Abstract:Background: Intervertebral foramen endoscopy has developed rapidly, but compared with transforaminal lumbar interbody fusion (TLIF), the progress of degeneration is unknown. We aim to compare the changes of intervertebral disc and intervertebral foramen in adjacent segments after TLIF and endoscopic discectomy for patients with lumbar disc herniation (LDH). Methods: From 2014 to 2017, 87 patients who were diagnosed with single-level LDH and received surgery of TLIF (group T, n = 43) or endoscopic discectomy (group F, n = 44) were retrospectively analyzed. X-ray, MRI, CT and clinical symptoms were recorded before operation and at the last follow-up (FU). The neurological function was originally evaluated by the Japanese Orthopaedic Association (JOA) scores. Radiological evaluation included the height of intervertebral space (HIS), intervertebral foramen height (FH), intervertebral foramen area (FA), lumbar lordosis (CA) and intervertebral disc degeneration Pfirrmann scores. Results: There was no significant difference in baseline characteristics, JOA improvement rate, reoperation rate and complications between the two groups. The age, average blood loss, average hospital stays and average operation time in group F were lower than those in group T. During the last FU, HIS, CA and FA decreased in both groups, and the changes in group T were more significant than those in group F (P < .05). There was no significant difference in FH changes between the two groups (P > .05). Conclusion: Both TLIF and endoscopic surgery can achieve good results in the treatment of LDH, but the risk of lumbar disc height loss and intervertebral foramina reduction in the adjacent segment after endoscopic surgery is lower.

Abstract:The clear indication for the transforaminal approach is simple or low migrated lumbar disc herniation (LDH). The incidence of migration of the disc fragment in LDH varies from 35% to 72%. The high up migrated disc herniation into MacNab’s hidden zone remains technical challenge. We provide a technical note of seldomly performed full endoscopic translaminar approach with its potential indications. A 52 years male patient presented with low back pain with left lower limb radiculopathy for 2 months. Visual analogue scale for back and leg was 2 and 8, respectively. MRI examination revealed high up migrated disc herniation with collapsed disc space at L4-5. Dynamic radiograph was not showing any instability. Based on the preoperative surgical planning working cannula was docked over left L4 lamina perpendicular to horizontal plane. With the help of 3.5 mm endoscopic burr translaminar keyhole was drilled in a concentric manner. The distance between lateral margin of pars and keyhole was intermediately inspected for preservation of adequate bone in pars interarticularis. Though the translaminar key hole subluxated tip of SAP, up-migrated disc fragment was removed. The VAS score of leg reduced significantly from 8 to 2 in immediate postoperative period. At the end of 6 months follow up period patients leg symptoms completely improved with minimal back pain. We measured the width of remaining pars postoperatively and found to be 5 mm. The full endoscopic translaminar approach is valid alternative approach for the high up migrated HNP.
Clinical application of microendoscopic discectomy for the treatment of lumbar disc herniation

Abstract:Objective To analyse the clinical effects of microendoscopic discectomy(MED)with traditional discectomy by laminectomy fenestration,and explore the advantages of MED.Methods Retrospective analysis of the clinical data of 121 cases of lumbar disc herniation were made.The study group(52 cases)were treated with MED,the control group(69 cases)treated with traditional discectomy by laminectomy fenestration(open surgery).The incision length,hemorrhage in operation,operation time,ratio of improved raising degree by straight limb test,the time of out of bed,residence time after operation,recovery time,the rate of excellent and good outcomes according to Macnab scale after 3 months operation in study group were compared to the control group.The complications were recorded.Results The rate of excellent and good outcomes in study group treated with MED was 98.08% compared with 88.41% in the control group,and there was no statistical difference in two groups(χ 2=2.75,P>0.05).But there were statistical differences in incision length,hemorrhage in operation,ratio of improved raising degree by straight limb test,the time of out of bed,residence time after operation,and recovery time of the two groups(t=30.27,22.75,1.46,11.47,13.85,14.69,16.04,all P< 0.05)except in operation time(t=1.46,P>0.05).There was one case suffered more pain after MED,two cases occurred dura mater ruputure in control group.Conclusions To treat lumbar disc herniation,MED has the advantages of minimaltrauma to lumbar tissue and less recovery time compared with open surgery.
Lumbar disc reherniation after transforaminal lumbar endoscopic discectomy

Abstract:Background: Transforaminal lumbar endoscopic discectomy is a minimally invasive surgical procedure that can be performed in awake patients through an incision less than 1 cm. The procedure requires very little bony removal to access the herniated disc material because the approach is through the foramen, and only a small amount of the superior articulating process is removed to access Kambin's triangle. This study describes our experience with transforaminal endoscopic lumbar decompression (TELD) for the treatment of lumbar disc herniation. We evaluate the risk for reherniation in the first year after surgery and the characteristics of the patients who experience reherniation. Methods: We describe the technique for the transforaminal endoscopic approach to treat lumbar disc herniations. Retrospectively, a series of 141 consecutive patients, who were operated on with lumbar radiculopathy, was analyzed. We excluded patients who had previous surgery at the lumbar level, surgeries done for disease adjacent to a fusion, and surgeries done for spondylolisthesis. A total of 84 consecutive patients were included who had single level lumbar non-revision surgery and at least 1-year follow up. Results: A series of 46 consecutive male and 38 female patients with an average age of 57.4 years (range, 28-87 years old) who underwent transforaminal endoscopic treatment for lumbar disc herniations between 2014 and 2016 is presented. Four patients required microdiscectomy due to reherniation at 5 months, 8 months, 9 months, and 10 months postoperatively. All the patients in the series improved immediately following their endoscopic procedures, and no patients presented with symptoms suggestive of reherniation until 5 months after their initial endoscopic surgery. Patients with reherniation tended to be young: 31, 45, 48, and 49 years of age: all less than the average patient age who underwent endoscopic surgery. Conclusions: Transforaminal endoscopic surgical access to lumbar disc herniations is an ultra-minimally invasive approach for the treatment lumbar degenerative disc disease. It allows for neural decompression by removing disc and foraminal pathology with minimal bone removal. This minimal bone removal prevents iatrogenic destabilization. However, the 1-year reherniation rate presented here is 4.7%. This suggests that the benefit of this technique may be that it is ultra-minimally invasive, but it may only be equal, not superior to microdiscectomy in its rate of reherniation.

Abstract:Surgery of lumbar disc herniation is still a problem since Mixter and Barr. Main trouble is dissatisfaction after the operation. Today there is a debate on surgical or conservative treatment despite spending great effort to provide patients with satisfaction. The main problem is segmental instability, and the minimally invasive approach via microscope or endoscope is not necessarily appropriate solution for all cases. Microsurgery or endoscopy would be appropriate for the treatment of Carragee type I and type III herniations. On the other hand in Carragee type II and type IV herniations that are prone to develop recurrent disc herniation and segmental instability, the minimal invasive techniques might be insufficient to achieve satisfactory results. The posterior transpedicular dynamic stabilization method might be a good solution to prevent or diminish the recurrent disc herniation and development of segmental instability. In this study we present our experience in the surgical treatment of disc herniations.
Minimally Invasive Transforaminal Lumbar Interbody Fusion for the Treatment of Degenerative Lumbar Diseases

Abstract:Study Design. Prospective cohort study.Objective. To determine whether minimally invasive transforaminal lumbar interbody fusion (TLIF) using the tubular retractor system reduces the approach-related morbidity inherent in conventional open surgery.Summary of Background Data. Posterior lumbar fusion using the tubular retractor system has been reported and described well. Supporters have claimed that minimally invasive techniques reduce soft-tissue trauma, blood loss, postoperative pain, transfusion needs, and the length of hospital stay, as compared with reports describing the traditional open procedure. However, there are few studies of minimally invasive TLIF, especially studies that directly compared minimally invasive and open approaches in a single center.Methods. Between May 2005 and December 2006, a total of 62 patients underwent 1-level TLIF by 1 surgeon in 1 hospital. Of 62 patients, 32 underwent minimally invasive TLIF using the tubular retractor system, and the other 30 underwent the traditional open procedure. The operative duration, blood loss, complications, and recovery time were recorded. The clinical outcomes were evaluated by the Oswestry Disability Index and the Visual Analog Scale. The soft-tissue injury was assessed by measuring serum creatine kinase. Radiographic images were obtained before surgery and during follow-up.Results. The minimally invasive group was found to have reduced blood loss, fewer transfusions, less postoperative back pain, lower serum creatine kinase on the third postoperative day, a shorter time to ambulation, and a briefer hospital stay. The Oswestry Disability Index and Visual Analog Scale scores were significantly lower in the minimally invasive group during follow-up. However, the open group had a shorter operative duration. The complications in the 2 groups were similar, but 2 cases of screw malposition occurred in the minimally invasive group.Conclusion. Minimally invasive TLIF as a management of 1-level degenerative lumbar diseases is superior to the traditional open procedure in terms of postoperative back pain, total blood loss, need for transfusion, time to ambulation, length of hospital stay, soft-tissue injury, and functional recovery. However, this procedure takes longer operative duration and requires close attention to the risk of technical complications. Longer-term studies involving a larger sample are needed to validate the long-term efficacy of minimally TLIF.
Combined Microscopic Tubular and Endoscopic Approach for Calcified Midline Thoracic Disc Herniation: A Bridge to Endoscopic Discectomy

Abstract:Symptomatic thoracic disc herniation (TDH) is relatively uncommon and can present with thoracolumbar pain, myelopathy, bladder dysfunction, and motor dysfunction. Midline TDHs and calcified discs are more challenging to access and treat compared to the cervical or lumbar region due to the narrow working corridor around the lungs, ribs, and thoracic spinal cord. Open approaches such as the transthoracic or retropleural approach are particularly morbid. Minimally invasive endoscopic techniques offer decreased tissue dissection and manipulation of the thecal sac but involve a more difficult learning curve. We present a posterolateral approach using a minimally invasive tubular retractor and microscope, which is like minimally invasive techniques many surgeons are already accustomed to using, combined with an endoscope through the tubular retractor. The patient is a 57-year-old female who presented with gait instability due to balance problems and mild bilateral leg "heaviness" and weakness. Her neurologic exam was remarkable for bilateral leg weakness, decreased sensation at the T12 level, hyperreflexia in the bilateral lower extremities, a positive Romberg test, and a wide-based gait. Magnetic resonance imaging revealed disc extrusion at T11-T12 and ligamentum flavum infolding causing mild central canal narrowing, resulting in a mass effect on the cord. We performed a minimally invasive discectomy using a tubular approach combined with an endoscope to access the ventral midline without manipulation of the spinal cord. A combined microscopic and endoscopic may allow surgeons already comfortable with microscopic surgery to master the learning curve of endoscopic techniques.

Scholars investigating lobbying in the EU have made great strides and are now fully integrated into an international community of scholarship, more so than in previous decades. Being clear about the wide range of actors, including government officials, who may play the role of advocates, understanding the complexities of the various venues of policy-making within the EU, and studying framing processes systematically are important challenges as the literature continues to move forward.

Abstract:PurposeTo date, various minimally invasive surgeries for lumbar disc herniation (LDH) have been performed in many countries, including microendoscopic discectomy (MED) and percutaneous endoscopic discectomy (PED). We herein report the surgical indications, current concepts, and techniques of minimally invasive discectomy for LDH, including MED and PED. The postoperative outcomes and risks of complications compared with conventional discectomy are also discussed.MethodsBy reviewing past literatures, the current concepts, surgical techniques, and clinical outcomes were described.ResultsMinimally invasive discectomy is generally performed using a transmuscular approach with an endoscopic technique and can reduce muscle and soft tissue damage. The minimal invasiveness can also reduce the severity of postoperative pain and improve patient satisfaction compared with conventional open discectomy. Based on the data of previous systematic reviews and meta-analyses, minimally invasive lumbar discectomy, such as MED and PED, appears to be safe and effective alternatives to open discectomy for patients with symptomatic LDH. These minimally invasive surgeries can provide beneficial effects to shorten the hospital stay and time to return to work. However, minimally invasive discectomy has a demanding learning curve. Only a well-trained surgeon and support team should perform these less invasive procedures.ConclusionsClinical evidence supporting the superiority of MED and PED is still limited. Well-designed prospective multicentered randomized controlled trials with large samples are therefore needed in order to draw a convincing conclusion.

Percutaneous Endoscopic Interlaminar Discectomy for Intracanalicular Disc Herniations At L5–S1 Using a Rigid Working Channel Endoscope

Abstract:Abstract OBJECTIVE: Percutaneous endoscopic transforaminal discectomy is often used as a minimally invasive procedure for lumbar disc herniation. However, a transforaminal approach posts limitations at the L5–S1 level owing to anatomic constraints, such as a high iliac crest or small intervertebral foramen and especially for migrated large intracanalicular disc herniations. We discuss the procedure and clinical results of percutaneous endoscopic interlaminar discectomy using a rigid working channel endoscope at the L5–S1 level and the relevant surgical anatomy. METHODS: We performed percutaneous endoscopic discectomy through the interlaminar approach in 67 patients who satisfied our inclusion criteria during the period from March 2002 to November 2002. All procedures were performed under local anesthesia. Under fluoroscopic guidance, we performed discography using indigocarmine mixed with radio-opaque dye. The 6-mm working channel endoscope was then introduced into the epidural space. Herniated disc material was removed using forceps and laser under clear endoscopic visualization. We retrospectively evaluated the 65 cases with more than 1.5 years of follow-up. The patients were evaluated using the visual analogue scale (VAS) and the Oswestry Disability Index (ODI). RESULTS: VAS for leg pain (preoperative mean, 7.89; postoperative mean, 1.58) and ODI (preoperative mean, 57.43; postoperative mean, 11.52) showed statistically significant (P = 0.00) improvement in their values at the last follow-up examination compared with preoperative scores. Of the study group, 90.8% individuals showed favorable result. The mean hospital stay was 12 hours. The average time to return to work was 6.79 weeks. Complications included two cases of dural injury with cerebrospinal fluid leakage, nine cases of dysesthesia that were transient, and one case of recurrence. Two patients required conversion to open procedure at the initial operation. There was no evidence of infection in any patients. CONCLUSION: Percutaneous endoscopic interlaminar discectomy is a safe, effective, and minimally invasive procedure for the treatment of intracanalicular disc herniations at the L5–S1 level in properly selected cases, especially when the transforaminal approach is not possible because of anatomic constraints.

Abstract:BACKGROUND:Thoracolumbar disc herniation (TLDH) is a rare disorder with unique characteristics that can result in undesirable surgical outcomes after traditional discectomy. In view of the widespread use of transforaminal endoscopic discectomy for lower lumbar disc herniation, we investigated treatment of TLDH by this procedure. The purpose of this study was to evaluate the clinical efficacy of transforaminal endoscopic discectomy for treating TLDH and share our technical experience.METHODS:We retrospectively evaluated the clinical data of 19 patients who had undergone transforaminal endoscopic discectomy for TLDH in our institution between April 2018 and July 2021. Operation time, follow-up time, blood loss, postoperative hospital stay, visual analog scale scores for low-back and leg pain, and Japanese Orthopedic Association (JOA) scores were evaluated.RESULTS:The differences between preoperative and postoperative JOA and visual analog scale scores were significant (P < 0.05). According to the JOA scores, 14 of the 19 patients had excellent improvement, 3 had good improvement, and 2 had fair improvement; thus, the rate of satisfactory improvement was 89.5%.CONCLUSIONS:Operation time, blood loss, postoperative hospital stay, and surgical outcomes were favorable. Transforaminal endoscopic discectomy is an ideal surgical procedure for treating TLDH.
Technology adoption as process: A case of integrating an information-intensive website into a patient education helpline

Abstract:This study followed the introduction of the Arthritis Source website into the existing teaching practices of Arthritis Foundation Helpline volunteers. The goal was to examine what factors may affect a particular group of educators adopt a potentially valuable Internet tool into an existing instructional environment. Defining the possible uses of the website in reference to the volunteers' actual job duties helped provide a clearer understanding of how the volunteers might use this new technology. The researchers used qualitative techniques to focus on three volunteers who experienced different physical, environmental and cognitive means that impacted their use of the new tool. Each volunteer experienced varying levels of motivation in areas of learning, satisfaction and responses to outside influences. Each volunteer also had varying amounts of opportunity prompts in which to interact or refer the website. Consequently, Helpline volunteers experienced different rates of adopting the information-intensive website into their traditional work system.

Abstract:Upper lumbar (L1-2, L2-3) disc herniations are distinct in their diffuse presenting clinical symptomatology and have poorer outcomes with surgical intervention than those following mid and lower lumbar disc herniations and disc surgery. The authors present the cases of 3 patients with L1-2 disc herniations and significant stenosis of the spinal canal. The surgical approach used here combined the principles of transforaminal percutaneous endoscopic discectomy and the extreme lateral lumbar interbody fusion procedures with intraoperative CT-guided navigational assistance. The approach provides a safe corridor of direct visualization to the ventral thecal sac with minimal bony resection and could, in principle, reduce neurological injury and biomechanical instability, which likely contribute to poor outcomes at this level.
Minimally Invasive, Far Lateral Lumbar Microdiscectomy with Intraoperative Computed Tomography Navigational Assistance and Electrophysiological Monitoring

Abstract:Background: Patients with far lateral disc herniation (FLDH) experience more severe pain and sensory symptoms compared with those with paracentral disc herniation. In addition, surgical intervention has both been more challenging and resulted in poorer outcomes. Methods: We report our experience with intraoperative computed tomography (iCT) navigation-assisted minimally invasive tubular microdiscectomy via a paramedian approach with electrophysiological monitoring for precise 3-dimensional anatomical localization and early electrophysiological identification of the exiting nerve. Results: Five patients presenting with weakness and pain refractory to conservative management underwent iCT navigation surgery for lumbar FLDH with electrophysiological monitoring. The mean decrease in the visual analog scale pain score was -7.1 and the modified MacNab criteria outcomes were good in 1 patient and excellent in 4 patients. Conclusions: These results from a small group of patients suggest this is a safe approach with the potential for improved outcomes in the surgical treatment of FLDH.
Suprapedicular Retrocorporeal Technique of Transforaminal Full-Endoscopic Lumbar Discectomy for Highly Downward-Migrated Disc Herniation

Abstract:Objective: Anatomical barriers (e.g., pedicles, narrow foramina) can hinder direct access to, and removal of, disc fragments that have migrated far downward. Using transforaminal full-endoscopic lumbar discectomy (FELD), we devised a modified technique, the suprapedicular retrocorporeal approach, for herniations in which the disc has migrated to the axilla of the traversing nerve roots. In the present report, we have described our preliminary results. Methods: Soft, highly downward-migrated disc herniation was treated with transforaminal FELD through the suprapedicular retrocorporeal approach in 22 patients from June 2017 to May 2019. The clinical outcomes were evaluated, including the preoperative and postoperative visual analog scale scores for the back and leg, Oswestry disability index, and MacNab criteria for surgical success. Results: The affected discs were at L4-L5 in 14 patients, L3-L4 in 6 patients, and L5-S1 in 2 patients. In each case, the affected disc was successfully removed using the suprapedicular retrocorporeal approach. The mean follow-up was 18.1 ± 5.7 months. The mean visual analog scale scores for back and leg pain improved significantly (P < 0.05 for both). The mean Oswestry disability index had decreased from 62.5 ± 14.2 preoperatively to 10.5 ± 5.9 postoperatively (P < 0.05). Using the MacNab criteria, 13 patients reported excellent outcomes and 9, good outcomes. No complications or recurrence developed during follow-up. Conclusions: The suprapedicular retrocorporeal technique is a feasible and effective surgical option in transforaminal FELD for the treatment of herniation in which the disc has migrated to the axilla of the traversing nerve roots.

Abstract:With technical advancements, the full-endoscopic transforaminal approach for lumbar discectomy (ETALD) is gaining popularity. This technique utilizes various tools and instruments, including a dilator, a beveled working sleeve, and an endoscope with a 20-degree angle and 177 mm length, equipped with a 9.3-diameter oval shaft and a 5.6 mm diameter working channel. Additionally, the procedure involves using a Kerrison punch (5.5 mm), rongeur (3-4 mm), punch (5.4 mm), tip control radioablator applying a radiofrequency current of 4 MHz, fluid control irrigation and suction pump device, 5.5 mm oval burr with lateral protection, burr round, and the diamond round. During the surgery, it is essential to identify significant landmarks, including the caudal pedicle, ascending facet, annulus fibrosis, posterior longitudinal ligament, and the exiting nerve root. The steps of the technique are relatively easy to follow, especially when utilizing the appropriate instruments and having a good understanding of the anatomy. Research studies have demonstrated comparable outcomes to open microdiscectomy techniques. ETALD presents itself as a safe option for lumbar discectomy, as it minimizes tissue disruption, results in low postoperative surgical site pain, and allows for early mobilization.

Abstract:Surgical management of far lateral disc herniations remains challenging. Current transforaminal full-endoscopic approaches require non-visualized docking in the Kambin's triangle and have been associated with significant risk of inadvertent nerve injury. We develop a full-endoscopic approach based on reliable bony landmarks allowing for visualization of the exiting nerve root prior to the far lateral discectomy.

Clinical Study on Posterior Approach Microendoscopic Discectomy for Treatment of Lumbar Disc Herniation

Abstract:Objective To evaluate the posterior approach to lumbar vertebral disc herniation by microendoscopic discectomy. Methods Thirty-six patients with lumbar disc herniation were posterior approached with microendoscopic discectomy from January, 2002 through December, 2006 and the outcomes were evaluated according to the NaKai standards. Results All the patients were successfully completed the surgical procedure. The complications included 1 cerebrospinal fluid leak and 1 nerve root injury. Neither infection nor major vascular injury and other complications occurred. Followed-up from 12 to 36 months, average 24 months, demonstrated that the functional recovery excellent rate was 91.67%. Conclusions Posterior approach microendoscopic discectomy for lumbar disc herniation have the advantages of minimal invasive surgical procedures, quicker recovery, definite effectiveness, and fewer complications, which is valid in treatment of lumbar disc herniation. The predominant factors for successful operation are skilled surgical technique and indication. Posterior approach microendoscopic discectomy for lumbar disc herniation is worthy to be recommended clinically.

Abstract:Objective:To investigate the safety and efficacy of the treatment of lumbar disc herniation with the technique of avascular zone space decompression through the intervertebral foramen approach.Methods:A retrospective analysis of patients with lumbar disc herniation treated with percutaneous transforaminal endoscopic discectomy in our hospital between October 2015 and December 2015. The necessity and the formation of the intervertebral foramen were recorded.Results:A total of 75 patients were included in this study, aged 14 ~ 71 years old, average 42.0±14.1 years old. There were 41 patients with lumbar L4-5 operation, 35 patients without foraminoplasty from puncture judgment, among them 32 patients achieving satisfactory decompression, only 3 patients needing foraminoplasty. And there were 34 patients with L5-S1 operation, 16 patients puncture positioning in line with the requirements of no foraminoplasty, among them 15 cases achieving satisfactory decompression, only 1 patient (1/16, 6.3%) needing articular process plasty. At follow-up, 2 patients (2.7%) had acute disc herniation. No nerve injury occurred, and no operation related complications occurred.Conclusion:“Avascular zone” decompression technique through the intervertebral foramen approach under endoscope can achieve satisfactory decompression. At the same time, it can avoid the destruction of the joint process, reduce the bleeding and pain in the operation.

Abstract:With the trend of minimally invasive spine surgery, full-endoscopic lumbar discectomy (FELD) has evolved with the advancement of the optics and instruments. Regarding the techniques, the transforaminal and interlaminar approach remain the major accesses in FELD. Transforaminal endoscopic lumbar discectomy (TELD) is an effective and safe treatment for herniation of the lumbar disc. More and more evidence supports the TELD in enhancing recovery and decreasing surgical complications. However, the learning curve of TELD remains steep, especially at the L5-S1 level. The iliac crest height is an essential factor in the operability of TELD at the L5-S1 level. In the situation of the high iliac crest, TELD is technically challenging even for an experienced surgeon. Therefore, the authors report their techniques of TELD with foraminoplasty step-by-step and the preliminary results in this report.
