Abstract:Introduction: Following the extraction of impacted mandibular third molars, there is a risk for developing new, or having persistentosseous periodontal defects on the distal aspects of the adjacent second molars.Objectives: To evaluate the effect of platelet rich fibrin (PRF) on bone healing distal to the second molar following removal ofimpacted mandibular third molar.Materials and methods: This was a parallel, controlled and randomized clinical trial. Twenty patients aged between 20-30 years,who have mesioangular position B impacted mandibular third molar indicated for surgical removal were selected for this study. Tenpatients were taken for the study group, where PRF was applied in the extraction socket of the mandibular third molar and 10 patientsfor the control group, where the extraction sockets were sutured without receiving PRF. The patients were evaluated clinically forpain, edema and trismus on the second, fourth and seventh day post-operatively and radiographically on the first, second and thirdmonth post-operatively. Standardized periapical x-ray films were taken for each patient preoperatively and on the first, second andthird postoperative months. Image-J Processing Program was used for the assessment of new bone formation at the distal surface ofthe second molar.Results: There was less pain and trismus in the study group than in the control group but the difference among them was notstatistically significant (p > 0.05). Edema was significantly lower in the study group than in the control group in the second andfourth postoperative days. There was significantly more bone density and higher bone level in the study group than in the controlgroup on the second and third postoperative months (p < 0.05).Conclusion: It is clear that PRF is biocompatible and can improve both soft tissue healing and bone regeneration after surgicalremoval of impacted mandibular third molar.

Abstract:AIM AND OBJECTIVES: This study attempted the evaluation of the efficacy of platelet-rich plasma (PRP) and platelet-rich fibrin (PRF) in alveolar defects after removal of bilateral mandibular third molars.MATERIALS AND METHODS: A total of 30 patients reporting to Department of Oral and Maxillofacial Surgery and having bilateral mandibular third molar impaction in both male and female aged between 18 and 30 years were included in this study. PRF and PRP were placed in extraction site and recalled at 2(nd), 4(th), and 6(th) month postoperatively. Data were statistically analyzed using IBM SPSS software for Windows, version 19.0. IBM Corp., Armonk, NY, USA.RESULTS: This study showed decreased probing depth in PRF group compared to PRP and control one. This signifies a better soft tissue healing of extraction sockets with PRF as compared to the PRP and the control group and increase in the bone density highlights the use of PRP and PRF certainly as a valid method in inducing hard tissue regeneration.CONCLUSION: This study indicates a definite improvement in the periodontal health distal to second molar after third molar surgery in cases treated with PRF as compared to the PRP group and control group. Hence, PRP and PRF can be incorporated as an adjunct to promote wound healing and osseous regeneration in mandibular third molar extraction sites.
High resistance to antiretroviral drugs: the Indian experience

Abstract:Effective antiretroviral treatment (ART) has significantly reduced the morbidity and mortality among people living with HIV/AIDS. The emergence of drug resistance is the major obstacle to the successful implementation of long-term treatment strategy of HIV disease. Hence, monitoring the resistance to ART is an essential task. The MGM Medical College in Navi Mumbai is the only national facility with the capacity to monitor ART resistance using tissue culture technique. The main objective of the study was to determine the prevalence of primary and secondary resistance between May 1997 and April 2003. Four hundred and sixty viral isolates of 74 individuals were tested for ART resistance using tissue culture-based protocol (McGill University, Canada). These included 287 primary and 173 follow-up viral isolates. Following activation of tissue culture, a panel of 12 antiretroviral drugs in concentrations from 0.038 mg/mL to 0.090 mg/mL was tested. The resistance was determined on the basis of the production of p24 antigen at the end of 72 and 144 hours of incubation. The overall resistance against reverse transcriptase inhibitors (RTIs) was 33.7% and that to protease inhibitors (PIs) was 21.5%. However, prevalence of primary resistance among ART naïve to RTIs was 14/208 (6.7%) of isolates and that to (PIs) was 2/79(2.5%) of isolates. The incidence of resistance to RTI was 22/500 years (4.4/100 years) and that to PIs was 8/253.6 years (3.2/100 years). It is possible that non-B subtypes of HIV-1 that are prevalent in most developing countries are likely to develop resistance rapidly after ART. However, common factors that influence development of resistance such as poor adherence, inadequate 'wild' doses and regimens, and poor monitoring were not validated in the study. Unless active surveillance is instituted, these factors can pose major challenge for the management of people on ART in developing countries.
Effect of platelet-rich fibrin on the recovery after third molar surgery: A systematic review and meta-analysis

Abstract:This meta-analysis aimed to elucidate the effects of platelet-rich fibrin (PRF) on the recovery of alveolar bone after surgical removal of the mandibular third molars. PubMed, Cochrane Library, Web of Science, and Embase databases were searched from the inception to February 2023 for relevant studies on the application of PRF after the extraction of impacted mandibular third molars, with the language limited to English. Literature screening was conducted by two independent researchers. The Cochrane risk-of-bias tool was adopted for quality evaluation, and Stata 15.0 was used for statistical analysis. A total of 33 randomized controlled trials were included in the present study. Following surgical removal of the mandibular third molars, 1139 tooth sockets were filled with PRF, while 1138 sockets were sutured after conventional saline irrigation. The meta-analyses showed that PRF can relieve pain [(RR 0.454; 95% CI 0.23, 0.891); (SMD -0.74; 95% CI -0.97, 0.52)], improve swelling (SMD -1.48; 95% CI -1.90, -1.06), alleviate trismus (SMD -0.35; 95% CI -0.51, -0.19), reduce dry socket (SMD -0.18; 95% CI -030, -0.05), and promote bone tissue healing (SMD 2.34; 95% CI 0.18, 4.51). The current study confirms that PRF can reduce some postoperative complications. Local application of PRF after lower third molar extraction is a viable method for relieving pain and swelling, reducing the incidence of dry socket and trismus, and increasing bone density. However, whether it can promote soft tissue healing remains unclear. For patients undergoing complicated surgical extraction, local application of PRF into the sockets might be a good option.

Abstract:Background: More and more studies were being conducted to improve healing after third molar surgery. The aim of this study was to evaluate the effect of PRF on soft tissue and periodontal pocket healing, alveolar bone resorption distal to the second molar after lower third molar surgical removal. Methods: A Randomized, controlled clinical trial and split-mouth design was performed on 26 healthy patients who had an indication of impacted lower third molars extraction on both sides with symmetrical orientation and the same difficult index. All patients had the same extraction protocol on both sides at the Department of Oral Surgery, Faculty of Odonto-stomatology, University of Medicine and Pharmacy, Ho Chi Minh City. PRF was applied on one side, whereas the other was left empty as the control group. Assessing the soft tissue healing, periodontal pocket depth and distal bone resorption of the study and control groups after third molar removal surgery. Data obtained were statistically analyzed. Results: There is a statistically significant difference in soft tissue healing on the 3rd and 7th postoperative days and distal bone resorption (p<0.05) on the 3rd and 6th postoperative months between two groups. Periodontal pocket depth of the study group showed more reduction than the control group but was not significantly different. Conclusion: Using PRF can help improve soft tissue healing and lower distal bone resorption after surgical extraction of impacted lower third molars. PRF appears to reduce periodontal pocket depth but still needs more research to conclude.

Abstract:It has become more common that after a mandibular third molar impaction operation, platelet concentrates have been shown to be useful in aiding the healing process. The patients were examined on day 1, day 2, day 3, day 7, the first month, and the second month following surgery. There was a slew of parameters to investigate and tally up. Materials and Methods: A total of 15 people with impacted mandibular third molars were included in the research. Patients' own blood platelets were used to make the transfusion more effective. One of the sockets was selected at random to serve as the test side. Instead of the other connector, the control socket was utilized. Results: There was no statistical difference between the test and control groups when it came to pain or soft tissue healing, according to a Mann–Whitney U test used to analyze the data. T test was performed to check for edema and density in the bones. Bone density measurements on the test side were significant, whereas measurements of edema on the test side were not. There was no alveolar osteitis on any of the subjects' limbs. Conclusion: Bone and soft tissue healing may be aided by platelet-rich plasma (PRP). Several growth components are included, which aid in the healing process.
Do the New Protocols of Platelet-Rich Fibrin Centrifugation Allow Better Control of Postoperative Complications and Healing After Surgery of Impacted Lower Third Molar? A Systematic Review and Meta-Analysis

Abstract:Purpose: Platelet concentrate generation protocols have undergone several modifications in recent years; in light of these new developments, this study review aims to evaluate the effects of platelet-rich fibrin (PRF) and the new centrifugation protocols, advanced platelet-rich fibrin (A-PRF), and leukocyte platelet-rich fibrin (L-PRF), after extraction of impacted mandibular third molar. Specifically, we assessed pain control, edema, trismus, and soft tissue healing, and also measured the degree of periodontal regeneration adjacent to the second molar. Methods: PubMed, MEDLINE, EMBASE, Web of Science, Virtual health library (BVS), and Cochrane Library were searched up to Julye 2021; randomized controlled studies were included. This report followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement and PICO (population, intervention, comparison, outcome) questions. This review has been registered at the International Prospective Register of Ongoing Systematic Reviews (PROSPERO) under the number CRD42019136701. The risk of bias screening and data extraction was performed according to the guidelines recommended by Cochrane. The quantitative analysis was performed using RevMan version 5.4. Results: Of 17 studies included in the systematic review, 11 were eligible for the meta-analysis. The use of L-PRF was not associated with better soft tissue healing at day 7. (standard mean difference = -0.70; 95% confidence interval, -3.50 to 2.10; Z = 0.49; P = .62; heterogeneity = 0.00001; I2 = 97%). With L-PRF, qualitative analysis revealed better pocket depth and insertion level, and also better pain control at 1 and 3 days. With A-PRF, a lower consumption of analgesics was observed than with L-PRF. With both A-PRF and L-PRF, better control of edema (but not trismus) was observed. Conclusions: The use of L-PRF and A-PRF allows better control of pain and edema compared with the use of standard PRF protocols, but neither has an effect on trismus. The PRF and L-PRF protocols improve soft tissue healing, although not to a statistically significant degree; however, they could improve probing depth at the third month after third molar surgery.

Abstract:BACKGROUND: Postoperative pain and swelling following third molar (M3) removal can be debilitating, and there is interest in using advanced platelet-rich fibrin (A-PRF) to reduce their severity.PURPOSE: This study compared postoperative pain and swelling between A-PRF and gelatin dressing in extraction sockets following mandibular M3 removal.METHODS, SETTING, SAMPLE: This split-mouth, single-blinded, randomized controlled trial was completed at the Oral Surgery clinic of University of Otago between November 2020 and July 2021. Patients aged between 16 and 40 years with bilaterally impacted mandibular M3 of similar Pederson index difficulty and deemed to be American Society of Anesthesiologists (ASA) I or II comprised the study sample.PREDICTOR/EXPOSURE/INDEPENDENT VARIABLE: The exposure variable was M3 socket management. One socket received A-PRF and 1 gelatin dressing, while the other received 2 gelatin dressings.MAIN OUTCOME VARIABLE(S): The outcome variables were postoperative pain and swelling over 7 days. Pain was measured using the visual analog scale (VAS), and swelling using stereophotogrammetry.COVARIATES: Demographic characteristics (gender, ethnicity, and age), dental anxiety, smoking status, Pederson index, and intraoperative surgical time were the covariates.ANALYSES: The two sides were compared using cross-tabulations and the McNemar test for categorical and paired t-tests for continuous variables. Statistical analysis used IBM SPSS Statistics for Windows (version 28).The Alpha level was 0.05.RESULTS: 76 (87.3%) of 87 patients who met the eligibility criteria participated in the study, and 70 patients (65.7% female; age range 16-30 years) were included in the analysis. Mean visual analog scale scores showed no statistically significant difference between the A-PRF and control sides, being 29.6 (95% CI 23.9, 35.3) and 29.5 (95% CI 23.5, 35.5) on day 2, and falling to 12.6 (95% CI 8.7, 16.5) and 14.2 (95% CI 10.0, 18.4) by day 7. Likewise, mean peak facial swelling on day 2 was recorded as 6.3 cm3 (95% CI 4.9, 7.7) and 6.6 cm3 (95% CI 5.5, 7.7), and by day 7 they were 1.1 cm3 (95% CI 0.5, 1.7) and 1.0 cm3 (95% CI 0.3, 0.7) on the A-PRF and control sides, respectively.CONCLUSIONS: A-PRF placement in M3 sockets did not reduce postoperative pain and swelling over gelatin dressing alone.

Clinical and Biochemical Evaluation of the Use of Alb-PRF versus L-PRF in Mandibular Third Molar Extractions: A Split-Mouth Randomized Clinical Trial

Abstract:Bone tissue engineering seeks biomaterials that enable cell migration, angiogenesis, matrix deposition, and tissue regeneration. Blood concentrates like platelet-rich fibrin (L-PRF) offer a cost-effective source of cells and growth factors to enhance healing. The present study aimed to evaluate heated serum albumin with liquid PRF (Alb-PRF) and L-PRF clinically and biochemically after placement in dental sockets following mandibular third molar extraction. In a controlled, split-mouth study involving 10 volunteers, 20 extracted molars were treated with either Alb-PRF or L-PRF. Post-extraction, pain, trismus, infection presence, and swelling were measured. The concentrations of different analytes in the surgical sites were also examined. The data were statistically analyzed, with significance defined at p < 0.05 (t-test). No significant difference was noted between the groups for pain and trismus, but Alb-PRF showed a significant reduction in swelling on day seven. The Alb-PRF group showed lower levels of pro-inflammatory cytokines (GM-CSF, IL-1b, IL-6, IFNy, IL-8, IL-15, RANTES, and MIP-1a) after seven days, with only higher expressions of MIP-1b, IL-1b, and MCP-1 found in the L-PRF group. Differences were observed in the release of analytes between L-PRF and Alb-PRF, with Alb-PRF significantly reducing edema after seven days. Alb-PRF reduced edema, while L-PRF increased inflammatory cytokines. When compared to L-PRF, Alb-PRF reduced edema and the release of inflammatory cytokines, suggesting promising effects in socket healing while underscoring the role of growth factors and cytokines in potential applications of blood concentrates.

Abstract:Objective: The surgeries for extraction of impacted lower third molars are more associated with complications such as pain and postoperative edema. The purpose of the present study was to evaluate the impact of the use of Leukocyte- and Platelet-Rich Fibrin (L-PRF) on pain and edema after extraction of impacted mandibular third molars. Material and Methods: A cross-sectional split-mouth study was conducted with 15 volunteer patients who had both impacted mandibular third molars with indication for extraction. The removal surgery was performed one side at a time, with an interval of 14 days. One side received L-PRF while the other did not. Patients were evaluated on the 2nd and 7th postoperative days for mouth opening, pain and edema. Results: In total, data from 14 patients (4 men and 10 women), with a mean age of 21.83 ± 2.8 years were evaluated. There was no statistically significant difference regarding mouth opening, pain and edema on the 7th postoperative day. On the second postoperative day, greater edema was observed in the control group with a statistically significant difference (p = 0.01). Conclusions: Under controlled conditions, the L-PRF does not appear to alter postoperative recovery after extraction of the lower third molars in terms of pain, edema and the ability to open the mouth. Despite the statistically significant difference in edema on the second day, the means were very close and were not clinically relevant.

Abstract:Introduction. Alveolar ostitis (AO) is the dissolution of blood clot due to enhanced local fibrinolysis and is caused by trauma to the jaw (direct) or because of bacterial involvement (indirect), which result in the activation of plasminogen pathway. Platelet-rich fibrin (PRF) is a platelet concentrate that comprises numerous autologous growth factors, and immune cells hence has the potential to expedite the healing process. The objective of the study was to determine the efficacy of PRF in the surgically extracted third molar in the context of its potential progress to AO. Materials and Methods. A total of 180 patients of 18–65 years with unilateral painful mandibular third molars due to caries, failed endodontics treatment, and pericoronitis were included in the study. Exclusion criteria were patients who were medically compromised, smokers, alcoholic, poor oral hygiene, third molar having associated periapical pathology, and receiving antibiotic regime in the last 2 weeks. Before starting surgical procedure, patients were randomly divided into two groups using lottery method. Group I received PRF in the extraction socket, while in Group II, the extraction site was left for normal healing as practiced in a standard procedure. Pain was assessed in terms of pain score, and it was recorded on a 10 mm visual analog scale on the first and third postoperative days. Results. Mean age of the patients was 41.35 ± 9.87 years. The mean age in Group I was 42.84 ± 10.52 years, and in Group II, it was 40.54 ± 9.52 years. Out of 180 patients, 90 (50.0%) were male and 90 (50.0%) were female, with a male-to-female ratio of 1 : 1. Frequency of AO following mandibular third molar surgery in Group I receiving PRF was 2.22% and in non-PRF group 12.22% (p-value = 0.010). Conclusion. The incidence of AO following mandibular third molar surgery was lower when PRF was used.

Abstract:PurposeThis study evaluates the efficacy of platelet rich plasma (PRP) & porous hydroxyapatite crystals in bone regeneration after surgical removal of mandibular third molar with the help of radiographs and its comparison with control side.Materials and MethodsA total of 40 patients; both male and female aged between 18 and 35 years, who had impacted mandibular third molars were randomly selected for this study. Twenty patients were taken for control group and 20 patients for study group. The extraction socket of the study group was packed with PRP and hydroxyapatite granules and that of control group was sutured without PRP and hydroxyapatite. The bone density of both extraction sockets were evaluated radiographically using gray level histogram and compared periodically on immediate postoperative day, 1st and 3rd month postoperatively and postoperative sequelae of both the control group and study group in terms of oedema & pain or any other adverse reactions were also assessed.ResultsData suggested evidence of early bone formation and maturation radiographically in study group as compared to control group. The percentage of facial swelling was numerically greater on the control side as compared to the study side, Pain was also assessed with VAS and it was found that the severity of pain was equal in both study and control groups and the results were not significant.ConclusionThis study clearly indicated a definitive improvement in the wound healing, increase in bone density, which signifies and highlights the use of PRP and hydroxyapatite granules, certainly as a valid method in inducing and accelerating bone regeneration.
Autologous platelet concentrates after third molar extraction: A systematic review

Abstract:Surgical removal of impacted mandibular third molars is often followed by postoperative sequelae like pain, swelling, trismus, etc. This systematic review explored the benefits of applying different autologous platelet concentrates (APCs) in the extraction socket of third molars. For this systematic review, PubMed, EMBASE, Web of Science, and Scopus have been utilized, initially yielding 544 papers. The search was narrowed to randomized controlled trials (RCTs, n = 59) published before 2024, all comparing the outcome of applying APCs in the extraction socket of surgically removed impacted mandibular third molars with unassisted healing (blood clot). Most RCTs primarily assessed the impact of APCs on postoperative sequelae. Some RCTs looked at soft‐ and hard‐tissue healing. Eleven studies used PRP, three PRGF, and 45 L‐PRF. A detailed analysis revealed a large heterogeneity between studies rendering a meta‐analysis impossible. Moreover, the risk of bias was considered high. In the majority of RCTs, the application of an APC resulted in statistically significant reductions of postoperative sequelae (lower pain intensity, lower consumption of analgesics, less postoperative edema, and a lower incidence of trismus and alveolar osteitis), as well as a faster soft tissue healing, and qualitatively and quantitatively better bone healing. A minority of studies reported significant differences in periodontal parameters distally from the second molar.

Abstract:Objectives: The current study aim was to evaluate the effect of Bioactive Hydrogel versus Advanced-PRF on bone regeneration following impacted mandibular third molar surgery. Material & Methods: sixty patients who had been scheduled for surgical removal of mesio-angular or horizontal impacted mandibular 3rd molar teeth were randomly divided into three groups. Group A involved 20 patients received A-PRF-Xenograft at the surgical site. Group B involves 20 patients received Xenograft-Hydrogel at the surgical site. While group C involved 20 patients left to heal without graft as control. Clinical and radiographic evaluation were performed immediate and at 6 months postoperatively. Data was collected and analysed statistically. Results: At 6-month postoperative evaluation, the current study found that A-PRF-Xenograft group showed significant shorter mean pain duration (5.1±1.4 days) compared to Hydrogel-Xenograft group (5.3±1.5 days) and control group (5.8±1.4 days). A-PRF-Xenograft group showed the lowest mean values of PPD (3.7±0.6) followed by Hydrogel-Xenograft group (5±0.8) and followed by control group (6.6±0.6). A-PRF-Xenograft group showed the highest mean values of bone density (605.3±85.5) followed by Hydrogel-Xenograft group (522.4±83.5) followed by control group (286.4±44.7) Conclusion: According to the results of the present study, there was significant improvement regarding pain duration, mouth opening limitation, periodontal pocket depth and bone density at the surgical site where A-PRF-Xenograft was applied. Hydrogel-Xenograft Group was more superior to the control group regarding pain duration, PPD, and bone density.
Laparoscopic Approach for Small Bowel Tumors

Abstract:Aim To review the outcome of laparoscopic small bowel resections as compared with open resection for small bowel tumors. Method Retrospective review comparing total laparoscopic to open resections for small bowel tumors from October 1998 to July 2005 in a single centre. Results During the study period, 9 patients underwent laparoscopic resection, whereas 11 patients underwent open small bowel resection. Majority of the patients (75%) suffered from gastrointestinal stromal tumors. Median operative time in both groups was comparable, but the median operative blood loss was significantly less in the laparoscopic group. (5 vs. 165 mL, P=0.005). The amount of oral dologesic used postoperatively was similar, whereas the use of intravenous morphine was significantly less in the laparoscopic group (7.4 vs. 21.3 mg, P=0.007). The median postoperative hospital stay was significantly shorter in the laparoscopic group (7 vs. 12 d, P=0.018). For patients with gastrointestinal stromal tumors, 2 patients (29%) in the laparoscopic group and 3 patients (38%) in the open group had recurrence of disease during follow-up (P=0.573). Conclusions Laparoscopic resection resulted in favorable short-term outcome compared with open resection for small bowel tumors in selected cases. The oncologic outcome is not compromised with this approach.

Abstract:Background and objectives: All surgical procedures produce some secondary effects such as pain, swelling, and some functional impairments like difficulty in chewing mouth opening, speaking, sleeping, etc. Controlling postoperative discomfort may improve the patient's quality of life and acceptance of the treatment. The goal of this randomized single-blind trial was to see if using platelet-rich fibrin during endodontic surgery had a positive impact on pain and other aspects of the patient's quality of life in the first week after surgery. Methods: Fifteen patients with periapical lesions were treated with endodontic surgical procedures (control group). In another 15 patients, platelet-rich fibrin was placed into the bone defect in the form of a clot, and a suture was applied in addition to the surgical procedure (test group). During the first week after surgery, all patients completed a questionnaire to assess their major symptoms and daily activities. The results of the two groups' questionnaires were statistically compared. Results: The test group reported less pain, symptoms (swelling and bad taste), and functional activities compared with the control group but the difference was not statistically significant. Conclusion: The adjunct of platelet-rich fibrin to the endodontic surgical procedure produced some beneficial effects on patients' quality of life during the early postoperative stage. There were insufficient data in the literature and more clinical trials with larger samples have to be carried out in the future to find out the outcome

Abstract:Introduction: Third molar extraction is a common surgical procedure that can have unintended consequences, such as periodontal disease that results in bone loss on the distal part of the second molar. By halting the atrophy of the alveolar ridge, socket and ridge preservation procedures enable restorations that are both functional and aesthetically acceptable. Maintaining the volume of the alveolar socket after tooth extraction is the primary objective of alveolar management, as it prevents substantial resorption and facilitates socket repair. Numerous methods have been reported on socket healing following tooth extractions. Grafting materials may promote healing of the post-extraction socket in three separate ways: osteogenesis, osteoinduction and osteoconduction. Need for the Study: This study is necessary because managing bony defects after the extraction of impacted mandibular third molars is critically dependent on successful interventions. Third molars that are impacted often need to be surgically removed, resulting in bony defects that require proper regeneration for the best possible outcome after surgery. An important consideration in this process is the selection of grafting materials. It is imperative to comprehend the relative effectiveness of dentine graft and Platelet-rich Fibrin (PRF) to assist clinicians in deciding the best treatment option. Both PRF and dentine graft have shown promise in fostering bone regeneration; however, to develop evidence-based guidelines for clinical practice, a direct comparison using a rigorous split-mouth study is essential. Aim: To compare and evaluate outcomes of dentine graft against PRF in postoperative impacted mandibular third molar bony defects. Materials and Methods: A prospective randomised splitmouth study will be conducted in the Department of Oral and Maxillofacial Surgery at Sharad Pawar Dental College, DMIHER, Sawangi, Wardha, Maharastra, India from April 2023 to January 2025. After the extraction of the 3rd molar, the patient will be instructed to bite on a sterile swab while waiting for the biomaterials to be ready. Following the examination of the extraction socket, the particulate dentine graft and liquid PRF preparation will be made and then placed in the extraction socket. The patient will then be instructed to use chlorhexidine mouth rinse (0.2%) twice a day in place of mechanical plaque management in the treated region for one week, as part of the postoperative regimen. Postoperative pain, wound healing, bone regeneration, and complications, if any will be assessed at a specified interval. Fisher’s-exact test will be used for statistical analysis, and the level of significance will be set at p<0.05.

Abstract:BACKGROUND Many patients with epilepsy travel abroad and drive automobiles with the assumption that policies, rules, and regulations on epilepsy and driving are similar to those of their home countries. This paper investigates the driving restrictions and other pertinent information on this issue in foreign countries. METHODS A questionnaire was sent to 231 neurologists (chosen from American neurological and epilepsy societies) from 84 countries and to 230 official (embassies and consulates) representatives of 134 countries asking for the local rules and regulations and their comments on driving and epilepsy. RESULTS One hundred and sixty-six responses were received from 96 of 134 (72%) countries. One hundred and six neurologists (of 231 queried [46%]) responded. In 16 countries, persons with epilepsy are not permitted to drive. In the remaining countries, these patients must have a seizure-free period of 6 to 36 months. This period varies according to the type of seizure. In five countries, physicians must report the names of these patients to their local authorities. In many countries, the rules and regulations are being reevaluated and changed. CONCLUSIONS Patients with epilepsy who plan to drive overseas are advised to contact local embassies and consulates, well before their trips (and keep records of the communications) to obtain the latest information on the rules and regulations governing the driving of automobiles in those countries.
