The study employed visual stimuli designed to isolate and reveal how spatial, temporal and chromatic aspects of visual processing change from normal sensitivity to extreme loss in glaucoma. The experiments carried out examined the fovea and each of four paracentral retinal regions, arranged diagonally away from fixation at an eccentricity of 6°. Spatial vision was investigated by measuring both absolute detection thresholds and functional contrast sensitivity using Landolt ring stimuli ( Ophthalmic Physiol Opt 2022;42:1363–1378). These psychophysical tests based on Landolt C optotypes measure both detection thresholds and gap orientation discrimination thresholds in the same experiments using randomly interleaved test locations. First‐order motion perception was examined by using moving stimuli embedded in static luminance contrast noise. Red/green (RG) and yellow/blue (YB) colour thresholds were measured with the Colour Assessment and Diagnosis (CAD) test, which makes use of random, dynamic luminance contrast noise (±45%) to isolate the use of RG and YB colour signals. Subjects were normal controls ( n = 65) and glaucoma patients with binocular visual field defects ( n = 15) classified based on their Humphrey Field Analyser mean deviation (MD) scores. The impairment of visual function varied depending on the stimulus attribute and location tested. The severity of loss increased monotonically with disease progression. For subjects with mild glaucoma (MD −0.01 to −6.00 dB), significantly more data points exceeded the upper normal threshold limits for the corresponding age for RG colour thresholds than for any other visual test ( Br Med Bull 2017;122:51–77), followed by motion thresholds. This was particularly the case for the parafoveal locations when compared with the equivalent thresholds measured at the fovea. The results suggest that a multifaceted measure of binocular visual performance based on foveal and parafoveal tests of RG colour vision and first‐order motion, can best inform on the progression of glaucoma and the overall loss of visual performance for comparison with other measures of changes in the quality of life in patients with glaucoma. Abstract:Purpose: To examine whether ocular hypertension and early glaucoma cause measurable changes in spatial vision, rapid flicker sensitivity and red/green and yellow/blue colour vision. Methods: We examined subjects aged 40 to 78 years with ocular hypertension (OHT), primary open angle glaucoma (POAG), normal tension glaucoma (NTG), and age matched controls. Monocular best corrected visual acuity (BCVA) was tested using EDTRS charts.VA and Functional Contrast Sensitivity (FCS) with Landolt ring stimuli of both positive and negative contrast were also measured monocularly with the Acuity‐ Plus test. Red/green (RG) and yellow/blue (YB) colour thresholds were measured with the Colour Assessment and Diagnostics (CAD) test. Photopic (cone‐enhanced) and mesopic (rod‐enhanced) rapid flicker thresholds were measured at the fovea (0°) and at an eccentricity of 5° in each of the four quadrants using the Flicker ‐Plus test. All subjects had their visual fields measured using the Humphrey visual field analyser (24–2 SITA standard protocol). Peripapillary retinal nerve fibre layer thickness (pRNFLT) and macular retinal layer thicknesses were measured using the Heidelberg Spectralis OCT. Results: The mean, logMAR BCVA for OHT patients was 0.00 and for POAG and NTG patients was −0.10 and 0.10, respectively. For control participants, mean logMAR BCVA was 0.00. Mean pRNFL thickness was significantly reduced for glaucoma patients (59.5 ± 7.7 μm) as compared to control subjects (94.6 ± 5.13 μm) and OHT patients (115.5 ± 3.5 μm). POAG and NTG patients showed significantly higher colour thresholds (mean red/green threshold 4.97, blue/yellow 5.28 CAD units) compared to control subjects (mean red/green threshold 2.07, blue/yellow 1.62 CAD units). One OHT patient showed acquired loss of blue/yellow colour vision (3.23 CAD units), with normal VA. The Acuity ‐Plus test which employs briefly presented stimuli, also revealed significant loss of VA (3.78 and 3.64 min of arc) and FCS (141.32% and 100%) in an NTG patient, with results outside normal limits for both positive and negative contrast, respectively. Central FCS was higher and outside the normal age limits for POAG patients (73.41% and 89.27%) as compared to age matched controls (61.27% and 33.63%). OHT patients, on the other hand, produced FCS thresholds within normal limits. Flicker modulation thresholds were higher for both rod and cone enhanced stimulus conditions in glaucoma and OHT patients, when compared to control subjects. Conclusion: The results reveal non‐uniform, losses that can affect selectively each of the visual attributes examined in this study. These preliminary findings suggest that in addition to VA and FCS tests, colour vision and rapid flicker sensitivity should also be examined in patients with optic nerve pathologies like glaucoma.

Abstract:BACKGROUND:Fundus changes associated with high myopia (HM) may mask those associated with primary open-angle glaucoma (POAG). This study aim to determine the characteristics of RNFL thickness changes in patients with both POAG and HM and compare these to changes in patients with only HM. The diagnostic capabilities of both OCT and GDxVCC in this subset of patients are also evaluated.METHODS:Twenty-two eyes with POAG and HM (spherical equivalent (SE) between -6.0 and -12.0 D) were evaluated, and 22 eyes with HM were used for comparison. Characteristic retinal nerve fiber layer (RNFL) thickness profiles in patients with POAG and HM were examined using optical coherence tomography (OCT) and scanning laser polarimetry with variable corneal compensation (GDxVCC), and the diagnostic capabilities of these imaging modalities were compared. RNFL parameters evaluated included superior average (Savg-GDx), inferior average (Iavg-GDx), temporal-superior-nasal- inferior-temporal (TSNIT) average, and nerve fiber indicator (NFI) on GDxVCC and superior average (Savg-OCT), inferior average (Iavg-OCT), nasal average (Navg-OCT), temporal average (Tavg-OCT), and average thickness (AvgThick-OCT) on OCT (fast RNFL scan). Visual field testing was performed and defects were evaluated using mean defect (MD) and pattern standard deviation (PSD).RESULTS:The RNFL parameters (P < 0.05) significantly different between groups included Savg-GDx, Iavg-GDx, TSNIT average, NFI, Savg-OCT, Iavg-OCT, Tavg-OCT, and AvgThick-OCT. Significant correlations existed between TSNIT average and AvgThick-OCT (r = 0.778), TSNIT average and MD (r = 0.749), AvgThick-OCT and MD (r = 0.647), TSNIT average and PSD (r = -0.756), and AvgThick-OCT and PSD (r = -0.784). The area under the receiver operating characteristic curve (AUROC) values of TSNIT average, Savg-GDx, Iavg-GDx, NFI, Savg-OCT, Iavg-OCT, Navg-OCT, Tavg-OCT, and AvgThick-OCT were 0.947, 0.962, 0.973, 0.994, 0.909, 0.917, 0.511, 0.906, and 0.913, respectively. The NFI AUROC was the highest value.CONCLUSIONS:RNFL thickness was significantly lower in all but the nasal quadrant in patients with POAG and HM, compared to patients with only HM. Measurements with OCT and GDxVCC were well-correlated, and both modalities detected RNFL thickness changes. However, GDxVCC was better than OCT in detecting POAG in HM patients.
Effects of Acute High Intraocular Pressure on Red-Green and Blue-Yellow Cortical Color Responses in Non-Human Primates

Abstract:Glaucoma is a leading cause of irreversible blindness worldwide, and intraocular pressure (IOP) is an established and modifiable risk factor for both chronic and acute glaucoma. The relationship between color vision deficits and chronic glaucoma has been described previously. However, the effects of acute glaucoma or acute primary angle closure, which has high prevalence in China, on color vision remains unclear. To address the above question, red-green or blue-yellow color responses in V1, V2, and V4 of seven rhesus macaques were monitored using intrinsic-signal optical imaging while monocular anterior chamber perfusions were performed to reversibly elevate IOP acutely over a clinically observed range of 30 to 90 mmHg. We found that the cortical population responses to both red-green and blue-yellow grating stimuli, systematically decreased as IOP increased from 30 to 90 mmHg. Although a similar decrement in magnitude was noted in V1, V2, and V4, blue-yellow responses were consistently more impaired than red-green responses at all levels of acute IOP elevation and in all monitored visual areas. This physiological study in non-human primates demonstrates that acute IOP elevations substantially depress the ability of the visual cortex to register color information. This effect is more severe for blue-yellow responses than for red-green responses, suggesting selective impairment of the koniocellular pathways compared with the parvocellular pathways. Together, we infer that blue-yellow color vision might be the most vulnerable visual function in acute glaucoma patients.

Abstract:PURPOSE. This study investigated a novel motion-on-color paradigm to functionally isolate the magnocellular pathway and evaluate its diagnostic value in preperimetric glaucoma patients.METHODS. Thirty patients with preperimetric primary open-angle glaucoma and 30 controls participated in this study. They were tested in both the foveal and peripheral locations. Contrast sensitivity was assessed for the direction discrimination of a moving luminance-modulated grating presented on top of a red/green isoluminant grating. The moving test grating was designed to target the magnocellular pathway, while the background red/green isoluminant grating was designed to saturate the parvocellular pathway. The luminance-modulated grating was presented at spatial frequency of 0.5 cyc/deg, moving horizontally at four temporal frequencies (3 Hz, 8 Hz, 15 Hz, 25 Hz). Participants were asked to indicate the direction of motion for the luminance grating. As a comparison condition, frequency-doubling stimuli were also presented in the periphery and participants were asked to detect the occurrence of the frequency-doubled pattern. Two-way repeated-measures analysis of variance was performed with temporal frequency modulations as within-subject factor and group as between-subject factor, while contrast sensitivity was the dependent variable. Receiver operating characteristic (ROC) analysis was used to characterize diagnostic performance of the new procedure in comparison with the frequency-doubling tests for preperimetric glaucoma.RESULTS. The contrast sensitivity function in both the fovea and the periphery showed an inverted "V" shape with highest sensitivity in the intermediate temporal frequencies, consistent with physiological properties of the magnocellular pathway. At the fovea, compared to the control group, the sensitivity for the glaucoma patients was slightly but not significantly reduced (P > 0.05), and there was no significant interaction between groups and temporal frequency (P > 0.05). In the periphery, patients' sensitivity was significantly lower (P < 0.001) than that of normal participants, especially in high temporal frequencies, as supported by a statistically significant interaction between groups and temporal frequency (P < 0.001). The areas under ROC curves (AUROC) obtained for the motion-on-color paradigm in the periphery were 0.957 (25 Hz), 0.870 (15 Hz), 0.758 (8 Hz), and 0.561 (3 Hz) and were 0.761 for the traditional frequency-doubling test.CONCLUSIONS. The motion-on-color paradigm revealed a loss of contrast sensitivity in the peripheral visual field in preperimetric glaucoma. When applied with stimuli at high temporal frequency, the new paradigm had higher diagnostic sensitivity and specificity than the traditional frequency-doubling test. The findings also support the viewpoint that selective evaluation of magnocellular pathway function could facilitate the earlier detection of functional defects in glaucoma before visual field defects by standard perimetry.
Selective Reduction of Fmri Responses to Transient Achromatic Stimuli in the Magnocellular Layers of the Lgn and the Superficial Layer of the Sc of Early Glaucoma Patients

Abstract:Glaucoma is now viewed not just a disease of the eye but also a disease of the brain. The prognosis of glaucoma critically depends on how early the disease can be detected. However, early glaucomatous loss of the laminar functions in the human lateral geniculate nucleus (LGN) and superior colliculus (SC) remains difficult to detect and poorly understood. Using functional MRI, we measured neural signals from different layers of the LGN and SC, as well as from the early visual cortices (V1, V2 and MT), in patients with early-stage glaucoma and normal controls. Compared to normal controls, early glaucoma patients showed more reduction of response to transient achromatic stimuli than to sustained chromatic stimuli in the magnocellular layers of the LGN, as well as in the superficial layer of the SC. Magnocellular responses in the LGN were also significantly correlated with the degree of behavioral deficits to the glaucomatous eye. Finally, early glaucoma patients showed no reduction of fMRI response in the early visual cortex. These findings demonstrate that 'large cells' in the human LGN and SC suffer selective loss of response to transient achromatic stimuli at the early stage of glaucoma. Slowed Saccadic Reaction Times in Seemingly Normal Parts of Glaucomatous Visual Fields

Abstract:Purpose: In eye movement perimetry, peripheral stimuli are confirmed by goal-directed eye movements toward the stimulus. The saccadic reaction time (SRT) is regarded as an index of visual field responsiveness, whereas in standard automated perimetry (SAP), the visual field sensitivity is tested. We investigated the relation between visual field sensitivity and responsiveness in corresponding locations of the visual field in healthy controls and in patients with mild, moderate and advanced glaucoma. Materials and Methods: Thirty-four healthy control subjects and 42 glaucoma patients underwent a 54-point protocol in eye movement perimetry (EMP) and a 24-2 SITA standard protocol in a Humphrey Field Analyzer. The visual field points were stratified by total deviation sensitivity loss in SAP into 6 strata. A generalized linear mixed model was applied to determine the influence of the various factors. Results: The generalized linear mixed model showed that the mean SRT increased with increasing glaucoma severity, from 479 ms in the control eyes to 678 ms in the eyes of patients with advanced glaucoma ( p < 0.001). Mean SRTs significantly increased with increasing SAP sensitivity loss. Even at the locations where no sensitivity loss was detected by SAP (total deviation values greater or equal than 0 dB), we found lengthened SRTs in mild, moderate and advanced glaucoma compared to healthy controls ( p < 0.05) and in moderate and advanced glaucoma compared to mild glaucoma ( p < 0.05). At locations with total deviation values between 0 and −3 dB, −3 and −6 dB and −6 and −12 dB, we found similar differences. Conclusions: The lengthened SRT in areas with normal retinal sensitivities in glaucomatous eyes, i.e., planning and execution of saccades to specific locations, precede altered sensory perception as assessed with SAP. Better understanding of altered sensory processing in glaucoma might allow earlier diagnosis of emerging glaucoma.

Abstract:Objectives: This study aimed to study the inner retina functional and morphological impairment of retinal ganglion cells (RGCs) from specific macular rings and sectors to identify whether selective macular regions were more vulnerable than others within the 20 central degrees in patients with open-angle glaucoma (OAG). Methods: In total, 21 OAG patients [mean age 50.19 ± 7.86 years, Humphrey Field Analyzer (HFA) 24-2 mean deviation (MD) between −5.02 and −22.38 dB, HFA 10-2 MD between −3.07 and −17.38 dB], providing 21 eyes, were enrolled in this retrospective case–control study. And 20 age-similar healthy subjects, providing 20 eyes, served as controls. The multifocal photopic negative response (mfPhNR) response amplitude density (RAD) from concentric rings and macular sectors and ganglion cell layer thickness (GCL-T) assessed by Spectral Domain–Optical Coherence Tomography (SD-OCT) was measured. Mean RAD and GCL-T values were compared between OAG and control ones by ANOVA. In OAG eyes, the relationship between mfPhNR and SD-OCT data was examined by linear regression analysis, and Pearson's correlation coefficients were computed. Results: In considering all rings and sectors, compared to the controls, the OAG group showed a significant (p < 0.01) reduction in mean mfPhNR RAD and in GCL-T values with the greatest reduction in the central area. In OAG eyes, a significant (p < 0.01) correlation between all mfPhNR RAD and GCL-T values, with significant (p < 0.01) correlation coefficients, were found. Conclusions: In OAG eyes, RGC dysfunction was detectable by abnormal mfPhNR responses in localized macular areas, mainly in the central one. Localized macular RGC dysfunction was linearly correlated with the GCL morphological changes.

The Pattern of Retinal Nerve Fiber Layer and Macular Ganglion Cell-Inner Plexiform Layer Thickness Changes in Glaucoma

Abstract:Background/Aims. To investigate the patterns of retinal ganglion cell damage at different stages of glaucoma, using the circumpapillary retinal nerve fiber layer (RNFL) and macula ganglion cell-inner plexiform layer (GCIPL) thicknesses. Methods . In 296 eyes of 296 glaucoma patients and 55 eyes of 55 healthy controls, the correlations of mean deviation (MD) with the superior and inferior quadrant RNFL/GCIPL thickness (defined as the average of three superior and inferior sectors, resp.) were analyzed. Results . In early to moderate glaucoma, most of the RNFL/GCIPL thicknesses had significant positive correlations with the MD. In advanced glaucoma, the superior GCIPL thickness showed the highest correlation with MD ( r = 0.495 ), followed by the superior RNFL ( r = 0.452 ) (all; P < 0.05 ). The correlation coefficient of the inferior RNFL thickness with MD ( r < 0.471 ) was significantly stronger in early to moderate glaucoma compared to that in advanced glaucoma ( r = 0.192 ; P < 0.001 ). In contrast, the correlations of the superior GCIPL thickness with MD ( r = 0.452 ) in advanced glaucoma was significantly stronger compared to that in early to moderate glaucoma ( r = 0.159 ; P < 0.001 ). Conclusions . The most preserved region in advanced glaucoma appears to be the superior macular GCIPL, whereas the most vulnerable region for initial glaucoma is the inferior RNFL around the optic disc.

Retinotopic Changes In The Gray Matter Volume And Cerebral Blood Flow In The Primary Visual Cortex Of Patients With Primary Open-Angle Glaucoma

Abstract:PURPOSE. To assess the cortical structure and cerebral blood flow changes in the brain of patients with primary open-angle glaucoma (POAG).METHODS. High-resolution anatomical magnetic resonance imaging (MRI) and arterial spin labeling (ASL)-MRI were performed in 23 POAG patients and 29 controls. Patients were further divided into early-moderate and advanced groups based on mean deviation (MD) cutoff of 12 dB. A baseline scan was obtained and repeated during visual stimulation to the central preserved visual field in the more affected eye of POAG patients and a randomly selected eye of controls. Gray matter volume (GMV) and cerebral blood flow (CBF) throughout the whole brain were compared between patients and controls.RESULTS. Compared to controls, a region with significant reduction of GMV was detected in the anterior calcarine fissure of advanced POAG patients (P < 0.001, voxels = 503, 1698 mm3). Patients with early-moderate POAG had resting CBF similar to that of controls. However, a region with marked CBF decrease was detected in the anterior calcarine fissure of advanced POAG patients (P < 0.001, voxels = 1687, 13,496 mm(3)). The region with CBF reduction in advanced POAG showed good colocalization with the region with GMV decrease in this group. Following visual stimulation, patients with advanced POAG showed significantly lower increase in CBF in the occipital lobes (P < 0.001, voxels = 112, 896 mm(3)) as compared to controls (P < 0.001, voxels = 1880, 15,040 mm(3)) and early-moderate POAG (P < 0.001, voxels = 2233, 17,864 mm(3)).CONCLUSIONS. Primary open-angle glaucoma patients demonstrate a disease severity-dependent retinotopic pattern of cortical atrophy and CBF abnormalities in the visual cortex. Cerebral blood flow may be a potential biomarker for the brain involvement in glaucoma.
Evaluation of the Glaucomatous Macular Damage by Chromatic Pupillometry

Abstract:This study aimed to examine the performance of binocular chromatic pupillometry for the objective and rapid detection of primary open-angle glaucoma (POAG), and to explore the association between pupillary light response (PLR) features and structural glaucomatous macular damage. Forty-six patients (mean age 41.00 ± 13.03 years) with POAG and 23 healthy controls (mean age 42.00 ± 11.08 years) were enrolled. All participants underwent sequenced PLR tests of full-field, superior/inferior quadrant-field chromatic stimuli using a binocular head-mounted pupillometer. The constricting amplitude, velocity, and time to max constriction/dilation, and the post-illumination pupil response (PIPR) were analyzed. The inner retina thickness and volume measurements were determined by spectral domain optical coherence tomography. In the full-field stimulus experiment, time to pupil dilation was inversely correlated with perifoveal thickness (r = − 0.429, P < 0.001) and perifoveal volume (r = − 0.364, P < 0.001). Dilation time (AUC 0.833) showed good diagnostic performance, followed by the constriction amplitude (AUC 0.681) and PIPR (AUC 0.620). In the superior quadrant-field stimulus experiment, time of pupil dilation negatively correlated with inferior perifoveal thickness (r = − 0.451, P < 0.001) and inferior perifoveal volume (r = − 0.417, P < 0.001). The dilation time in response to the superior quadrant-field stimulus showed the best diagnostic performance (AUC 0.909). In the inferior quadrant-field stimulus experiment, time to pupil dilation (P < 0.001) correlated well with superior perifoveal thickness (r = − 0.299, P < 0.001) and superior perifoveal volume (r = − 0.304, P < 0.001). The use of chromatic pupillometry offers a patient-friendly and objective approach to detect POAG, while the impairment of PLR features may serve as a potential indicator of structural macular damage.
Optic Nerve Head Hemoglobin Levels in Glaucoma: A Structural and Functional Correlation Study

Abstract:Purpose: To investigate structural and functional correlations in glaucoma patients using optic nerve head hemoglobin (ONH Hb) measurements as determined by automated colorimetric analysis of conventional retinography. Methods: We prospectively enrolled healthy participants and glaucomatous patients with a wide range of disease stages. All participants underwent visual field (VF) testing (standard automated perimetry, SAP), color fundus imaging (mydriatic retinography), and peripapillary retinal nerve fiber layer (pRNFL) assessment through spectral-domain optical coherence tomography (SD-OCT). Software Laguna ONhE was used to estimate the amount of ONH Hb and to determine the glaucoma discriminant function (GDF) index. Scatter plots were constructed, and regression analysis was used to investigate the correlations between GDF, average pRNFL thickness, and VF mean deviation (VFMD) index values. A secondary analysis was performed to compare each parameter between three different glaucoma groups divided according to VFMD values (mild, >-6 dB; moderate, -6 to -12 dB; and advanced, <-12 dB). Results: One hundred ninety-six eyes from 123 participants (69 with glaucoma and 54 controls) were enrolled. Overall, all parameters evaluated differed significantly between glaucomatous and control eyes (p ≤ 0.001). The comparison of each parameter according to groups of disease stages revealed significant differences between controls and each of the glaucomatous groups (p < 0.001). More pronounced changes in GDF values were observed in early disease stages. We found significant nonlinear correlations between GDF and VFMD values (R 2 = 0.295, p < 0.001) and between pRNFL thickness and VFMD (R 2 = 0.598, p < 0.001). A linear correlation was found between GDF and pRNFL thickness values (R 2 = 0.195, p < 0.001). Conclusion: Our results showed significant associations between ONH Hb values and both structural and functional damage in glaucoma obtained by SD-OCT and SAP, respectively. The nonlinear correlation we found and the GDF behavior along different disease stages suggest that ONH Hb levels' reduction may precede visual function changes in early glaucoma stages.

Abstract:Over 60 million people worldwide are diagnosed with glaucomatous optic neuropathy, which is estimated to be responsible for 8.4 million cases of irreversible blindness globally. Glaucoma is associated with characteristic damage to the optic nerve and patterns of visual field loss which principally involves the loss of retinal ganglion cells (RGCs). At present, intraocular pressure (IOP) presents the only modifiable risk factor for glaucoma, although RGC and vision loss can continue in patients despite well-controlled IOP. This, coupled with the present inability to diagnose glaucoma until relatively late in the disease process, has led to intense investigations towards the development of novel techniques for the early diagnosis of disease. This review outlines our current understanding of the potential mechanisms underlying RGC and axonal loss in glaucoma. Similarities between glaucoma and other neurodegenerative diseases of the central nervous system are drawn before an overview of recent developments in techniques for monitoring RGC health is provided, including recent progress towards the development of RGC specific contrast agents. The review concludes by discussing techniques to assess glaucomatous changes in the brain using MRI and the clinical relevance of glaucomatous-associated changes in the visual centres of the brain.

Chromatic vision and structural assessment in primary congenital glaucoma

Abstract:Primary congenital glaucoma is a rare disease that occurs in early birth and can lead to low vision. Evaluating affected children is challenging and there is a lack of studies regarding color vision in pediatric glaucoma patients. This cross-sectional study included 21 eyes of 13 children with primary congenital glaucoma who were assessed using the Farnsworth D-15 test to evaluate color vision discrimination and by spectral domain optical coherence tomography to measure retinal fiber layer thickness. Age, visual acuity, cup-to-disc ratio and spherical equivalent data were also collected. Global and sectional circumpapillary and macular retinal fiber layer thicknesses were measured and compared based on color vision test performance. Four eyes (19%) failed the color vision test with diffuse dyschromatopsia patterns. Only age showed statistical significance in color vision test performance. Global and sectional circumpapillary and macular retinal fiber layer thicknesses were similar between the color test outcomes dyschromatopsia and normal. While the color vision test could play a role in assessing children with primary congenital glaucoma, further studies are needed to correlate it with damage to retinal fiber layer thickness.

Abstract:PURPOSE: To evaluate and compare the diagnostic specificities of peripapillary retinal nerve fiber layer (RNFL) thickness, macular ganglion cell-inner plexiform layer (GC-IPL) thickness, and optic nerve head (ONH) measurements in nonglaucomatous myopic individuals.METHODS: In a prospective, cross-sectional study, participants underwent a complete ophthalmic examination, a screening automated visual field test, and axial length measurement. The study eye then underwent optic nerve head and macular scanning using spectral-domain optical coherence tomography (OCT) instrumentation to determine RNFL thickness, GC-IPL thickness, and ONH measurements. False-positive rates for each of the OCT-derived parameters, using predefined criteria for an abnormal test, were calculated. Comparative analysis was performed using the McNemar test.RESULTS: Data from 43 eligible subjects were analyzed. The mean age was 30±6.8 years (range, 22 to 50 y) with an average axial length of 25.26±1.21 mm (range, 23.06 to 29.07 mm) and mean spherical equivalent of -4.50±1.93 D (range, -1.00 to -9.00 D). The false-positive rate was higher when using RNFL parameters compared with both ONH (47% vs. 7%, respectively; P<0.001) and GC-IPL (47% vs. 26%, respectively; P=0.049) parameters. The false-positive rate was higher when using GC-IPL parameters, compared with ONH parameters (26% vs. 7%, respectively; P=0.039).CONCLUSIONS: Caution should be exercised when relying on OCT-derived RNFL and GC-IPL thickness values to diagnose glaucoma in myopic individuals. OCT-derived ONH parameters perform better than RNFL and GC-IPL parameters and may increase diagnostic specificity in this population.

Higher contrast thresholds for vanishing optotype recognition in macular visual fields among glaucoma patients: a structure-function analysis

Abstract:Background/aims: We aimed to explore the impact of glaucomatous macular damage, specifically retinal ganglion cell (RGC) loss, on macular pattern vision measured by the vanishing optotype (VO) recognition contrast threshold. Methods: Seventy-two patients (mean age, 33.51±7.05 years) with primary open-angle glaucoma and 36 healthy controls (mean age, 30.25±6.70 years) were enrolled. VO recognition contrast thresholds of each participant were measured at the 16 preset test locations covering the central 5° visual field (VF). Macular sensitivity (MS) was tested by macular threshold test of Humphrey Field Analyzer. Macular RGC plus inner plexiform layer (GCIPL) thickness was also measured by spectral domain optical coherence tomography. Results: The VO contrast threshold demonstrated weak-to-moderate correlations (rho=-0.275 to -0.653) with MS (p<0.001). There was a significantly higher VO contrast threshold in glaucoma group (p<0.0001). At similar levels of MS, patients with glaucoma with GCIPL damage showed remarkably higher VO contrast thresholds than those with preserved GCIPL (p=0.0079). The structure-function relationships between VO contrast threshold and GCIPL thickness (rho=-0.725 to -0.802) were remarkably stronger than those between MS and GCIPL thickness (rho=0.210 to 0.448). VO contrast threshold showed stronger correlation with average GCIPL thickness (rho=-0.362 to -0.778) than MS (rho=0.238 to 0.398) at multiple test locations in glaucoma group. Conclusions: Glaucomatous eyes have higher contrast thresholds for VO recognition in fovea-around VF. Stronger structure-function relationships indicate that VO contrast threshold is more vulnerable to RGC damage.

Abstract:Purpose: It is well known that glaucoma results in performance impairments on tasks processed early in the visual pathways. Glaucoma should also impair cortical visual processing because of reduced input from retinal ganglion cells and also possibly because of abnormal cortical function. This study was undertaken to assess whether cortically processed global percepts are disrupted in glaucoma in areas of visual field classified as normal by standard automated perimetry (SAP). Performance on global tasks (motion and form) was compared to measures of presumed precortical magnocellular and parvocellular function in the same individuals. Methods: Fifteen control subjects and 12 patients with primary open-angle glaucoma participated. Testing was performed foveally and midperipherally (12.5 degrees). Contrast-discrimination thresholds were measured by using the steady-pedestal (magnocellular) and pulsed-pedestal (parvocellular) contrast-discrimination tasks of Pokorny and Smith. Global motion coherence and global form coherence thresholds were measured at high and low contrast. Results: Patients with glaucoma demonstrated higher global motion and form-coherence thresholds than did control subjects for targets presented in the midperiphery (P < 0.05), but not foveally. Different individuals performed poorly on the motion and form tasks. The subjects with the greatest presumed magnocellular and parvocellular loss were those with the largest deficits on the global motion and form tasks, respectively. Conclusions: Some subjects with glaucoma demonstrate profound impairments of global motion or global form integration in areas of visual field classified as normal by SAP. This finding implies that some people with glaucoma may have far greater difficulty with complex visual tasks (for example, navigation through the environment or face recognition) than is predicted by their visual field loss.

Retinal Ganglion Cell Functional Recovery after Intraocular Pressure Lowering Treatment Using Prostaglandin Analogs in Glaucoma Suspects: A Prospective Pilot Study

Abstract:Aim and background: To evaluate the ability of pattern electroretinogram (PERG) to detect improvement of retinal ganglion cell (RGC) function in glaucoma suspects (GS) after medically reducing intraocular pressure (IOP) using prostaglandin analog drops. Materials and methods: Six subjects (eight eyes) received topical IOP lowering treatment based on their clinical examination and were observed at Manhattan Eye, Ear & Throat Hospital over an average of 3.1 ± 2.2 months. During this time, participants underwent a full ophthalmologic exam and were evaluated with a Humphrey visual field analyzer (HFA) 24-2 [24-2 mean deviation (MD), 24-2 pattern standard deviation (PSD), and 24-2 visual field indices (VFI)], Diopsys NOVA PERG optimized for glaucoma [magnitude (Mag), magnitudeD (MagD), and magnitudeD/magnitude ratio (MagD/Mag ratio)] and optical coherence tomography (OCT)-derived average retinal nerve fiber layer thickness (avRNFLT) and average ganglion cell layer + inner plexiform layer (avGCL + IPL) thicknesses at baseline visit (pretreatment) and 3 months later (posttreatment). Goldman applanation tonometry was used to measure IOP at each visit. Paired sample t-tests were conducted to determine the statistical significance of the change in IOP, HFA indices, PERG parameters, and OCT thickness measurements between the two visits. Results: Lowering IOP by 22.29% resulted in a significant increase (32.98 and 15.49%) in MagD [t (7) = -3.174, 95% confidence interval (CI) = -0.53, -0.08, p = 0.016] and MagD/Mag ratio [t (7) = -3.233, 95% CI = -0.20, -0.03, p = 0.014], respectively. There was a positive percentage change for all variables of interest, however, 24-2 MD, Mag, avRNFLT, and GCL+ IPLT did not reach statistical significance. Conclusion: After reducing IOP by 22.29% for a duration of 3.1 months, the PERG parameters, MagD and MagD/Mag ratio, significantly improved by 32.98 and 15.49%, respectively. Clinical significance: Pattern electroretinogram (PERG) may be a crucial tool for clinicians to locate a window of opportunity in which degenerating yet viable RGCs could be rescued from irreversible damage. We suggest consideration of PERG as a tool in early retinal ganglion cell (RGC) dysfunction detection as well as for monitoring IOP lowering treatment.
