Abstract:Introduction & importance: Branchial fistulas and cysts are uncommon anomalies of embryonic development that involve soft tissues of the neck. According to Bailey-Proctor classification, second BCCs are classified into four types: Type-I cysts are situated along the anterior border of the sternocleidomastoid muscle beneath the superficial cervical fascia. Type-II ones are the most common and lie just laterally to great vessels beneath enveloping fascia of the neck. Type-III ones pass between internal and external carotid arteries. Type-IV cysts are situated in the pharyngeal mucosal space just deep to the palatine tonsil and medial to great neck vessels, often extending upward towards the skull base. Most second BCCs comprise the first three types, while type-IV cysts are extremely rare. Case presentation: 17 years old male patient from Baghdad/Iraq, single, a student, and living with his family. Clinical discussion: The patient presented to Al-kindy Teaching Hospital for general surgery consultation due to the presence of a lump in the upper third of the anterior border of the sternocleidomastoid muscle for several years ago, the lump was painless but gradually increase in size with discomfort but without fever, anorexia or weight loss. There were no relieving factors. Regarding the review of systems nothing positive and the history was negative also patient had no past drug history and no psychological illness. Physical examination of the lump showed a smooth, non-tender, fluctuant cyst located at the upper third of the anterior border of the left sternocleidomastoid muscle about 7 × 4 cm and there were no enlarged lymph nodes. Regarding examination of the other systems there was nothing positive. Laboratory and radiological investigation showed that the cystic lesion mostly was a branchial cyst, so the operation is done for the patient by complete excision of the cyst with its tract which was located between external and internal carotid vessels. A histopathological study revealed a squamous epithelium-lined cyst with lymphoid infiltration, consistent with a branchial cleft cyst. The patient was discharged without any complication or any evidence of recurrence for 14 months follow-up. Conclusion: Branchial anomalies remain asymptomatic and can present later in life. They can be misdiagnosed. CT scans and MRI neck are helpful in the diagnosis of the cyst and its anatomical extensions. A proper history and physical examination are required to look for other anomalies like craniofacial syndromes. The treatment of branchial cyst is complete surgical excision to prevent recurrence and removal of these lesions at an earlier stage will increase the quality of life of the patient. In addition, since they are rarely malignant, more successful results will be obtained with early diagnosis and treatment.

Abstract:Branchial cleft cysts are considered one of the most common cystic lesions in neck and are commonly seen in pediatric age group and young adulthood with most of the cases presenting within second and third decade. Here we intend to discuss a case of a 43 year old male who presented with a very short history of a painful lateral neck mass and was clinico-radiologically suggested as either myositis and abscess of sternocleidomastoid muscle or necrotic/cystic lymphadenopathy. An ultrasound guided needle aspiration cytology did not show any microorganism thus an excisional biopsy of the mass was done suspecting it to be lymph nodal mass. However the histopatholgical examination confirmed it to be an inflamed branchial cyst. Thus we would like to highlight the importance of keeping branchial cleft cysts as a possible differential while managing lateral neck masses of any duration in adults as well as in children.

Surgical excision is a definitive treatment for branchial cleft cysts. This article presents the clinical report where the big inflamed cysts were excised under local anesthesia. It also presents guidelines for minimally invasive surgery under local anesthesia, which can be used in adequately selected cases.

Abstract:Branchial cleft anomalies are the second most common head and neck congenital lesions in children. It may sometimes be a part of branchio-oto-renal (BOR) syndrome, so in patients with branchial cleft anomalies associated with a complaint of auricular deformity or a similar history and findings in other family members, we should take an additional examination to find the possibility of BOR syndrome. Complete excision is essential for good prognosis. For the management of branchial cleft anomalies, various methods have been reported. Endoscopically assisted dissection technique and transoral robot-assisted surgery were used in the management of fistula and allowed excellent visualization of the pharyngeal component of the lesion and a minimally invasive approach. It is essential for the surgeon to fully comprehend the congenital lesions to attain the correct preoperative diagnosis and plan for an appropriate surgical approach to prevent the most common complication and recurrence in these lesions. The following sections discuss the anatomy, common presentation, auxiliary examination, differential diagnosis, the current principles of surgical treatment and prognosis for second branchial cleft anomalies in children, and discussed the branchio-oto-renal syndrome.

Abstract:Purpose: To explore the applied value of the selective neck dissection to treat second, third, and fourth branchial cleft anomalies with recurrent or repeated neck infections. Method: We made a retrospective study about 29 patients with the second, third, and fourth branchial cleft anomalies with recurrent or repeated neck infections who were treated by surgery from 2002 to 2018 in Sichuan Province Cancer Hospital. According to the characteristics of branchial cleft anomaly on embryology and anatomy, different types of selective neck dissection were chosen to remove pathological scar tissue or inflammatory tissue en bloc. Result: Of 29 cases, 28 had primary healing, and 1 had local infection, healing after dressing change for a long time. In 1 case, branchial cleft anomalies adhered to the internal jugular vein, which was ruptured and sutured. During the follow-up time of 12 similar to 195 months with an average of 91.76 months, there were no recurrent cases. Conclusion: The selective neck dissection technique is safe and effective in the treatment of branchial cleft anomalies with recurrent or repeated neck infection.

Abstract:OBJECTIVE To explore the clinical and microbiological features of deep neck multispace abscesses and review the experience in its diagnosis and treatment.METHODS The clinical data of 9 cases with deep neck multispace abscesses diagnosed and treated in our hospital from Jan.2006 to Jun.2010 were retrospectively reviewed.RESULTS All cases were infected more than 3 deep neck spaces.Six of the 9 cases had positive result of bacterial culture of the pus.Three cases were found the tubercle bacillus,and other cases were found candida albicans.Four cases were infected with multiple bacteria.They were treated by drainage via incising lateral neck and application of antibiotics.All the cases were cured.CONCLUSIONOnce deep neck multispace abscesses are diagnosed,surgical drainage via incising lateral neck,bacterial culture of the pus should been done.Washing the infected spaces after the surgical drainage can increase the cure rate and shorted the days of hospitalization.

Abstract:Branchial cleft anomalies are a common cause of congenital neck masses and can present as a cyst, sinus, or fistula. A comprehensive understanding of the embryologic basis of these anomalies aids in diagnosis and surgical excision. Fistulas tend to present at an earlier age than sinuses or cysts, with most lesions presenting as either a neck mass, draining sinus, or recurrent infections. The eventual management of each is complete surgical excision, which is curative. A history of recurrent preoperative infections leads to a higher rate of recurrence.

Abstract:Background. Second branchial cleft cysts (also called cervical lymphoepithelial cysts) usually present in adulthood, and cosmesis of surgical access is an important issue. This study was performed to assess the feasibility and outcome of a retroauricular approach for removing second branchial cleft cysts Methods. Eight patients underwent removal of second branchial cleft cysts using a retroauricular approach. Results. All 8 second branchial cleft cysts were removed successfully, and the wounds healed uneventfully. None of the patients suffered from weakness of the great auricular nerve. Marginal nerve palsy did not occur postsurgery. Average follow-up period was 14.5 months (range, 6-20 months) with no tumor recurrence. The incision scars were invisible. Conclusion. Abstract:Cervical lymphadenopathy, a condition with multifactorial etiology is one of the common presentations in patients reporting to dental clinic. It is thus imperative that oral physicians have to be well versed with the causes of cervical lymphadenopathy. The present article describes a case of 24-year-old male who reported to his dentist with complaint of painless swelling of 4 months duration on right side of neck. Oral examination revealed missing mandibular third molars on both sides, which was confirmed on radiograph. Considering reactive lymphadenitis secondary to inflamed impacted molar as a reason for neck swelling, the surgical removal of third molar of right side was performed under antibiotic coverage which was uneventful. As the neck swelling persisted even after removal of molar, patient had undergone fine needle aspiration cytology and then the swelling was diagnosed as tubercular lymphadenitis. He was treated by antitubercular therapy for 6 months regimen, after then he was perfectly alright. The present report aims to highlight contemplative role of oral physician in early diagnosis of neck swellings that can result in on-time management.

Abstract:While branchial cleft cysts are often considered benign pathologies, the literature discusses cases of squamous cell carcinoma (SCC) arising from these cystic lesions as either a primary or metastatic tumor. We illustrate our institutional experience and review the current literature to identify recommendations for best diagnostic, surveillance, and treatment guidelines for SCC identified in a branchial cleft cyst. A 61-year-old male presented with a right sided neck mass, with suspicion of a branchial cleft cyst due to benign findings on fine needle aspiration. Following surgical excision, a focus of SCC was found on surgical pathology. Despite PET/CT and flexible laryngoscopy, no primary tumor was identified prompting routine surveillance every 3 months with cervical ultrasonography and flexible nasolaryngoscopy. Two and a half years following his initial presentation, pathologic right level II lymphadenopathy was detected on ultrasound without evidence of primary tumor. Subsequent transoral robotic surgery with right tonsillectomy and partial pharyngectomy, with right lateral neck dissection revealed a diagnosis of pT1N1 HPV-HNSCC and he was referred for adjuvant chemotherapy and radiation. To our knowledge there are less than 10 cases of confirmed HPV-associated oropharyngeal SCC arising from a branchial cleft cyst. Here we demonstrate the utility of ultrasound as a surveillance tool and emphasize a higher index of suspicion for carcinoma in adult patients with cystic neck masses.

Abstract:Background: Deep neck space infections (DNSI) are significantly less common today than in the past. But acute severe neck abscesses are still associated with significant morbidity and mortality rates due to lack of seasonable diagnosis and treatment. Case Report: we present a rare patient with severe deep neck space infection, the abscesses spread a covert way led to axilla and lateral chest abscesses through the pre-vertebral space. Following multiple surgical procedure and antimicrobial therapy, the patient made a full recovery. Conclusion: Severe deep neck space infections usually spread to the mediastinum, anterior chest wall easily. However, because of the most insidious pre-vertebral space, the axilla and left lateral chest wall also should to be paid attention in clinical. Using corticosteroids on patient with deep neck space infections should be cautious, or else it will make the infection spread and deteriorate. Multidisciplinary of diagnosis and therapy in severe deep neck space infections may recognize again by Clinicians.
Clinical Analysis of Branchial Cleft Cyst(fistula):Report of 284 Cases

Abstract:PURPOSE:To investigate the clinical manifestation of branchial cleft cyst (fistula) and provide some clinical experience on its diagnosis and treatment.METHODS:From June 1993 to December 2006, two hundred and eighty-four patients with branchial cleft cyst or fistula underwent surgical treatment were retrospectively reviewed, the preoperative examinations, preoperative diagnosis and postoperative pathological diagnosis were recorded, and the data was analyzed with SAS6.12 software package.RESULTS:Among the 284 patients with branchial cleft cyst or fistula confirmed by postoperative pathological diagnosis, there were 132 patients (46.5%) with first branchial cleft cyst or fistula, one hundred and forty-five patients (51.1%) with second branchial cleft cyst or fistula, and 7 patients (2.4%) with third branchial cleft cyst or fistula. First branchial cleft cyst or fistula often occurred in patients older than 40 years, second and third branchial cleft cyst or fistula often occurred in patients younger than 40 years. The pathological diagnosis confirmed 188 primary lesions, thirty-three recurrent lesions, forty-four lesions with internal fistula, fourteen lesions with secondary infection and external fistula, and three lesions with both internal and external fistulae. One patient was found with tuberculosis simultaneously, two patients with multiple cysts, one patient with venous malformation. Six lesions were found malignant, five from first branchial cleft and one from second branchial cleft. The highest accuracy of preoperative diagnosis was 66.20% using ultrasonography. During the follow-up period, 5.98% of patients had recurrence.CONCLUSIONS:First and second branchial cleft cysts (fistula) are the most common lesions. Preoperative ultrasonography is the first choice for diagnosis of branchial cleft cyst (fistula). Complete resection including the cyst and fistula is the key to successful surgical treatment. Abstract:Branchial cleft carcinoma is a rare malignancy, there still exist some controversies regarding to the differentiation of branchial cleft carcinomas. This article is aimed to familiarize clinicians with the presentation and treatment of the tumour and explores its origin. A 41-year old man who was seen with a large lesion of branchial cleft carcinoma with a slowly growing mass in the area of the left parotid gland was reported. The histological finding of dysplastic epithelium was next to direct invasive carcinoma. The diagnosis of a branchial cleft carcinoma requires the fulfillment of strict criteria. This case supports the origin of the carcinoma as being from an epithelial-lined cyst.

Abstract:Epidermoid cysts (ECs) are usually small, benign, keratin-filled cysts, can be congenital or acquired and encountered anywhere in the body. EC and dermoid cyst constitute approximately 7% of all cysts in the head and neck region and tend to occur in areas of embryonic fusion. Neck masses are commonly present in children, and there is often a diagnostic dilemma clinically with common differential diagnoses of this region such as thyroglossal cyst, pre-tracheal lymph nodes, thyroid mass, EC and dermoid cyst. EC is mostly present in middle-aged males, and here, we are going to present an EC of the midline neck in an 8-year-old girl, which is not common.

Abstract:Objective: The aim of this study was to approach the clinical and pathology of the branchial cleft cyst and fistula.Methods: The clinical feature,histopathology,diagnosis,treatment and prognosis of 199 patients(168 branchial cleft cysts and 31 branchial cleft fistulae) in our department between Jan 2007 and Aug 2011 were reviewed.SPSS 15.0 software package was used for χ 2 test.Results: There were 80 first branchial cleft cysts,106 second branchial cleft cysts, 3 third branchial cleft cysts and 10 fourth branchial cleft cysts.There were 100 patients with infection,11 patients with slightly paraplasm,all patients with histopathology exam(1 case with pleomorphic adenoma;1 case with adenolymphoma; 1 case with papillary cystadenoma;1 case with tuberculosis),88 patients with immunohistochemisty(69.8% were CKp+; 14% were CKp+ and CK8+)and 31 patients with fistulas(13 cases had exterior and interior orificium fistulae;18 cases had exterior orificium fistula only).Complete surgical extirpation was the only treatment of choice and it is much important to eliminate the interior orificium fistula especially.During the follow-up period,19.6%(39/199) of patients had withdrawed and 5.6%(9/160) of patients had recurrence.Conclusion: First and second branchial cleft cysts and fistula are the most common lesions.It affected much more to the patients due to its diversity and polytropy.Diagnosis needs histopathology.It′s consequential to excise the cyst and orificium fistula completely.Important tissues must be protected in surgical procedures.

Abstract:OBJECTIVE:To evaluate the utilization of functional neck dissection for treatment of recurrent branchial cleft anomalies.METHOD:Fifteen patients with recurrent branchial cleft anomalies (fistula and cyst) undergoing functional neck dissection were retrospectively analyzed.RESULT:Complications included 2 incisions secondary healing, 1 postoperative choking persisting for 1 months and 1 Horner's syndrome. There was no recurrence after a follow up from 2 months to 6 years,except 2 cases were lost to follow-up.CONCLUSION:Functional neck dissection is an effective and safe surgical management for recurrent second and third branchial cleft anomalies(fistula and cyst).

Abstract:Background Branchial-cleft anomalies are second only to thyroglossal duct anomalies among congenital malformations of the neck, and second branchial-cleft anomalies are the most common. These include branchial cysts, branchial sinuses, and branchial fistulas. Clinical symptoms include neck swelling and a discharging sinus or fistula opening. In a small number of cases, they can lead to major complications like abscesses or malignant changes. Surgical resection is the treatment of choice. Various approaches to resection and sclerotherapy have been tried. In this study, we present our treatment outcome with branchial cleft anomalies at a rural tertiary medical care hospital. Objectives To document the various presentations, clinical features, and outcomes of treatment with second branchial cleft anomalies. Methods This retrospective observational study included 16 patients operated on for second branchial-cleft anomalies. A detailed medical history was elicited, and an accurate clinical examination was done. A contrast-enhanced computed tomography (CECT) scan was done in all cases. A few cases required a fistulogram. The cysts, sinuses, or fistulas were resected en bloc by a single neck crease incision. Primary closure was done in all cases. A recurrence or pharyngocutaneous fistula required axial flap reconstruction. The complications and recurrences were documented. Result There were six children and 10 adults in our study. Seven cysts, five sinuses, and four fistulas were present, of which four were iatrogenic. In seven patients, imaging could not show the entire tract. There were four fistulas from the oropharynx to a cutaneous opening in the neck. A complete resection was done for all. Two pharyngocutaneous fistulas were treated with a pectoralis major myocutaneous (PMMC) flap. Three patients had wound dehiscence postoperatively. None of the patients had neurological or vascular injuries. Conclusion Second branchial cleft anomalies can be completely excised by a single neck crease incision. Meticulous surgery results in a low recurrence or complication rate. Following complete excision, in type IV anomalies, a purse-string suture at the pharyngeal opening ensures good closure and no recurrences.
An Effective Surgical Approach for the Management of Recurrent First Branchial Cleft Fistula with Cysts: A Case Report

Abstract:The goal of this case report is to describe an unusual case of recurrent first branchial cleft fistula with cysts and the effective surgical approach of complete removal with preservation of the facial nerve. A 27-year-old woman presented to our clinic after unsuccessful removal twice during the last 20 years. We have achieved satisfactory outcomes by using an approach to identify the facial nerve at the stylomastoid foramen with canal wall up mastoidectomy under a microscope. No sign of facial palsy, hearing loss, or recurrence were noted in postoperative follow-up. This approach might facilitate the complete dissection of scar and lesions of recurrent first branchial cleft anomalies around the main trunk of the facial nerve in the parotid gland.

Abstract:Branchial arch anomalies commonly manifest as cyst, sinus, or fistulous opening in the neck. They commonly arise from the 2nd arch and rarely from other arches. Among them, anomalies from the 4th arch are very rare and they usually present as recurrent abscess/swelling in the lower part of the neck. Also, history of repeated incision and drainage (I&D) is often present. In 4th arch anomaly, the tract passes from the skin into the thyroid lobe and then opens internally into the apex of the pyriform sinus. Hence, hemithyroidectomy is required for complete excision of the tract. Here, we describe four cases of recurrent neck abscess, which were evaluated and treated in our tertiary care hospital in India.
