Abstract:Introduction and importance: Odontogenic keratocysts (OKC) are benign intraosseous cysts with expansive growth. They account for approximately 7.8 % of all jaw cysts and have a high recurrence rate. Herein, we present a minimally invasive approach for the surgical treatment of a remarkable variation of OKC with a 15-year radiological and clinical follow-up. Presentation of the case: We present the case of a 42-year-old female patient with a large cyst in the mandible between teeth 35 and 45, who reported spontaneous swelling and paresthesia of the lower lip. Radiological imaging is crucial for treatment planning. The cyst was surgically treated with a single enucleation combined with adjuvant therapy to minimise recurrence. A titanium plate was inserted because of the size of the defect. Recurrence was observed one year later and treated with single enucleation and adjuvant therapy. After 15 years, complete healing, no signs of recurrence, and complete remodeling of the mandible were observed. Clinical discussion: The treatment of OKC remains the subject of varying approaches in the literature due to the lack of established general guidelines. One treatment option is single enucleation combined with adjuvant therapy to minimise recurrence, which can result in complete clinical and radiological remodeling of the bone. Conclusion: Direct enucleation combined with adjuvant therapy is a practical approach for treating large OKCs. It is associated with less morbidity and burden on the patient than enucleation with prior decompression or radical resection. Additionally, it shows no deficits in bone defect healing.

Abstract:AIM:Aggressive resection of a large mandibular cystic ameloblastoma may cause severe deformity and dysfunction, while simple enucleation may carry a high risk of recurrence. The purpose of this study was to evaluate, in terms of both recurrence and the preservation of contour and function, the effectiveness of enucleation combined with peripheral ostectomy (Enu/PO) in managing large mandibular cystic ameloblastomas.MATERIALS AND METHODS:Fourteen patients who had large mandibular cystic ameloblastoma (11 unicystic, 3 multicystic) and had been treated with the Enu/PO were reviewed.RESULTS:The follow-up period was 19-117 months. Using Enu/PO, mandibular continuity was preserved in all patients, and only one patient complained of permanent lip numbness. With bone regeneration, the thinned bone plate significantly thickened and the residual cavity shrank. No pathological fracture occurred. There was recurrence in 3 patients. Two recurrences were treated with radical surgery, and the other with Enu/PO. No new recurrence was found thereafter. All patients were satisfied with their facial appearance.CONCLUSIONS:The Enu/PO technique is excellent in preserving appearance and functions and with a low risk of recurrence, is a reliable option for the management of large mandibular cystic ameloblastomas.

Abstract:Odontogenic keratocyst (OKC) is a common cystic benign lesion of the jawbones, previously believed to exhibit locally aggressive growth and a tendency for postoperative recurrence, and was thus considered a tumor. [ 1 , 2 ] However, with a deeper understanding of the biological behavior of this disease, the World Health Organization (WHO) reclassified OKC as a cyst in the latest (5th) edition of the WHO Classification of Head and Neck Tumors released in May 2022. [ 3 ] This reclassification signifies a shift in the surgical treatment concept for OKC from radical to a combination of radical and functional approaches. Specifically, for initial, localized OKCs without high-risk factors for recurrence, enucleation is the preferred treatment. [ 4 , 5 ] However, for large cystic lesions of the jaw involving multiple adjacent teeth, the inferior alveolar nerve canal, maxillary sinus, or nasal cavity, or for children and adolescents with cysts containing functional teeth, marsupialization is recommended first, followed by enucleation after the cyst has reduced in size to achieve definitive treatment. [ 4 , 5 ] Combining radical and functional treatment approaches is crucial for adequately protecting the anatomical structures of the patient's oral cavity, actively maintaining physiological function, and improving quality of life. [ 4–6 ]

[Clinicopathological Analysis of 844 Cases of Odontogenic Keratocysts].

Abstract:OBJECTIVETo investigate the clinicopathologic features and prognostic factors in odontogenic keratocyst (OKC), and to provide new reference for clinic treatment and management of these patients.METHODSClinicopathological data of 844 cases initially diagnosed as or associated with OKC at Department of Oral Pathology, Peking University Hospital of Stomatology from 2000 to 2018 were collected. The cases were divided into 4 groups: sporadic OKCs (intraosseous, cystic lesion irrelevant to nevoid basal cell carcinoma syndrome), syndromic OKCs, solid OKCs and peripheral OKCs. The patients were follow-up for 6 to 216 months and the factors that might relate to recurrence were analyzed.RESULTSThere were 805 cases (95.4%) of sporadic OKCs, 32 cases (3.8%) of syndromic OKCs, 3 cases of solid OKCs and 4 cases of peripheral OKCs. The main age of sporadic OKCs was 36.03 years with the peak at the second and third decades. Ratio of male and female was 1.27:1. The predilection site was the molar and ramus area of mandibular (56.2%). In the study, 428 cases (71.2%) were unilocular in radiography while 28.8% were multilocular. The recurrent rate of enucleation with the follow-up was 20.1% (118/588) while most of them occurred in 1-3 years after surgery. The recurrent rate of multilocular patients (39.0%) was significantly higher than that of the unilocular. Enucleation after marsupialization (43 cases) or enucleation only (545 cases) showed no difference in recurrence (P>0.05). The syndromic OKCs was younger (main 20.97) and preferred to be multiple compared with sporadic OKCs (30/32, 93.7%). The predilection site was also molar and ramus area of mandibular (41.7%). Age and gender distribution of multiple cases had no significant difference with those in sporadic OKCs. More daughter cysts and epithelial islands were seen (56.3% and 17.9%). Furthermore, the recurrent rate was significantly higher than that of the sporadic OKCs (13/29, 44.9%). But there was no evidence of recurrent-related factors. The age of solid and peripheral OKCs, averaged at 45.00 and 65.75 years, were older than others. Four of peripheral OKCs showed no recurrence after enucleation.CONCLUSIONThe recurrence rate of sporadic OKCs after enucleation is 20.1%. The multilocular lesions prefer to be recurrent. There is no significant difference of recurrence with enucleation only or enucleation after marsupialization. Compared with sporadic OKCs, the syndromic patients are younger and easier to be multiple. It tends to be recurrent frequently and rapidly. There are no related factors about recurrence of syndromic patients. The clinicians should considerate comprehensively and make an individual management of therapy and follow-up. Solid and peripheral OKCs are rare and older.

Abstract:A 69-year-old male patient was referred with a complaint of a tumor in the mandible. During the physical examination, he reported a history of an abscess in the anterior region of the mandible. Clinical examination revealed an increase in intraoral volume with no pain, foul odor, or purulent discharge. Imaging exams showed an extensive radiolucent area in the body of the mandible crossing the midline, involving teeth 32 to 46. The lesion was well-defined, extending from the right ramus of the mandible to the chin, with expansion of the bone cortices.

Abstract:A 60-year-old male patient with numbness complaint on the right side of the jaw. During examination, he reported a previous history of treatment of a lesion in the same region 10 years ago, with a diagnosis of odontogenic keratocyst. The intraoral exam showed a slight increase in volume in the retromolar region on the right side, along with a surgical scar on the gingiva. Image exams revealed the presence of a well-defined, circumscribed, radiolucent area located distally to tooth 48, above the mandibular canal.

Abstract:Uridine diphosphate-glucose pyrophosphorylase (UGPase) represents a ubiquitous enzyme, which catalyzes the formation of UDP-glucose, a key metabolite of the carbohydrate pathways of all organisms. In the protozoan parasite Leishmania major, which causes a broad spectrum of diseases and is transmitted to humans by sand fly vectors, UGPase represents a virulence factor because of its requirement for the synthesis of cell surface glycoconjugates. Here we present the crystal structures of the L. major UGPase in its uncomplexed apo form (open conformation) and in complex with UDP-glucose (closed conformation). The UGPase consists of three distinct domains. The N-terminal domain exhibits species-specific differences in length, which might permit distinct regulation mechanisms. The central catalytic domain resembles a Rossmann-fold and contains key residues that are conserved in many nucleotidyltransferases. The C-terminal domain forms a left-handed parallel β-helix (LβH), which represents a rarely observed structural element. The presented structures together with mutagenesis analyses provide a basis for a detailed analysis of the catalytic mechanism and for the design of species-specific UGPase inhibitors.

Abstract:Background Odontogenic keratocyst (OKC) has high recurrence rate. We sometimes extract the teeth in contact with the lesion to prevent recurrence. Case presentation A 14‐year‐old male was referred to our hospital by his family dentist for the evaluation of a posterior left mandible lesion. We extracted the mandibular left second and third molar and performed enucleation and curettage of the OKC. After the operation, we performed intertemporal autotransplantation. Conclusion The postoperative course has been good with no mobility and less than 3 mm periodontal pockets of the donor and with no recurrence of OKC.

Abstract:The present study aimed to investigate outcomes following open surgery for extensive skull base ORN. Open surgery through a personalized sequential approach was employed to deal with five cases of extensive skull base ORN. Two patients with mild cases underwent regional debridement and sequestrectomy, and three patients with severe cases underwent extensive resection with reconstruction using free anterolateral thigh (ALT) flap. Biological glues and vascularized flaps were used for obturation of the skull base bony defect to prevent postoperative cerebrospinal fluid (CSF) leakage. The infections were controlled by antibiotic administrations which strictly followed the principles of antimicrobial stewardship (AMS). As results, both regional debridement plus sequestrectomy and extensive resection achieved satisfied outcomes in all patients. No severe complications and delayed hospitalization occurred. During the follow-up period (8-19 months), all patients were alive, pain free, without crusting or purulent discharge, and no sequestration or CSF leakage occurred. In conclusion, a personalized sequential approach including open surgery, pedicled/vascularized free flap reconstruction and AMS was advocated for patients with extensive skull base ORN.

A 44-year-old man reported jaw tenderness, and clinical examination revealed increased intra and extraoral volume in the posterior region of the right mandible. Panoramic radiography and computed tomography unveiled a well-delimited osteolytic area, unilocular, with a radiopaque halo, spanning teeth 45 to 47, extending to the ascending branch, involving the coronal portion of tooth 48, which exhibited root resorption, areas of expansion, and disruption of cortical bone.

Abstract:Four new compounds, RbSiBiS(4), RbGeBiS(4), CsSiBiS(4), and CsGeBiS(4), have been synthesized by means of the reactive flux method. The isostructural compounds RbSiBiS(4), RbGeBiS(4), and CsGeBiS(4) crystallize in space group P2(1)/c of the monoclinic system with four formula units in cells of dimensions at 153 K of a = 6.4714(4) A, b = 6.7999(4) A, c = 17.9058(11) A, and beta = 108.856(1) degrees for RbSiBiS(4), a = 6.5864(4) A, b = 6.8559(4) A, c = 17.9810(12) A, and beta = 109.075(1) degrees for RbGeBiS(4), and a = 6.5474(4) A, b = 6.9282(4) A, c = 18.8875(11) A, and beta = 110.173(1) degrees for CsGeBiS(4). CsSiBiS(4) crystallizes in a different structure type in space group P2(1)/c of the monoclinic system with four formula units in a cell of dimensions at 153 K of a = 9.3351(7) A, b = 6.9313(5) A, c = 12.8115(10) A, and beta = 109.096(1) degrees. The two structure types are closely related and consist of [MBiS(4)(-)] (M = Si, Ge) layers separated by bicapped trigonal-prismatically coordinated alkali-metal atoms. In each, the M atom is coordinated to a tetrahedron of four S atoms and the Bi atom is coordinated to seven S atoms comprising five close S atoms at the corners of a square pyramid with Bi near the center of the basal plane and the sixth and seventh S atoms further away to complete a distorted monocapped trigonal prism. The optical band gaps of 2.23 eV for RbGeBiS(4) and 2.28 eV for CsGeBiS(4) were deduced from their diffuse reflectance spectra. From a band structure calculation, the optical absorption for RbGeBiS(4) originates from the [GeBiS(4)(-)] layer. The Ge 4p orbitals, Bi 6p orbitals, and S 3p orbitals are highly hybridized.

Abstract:Odontogenic keratocysts (OK) are benign odontogenic cysts known for their aggressive behavior and high recurrence rate. A 39-year-old female patient with a history of breast cancer six years ago, without bone metastases or antiresorptive medication use, was referred to the oral surgery service due to a radiographic finding. The clinical examination showed no intraoral or extraoral changes. Imaging revealed a well-defined unilocular radiolucent area distal to tooth 37. An incisional biopsy confirmed the diagnosis of OK.

Abstract:The odontogenic keratocyst is a benign yet aggressive cyst, often prone to recurrence, primarily affecting the mandible. A 28-year-old female patient was referred to the oral surgery service due to a radiographic discovery. Clinical examination revealed a slight increase in volume in the left retromolar trigone region, detectable only through palpation. Imaging examinations unveiled a well-defined, unilocular radiolucent lesion, extending from the distal area of tooth 38 within the ascending mandibular branch to the coronoid process, featuring regions of cortical bone erosion and mandibular canal involvement.

[Clinical Research of Resection of Mandibular Benign Tumors and Primary Reconstruction with Autogenous Bone Graft Via an Intraoral Approach].

Abstract:OBJECTIVETo investigate the clinical outcomes of resection of mandibular benign tumors and primary reconstruction with autogenous bone graft via an intraoral approach.METHODSFifteen patients with mandibular benign tumors were treated between January 2009 and September 2012. There were 7 males and 8 females, aged from 18 to 45 years (mean, 30 years). The pathological diagnosis identified 11 cases of ameloblastoma, 3 cases of odontogenic keratocyst, and 1 case of odontogenic myxoma. According to the Urken's CRBS (Condyle, Ramus, Body, Symphysis) classification criteria based on the location of the mandibular defect, there were 3 cases of body type (B type), 3 cases of ramus type (R type), and 9 cases of body and ramus type (BR type). The surgeries were performed via an intraoral approach, except 1 patient with the lesion at the level of sigmoid notch via an auxiliary preauricular incision. To fix the bone grafts to the dissected mandibular defects, reconstructive titanium plates were used, either indirectly according to the computer aided design/computer aided manufacturing mandibular models before surgery (9 patients) or directly according to the exposed mandibles during surgery (6 patients). The patients received benign mandibular tumor resection and primary autogenous bone graft reconstruction with free iliac bones (11 cases) or vascularized fibular flaps (4 cases). The mandibular inferior alveolar nerves were preserved in 6 cases.RESULTSPrimary healing of incision was obtained in 14 patients, while secondary healing in 1 patient suffering from bone graft infection. All the patients were followed up 1-4 years (mean, 2.5 years). At last follow-up, no patients showed facial nerve damage; occlusion of remaining teeth was similar to preoperative conditions; the chewing function was satisfactory; mouth opening was 30-35 mm (mean, 33 mm); and swallowing and speaking functions were normal. Only slight extraoral scars caused by the auxiliary incision and the transbuccal appliances were observed, and all the patients were satisfied with the facial appearance. Lower lip numbness was relived in patients with preserved inferior alveolar nerves. There was no tumor recurrence during follow-up period.CONCLUSIONThe intraoral approach is a feasible and proper approach for resection of benign mandibular tumors and primary reconstruction with autogenous bone grafts, with the advantages of inconspicuous facial scars, minimum damage to the facial nerve, and expectable aesthetic appearance.

Abstract:Keratocystic odontogenic tumors (KCOTs, previously known as odontogenic keratocysts) are aggressive, noninflammatory jaw lesions with a putative high growth potential and a propensity for recurrence. This article puts together a summary of the serial studies related to KCOTs undertaken by the author's research group in recent years. Intraosseous jaw cysts with a solely orthokeratinized lining epithelium have been suggested to differ from the typical KCOTs. We report 20 cases of such cyst type under the term of 'orthokeratinized odontogenic cyst (OOC)'. Apart from the presence of a keratinizing epithelial lining, the OOC lacks the other histological features of KCOT, exhibits little if any tendency to recur, has no apparent association with NBCCS, may be cured by simple enucleation, and may thus constitute its own clinical entity. Mutations in PTCH1 gene are responsible for NBCCS and are related in tumors associated with this syndrome. We have so far detected 26 PTCH1 mutations (2 mutations occurred twice) in 10 out of 34 (29.4%) sporadic and 14 out of 16 (87.5%) NBCCS-associated KCOTs. The 26 mutations consisted of 10 frameshift, 2 nonsense, 3 aberrant splicing, 4 in-frame insertion/deletion/ duplication and 7 missense mutations. Two missense mutations in PTCH2 were also detected in 2 out of 15 NBCCS related KCOT patients. By contrast, no pathogenic mutation was detected in SMO. Thus, our data, together with reports from other groups, indicate that defects of PTCH1 are involved in the pathogenesis of syndromic as well as sporadic KCOTs. The pathogenic role of PTCH2 requires further investigation. A series of in vitro studies on bone resorption of KCOTs and ameloblastomas were undertaken by this group. The results indicate that odontogenic lesions could promote bone resorption in vitro and it is likely to be related to some of the cytokines secreted by the lesions.

Abstract:OKC is an odontogenic cystic lesion with a high rate of recurrence, mainly after long-term follow-up. Imaging evaluation of the treated area is performed using panoramic radiography (PR) or cone beam computed tomography (CBCT). The two cases reported here show different conclusions between PR and CBCT imaging evaluations. Case 1, a 36-year-old male, and case 2, a 33-year-old female, both presenting OKC, underwent decompression and cyst enucleation. After 6 months, both cases showed a radiopaque area on PR, indicating new bone formation of the entire site.

Although it is generally agreed that some features of OKCs are those of a neoplasia, notably the relatively high proliferative rate of epithelial cells, controversies over the behavior and management of OKCs still exist. This article is intended to review this intriguing entity and to summarize the findings of recent studies related to the nature of OKCs and their clinical and therapeutic implications. Recent advances in genetic and molecular research, i.e., PTCH1 mutations and involvement of the Hedgehog signaling pathway, have led to increased knowledge of OKC pathogenesis which hints at potential new treatment options, although the question of whether the OKC is a cyst or a cystic neoplasm is yet to be answered with certainty. Since some advocate a more conservative treatment for OKCs, notably marsupialization and decompression, future treatment strategies may focus on molecular approaches and eventually reduce or eliminate the need for aggressive surgeries.

Abstract:Aims: Intraosseous jaw cysts with a solely orthokeratinized lining epithelium hare been suggested to differ from the typical odontogenic keratocysts (OKC) by exhibiting a less aggressive behaviour, We Te report 15 cases of such cyst type under the term of 'orthokeratinized odontogenic cyst (OOC)' and compared their clinical, histological and immunocytochemical features with that of OKC.Methods and results: The cysts of the present series were all solitary lesions, occurred mostly in young male patients, and showed a predilection for the posterior mandible areas, Follow-up of 14 patients, nine of whom were treated by simple enucleation, revealed no recurrence over a period of 3.5-12 years after surgery, None of the patients had any association with the naevoid basal cell carcinoma syndrome, Furthermore, histological and immunocytochemical comparison between OOC and OKC revealed marked differences in their morphology and epithelial expression, The lining epithelium of OOC lacked the typical features of OKC and appeared to show a lower proliferative activity,Conclusion: These findings suggest that OOC is clinicopathologically separate from other types of odontogenic cysts and may thus constitute a distinct clinical entity.
