Abstract:BACKGROUND:Operating Room (OR) is the core sector in hospital expenditure, the operation management of which involves a complete three-stage surgery flow, multiple resources, prioritization of the various surgeries, and several real-life OR constraints. As such reasonable surgery scheduling is crucial to OR management.OBJECTIVE:To optimize OR management and reduce operation cost, a short-term surgery scheduling problem is proposed and defined based on the survey of the OR operation in a typical hospital in China.METHOD:The comprehensive operation cost is clearly defined considering both under-utilization and overutilization. A nested Ant Colony Optimization (nested-ACO) incorporated with several real-life OR constraints is proposed to solve such a combinatorial optimization problem.RESULTS:The 10-day manual surgery schedules from a hospital in China are compared with the optimized schedules solved by the nested-ACO. Comparison results show the advantage using the nested-ACO in several measurements: OR-related time, nurse-related time, variation in resources' working time, and the end time.CONCLUSIONS:The nested-ACO considering real-life operation constraints such as the difference between first and following case, surgeries priority, and fixed nurses in pre/post-operative stage is proposed to solve the surgery scheduling optimization problem. The results clearly show the benefit of using the nested-ACO in enhancing the OR management efficiency and minimizing the comprehensive overall operation cost.

Abstract:The managerial aspect of providing quality, cost-effective health services to hospital patients is becoming increasingly important. However, merely focusing on cost reduction and quality improvement may jeopardize personnel satisfaction. Therefore, a balanced approach based on multiple considerations is necessary. Operating rooms (ORs) account for almost 33% of a hospital’s budget, and nursing labor costs represent more than 40% of a hospital’s budget. The concerns around health-care quality, personnel motivation, staff satisfaction, patient length of stay, and patient flow rate are greatly influenced by these two units with regard to day-to-day operations management. A possible approach is to look into the related essential problems, such as the nursing staff time scheduling in nursing units, and the doctor time scheduling in ORs. After surgery in the OR, patients stay in the subsequent ward for a period of time. This causes loading interdependency between the anterior OR and the posterior ward nursing unit; as a result, an attempt to schedule these two departments simultaneously is necessary for operation efficiency enhancement, service quality improvement, hospital budget reduction, and personnel satisfaction. In order to achieve the multiple objectives describing the conflicting priorities and differing preferences within nursing units and ORs, a compromise programming is adopted in this study as a balancing tool for multi-objective optimization to realize improved OR and nursing unit scheduling.
Multi-phase and Integrated Multi-objective Cyclic Operating Room Scheduling Based on an Improved NSGA-ⅡApproach

Abstract:The operating room (OR) is an important department in a hospital, and the scheduling of surgeries in ORs is a challenging combinatorial optimization problem. In this paper, we address the problem of multiple resource allocation of ORs and propose a surgery scheduling scheme for OR units. To solve this problem, a multi-phase and integrated multi-objective linear programming model is proposed. The first phase of the proposed model is a resource allocation model, which mainly focuses on the allocation of ORs for each surgical specialty (SS). Based on the results of the first phase, the second phase is the cyclic Master Surgical Schedule model, which aims to schedule the surgeries in each SS. The proposed models are solved by the Non-dominated Sorting Genetic Algorithm II (NSGA-II), which was improved. Finally, two numerical experiments based on practical data are provided to verify the effectiveness of the proposed models as well as to evaluate the performance of the improved NSGA-II. Our final results illustrate that our proposed model can provide hospital managers with a series of “optimal” solutions to effectively allocate relevant resources and ORs for surgeries, and they show that the improved NSGA-II has high computational efficiency and is more suitable in solving larger-scale problems.
Maximizing the Efficiency of Use of Nurses under Uncertain Surgery Durations: A Case Study.

Abstract:This paper studies a nurse scheduling problem of assigning a set of nurses to surgeries scheduled on each workday in an operating room (OR) suite. This problem plays a decisive role in utilizing nurses efficiently, which is of paramount importance for OR suites to provide high-quality service at ever reduced cost. Due to significant uncertainty in surgery durations, designing schedules that achieve high nurse efficiency is complicated by the competing objective of ensuring on-time starts of surgeries. For trading off between the two performance criteria, we formulate the problem as a mixed integer programming (MIP) model with explicit probability modeling of uncertainty. We are concerned about improving nurse efficiency in terms of overtime and idle time of nurses while mastering the risk of delay of surgeries. The MIP model is applied in a large size Chinese hospital, and the results are compared with the actual performance of the OR suite. The comparisons reveal that through examining the trade-off between the performance criteria, important nurse efficiency improvements can be achieved with good on-time start performance. Moreover, the applicability of the MIP model in various problem settings is also investigated.

Abstract:Operating rooms are considered as the most valuable resource in hospital, which is important for the service quality to patients and the efficiency improvement in hospital operations management. This paper studies the week scheduling of operating rooms integrating other limited key resources including ICU beds and the workload of surgeons. A mixed integer program model has been proposed to optimize the operating room schedule. The results showed the utilization of each operating room has been balanced and maximized. Along with the sensitivity analysis, this model can be used to assist hospital managers in operating room decision making.

Abstract:Facing an acute shortage of surgical nurses, operating room suites in China are more and more aware of the need to use surgical nurses efficiently. This goal is hard to achieve due to the strong interactions between the nurse and the surgery scheduling process. This paper addresses the benefit of integrating elective surgery and surgical nurse scheduling in terms of nurse utilization. First, an integer programming (IP) model is proposed to schedule elective surgeries and surgical nurses simultaneously. Then an efficient genetic algorithm (GA) is proposed based on the IP formulation due to the computational complexity of the integrated scheduling problem. A case study using real-life data is presented to validate the performance of the integrated approaches (the IP model and the GA) by comparing them with a two-stage approach that schedules elective surgeries and surgical nurses sequentially.

Abstract:The problem studied in this paper is elective surgery scheduling, with resource constraints in each of the three following stages: preoperative, perioperative, and postoperative stages. With the integrated availability of hospital beds in wards and operating rooms, the aim is to determine operation start times of surgeries and allocate the hospital beds to patients while getting patients treated as soon as possible. This task is crucial in providing timely treatments for the patients while ensuring the hospital's resource utilization balance. For the problem, we first formulate it as mixed-integer programming, which is NP-complete. Then, we propose several heuristics to overcome the long computation time. To make the solution better, we also propose improved algorithms. Finally, we conduct a series of numerical studies to illustrate the efficiency of our proposed algorithms and examine the impact of the number of jobs, beds, and surgery blocks on the performance measure. Computational experiments showed the superior performance of our heuristics in makespan.

Abstract:With the advancement of surgery and anesthesiology in recent years, surgical clinical pathways have changed significantly, with an increase in outpatient surgeries. However, the surgical scheduling problem is particularly challenging when inpatients and outpatients share the same operating room blocks, due to their different characteristics in terms of variability and preferences. In this paper, we present a two-phase stochastic optimization approach that takes into account such characteristics, considering multiple objectives and dealing with uncertainty in surgery duration, arrival of emergency patients, and no-shows. Chance Constrained Integer Programming and Stochastic Mixed Integer Programming are used to deal with the advance scheduling and the allocation scheduling, respectively. Since Monte Carlo sampling is inefficient for solving the allocation scheduling problem for large size instances, a genetic algorithm is proposed for sequencing and timing procedures. Finally, a quantitative analysis is performed to analyze the trade-off between schedule robustness and average performance under the selection of different patient mixes, providing general insights for operating room scheduling when dealing with inpatients, outpatient, and emergencies.

Abstract:Proper planning and scheduling of activities involved in health can improve productivity in this area. In this regard, hospitals are one of the most critical components, and in hospitals, the operating room is one of the most important ones. Since the operating room is a very costly facility, the scheduling of the patients and involved resources is an important issue. In this study, the problem of scheduling and rescheduling of the operating room in a finite horizon is investigated to minimize the total waiting time and tardiness of patients. The constraints in the problem under consideration are the number of surgeons and the number of beds available. Furthermore, emergency patients as well elective patients are considered simultaneously in our proposed model. In addition to operating room scheduling in this study, rescheduling is also done to canceled patients. To further fit the model presented with reality, uncertainties in parameters such as operating time and the number of beds in the post-anesthesia care unit are also considered. In this study, robust optimization is used to deal with uncertainties in the model. After applying the Bertsimas and Sim approach, due to the complexity of the problem under investigation, the genetic algorithm is used to solve the proposed model. To validate the mentioned algorithm, the particle swarm optimization algorithm is selected according to the literature. The results of the comparison show the superiority of the proposed algorithm compared to the particle swarm optimization algorithm in terms of the objective function and running time.

An examination of the usefulness of a quantitative appraisal method in nursing human resource management in primary hospital operating rooms: An example of integrated collaborative scheduling

Abstract:In recent years, China medical and health services have made great development. However, the management of nursing human resources in operating room of primary hospitals still faces a series of challenges. In the nursing work of operating room, high-quality nursing human resource management is important for improving the efficiency of operating room and ensuring the safety of patients. From January 2022 to December 2022, comprehensive collaborative scheduling and quantitative scoring evaluation methods were carried out in our hospital, and relevant data were collected. The flexible scheduling combined quantitative scoring performance appraisal system and the traditional scheduling plus average distribution performance appraisal system were statistically analyzed and compared in terms of annual surgical cases, annual overtime hours, annual back work hours, annual compensatory rest hours, and average daily working hours. This study was based on 30 medical staff (27 females and 3 males) in the operating room of a primary hospital. The annual operation volume increased by 387 cases compared with before, and the attitudes of patients to the service attitude and preoperative waiting time were significantly improved, reaching more than 95%. In addition, in the survey of surgeons, it was found that their satisfaction with preoperative preparation and operation time was significantly higher than that of the traditional scheduling method, and reached more than 95%. In the survey of nursing staff, it was found that the satisfaction with the traditional scheduling method was about 80%, and the satisfaction directly reached 100% after the comprehensive collaborative scheduling system. Based on the above survey, the satisfaction of nurses, doctors and patients with the new comprehensive collaborative scheduling system has improved compared with before. After the implementation of the comprehensive collaborative scheduling system, the annual surgical volume has increased significantly, and the average daily working hours of nursing staff have decreased. Comprehensive collaborative scheduling is an effective method of nursing human resource management in operating room, which can effectively improve the work efficiency of nurses and the satisfaction of patients, doctors and nurses. In practice, this method needs to be continuously explored and refined to adapt to different application scenarios and requirements.

Two metaheuristics for solving no-wait operating room surgery scheduling problem under various resource constraints

Abstract:The problem studied in this paper is operating room surgery scheduling, with resource constraints in each of the three following stages: preoperative, intraoperative, and postoperative stages. The availability of material resources, specialties and qualifications of human resources are integrated, and the aim is to schedule surgeries while minimizing the maximum end time of last activity in stage 3 and the total idle time in the operating rooms. Two metaheuristics, an iterative local search approach and a hybrid genetic algorithm, are provided and tested on real workday instances from the literature. Computational experiments showed that our metaheuristics outperformed the current state-of-the-art solving algorithm which is an ant colony optimization. The hybrid genetic algorithm reached small superiority vs. the iterative local search algorithm. The average reduction in the end time (the total idle time) was 24% (59%) with the iterated local search approach and 24% (70%) with the hybrid genetic algorithm vs. 14% (55%) with the ant colony optimization algorithm.

Abstract:<h3 class="a-plus-plus">Abstract</h3> <p class="a-plus-plus">Operating rooms (ORs), one of the most crucial hospital resources that generate among the highest costs, are prone to bottlenecking. This paper, therefore, proposes optimization models for multiple-period scheduling of patients in ORs and intensive care units (ICUs) as well as for sequencing of patients in the operating theatre. The first model considers scheduling at a minimal total cost, comprising hospitalization, undertime, overtime, and cancelation costs. The second model involves ICU scheduling and patient sequencing in the operating theatre at a minimal total overtime cost. The scheduling and sequencing models were implemented at a hospital that offers comprehensive surgical procedures for all surgical specialties. The research results are expected to improve patient satisfaction and resource efficiency as well as reduce hospital expenses by minimizing overtime, undertime, hospitalization costs, and cancelation costs. These optimization models may provide substantial assistance to decision makers and planners in facilitating coordination among available hospital resources, including ORs, surgeons, and patients, thereby increasing the productivity and efficiency of ORs and ICUs in providing high-quality care. Future research should consider OR scheduling for unexpected events.</p>

Abstract:Operating rooms (ORs) account for a significant proportion of the costs of maintaining a hospital. Surgery scheduling is optimised to reduce the costs in ORs. However, an important issue that directly affects surgery scheduling is often neglected, i.e. surgeon collaboration. In reality, a surgery requires two surgeons: the main surgeon and an assistant. The surgeon of one surgery might be assigned as the assistant of another surgery. Thus, this paper proposes a two-stage robust model that considers such collaborations. In Stage 1, it determines surgery allocations and surgeon assignments, while in Stage 2, the start time of each surgery is decided. The properties of the solutions are detected, based on which we develop a simple algorithm to solve the two-stage model. In addition, the column-and-constraint generation algorithm is applied to solve the model with a large number of surgeries. The numerical experimentation shows the good performances of the proposed models and algorithms.

Abstract:In this paper, we address an integrated operating room planning and scheduling problem that includes, with fine detail, constraints commonly encountered in practice ( i.e. , sequence, capacity and due date constraints) and for human resources other than surgeons, i.e. , nurses. A new model of the sequence-dependent operating room cleaning times that arise because of surgeries with different infection levels is considered. To solve this difficult integrated planning and scheduling problem, we devise a branch-and-price-and-cut algorithm based on the time-indexed formulation of the problem. The basic column generation scheme relies on a label-correcting algorithm that we purposely developed for solving the pricing problems that are modeled as single operating room scheduling problems with time-dependent costs and sequence-dependent cleaning times. The pricing problems are strongly NP-Hard. The efficiency of the label-correcting algorithm is ensured by dominance rules among labels and by two algorithms for computing the upper and lower bound of labels. An effective cutting procedure, inspired by Benders' decomposition and based on duality theory for linear programming, is developed for tightening the linear relaxation of the problem. With instances from the literature and that we generated, we conduct a numerical study to demonstrate the computational effectiveness of the solution method.

Multi-period and multi-resource operating room scheduling under uncertainty: A case study

Abstract:Efficient planning and scheduling is essential for timely treatment of patients and improving the quality of operating room services and activities. In the present study, attempts are made to investigate a multi-period and multi-resource operating room integrated planning and scheduling problem under uncertainty. To this end, a mixed integer linear programming model has been developed for minimizing the tardiness in surgeries, overtime and idle time. Constraints related to human resources, equipment, as well as beds in pre-operative holding unit, recovery unit, ward and intensive care unit are taken into consideration. The durations of surgeries and recoveries are assumed uncertain, and a robust optimization approach has been used to manage the uncertainty. Due to the complexity of the model and the inability to solve large-scale problems, a metaheuristic method based on the genetic algorithm and a constructive heuristic approach have been proposed. After setting the parameters of the solution approaches using the Taguchi method, numerical experiments are performed based on various instances, and the results obtained from solving the mathematical model are compared to the results of the proposed metaheuristic and heuristic approaches. The results indicate that the proposed methods have a very good performance and the heuristic approach outperforms the genetic approach because the objective function of the proposed constructive heuristic is on average, about 19% better than the objective function of the genetic approach. A case study is also conducted in a hospital. The results obtained from the comparison of the proposed approaches with the hospital scheduling show that overtime and idle time are significantly improved in the proposed approaches.

A Novel Model and Algorithm for Integrated Nurse Scheduling Considering Balanced Workload

Abstract:To utilize the limited number of nurses effectively and improve the job satisfaction of nurses, the integrated nurse scheduling problem is studied considering workload balance. A mixed integer fractional programming model is first developed to design an integrated nursing staffing, scheduling, and assignment plan for minimizing the number of employed nurses under the balanced workload of nurses and the limited working time. An Accelerated Logic Benders exact decomposition algorithm is designed to solve this model. Specifically, based on the hierarchical characteristics of the developed model, the Benders algorithm framework is utilized to decompose the original problem into one master problem and multiple sub-problems, which effectively eliminates the nonlinearity. Two acceleration strategies called ESMP enhancement strategy and Accelerated Logic Benders cut strategy, are proposed to improve the solution efficiency. The experimental results show that considering the workload balance does not incur extra labor costs and makes the workload of nurses more balanced. The ESMP enhancement strategy and the Accelerated Logic Benders decomposition algorithm are feasible and effective. Finally, some important management implications of the workload balance strategy are provided for nursing institutions. To sum up, the proposed model and algorithm provide powerful decision-supporting tools for nursing institutions to make integrated nurse scheduling decisions, which can effectively reduce their employment cost and improve the job satisfaction of nurses.
Surgery scheduling heuristic considering OR downstream and upstream facilities and resources

Abstract:Background: Surgical theater (ST) operations planning is a key subject in the healthcare management literature, particularly the scheduling of procedures in operating rooms (ORs). The OR scheduling problem is usually approached using mathematical modeling and made available to ST managers through dedicated software. Regardless of the large body of knowledge on the subject, OR scheduling models rarely consider the integration of OR downstream and upstream facilities and resources or validate their propositions in real life, rather using simulated scenarios. We propose a heuristic to sequence surgeries that considers both upstream and downstream resources required to perform them, such as surgical kits, post anesthesia care unit (PACU) beds, and surgical teams (surgeons, nurses and anesthetists). Methods: Using hybrid flow shop (HFS) techniques and the break-in-moment (BIM) concept, the goal is to find a sequence that maximizes the number of procedures assigned to the ORs while minimizing the variance of intervals between surgeries' completions, smoothing the demand for downstream resources such as PACU beds and OR sanitizing teams. There are five steps to the proposed heuristic: listing of priorities, local scheduling, global scheduling, feasibility check and identification of best scheduling. Results: Our propositions were validated in a high complexity tertiary University hospital in two ways: first, applying the heuristic to historical data from five typical ST days and comparing the performance of our proposed sequences to the ones actually implemented; second, pilot testing the heuristic during ten days in the ORs, allowing a full rotation of surgical specialties. Results displayed an average increase of 37.2% in OR occupancy, allowing an average increase of 4.5 in the number of surgeries performed daily, and reducing the variance of intervals between surgeries' completions by 55.5%. A more uniform distribution of patients' arrivals at the PACU was also observed. Conclusions: Our proposed heuristic is particularly useful to plan the operation of STs in which resources are constrained, a situation that is common in hospital from developing countries. Our propositions were validated through a pilot implementation in a large hospital, contributing to the scarce literature on actual OR scheduling implementation.
An improved multi-objective imperialist competitive algorithm for surgical case scheduling problem with switching and preparation times

Abstract:Surgical case scheduling is a key issue in the field of medician, which is a challenging work because of the difficulty in assigning resources to patients. This study regards the surgical case scheduling problem as a flexible job shop scheduling problem (FJSP). Considering the switching and preparation time of patients in different stage, an improved multi-objective imperialist competitive algorithm (IMOICA), which adopts the non-dominant sorting method, is proposed to optimize the whole scheduling. First, the social hierarchy strategy is developed to initialize the empire. Then, to enhance the global search ability of the algorithm, the concept of attraction and repulsion (AR) is introduced into the assimilation strategy. Moreover, to increase the diversity of the population, the revolution strategy is utilized. Finally, the variable neighborhood search (VNS) strategy is embedded to improve its exploitation capacity further. Experiments show that scheduling in advance saves time and cost, and IMOICA can solve the surgical case scheduling problem studied efficiently.
