=0.99). There was not a clinically meaningful difference in median admission time from the ED on AHD weekdays (125min) compared to other weekdays (130min, p=0.04). There was no significant difference in median discharge time on AHD vs other weekdays (p=0.13). Conclusions This study suggests that there is no significant difference in patient safety or workflow with the implementation of AHD. This study supports prior studies that residents strongly prefer AHD. AHD may be a useful framework for resident education without compromising patient care.

Classroom-Based Learning in an Academic Obstetrics and Gynecology Residency Training Program

Abstract:Objectives: Classroom-based learning such as academic half days (AHDs) are complementary to workplace learning in postgraduate medical education. This study examined three research questions: the purpose of AHDs, elements of an effective AHD, and factors that make AHD sustainable. Methods: We conducted a case study of the AHD in a large Obstetrics and Gynecology residency program at the University of British Columbia. Residents were interviewed in 2013 (n = 11) and 2018 (n = 7) and the program administrator was interviewed in 2018. The themes in each research question were identified by modified inductive analysis. Results: Residents expressed that the purposes of AHD included: providing organization and an overview for their knowledge acquisition; preparation for their Royal College specialty exam; and to provide a venue for peer support and mentorship. Elements of an effective AHD include the repetition of key concepts; formative assessments such as quizzes, a suitable balance of faculty input and resident active participation, and protection from clinical duty during AHD. Regarding the sustainability of AHD, themes included: addressing barriers to faculty participation, providing administrative support for logistical needs, and providing feedback to faculty. Conclusions: This work provides important insights into the purpose, effectiveness, and sustainability of AHDs for those who design and implement classroom learning for residents.
Journal club as a platform for Paediatric post-graduate medical education: lessons learnt from a tertiary care training center in the United Kingdom

Abstract:Background: Journal Club is an educational platform for junior doctors and is highly useful for sharing knowledge and practice, which in turn leads to improved care. However, this forum was infrequently held, poorly attended and given less priority and trainees had less interest according to a 2018 online survey at the Department of Paediatrics, John Radcliffe Hospital, Oxford. Methods: Journal Club was conducted over one year with incorporation of new organisational measures. Trainees were invited to present had the ownership of the choice of article. Presenters were encouraged with formal written feedbacks for their e-portfolio. Junior doctors were guided but had their own choice for selecting themes. Each session was advertised by regular weekly emails to improve attendance. Resource articles were distributed in advance to promote discussion and peer learning. An online survey was conducted to assess the impact of the changes after thirteen months following these interventions. Results: Fifty weekly Journal Clubs were held over fourteen months. Fifty five junior and senior trainees presented with presentation rate being 93%. Forty trainees and consultants responded to the online survey. Eighty seven percent were satisfied about session length. 100% believed the presented topics were relevant. Ninety six percent of responders believed that the Journal Club was at least somewhat relevant to their clinical practice. 100% believed that alert emails were useful. All responders believed that resource articles were at least somewhat helpful. Conclusion: Provision of written feedback, regular and friendly reminders for participation and distribution of resources articles prior to the Journal Club meeting were associated with improved participation and overall satisfaction among both presenters and attendees.
Enhancing Advance Care Planning Communication: An Interactive Workshop With Role-Play for Students and Primary Care Clinicians

Abstract:Introduction: Increased clinician training on advance care planning (ACP) is needed. Common barriers to ACP include perceived lack of confidence, skills, and knowledge necessary to engage in these discussions. Furthermore, many clinicians feel inadequately trained in prognostication. Evidence exists that multimodality curricula are effective in teaching ACP and can be simultaneously targeted to trainees and practicing clinicians with success. Methods: We developed a 3-hour workshop incorporating lecture, patient-oriented decision aids, prognostication tools, small-group discussion, and case-based role-play to communicate a values-based approach to ACP. Cases included discussion of care goals for a patient with severe chronic obstructive pulmonary disease and one with mild cognitive impairment. The workshop was delivered to fourth-year medical students, then adapted in two primary care clinics. In the clinics, we added an interprofessional case applying ACP to management of dental pain in advanced dementia. We evaluated the workshops using pre-post surveys. Results: Thirty-four medical students and 14 primary care providers participated. Self-reported knowledge and comfort regarding ACP significantly improved; attitudes toward ACP were strongly positive both before and after. The workshop was well received. On a 7-point Likert scale (1 = unacceptable, 7 = outstanding), the median overall rating was 6 (excellent). Discussion: We developed an ACP workshop applicable to students and primary clinicians and saw improvements in self-reported knowledge and comfort regarding ACP. Long-term effects were not studied. Participants found the role-play especially valuable. Modifications for primary care clinics focused on duration rather than content. Future directions include expanding the workshop's content.

Abstract:Surveys of senior house officers and registrars were undertaken by postal questionnaire to ascertain views on the need for and content of induction training, career counselling, and performance review. The questionnaire was sent out in May 1990 and repeated in May 1996, after measures had been taken to improve induction training, and assessment and appraisal of trainees. In 1990 there was a clear wish to receive information on career prospects, research and education opportunities, and clinical audit, but more ambivalence regarding information or training in communication, discharge policies, standards, and encoding procedures. There was also a firm view that career counselling could be improved and formal goal setting and performance appraisal interviews would be welcomed. In 1996 there was disappointingly little change in the views expressed by the junior medical staff, though there was a significant increase in confidence in the role of the consultant in career counselling.

Abstract:Background Advance care planning (ACP) is the process of having conversations with patients to ensure preferences are known and support patient healthcare goals. ACP and the Age‐Friendly Health Systems (AFHS) Initiative's, "What Matters," are synergistic approaches to patient‐centered conversations. Implementation and measurement of ACP in primary care (PC) are variables in quality and consistency. We examined whether participation in an ACP learning collaborative (LC) would improve knowledge and ability to conduct ACP discussions and increase the frequency of documented ACP in participating practices. Methods The WWAMI (Washington, Wyoming, Alaska, Montana, and Idaho) region Practice and Research Network (WPRN) and the Northwest Geriatrics Workforce Enhancement Center collaboratively organized a 9‐month virtual LC. It consisted of 4 synchronous, 1.5‐h sessions, technical support, and a panel of ACP experts. A Wilcoxon rank sum test assessed differences in knowledge from a pre–post survey. Documentation of ACP in the EHR was collected after at least one plan–do–study–act cycle. Results We enrolled 17 participants from 6 PC practices (3 hospital‐affiliated; 3 Federally Qualified Health Centers) from the WPRN. Two practices did not complete all LC activities. Abstract:Background: Advance Care Planning (ACP) enables patients and relatives to define and share values, goals and preferences for future medical treatment and care. The IMplementing Pediatric Advance Care Planning Toolkit (IMPACT), developed in the Netherlands, is a method for conducting ACP in pediatric palliative care. Healthcare professionals who were trained to use IMPACT, indicated their need for ongoing support to practice ACP communication skills optimally over time. Therefore, we developed a team-based learning program aimed at teaching participants how to transfer knowledge on ACP, continue practicing ACP communication skills and reflect on ACP conversations within their own team context. The aim of this study was to evaluate the program's transfer of knowledge as well as the professionals' experience and team reflection on ACP. Methods: A one-day IMPACT train-the-trainer course was developed and a selection of healthcare professionals (facilitators) from pediatric palliative care teams (PPCTs) from all seven Dutch university hospitals and the specialized Center for Pediatric Oncology were invited to participate. Hereafter, facilitators were asked to transfer their course-acquired knowledge to their team members (learners) by organizing two coaching-on-the-job sessions. A mixed-methods design, combining questionnaires and field notes, was used to evaluate the level of knowledge transfer and team reflection achieved. Results: Eighteen healthcare professionals in the role of facilitator participated in the train-the-trainer course. In seven PPCTs one (n = 3) or two (n = 4) coaching-on-the-job session(s) took place, attended by 29 and 17 learners, respectively. In the questionnaires, 11 facilitators indicated that they had to some extent transferred acquired knowledge to their team members as intended. Sixteen out of 21 learners who participated in at least one coaching-on-the-job session, reported (somewhat) increased self-confidence for conducting ACP conversations. The reported main strength of the program was practicing with/learning from colleagues whereas dealing with workload and variation in existing ACP skills within PPCTs need more attention. Conclusions: The newly developed team-based learning program resulted in intended transfer of knowledge and methodical reflection on ACP in coaching-on-the-job sessions in most participating PPCTs. Planning coaching-on-the-job sessions regarding ACP in pediatric palliative care with multiple healthcare professionals is challenging and needs more emphasis in the training.
Design and Implementation of a Didactic Curriculum in a Large Neonatal–Perinatal Medicine Fellowship Program: A Single-center Experience

Abstract:Objective Although the Accreditation Council for Graduate Medical Education and American Board of Pediatrics (ABP) provide regulations and guidance on fellowship didactic education, each program establishes their own didactic schedules to address these learning needs. Wide variation exists in content, educators, amount of protected educational time, and the format for didactic lectures. This inconsistency can contribute to fellow dissatisfaction, a perceived poor learning experience, and poor attendance. Our objective was to create a Neonatal–Perinatal Medicine (NPM) fellow curriculum based on adult learning theory utilizing fellow input to improve the perceived fellow experience. Study Design A needs assessment of current NPM fellows at Cincinnati Children's Hospital was conducted to guide the development of a new curriculum. Fellow perception of educational experience and board preparedness before and after introduction of the new curriculum was collected. Study period was from October 2018 to July 2021. Results One hundred percent of the fellows responded to the needs assessment survey. A response rate of 100 and 87.5% were noted on mid-curriculum survey and postcurriculum survey, respectively. Key themes identified and incorporated into the curriculum included schedule structure, content, and delivery mode. A new didactic curriculum implementing a consistent schedule of shorter lectures grouped by organ system targeting ABP core content was created. After curriculum implementation, fellows had higher self-perception of board preparedness, and overall improved satisfaction. Conclusion Our positive experience in implementing this curriculum provides a framework for individual programs to implement similar curricula, and could be utilized to aid in development of national NPM curricula. Key Points Fellowship didactic education varies significantly resulting in learner dissatisfaction and poor attendance. Widespread need to restructure didactic curricula exists. Advance Care Planning and Communication Skills Improve after an Interprofessional Team Simulation with Standardized Patients

Abstract:Background: Improving rates of advance care planning (ACP) and advance directive completion is a recognized goal of health care in the United States. No prior study has examined the efficacy of standardized patient (SP)-based student interprofessional ACP trainings. Objectives: The present study aims to evaluate an interprofessional approach to ACP education using SP encounters. Design: We designed a pre-post evaluation of an innovative interprofessional ACP training curriculum using multimodal adult learning techniques to test the effects of completing ACP discussions with SPs. Three surveys (pre-training T1, post-training T2, and post-clinical encounter T3) evaluated student knowledge, Communication Self-Efficacy (CSES), ACP self-efficacy, and interprofessional teamwork (using SPICE-R2). Setting/subjects: Students from the schools of medicine, nursing, and social work attended three training modules and two SP encounters focused on ACP. Measurements/results: During academic year 2018-2019, 36 students participated in the training at University of Maryland. Results demonstrated statistically significant improvements in ACP self-efficacy, M T1 = 2.9 (standard deviation [SD]T1 = 0.61) compared with M T3 = 3.9 (SDT3 = 0.51), p < 0.001, and CSES, M T1 = 4.6 (SDT1 = 1.35) versus M T3 = 7.3 (SDT3 = 0.51), p < 0.001, from T1 to T3. There was a medium-to-large improvement in knowledge from an average score of 4.3 (SD = 1.0) at T1 to an average score of 5.5 (SD = 1.4) at T2, p = 0.005, d = 0.67. Conclusions: Our interprofessional training module and SP encounter was successful in improving medical, social work, and nursing students' self-reported communication skills and knowledge regarding ACP.
Narrative Performance Level Assignments at Initial Entrustment and Graduation: Integrating EPAs and Milestones to Improve Learner Assessment

Abstract:<span>Purpose: To determine which narrative performance level for each general pediatrics EPA reflects the minimum level clinical competency committees (CCCs) felt should be associated with graduation as well as initial entrustment and compare the narratives associated with the expected narrative performance levels (ENPLs) for each EPA with actual narrative performance levels (ANPLs) assigned to residents at initial entrustment.Method: A series of 5 narratives, corresponding to the 5 milestone performance levels, were developed for each of the 17 general pediatrics EPAs. In AY 2015–2016, the CCCs at 22 Association of Pediatric Program Directors Longitudinal Educational Assessment Research Network member sites reported ENPLs for initial entrustment and at time of graduation. From AYs 2015–2016 to 2017–2018, programs reported ANPLs for initial entrustment decisions. ENPLs and ANPLs were compared using a logistic mixed effects model.Results: ENPLs for graduation and entrustment were most often level 3 (competent) followed by level 4 (proficient). For 8 EPAs, the ENPLs for graduation and entrustment were the same. For the remaining 9, some programs would entrust residents before graduation or graduate them before entrusting them. There were 4,266 supervision level reports for initial entrustment for which an ANPL was provided. Perspectives of Nursing Home Staff in Advance Care Planning Conversations: Experiences from the APPROACHES Project

Abstract:Objectives: Advance care planning (ACP) is considered a best practice in the nursing home setting; however, there is a lack of consistency in the training of nursing home staff and implementation of structured ACP programs. A qualitative study interviewing ACP specialists in nursing homes was conducted to understand the experience of staff engaged in Aligning Patient Preferences - a Role offering Alzheimer's patients, Caregivers, and Healthcare providers Education and Support (APPROACHES), an embedded pragmatic clinical trial to improve ACP. Design: Qualitative interviews regarding ACP specialists' experiences and perceived intervention impact. Setting and participants: Staff of intervention-assigned nursing home facilities who completed a minimum of 10 ACP conversations with residents during APPROACHES program implementation. Methods: Fourteen staff were interviewed. Interviews were transcribed and coded by the research team. Results: There were 21 codes identified that were then distilled into the following 5 themes: (1) experiences with the ACP specialist program, (2) engaging in ACP conversations, (3) considerations related to dementia, (4) benefits and challenges of ACP, and (5) involvement of multiple people in the ACP process. Participant responses suggested variability in experiences with the ACP specialist program and highlighted many aspects relevant to engaging in conversations with families and residents, particularly those living with dementia. Benefits of ACP, including relationship building and increased preparedness for changes in health status, were balanced with challenges related to sensitive conversation topics and logistical difficulties in scheduling ACP discussions. ACP specialists discussed the multiple roles that others played in the ACP process. Conclusions and implications: Findings from this analysis provide insights into tailoring APPROACHES and other ACP programs for full-scale implementation in the nursing home setting. Nursing home staff experiences tailoring the program to fit their environments were reflective of the pragmatic nature of the ACP specialist program.
